Common superficial and deep cutaneous bacterial infections in domestic animals: A review.

The skin covers the external surface of animals, and it is constantly exposed to and inhabited by different microorganisms, including bacteria. Alterations in the skin barrier allow commensal and/or pathogenic bacteria to proliferate and penetrate deep into the lower layers of the skin. Being the first barrier to the external environment, the skin is prone to injuries, allowing the penetration of microorganisms that may lead to severe deep infections. Companion animals, especially dogs, are prone to bacterial infections, often secondary to allergic dermatitis. When environmental conditions are unfavorable, horses, cattle, sheep, and goats can develop superficial infections, such as those caused by Dermatophilus congolensis. Deep inflammation is commonly caused by Mycobacterium spp., which results in granulomatous to pyogranulomatous dermatitis and panniculitis. Likewise, bacteria such as Nocardia spp. and Actinomyces spp. can cause deep pyogranulomatous inflammation. Bacteria that lead to deep necrotizing lesions (eg, necrotizing fasciitis/flesh-eating bacteria) can be severe and even result in death. This review includes an overview of the most common cutaneous bacterial infections of domestic animals, highlighting the main features and histologic morphology of the bacteria, cutaneous structures involved, and the type of inflammatory infiltrates.

Pulmonary Pathology Society Survey on Practice Approaches in the Histologic Diagnosis of Fibrotic Interstitial Lung Disease: Consensus and Opportunities.

The pathologic diagnosis of usual interstitial pneumonia (UIP) remains a challenging area, and application of histologic UIP guidelines has proved difficult. To understand current practice approaches by pulmonary pathologists for the histologic diagnosis of UIP and other fibrotic interstitial lung diseases (ILDs). The Pulmonary Pathology Society (PPS) ILD Working Group developed and sent a 5-part survey on fibrotic ILD electronically to the PPS membership. One hundred sixty-one completed surveys were analyzed. Of the respondents, 89% reported using published histologic features in clinical guidelines for idiopathic pulmonary fibrosis (IPF) in their pathologic diagnosis; however, there was variability in reporting terminology, quantity and quality of histologic features, and the use of guideline categorization. Respondents were very likely to have access to pulmonary pathology colleagues (79%), pulmonologists (98%), and radiologists (94%) to discuss cases. Half of respondents reported they may alter their pathologic diagnosis based on additional clinical and radiologic history if it is pertinent. Airway-centered fibrosis, granulomas, and types of inflammatory infiltrates were considered important, but there was poor agreement on how these features are defined. There is significant consensus among the PPS membership on the importance of histologic guidelines/features of UIP. There are unmet needs for (1) consensus and standardization of diagnostic terminology and incorporation of recommended histopathologic categories from the clinical IPF guidelines into pathology reports, (2) agreement on how to incorporate into the report relevant clinical and radiographic information, and (3) defining the quantity and quality of features needed to suggest alternative diagnoses.

Mouse Models for Atopic Dermatitis.

Atopic dermatitis (AD) is a multifactorial disease with underlying barrier disruption and altered microbial flora, resulting in dry skin and eczematous inflammation with persistent pruritis. Mouse models have been heavily used to investigate AD pathophysiology. Among various AD mouse models, AD-like inflammation induced by topical calcipotriol, a vitamin D3 analog referred to as MC903 in experimental settings, is a versatile model that can be applied to any strain of mice, which can be used for immunologic and morphologic studies. Herein, we provide basic protocols for the topical application of MC903 and approaches to assess phenotypes. After inducing AD-like inflammation, the skin is harvested for flow cytometry analysis, as well as for histologic and immunofluorescence microscopy analyses. The combination of these approaches enables accurate characterization of the degree of inflammation, type of inflammatory infiltrate, and localization of immune infiltrates. Published 2023. Coronary perivascular adipose tissue inflammation – An autopsy-based histomorphological study from a tertiary care hospital in South India

