COVID is an infectious disease caused by a newly discovered Coronavirus. Most people infected with the Corona virus will have symptoms of mild to moderate respiratory illness, fever, sore throat etc. Modern Western Medicine (MWM) is still in search of treatment which is specific to COVID. In the present case both the patients after diagnosed of being COVID positive consulted for Ayurveda medications for the cure of moderate symptoms of corona and also as a preventive measure for their children. Both the patients had symptoms of high fever, severe body pain, tiredness and throat ache. The progress of the disease could be arrested within a short period by Ayurveda medicines. And Ayurveda medicines also worked in a preventive aspect for the children who were the primary contact of the parents. This illustrates that there is a wide scope to explore the treatment measures for curative and preventive aspects for pandemics in Ayurveda litreture which can be used rationally to suit every stage of the infectious diseases. Rationale of Ayurveda is different from Modern Western Medicine (MWM) is Ayurveda adopts holistic approach both for curative and preventive aspects of treatment. This article will encourage the health fraternity to seek Ayurveda both as curative and preventive medicine in present pandemic situation.

Platelet-Rich Plasma in Regenerative Medicine: Possible Applications in Management of Burns and Post-Burn Scars: A Review.

Contribution of platelets in tissue regeneration and their possible application in regenerative medicine, which is primarily mediated via secretion of granular components following platelet activation, has been well established in the recent decades. Therefore, platelet rich plasma (PRP), as a portion of plasma with higher concentrations of platelets than the baseline level, is now an attractive therapeutic option in various medical fields mainly for tissue repair and regeneration following injuries. Burn injuries are devastating trauma with high rate of morbidities affecting several aspects of the patient's life. They require a long-time medical care and high costs. However, even following the best treatment procedures, post-burn scars are inevitable consequence of burn healing process. Therefore, development of new treatment modalities for both burn healing and prevention of post-burn scar establishment seems to be necessary. Regarding the well-known role of PRP in wound healing, here we aimed to provide a comprehensive insight in the possible application of PRP as an adjuvant therapy for the management of burn injuries and subsequent scars. In terms of the following keywords (individually or in combination), original/review articles were searched in PubMed, Scopus, and Google Scholar databases from 2009 to 2021: platelet rich plasma, PRP therapy, platelet biology, platelet function, burn healing, burn scar, scar formation, burn management, wound healing, regenerative medicine. All type of articles or book chapters in English language and relevant data were included in this review. This review initially focused on PRP, its mechanisms of action, preparation methods, and available sources. Then, pathophysiology of burns and subsequent scars were discussed. Finally, their current conventional therapeutic modalities and implication of PRP in their healing process were highlighted.

Importance of Instant Thrombectomy to Treat Cerebral Vessel Occlusion During TAVI in Third World Country

Background: Transcatheter aortic valve implantation (TAVI) is considered to be one of the treatments for symptomatic severe aortic stenosis. This procedure has been associated with cerebrovascular events be it thromboembolic or hemorrhagic in nature that increase the periprocedural mortality and morbidity. Strokes post TAVI are mostly ischemic in origin; they can be either due to symptomatic or silent emboli. These thromboembolic events may occur during the procedure or up to 2 months after the procedure. In the acute setting, especially during the procedure, immediate intervention is crucial for better patient outcome. Mechanical thrombectomy is the best treatment modality for immediate revascularization and stroke prevention. Case description: We present a case of a patient who developed symptomatic signs of hypoperfusion during the procedure that required an urgent mechanical thrombectomy in a third world country. The patient didn’t undergo CT imaging and wasn’t transferred to the brain angiography suite; the procedure was done using the x-ray machine and in the cardiology catherization lab which lacks proper equipment to do cerebral DSA (Digital subtraction angiography). This case is the 8th to be reported in the literature where mechanical thrombectomy was done on the table during a TAVI procedure. Based on this case and the literature review, we will propose an algorithm for dealing with similar cases. Conclusion: We highlight the importance of instant thrombectomy that should be done in a patient suffering from a thromboembolic event while performing TAVI procedure. Using the same endovascular access, in the same setting in the cardiac catherization lab to assess thrombus location, even if done sub-optimally without DSA, in correlation with patient’s symptoms, without taking the patient to perform a CT scan, is life saving and guarantees quick reperfusion after clot aspiration with clinical improvement afterwards and better patient outcome.

