Importance of Early Intervention of Traumatic Cataract in Children

Aim: To evaluate the intraoperative complications & post- operative outcome of early intervention of traumatic cataract in children due to open globe injuries.  Materials and Methods: 40 cases of unilateral cataract having repair of open globe injuries previously were included in this prospective study. Primary repair of corneal injuries were done with 10-0 Nylon under general anesthesia. Secondary intervention with IOL implantation were done within 4 weeks of primary repair. Age range was 4-14 years and minimum follow up 3 months. Intraoperative and postoperative complications and final best-corrected visual outcome were recorded.  Results: All cases had pre-existing anterior capsular tear and flocculent lens matter in anterior chamber. IOL were implanted in the bag in 90% cases. Intra-operatively 4 cases revealed pre-existing posterior capsular hole and IOL were implanted in the sulcus. Synechiolysis was done in 3 cases. There was no iatrogenic PC rent. Postoperatively no cases showed synechiae formation, pupil capture and IOL displacement. Visual acuity was 6/9 to 6/60 in all cases and intraocular pressures were digitally normal. No optic atrophy and RD were noticed at last follow up. Most common causes of decreased vision were corneal scar and astigmatism.  Conclusion: Early cataract surgery with IOL implantation after repair of open globe injuries carries favorable intraoperative and postoperative outcome.

Clinical and Epidemiological Aspects of Traumatic Cataracts in Children at CHU Campus of Lome

Introduction: Traumatic cataracts are the leading cause of unilateral cataracts, with a risk of amblyopia in young children. The objective of this study was to describe the epidemiological and clinical aspects of traumatic cataracts in children at CHU Campus of Lome. Methods: This was a retrospective descriptive study conducted using records of children with cataract due to blunt or penetrating trauma who were admitted to the ophthalmology department at CHU Campus from January 2015 to December 2020 (6 years). Results: Eighty over the 7,800 children examined had traumatic cataracts. The mean age was 7.70 ± 3.64 years [1.1 years; 15 years]. Patients aged 5 to 10 years were the most represented with 47.50% of cases. The predominance was male, with a sex ratio of 1.5. Play was the main cause of ocular trauma in 32.50% of cases. The most common vulnerating agent was wood in 29.48% of cases, followed by a punch in 26.93%. Trauma was blunt in 57.50% of cases and penetrating in 42.50%. Anatomically, total cataract was the most common type in 70.59% of children, with a low vision on admission ≤ 1/10 in 51.25% of cases. Conclusion: Traumatic cataracts in children are uncommon but serious. Play was the primary circumstance of onset, as well as wood for the vulnerating agent. Cataracts were mostly unilateral and total, with a risk of amblyopia in young children.

Eye ball deviation patterns in traumatic cataract among children in Uttar Pradesh of India

Paediatric cataract is an important cause of treatable childhood blindness, provided it is recognized and managed early. Timely diagnosis, management, and visual rehabilitation of a child with a paediatric cataract are crucial determinants of a favourable prognosis. This particular topic, study of pattern of deviation of eye ball after traumatic cataract in children was selected because of the difference between the firm literature of Keith Lyle and the clinical observations. This study includes analysis of 25 patients. The key findings of this study were: Maximum incidence of traumatic cataract was seen in 6-10 years of age group, that too more commonly in boys and it was 2.5 times more than that of girls. lncidence of perforating injuries leading to cataract was 1.5 times more common than blunt trauma. In paediatrics cataract, the Diminution of vision in cases of traumatic cataract is usually at the level of mature or hypermature cataract. 40% of cases showed deviation in the range of 11-20° (irrespective to divergence or convergence). There was higher incidence of Exotropia than esotropia. Originally eye was divergent as per evolution of binocular vision in primates & our study also came out with a conclusion that when there is no force or compulsion of binocular Vision, most of the eyes tended to be Divergent in children after traumatic cataract.

