Seropositivity and Trends of Transfusion Section Transmitted Infections among Blood Donors: Five Years Cross-sectional Study on 20,392 Blood Donors in a Tertiary Care Hospital of Ahmedabad, Gujarat, India

Introduction: Blood transfusion is a lifesaving intervention. However, it can also be a source of Transfusion-transmissible Infections (TTIs), posing a potential threat to the recipient. Testing for TTIs before blood transfusion is crucial for the safety of recipients. However, donations occurring during the window period, the prevalence of asymptomatic carriers, viral strains with high genetic variability, and technical errors are responsible for TTIs and remain one of the greatest obstacles in transfusion medicine to deal with. All blood donors are to be screened against five major infections-Human Immunodeficiency Virus (HIV), Hepatitis B Virus (HBV), Hepatitis C Virus (HCV), Syphilis, and Malaria. Aim: To study the seroprevalence and trends of TTI among healthy blood donors at the Blood Centre of Ahmedabad, Gujarat, India. Materials and Methods: A cross-sectional study was carried out by reviewing blood donors’ records over a period of five years from January 1, 2018, to December 31, 2022, at the Blood Centre, Department of Pathology, of Sheth L.G. General Hospital, Maninagar, Ahmedabad, Gujarat, India. The data collected for each seropositive donor included the results of TTI testing, the age group of donors, the type of donation, the frequency of donation, and co-infection. The results were expressed in numbers and percentages. Results: Among a total of 20,392 healthy blood donors during the five-year period, the total number of seropositive cases (n) was 243 (1.19%). TTI seropositive donors in years 2018, 2019, 2020, 2021, and 2022 were 60 (1.29%), 50 (1.16%), 39 (1.22%), 45 (1.14%), and 49 (1.16%), respectively. A maximum of 118 (48.56%) seropositive donors were recorded in age group of 18-29 years. Individual seropositive donors during the fiveyear period for HIV, HBV, HCV, syphilis, and malaria were 19 (0.09%), 155 (0.76%), 28 (0.14%), 41 (0.20%), and 00 (0.00%), respectively. Voluntary Blood Donors (VBD) 10,933 (53.61%) exceeded Replacement Donors (RD) 9,459 (46.39%), and repeat blood donors 14,462 (70.92%) surpassed first-time donors 5,930 (29.08%). The seroprevalence of TTI among replacement, voluntary, first-time, and repeat blood donors was 2.41%, 0.14%, 2.06% and 0.84% respectively. Out of 243 seroreactive donors, two donors (0.82%) showed dual reactivity for TTI during the five-year period. Conclusion: Implementation of strict donor selection criteria, utilisation of sensitive screening modalities, promoting public awareness, and dispelling myths regarding the merits of voluntary blood donation, judicious use of blood products, and mass immunisation for Hepatitis B infection are essential interventions needed to curb TTI to a significant extent.

Evaluation of Transfusion Transmissible Infections Prevalence and Trend Among Blood Donors Attended at Bule Hora Blood Bank, West Guji, South Ethiopia

Transfusion transmissible infections (TTIs) are among the greatest threats to blood safety for recipients and the economy of the developed and developing countries. Therefore, the study of the prevalence and trends of TTIs is important to assess for its prevention and control strategies. This study aimed to evaluate the prevalence and trend of transfusion transmissible infections among blood donors at the Bule Hora Blood Bank, West Guji, Ethiopia, from 2019 to 2021. A retrospective study was conducted on all blood donors at the Bule Hora Blood Bank from 2019-2021. The data of 4193 blood donors were collected from the databank (logbook) of the blood bank using a checklist. Data were analyzed by using SPSS software, and the chi-square test was used to assess the association between independent variables and transfusion transmission infection. Different types of graphs, charts and tables were used to present the data. P values less than .05 were considered statistically significant. The overall transfusion transmissible infection among blood donors was 4.6% (192). Out of the tested blood donors, 86 (2.1%) were found to be seropositive for hepatitis B virus. The trend analysis showed that the positivity rates of the screened tests were 28 (4.72%) in 2019, decreased to 77 (4.26%) in 2020 and increased to 87 (4.86%) in 2021. The sex of blood donors was significantly associated with TTIs (P value = .021), and the positivity rates were significantly higher among male than female blood donors. The study results showed that overall seropositivity rates increased along the age groups of the donors. The study reported a moderate prevalence of transfusion transmissible infections among blood donors visiting the Bule hora blood bank. Health education about the transmission, prevention and create awareness of TTIs should be strengthened by the government and non-government organization.

