Clinical assessment of Citrosept Dental effectiveness in treatment of Candida-associated denture stomatitis, in patients using removable acrylic dentures

Introduction. For patients using removable dentures the level of comfort and thus the life quality has substantially increased with the development of prosthetic dentistry. However, denture stomatitis, especially accompanied by fungal infections, remains a problem and affects many patients using prosthetic restorations. It is mainly caused by an increasing resistance of fungi to antifungal medications. Aim of the study. To assess clinically the effectiveness of a herbal grapefruit seed extract product called Citrosept Dental in treating Candida-associated denture stomatitis, in patients using acrylic removable dentures. Materials and methods. The studied group included 32 patients (18 women and 14 men) of the Department of Prosthetic Dentistry, Medical University of Warsaw, aged 41 to 83 years (average age 62 years), with symptoms of denture stomatitis accompanied by fungal infections, which were confirmed by a mycological test (group 1). Patients with a positive mycological test result used 1% Citrosept Dental gel on mucosal lesions three times a day after meals, for three weeks. Results. The lesions disappeared completely in 24 patients (75%) after three weeks of Citrosept Dental application. The mucosa of 13 patients with type 1 stomatitis (81.25%) was pink and healthy; that was also the case of 78.57% patients with diffuse erythema. In the three remaining patients with type 2 stomatitis lesions healed after using Citrosept Dental and only had single pinpoints of hyperaemia. There was no change in the oral mucosa of the patient with type 3 stomatitis. Conclusions. Citrosept Dental in in vivo studies reduced denture-associated mucosal inflammations in patients with denture stomatitis accompanied by fungal infections, and also produced resolution of subjective symptoms in a significant majority of patients.

Comparison of the Effects of Miconazole and Propolis in the Treatment of Candida-Associated Denture Stomatitis

PURPOSE: This study compared between the efficiency of propolis and miconazole gel as treatment modalities for Candida-associated denture stomatitis. MATERIALS AND METHODS: 20 patients suffering from Candida-associated denture stomatitis were divided into two treatment groups: Group I (control group): 10 patients received 20mg/g miconazole oral gel and Group II (study group): 10 patients received Bio-Propolis Capsules 2% (20mg/g). The patients were asked to use the miconazole oral gel or the propolis powder twice a day for 14 days. Examination was done on 1st, 7th and 14th days. Newtons’s criteria were used to classify the Pre-treatment and post-treatment (CHRO Magar Candida®) and counted through the colony forming unity count (CFU/mL). The intragroup comparison before and after each treatment was done using the Wilcoxon’s test while the Kruskall-Wallis’s test was used to compare the results of the two treatment groups. RESULTS: there was a considerable decrease or total cure of denture stomatitis (P<0.05) and a remarkable reduction of the number of Candida colonies in the two groups. Intergroup comparison revealed that no difference existed between both groups. Newton’s score were reduced in the two groups signifying the efficiency of both treatments. CONCLUSION: propolis powder has an antifungal activity similar to miconazole, in the treatment of Candida-associated denture stomatitis. Propolis is a safe, inexpensive, natural substance without any known side effects until now. It may therefore be safely to treat Candida-associated denture stomatitis.

Fabrication and Evaluation of Nanostructured Herbal Oil/Hydroxypropyl-β-Cyclodextrin/Polyvinylpyrrolidone Mats for Denture Stomatitis Prevention and Treatment.

This work aims to develop the herbal oil-incorporated nanostructure mats with antifungal activity for the prevention and treatment of Candida-associated denture stomatitis. The nanofiber mats loaded with betel oil or clove oil were fabricated via electrospinning process. The morphologies and physicochemical properties of the herbal oil loaded nanofiber mats were examined using scanning electron microscopy (SEM), Fourier transform infrared spectroscopy (FT-IR), differential scanning calorimetry (DSC), and mechanical testing. The release characteristic, antifungal activity, and cytotoxicity were also investigated. The SEM images confirmed the homogeneous and smooth nanoscale fibers. The addition of the herbal oil into the nanofiber mats reduced the fiber diameters. The DSC and FT-IR results confirmed the presence of the oil in the nanofiber mats. The herbal oils can be released from the mats in a very fast manner and inhibit the growth of candida cells within only few minutes after contact. These nanofiber mats may be beneficial for the prevention and treatment of denture stomatitis.

