Analysis of the Availability of Medical Laboratory Technology Experts (ATLM) Based on the Minimum Energy Standard of Permenkes No. 43 of 2019 in Indonesia Year 2023

Introduction: Medical Laboratory Technology Experts (ATLM) as one type of health worker, are authorized to organize or practice in the health sector according to their expertise. The ideal ratio of Community Health Centers (Puskesmas) to districts is at least 1 Puskesmas per sub-district, and the minimum staffing standard based on Minister of Health Regulation No. 43 of 2019 for ATLM personnel in Puskesmas is 1 (one), both in Urban, Rural, Remote, and Very Remote Puskesmas. Method: The purpose of this study is to analyze the suitability of ATLM personnel with minimum staffing standards based on the Minister of Health Regulation No. 43 of 2019 in Indonesia. The qualitative research method describes the availability of ATLM personnel at the district/city/provincial level in Indonesia, using secondary data from the Ministry of Health's Health Human Resources Information System (SISDMK) for 2022. Result: The results showed that there is still a shortage of medical laboratory technologists in Puskesmas at the district/city/provincial level in Indonesia, with 1,486 Puskesmas experiencing a shortage out of a total of 10,436 Puskesmas in Indonesia.

Evaluating the World Health Organization's SkinNTDs App as a Training Tool for Skin Neglected Tropical Diseases in Ghana and Kenya: Cross-Sectional Study.

Neglected tropical diseases (NTDs) affect over 1.5 billion people worldwide, primarily impoverished populations in low- and middle-income countries. Skin NTDs, a significant subgroup, manifest primarily as skin lesions and require extensive diagnosis and treatment resources, including trained personnel and financial backing. The World Health Organization has introduced the SkinNTDs app, a mobile health tool designed to train and be used as a decision support tool for frontline health care workers. As most digital health guidelines prioritize the thorough evaluation of mobile health interventions, it is essential to conduct a rigorous and validated assessment of this app. This study aims to assess the usability and user experience of World Health Organization SkinNTDs app (version 3) as a capacity-building tool and decision-support tool for frontline health care workers. A cross-sectional study was conducted in Ghana and Kenya. Frontline health care workers dealing with skin NTDs were recruited through snowball sampling. They used the SkinNTDs app for at least 5 days before completing a web-based survey containing demographic variables and the user version of the Mobile Application Rating Scale (uMARS), a validated scale for assessing health apps. A smaller group of participants took part in semistructured interviews and one focus group. Quantitative data were analyzed using SPSS with a 95% CI and P≤.05 for statistical significance and qualitative data using ATLAS.ti to identify attributes, cluster themes, and code various dimensions that were explored. Overall, 60 participants participated in the quantitative phase and 17 in the qualitative phase. The SkinNTDs app scored highly on the uMARS questionnaire, with an app quality mean score of 4.02 (SD 0.47) of 5, a subjective quality score of 3.82 (SD 0.61) of 5, and a perceived impact of 4.47 (SD 0.56) of 5. There was no significant association between the app quality mean score and any of the categorical variables examined, according to Pearson correlation analysis; app quality mean score vs age (P=.37), sex (P=.70), type of health worker (P=.35), country (P=.94), work context (P=.17), frequency of dealing with skin NTDs (P=.09), and dermatology experience (P=.63). Qualitative results echoed the quantitative outcomes, highlighting the ease of use, the offline functionality, and the potential utility for frontline health care workers in remote and resource-constrained settings. Areas for improvement were identified, such as enhancing the signs and symptoms section. The SkinNTDs app demonstrates notable usability and user-friendliness. The results indicate that the app could play a crucial role in improving capacity building of frontline health care workers dealing with skin NTDs. It could be improved in the future by including new features such as epidemiological context and direct contact with experts. The possibility of using the app as a diagnostic tool should be considered. RR2-10.2196/39393.