Estimating the Transmission Risks of Viral Hepatitis and HIV Among Blood Donors in Hossana, Southern Ethiopia.

PurposeScreening of viral transfusion-transmissible infections (TTIs) among blood donors is of public health concern. It is a cost-effective method to monitor the occurrence, distribution, and trends of TTIs in healthy people. This study aimed to estimate the magnitude of the three common viral TTIs among blood donors in Hossana, Ethiopia.MethodsA cross-sectional study was conducted among 417 blood donors from April to May 2020 in Southern Ethiopia. Data were collected using a structured questionnaire and laboratory blood screening for hepatitis B virus (HBV), hepatitis C virus (HCV), and human immunodeficiency virus (HIV) using Wantai AiDTM HBsAg, anti-HCV, and HIV 1 + 2 Ag/Ab ELISA. Data were entered into Epi-Data version 3.1 and analyzed using SPSS version 21.0. A binary logistic regression model was fitted to identify factors associated with each viral infection. The odds ratio with a 95% confidence interval was calculated. A p-value <0.05 was considered statistically significant.ResultsA total of 417 blood donors participated in this study producing an overall prevalence of viral TTI was 14.38%. HBV, HCV, and HIV prevalence were 9.83%, 2.39%, and 4.31%, respectively. HBV-HIV was a common co-infection, which had 1.2%. In multivariate logistic regression analysis, family history of hepatitis (AOR=5.2, 95% CI (2.92, 7.41)) and multiple sexual contacts (AOR=4.2, 95% CI (2.32–7.43)) were significantly associated with HBV; low educational level (AOR=3.1, 95% CI (2.58–15.25)) and multiple sexual contacts (AOR=4.9, 95% CI (3.51–7.96)) were significantly associated with HIV, but the only variable alcohol consumption (AOR=2.7, 95% CI (6.72–23.76)) was also associated with HCV infection.ConclusionIn this study, the magnitude of viral TTIs among blood donors is high. This indicates that there are high risks of transmission for these infectious pathogens. Therefore, effective stringent donor selection and screening protocols should be developed.

Seroprevalence and trends of transfusion transmissible infections among retrospective blood donors in Western Azerbaijan Regional Blood Transfusion Center, Iran: A ten-years evaluation

Transfusion transmissible infections (TTIs) have been a public health challenge for the accessibility, quality and safety of blood transfusion. The present study aimed to consider the prevalence and the trends of hepatitis B virus (HBV), hepatitis C virus (HCV), Human T-cell leukemia virus type 1 (HTLV-1), human immunodeficiency virus (HIV) and syphilis across the ten years among retrospective blood donors. A retrospective investigation of blood donors’ data covering the period from 22 May 2009 to 22 May 2019 was done. Data was accumulated and analyzed from Blood Transfusion Center records, pertaining to all donors who were screened for various TTIs using respective immunological techniques. Out of the 682,171 screened donors in the 2009–2019 study period, 2470 (0.36 %) were infected with at least one infectious agent. The overall prevalence of HBV, HCV, HTLV-1, HIV and syphilis were 1700 (0.25 %), 184 (0.027 %), 335 (0.05 %), 4 (0.0.05 %) and 247 (0.036 %), respectively. The study showed male dominated donor pool (96.79 %) with higher prevalence (0.34 %) of TTIs compared to female donors (0.02 %) with 3.21 % population. Despite the low prevalence of TTIs in our study, HBV, HCV, syphilis and HIV have remained a big threat to safe blood transfusion in Iran. Strict adherence to selection criteria, algorithm of donor screening, use of highly sensitive and specific methods for detection of TTIs, regular consultation and health education programs, prevention and sanitization strategies to reduce the risk of TTIs are recommended to reduce the risk of TTIs and ensure the safety of blood transfusion for recipient.