Visual Outcomes and Complications After Lensectomy for Traumatic Cataract in Children

The Pediatric Eye Disease Investigator Group Cataract Registry provides a multicenter assessment of visual outcomes and complications after lensectomy for traumatic pediatric cataract. To report visual acuity (VA) and the cumulative proportion with strabismus, glaucoma, and other ocular complications by 15 months after lensectomy for traumatic cataract among children younger than 13 years at the time of surgery. From June 18, 2012, to July 8, 2015, 1266 eyes of 994 children from 33 pediatric eye care practices seen within 45 days after lensectomy were enrolled in a multicenter, prospective observational registry. Of these, 74 eyes of 72 participants undergoing lensectomy for traumatic cataract were included in a cohort study. Follow-up was completed by November 2, 2015, and data were analyzed from March 20, 2018, to July 7, 2020. Lensectomy after ocular trauma. Best-corrected VA from 9 to 15 months after lensectomy for traumatic cataract (for those 3 years or older) and the cumulative proportion with strabismus, glaucoma, and other ocular complications by 15 months. Of 994 participants in the registry, 84 (8%) had traumatic cataract. The median age at lensectomy for 72 participants examined within 15 months after surgery was 7.3 (range, 0.1-12.6) years; 46 (64%) were boys. An intraocular lens was placed in 57 of 74 eyes (77%). In children 3 years or older at outcome, the median best-corrected VA was 20/250 (range, 20/20 to worse than 20/800) in 6 eyes with aphakia and 20/63 (range, 20/20 to 20/200) in 26 eyes with pseudophakia. Postoperative visual axis opacification was reported in 18 of 27 eyes with pseudophakia without primary posterior capsulotomy (15-month cumulative proportion, 77%; 95% CI, 58%-92%). The cumulative proportion with strabismus was 43% (95% CI, 31%-58%) in 64 participants with ocular alignment data; exotropia was present in 14 of 23 participants (61%). The cumulative proportion with glaucoma was 6% (95% CI, 2%-16%). Trauma was not a common cause of pediatric cataract requiring surgery. For children with traumatic cataract, substantial ocular morbidity including permanent vision loss was found, and long-term eye and vision monitoring are needed for glaucoma, strabismus, and capsular opacification.

Outcome of surgery for traumatic cataract in children in a child eye health tertiary facility, Ibadan, Nigeria.

Objective:The objective of this study was to describe the outcome of surgery for traumatic cataract and associated factors in children aged 16 years and below operated in a tertiary facility.Methods:This was a retrospective review of records of children who had surgery for traumatic cataract between August 2015 and August 2019. Information on biodata, preoperative visual acuity, surgical methods, complications, and postoperative visual acuity were retrieved. Data were analyzed using IBM SPSS Statistics 20.0.Results:Traumatic cataract accounted for 87 (14.7%) of 593 eyes operated for childhood cataract during the period. Of these, a total of 79 records were available for review. There were 56 (70.9%) males with a mean age of 10.11 (±3.39) years. All injuries were unilateral; closed globe injuries accounted for 70 (88.8%) of the cataracts and the left eye was affected in 42 (53.2%) patients. Fifty-two (67.09%) eyes had other ocular injuries apart from cataracts. The morphology of the cataract was membranous in 44 (55.7%) eyes, and 76 (96.2%) eyes had cataract surgery with intraocular lens (IOL) implantation; 39 (51.3%) of these IOLs were implanted within the capsular bag. The preoperative best corrected visual acuity was worse than 6/18 in all 79 (100%) eyes and improved to 6/18 or better in 32 (40.5%) eyes at 3 months postoperatively.Conclusion:Traumatic cataract accounted for less than a quarter of all childhood cataracts in our center. Majority of the eyes had successful IOLs implantation during surgery, and the best corrected visual acuity improved in a moderate proportion of these patients.

Frequency of pediatric traumatic cataract and simultaneous retinal detachment

Traumatic cataract in children is a treatable cause of vision loss. In cases of simultaneous retinal detachment, the prognosis for visual recovery is often poor. The purpose of this study was to investigate risk factors for concurrent retinal detachment in patients with traumatic cataract. A retrospective review of patients diagnosed with traumatic cataract at Children's Hospital Colorado between 2005 and 2014 was conducted. Demographics, mechanism of injury, and incidence of retinal detachment were recorded. Logistic modeling with generalized estimating equations to account for correlation of eyes within patients was used to analyze associations between potential risk factors and retinal detachment. A total of 62 total eyes with traumatic cataract were included: 52 patients presented with unilateral cataract; 5 presented with bilateral cataracts. Mean patient age was 8.4±4.1years (range, 0-16years), and 83% of patients were male. A total of 9 eyes (14.5%) had comorbid retinal detachment. Traumatic cataracts caused by self-injurious hitting were more likely to present with simultaneous retinal detachment than those caused by other mechanisms of injury (OR=24.0; 95% CI, 3.8-153.3; P=0.0010). Patients with traumatic cataract who display self-injurious behavior are at higher risk for concurrent retinal detachment. These patients can often only be examined under sedation. Ophthalmologists should counsel families of high-risk patients and consider involving retinal specialists in surgical planning.