Background: The connective tissue around the blood vessels in the body (except cerebral vessels) is called perivascular adipose tissue (PVAT). Whenever there is a vascular pathology, PVAT increases in volume and becomes dysfunctional. This change promotes dedifferentiation of adipocytes. PVAT inflammation (PVATi) starts secreting anti-vasodilation and pro-inflammatory substance, leading to infiltration of inflammatory immune cells, endothelial dysfunction which initiates atherosclerosis followed by stenosis. It is now recognized that PVATi plays an important role in many vascular pathologies. Aims and Objectives: The objectives of the study were (1) to estimate the prevalence of coronary PVATi in an autopsy case, (2) to determine the coronary atherosclerotic changes and cardiac changes associated with PVATi, and (3) to correlate the association of PVATi with age and sex. Materials and Methods: Autopsy-based retrospective study done for a period of 1 year. From the autopsy requisition form, details such as age, gender, cause of death (COD), and gross findings of the organs were noted. The left coronary artery, right coronary artery, left anterior descending artery, left circumflex artery, and myocardium slides were reviewed microscopically to note the presence of PVATi, type of inflammatory infiltrate, atherosclerotic changes, percentage of stenosis, and associated cardiac changes. Range, frequencies, percentage, mean, standard deviation, and P-value were calculated. P<0.05 was taken as statistically significant. Results: Fifty-two cases showed PVATi among 400 autopsy cases performed during the period of 1 year. Forty-two cases were from males and 10 cases were from female. The most common COD noted was sudden death following chest pain. Ten cases showed PVATi involving major branches of the coronary artery. Thirty-six cases showed hypertrophic changes in myocardium. Acute myocardial infarction (MI) and old healed MI were noted in 7 and 23 cases, respectively. Conclusion: PVATi of coronary arteries has significant association with atherosclerosis and sudden death cardiac death.

Spectrum of Histopathological Lesions in patients undergoing Gastroduodenal Biopsies in a Tertiary Care Centre in North Karnataka, India

Introduction: Patients with Gastrointestinal (GI) symptoms usually present with alarming symptoms. The use of modern flexible gastroduodenoscope and the acquisition of the mucosal biopsy specimens have opened a window to the living tissues and have greatly improved the diagnostic facility for the GI lesions. It provides an in depth visual assessment of GI mucosa and allows sampling of tissue to the pathologist for further assessment. Aim: The aim of this study was to evaluate the spectrum of histopathological incidence of neoplastic and non-neoplastic lesions in patients undergoing gastroduodenal biopsy. Materials and Methods: This was a prospective study of 100 patients conducted over a period of one year and seven months, December 2010 to June 2012 with symptoms of gastroduodenal diseases who attended endoscopic unit of the tertiary care hospital in Bellary North Karnataka, South India. All biopsies done for various upper abdominal symptoms like pain abdomen, dyspepsia, haematemesis, mass per abdomen, and vomiting were included. Routine haematoxylin and eosin, special stains like Periodic Acid-Schiff (PAS) and Giemsa were done. Histopathological parameters evaluated included mucosa evaluation (ulceration, metaplasia, dysplasia, atrophy), type of inflammatory infiltrate, H.pylori infection and type of tumour. Data was analysed using descriptive analysis. Results: Among 100 patients, 76 were males and 24 were females. Highest percentage of study participants were in the age group of more than 58 years 30 (30%). The most common endoscopic presentation was gastric erythema in 42 (42%) cases followed by benign gastric ulcer 16 (16%) cases, ulcero proliferative growth 15 cases, and duodenal erythema 13 (13%) cases. The most common histopathological diagnosis amongst non-neoplastic lesions were chronic gastritis 44 (44%) cases chronic duodenitis 15 cases, and benign gastric ulcer 11 (11%) cases and amongst neoplastic lesions, most common was gastric adenocarcinoma 15 (15%) cases. Conclusion: Endoscopy gross findings are not disease specific hence, histopathological study of biopsy specimen can help in confirmation of the diagnosis.

The role of histopathology in the diagnosis and management of coeliac disease and other malabsorptive conditions.

