Evaluating the Long-Term Outcomes of Periodontal Surgery vs. Non-Surgical Treatment in Aggressive Periodontitis.

Aggressive periodontitis is a severe form of periodontal disease characterized by rapid tissue destruction and tooth loss. The optimal treatment approach for managing this condition remains a topic of debate. A retrospective cohort study was conducted, involving patients diagnosed with aggressive periodontitis who received either surgical or non-surgical treatment between 2010 and 2020. Clinical and radiographic data were collected at baseline and regular intervals over a 5-year follow-up period. Surgical interventions included flap surgery, guided tissue regeneration, and bone grafting, while non-surgical treatments comprised scaling and root planning with or without adjunctive antibiotics. The primary outcomes assessed included changes in probing depth, clinical attachment level, tooth loss, and patient-reported quality of life measures. A total of 120 patients were included in the study, with 60 patients in each treatment group. The surgical group demonstrated significantly greater reductions in probing depth and gains in clinical attachment level compared to the non-surgical group (P < 0.05). Tooth loss was significantly lower in the surgical group over the 5 years (P < 0.01). Patient-reported outcomes also favored the surgical group, with improved oral health-related quality of life. However, the surgical group had a higher incidence of postoperative complications. This study suggests that periodontal surgery yields superior long-term outcomes in the management of aggressive periodontitis compared to non-surgical treatment.

Introduction: The main of non-surgical periodontal treatment is mechanical debridement. However, the treatment success rate is greatly influenced by ease of mechanical instrument access. Therefore, adjunctive antimicrobial therapy is indicated for complete removal of periodontal pathogen. Since long term systemic antimicrobial treatment can cause resistency, local antimicrobial treatment such as photodynamic therapy may serve as a better choice. Literature review: Photodynamic therapy (PDT) consists of 3 elements: photosensitizer, oxygen, and visible light. After photosensitizer binds with target cells, they are activated by visible light with suitable wavelength, thus producing singlet oxygen and Reactive Oxygen Species (ROS) to create toxic environment for anaerobic periodontal pathogen. PDT increases clinical attachment gain in both chronic and aggressive periodontitis treatment. It also decreases bleeding on probing and pocket depth. Discussion: The advantages of PDT are non invasive, short treatment time required, and reduced possibility of antimicrobial resistency. The disadvantages of PDT are tissue thermogenesis effect and temporary pigmentation of gingiva by photosensitizer. Several precautions should be made while using PDT such as adequate eye protection, irrigation, and isolation. Conclusion: Combination of mecahnical debridement and PDT is proven effective to increase clinical parameters and decrease bacterial burden in peridontitis treatment.

Antimicrobial photodynamic therapy in treatment of aggressive periodontitis (stage III, grade C periodontitis): A comparison between photodynamic therapy and antibiotic therapy as an adjunct to non-surgical periodontal treatment.

Treatment of aggressive periodontitis (stage III, grade C periodontitis) represents a challenge. The aim of the study was to compare the long-term results of antimicrobial photodynamic therapy (aPDT) and antibiotic therapy as an adjunct to conventional non-surgical therapy in patients with aggressive periodontitis. Twenty subjects with untreated aggressive periodontitis (stage III, grade C periodontitis) were divided into two groups: the test group (TG) received non-surgical therapy and two sessions of aPDT using a laser (HELBO TheraLite laser) with a wavelength of 670nm associated with HELBO Blue photosensitizer, and the control group (CG) received non-surgical therapy and antibiotics (amoxicillin 500mg and metronidazole 400mg, 7 days). Clinical parameters of probing depth, clinical attachment level and bleeding on probing (BOP) were assessed at baseline, 3, 6, 9 and 12 months after treatment. The mean probing pocket depths at baseline were 3.68mm in TG and 3.51mm in CG. These values decreased to 2.77mm (p < 0.05) and 2.54mm (p < 0.05) 3 months after treatment and stayed decreased after 12 months. Clinical attachment levels at baseline were 3.88mm in TG and 3.70mm in CG. These values decreased to 3.06mm (p < 0.05) and 2.80mm (p < 0.05) after 3 months and stayed decreased after 12 months. We also found a decrease in BOP after 3, 6, 9 and 12 months in TG and in CG. aPDT and antibiotics as an adjunct to non-surgical periodontal treatment lead to a comparable improvement in long term periodontal parameters.