Significant improvements show varying results depending on the type of health worker, care unit, and type of intervention. Pengetahuan, Sikap, dan Perilaku Penggunaan Suplemen dan Obat Tradisional pada Tenaga Kesehatan di Jember selama Pandemi COVID-19

Healthcare workers play the role as the front line in treating COVID-19 patients, so they are at risk of being exposed by this virus therefor it is necessary to always maintain health and increase their immunity. One way is taking supplements or traditional medicines. The behavior of taking supplements or traditional medicines is influenced by knowledge and attitudes. The purpose of this study was to determinate the knowledge, attitudes, and behavior of using supplements and traditional medicines among healthcare workers in Jember during the COVID-19 pandemic. This research was an observational analytic study with a cross-sectional design involving 376 healthcare workers in Jember selected by convenience sampling method.The data collection instruments was questionnaires. The results showed that the majority of respondents had good knowledge (52.1%) and moderate attitudes (59.0%). Sociodemographic factors that influence knowledge are level of education and the type of healthcare worker, while the factors that influence attitudes are age, level of education, type of health worker, and working time. There is a significant relationship (p≤0.05) between knowledge and attitudes which can be concluded that knowledge influences attitudes towards the use of supplements and/or traditional medicines

Health workers’ perceptions of leprosy and factors influencing their perceptions in endemic countries: A systematic literature review

Objectives To review work done to date on the perceptions of health workers towards leprosy and persons affected by leprosy. We aimed to create an overview of the different perceptions, review the differences and commonalities, and examine the drivers of these perceptions. Methods A systematic literature review was conducted in PubMed, Web of Science, PsycINFO and Infolep.org. Results The search identified 503 articles, 21 articles were included in the review. The findings show knowledge gaps among health workers, and varying levels of negative attitudes towards persons affected by leprosy, which are influenced by local beliefs and by emotions such as fear. For example, we found that not all health workers are willing to treat persons affected by leprosy, often out of fear of contamination. We found that perceptions differed between countries, regions, health facilities and between different types of health workers. Factors associated with ‘positive’ perceptions of leprosy include male gender, knowing someone affected by leprosy, having received in-service training on leprosy, having graduated over five years ago, having received formal education on leprosy, and having knowledge of leprosy. Conclusions Health care workers’ perception of leprosy varied greatly between countries and by context. We found knowledge gaps and diverse local beliefs that may contribute to negative attitudes, which are also influenced by fear of leprosy. It is important to examine perceptions of leprosy and to contextualise interventions when designing interventions to change health workers’ perceptions of leprosy. Future interventions may consider including elements found to be determinants of a ‘positive’ perception in this review (for example, in-service training).

Acknowledging the Role of Community Health Workers in Providing Essential Healthcare Services in Rural India-A Review.

In underserved communities, the community health worker (CHW) concept has been employed to improve health and lessen unfavourable health consequences. In India's rural healthcare delivery system, auxiliary nurse midwives (ANMs), accredited social health activists (ASHA workers), and Anganwadi workers (AWWs) are the primary field-level frontline officials who come into direct contact with the population. They bear a large portion of the burden of carrying out health services. This review investigated the various contributions made by these CHWs, ANMs, ASHA workers, and AWWs to the advancement of basic healthcare in Indian rural areas. The goal of reviewing this paper was to learn more about what CHWs do to provide the target demographic with high-quality healthcare. A thorough literature search was conducted using crucial databases including PubMed, Google, and Google Scholar. Recent studies were examined to determine how well CHWs perform essential healthcare services in low and middle-income nations. Numerous studies demonstrate how their work has a good effect on society. The length of time CHWs spend at work each day and how well they perform as a whole depends on several variables. This review study showed that, globally, there is a growing interest in CHWs' performance. In terms of incentives, pay, and training expenses, CHWs are thought to be a more affordable option than other types of health workers. They are recognised as the main factors in providing promotive, preventive, curative and rehabilitative healthcare services, achieving enhanced neonatal and maternal health and the development of children and adolescents. The current review also examined previous studies on the work done by CHWs and their potential benefits for enhancing primary healthcare in rural India. It focused on the routine work done by these health workers to increase service accessibility and access to high-quality healthcare, particularly for individuals living in rural areas. Hence, it is necessary to evaluate the functions and general status of community health workers (CHWs), as well as recognise their role, to improve their efficiency in providing basic healthcare services to society and make necessary changes in the future.

