Multiphase multidetector CT in the diagnosis of haemobilia: a potentially catastrophic ruptured hepatic artery aneurysm complicating the treatment of a patient with locally advanced rectal cancer

Acute haemobilia is an unusual and potentially catastrophic cause of gastrointestinal bleeding. We describe such a case presenting as a rare complication of a hepatic artery aneurysm following the development and successful treatment of subacute bacterial endocarditis during a radical downstaging chemoradiotherapy regime for locally advanced rectal cancer. We suggest that multiphase multidetector-row CT can have an important role in the diagnosis of acute haemobilia and discuss imaging findings associated with the condition. This case raises awareness of benign conditions mimicking malignancy in oncological patients and reinforces the importance of reviewing historical imaging.

Presentation of CaseA 67-year-old man was admitted to the hospital because of a ventricular septal defect and increasing exertional dyspnea.There was a history of a heart murmur since childhood. Oral treatment of subacute bacterial endocarditis in children.

Fourteen episodes of subacute bacterial endocarditis in 11 children were successfully treated with antibiotics, which in 13 cases were given largely by mouth. It is suggested that oral treatment of bacterial endocarditis should be considered for children who are only moderately but not severely ill, and who have sensitive organisms.

THE PRESENT methods of treatment of subacute bacterial endocarditis due to α- or γ-streptococci in penicillin-hypersensitive patients are the following: penicillin given cautiously in the hope that the history is in error or that there will not be a repeat occurrence of hypersensitivity; cortisone or one of its derivatives given concurrently with penicillin; desensitization to penicillin; ristocetin or cephalothin given instead of penicillin. 1-4 The two cases detailed below suggest that therapy with the combination of lincomycin and streptomycin is another method of therapy for use in penicillinhypersensitive patients who have endocarditis due to α- or γ-streptococci. 5-6 Report of Cases Case 1. —The patient was a 55-year-old white man admitted to Mount Sinai Hospital on Dec 20, 1965, because of chills and fever of one month's duration. Penicillin hypersensitivity with eosinophilia: A case report with immunologic studies

A case of penicillin hypersensitivity with marked eosinophilia and a transient skin rash is presented. The patient continued to receive large doses of penicillin intravenously for treatment of subacute bacterial endocarditis, and underwent successful open-heart surgery with replacement of the mitral valve. By means of the passive hemagglutination technic, direct skin tests and Schultz-Dale technic using unheated, heated and mercaptoethanol-treated serum, it was possible to demonstrate antibodies to penicillin in both IgG and IgM immunoglobulins during the first three months of the illness. At the thirtieth week only skin-sensitizing antibodies (probably IgA) to penicillin were demonstrable by these technics. The peripheral lymphocytes in a five day culture were stimulated and showed blast cell transformation when either penicillin G or penicilloyl polylysine was added. Direct Schultz-Dale experiments with the patient's cardiac muscle showed a contraction when penicillin G was added. This finding indicates the presence of anti-bodies fixed to the heart muscle. A strong contraction was observed with both the patient's and a control cardiac muscle when bradykinin, serotonin and epinephrine were added to the bath, but these tissues seem to be relatively insensitive to the action of histamine.

Four patients with subacute bacterial endocarditis are reported. These patients had demonstrated hypersensitivity to penicillin. The patients were treated simultaneously with penicillin and steroids. Complete and uneventful recovery occurred in three. The fourth patient tolerated the combined steroid-penicillin therapy and the streptococcus infection was eradicated. However, she subsequently developed subacute bacterial endocarditis due to Candida.

That phenoxymethyl penicillin (penicillin V) could be successfully employed in the treatment of subacute bacterial endocarditis was first demonstrated by Quinn and his associates, in 1956. 1,2 Some of their patients were treated with phenoxymethyl penicillin alone and some with this antibiotic in conjunction with intramuscular streptomycin. The following year Walker and Hamburger 3 reported four cases of subacute bacterial endocarditis caused by penicillin-sensitive streptococci successfully treated with two weeks therapy of oral phenoxymethyl penicillin and intramuscular streptomycin. As far as we know, however, no cases of Streptococcus faecalis endocarditis treated with oral penicillin have yet been reported. According to Hunter, 4 the acceptable form of therapy of S. faecalis endocarditis is a combination of 10,000,000 to 15,000,000 units of penicillin G per day, plus streptomycin. THE current treatment of subacute bacterial endocarditis requires the intramuscular administration of large amounts of penicillin. Although this route is generally well tolerated, pain and inflammation at local sites of injection can become a significant problem in the management of certain patients. Because of low and unpredictable serum levels attained after oral administration of penicillin G, effective oral therapy has not previously been available.Recent studies on phenoxymethyl penicillin (penicillin V) have suggested that this agent may represent a better means of oral administration. A case of subacute bacterial endocarditis is reported, in which the blood culture was positive only after one month showing a non-hemolytic streptococcus, Penicillin- Streptomycin- Terramycin- and Chloramphenicol- resistant, lightly Aureomycin-sensitive and distinctly Erythromycin-sensitive.2. The combined use of massive doses of Penicillin and Streptomycin and the use thereafter of Terramycin given intravenously which took place before the blood cultures became positive, proved ineffective.3. Clinical recovery, maintained since five months, followed the use of Erythromycin given alone, in doses of 2 g daily for one month and in progressively decreased doses for another month.4. Erythromycin given alone may be effective in the treatment of subacute bacterial endocarditis.

The use of penicillin has modified considerably the treatment and the end-results of subacute bacterial endocarditis. When the broad spectrum antibiotics came into use, hopes were fulfilled for reducing even further the mortality figures of this malady. Friedberg 1 considered the theoretical possibility that 95% of the cases of endocarditis could be cured. Unfortunately, this has not been the case. 2 There are still more than 5% of cases that must be considered therapeutic failures. On the other hand, the frequency of the disease seems to be decreasing, 3 perhaps due to the widespread use of the antibiotics. SINCE the introduction of antibiotics, considerable progress has been made in the treatment of subacute bacterial endocarditis. From the period of almost hopeless prognosis which prevailed before 1944 advance has been made to the stage where about 80% of the persons ill with this disease may be expected to respond successfully to treatment. Penicillin, the agent first used successfully, remains the most widely used and the drug of choice. Efforts to reduce the failure rate further have been directed mainly to a search for other antibiotics with wider or different spectrums of antibacterial activity. Reports of the use of streptomycin and aureomycin have appeared in the literature and indicate some degree of success. Another approach to the reduction of the failure rate is the use of combinations of antibiotics to take advantage of a possible additive or synergistic effect.
