Task force of the Brazilian Society of Otology — evaluation and management of peripheral facial palsy

ObjectiveTo review key evidence-based recommendations for the diagnosis and treatment of peripheral facial palsy in children and adults. MethodsTask force members were educated on knowledge synthesis methods, including electronic database search, review and selection of relevant citations, and critical appraisal of selected studies. Articles written in English or Portuguese on peripheral facial palsy were eligible for inclusion. The American College of Physicians’ guideline grading system and the American Thyroid Association’s guideline criteria were used for critical appraisal of evidence and recommendations for therapeutic interventions. ResultsThe topics were divided into 2 main parts: (1) Evaluation and diagnosis of facial palsy: electrophysiologic tests, idiopathic facial palsy, Ramsay Hunt syndrome, traumatic peripheral facial palsy, recurrent peripheral facial palsy, facial nerve tumors, and peripheral facial palsy in children; and (2) Rehabilitation procedures: surgical decompression of the facial nerve, facial nerve grafting, surgical treatment of long-term peripheral facial palsy, and non-surgical rehabilitation of the facial nerve. ConclusionsPeripheral facial palsy is a condition of diverse etiology. Treatment should be individualized according to the cause of facial nerve dysfunction, but the literature presents better evidence-based recommendations for systemic corticosteroid therapy.

A Combined Approach of Facial Neuromuscular Rehabilitation and Surgical Reconstruction in the Remediation of Facial Palsy: A Multidisciplinary Team Approach.

Facial neuromuscular rehabilitation (fNMR) is an evidence-based practice for the treatment of peripheral facial palsy (PFP). Surgical reconstruction can be indicated for patients who demonstrate poor or no recovery to support symmetry, function, and aesthesis. There is paucity of research demonstrating the therapeutic benefit of a multidisciplinary team (MDT) in facial recovery of this specific subpopulation of patients. This article will outline the role of specialist facial therapy in the remediation of PFP, focusing on those who undergo surgical reconstruction to optimize their facial recovery. Case studies are used to demonstrate surgical and therapeutic outcomes as well as the results of a patient survey conducted for a service evaluation. We discuss the role of the MDT in supporting recovery as well as the role of targeted fNMR. The term fNMR is often used interchangeably with facial therapy or facial rehabilitation. We will refer to fNMR as a technique of facial rehabilitation.We aim to demonstrate that an MDT approach to the treatment of people with facial palsy provides positive outcomes for this surgical population and that future research would be beneficial to support this service delivery model.

Deep Learning-Based Surface Nerve Electromyography Data of E-Health Electroacupuncture in Treatment of Peripheral Facial Paralysis.

This study was aimed at exploring the application value of electroacupuncture in the treatment of peripheral facial palsy using surface nerve electromyogram (EMG) image data based on deep learning. The surface nerve EMG recognition model was constructed based on multiview convolutional neural network, and the differences between it and the traditional single-view convolutional neural network were analyzed. Meanwhile, the influence of the multiview aggregation method based on pooling of view and decision fusion on facial recognition accuracy was compared and analyzed. 150 patients with peripheral facial paralysis were randomly divided into the control group (n = 70, basic treatment) and treatment group (n = 80, basic treatment + electroacupuncture). After 4 weeks of treatment, the therapeutic effect was evaluated by surface EMG parameters based on Horsfall-Barratt (H-B) scale and multiview convolutional neural network. The results showed that the face recognition accuracy of multiview convolutional neural networks was significantly higher than that of all single-view convolutional neural networks. The multiview aggregation network proposed in this research had a higher accuracy in facial recognition than the pooling of the view method and decision fusion-based multiview aggregation method. According to the evaluation results of H-B scale, the number of patients who recovered, significantly effective, effective, and ineffective in the control group was 39, 17, 3, and 11, respectively. The number of patients in the treatment group who recovered, significantly effective, effective, and ineffective was 51, 15, 9, and 5, respectively. Total effective rate of patients in the control group was 84.29%, and that of the treatment group was 93.75%, which was significantly higher than the control group (P < 0.05). According to surface EMG assessment results, compared with the control group, the mean root mean square (RMS), median frequency (MF), and mean power frequency (MPF) of the buccal and frontalis muscles in the treatment group increased significantly (P < 0.05). Compared with that before treatment, the mean buccal and frontalis RMS of patients in the control and treatment groups increased significantly after treatment (P < 0.05). In conclusion, electroacupuncture treatment could significantly improve the muscle strength of patients with peripheral facial paralysis.