Traumatic Cataract in Children in Eastern China: Shanghai Pediatric Cataract Study

Traumatic cataract is a main cause of visual impairment in pediatric populations and is preventable. Awareness of the causes and consequences of pediatric eye trauma play roles in health education and prevention of blindness. We conducted a retrospective chart review based on 5-year clinical data of pediatric traumatic cataract cases treated at the Eye and Ear, Nose and Throat Hospital of Fudan University, Shanghai, China. Clinical features including demographic data, injury mechanism, and causative agents were analyzed. A total of 321 eyes of 321 children (male, 74.1%) were included. Penetrating injury accounted for 76.6% of all injuries; 65% of children with such injuries underwent their first surgery on the day of injury. The average age at injury was 6.3 ± 3.7 years, while the peak age was from two to eight years. The main causes of ocular injury were sharp metal objects, toys, and wooden sticks. The causative agent changed with increasing age; however, scissors were the leading cause within almost all age groups. Cataract surgery significantly improved visual acuity in children with traumatic cataract. These findings add information regarding the characterization of pediatric traumatic cataract in China and can help guide safety education and preventative measures.

Comparison of outcomes of primary versus secondary intraocular lens implantation for traumatic cataract in children

Objective To assess and compare the outcomes of primary versus secondary intraocular lens (IOL) implantation for traumatic cataract in children.Methods 134 cases (134 eyes) of pediatric traumatic cataract in our hospital were randomly divided into two groups:group A (64 eyes,treated by primary IOL implantation) and group B (70 eyes,treated by secondary IOL implantation).Follow up was from 6 to 24 months.Results The recovery rate from blindness and disability were 90.6％ and 62.5％ in group A and 91.5％ and 80.0％ in group B at 6 months after surgery.There was no significant difference in recovery rate from blindness between two groups (x2 =0.026,P ＞ 0.5).The recovery rates from disability weresignificantly different between two groups (χ2 =5.03,P ＜ 0.05).The postoperative complications were severer in group A than group B in early and late stage.The incidences of pupil capture of IOL,posterior capsular opacity and secondary glaucoma were higher in group A than group B.The differences were statistically significant.Conclusion Traumatic cataract in children treated by secondary IOL implantation can have better visual outcomes and less surgical complications compared with primary IOL implantation. Effect of penetrating and blunt ocular trauma on the outcome of traumatic cataract in children in northern India

We studied the demographic profile and outcome of traumatic cataract after penetrating and blunt ocular trauma in children in northern India. We reviewed the medical records of children with penetrating and blunt ocular trauma associated with traumatic cataract who underwent cataract surgery with posterior chamber intraocular lens implantation and having more than 1 year follow up. We evaluated the demographic characteristics and visual outcome in the affected eye. Of the 100 children (100 eyes), 65 were penetrating trauma and 35 were blunt trauma. The common modes of injury in the penetrating group were wooden splinters in 30.8% and bow and arrow in 15.4%. Fire crackers and trauma during play with ball were seen in 34% and 11.4% of blunt eye trauma, respectively. The age ranged from 1 to 14 years (mean, 7.8 years) in the penetrating trauma and 2 to 15 years (mean, 8.3 years) in the blunt trauma group. The best corrected visual acuity of 20/40 or better was achieved in 57.6% eyes in penetrating group compared with 71.4% in the blunt trauma group. Visually significant posterior capsular opacification developed in 32.3% eyes in penetrating group and 28.6% eyes in blunt trauma group. Glaucoma was diagnosed in 4.6% eyes in penetrating group and 8.6% in blunt trauma group. Eye injuries with traumatic cataract are associated with significant visual impairment. There were more children with penetrating eye injuries as compared with blunt trauma. Cataract surgery with intraocular lens implantation is a preferred method for visual rehabilitation in these children. Epidemiologic study, level IV.