Prevalence and Trends of Transfusion Transmissible Infections in Blood Donors in a Tertiary Care Centre- An Institutional 4-year Retrospective Study

Introduction: In today’s time, safe transfusion begins with the screening and confirmation of availability of healthy donors. An important criterion to make safe blood available for one and for all is to try and prevent Transfusion Transmitted Infections (TTIs); and once if a donor is identified to be reactive for any one of the TTIs, then to notify and counsel them. Aim: To study the seroprevalence and trends of TTIs in blood donors at MGM Medical College and Hospital, Navi Mumbai, Maharashtra, India. Materials and Methods: This retrospective study was done in the Department of Immunohaematology and Blood Transfusion (IHBT) at MGM Medical College and Hospital, Navi Mumbai, Maharashtra, India for period of four years from January 2017 to December 2020 (48 months). Screening for anti-Human Immunodeficiency Virus (HIV)-1/2, anti-Hepatitis C Virus (HCV), and Hepatitis B surface Antigen (HBsAg), Venereal Disease Research Laboratory (VDRL) for syphilis, and slide test for malaria were done. Data was collected from various registers within the blood centre. It was entered in MS Excel spreadsheets and calculation was done for calculating the prevalence. R-square value was used to determine the trend of TTIs. Results: A total of 19,864 donors were analysed for prevalence of TTIs from 2017 to 2020. Of these 99.16% were voluntary donors and 0.84% were replacement donors. Prevalence of TTI in total donors was 1.11%. Prevalence of hepatitis B was highest (0.84%) followed by syphilis (0.11%), HIV (0.07%) and HCV (0.09%) and malaria (0%). Prevalence was more in male replacement donors. Conclusion: The prevalence of TTI in total donors was 1.11%. The number of total blood donations has been on the rise. Cases of HCV were steadily declining, but HIV, HBsAg, and syphilis showed a non linear trend. Strict adherence to the donor selection rules is needed to supply quality and safe blood to the patient.

Sero-prevalence and trends of transfusion transmissible infections among blood donors in a rural tertiary care centre- A 7 years study

Blood transfusion saves millions of life in emergency situations and is considered has life saving drug. Blood transfusion is associated with several risks which also includes transfusion transmissible infections. With the implementation of strict donor selection, effective and sensitive screening tests can ensure the least reduction risk of acquiring transfusion transmissible infections. The prevalence of transfusion transmissible infections among blood donors reflect the burden of diseases among particular population. The aim of this study is: 1. To estimate the prevalence of transfusion transmissible infections among blood donors in a rural tertiary care centre; 2. To analyse the trends of transfusion transmissible infections over study period. A total of 21,089 voluntary blood donors were studied. There were 20,356 male donors and 733 female donors with ratio of 28:1. The prevalence of HIV, HBV, HCV, Syphilis and Malaria was found to be 0.14%, 0.82%, 0.02%, 0.13% and 0.03% respectively. The prevalence of HBV was highest (0.82%) followed by HIV(0.14%). Our study also showed decrease in trends of HCV and Syphilis over study period. There was strong association of transfusion transmissible infections among O positive blood group. The prevalence of transfusion transmissible infections has decreased over time, hence implementation of strict donor selection criteria and use of sensitive screening tests has lead to decrease in trends.

BackgroundMillions of lives around the world are being saved annually through blood transfusion. However, blood transfusion is among the essential vehicles for transmitting infections. The overall prevalence of Transfusion Transmissible Infections among blood donors differs around the world, reflecting the variation in the prevalence of these infections. This study aims to assess the prevalence and trends of Transfusion Transmissible Infections among blood donors in Qatar.MethodsThis is a cross-sectional study utilizing donation records of 5 years from January 2013 to December 2017. We included in the study results for all screening and confirmatory tests for Hepatitis B Virus, Hepatitis C Virus, Human T-lymphotropic Virus-I/II, Syphilis and Malaria.ResultsAmong the 190,509 donations received at the donation centre during the study period, about 91% of donations were received from males and 9% from females. The overall positivity rate for all tests was 1.87, 2.23, 1.78, 2.31, 2.67% for the years 2013 through 2017, with an increasing yearly trend by 6% each year. The overall positivity rates for Hepatitis C Virus, Human T-lymphotropic Virus-I/II, Hepatitis B Virus, Syphilis and Malaria (2013–2017) were 0.60, 0.18, 0.30, 0.43 and 0.20%, respectively.ConclusionThe overall positivity rate of all tests combined for the Transfusion Transmissible Infections demonstrated a gradually increasing trend from 2013 to 2017. However, the trend for each infection (Hepatitis C Virus, Hepatitis B Virus, Syphilis and Malaria) was fluctuating except for Human T-lymphotropic Virus-I/II, which was increasing. Supporting the development of effective prevention and control strategies requires further comprehensive investigations for better estimation of the burden of these infections.