Nanohydroxyapatite-Chitosan Hydrogel Scaffold with Platelet Rich Fibrin and Buccal Fat Pad derived Stem Cell for Aggressive Periodontitis Treatment: A Narrative Review

Periodontal disease is oral health problem which attacks 20-50% of the global population. In Indonesia, 60% population suffers periodontal disease and become the second most common disease in the oral cavity. Aggressive periodontitis (AP) is one of periodontal disease, mainly attacks individual under 30 years-old, with rapid alveolar bone destruction. Bone graft implantation surgery is the current treatment for AP, but it is invasive and high risk infection. Nanohydroxyapatite-chitosan (nHPA-CS) hydrogel injectable scaffold is non-invasive treatment that contains bone mineral hydroxyapatite that derived from bovine bone and chitosan (CS) biopolymer with low immunogenicity. Platelet-Rich-Fibrin (PRF) contains huge amount growth factor which is biodegradable to human body. Buccal-Fat-Pad Stem-Cell (BFPSC) was chosen because it is easy to harvest and able to differentiate into mesenchymal lineage tissue. The aim of this narrative review is to describe the potential combination of nanohydroxyapatite-chitosan hydrogel scaffold, platelet-rich-fibrin, and buccal-fat-pad stem-cells for regenerating alveolar bone in aggressive-periodontitis patients. Data was obtained by literature study scientific research articles. In patients with AP, BFPSC can differentiate into osteoblast to improve osteogenesis, nHPA-CS scaffold creates host like tissue microenvironment to stimulate osteoinduction and osteoconduction activity of BFPSC, while PRF stimulates osteogenic differentiation and proliferation of BFPSC with insulin growth-factor, platelets derived growth-factor, transforming growth-factor, fibroblast growth-factor, vascular endothelial growth-factor. this combination increasing runt-related transcription-factor2, osterix, bone alkaline phosphatase, and controls pro-inflammatory cytokines such as interleukin-1beta (IL-1b) and tumor necrosis factor-alpha (TNF-a) so it can enhance bone regeneration and avoid bone destruction. The combination of nHPA-CS, PRF, BFPSC has the potential for alveolar bone regeneration in Aggressive-Periodontitis patients.

Treatment of aggressive periodontitis with photodynamic therapy and ozonated water irrigation as an adjunct to scaling and root planing: A case report

Aggressive periodontitis (AgP) is a type of periodontitis which causes rapid destruction of periodontal ligament and alveolar bone and occurs in otherwise systemically healthy individuals generally of a younger age group, but patients may be older. Ozonated water strongly inhibits the formation of oral biofilm and reduces the number of subgingival microbes. The photodynamic therapy (PDT) leads to formation of highly reactive nacent oxygen which causes bacterial cell death. This case report compares the treatment outcome of AgP with antimicrobial photodynamic therapy and ozonized water. The right side of a AgP diagnosed patient was treated by scaling and root planing (SRP) followed by subgingival irrigation with ozone water, while the left side was treated with SRP followed by PDT. After four sessions of irrigation, higher pocket reduction, more clinical attachment gain, and lower bacterial load were found in the segment treated with PDT.

Adjunctive Application of Systemic Antibiotics in Non-surgical Aggressive Periodontitis Treatment: Clinical and Microbiological Findings

BACKGROUND: Aggressive periodontitis (AgP) is associated with an extensive and rapid destruction of periodontal tissues. Unpredictable treatment outcomes of the disease are consequences of various cross-linked factors. Antimicrobial adjunctive treatment is routinely used as a part of non-surgical periodontal treatment (NSPT) of AgP. AIM: Therefore, the aim of this study was to compare the effects of active periodontal treatment (APT) combined with systemic antibiotics with mechanical debridement alone, in patients with generalized AgP (GAgP). METHODS: Two groups, consisting of 10 participants each, were randomly formed. Both groups were clinically and microbiologically tested and monitored for 3 months. Test group received APT with antimicrobial supplementation (amoxicillin and metronidazole [AMX-MET]). Patients from the control group were treated by APT only. Clinical and microbiological parameters were recorded at baseline and 3 months following the NSPT. Clinical measurements involved probing depth, clinical attachment level, bleeding on probing, and plaque index. Samples of subgingival crevicular fluid were analyzed by qualitative polymerase chain reaction. RESULTS: Significant improvement of all clinical parameters was observed 3 months following the NSPT (p &lt; 0.05), but significant difference between groups was not found (p &gt; 0.05). Periodontal pathogens’ detection between baseline and 3 months follow-up was also not statistically significant in both examined groups (p &gt; 0.05). CONCLUSION: Similarity of results obtained in both treatment groups underlined the key role of APT in the treatment of AgP. The AMX-MET supplementation did not improve clinical and microbiological outcomes, when compared to APT alone. Longer follow-up period, with larger sample, could provide a more comprehensive insight into this issue.