Most absorption of nutrients takes place in the proximal small intestine, and the most common disorders leading to malabsorption are associated with a morphological abnormality in the duodenal mucosa that is appreciable in histological sections of biopsy specimens. Coeliac disease is the most well-known example, causing intraepithelial lymphocytosis, inflammation and villous atrophy in the duodenum. Remarkably similar inflammatory changes can be induced by other processes, including medications, e.g. angiotensin II receptor blockers and immune checkpoint inhibitors, immune dysregulation disorders, e.g. common variable immunodeficiency and autoimmune enteropathy, infections, collagenous sprue, and tropical sprue. However, there are often subtle histological differences from coeliac disease in the type of inflammatory infiltrate, the presence of crypt apoptosis, and the extent and type of inflammation beyond the duodenum. The clinical setting and serological investigation usually allow diagnostic separation, but some cases remain challenging. Histopathology is also important in assessing the response to treatment, such as the change in villous architecture caused by a gluten-free diet, or the response to cessation of a potentially causative medication. This review examines the practical role that histopathology of duodenal biopsy specimens plays in the assessment and management of inflammatory malabsorptive processes of the proximal small intestine, with a particular emphasis on coeliac disease.

Inflammatory Myofibroblastic Tumor of Nasal Septum after Septoplasty: A Case Report

Inflammatory myofibroblastic tumor is an uncommon tumor composed of myoblasts and various types of inflammatory infiltrates. Inflammatory myofibroblastic tumor is most common in the lungs but can be rarely found in the nasal cavity. Inflammatory myofibroblastic tumor is a rare entity that represents a diverse histologic pattern that can mimic malignant tumors. We report a case of inflammatory myofibroblastic tumor of the nasal septum in a 45-year-old man who presented with a tumor-like lesion of the nasal septum after two rounds of septoplasty.

Cutaneous Vasculitis: Review on Diagnosis and Clinicopathologic Correlations.

Cutaneous vasculitis is an inflammatory disease affecting the dermal blood vessel walls. The skin is a privileged organ in the setting of vasculitis since it is easily accessible for physical examination and safe biopsy, allowing an accurate characterization of inflammatory lesions. The skin is often involved. Also, cutaneous vasculitis can reflect a cutaneous component of a systemic vasculitis, a skin-limited or skin-dominant expression or variant of a systemic vasculitis, or be a single-organ vasculitis per se. Vasculitis lesions are multiple and polymorphic. They may induce a wide spectrum of clinical manifestations depending on the location and the size of the vessels involved. The depth of affected vessels is correlated with the type of cutaneous lesions. Involvement of small superficial vessels results mostly in urticarial, but relatively persistent plaques, papules, and palpable purpura. Involvement of vessels in the dermohypodermic junction or hypodermis results in ulcers, nodules, or livedo. The type of inflammatory infiltrate is also a key finding for the diagnosis of cutaneous vasculitis. Leukocytoclastic vasculitis is not a disease per se but the result of a pathophysiological process common to different causes. A better knowledge of the vascular anatomy of the skin, elementary lesions, and histological characteristics of dermatologic manifestations would allow a more relevant and more efficient diagnostic approach. We also propose a list of additional exams to be performed in front of skin lesions suggestive of vasculitis. The aim of our article is to provide an overview of elementary skin lesions and clinicopathologic correlations in cutaneous and systemic vasculitis.