Prevalence and trends in transfusion-transmissible infections among blood donors in Brazil from 2010 to 2016

BackgroundAssessing trends in the rate of transfusion-transmissible infections (TTIs) in blood donors is critical to the monitoring of the blood supply safety and the donor screening effectiveness. The objective of this study was to conduct a trend analysis of TTIs and associated demographic factors of donors at a public blood bank in the central Brazil. MethodsA retrospective analysis (2010–2016) of blood donation data was performed to determine the prevalence of markers for TTIs. Multinomial and multivariate logistic regression were used to verify the association between the explanatory variables and TTIs. The trend was evaluated with the Prais Winsten’s regression analysis. ResultsThe prevalence of TTIs was 4.04% (5,553 donors) among 137,209 donors, with a steady trend in the analyzed period. The seroprevalence for the hepatitis B virus (HBV), syphilis, hepatitis C virus (HCV), human immunodeficiency virus (HIV), Chagas disease, and human T-lymphotropic virus (HTLV) were 1.63%, 0.87%, 0.46%, 0.21%, 0.21% and 0.09%, respectively. The prevalence of HBV decreased (b = −0.021, p < 0.001), while syphilis increased (b = 0.112; p = 0.001), during the period investigated. The probability for a positive test for TTI was higher among donors with a low level of education, aged ≥30 years old, without stable marital status, and first-time donors. ConclusionsEven with the reduction in HBV, the increased rate of syphilis may have contributed to the fact that the overall rate of TTIs did not decrease in the evaluated period.

Seroprevalence and trends in transfusion transmissible infections among voluntary non-remunerated blood donors at the Malawi Blood Transfusion Service-a time trend study.

Collecting blood from voluntary non-remunerated blood donors from low risk populations is a key strategy for blood safety. Identifying such populations involves analysis of population and blood donor data to identify risk factors for transfusion transmissible infections (TTIs). There are no recent seroprevalence statistics for blood donors in Malawi. This study fills this gap by describing characteristics of blood donors, trend of annual prevalence of HIV, HBV, HCV and syphilis and factors associated with each TTI. Retrospective analysis of blood donors' records in the MBTS database from 2011 to 2015 was undertaken. Summary statistics were performed to describe characteristics of the blood donors. Univariable and multivariable logistic regression analyses were performed to determine association between prevalence of infections and socio-demographic factors. Time trend analysis was done to assess changes in prevalence. P-value <0.05 was considered statistically significant. The number of blood donors screened over the 5 year period was 125,893. The mean number of donors donating blood per year was 39, 289; median age was 19 years; 82% were male, 87% single and 72% students and56% were repeat blood donors. Overall prevalence of each TTI decreased. The 2015 prevalence was: 3.6% for HBV; 1.9% for HIV; 2.6% for Syphilis and 1.0% for HCV while the 2011 prevalence was 4.7% for HBV; 3.5% for HIV 3.2% for syphilis and 2.4% for HCV.Repeat blood donors had significantly lower prevalence of TTIs than first time donors. Females were associated with lower risk for HBV, HCV and syphilis. Age ≥25 years and being out of school were associated with HIV. Age ≥25 years was associated with reduced risk for HCV and being self-employed and married were each associated with syphilis. The typical blood donor is a young single male student. Repeat blood donation improves safety of the blood supply.

Sero-prevalence and trends of transfusion-transmissible infections among blood donors at Bahir Dar district blood bank, northwest Ethiopia: A four year retrospective study.

