Optimizing Pain and Rehabilitation After Knee Arthroplasty: A Two-Center, Randomized Trial.

This randomized trial compared (1) continuous femoral nerve block (cFNB), (2) single femoral nerve block (sFNB), and (3) local infiltration analgesia (LIA) with respect to analgesic and functional outcomes after primary tricompartmental knee arthroplasty (TKA). One hundred twenty patients undergoing primary tricompartmental knee arthroplasty were randomly assigned to 1 of 3 interventions for postoperative analgesia: (1) cFNB-preoperative bolus of ropivacaine 0.5% 20 mL followed by ropivacaine 0.2% 5 mL per hour for 48 hours; (2) sFNB-preoperative bolus of ropivacaine 0.5% 20 mL with placebo 0.9% saline 5 mL per hour for 48 hours; or (3) LIA-intraoperative tricompartmental injection of ropivacaine 0.2% (150 mL) with epinephrine (10 µg/mL) and ketorolac 30 mg with femoral placebo 0.9% saline 20 mL preoperative bolus and 0.9% saline placebo 5 mL per hour for 48 hours. All participants received an identical, standardized, postoperative multimodal analgesic regimen. Participants, health care providers, data collectors, and analysts were blinded. All participants received identical perineural catheters and perineural/LIA solution (depending on randomized intervention) to maintain blinding. The primary outcome measure was numeric rating scale for pain (NRS) during physiotherapy on postoperative day (POD) 2 at 9:00 AM. Secondary outcomes included opioid consumption, NRS on POD 1 (rest/physiotherapy/worst), functional outcomes, and block complications. For the primary outcome, pain during physiotherapy on POD 2 at 9:00 AM, the overall analysis of covariance (ANCOVA) was significant (P = .049), but pairwise comparisons did not demonstrate any significant differences between treatment arms. NRS was 4.6 (95% confidence interval [CI], 3.3-6.0) for the cFNB group, 4.6 (95% CI, 3.3-6.0) for the sFNB group, and 3.4 (95% CI, 2.2-4.8) for the LIA group. The following is the mean difference in NRS on POD 2 at 9:00 AM among groups: cFNB-LIA (1.2, 95% CI, -0.1 to 2.5; P = .073); sFNB-LIA (1.2, 95% CI, -0.2 to 2.5; P = .097); cFNB-sFNB (0.0, 95% CI, -1.3 to 1.4; P = .996). There were no statistically significant differences between groups in cumulative 48-hour opioid consumption or functional outcomes. cFNB and LIA were superior to sFNB for NRS on POD 1 for worst pain experienced and pain during physiotherapy, respectively. There were no adverse events associated with study procedures reported among participants in the 3 groups. Our findings suggest no clinically significant differences between cFNB, LIA, and sFNB for pain during physiotherapy on POD 2 after TKA. Secondary analyses suggest that cFNB and LIA are superior to sFNB for early analgesic outcomes (NRS on POD 1) after TKA.

Joint perception after hip or knee replacement surgery

Knee and hip arthroplasties are recognized as being effective. However, subjects with a prosthetic joint rarely report returned sensation comparable to their native joint. Joint perception by patients following hip joint replacement is better than following knee replacement and in both cases this perception is directly related to the clinical results measured with currently validated scores. Patient joint perception in prosthetic reconstruction was evaluated in 347 patients, 46 who underwent unicompartmental knee arthroplasty (UKA), 119 tricompartmental knee arthroplasty (TKA), 93 hip resurfacing (HR), and 89 total hip arthroplasty (THA). The subjects' joint perception, their satisfaction, and the WOMAC clinical score were recorded and compared. Joint perception was significantly worse for knee arthroplasties (TKA and UKA) compared to hip arthroplasties (THA or HR) (P<0.001). The WOMAC score was also significantly less favorable for knee arthroplasties than for hip arthroplasties (P<0.0001). However, there was no significant difference for the clinical scores between TKA and UKA as well as between HR and THA. Joint perception was strongly correlated with the WOMAC score for all groups (R(2)=0.951). No difference was found after more conservative surgeries such as HR or UKA compared to traditional arthroplasty procedures (THA or TKA). Demonstrating inferior results in comparison to the hip, knee arthroplasties deserve particular attention and can still be improved. The assessment of joint perception used in this study can be considered a valuable clinical tool that is strongly correlated to validated, but more complex to apply, clinical scores.

