Observation the effect of double path endoscopic surgery in the treatment of fungal sinusitis

Objective To analyze the curative effect of double path endoscopic surgery in the treatment of fungal sinusitis, and to compare with the effect of simple in endoscopic nasal passages windowing. Methods The clinical data of fungal sinusitis patients were analyzed.According to the different surgery way, the patients were divided into two groups.The observation group: nasal endoscopic nasal passages open a window in the joint of fangs nest small windows double path; Control group: simple in nasal endoscopic fenestration. Results The observation group had 35 cases and the control group had 37 cases.After treatment, the symptoms score of the observation group was significantly lower than the control group [(1.09±0.83)points vs (1.47±0.77)points; t=2.015, P=0.048].The effective rate of the observation group was significantly higher than the control group (94.29% vs 75.68%; χ2=4.813, P=0.028).7 days after treatment, the C-reactive protein(t=4.971, P 0.05).2(t=5.978, P<0.01), 4(t=6.335, P<0.01), 6(t=9.348, P<0.01), and 8 days(t=8.226, P<0.01) postoperatively, 1-3-β-glucan D levels were significantly lower than the control group, the differences were statistically significant (P<0.05).The postoperative recurrence rate of the observation group was significantly lower than the control group (8.57% vs 27.03%, the Log-rank χ2=4.061, P=0.044). Conclusion In fungal sinusitis treatment, the use of endoscopic nasal passages open window joint fangs nest small windows double path can more effectively remove lesions, its forward curative effect is superior to pure in nasal endoscopic fenestration. The article presents main conceptions about the topical problem of the last decade, i.e. fungal or fungal-bacterial sinusitis. The contemporary data on etiology, pathogenesis features, clinical course, classification, approaches to diagnosis and treatment for fungal sinusitis have been given.

Mycotic infection of paranasal sinus could be the etiological factor of chronic sinusitis. The increase in number of fungal sinusitis cases have been reported recently among nonimmunocompromised patient after endodontic treatment of maxillary teeth. Nonspecific clinical signs and incorrect radiologic pictures interpretation as well as loss of therapeutic standards seems to be the cause of false negative diagnosis and difficulties in treatment of fungal sinusitis. Clinical and radiological picture of maxillary sinus aspergillosis was described in this paper. In the period of 2006-2009 in the Department of Maxillo-Facial Surgery 19 patient with fungal maxillary sinusitis was treated. The endodontic treatment of maxillary teeth of the related side was performed previously in 80% examined cases. In 2 cases there were immunocompromised patients with immunosuppressive treatment. In 16 cases patients were referred to our Department due to metallic foreign body of the maxillary sinus. Routine diagnostic radiological imaging was performed in each case: paranasal sinus view--Water's view and panoramic radiograph (orthopantomograph). In 4 cases imaging was extended with computer tomography (CT) visualization. The surgical treatment was performed in each case. The final diagnosis was puted on histopathological examination and fungal culture. In 16 cases of analysed group histopathological examination and fungal culture revealed aspergilosis. In 2 cases fungal culture was negative, but histopathology slices confirm presence of hyphae of Aspergillus. In 1 case the root canal sealer was found in the maxillary sinus. In none case invasive form of aspergillosis was confirmed. In all cases Water's view of paranasal sinuses and ortopantomograph showed partially or totally clouded sinus with well-defined, single or multifocal radiopaque object similar to metallic foreign body. Characteristic finding in CT imaging was well-defined radiodence concretions that have been attributed to calcium deposits in inflammatory changed mucosa, that might suggest "foreign body" picture. In 1 to 3 years follow-up control there was a recurrence of symptoms in one case. Foreign body of maxillary sinus have to be differentiated with aspergilosis. Metallic "foreign body" view in maxillary sinus seems to be characteristic sign of aspergillosis. The most often form of maxillary sinus aspergilosis is aspergilloma.

In the last few years an increase has been observed in the incidence of fungal sinusitis in immunocompetent subjects. We present a case of mycotic sinusitis in a 12-year-old patient, in which there were several aspects that could have made the diagnosis more difficult, thus delaying her definitive treatment. This case illustrates how to arrive at a correct diagnosis of this pathology. It must be based on the histological and radiological studies, but always without forgetting a key aspect as it is a suspected diagnosis. The treatment of fungal sinusitis is surgical, and currently, endoscopic sinus surgery is the choice. We describe the clinical characteristics of this pathology and the diagnostic methods required.

Primary and secondary fungal infections of the paranasal sinuses: Clinical features and treatment outcomes

Conclusion. In a retrospective study of 239 cases of fungal sinusitis, noninvasive paranasal sinus aspergillosis was most common and successfully treated by endoscopic sinus surgery (ESS) with postoperative sinus irrigation. For the treatment of fungal sinusitis, ESS with or without antifungal agents and control of predisposing factors for secondary cases are recommended. Objective. The aim of this study was to investigate the clinical characteristics of fungal sinusitis and evaluate the treatment outcomes of primary and secondary fungal infections of the paranasal sinuses. Materials and methods. Two hundred thirty-nine cases of fungal infection of the paranasal sinuses seen between January 1997 and December 2006 were retrospectively analyzed by reviewing their medical records. There were 200 cases of primary fungal infection and 39 cases of secondary fungal infection. Results. The symptoms of chronic rhinosinusitis such as nasal obstruction, purulent rhinorrhea, and postnasal drip were commonly present in both primary and secondary infections, and the sphenoid sinus was commonly involved in secondary infection. The radiological findings in fungal sinusitis included haziness, calcification, and bone destruction of the involved sinuses. CT scans in 80% of the primary and 69% of the secondary cases revealed calcific densities in a paranasal soft tissue mass. Twenty-eight of 38 cases which had MR showed decreased signal intensities on T1-weighted images and markedly reduced signal intensities surrounded by bright signal on T2-weighted images. In secondary cases, the most common concomitant disease was diabetes mellitus. All patients received surgery including biopsy, ESS, and Caldwell-Luc's operation. Mucosal hypertrophy with fungus ball, which was the most common finding in both types, was found in 124 cases (62%) with primary and in 26 cases (67%) with secondary cases, and aspergillosis was most common, followed by unidentifiable colony, and mucormycosis. Eleven cases received amphotericin-B postoperatively. Two hundred thirty-eight cases showed no recurrence during the mean follow-up period of 11 months.