Visual Outcome of Traumatic Cataract in Pediatric Age Group

To review results of traumatic cataracts in children. This is a retrospective cohort study done at a tertiary eye care center at the junction of Gujarat, Madhya Pradesh, and Rajasthan states in central western India. We enrolled children with specific inclusion criteria, examined their eyes to review the comorbidities due to trauma, performed surgery for traumatic cataracts, and implanted a lens, treating amblyopia if applicable. The patients were re-examined 6 weeks postoperatively. We divided the traumatic cataract cases into open-globe (group 1) and closed-globe (group 2) groups according to the ocular trauma based on the Birmingham Eye Trauma Terminology System and compared the determinants of visual acuity. Our cohort of 354 eyes with traumatic cataracts in children included 287 eyes in group 1 and 67 in group 2. Six weeks postoperatively, the visual acuity in the operated eye was >20/200 in 181 (63%) and =20/40 in 109 (38%) eyes in the open-globe group and >20/200 in 36 (53%) and =20/40 in 16 (22.4%) eyes in the closed-globe group (p = 0.143), and the difference between the groups was not significant in children. Overall, 125 (35.3%) eyes gained a visual acuity at 6 weeks of =20/40 and >20/200 in 214 (61.3%) cases. Satisfactory visual outcome can be achieved in children with traumatic cataracts, with no significant difference found among open- and closed-globe injuries in the pediatric age group.

Causes, epidemiology, and long-term outcome of traumatic cataracts in children in rural India

Purpose:To describe preoperative factors, long-term (>3 years) postoperative outcome and cost of traumatic cataracts in children in predominantly rural districts of western India.Subjects:Eighty-two traumatic cataracts in 81 children in a pediatric ophthalmology department of a tertiary eye-care center.Materials and Methods:Traumatic cataracts operated in 2004–2008 were reexamined prospectively in 2010–2011 using standardized technique. Cause and type of trauma, demographic factors, surgical intervention, complications, and visual acuity was recorded.Statistical Analysis:Data analysis done by using SPSS (Statistical package for social sciences) version 17.0 We have used Chi-square test, Fisher's exact test, paired t-test to find the association between the final vision and various parameters at 5% level of significance; binary logistic regression was performed for visual outcome ≥6/18 and ≥6/60.Results:The children were examined in a 3–7 year follow-up (4.35 ± 1.54). Average age at time of surgery was 10.4 ± 4.43 years (1.03 to 18). Fifty (61.7%) were boys. Forty (48.8%) were blunt and 32 (39%) were sharp trauma. The most common cause was wooden stick 23 (28.0%) and sharp thorn 14 (17.1%). Delay between trauma and presentation to hospital ranged from same day to 12 years after the injury with median of 4 days. The mean preoperative visual acuity by decimal notation was 0.059 ± 0.073 and mean postoperative visual acuity was 0.483 ± 0.417 (P < 0.001). Thirty-eight (46.3%) had best corrected visual acuity (BCVA) ≥6/18 and 51 (62.2%) had BCVA ≥ 6/60. In univariable analysis, visual outcome (≥6/18) depended on type of surgery (P = 0.002), gender (P = 0.028), and type of injury (P = 0.07)–sharp trauma and open globe injury had poorer outcomes; but not on age of child, preoperative vision, and type of surgeon. On multivariable binary logistic regression, only gender was significant variable. Of the 82 eyes, 18 (22%) needed more than one surgery. The parents spent an average of 2250 ($45) for the surgery and 55 (66.4%) were from lower socio-economic class.Conclusion:The postoperative visual outcomes varied and less than half achieved ≥ 6/18.