Two Year Follow-up of the Preservation Unicompartmental Knee Implant

Reported results of unicondylar knee arthroplasty (UKA) have mixed reviews in comparison with results of tri-compartmental knee arthroplasty (TKA). We prospectively evaluated the short-term results (2years) of a newer design of a UKA implant (Preservation UKA) with a cobalt-chromium femoral component and an all polyethylene tibial component. Seventy-two patients with intact ligaments and loss of only medial articular cartilage received the Preservation prosthesis. Data were obtained using WOMAC, Knee Society score (KSS), and standard radiographs. WOMAC scores improved by 24 points and KSS improved by 33 points at 2-year follow-up. Mean flexion increased by 4° to126° at 2years. On X-ray, only one patient had a radiolucency. No fractures occurred. Two knees were revised due to clinical symptoms of medial compartment pain. This 2-year follow-up study of the Preservation UKA shows promising early results. Long-term data would be necessary to compare results with TKA or other unicompartmental replacements.

Influence of bi- and tri-compartmental knee arthroplasty on the kinematics of the knee joint

BackgroundThe cruciate ligaments are important stabilizers of the knee joint and determine joint kinematics in the natural knee and after cruciate retaining arthroplasty.No in vitro data is available to biomechanically evaluate the ability of the anterior cruciate ligament (ACL) to maintain knee joint kinematics after bicruciate-retaining bi-compartmental knee arthroplasty (BKA).Therefore, the objective of the current study was to investigate the kinematics of the natural knee joint, before and after installing bicruciate-retaining BKA and posterior cruciate retaining total knee arthroplasty. Specifically, we incorporated a dynamic knee simulator to simulate weight-bearing flexions on cadaveric knee specimen before and after surgical manipulations.MethodsIn this cadaveric study we investigated rotational and translational tibiofemoral kinematics during simulated weight-bearing flexions of the intact knee, after bi-compartmental knee arthroplasty (BKA+), after resecting the ACL in BKA (BKA-), and after posterior cruciate retaining total knee arthroplasty (TKA).ResultsRotation of BKA+ is closest to the intact knee joint, whereas TKA shows significant differences from 30 to 90 degree of flexion. Within the tested flexion range (15 to 90 degree of flexion), there was no significant difference in the anterior-posterior translation among intact, BKA+, and TKA knees. Resecting the ACL in BKA leads to a significant anterior tibial translation.ConclusionsBKA with intact cruciate ligaments resembles rotation and translation of the natural knee during a simulated weight-bearing flexion. It is a suitable treatment option for medial and patellofemoral osteoarthritis with advantages in rotational characteristics compared to TKA.

Incidence of Radiographic Unicompartmental Arthritis in Patients Undergoing Knee Arthroplasty

Unicompartmental knee arthroplasty is increasing in popularity with the advent of less invasive procedures for knee arthritis. The percentage of patients undergoing knee arthroplasty who could be candidates for unicompartmental knee arthroplasty depends on the surgeon's evaluation of the radiographs, and this evaluation may depend on the surgeon's bias regarding partial knee arthroplasty. A retrospective radiographic and chart review was performed on a consecutive series of patients who had undergone tricompartmental knee arthroplasty to determine the percentage of those patients who could have been candidates for unicompartmental knee arthroplasty. Two hundred eighty-eight patients who underwent 308 tricompartmental knee arthroplasties over a 3-year period at a Veteran's Administration Hospital comprised the study group. Assessment of preoperative radiographs was done by 2 surgeons, 1 who favored unicompartmental knee arthroplasty and the other who preferred tricompartmental knee arthroplasty, to determine the percentage of these patients, from each surgeon's viewpoint, who had unicompartmental arthritis. Patients who had radiographic unicompartmental arthritis were then eliminated as candidates for unicompartmental knee arthroplasty, if, on chart review, they had a flexion contracture >10°, an arc of motion <100°, or inflammatory arthritis. The surgeon who was a proponent of unicompartmental knee arthroplasty found that 26% of these patients had acceptable radiologic and clinical indications for unicompartmental knee arthroplasty, whereas the surgeon who had a bias against the procedure felt that only 12% of these patients were unicompartmental knee arthroplasty candidates. A considerable percentage of Veteran's Administration patients undergoing arthroplasty for knee arthritis may be potential candidates for unicompartmental knee arthroplasty, despite the surgeon's bias for or against the procedure.