Recent Advances in Adult Post-Transplant Lymphoproliferative Disorder.

PTLD is a rare but severe complication of hematopoietic or solid organ transplant recipients, with variable incidence and timing of occurrence depending on different patient-, therapy-, and transplant-related factors. The pathogenesis of PTLD is complex, with most cases of early PLTD having a strong association with Epstein-Barr virus (EBV) infection and the iatrogenic, immunosuppression-related decrease in T-cell immune surveillance. Without appropriate T-cell response, EBV-infected B cells persist and proliferate, resulting in malignant transformation. Classification is based on the histologic subtype and ranges from nondestructive hyperplasias to monoclonal aggressive lymphomas, with the most common subtype being diffuse large B-cell lymphoma-like PTLD. Management focuses on prevention of PTLD development, as well as therapy for active disease. Treatment is largely based on the histologic subtype. However, given lack of clinical trials providing evidence-based data on PLTD therapy-related outcomes, there are no specific management guidelines. In this review, we discuss the pathogenesis, histologic classification, and risk factors of PTLD. We further focus on common preventive and frontline treatment modalities, as well as describe the application of novel therapies for PLTD and elaborate on potential challenges in therapy.

CURRENT CONCEPTS AND RECENT ADVANCES IN PERI-IMPLANT DISEASES: - A NARRATIVE REVIEW.

The insertion of the dental implant in order to replace missing teeth in partially and completely edentulous patients is a successful treatment modality for over 50 years now. However, its predictability and applicability for a variety of treatment options is still a clinical dilemma. Peri- implantitis represents a serious condition after implant therapy and affects both hard and soft tissues surrounding an implant. While implant therapy represents a safe treatment option with high long-term success, reported complications are associated with improper treatment planning, surgical and prosthetic replacement, material failure and maintenance. Peri-implant lesions may develop after several years of implant placement therefore regular check-ups and life-long supportive periodontal therapy is a must. The management protocol for peri-implant diseases consists of various conservative and surgical approaches. Peri-implant mucositis and moderate peri-implant lesions can be treated with a conservative approach using non-surgical therapy and local and systemic antibiotics. Resective and regenerative therapy are advocated for more severe lesions to completely eliminate the disease. However, due to the lack of prospective randomized long-term follow-up studies, no ideal implant therapy can be implicated. The aim of this review is to provide an overview of current data and to suggest different treatment modalities for diagnosis, prevention and management of peri-implant diseases.

Comprehensive Management of Molar-Incisor-Hypomineralization by Preventive, Palliative, and Restorative Treatment Modalities in a Pediatric Patient: A Case Report and Literature Review

Molar–incisor–hypomineralization (MIH) is a developmental defect of enamel dental tissue caused by the disturbances in the maturation stage of amelogenesis. It usually affects the permanent first molars and incisors. In children, MIH condition clinically appears as the demarcated discolored hypomineralized defects of affected teeth, post-eruptive breakdown (PEB) of affected enamel, dentine exposure, hypersensitivity, increased susceptibility to dental caries and behavioral management challenges owing to dental anxiety/fear and dental esthetic concerns. This case report describes the comprehensive dental management of MIH in an 8-year-old male pediatric patient by conservative and noninvasive treatment approach with preventive, palliative, and restorative treatment modalities under nonpharmacological behavior management techniques.

Noma: A Neglected Area for Research

Noma, a debilitating and destructive orofacial gangrene, remains endemic in the poor countries of sub-Saharan Africa and other noma hotbeds across the globe, mainly in countries characterized as underdeveloped economies with significant impoverished populations. Noma mostly affects children and infants. This is in spite of the universally held notion that noma is a preventable disease. Indeed, the current noma status quo has been cast as a human rights shortfall, since this devasting disease overwhelmingly affects children from poor countries. At the recently held Noma Research Day, a renewed call for the World Health Organization (WHO) to recognize and include noma as one of the neglected tropical diseases was accompanied by a recognition that research into all aspects of noma has waned or remained completely lacking—particularly that which addresses the basic science questions of the etiology, pathophysiology/pathobiology, and underlying mechanisms of the disease. Yet, a lack of incremental knowledge on the various aspects of noma continues to hamper our composite understanding of its biology. Without a fundamental understanding of the biology of noma, current preventive measures and treatment modalities will continue to fall short of the goals of prevention and eradication. This opinion piece draws renewed attention to the urgency of listing noma as a neglected tropical disease by the WHO. It also calls for major international research funding agencies, including the WHO and the National Institutes of Health, to renew their resolve to robustly fund structured, collaborative, and coordinated proposals that address questions on the epidemiology, etiology, pathophysiology/pathobiology, and molecular mechanisms of the disease. This is with a view to achieving more effective public health approaches toward prevention and to designing potential therapeutic regimens for early lesions. These steps are key to the ultimate eradication of noma.