Analysis of Clinical Research Trends for Acupotomy Treatment of Peripheral Facial Palsy

The purpose of this review was to investigate acupotomy treatment for peripheral facial palsy. By reviewing recent clinical trends, this may contribute to standardizing acupotomy treatment methods. There were 7 randomized controlled trials and 6 case series using acupotomy treatment for peripheral facial palsy published between January 01, 2014 and April 05, 2021, which were retrieved from 9 online databases. The number and characteristics of participants, main treatment sites, combination treatments, size of acupotomy needle, frequency and total period of treatment, evaluation indices, efficacy, and adverse events were analyzed. “Tender point or induration,” “infraorbical foramen,” and “buccal mucosa” were the most used treatment sites. The sizes of acupotomy needles varied from 20 mm to 80 mm in length, and 0.35 mm to 1.0 mm in diameter. One treatment cycle was performed every 3 to 5-7 days, and the number of treatments per treatment session ranged from 3 to 5-9 cycles. The results were evaluated using 1 to 4 evaluation indices and 9 different evaluation indices were used overall. The efficacy rate was the most used index, followed by the House-Brackmann grade, and electrocardiography. The “Risk of Bias 2,” categorized most studies as having “some concerns.” There were few adverse events reported.

Analysis of medical services provided to patients with peripheral facial palsy in Korea: a descriptive, cross-sectional study of the health insurance review and assessment service national patient sample database

BackgroundThis cross-sectional, retrospective, observational study analyzed the demographics of patients with peripheral facial palsy in South Korea and their use of healthcare services.MethodsThe 2016 Korean Health Insurance Review and Assessment National Patient Sample dataset was used; a total of 4790 patients, diagnosed with facial palsy, who had used healthcare services at least once between January 2016 and December 2016 were included, and data on the use of medical services, hospitalizations, sociodemographic factors, treatments, and medications were analyzed.ResultsOverall, 326 patients per 100,000 individuals used healthcare services at least once because of peripheral facial palsy in 2016, with higher numbers for women and those aged 50–59 years. The percentage of patients who used Korean traditional medicine (KM), Western medicine (WM), and both KM and WM was 54.4, 23.3, and 22.3%, respectively. Users of both WM and KM had higher per capita medical costs, more visits, and longer treatment durations. Physiotherapy was the most frequent WM treatment (44.4%), and “examinations” was the costliest (24.7%) category. “Procedures” was both the most frequent and costliest KM category (99.9 and 57.3%, respectively). “Continuous intravenous injections” (8.6%) and “superficial heat therapy” (8.3%) were the most frequent WM treatments, while acupuncture accounted for 98% of all KM treatments.ConclusionsThis study analyzed the demographic characteristics and medical service use of patients with peripheral facial palsy in detail. These results can be used as basic information to improve clinical and policy strategies for the management and treatment of peripheral facial palsy.

A Clinical Study of Collaboration between Western and Korean Medicine for the Treatment of Peripheral Facial Palsy in a Korean Medicine Hospital

Background: The purpose of this study was to investigate the characteristics and direction of treatment for peripheral facial palsy (PFP) based on medical collaboration between Western and Korean medicine departments.&lt;br/&gt;Methods: There were 195 outpatients with PFP identified retrospectively by examining electronic medical records. These patients were treated with Korean and Western medicine from January 1st, 2018 to December 31st, 2018 at the Kyung Hee University hospital. Records were analyzed according to the patients’ demographic characteristics and clinical features of the collaborative combined treatment.&lt;br/&gt;Results: According to the collaborative treatment pathway, the number of patients consulted from Western medicine departments was more than consulted from Korean department for the first time. The time taken by the Western medicine departments to consult with the patients at the Korean center for the first time was 14.9 days from the onset of symptoms. Acupuncture was the most frequently used Korean medicine treatment. The total treatment period for Korean medicine sessions and intervals were 91.9 days, 23 times and 3.6 days, respectively. When the Korean medicine center consulted with Western medicine departments, the time taken until the first consultation was 8.5 days from the onset of symptoms. Medication was the most used treatment, prescribed after 3.1 days. The most frequently used clinical test was an electromyogram, and this was performed after 20.5 days. The total treatment period, sessions and intervals were 21.2 days, 2.8 times and 5 days, respectively.&lt;br/&gt;Conclusion: The clinical status of collaborative treatment for PFP was determined including the timing and interval of consultation according to treatments.