Conversion of lateral unicompartmental arthroplasty to anterior cruciate retaining tricompartmental knee arthroplasty

This case report presents the conversion of a lateral unicompartmental arthroplasty (UKA) to an anterior cruciate retaining tricompartmental knee arthroplasty. The patient presented with disease progression to the medial and patellofemoral compartments of the knee, in addition to significant varus deformity. During revision surgery, the previously implanted UKA device was found to be well fixed and in good condition. The conventional treatment option would be conversion to total knee arthroplasty (TKA). However, in this case conversion to a tricompartmental, ligament sparing arthroplasty via implantation of a bicompartmental knee arthroplasty (BKA) device was found to be feasible. In UKA revision cases where the device is functional, the current surgical approach may be an appropriate alternative to conventional TKA.

Osteonecrosis of the Knee After Laser or Radiofrequency-Assisted Arthroscopy: Treatment with Minimally Invasive Knee Arthroplasty

Osteonecrosis of the knee after various arthroscopic procedures associated with the use of laser or radiofrequency devices has been described in a few case reports. The purpose of this study was to report on a series of nineteen patients with osteonecrosis of the knee after arthroscopic procedures. A literature search was done to compare this series of patients to previously reported cases. In addition, we analyzed the outcome after treatment with minimally invasive knee arthroplasty. We studied patients who had development of osteonecrosis of the knee after a routine arthroscopic procedure. Preoperative and postoperative clinical notes, radiographs, and magnetic resonance images of patients were analyzed. Only those patients with no evidence of osteonecrosis on preoperative magnetic resonance imaging who later had development of osteonecrosis and subsequently required a knee arthroplasty were included. We conducted a search of the current literature to compare the results seen in our patient population with those seen in other patients with this entity. Patients were followed both clinically and radiographically for a mean of sixty-two months. A total of nineteen patients met the inclusion criteria. There were fourteen women and five men with a mean age of sixty-nine years. Six patients underwent an arthroscopy with associated holmium or yttrium-aluminum-garnet laser treatment, ten patients had associated radiofrequency treatment, and three patients had microfracture surgery. Subsequent arthroplasty procedures included four unicompartmental knee arthroplasties and fifteen tricompartmental knee arthroplasties. At the time of final follow-up, the mean Knee Society objective score was 95 points. Arthroscopic procedures may play a role in the development of osteonecrosis of the knee. To our knowledge, this is the largest series of patients to have development of this condition after arthroscopy with associated laser, radiofrequency, or microfracture surgery. The midterm results of knee arthroplasty in this unique patient population are comparable with those of patients undergoing knee arthroplasty for osteoarthritis of the knee. Therapeutic Level IV. Medial unicompartmental knee arthroplasty with the Miller-Galante prosthesis.

Unicompartmental knee arthroplasty has become a popular treatment alternative for osteoarthritis that is confined to the medial part of the knee. Excellent intermediate-term results recently have been reported in association with the Miller-Galante unicompartmental implant. The purpose of the present study was to report on our longer-term experience with the Miller-Galante medial unicompartmental knee implant. We evaluated the results of 113 medial unicompartmental knee arthroplasties that had been performed with use of the Miller-Galante implant in eighty-four patients between 1989 and 2000. The mean age of the patients at the time of surgery was sixty-eight years. Forty-five patients were men, and thirty-nine were women. Thirteen patients (sixteen knees) died at a mean of seven years after the index arthroplasty. No patient was lost to follow-up. The remaining seventy-one patients (ninety-seven knees) were followed for a mean of ten years and were evaluated with use of the Knee Society clinical and radiographic rating system. Eleven knees were revised at a mean of four years after the index procedure. The mean Knee Society knee and function scores for the sixty-one patients (eighty-six knees) who were living and who had not had a revision improved from 48 and 53 points preoperatively to 93 and 80 points at the time of the most recent evaluation. The five and ten-year rates of survival were 94% and 90%, respectively, with revision to tricompartmental knee arthroplasty as the end point and 93% and 86%, respectively, with revision or radiographic loosening as the end point. The Miller-Galante medial unicompartmental knee arthroplasty provided excellent pain relief and restoration of function in carefully selected patients and demonstrated durable implant survival at ten years.

Unicompartmental knee arthroplasty for younger patients: an alternative view.