Noninvasive Systemic Modalities for Prevention of Head and Neck Radiation-Associated Soft Tissue Injury: A Narrative Review.

Radiation-associated soft tissue injury is a potentially devastating complication for head and neck cancer patients. The damage can range from minor sequelae such as xerostomia, which requires frequent daily maintenance, to destructive degenerative processes such as osteoradionecrosis, which can contribute to flap failure and delay or reverse oral rehabilitation. Despite the need for effective radioprotectants, the literature remains sparse, primarily focused on interventions beyond the surgeon's control, such as maintenance of good oral hygiene or modulation of radiation dose. This narrative review aggregates and explores noninvasive, systemic treatment modalities for prevention or amelioration of radiation-associated soft tissue injury. We highlighted nine modalities with the most clinical potential, which include amifostine, melatonin, palifermin, hyperbaric oxygen therapy, photobiomodulation, pentoxifylline-tocopherol-clodronate, pravastatin, transforming growth factor-β modulators, and deferoxamine, and reviewed the benefits and limitations of each modality. Unfortunately, none of these modalities are supported by strong evidence for prophylaxis against radiation-associated soft tissue injury. While we cannot endorse any of these nine modalities for immediate clinical use, they may prove fruitful areas for further investigation.

Abstract TP92: Inappropriately Prolonged Dual Antiplatelet Therapy In The Treatment Of Stroke And Transient Ischemic Attack At An Urban Safety Net Hospital

Introduction: Time-limited dual antiplatelet therapy (DAPT) with aspirin and clopidogrel has emerged as an important treatment modality for secondary stroke prevention in patients with symptomatic intracranial atherosclerotic disease, minor stroke, or high-risk transient ischemic attack. However, continuation beyond the indicated treatment period may expose patients to bleeding risk without additional benefit. Methods: We conducted a retrospective study of a prospectively maintained registry (the Get-With-The-Guidelines database at our hospital) evaluating patients discharged on time-limited DAPT for secondary stroke prevention between January 1st, 2019 and July 31st, 2020. Intended and actual duration of therapy were extracted from clinical notes and prescription records. We assessed the frequency of inappropriate continuation within the entire cohort and performed univariate analyses to assess for differences in outcome with respect to clinical and demographic characteristics, including social determinants of health (such as race and primary spoken language). Results: Of the 127 patients included, 17 (13%) had inappropriately prolonged treatment with DAPT. Black patients had a significantly higher incidence of inappropriate continuation than non-Black patients and individuals speaking languages other than English or Spanish exhibited a trend toward more frequent occurrence. Patients intended to transition to monotherapy with clopidogrel and those initially presenting with stroke (rather than transient ischemic attack) had similar trends toward more frequent occurrence. Conclusions: Our study represents the first attempt to approximate the incidence of inappropriate prolongation of time-limited DAPT for secondary stroke prevention at a hospital caring for a large underserved population. Our results suggest that social determinants of health may contribute to this phenomenon, but further studies are required to replicate our observations in larger multicenter cohorts. Systems-based interventions should be aimed at ensuring proper duration of therapy to minimize bleeding risk and other side effects.