Outcome of in-the-bag implanted square-edge polymethyl methacrylate intraocular lenses with and without posterior capsulotomy in pediatric traumatic cataract

Dear Editor, We read with interest the article “Outcome of in-the-bag implanted square-edge polymethyl methacrylate intraocular lenses with and without posterior capsulotomy in pediatric traumatic cataract”[1] by Verma et al., in the September 2011 issue of IJO, but we have a few points and questions to make the article more pertinent. The main purpose of the article is to study the outcome of square-edge Poly Methyl methacrylate lenses with or without primary posterior capsulotomy in traumatic cataract in children. Several studies have been done to compare various types of lenses in children (which included traumatic cataracts) with and without primary posterior capsulotomy. Acrylic foldable lenses have shown better outcome as compared with PMMA lenses in these studies.[2] Therefore, we assume that the variable in this study would be the primary posterior capsulotomy. But, primary posterior capsulotomy in cataract surgery in children less than 6 years of age has become a standard of care[34] – why should there be any difference in surgery involving traumatic cataracts? The pathology of posterior capsular opacification is not going to change whether the cataract is developmental or traumatic? There should be some reason for not doing a primary posterior capsulotomy as visual axis opacification (VAO) is almost universal in children.[5] Hence, the second part is also a foregone conclusion. Was any upper limit of age considered for a primary posterior capsulotomy? There is no mention of this. Children older than 6 years of age can cooperate for a Yttrium Aluminium Garnate capsulotomy and hence a primary posterior capsulotomy may not be necessary. The preoperative vision was recorded as ranging from perception of light with inaccurate projection to 20/80 in both groups. Was any thought given to why the patients had inaccurate projection and what was the final visual acuity in these patients? The time interval between injury and trauma ranged from 0.7 to 7 months in group A and 0.2 to 25 months in group B. What was the reason for surgery as early as 0.2 months (as early surgery results in severe postop inflammation, posterior synechia, lens deposits and VAO and, possibly, cystoid macular edema, which is difficult to diagnose in children. Delaying the surgery by at least 6 weeks stabilizes the blood aqueous barrier and results in less inflammation and possibly decreases the possibility of CME. What was the reason for delaying surgery for as many as 25 months? (As late surgery will result in amblyopia.)

Surgical intervention for traumatic cataracts in children: Epidemiology, complications, and outcomes

To describe the presentation, treatment, and outcomes of a series of patients with unilateral traumatic cataracts. Retrospective, noncomparative review of 25 consecutive children with unilateral traumatic cataracts managed at a single institution during a 5-year period with regard to demographic data, mode of injury, clinical course, and outcome. Paintball and BB gun injuries were the most common mode of injury, accounting for 28% of all cases, followed by injury from pens and pencils (16%). Preoperative visual acuity was poor in 19 patients (76%), and final postoperative visual acuity was poor (20/200 or worse) in 4 patients (16%); this difference was statistically significant (p < 0.001). Twenty-one percent of children with poor vision on initial examination had poor vision at their final examination, and no patients with vision better than 20/200 on initial examination developed poor vision postoperatively (p = 0.29). Patients who developed poor final vision were younger on average than those who achieved satisfactory vision (5.25 years vs 7.5 years). Poor visual outcome did not appear to depend on the time from injury to presentation and time interval between injury and cataract extraction. Children with visually significant traumatic cataracts can have good outcomes if they are managed aggressively and appropriately. In our patient population, paintballs and BB guns accounted for more than 25% of traumatic cataract injuries, and a large number of traumatic cataracts were sustained by pens and pencils.

Functional results of posterior chamber lens implantation for traumatic cataracts in 22 children younger than seven years--a retrospective study

Surgical correction of aphakia following traumatic cataract in children is discussed controversely in literature. This study shows the functional results after cataract surgery with posterior chamber intraocular lens implantation in children under 7 years of age. This retrospective study analyzes the visual function of 22 children, who were operated after traumatic cataract between 1987 and 1997 at the department of ophthalmology, university of Erlangen-Nürnberg with PCL-implantation. Mean age at injury was 3 years 10 months +/- 1 year 8 months (range 15 to 76 months); the interval between injury and cataract surgery: 2.7 +/- 4.7 months (range 0 to 17 months). The follow-up time