BackgroundBlood transfusion is an effective treatment for saving millions of lives even though transfusion- transmissible infections are the major problem. Therefore, the aim of this study was to assess the sero-prevalence and trend of transfusion-transmissible infections among blood donors.MethodsA retrospective study was conducted form July 2014 to June 2018 at Bahir Dar district blood bank. Descriptive statistics was presented using percentages, medians and interquartile ranges. Logistic regression was used to explore risk factors associated with each transfusion transmissible infections.ResultFrom a total of 35,435 blood donors 2130 (6.0%) of them had serological evidence for at least one infection and 50 (0.14%) of them were confirmed as having multiple infections. The overall sero-prevalence of HBV, HCV, HIV and syphilis was 230 (6.0%) with 3.9%, 0.6%, 0.5% and 1.2% respectively. From those who had co-infection majority of them 22 (44.0%) were attributed to HBV-Syphilis co-infection and 1 (2.0%) study participant was co-infected with HBV-HIV- Syphilis infection. There was an increment in the overall prevalence of transfusion-transmissible infection;183 in 2014/2015 to 624 in 2017/2018. The sero-prevalence of HBV show a significant increment tend with respect to year of donation. On the other hand HCV and HIV sero-prevalence show an increasing trend from 2014 and decrease in 2018. The sero-prevalence of syphilis was 67 (1.3%) in 2015 and duplicate in 2016, 138 (1.5) but subsequently decrease to 110 (1.1%) in 2017 and in 2018 it was 114 (1.0%).Conclusion and recommendationsHis finding showed growing evidence in the burden of transfusion-transmissible infection in blood donors despite which requires advanced and vigilance screening of donated blood prior to transfusion. More over there should be strategies for monitoring the implementation of post donation counseling for recruitment and retention of safe regular donors.

Studies on the trends of transfusion-transmissible infections in a district-level hospital in West Bengal, India

BACKGROUND: Therapeutic use of blood is the most sophisticated part of treatments in medical sciences. Transfusion of blood saves patients' lives as well as it has also the risk of infective disease, if it is contaminated. AIMS AND OBJECTIVES: The current study focuses on the status of TTI amongst the voluntary Blood donors over a period of time.MATERIALS AND METHODS: In this study, HIV, HCV, HBsAg and Syphilis (by RPR) screening test report for the year 2011 to 2017 of a district Hospital Blood Bank in West Bengal has been presented. Tests were done by ELISA and Syphilis by Rapid Plasma Reagin test.RESULTS: No significant changes in the incidence of above noted cases have been observed and no Malaria positive case was detected.CONCLUSION: This indicates a good awareness amongst the people regarding these diseases having immense public health importance.

Seroprevalence and Trends of Transfusion Transmissible Infections in Blood Donors in Andaman and Nicobar Islands- An Institutional Retrospective Study.

Transfusion Transmissible Infections (TTIs) such as Human Immunodeficiency Virus (HIV), Hepatitis B virus (HBV), Hepatitis C Virus (HCV) and syphilis pose a threat to blood safety in a developing country like India. To determine the prevalence of TTIs among healthy donors in Andaman and Nicobar Islands, India, and to further help in determination of the population subset to be focused on for enhancing blood donor pool. This retrospective cross-sectional study was conducted on all voluntary and replacement donors in a tertiary health care centre in Andaman and Nicobar Islands for a period of three years. Donor blood was screened for HIV, HBV, HCV, Syphilis and malarial parasite using rapid kits and ELISA. Out of 12,118 donors, 97.01% were males and 2.99% were females. 9409 (77.64%) were voluntary donors and 2709 (22.35%) were replacement donors. Majority of the screened donors belonged to 25 to 40 age group. Out of 12,118 donors, 265 (2.18%) were seropositive. Among them, 8 (0.066%) were HIV positive, 15 (0.124%) HCV positive, 128 (1.056%) HBsAg positive, 30 (0.247%) Syphilis positive and 84 (0.693%) were positive for malarial antigen. Yearly prevalence of seropositivity was 2.370%, 2.113% and 2.082% for three consecutive years. With high prevalence of TTIs among healthy donors, safe blood transfusion is still a challenge in developing countries. This continues to place unacceptable burden on health sector. There is a need for encouragement of voluntary blood donation, strict donor selection criteria and sensitive screening tests to minimize TTIs in Indian setting.