Awareness and Perception of Oral Health Care in Infants and Children among Pediatricians in Srinagar City

Introduction: Parents frequently seek Paediatricians supervision during normal growth and development of their children. The repeated contact between patients and paediatricians renders an ideal prospect for Paediatricians to determine risks associated with oral health and enlighten parents on counselling and preventive treatment modalities during development phases. The study was thus aimed to gather information and assess knowledge and attitude of the Paediatricians from Srinagar city regarding oral health in infants and children. Materials and Methods: A cross-sectional survey was organized between January 2019 and May 2019 to assess Paediatricians awareness regarding dental health and their attitudes in relation to oral health. This study was performed among 50 randomly selected Paediatricians of Srinagar city. Data was collected and subjected to descriptive analysis. Results: Majority of the Paediatricians were familiar of the speciality of pediatric dentistry, the significance of milk teeth and considered the importance of periodic dental visits for children. It was also revealed that most of the Paediatricians showed significantly less awareness (P<0.05) of the first dental visit, early childhood caries, fissure sealants, importance of fluoride and role of medicated syrups in promoting dental caries. Conclusion: This survey reveals that Paediatricians strongly believed in having an active role in promoting oral health. Dental training should be incorporated in curriculum during medical training and Paediatricians need to regularly rejuvenate themselves on recent advancements and updates related to infant oral health which can be transmitted to their patients.

Non-surgical approach towards uterine fibroid and ovarian cyst with Yoga practice

A case of a 43 years old female with uterine fibroid and ovarian cyst was managed by an alternative, Non-surgical approach of yoga intervention. Yoga was practised under supervision daily twice a day for three months showed significant recovery from ovarian Cyst and reduction in uterine fibroid size. A diagnosis, where surgery was suggested as the only option, yoga has been found to be effective treatment modality in prevention and management.

Delayed Spontaneous Eruption of Severely Infraoccluded Primary Second Molar: Two Case Reports

Infraocclusion occurs at an early age and becomes worse with age, causing increased damage in young children. Extraction of affected teeth is the preferred treatment modality for prevention of possible complications. It is rare for a primary molar to temporarily exhibit secondary failure of eruption, followed by regeneration of full eruptive capacity. This report was written to describe two patients who experienced spontaneous eruption of an infraoccluded primary molar at approximately 7 years of age. While watchful waiting is not always a suitable treatment option, we propose that extraction be deferred until the first permanent molar erupts, unless significant problems occur.

Parents outlook on preventive dental modalities for their children in Udham Singh Nagar, India.

AIM:The aim is to evaluate the knowledge, attitude, and practices of parents toward preventive dental treatment modalities.METHODOLOGY:A convenient sample of parents was selected randomly from four schools of Udham Singh Nagar, India. The data were collected through questionnaire on dental preventive modalities. The questionnaire includes questions pertaining to knowledge and practices toward pit and fissure sealants, fluoride application, and dental visits. Questions pertaining attitude toward the same were asked after the motivational program.RESULTS:The majority of the parents were unaware that dental caries could be prevented by preventive treatment modalities. None of their children had undergone preventive procedures. Even after visiting dentist, 70% of parents were not informed about the preventive treatments by their respective dentists. Even after our motivational program, around one-third of the parents were reluctant to accept preventive procedures for their children.CONCLUSION:Parent's knowledge about the preventive dental procedures for their children was insufficient. Dentists are not encouraging parents about the benefits of preventive treatment modalities. Based on our findings, promotional efforts encouraged 64% of parents to embrace preventive procedures for their children. The present study emphasizes the need to educate parents and their children.

Effects of Treatment-Achieved HbA1c on Incidence of Micro-/Macrovascular Complications in Patients with Diabetes Mellitus

Although a so-called “J”- or “U”-shaped relationship between treatment-achieved HbA1c values and risk of coronary artery disease (CAD) has often been reported, it is not yet clarified whether the association is maintained across various treatment modalities including diet only (D) for macro/microvascular complications of diabetes. Thus, we investigated effects of treatment-achieved HbA1c on CAD events and treatment-required diabetic eye disease (TRDED) in 4 treatment subgroups: D, insulin (I), sulfonylurea (S) and antihyperglycemic agents other than glinides, S or I. We utilized a nationwide claims database of 14,633 patients with diabetes in Japan (mean age 50.3 y, HbA1c 7.2%). Cox regression examined CAD and TRDED risk over a 5.1 y follow-up. A significant linear trend in the association between HbA1c and CAD events was only seen in the D group. Significantly higher risks for CAD were observed in the I and S groups whose HbA1c was ≤7.0% or S compared to D group patients with HbA1c ≤7.0%. Conversely, risk for TRDED was strongly dependent on achieved HbA1c regardless of treatment modalities. However, risks of TRDED did not differ significantly between categories of ≤7.0% and 7.1-8.0% among S and I groups. These results implied the necessity of setting different target HbA1c goals according to treatment modalities for prevention of micro-/macrovascular complications. Disclosure M. Harada: None. K. Urinary tract infections (UTIs) affect approximately 150 million people worldwide per year, causing annual health costs of over three billion dollars just in the USA. Every second woman experiences at least one UTI in her lifetime, with every one in four experiencing recurrence. Uncomplicated infections like single or recurrent cystitis and pyelonephritis can be distinguished from complicated disease. UTIs in men can spread to the male glands, causing prostatitis and epididymitis. Antibiotic therapy is the standard procedure for UTIs. However, the extensive and sometimes irrational use of antibiotics for the treatment of infections has led to an increase in the incidence of multiresistant pathogens in recent years. Therefore, preventive nonantibiotic approaches are of great interest. This article provides an overview of the current management of urological infections as well as an outline of nonantibiotic preventive treatment modalities.