Behavior of Cellulose Biosynthetic Membrane as a Peritendinous Implant in Foals

Background: Tendon injuries are common in horses and are commonly associated with lameness and athletic career disruption. Adhesions formed between the tendons and the surrounding tissues compromise the sliding and movement of the structures, compromising their functionality. Therefore, the control of adhesion formation is critical to restore the structural integrity of the tendon, as well as its biomechanical function. The aim of this study was to evaluate the behavior of the biosynthetic cellulose membrane implanted in foals with surgically induced tendinitis of the superficial digital flexor.Materials, Methods & Results: Six healthy foals were used, which underwent tendinitis induction in the superficial digital flexor of the right and left forelimbs. The lesions was induced by local ischemia by crushing the tendon with hemostatic forceps. The biosynthetic cellulose membranewas implanted only in the right forelimb, involving the superficial digital flexor tendon in the region of the ischemia and the left forelimb was used as control. After surgery, both forelimb were immobilized with synthetic plaster cast for 15 days. Ultrasonography was performed in six foals immediately before (M0), at 15° (M1) at and 30° (M2) day, and three were evaluate at 45° (M3) and 60° (M4) day after surgery. Incisional biopsies were performed in three animals on the 30° day and in three animals on the 60° day after surgery. Histopathological examination involved the analysis of tissue disorganization, presence and type of inflammatory infiltrate and neovascularization, according to the score of 0 to 3. Ultrasonography allowed visualization of the membrane, which was characterized as a continuous hyperechoic line at the edges of the tendon at 15 days (M1). In addition, at 30 and 45 days after surgery, interruption of the hyperechoic line and reduction of echogenicity were observed, and no echogenic lines were observed at 60 days after surgery. During the biopsies, the fragments obtained from the treated limb were presented with easily detachable layers and separate layers of tissue during histological cutting and the slides preparations. There was no significant difference in relation to the histopathological scores between the treated and control member. The inflammatory infiltrate was predominantly of mononuclear cells and fibroblasts, with identification of giant cell in a foal at 30 days. Neovascularization was observed in all limbs, treated and controls at 30 days and in two treated limbs and three control at 60 days. It was possible to identify the synthetic cellulose membrane by histopathology in only two limbs treated at 30 days and in no limb at 60 days.Discussion: The degree of tendonitis induced in the present study was considered mild according to ultrasonographic analysis at 15 days after surgery. This fact limited the evaluation of the biosynthetic cellulose membrane in the prevention of adhesions, since they were not observed in both groups. However, the ability of the membrane to minimize adhesion formation can be suggest by the macroscopically observed individualization of the tissue layers during histological sections and slides preparation. Ultrasonographic analysis allowed the visualization of the membrane, as well as its positioning and the absorption process. Therefore, ultrasonography can be used in post-implantation monitoring of the membrane. The comparison of the histopathological scores demonstrates the biocompatibility of the biosynthetic cellulose membrane when implanted in foals submitted to surgically induced tendinitis, since there was no difference when compared to treated and control limbs. Therefore, it can be used safely in the treatment of tendon injuries in horses.

A study of the histopathological features of alopecias on transverse sections of scalp biopsies

Background:Transverse sections of scalp biopsies are performed for the assessment of alopecias and are considered advantageous over vertical sections.Aim:The aim was to study the histopathological features of alopecias on transverse sections of scalp biopsies.Methods:It was a descriptive study. Clinically confirmed cases of noncicatricial and cicatricial alopecias were subjected to 4 mm scalp biopsies, which were sectioned transversely and analyzed. Biopsies obtained from occipital region of androgenetic alopecia (AGA) cases were taken as controls.Results:Biopsies from 41 cases were assessed, including male and female AGA, alopecia areata (AA), trichotillomania, lichen planopilaris (LPP), discoid lupus erythematosus (DLE), and folliculitis decalvans (FD). Normal scalp (control) biopsies showed the median total number of hair follicles of 35 (32–37), anagen:telogen/catagen ratio of 17.5 (16.5–31), and terminal:vellus ratio of 15 (10.7–17.5). In AGA and AA, miniaturization and shift toward telogen and catagen hair were consistently observed. Peribulbar inflammation was seen in two-third of AA. Trichotillomania showed increased catagen hair and numerous pigment casts. In DLE, besides perifollicular inflammation, prominent peri-arrector pili and peri-eccrine inflammation were observed. Type of inflammatory infiltrate was similar in DLE and LPP (lymphocytic), whereas FD showed neutrophilic and plasma cell infiltrate, both around follicles and interstitially. Basal cell damage in the follicles and pigment incontinence were seen in majority of DLE and LPP patients. DLE also showed basement membrane thickening, mucin deposition, and telangiectasia. Reduction/absence of sebaceous glands and perifollicular fibrosis were observed in almost all cicatricial alopecias.Conclusion:Transverse sectioning may be a useful tool in the diagnosis of alopecias.

Distinguishing Epidermolysis Bullosa Acquisita From Bullous Pemphigoid Without Direct Immunofluorescence.

