The recognition of viral and chlamydial infection in cervical smears

In the 12 mth ended 30 June 1976, 14,868 cervical smears (from 13,212 patients) were examined in the Cytology Department, King George V Hospital, Sydney. Forty-two smears (1 in 354), from 37 patients, showed features of viral or chlamydial infection. Appearances were considered diagnostic in 21 smears and suggestive of infection in a further 21 smears. Changes were seen which were attributable to herpes virus, the virus of condyloma acuminatum, cytomegalovirus, adenovirus and TRIC agents. Six smears showed changes consistent with infection by virus other than those mentioned. The most characteristic feature of viral and chlamydial infection is the presence of inclusion bodies in endocervical and metaplastic squamous cells. Intranuclear inclusions are seen in herpes (where the cells are often multinucleate), cytomegalovirus and adenovirus infection. These are eosinophilic in herpes and adenovirus and basophilic in cytomegalovirus infection. Eosinophilic intracytoplasmic inclusions are seen in TRIC agent infection and sometimes in cytomegalovirus infection. Recognition is important for several reasons. Cytologically, changes due to viral and chalmydial infection may be present in association with or may also to some extent simulate precancerous or cancerous changes. Epidemiologists have considered the possibility that a sexually transmitted viral carcinogen is important in the aetiology of cervical cancer. Clinically infection may be asymptomatic or be responsible for severe, even necrotizing, cervicitis. The presence of herpes, cytomegalovirus and TRIC agents in the genital tract in early pregnancy is associated with abortion, prematurity and dysmaturity. Some obstetricians consider the presence of herpes or cytomegalovirus in late pregnancy an indication for caesarean section before membrane rupture in order to prevent serious, and sometimes fatal, infection of the neonate occurring during delivery through an infected birth canal.

Light and electron microscopic study of Chlamydia trachomatis infection of the uterine cervix.

Cervical biopsy specimens from two patients with infections caused by Chlamydia trachomatis (TRIC agent) were studied by means of light and electron microscopy. Intraepithelial vesicles containing C. trachomatis were present in the cervical mucosa. These intramucosal vesicles were readily recognized in paraffin sections by means of light microscopy. The developmental stages of C. trachomatis, which have previously been seen in tissue cell culture, chicken embryo, and conjunctival infections, were also apparent in cervical infection.

The development of a TRIC agent (Chlamydia trachomatis) and its associated polysaccharide in suspended cell cultures.

SUMMARY: The growth of large quantities of a TRIC agent and the concomitant production of polysaccharide were studied in BHK21 and L cell cultures in suspension. The maximum concentrations of elementary bodies and polysaccharide/culture were attained at about the same time (60 h) after infection in both cell systems. The yield of elementary bodies from a 11 culture was equivalent to the maximum obtained from a total area of 8000 cm2 of BHK21 monolayers. Polysaccharide found in the medium was probably derived from disrupted inclusion bodies. It is postulated that synthesis of the polysaccharide is directed by the TRIC agent but controlled by the host cell.

Sensitivity of trachoma agent to streptomycin and related antibiotics.

For isolating trachoma (TRIC) agent from conjunctival scrapings, both streptomycin and neomycin were effective in preventing bacterial contamination, but at high concentrations neomycin was rather more inhibitory to TRIC agent. Prolonging storage of scrapings with neomycin at 4 degrees from 30 min. to 24 hr. reduced the bacterial contamination rate, but also diminished the chance of isolating TRIC agent at the 1st passage. Two freshly isolated TRIC agents differed in their susceptibility to neomycin and streptomycin. Kanamycin and framycetin appeared to be less suitable than streptomycin for use in isolation of TRIC agent. In isolation studies, the possibility of inhibiting TRIC agent by high concentrations of antibotics, including streptomycin, should be borne in mind.

Haemagglutination by the TRIC group of Chlamydia.

Summary Several strains of TRIC agent produced agglutinins for mouse and rat erythrocytes during growth in embryonated hens' eggs. The agglutinins appeared irregularly and in a low percentage of inoculated eggs. A small inoculum dose was more likely to give a good yield, and a lethal inoculum failed to produce detectable amounts of haemagglutinin. The haemagglutinins were separable from the infectious particles and, with the exception of those associated with the T'ang strain, were unstable on storage. The agglutinin for rat erythrocytes was more often present in the allantoic fluid whilst the mouse cell agglutinin was more cell-associated. The T'ang strain also produced a haemolysin.

TRIC agent as a cause of neonatal eye sepsis.

Among 2,700 babies born in a maternity hospital there were 71 (2.6%) cases of eye sepsis, mostly mild. Forty-four of these cases were screened for inclusions of TRIC agent. Four certain positives and a further four very suggestive cases were found-an incidence of 18% of the cases of eye sepsis investigated. The true incidence is probably somewhat higher.Follow-up showed that even minimal and transitory eye sepsis from TRIC infection in the neonate can lead to permanent scarring of the conjunctiva and cornea, with or without pannus.

Genital infection with TRIC agents in Taiwan

Agents of the trachoma-inclusion conjunctivitis type (TRIC agents) have been recovered from the ocular and genital tracts of men and women. This study is an attempt to define the prevalence of cervical TRIC agents in Taipei. Two of 86 pregnant Chinese women yielded TRIC agents one or more times. One infant developed inclusion conjunctivitis. None of 33 pregnant American women and one of 29 American and Chinese women attending family planning clinics in Taipei harbored cervical TRIC agents. Two of 59 prostitutes yielded the same cervical organisms. Only the pregnant women had significant cervicitis. None of the genital strains recovered were of the same two types of TRIC agent recovered on numerous occasions from ocular trachoma in Taiwan. The possibility that endemic ocular trachoma and genital TRIC agent infections are distinct etiologic entities is discussed.