Transfusion transmissible infections in blood donors: A 7-year study in central Gujarat

<b>Context:</b> One of the major adverse effects of blood transfusion is transfusion transmissible infections (TTIs). Serological testing can help to reduce, but not eliminate occurrences of TTIs. Study of TTIs in different blood donor groups help to find safe blood for patients. <b>Aims:</b> The aim of this study was to determine the prevalence and trends in TTIs in voluntary and replacement blood donors. <b>Settings and Design:</b> Voluntary and replacement blood donors were studied for TTIs over the duration of 7 years. TTIs were documented with particulates like gender and type of donation (voluntary or replacement). <b>Materials and Methods:</b> Blood donation taken after predonation examination. Tests for TTIs carried out from donated blood and results were documented. <b>Statistical Analysis Used:</b> Data entered into Microsoft Excel sheets and studied for prevalence, trend, and odds ratio. <b>Results:</b> Total blood units taken during the period from 2007 to 2013 were 15,368. 93.1% were male donors and 6.9% were female donors. Prevalence of TTIs was 0.72%. Human immunodeficiency virus, hepatitis B surface antigen, hepatitis C virus, and syphilis were detected in 0.14%, 0.38%, 0.06%, and 0.14% of donors, respectively. No female donors had TTI. TTIs in voluntary donors (VDs) and replacement donors were 0.64% and 1.15%. <b>Conclusions:</b> Outcome of the study shows increasing trend for total blood donation, voluntary donations, and female blood donors. All TTIs were reducing except syphilis. To provide safe blood to patients, blood donation from VDs and female donors should be encouraged.

Trends in transfusion‐transmissible infections among Australian blood donors from 2005 to 2010

Routine monitoring of trends in transfusion-transmissible infections (TTIs) is essential to maintaining and improving transfusion safety. Although periodic studies have been published there is no comprehensive trend analysis for TTIs in Australian donors. This study determined recent trends in TTIs for which testing is conducted in Australia and described key attributes of infected blood donors. This is a retrospective analysis using data on donation testing for TTIs (2005-2010) from the national blood service donor database and data on postdonation interviews with TTI-positive donors (2008-2010) from a risk factor database incorporating responses to standardized interview questions. The study measured the prevalence and incidence of TTIs in Australia and assessed their time trends. Multivariate analysis of time trends was conducted using Poisson regression models. Overall, the prevalence and incidence of TTIs in 2005 to 2010 remained low and steady. The prevalence of hepatitis C virus decreased (rate ratio [RR], 0.93; 95% confidence interval [CI], 0.89-0.97) and the prevalence of active syphilis increased (RR, 1.51; 95% CI, 1.15-1.99) significantly during the study period. Prevalence of TTIs among Australian blood donors was substantially lower than that in the general population and no unique risk factors were identified in test-positive blood donors when compared with the general population. Both the prevalence and the incidence of TTIs in Australian blood donors remained low, with a steady or declining trend for most infections except active syphilis. The lower prevalence of TTIs in blood donors compared with the general population reflects the effectiveness of donor education and donor selection measures in Australia.

This study investigated the prevalence of serologic markers for transfusion transmissible infections (TTIs) among four Chinese blood centers.MethodsWe examined whole blood donations collected from January 2000 through December 2010 at four Chinese blood centers. Post-donation testing of TTIs (HIV, HBV, HCV and syphilis) were conducted using two different enzyme-linked immunosorbent assay kits for each seromarker. The prevalence of serologic markers for TTIs (%) was calculated and additional analysis was conducted to examine donor characteristics associated with positive TTIs serology.ResultsOf the 4,366,283 donations, 60% were from first-time donors and 40% were from repeated donors. The overall prevalence of HIV, HBsAg, HCV and syphilis was 0.08%, 0.86%, 0.51% and 0.47%, respectively. The prevalence profile of TTIs varied among different blood centers and appeared at relatively high levels. Overall, the prevalence of HBsAg and HCV demonstrated a decline trend among four blood centers, while the prevalence of HIV and syphilis displayed three different trends: constantly steady, continually increasing and declining among different centers.ConclusionsThis study reflects the risk of TTIs has been greatly reduced in China, but blood transfusion remains an ongoing risk factor for the spread of blood-borne infections, and further work and improvements are needed to strengthen both safety and availability of blood in China.