Comparing the efficiency of Er,Cr:YSGG laser and diode laser for the treatment of generalized aggressive periodontitis

This study aims to determine the effects of Er,Cr:YSGG and diode laser treatments on IL-1β, IL-8, and TNF-α levels in patients with generalized aggressive periodontitis. Twenty-six generalized aggressive periodontitis patients were enrolled in the study. We performed three treatment models: "scaling and root planning (SRP-only)," "SRP + Er,Cr:YSGG laser," and "SRP + diode laser." Each experimental quadrant was randomly allocated to the control group or the test group. The IL-1β, IL-8, and TNF-α levels were analyzed with an enzyme-linked immune-sorbent assay. When the baseline and post-treatment IL-1β, IL-8, and TNF-α levels were compared, the most significant difference was observed in the SRP + Er,Cr:YSGG group and the least difference was observed in the SRP-only group. The use of Er,Cr:YSGG laser as an addition to the conventional mechanical periodontal treatment was found to be more successful than the diode laser + SRP use in aggressive periodontitis treatment.

A multilevel analysis of a randomized clinical trial comparing adjunctive moxifloxacin versus amoxicillin/metronidazole for the treatment of aggressive periodontitis

It was documented that the clinical outcomes of mechanical periodontal treatment can fluctuate not merely concerning patients but equally among various tooth sites in the subject. This trial evaluates the clinical parameters related with the patient, tooth, and site that generate more changes in clinical attachment level (CAL) gain and probing depth (PD) reduction, using moxifloxacin (MOX) versus amoxicillin plus metronidazole (AMOX + ME) as adjuncts to scaling and root planing (SRP), in comparison to SRP only, post-therapy in generalized aggressive periodontitis (GAgP). The analysis of this clinical trial included 6012 tooth sites at 1002 teeth in 36 patients; they were randomly assigned to three protocols: Systemically intake of MOX or AMOX + ME plus SRP, or SRP + placebo for 7 days. The clinical effect of the patient, tooth, and site characteristics, in terms of CAL gain and PD reduction, was explored using a multilevel linear model. P < 0.05 was statistically significant. Following 6 months of treatment, the differences between the groups were statistically significant, favoring the MOX and AMOX + ME protocols (P < 0.0001). Moreover, the multilevel model showed that adjunctive MOX, AMOX + ME, non-molar, and interproximal sites were the features that contribute significantly to CAL improvement, and PD decreases in GAgP (P ≤ 0.001 for all). The most relevant characteristics for the changes in CAL increase and PD diminution, after adjunctive antimicrobials, were ascribable to the features related to the tooth. MOX and AMOX + ME, non-multi-radicular-tooth, and interdental sites indicated superior clinical gains at the tooth and site levels in GAgP.

Local delivery therapies as adjuvants to non-surgical periodontal treatment of periodontitis grade C: a systematic review.