It has been postulated that periodic acid-Schiff staining of basement membrane can predict direct immunofluorescence patterns seen in epidermolysis bullosa acquisita and bullous pemphigoid. It has also been suggested that the type of inflammatory infiltrate or presence of fraying of basal keratinocytes may differentiate these two conditions. In this study, we aimed to confirm these observations. We reviewed 13 cases of direct immunofluorescence-confirmed epidermolysis bullosa acquisita and 19 cases of direct immunofluorescence-confirmed bullous pemphigoid, all with a subepidermal blister in the routinely processed specimen. The gold standard for diagnosis of epidermolysis bullosa acquisita vs bullous pemphigoid was taken to be identification of immune deposits on the dermal side ('floor' for epidermolysis bullosa acquisita) or the epidermal side ('roof' for bullous pemphigoid) of the salt-split direct immunofluorescence specimen. Our tests to distinguish epidermolysis bullosa acquisita from bullous pemphigoid on the routinely processed biopsy included periodic acid-Schiff basement membrane on the blister roof, neutrophilic infiltrate, lack of eosinophilic infiltrate, and absence of keratinocyte fraying. Sensitivity and specificity for each test were as follows: periodic acid-Schiff staining of roof (sensitivity 25%, specificity 95%), neutrophilic infiltrate (sensitivity 54%, specificity 74%), lack of eosinophilic infiltrate (sensitivity 92%, specificity 68%), and absence of keratinocyte fraying (sensitivity 62%, specificity 58%). Features in the routinely processed biopsy were unable to reliably distinguish between epidermolysis bullosa acquisita and bullous pemphigoid. Direct immunofluorescence on salt-split skin remains the standard for differentiation.

Ultrasound examination of pneumonia-infected lungs

In the last few decades the content of diagnostic information provided by ultrasound examination of the chest in a wide spectrum of diseases (pneumonia, peripheral tumors, pleural diseases, pneumothorax) has been widely discussed. The advantages of ultrasound examination (the lack of radiation exposure, image in real time, the distinct visualization of pulmonary subpleural portions and costal diaphragmatic sinuses) provide the possibility to use ultrasound in pediatric practice for frequent monitoring of pneumonia dynamics. The use of ultrasound examination in the diagnostic algorithm in adult patients with pneumonia is not widely used in Russian clinical practice. The article gives the details about the technique of ultrasound examination of the chest, ultrasound anatomy, the impact of the morphological substrate of the pathological formation on the ultrasound picture, and its localization and length. The review of bibliography data about ultrasound semiotics of pneumonia depending on the type of inflammatory infiltrate (hypo and hyperechoic structure of various shapes, length, artifacts, A-line, -line). The necessity of ultrasound use for monitoring pneumonia in order to assess the therapy efficacy has been proven (this is a very strong statement, I would say it “has been supported”. It is indicated that the possibility of ultrasound examination depending on morphological forms of inflammation has not been disclosed and there are no data about the comparison of ultrasound, X-ray and computed tomography. The recommended frequency of the ultrasound monitoring of different inflammatory lung diseases has not yet been determined (this makes it sound like there are no results). This article indicates that ultrasound examination can take its important place as an option in the diagnosis in patients with inflammatory lung diseases due to the safety and wide availability of this method in combination with the CDK.

Salivary glands and human congenital cytomegalovirus infection: What happens in early fetal life?

Salivary glands are a site of human cytomegalovirus (CMV) replication, latency, and persistence. Prolonged secretion of virus in saliva for months following a primary infection contribute to horizontal transmission. In order to better understand the early effects of CMV on salivary glands and the mechanisms of viral persistent replication, submandibular glands of six CMV congenitally infected fetuses at 21 weeks gestation were studied. Three fetuses at the same gestational age from CMV-seronegative women were compared as negative controls. Tissue viral load and the type of inflammatory infiltrate were evaluated. Moreover, development and branching of salivary glands, the number of myoepithelial cells, cellular proliferation, and expression of secretory proteins of the saliva (Gross Cystic Disease Fluid Protein-15 and lysozyme) were studied. A low viral load and rare CMV-positive cells associated with T CD8 cytotoxic lymphocytes were observed. Branching was impaired with a decrease in terminal acinar structures, the number of myoepithelial cells, and cellular proliferation were reduced. In addition, a compromised secretion of defense proteins involved in the oral humoral immunity was observed. These findings suggest that CMV may affect salivary glands, impairing structure development and secretion of defense proteins, probably responsible for the prolonged viral shedding in saliva. J. Med. <b>Histological evaluation of the rat dental pulp after indirect capping with sildenafil or L-NAME incorporated into a bioadhesive thermoresponsive system