Development of and User Feedback on a Board and Online Game to Educate on Antimicrobial Resistance and Stewardship.

Antimicrobial resistance (AMR), particularly antibiotic resistance, is one of the most challenging global health threats of our time. Tackling AMR requires a multidisciplinary approach. Whether a clinical team member is a cleaner, nurse, doctor, pharmacist, or other type of health worker, their contribution towards keeping patients safe from infection is crucial to saving lives. Existing literature portrays that games can be a good way to engage communities in joint learning. This manuscript describes an educational antimicrobial stewardship (AMS) game that was co-created by a multidisciplinary team of health professionals spanning across high- and low- to middle-income countries. The online AMS game was promoted and over 100 players across 23 countries registered to participate on 2 occasions. The players were asked to share feedback on the game through a short online form. Their experiences revealed that the game is relevant for creation of awareness and understanding on antimicrobial stewardship in both high- and low-to-middle income settings worldwide.

Forecasting the Number of Child Development Therapy Patients at Mentari Anakku Clinic Tangerang in 2021 with the Double Exponential Smoothing Method

Clinic is a health service facility that organizes and provides basic and specialist medical services, organized by more than one type of health worker and led by a medical worker. This study aims to determine fluctuations and predict the number of therapy patients at the Child Development Clinic (Mentari Anakku) using the time series method, namely double exponential smoothing. Based on the results of the forecast using the double exponential smoothing method in the 2021 period, it is known that the total number of child development therapy patients is 7,062 people. It can be seen that the total number of patients has increased quite significantly and it is necessary for the clinic to improve the performance of the workers.

Doctor Placement’s Policy and Its Implications in Indonesia: Legal Qualitative Study

BACKGROUND: Remote, Underdeveloped Areas, Frontiers, and Islands (RUAFI) in Indonesia have a less equal distribution of doctors compared to other more developed areas, causing a decline in healthcare service quality in RUAFI and the health degree of the overall population. AIM: This research aims to describe the policy and the implication of doctor distribution in Indonesia and to provide a proportional justice-based doctor distribution policy concept. METHODS: This is a mixed qualitative research of juridical-normative and literature review. The laws that regulate the distribution of doctors in Indonesia are the 1945 Constitution, Law No. 36 of 2009, Law No. 26 of 2014, Governmental Decree No. 67 of 2019, Presidential Decree No. 72 of 2012, and the Decree of the Minister of Health No. 16 of 2017. RESULTS: The unequal doctor distribution is mainly caused by the low motivation for recruitment and retention in RUAFI. The affecting factors include disparity of incentives between doctors, low regional government involvement in the healthcare system in RUAFI, and the lack of career development for doctors being placed in RUAFI. CONCLUSION: The concept of proportional justicebased policy proposed is as follows: (a) Intensive proportionality between doctors and other types of health workers, (b) a direct regional government function of control, and (c) providing career and educational prospects.

Interventions in increasing availability of skilled healthcare providers in rural areas: A Case Study of Bihar (India)

Population of rural areas face distinct health challenges due to economic conditions, cultural/behavioural factors, and health provider shortages that combine to impose striking disparities in health outcomes among them. The process of recruitment takes about four to six months for Recruitment of Medical officers and paramedics. The number of applicants is quite limited because of dearth of doctors and paramedics in the State. It was felt that the health staffs incentives will help to increase the turnover of health staffs to some extent in the rural and remote areas. Monitoring cell has been constituted at the state level. The trainings are being monitored at regular intervals of time. The motivational level of health staff at all levels seems to be low. Continuous communication and feedback by state level programme officers is needed on regular basis. Placement of the suitable trained personnel is needed at those health facilities where sufficient infrastructure is available. Since 2010-11, there has been a continuous focus on the capacity building of the existing manpower in the state. Trainings as per GOI guidelines on Immunization, IMNCI, EmOC, LSAS, SBA and Minilap/MVA etc. have been taken up with full strength. In addition, the State wide training on immunization for Medical Officers, IPC skills for breast feeding and basic training in neonatal resuscitation also has been taken up at various levels. More than four-fifth of the total staffs in the health facilities were agreed on all the educational interventions for retention of health staffs in rural areas. For the regulatory interventions such as enhanced scope of practice, different types of health workers; multi skilling of alternate service providers, compulsory rural service which may be mandatory for obtaining license to practice or can be a prerequisite for entry into specialization and subsidized education in return of assured services were agreed by four-fifth of the total staffs. For the interventions related to professional and personal support such as better living conditions (water, sanitation, electricity, telecommunications, schools, etc.), safe and supportive working environment, outreach activities to facilitate cooperation between health workforce from better served and underserved areas; use of tele-health, designing career development programmes linked with rural service: more senior posts in rural areas and professional networks for rural areas such as rural health professional associations, rural health journals, etc. about 88 percent of the HR categories of Staffs were agreed in the health facilities