The aim of this article was to perform a systematic review on the effectiveness of local adjuvant therapies in the treatment of aggressive periodontitis (AgP), now reported as periodontitis grade C. The authors selected randomized clinical trials of AgP patients who received local therapy as adjuvants to non-surgical periodontal with a duration of at least 90 days. Seven databases were searched up to January 2020. The gain in clinical attachment level (CAL) and reduction of probing depth (PD) were the outcomes of interest. Of the 3583 studies found, only five articles were included in the qualitative analysis. Among the substances analyzed, only 1.2 mg of simvastatin gel (SMV) (1.2 mg/0.1 ml), 1% of alendronate gel (ALN) (10 mg/ml), and 25% metronidazole gel (MTZ) (Elyzol@) showed a significant decrease in the probing depth when compared with their respective control groups. The gain CAL was shown using 1.2 mg SMV gel (1.2 mg/0.1 ml) and 1% ALN gel (10 mg/ml). Although 1.2 mg SMV gel (1.2 mg/0.1 ml), 1% ALN gel (10 mg/ml), and 25% MTZ gel (Elyzol) have shown better results, local therapies adjuvant to SRP the data found were limited. Future clinical studies with appreciable methodological quality should be conducted. Despite some benefits of local delivery therapy, up to now, it has not been possible to prove the efficacy of local therapy as an adjunct to standard treatment of AgP (periodontitis grade C).

Clinical and Microbiological Evaluation of Surgical and Nonsurgical Treatment of Aggressive Periodontitis.

The present study aimed to evaluate clinical and microbiological effects of surgical and nonsurgical periodontal therapy in generalized aggressive periodontitis (GAgP) treatment. Sixteen GAgP patients were included in this randomized split-mouth design clinical trial. Maxillary quadrants were allocated into two groups: Nonsurgical Therapy (NST) and Surgical Therapy (ST). The following clinical parameters were assessed: plaque index (PI), bleeding on probing index (BoP), probing depth (PD), clinical attachment level (CAL) and gingival margin position (GMP). Concentrations of Porphyromonas gingivalis (Pg) and Aggregatibacter actinomycetemcomitans (Aa) in the subgingival biofilm were also determined. Clinical and microbiological parameters were assessed at baseline (n=16), 3 (n=15), 6 (n=15) and 12 months (n=8) after treatment. ST was able to promote higher PD reduction compared to NST in deep pockets at 12 months (p<0.05) and in posterior teeth at 6 months (p<0.05). In addition, higher gingival recession was observed in posterior teeth of the ST group at the 6th month (p<0.05). However, ST failed to promoted additional CAL gain in any timepoint (p>0.05). Moreover, microbiological evaluation showed no statistical difference in levels of Aa and Pg for both groups at all follow-up periods. Surgical therapy promoted similar clinical benefits to GAgP therapy. Moreover, both therapies failed to reduce Aa and Pg levels at different follow-up times.

Local IL-10 level as a predictive factor in generalized aggressive periodontitis treatment response.

Generalized aggressive periodontitis (GAgP) presents a reduced response to non-surgical therapy. However, it is not clear if the initial clinical, microbiological or immunological characteristics are impacting the worse response to treatment. This study aimed to identify the predictive value of clinical, microbiological and immunological patterns on the clinical response to therapy in GAgP patients. Twenty-four GAgP patients were selected, and gingival crevicular fluid (GCF) and subgingival biofilm were collected. Aggregatibacter actinomycetemcomitans, Porphyromonas gingivalis and Tannerella forsythia levels were evaluated by qPCR, and IL-1β and IL-10 concentration by ELISA. Twelve patients were treated with SRP (scaling and root planning), and twelve with SRP plus 375mg amoxicillin and 250mg metronidazole (8/8hours, 7days) (SRP+AM). The clinical changes (Probing Pocket Depth [PPD] reduction and Clinical Attachment Level [CAL] gain) 6months post-treatment were correlated to the initial clinical, inflammatory and microbiological variables using stepwise logistic regression (α=5%). CAL gain at 6months was 1.16±0.77 for SRP and 1.74±0.57mm for SRP+AM (P>.05). PPD reduction was 1.96±0.82 for SRP and 2.45±0.77mm for SRP+AM (P<.05). In the SRP group, IL-10 showed a predictive value for clinical response. The higher the IL-10 concentration at baseline, the higher the reduction in PPD at 6months (P=.01, r=.68). However, when antimicrobials were administered, no significant influence was detected (P>.05). It can be concluded that the IL-10 levels in GFC act as a predictor of clinical response to GAgP. Moreover, the intake of antimicrobials appears to overlap the influence of the inflammatory response on clinical response to treatment. Clinical trial registration number: NCT03933501.