We evaluated the histological dental pulp state in vivo after indirect pulp capping using sildenafil or L G -nitro-L-arginine (L-NAME), incorporated into a new bioadhesive thermoresponsive system (BTS). Male Wistar rats were subjected to an upper and lower first molar class I cavity preparation followed by indirect pulp capping with sildenafil or L-NAME. Calcium hydroxide (CaOH 2 ) was used as a control. The teeth and surrounding bone were properly dissected and processed for Nissl’s staining. Pulp state was evaluated considering the morphological aspects of the inflammatory response, type of inflammatory infiltrate, organization of the odontoblast layer, blood vessel condition, and presence of abscesses or necrosis. The results were expressed as average of observations. The most intense inflammatory response was observed 3 days after the cavity preparation. No identified changes were detected in the dental pulp response of the molars treated with L-NAME compared with those treated with CaOH 2 . A dual effect was observed in the teeth treated with sildenafil. While low sildenafil concentration (0.015% w w -1 ) promoted effects comparable to CaOH 2 , at a higher concentration (0.15% w w -1 ), sildenafil caused a severe inflammatory response and pulp necrosis. This pioneering suggest that NO pathway activity may be a determinant in the process of dental pulp healing.

Pathological Features of Bovine Pericardium Implanted Into Mice Abdominal Wall after Preservation with Glutaraldehyde or Glycerin

Background: Lately, the use of biological materials has been widely indicated in surgical procedures to restore structure and function of injured tissues. Bioimplants require different conservation techniques; among these, glutaraldehyde preservation stands out owing to its higher antimicrobial efficiency as compared to glycerin. In view of the need to determine a concentration of glutaraldehyde that can act as a biocide but do not cause undesirable tissue reactions, this study aimed to identify and quantify gross and microscopic tissue alterations after implantation of bovine pericardium, which was preserved in various concentrations of glutaraldehyde, in the abdominal wall of mice.Materials, Methods & Results: Fresh pericardia from 18 bovines were fractioned into 1cm2 samples and treated with a 98% glycerin solution for 30 days (control group), or 0.625%, 1%, and 1.5% glutaraldehyde solution for 18 days (experimental groups). An abdominal muscle fragment was excised from each mouse, and a 1-cm2 fragment of preserved pericardium was implanted in the area. Sixty mice (n = 15 per treatment) divided into groups were observed for 7, 14, and 30 days, and five animals from each group were euthanized at each time point for gross and microscopic examination. Fragments of the implants and adjacent skin lesions were harvested, fixed in formalin, and processed for routine histology and microscopic analysis. Both the type of inflammatory infiltrate and the repair process of the tissue response were similar between the groups that received glycerin-preserved pericardium and those that were subjected to pericardium preserved with 0.625% glutaraldehyde. Animals that received 1% glutaraldehyde-preserved implants and were examined 30 days thereafter exhibited a chronic, intense reaction with fibrosis and necrosis of the abdominal wall muscles, as well as calcification and presence of giant cells, when compared to the animals examined at 7 and 14 days in the same treatment group. These changes were also present and more intense in animals that received 1.5% glutaraldehyde-preserved pericardium examined at 14 and 30 days later, with tissue destruction and impaired incorporation of the implant into the adjacent muscle tissue.Discussion: The continuous cell destruction observed in animals treated with implants preserved with 1% or 1.5% glutaraldehyde is a hallmark of chronic inflammation, since several inflammatory cell molecules contribute to this lesion. A cycle is created: continuous degradation sustains inflammation, and inflammatory molecules contribute to the process of cell destruction. Consequently, we conclude that the use of glutaraldehyde at concentrations of 1 or 1.5% is not feasible for preservation of biological materials. Tissue repair was chronologically more effective in the group treated with glycerinpreserved implants, since animals treated with glutaraldehyde-preserved implants needed a longer period to restore due to presence of a persistent inflammatory response, immunogenicity, calcification, and deficient remodeling. The ideal preservative for biological materials should not cause chronic and/or intense inflammatory reaction in order to preserve the implant’s structure and allow its perfect incorporation into the tissue, even if the chosen preservative is flexible and exhibits disinfectant properties. Therefore, we conclude that glutaraldehyde at concentrations of 0.625% to 1% is suitable as a preservative for biomaterials because the tissue reaction it causes is tolerable; additionally, glutaraldehyde at concentrations close to 1% has been described to have sterilizing properties.