Mixed mania followed by a short duration of corticosteroid administration in a patient with peripheral facial palsy: a case report

Corticosteroids are routinely used in the treatment of peripheral facial palsy. Affective symptoms have been reported as a possible side effect due to use of corticosteroids. After corticosteroid use psychiatric symptoms are often seen in the first weeks. More than half of them are seen in the first two weeks. Euphoria and hypomania are reported as the most common side effects of short-term steroid therapy and in chronic use depression is observed more frequently than mania. Lithium and olanzapine are used effectively in the management of psychosis related to the use of corticosteroids. Clinical aspects and response to olanzapine and lithium treatment have been discussed in a patient with peripherial facial palsy, who developed mixed mania due to short term use of corticosteroid treatment.

To Preliminary study of facial nerve F wave evaluation hypoglossal-facial nerve reconstruction with pre-degenerated nerve graft for peripheral facial palsy. The clinical data were retrospectively reviewed for 30 patients with peripheral facial palsy after cerebellontine angle underwent nerve anastomosis were analyzed between November 2011 and February 2014. F wave and M wave detection were used to evaluate the pre-, post-opertive and status.The hypoglossal-facial nerve by "side- to-side" anastomisis was performed. All patients were followed-up.The mean time of follow-up was 8 months (ranged from 3 to 20 months). 12 patients were found a higher recovery of facial nerve function after the nerve anastomosis.16 patients who F wave of post-opertive were found a higher recovery compared to F wave of pre-opertive (P < 0.05). The hypoglossal-facial nerve by "side"-to-side anastomisis with pre-degenerated auto-nerve graft was effective for the treatment of peripheral facial palsy after CPA tumor resection, F wave can be used as one of the objective index for the effect of the operation.

Our aim is to validate a protocol for the diagnosis and treatment of peripheral facial palsy. A cross-sectional and retrospective descriptive study was made of 63 patients diagnosed with facial palsy and given out-patient follow-up care. The average age was 41 years, with a 60 % preponderance of men, and the predominant side was the left one with a House-Brackmann degree of III on debut. The average time of evolution was 24 hours and the most frequent symptoms were mastoid pain and algiacusia. The treatment used was oral corticoids. Most were discharged in the first 60 days, without any relation with treatment or with the initial degree. Our protocol is valid and it allows suitable monitoring of this pathology. Most of the cases are resolved within the first 60 days. The tests habitually done have a low diagnostic yield, except for the electroneuronography, very useful within the first 21 days.

Therapeutic effect observations on individualized treatment of peripheral facial palsy

To investigate the curative effect of individualized treatment on peripheral facial paralysis. Methods:A treatment group of 121 patients was treated with acupuncture under an individualized plan based on the condition of disease. For a control study,118 patients were treated with conventional acupuncture. The courses of treatment and the curative effects were compared. Results:The cure and marked efficacy rate was 90.9% in the treatment group and 69.5% in the control group. There was a significant difference (P＜0.01).There was also a significant difference in the cure and marked efficacy rate in each courses of treatment between the two groups (P＜0.01). Conclusion:Individualized acupuncture treatment is better in the effect and shorter in the courses than conventional acupuncture treatment for peripheral facial paralysis.

Observation on the therapeutic effect of the comprehensive therapy in the treatment of peripheral facial palsy

Thirty cases of peripheral facial palsy were treated with the comprehensive therapy, and 30 cases of peripheral facial palsy were treated with shallow electro-acupuncture as control. The results showed that the cure rate in treatment group was higher than that in the control group (P<0.05), and no significant difference was found in effective rate between two groups.