Fear of COVID-19 among Health Workers during the Early Stage of Pandemic in Nepal

Introduction: Fear is an emotion that has effects on behavior of people and fear of COVID-19 has significantly impacted the psychological and mental well-being of health workers. The study aimed to assess the fear of COVID-19 among health workers of different cadre during COVID-19 pandemic in Nepal.
 Methods: This was cross-sectional online survey involving 427 health workers currently working in Nepal from April 25 to June 10, 2020. COVID-19 fear was measured using a standard scale of seven items. Questionnaire was prepared in Google form and was sent to study population through social medias and emails. Descriptive and inferential statistics were computed at 5% level of significance. Ethical approval was taken from Nepal Health Research Council, Kathmandu.
 Results: Of the total, 49.6% respondents were male and 50.4% were female. Among total, 58.8 % respondents were in the age group of 19 to 29 years, and 38.4% respondents were from government organizations. Nearly half of the respondents were doctors. Out of maximum 35 COVID-19 fear score, mean COVID-19 fear was16.38 among males and 19.00 among females, 19.48 among nursing professional and 16.80 among doctors. Mean COVID-19 fear was significantly different across sex and type of health workers. Type of the organization did not have any significant relation with mean fear of COVID-19. Most of the items of COVID-19 fear were significantly associated with sex and type of health workers.
 Conclusion: A high level of COVID-19 fear among health workers was found in Nepal during the early stage of COVID-19 pandemic. Mean COVID-19 fear was significantly different across sex and type of health workers. Sex of the health workers and type of profession should be considered while planning psychological intervention among health workers to address the impact of COVID-19 fear.

Present as a follow-up to the post-earthquake service process in Yogayakarta-Central Java on May 26, 2006 Inaugurated on November 13, 2006 by the Chairperson of the MPR. RI Dr. Hidayat Nurwahid, MA and the general chairman of BSMI DR.dr. H. Basuki Supartono, SP.OT, FICS, MARS.
 So far, the recording of patient medical records at the BSMI Klaten Primary Clinic is still using book media. This means that all these activities are carried out without the assistance of a computer so that it can result in lost or lost clinical records of various data. This, according to the author, is still very ineffective because it makes it difficult when presenting data such as an example of presenting data on treatment history or medical records for each patient which will take a long time.
 To overcome this problem, an information system that is able to provide fast information to patients and the public is needed. In order to improve services and improve the performance of each unit which is interconnected and integrated.
 Analysis and design of management information systems at the BSmi Klaten Pratama clinic which was made using the PHP programming language and the server using MySQL. The designed process consists of a registration unit, an initial examination of the patient by a nurse, a doctor's examination, a medical support unit, a pharmacy and a cash payment system, each of which can be data-integrated in a Clinical Management information system.