ИССЛЕДОВАНИЕ БИОХИМИЧЕСКИХ ПОКАЗАТЕЛЕЙ КОСТНОГО МЕТАБОЛИЗМА РОТОВОЙ ЖИДКОСТИ ПРИ ЛЕЧЕНИИ АГРЕССИВНЫХ ФОРМ ПАРОДОНТИТА

Background. The analysis of the dynamics of biochemical parameters of bone metabolism of the oral fluid in the treatment of rapidly progressive periodontitis with the use of drugs Eplun, Metronidazole, Osteogenon and feel better. The most effective scheme of medical treatment was determined. We propose the use of EPLAN and Osteogenon in the complex treatment of aggressive periodontitis, and the use of Parodontax COMPREHENSIVE PROTECTION and Parodontax Classic mouthwash without alcohol for a rational hygiene of oral cavity. Objectives ― study the dynamics of biochemical parameters of bone metabolism of oral fluid in the treatment of aggressive forms of periodontitis. Methods. 100 patients aged from 26 to 35 years with a diagnosis of moderate CGP were examined. Depending on the prescribed treatment, all patients were divided into 4 groups by stratified randomization of 25 people. In the treatment of patients used medicines that are allowed to use: Eplun, Metronidazole, Osteogenon, homeopathic remedy. The study of biochemical parameters of oral liquid (calcium, phosphorus, alkaline phosphatase) in patients with present study included was carried out on the time frame: before treatment, after 3, 7 days, 1, 3, 6 and 12 months follow-up, patients in the control group – once. Results. The most pronounced changes in the biochemical parameters of the oral fluid, which led to the normalization of calcium, phosphorus and schf levels, were registered in the 4th group, in the complex therapy of which the drugs "EPLAN" and "Osteogenon" were used, and for rational oral hygiene, toothpaste Parodontax COMPLEX PROTECTION and rinse Rarodontax Classic without alcohol were used. Conclusions. The inclusion in the scheme of complex treatment of drugs "EPLAN" and "Osteogenon", and for rational oral hygiene, the use of toothpaste Parodontax COMPLEX PROTECTION and rinse Parodontax Classic alcohol-free was the most effective in the treatment of rapidly progressive periodontitis.

Knowledge, attitude, and practice of dental practitioners regarding antibiotic prescribing for aggressive periodontitis in Morocco

Background: The use of systemic antibiotics in complement of the periodontal debridement in the treatment of aggressive periodontitis is supported by evidence published in the literature. The appropriate use of antibiotics allow to avoid practices that may enhance microbial resistance. Aims: The objective of this study is to evaluate the knowledge, attitudes, and practices of a population of dental practitioners in Morocco regarding antibiotics prescribing for patients with aggressive periodontitis. Materials and Methods: This cross-sectional study was carried among practitioners from cities of Casablanca, Rabat, Sale, Fez, Kenitra, and Temara from January to April 2016. The data collected were analyzed through the Statistical Package for Social Sciences software (SPSS version 13.0). Results: A total of 750 replies were received giving a response rate of 75%. Nearly 63.7% of the participants were female and 68% had <10 years' practice experience. The present study showed that dental practitioners had good knowledge regarding antibiotic use. The median score of knowledge was 3 (maximum possible score of 4). The results of attitudes and practices toward prescribing antibiotics were poor. Four hundred and twenty-two (67.5%) choose inappropriate dose of systemic metronidazole combined with amoxicillin. Gender, age, duration of practice, graduation, and continuing education in periodontology were significantly associated with the prescription of correct dose and duration of antibiotic treatment. Conclusion: This study lends support that scientific basis for prescribing antibiotics for aggressive periodontitis was not respected by the majority of the participants in this Moroccan population of dental practitioners. The attitude towards the prescription of antibiotics should therefore be modified. Also, it adds to the evidence the needed to designing periodontal national guidelines for studied dental practitioners.

Periodontal Regeneration with Enamel Matrix Derivative in the Management of Generalized Aggressive Periodontitis: A Case Report with 11-Year Follow-up and Literature Review.