Inhibition of lipopolysaccharide-induced osteoclast formation and bone resorption in vitro and in vivo by cysteine proteinase inhibitors.

Inflammation-induced bone destruction is a major treatment target in many inflammatory skeletal diseases. The aim of this study was to investigate if the cysteine proteinase inhibitors cystatin C, fungal cysteine proteinase inhibitor (E-64), and N-benzyloxycarbonyl-arginyl-leucyl-valyl-glycyl-diazomethane acetate (Z-RLVG-CHN2) can inhibit LPS-induced osteoclast formation. Mouse bone marrow macrophages (BMMs) were isolated and primed with receptor activator of NF-κB ligand (RANKL) for 24 h, followed by stimulation with LPS, with and without inhibitors. Adult mice were injected locally with LPS and then treated with E-64 and osteoclast formation assessed by the number of cathepsin K+ multinucleated cells. Cystatin C inhibited LPS-induced osteoclast formation time and concentration dependently (IC50 = 0.3 μM). The effect was associated with decreased mRNA and protein expression of tartrate-resistant acid phosphatase (TRAP) and cathepsin K and of the osteoclastogenic transcription factors c-Fos and NFATc1. LPS-induced osteoclast formation on bone slices was also inhibited by cystatin C, resulting in decreased pit formation and release of bone matrix proteins. Similar data were obtained with E-64 and Z-RLVG-CHN2 Cystatin C was internalized in BMMs stimulated by LPS but not in unstimulated BMMs. Osteoclast formation induced by LPS was dependent on TNF-α, and the 3 inhibitors abolished LPS-induced TNF superfamily 2 (gene encoding TNF-α; Tnfsf2) mRNA expression without affecting Il1b, Il6, or oncostatin M (Osm) expression. Formation of osteoclasts in the skull bones after local LPS stimulation was inhibited by E-64. It is concluded that cysteine proteinase inhibitors effectively inhibit LPS-induced osteoclast formation in vivo and in vitro by inhibition of TNF-α expression. The targeting of cysteine proteinases might represent a novel treatment modality for prevention of inflammatory bone loss.

Perception of dental professionals of Gujarat on their role in deaddiction services

Background: Oral health professionals can play a vital role in battling tobacco epidemic. Hence, the current study is carried out with an objective to assess the perception and attitudes of dental professionals in Gujarat toward deaddiction services as an integral part of oral health. Methods: A pretested semi-structured questionnaire prepared with Google form was mailed to 400 dental surgeons of Gujarat state for online survey over a period of 7 months. The form was filled up by them after taking their consent. Results: Majority (96.7%) of the responding dental surgeons thought they have a leading role as a life saver for addicted patients before psychiatrist, and many (89%) of them were interested in counseling patients to quit tobacco but relatively few (59.8%) of them were actively involved in tobacco cessation counseling activities, and only 38.2% dentists refer these addictives to psychiatrists. Conclusion: Although majority of dental professionals perceived and showed a positive attitude in the role of tobacco cessation/deaddiction services but very few of them translated their attitude into practice. Being professionally respected and popularly revered, there is an urgent need that dental personnel should spearhead a national anti-tobacco crusade by delivering tobacco cessation deaddictive services in their regular preventive and therapeutic treatment modalities.