The dilemma of COVID-19 vaccination among Health Care Workers (HCWs) of Uttar Pradesh

Background: Immediately after the outbreak of COVID-19 pandemic, with an unprecedented cooperation between biomedical, pharmaceutical, technological, and political sectors, new vaccines were developed and approved in record times. However, doubts were raised on their efficacy and adverse effects. Globally, it was agreed that the first recipients for vaccines would be the health care workers (HCWs). Logically, it was bound to raise some concerns and result in hesitancy among the HCWs. Aims: The current study was planned to study the proportion of HCWs having hesitancy towards COVID-19 vaccination and factors effecting it. Settings and Design: Cross-sectional study conducted among HCWs of Uttar Pradesh. Methods and Material: The survey was conducted both in online and offline mode and attempted by 254 HCWs eligible for receiving COVID-19 vaccine. Statistical analysis used: t-test, chi-square test, proportion, mean, SD Results: Vaccine hesitancy was present in 35.8% HCWs. Only social factors like caste (p=0.023) and religion (p<0.001) were found to be significantly associated with vaccine hesitancy. Gender, type of health worker, fear of COVID-19 infection, fear of lethality or pre-existing diseases did not affect vaccine hesitancy. The maximum number of HCWs (71.4%) were hesitant because they were unsure of the side-effects followed by the reason of being unsure about its effect on their own health (53.8%). When asked about their attitude towards compulsory COVID-19 vaccination for HCWs, should it be made mandatory by the government, 42.9% were in favour and 40.6% were against any such mandate. Conclusions: The study concluded that social factors like religion and caste are more deterministic for vaccine hesitancy.

Knowledge of Five Different Types of Indonesian Health Workers Regarding Oral Health Services for Pregnant Women

Background: Oral health care for pregnant women is often not provided by five health professionals in Indonesia: dentist, general practitioner, gynaecologist, promotion health officer, and midwives. While pregnant women also neither seek nor receive oral health treatment, even with apparent oral disease signs. This condition might occur due to a lack of understanding regarding the importance of oral health care for a healthy pregnancy. Objective: This study aimed to determine the knowledge of five different types of Indonesian health workers regarding oral health services for pregnant women. Methods: A cross-sectional study was conducted on a population of five types of health workers in Indonesia. The sampling technique used was non-probability sampling with consecutive sampling technique from August until September 2019. The sample size was 191 health workers; calculation of sample size was conducted using the survey population to estimate the population proportion formula. The research data was collected using Google® Form questionnaire, which consisted of questions regarding regulations and socialisation about the oral health of pregnant women, the minimum number of visits of pregnant women to health service facility during pregnancy, the necessity for oral health examination during pregnancy, oral health treatment package for pregnant women, advisory for pregnant women to have an oral health examination, maternal and child health manual book. The questionnaire was distributed online to the five types of health workers in Indonesia. Data were analysed using Microsoft® Excel and presented in tabular form. Results: Nine out of thirteen knowledge criteria were in the 'less' category, which means 69% of health workers lacked knowledge regarding oral health services for pregnant women. Likewise, the number of respondents who knew the correct knowledge was found in 9 of the 13 criteria, indicated that only a small proportion of health workers had adequate knowledge regarding oral health services for pregnant women. Conclusion: Five different types of Indonesian health workers are not knowledgeable enough regarding the importance of oral health services for pregnant women. Only a small portion of Indonesia's health workers have adequate knowledge regarding appropriate oral health services for pregnant women.

THE ANALYSIS OF ISLAMIC CHARACTERS IN FORMINGCARING BEHAVIOR OF NURSING STUDENT

Nurse is the type of health workers who interacts most with clients so that their services become one of the benchmarks for the success of health services in the hospital. The ability to pay attention to others, intellectual, technical and interpersonal skills of nurses are reflected in their caring behavior when providing nursing care. Nurse's caring behavior is closely related to the Islamic character. Islam itself is seen not only as a religion but also as a way of life that guides people in peace and compassion. The main principle of caring from an Islamic perspective is based on God's revelations and those who practice caring for others will receive many blessings. Muslims believe that the teachings of their religion carry all aspects of life and they seek to keep God at the center of consciousness, intention and action. The research objective was to determine the relationship between Islamic characters and the caring behavior of nursing students. The study used a descriptive analytic cross sectional design and analyzed using the Spearman Rank test with 93 respondents. The results showed that the majority of respondents had good caring behavior, with the conclusion that there was a significant relationship between Islamic characters and student caring behavior. Religion and the environment can be important factors in the formation of Islamic character, which is shown by the nurse's caring behavior towards clients.