Unicompartmental knee arthroplasty has been a controversial operation for the past 3 decades. Many surgeons in the 1980s and 1990s found little or no indication for the procedure. Others were enthusiastic for its use in selected patients with osteoarthritis who had unicompartmental disease. Initially, unicompartmental knee arthroplasty was thought to be appropriate for the elderly patient who was sedentary. With the advent of minimally invasive techniques, indications have expanded to include its use in younger patients (especially females) as an alternative to osteotomy or tricompartmental knee arthroplasty. Advantages over osteotomy include higher initial success, greater longevity, and fewer early complications. If done conservatively, salvage is not difficult. The extent of safe postoperative activity levels has yet to be established. Failure rates seem to be higher in heavy active males. A metallic interposition hemiarthroplasty in the form of a McKeever or Macintosh prosthesis has been available for 50 years as a unicompartmental arthroplasty, but with limited use. It still may have a role in selected patients as a conservative temporizing procedure.

The indications for unicompartmental arthroplasty are quite specific. Overall, there should be involvement of 1 tibiofemoral compartment, mild malalignment, and ligamentous stability. In a review of 250 osteoarthritic knees, these pathologic findings were present in 19 patients (8%). Technique, despite improvement in instrumentation, still is more demanding than tricompartmental knee arthroplasty, and failure to achieve an optimal arthroplasty occurs more frequently. In the literature, long-term results with unicompartmental arthroplasty have been inferior to tricompartmental arthroplasty. The concept that unicompartmental arthroplasty is a temporizing procedure is not a valid one in that the patient has to undergo another arthroplasty with all the risks of revision arthroplasty surgery. Many of these revision procedures require bone grafting or component augmentation. Tibial osteotomy remains the procedure of choice in younger, active patients with unicompartmental arthritis (in highly selected patients), and tricompartmental arthroplasty is preferred in older, lower demand patients. Unicompartmental knee arthroplasty in middle-aged patients: A 2- to 6-year follow-up evaluation

Twenty-eight unicompartmental knee arthroplasties performed as an alternative to high tibial osteotomy or tricompartmental knee arthroplasty in patients under 60 years of age were reviewed after 2 to 6 years of follow-up. The patient's age at the time of operation averaged 52 years. Using the Knee Society Score, 90% were rated good or excellent results in terms of function and pain relief. The average flexion angle obtained was 124°, and the average postoperative alignment was 4° of anatomic valgus for varus deformities and 8° for valgus deformities. The average activity level according to the Tegner and Lysholm score slightly improved (preoperative, 2.3; follow-up, 2.7 points). Of the 28 knees, 9 (32%) presented radiolucent lines about the tibial component and two had incomplete radiolucent lines at the bone-cement interface on the femoral side. There was no correlation between activity level and tibial radiolucent lines. Two revisions were performed because of loosening of the femoral component at the prosthesis-cement interface. One was converted to another unicompartmental arthroplasty and the other to a tricompartmental arthroplasty. One tibial component exhibited an asymptomatic slowly progressive radiolucency. Unicompartmental knee arthroplasty in middle-aged patients yields 2- to 6-year results competitive with osteotomy but inferior to tricompartmental arthroplasty in terms of revision. The specific prosthetic design used in this series appeared to be vulnerable to femoral component loosening possibly because of constrained tibial topography and smooth tapered femoral fixation lugs.

Contact pressure and three-dimensional tracking of unresurfaced patella in total knee arthroplasty

Ten fresh-frozen cadaver knees were studied to investigate the contact pressure distribution and three dimensional (3-D) tracking of the patella under consecutive three different conditions. They have included (a) an intact knee, (b) the unresurfaced patella in bicompartmental knee arthroplasty (Bi-TKA), and (c) the resurfaced patella in tricompartmental knee arthroplasty (Tri-TKA). A pressure-sensitive conductive rubber (PSR) sensor system was developed to measure the patellar contact pressure dynamically, and two sets of high resolution video cameras with direct linear transformation method was used to measure the 3-D patellar tracking. Total contact area on the patella was decreased in Bi-TKA, and further decreased in Tri-TKA. However, patellar contact area was significantly less in Tri-TKA than in the intact knee in 0 to 100 degrees of knee flexion ( P < 0.01), and no statistical difference was seen between Bi-TKA and the intact knee. The mean contact pressure was conversely highest in Tri-TKA, followed by Bi-TKA, and intact knee. The mean contact pressure of Tri-TKA was significantly higher than intact knee in 50 to 100 degrees of knee flexion ( P < 0.05), but no statistical difference was seen between Bi-TKA and intact knee. Three-dimensional patellar tracking showed the same tracking pattern in intact knee, Bi-TKA, and Tri-TKA. The results of this study suggest that in this type of TKA system the patellofemoral congruity is not significantly reduced in Bi-TKA, and we could leave the patella unresurfaced during TKA.