Nitrogen-containing bisphosphonate therapy: assessment of the alveolar bone structure in rats - a blind randomized controlled trial.

This study aimed to assess the effect of zoledronic acid exposure on structures of the alveolar bone of rats. The sample was composed of 42 male Wistar rats. Animals in the T1 and T2 groups received weekly doses of 0.2 mg/kg intraperitoneal zoledronic acid for 3 weeks, while animals in the T3 group received the same treatment for 8 weeks. The control groups C1, C2 and C3 received equivalent doses of saline. The first upper molars of Wistar rats in the C2, T2, C3 and T3 groups were extracted. Cone-beam computerized tomography scans were performed, and the image density was analysed by grey levels. The presence and type of inflammatory infiltrate, vascularization and bone necrosis were assigned by histological qualitative scores. Histomorphometric analysis of bone density was performed in the groups without extraction. No significant differences were found in the bone grey density estimated by grey-level value and histomorphometric analysis between the C1 and T1 groups (P > 0.05). The grey levels in the T3 group were lower (P < 0.05) than in the C3 group, corresponding to the bone defect. Histological assessments showed the presence of bone necrosis in the T3 group and lower levels of bone remodelling in the test groups (T2 and T3) compared to the control groups (C2 and C3). The results of qualitative analyses did not differ significantly between the groups (P > 0.05). Zoledronic acid-exposed animals showed maxillary changes including reduced grey levels, the presence of bone necrosis and a higher prevalence of inflammatory signs.

Tissue responses to postoperative laser therapy in diabetic rats submitted to excisional wounds.

In a previous study about low-level laser therapy biomodulation on a full-thickness burn model we showed that single and fractionated dose regimens increased wound healing and leukocyte influx similarly when compared with untreated control. In order to verify if this finding would be similar in an impaired wound model, we investigated the effect of single and multiple irradiations on wound closure rate, type of inflammatory infiltrate, myofibroblasts, collagen deposition, and optical retardation of collagen in diabetic rats. Female Wistar rats in the same estrous cycle had diabetes induced with streptozotocin and an 8-mm excisional wound performed with a punch. The experimental groups were: control group – untreated ulcer; single-dose group – ulcer submitted to single dose of diode laser therapy (λ = 660 ± 2 nm; P = 30 mW; energy density: 4 J/cm2) and fractionated-dose group – ulcer submitted to 1 J/cm2 laser therapy on Days 1, 3, 8, and 10. The ulcers were photographed on the experimental days and after euthanasia tissue samples were routinely processed for histological and immunohistochemistry analyses. Independently of the energy density, laser therapy accelerated wound closure by approximately 40% in the first three days in comparison to the control group. Laser therapy increased acute inflammatory infiltrate until Day 3. Both laser groups exhibited more myofibroblasts and better collagen organization than the control group. The findings demonstrate that low-level laser therapy in the immediate postoperative period can enhance the tissue repair process in a diabetes model. Similar effects were achieved with laser therapy applied a single time with an energy density of 4 J/cm2 and applied four times with an energy density of 1 J/cm2. The application of laser therapy in the inflammatory phase was the most important factor to the enhancement of the tissue repair process.

Repeated inhalation of sevoflurane inhibits airway inflammation in an OVA-induced mouse model of allergic airway inflammation.