Hubungan Tingkat Pengetahuan dan Perilaku Perawat terhadap Risiko Kegiatan Pembuangan Limbah Medis Padat Di Ruang Syifa Dan UGD RS Haji Jakarta Tahun 2015

Hospital is one of the health care facilities that have complex characteristics especially concerning the interaction between the various processes, scientific devices and various types of health workers who are related to each other. In addition to a positive impact, the Hospital had negative impacts that generate waste during its activities; one of waste is solid medical waste. Solid medical waste is one of the sources of pollution, so it is necessary to conduct an integrated management starting from segregation to the disposal of solid medical waste. Researcher raised this issue because there were incident afflicting health workers and cleaning services at Haji Hospital Jakarta, they were punctured used needles. In a period of 2 years from 2014 to 2015, there had been 5 cases of used needles should not happen. This research use quantitative and qualitative methods. Quantitative research conducted by distributing a questionnaire to measure the level of knowledge (age, length of work, education, training), facilities and behavior of nurses and cleaning services to the risk of solid medical waste disposal activities. The qualitative research conducted with in-depth interviews. Result of the relation between nurse knowledge against the risk of solid medical waste disposal based on the result of the t-test 1.661 < t-table 2000. Result of the relation between nurse behavior with the risk of medical waste disposal activities based on the result of the t-test 1.975 < t-table 2000. Result of the relation between of the level nurse knowledge and nurse behavior with the risk of solid medical waste disposal based on the result f-count 3.036 < f-table 3.35. So there is no relation between nurse knowledge and nurse behavior with the risk of medical waste disposal activities at Haji Hospital Jakarta.

Health workforce malldistribution: Lesson from central Lampung Regency in Indonesia

The purpose of this study is to explain: (1) Distribution of health workers in basic health services in Central Lampung, and (2). Factors that influence the process of procurement and distribution of health workers in primary health care in Central Lampung. Research conducted with this descriptive qualitative method involved health stakeholders in 34 health centers and 17 hospitals in the region. The results of this study include: (a). Analysis of the Distribution of Health Workers by Region, it is known that the pattern of distribution is not balanced and there are some districts that do not have several types of health workers. (b). Analysis of power distribution based on health workplaces is known that the majority of health workers are in health centers, even midwives and nurses are mostly in health centers and some types of health workers are not yet available in health centers. Also known factors that influence the process of procurement and distribution of health workers in primary health care include structural and environmental factors.

An intervention of physical activity promotion in an university hospital. A prospective cohort study

Abstract Background Rouen University Hospital implements a policy of health promotion hospital since 2016. The objective of this study was to evaluate prospectively the impact of physical activity promotion in hospital health workers (HW), using pedometers over a period of six months. Methods Physical activity was measured by electronic pedometers distributed to HW, with the aim to improve their physical activity and to measure it during six months. Online questionnaires collected at different points of follow-up, from inclusion to sixth month (a total of seven points of follow-up). Socio-demographic and characteristics on type of health work were collected as well as the number of steps and the behavior regarding their routine physical activity. Results A total of 680 HW were included in the cohort. The sex ratio M:W was 0.16. The mean age was 41.6 years (SD = 10.7). 44.0% were non-medical caregivers, 32.1% were in administrative section, 14.3% were technical workers and 9.7% were physicians or pharmacists; 53.7% of HW reported doing sport regularly. At inclusion the mean number of daily steps was 8662. The mean progression for all professional profiles between first week and sixth month was of 1082 steps daily, with difference according to position: 1319 in physicians, 1234 in non-medical caregivers, 1106 in administrative workers and 314 in technical workers. In linear regression model, the mean number of progression of daily steps was 964 in physicians, 752 in non-medical caregivers. Conclusions The study shows a significant increase in the number of daily steps measured objectively using an incentive tool, as pedometer during six months of intervention. Further researches are needed to determine if these changes are sustainable over a long period of time Key messages Despite high level of daily steps at baseline, the longitudinal study shows an increase of daily steps over a 6-month period. Such public health intervention is a relevant method to engage people in self-monitoring their physical activity.