Intraocular lens implantation for traumatic cataract in children in East Africa

Purpose: To review the visual outcomes and complications after intraocular lens (IOL) implantation in children with traumatic cataract in sub-Saharan Africa, where contact lenses for unilateral aphakia are impractical in most patients. Setting: PCEA Kikuyu Eye Unit, Nairobi, Kenya, East Africa. Methods: All children younger than 16 years having IOL implantation for traumatic cataract between February 1993 and December 1998 (215 eyes) were retrospectively reviewed. Complications and visual outcomes were evaluated. Results: The study group comprised 147 boys and 68 girls. The median interval between injury and cataract surgery was 8 weeks. The most common causes of injury were stick (36.3%) and thorn (10.7%). Extracapsular cataract extraction with IOL implantation was performed in all patients. The most frequent early complication was fibrinous uveitis in 110 eyes (51.2%). One hundred sixty-seven eyes (77.7%) had 1 month or more follow-up. Of those, 108 eyes (64.7%) had a best corrected visual acuity of 20/60 or better. Twenty-one eyes (12.6%) had a visual acuity of worse than 20/200, with the most common causes being amblyopia (9/21) and retinal detachment (5/21). Amblyopia was found in 42 of 108 (38.9%) children aged 8 years or less at the time of injury. Eyes with the IOL in the capsular bag were significantly less likely to require subsequent capsulotomy ( P < .01) during the 2-year follow-up. Conclusion: The results indicate that posterior chamber IOLs can be safely implanted by experienced surgeons in most children older than 2 years with traumatic cataract and should be the standard of care throughout the world.

Visual results and postoperative complications of capsular bag and ciliary sulcus fixation of posterior chamber intraocular lenses in children with traumatic cataracts

Purpose To evaluate the visual results and postoperative complications of capsular bag and ciliary sulcus fixation of posterior chamber intraocular lenses (IOLs) for traumatic cataracts in children. Setting Department of Ophthalmology, Postgraduate Institute of Medical Education and Research, Chandigarh, India. Methods Twenty children (20 eyes) with traumatic cataracts had extracapsular cataract extraction (ECCE) and posterior chamber IOL implantation. They were randomly divided into 2 groups. Capsular bag fixation was performed in 10 children (Group A) and ciliary sulcus fixation in the other 10 (Group B). Traumatic cataracts associated with large corneal lacerations (10.0 mm or more), hyphema, angle recession, or posterior segment involvement were excluded. The best corrected visual acuity (BCVA) as well as early and delayed postoperative complications were prospectively evaluated in both groups. Results The BCVA was 6/12 or better in 9 eyes (90%) in Group A and 8 eyes (80%) in Group B at the end of the mean follow-up (24.6 months ± 10.6 [SD]). Amblyopia (1 eye in Group A) and corneal scar and commotio retinae (1 eye each in Group B) accounted for a visual acuity of worse than 6/12. The residual refractive error did not exceed 3.50 diopters in either group. The incidences of fibrinous anterior uveitis and pupillary capture were significantly higher in Group B ( P < .05, Fisher exact test). Conclusion Capsular bag fixation of posterior chamber IOLs provided visual results similar to those with ciliary sulcus fixation but was associated with fewer postoperative complications, particularly uveitis and pupillary capture. This represents another important reason to attempt in-the-bag fixation in cases of traumatic cataract.

Traumatic Cataract in Children: Correction of Aphakia by Contact Lens or Intraocular Lens

To compare the postoperative complications, visual outcome, and incidence of strabismus in children suffering from traumatic cataract corrected with contact lens or intraocular lens and to follow up the refractive changes in these eyes for an extended period of time. Forty children, 2 to 13 years old at time of surgery for unilateral traumatic cataract, were followed up for 1.5 to 11 years. Seventeen children were corrected with contact lenses and 23 with intraocular lenses. Thirty-two underwent a primary posterior capsulectomy and anterior vitrectomy. The mean follow-up after surgery was 7.4 years for the children with contact lenses and 6.2 years for those with intraocular lenses. The incidence of secondary surgical interventions was higher among the children corrected with contact lenses. The eight children (five with contact lenses, three with intraocular lenses) who did not undergo primary posterior capsulectomy had Nd:YAG capsulectomy within 1 year after surgery. Fifteen of the 23 children with intraocular lenses (65.2%) achieved a best-corrected visual acuity of 20/40, and 17 children (73.9%) had a final visual acuity of 20/50, but only five of 17 children with contact lenses (35.3%) achieved this level of visual acuity. Correction of unilateral aphakia by intraocular lens in children after traumatic cataracts results in better final visual acuities and binocularity with a smaller incidence of strabismus than when correction is carried out by contact lens. Intraocular lens implantation should be considered the primary aphakic correction in children with traumatic cataract.