Repeated inhalation of sevoflurane (SVF) can benefit asthmatic patients by bronchodilation. However, the impact of repeated inhalation of SVF on allergic airway inflammation has not been clarified. This study was aimed at investigating the effects of repeated inhalation of SVF on airway inflammation in mice. Female C57BL/6 mice were sensitized with ovalbumin (OVA) and treated by inhalation with SVF or vehicle daily for seven consecutive days, immediately followed by OVA challenge. Airway inflammation was evaluated by counting the numbers of different types of inflammatory infiltrates in bronchoalveolar lavage fluid (BALF), histology, cytokine measurements and mucus production in individual mice. In comparison with the OVA group, repeated inhalation of SVF significantly reduced the numbers of total cells, eosinophils, lymphocytes, macrophages and neutrophils (P < 0.05 to P < 0.01), and the levels of BALF tumour necrosis factor-α and lung high-mobility group box 1 (P < 0.01), accompanied by elevated levels of BALF interleukin-10 in allergic mice (P < 0.05). Repeat inhalation of SVF decreased the levels of serum OVA-specific immunoglobulin E (IgE) and mitigated allergic airway epithelial goblet cell hyperplasia and mucus hypersecretion in allergic mice (P < 0.01). Repeated inhalation of SVF inhibits allergic airway inflammation by reducing inflammatory infiltrates, improving the imbalance of cytokine responses and mitigating allergen-specific IgE responses and goblet cell hyperplasia in mice.

Histological and immunohistochemical features in fatal acute fulminant hepatitis E

Hepatitis E is being increasingly recognized as an emerging infection in developed countries. Data on histological findings and nature of inflammatory cell infiltrate in liver in this disease are quite sparse. This study was planned to study the histological features and the type of inflammatory infiltrate in liver biopsies of patients with acute fulminant hepatitis E. We retrieved postmortem liver biopsies of 11 Indian patients with fulminant hepatitis E, and compared these with biopsies from seven patients with fulminant hepatitis B. Biopsies from acute fulminant hepatitis E showed varying degrees of hepatocyte necrosis, mixed portal and lobular inflammation, accompanied by bile ductular proliferation, lymphocytic cholangitis, Kupffer cell prominence, cholestasis, apoptotic bodies, pseudo-rosette formation, steatosis, and presence of plasma cells in portal tracts. Interface hepatitis was more frequent in acute hepatitis B than in acute hepatitis E (100% vs 20%; P<0.05). These findings differ from those reported in cases with autochthonous hepatitis E in Europe. On immunohistochemistry, lymphocyte infiltrate consisted predominantly of CD3 + T cells in both hepatitis E and hepatitis B; these cells contained a predominant cytotoxic (CD8 + ) cell subpopulation in 81.8% of cases with hepatitis E and in 50% of cases with hepatitis B. Our findings suggest that histological changes in HEV infection may vary with geographical location because of prevalent HEV genotypes, and that CD8 + lymphocytes play a role in HEV-induced liver injury.

T-Helper Cell Type 17/Regulatory T-Cell Immunoregulatory Balance in Human Radicular Cysts and Periapical Granulomas

Introduction Cysts and granulomas are chronic periapical lesions mediated by a set of inflammatory mediators that develop to contain a periapical infection. This study analyzed the nature of the inflammatory infiltrate, presence of mast cells, and in situ expression of cytokines (interleukin [IL]-17 and transforming growth factor [TGF]-ß), chemokines (macrophage inflammatory protein [MIP]-1ß and monocyte chemotactic protein [MCP]-1), and nuclear transcription factor (FoxP3) in human periapical granulomas and cysts compared with a control group. Methods Fifty-five lesions (25 periapical cysts, 25 periapical granulomas, and 5 controls) were analyzed. The type of inflammatory infiltrate was evaluated by hematoxylin-eosin staining, and the presence of mast cells was analyzed by toluidine blue staining. Indirect immunohistochemistry was used to evaluate the expression of cytokines, chemokines, and FoxP3. Results The inflammatory infiltrate mainly consisted of mononuclear cells. In cysts, mononuclear infiltrates were significantly more frequent than mixed (polymorphonuclear/mononuclear) infiltrates ( P = .04). Mixed inflammatory infiltrates were significantly more frequent in patients with sinus tract ( P = .0001). The number of mast cells was significantly higher in granulomas than in cystic lesions ( P = .02). A significant difference in the expression of IL-17 ( P = .001) and TGF-ß ( P = .003) was observed between cysts and granulomas and the control group. Significantly higher IL-17 levels were also observed in cases of patients with sinus tract ( P = .03). Conclusions We observed that chronic periapical lesions might experience a reagudization process that is correlated with an increased leukocyte infiltration, with the predominance of neutrophils attracted by a chemokine milieu, as well as the increased presence of IL-17.