Inhibitory Effect of Alpha-Mangostin on Adhesion of Candida albicans to Denture Acrylic.

Objective:Candida-associated denture stomatitis is a very common disease affecting denture wearers. It is characterized by the presence of yeast biofilm on the denture, primarily associated with C. albicans. The investigation of agents that can reduce C. albicans adhesion may represent a significant advancement in the prevention and treatment of this disease. This study aims to investigate the effect of alpha-mangostin on the in vitro adhesion of C. albicans to denture acrylic and germ tube formation by C. albicans and to compare its activity with clotrimazole which is a topical antifungal agent commonly used for the treatment of Candida-associated denture stomatitis. Materials and Methodology:Alpha-mangostin was extracted by thin layer chromatography. The effect of alpha-mangostin on adhesion of C. albicans to denture acrylic was determined by using a colorimetric tetrazolium assay and germ tube formation by C. albicans was determined by using the counting chamber. Results:A significant reduction of C. albicans adhesion to denture acrylic was evident after exposure to 2,000 µg/ml of alpha-mangostin for only 15 min. In addition, the 2,000 µg/ml of the alpha-mangostin-treated C. albicans had a reduced ability for germ tube formation. These inhibitory effects of alpha-mangostin were as effective as clotrimazole.Conclusion: Alpha-mangostin has antifungal property against C. albicans by inhibiting the adhesion to denture acrylic and germ tube formation in vitro. These results suggest the potential application of alpha-mangostin as a topical medication or a natural oral hygiene product for treatment of Candida-associated denture stomatitis.

Silver colloidal nanoparticle stability: influence on Candida biofilms formed on denture acrylic.

Our aim in this study was to evaluate how the chemical stability of silver nanoparticles (SNs) influences their efficacy against Candida albicans and C. glabrata biofilms. Several parameters of SN stability were tested, namely, temperature (50ºC, 70ºC, and 100ºC), pH (5.0 and 9.0), and time of contact (5 h and 24 h) with biofilms. The control was defined as SNs without temperature treatment, pH 7, and 24 h of contact. These colloidal suspensions at 54 mg/L were used to treat mature Candida biofilms (48 h) formed on acrylic. Their efficacy was determined by total biomass and colony-forming unit quantification. Data were analyzed using analysis of variance and the Bonferroni post hoc test (α = 0.05). The temperature and pH variations of SNs did not affect their efficacy against the viable cells of Candida biofilms (P > 0.05). Moreover, the treatment periods were not decisive in terms of the susceptibility of Candida biofilms to SNs. These findings provide an important advantage of SNs that may be useful in the treatment of Candida-associated denture stomatitis.

Brazilian Green Propolis Compared to Miconazole Gel in the Treatment ofCandida-Associated Denture Stomatitis

Aim. To evaluate the efficacy of Brazilian green propolis in comparison to miconazole gel in the treatment of Candida-associated denture stomatitis. Methods. Forty-five denture stomatitis patients, with palatal mucosa erythema levels classified according to Newtons's criteria and with positive culture to Candida spp., were randomly divided into three treatment groups: 15 received miconazole gel 2%, 15 received propolis gel 2,5%, and 15 received propolis 24% for mouthwash. After four daily use lasting two weeks, they were reexamined for the denture stomatitis degree and for a second culture of Candida. The Wilcoxon's test was applied to compare the results of clinical classification of the denture stomatitis and the Candida spp. colonies numbers, before and after each treatment. The Kruskall-Wallis's test was used to compare efficacy among the three treatment groups. Results. There were a significant reduction or complete remission of denture stomatitis (P < 0.05) and a significant decrease of Candida colonies for the three groups (P < 0.05). There was no difference in the efficacy among the treatment groups (P > 0.05). Conclusion. Brazilian green propolis has a similar effect as miconazole in the treatment of Candida-associated denture stomatitis being an alternative in the therapeutics of this condition.