Objectives:Aggressive periodontitis (AgP) represents an uncommon but rapidly advanced inflammatory process, which involves the destruction of periodontal tissues. This study aimed to report a case of generalized AgP (GAgP), where the treatment approach consists of the utilization of the full-mouth disinfection protocol (FMDP) in conjunction with flap curettage and regenerative appliance of enamel matrix derivatives (EMDs). The associated literature was also reviewed.Materials and Methods:A 19-year-old female patient was diagnosed with GAgP. The treatment was initiated with FMDP and administration of antibiotics. Afterward, open flap debridement was performed, and EMD was selected as the regenerative material for the reconstruction of the periodontal defects. Over an 11-year period and during all the phases of the treatment, the outcomes were regularly evaluated with clinical measurements and radiographic controls.Results:The 11-year results demonstrated no recurrence of disease, and the patient's periodontal health exhibited evident improvement. Overall, the pocket depths presented satisfactory reduction while the clinical attachment loss (CAL) was improved. Both our limited experience and available literature data revealed that the use of EMD in AgP treatment contributes to bone fill of the intrabony defects as well as regeneration of the destructed periodontal apparatus.Conclusions:Although the outcomes of this treatment approach have not been widely evaluated, it seems that the use of EMD may be an effective means of periodontal regeneration in patients with GAgP. Additional prospective studies with adequate number of GAgP patients are essential to thoroughly assess the effectiveness of this approach.

Does the Autologue Fibrin Glue Enhance the Regeneration of Intrabony Defects in Aggressive Periodontitis?

Aggressive periodontitis is a rapid periodontal destruction that develops in early and affects systemically healthy individuals. As in all periodontal diseases, the main purpose of aggressive periodontitis treatment is to enable the patient to maintain his function and aesthetics with her natural teeth in optimal health conditions. The most appropriate treatment for this purpose is periodontal regeneration methods. In this case report, regenerative treatment of intrabony defects with xenograft and autologue fibrin glue was presented.

Application of minocycline in the treatment of aggressive periodontitis

Objective To study the application effect of minocycline in the treatment of aggressive periodontitis. Methods From January 2014 to January 2017, 116 cases of invasive periodontitis in the People's Hospital of Panan County were selected in the research.According to the different treatment methods, the patients were divided into observation group and control group with 58 cases in each group.The control group was given basic treatment, the observation group was given minocycline on the basis of basic treatment.The levels of tumor necrosis factor α(TNF-α), interleukin-17(IL-17), matrix metalloproteinase-1(MMP-1)and matrix metalloproteinase-8(MMP-8)in the two groups were observed.The changes of gingival index(GI), periodontal exploration depth(PD), attachment loss(AL), periodontitis gingival crevicular fluid(NO)were compared. Results After treatment, the total effective rate in the observation group was 93.1%(58/54), which was significantly higher than that in the control group[80.0%(58/46)], the difference was statistically significant(χ2=9.469, P=0.002). After treatment, the levels of TNF-α, IL-17, MMP-1, MMP-8 in the gingival crevicular fluid of the observation group were(2.98±0.77)ng/mL, (20.87±5.34)pg/mL, (1 422.45±175.45)pg/mL, (1 341.58±169.56)pg/mL, respectively, which were lower than those of the control group[(3.98±0.99)ng/mL, (31.65±7.88)pg/mL, (1 781.44±199.55)pg/mL, (1 678.99±185.44)pg/mL](t=6.072, 8.625, 10.289, 10.226, P<0.05). The levels of GI, PD, AL and NO of the observation group were(1.21±0.38)mm, (1.51±0.61)mm, (2.43±0.39)mm, (45.78±8.21)μmol/L, respectively, which were lower than those of the control group[(3.31±0.29)mm, (4.21±0.89)mm, (3.59±0.62)mm, (64.34±12.88)μmol/L](t=33.4572, 19.057, 12.061, 9.254, all P<0.05). After treatment, the levels of TNF-α, IL-17, MMP-1 and MMP-8 in the gingival crevicular fluid of the two groups were significantly lower compared with those before treatment(observation group: t=14.590, 18.885, 23.019, 3.789, control group: t=7.809, 12.993, 10.905, 7.608, all P<0.05). Conclusion The use of minocycline in the treatment of invasive periodontitis can significantly reduce the levels of TNF-α, IL-17, MMP-1, MMP-8 in gingival crevicular fluid, relieve periodontal plaque, promote periodontal tissue regeneration, the clinical efficacy is good.