Early failure of the porous coated anatomic cemented unicompartmental knee arthroplasty: Aids to diagnosis and revision

Unicompartmental knee arthroplasty (UKA) is performed less frequently than total (tricompartmental) knee arthroplasty (TKA). This study examined the range of presenting symptoms and usefulness of diagnostic tests to determine the failure mode; the outcome of surgical revision of failed UKA is also reported. From a consecutive prospective series of 43 Porous Coated Anatomic (Howmedica, Rutherford, NJ) UKAs performed between 1985 and 1992 and followed for an average of 64 months, 12 cases have come to revision surgery (28%). The average time to failure was 37 months. Symptoms preceded revision surgery by an average of 10 months. The most common presenting symptom was pain (100%), followed by swelling (92%), reduced range of motion (42%), instability (42%), and clicking (17%). Failure was caused by polyethylene wear in 50%, loosening of the femoral component in 42%, and progression of patellofemoral arthritis in one patient. The combination of single leg standing anteroposterior radiographs and supine lateral radiographs detected most causes of UKA failure. A bone scan was confirmatory in every case of suspected loosening of the femoral component. Arthroscopy diagnosed polyethylene wear in two cases and progression of joint arthritis in one case and was not helpful in one case. Revision surgery was done with primary TKA components, and follow-up periods averaged 27 months. Bone stock deficiency was found in 58%, but required bone-grafting in only one case. Revision surgery successfully restored pain-free function and range of motion in all cases. Two-year postrevision Hospital for Special Surgery scores are equal to those for primary TKA. Survivorship analysis showed a 33% failure rate at 57 months after Porous Coated Anatomic UKA when revision was the endpoint and a 41% failure rate when unsatisfactory clinical status was the endpoint. Regular follow-up evaluation is suggested for the Porous Coated Anatomic UKA.

Cell quality of salvaged blood after total knee arthroplasty. Drain blood compared to venous blood in 32 patients.

We used the ConstaVac drainage retransfusion system in 32 patients operated on with cemented tricompartmental knee arthroplasty. The mean total postoperative bleeding was 1.1 L of which 0.7 L was retransfused within 8 hours postoperatively. Samples were taken from the venous blood and drain blood at 2 hours and 6-8 hours postoperatively. The drain blood had low counts for leukocytes and thrombocytes as compared to venous blood, and fibrinogen was almost absent. Incubation at room temperature slightly lowered glucose and pH which made the erythrocytes swell. There was a slight hemolysis in the drain blood at 2 hours. At 6-8 hours the shed blood was closer to normal, especially with regard to hemolysis, but there was a further decrease in glucose. Within this time, there was no change in acidity and no further swelling of the erythrocytes. No clinical adverse reactions were observed and we consider the observed cellular and chemical changes to be of little importance. The quality of filtered drain blood within the analyzed time limit is considered acceptable for clinical use.

Medial Unicornpartmental Arthroplasty

One hundred twenty-one knees with medial compartment osteoarthrosis were treated by unicompartmental arthroplasty with the Oxford Knee. The strict selection criteria were (1) the presence of a functioning anterior cruciate ligament, (2) fully correctable deformity, and (3) full thickness of articular cartilage in the lateral compartment. The mean elapsed time from surgery was 44.4 months. One knee has required revision for a loose tibial component. The results at this stage are as acceptable as those of tricompartmental knee arthroplasty and better than those of high tibial osteotomy.

A Comparison of Tricompartmental and Unicompartmental Arthroplasty for the Treatment of Gonarthrosis

An historical prospective study was performed to compare two surgical management alternatives in the treatment of patients with knee arthritis. There were 120 unicompartmental and 81 tricompartmental knee arthroplasties in 98 and 66 patients, respectively. All living patients were available for follow-up observation, and survivorship data on all arthroplasties were obtained. The average follow-up interval was 78 months (range, eight-162 months) in the unicompartmental series and 68 months (range, two- 186 months) in the tricompartmental group. Patients receiving the unicompartmental arthroplasty were treated with nonmetal-backed polyethylene tibial components. Prosthetic survivorship was 92% at ten years in the unicompartmental patient group. There were no statistically significant differences in aseptic loosening between these two patient groups. In appropriately selected patients unicompartmental arthroplasty was associated with better range of motion and ambulatory function than patients being treated with tricompartmental knee replacement.