&nbsp; The aim of this study was to determine the the prevalence of&nbsp;in vitro&nbsp;resistance amongstCandida&nbsp;species isolated from the oral cavity of denture wearers. The&nbsp;in vitrosusceptibility of 156&nbsp;Candida&nbsp;isolates to amphotericin B, fluconazole, 5- fluorocytosine, caspofungin and terbinafine was determined. The Clinical Laboratory Standards Institute&rsquo; (CLSI; formally National Committee for Clinical Laboratory Standards) broth microdilution method was used and MIC50&nbsp;and MIC90&nbsp;determined.&nbsp;Candida&nbsp;albicans,&nbsp;the most frequently isolated strains, are sensitive to amphotericin (61%) and fluconazole (44%), frequently used agents in the treatment of&nbsp;Candida-associated denture stomatitis.&nbsp;A 100% susceptibility to&nbsp;5-fluorocytosine was observed among the 109 isolates of&nbsp;C. albicans. Among non&nbsp;C. albicans&nbsp;strains only 1&nbsp;Candida kefyr&nbsp;strain was determined as susceptible dependent upon dose for 5-fluorocytosine. Among&nbsp;Candida glabrata, the second most common isolate, a 100% susceptibility to caspofungin and 5-fluorocytosine were observed.&nbsp;Since the isolates are sensitive to Caspofungin and&nbsp;5-fluorocytosine, rarely used in the treatment of oral fungal infections, it is suggested that these antifungal agents be used as alternative medicine in the treatment of oral infections especially caused by strains resistant to amphotericin B and fluconazole. &nbsp; Key words:&nbsp;Candida&nbsp;species, antifungal, denture wearers.

Miconazole gel compared with Zataria multiflora Boiss. gel in the treatment of denture stomatitis

An open, randomized, controlled study with two parallel treatment groups was done to evaluate the efficacy of a miconazole 2% gel compared with Zataria multiflora 0.1% gel applied four times daily for 2 weeks, in the treatment of Candida-associated denture stomatitis. Twenty four patients were included in the study. The efficacy variables were the colony count of Candida from the palatal mucosa and the denture surface and the erythema surface of the palatal mucosa on days 0, 7, 14, 21 and 28 after commencement of therapy. Twelve patients received miconazole gel and twelve Z. multiflora gel. The erythema surface was significantly reduced by both gels. No significant difference was seen between the two groups (p < 0.05). There was a significant reduction in the colony count of the palatal mucosa in both groups (except on days 21 and 28 in the Zataria group p = 0.07 and 0.08). Miconazole treatment reduced the number of denture colonies more efficiently than Z. multiflora except for day 21 when the efficacy of both groups was similar (p = 0.17). The results indicate that Z. multiflora gel reduced the surface erythema of the palate more efficiently than miconazole gel but did not reduce the colony count of the denture surface as efficiently as miconazole.

Feasibility of using collagen as the base of the antifungal drug, miconazole

The feasibility of using collagen as the base of miconazole was investigated. The addition of 33% collagen to a miconazole solution did not affect the minimal inhibitory concentration (MIC80) of the miconazole solution for Candida albicans. When 1 microg mL(-1) of miconazole in 33% collagen solution was plated on resin discs and dried to yield a thin membrane, the growth of C. albicans on the resin discs was nearly completely inhibited. In addition, we compared the antifungal effect of this collagen solution that contained 1 microg mL(-1) miconazole, with the antifungal effect of miconazole gel that had been diluted with glycerol (the main component of miconazole gel) to yield a final concentration of 1 microg mL(-1) of miconazole; as a result, we found that the collagen solution containing 1 microg mL(-1) miconazole had a stronger antifungal effect. In conclusion, our results demonstrated that it may be feasible to use collagen as the base of miconazole instead of glycerol, and suggest that a collagen-based miconazole solution would have a stronger antifungal effect than commercially available miconazole gel. Collagen-based miconazole solution may be useful for the treatment of Candida-associated denture stomatitis.
