Heel skin microclimate control: Secondary analysis of a self-controlled randomized clinical trial

Objectivethis study was undertaken to evaluate the efficacy of multilayer polyurethane foam with silicone (MPF) compared to transparent polyurethane film (TPF) dressings in the control of heel skin microclimate (temperature and moisture) of hospitalized patients undergoing elective surgeries. Methodthe study took of a secondary analysis of a randomized self-controlled trial, involving patients undergoing elective surgical procedure of cardiac and gastrointestinal specialties in a university hospital in southern Brazil, from March 2019 to February 2020. Patients served as their own control, with their heels randomly allocated to either TPF (control) or MPF (intervention). Skin temperature was measured using a digital infrared thermometer; and moisture determined through capacitance, at the beginning and end of surgery. The study was registered in the Brazilian Registry of Clinical Trials: RBR-5GKNG5. Resultssignificant difference in the microclimate variables were observed when the groups (intervention and control) and the timepoint of measurement (beginning and end of surgery) were compared. When assessing temperature, an increase (+3.3 °C) was observed with TPF and a decrease (−7.4 °C) was recorded with MPF. Regarding skin moisture, an increase in moisture (+14.6 AU) was recorded with TPF and a slight decrease (−0.3 AU) with MPF. ConclusionsThe findings of this study suggest that MPF is more effective than TPF in controlling skin microclimate (temperature and moisture) in heels skin of hospitalized patients undergoing elective surgeries. However, this control should be better investigated in other studies.

Transparent, multifunctional and multiform ionic sensors prepared by one-step impregnation

Under current technology trends, wearable devices with high levels of transparency and flexibility have become hotspots to improve aesthetics or enhance security for civilian and military applications. Simultaneously, the single sensor can no longer satisfy the various needs, like temperature and humidity, stress and strain and so on. However, it is still challenging that how to fabricate multifunctional transparent sensors nowadays. In this paper, the transparent polyurethane (PU) film was easily prepared by impregnating the ionic liquid (IL) into the pure PU film (PU@IL). And the effects of film thickness, ionic concentration and fiber morphology on transparency are investigated by experiments and simulations. On the basis of it, several sensors based on PU@IL film are further developed. The resistive sensor with PU@IL film shows different sensing abilities for stress, strain and temperature. Capacitive sensor based on indium tin oxide (ITO)-PU@IL-ITO has a lower stress detection limit (0.51 kPa) and faster response/recovery time (136.8 ms/68.4 ms). This paper may provide a novel strategy to design and fabricate multifunctional and multiform sensors with good mechanical properties, transparency and wide applications (preventing scalds, monitoring physiological activities).

The use of occlusive dressings: influence on excisional wound healing in animal model.

To analyze the influence of occlusive dressing on the healing of excisional skin wounds in mice. Pre-clinical, comparative, and translational study. Mice were divided into three experimental groups: wounds occluded with hydrocolloid (HD) dressings, transparent polyurethane film (TF) dressings, and without occlusion (WO), monitored at three, six and 14 days, with eight animals each. Closure rate, infiltration of neutrophils and macrophages, measurement of tumor necrosis factor-α (TNF-α) and vascular endothelial growth factor (VEGF) and, histologically, angiogenesis were evaluated. Wound closure was accelerated in the occlusive groups. There was a decrease in TNF-α levels in the HD group when compared to the WO and TF groups. Neutrophils accumulation decreased in the HD group. Increased dosages of macrophages were evidenced in the HD group, compared to the WO and TF groups. Levels of VEGF were increased in the TF and HD groups. It is suggested that the occlusion of wounds modulates the inflammatory response.

Development and Characteristics of Multipurpose Transparent Polyurethane Film.

In industry, recent research developments include flexible films and foldable films. The next step is the development of stretchable films, and studies are being intensively carried out. Research on the development of stretchable and transparent materials is also increasing greatly. Currently, polydimethylsiloxane (PDMS) is the most commonly used film in the industry. However, PDMS surfaces are hydrophobic, so their use is limited to making materials and compounds with hydrophilic properties. In this study, we developed a transparent polyurethane film that can be used for multiple purposes. A transparency comparison between the transparent polyurethane film and the general polyurethane film was used to verify their future application. The conventional polyurethane films showed a transmittance rate of 2.2 percent, but the transparent polyurethane films achieved a high transmittance rate of 85 percent. To determine whether the film can be realized, we produced a conductive paste using resin for the transparent polyurethane film. In addition, a conductive paste was made based on the material used in the transparent polyurethane film to verify the hardness and reliability of the adhesion of electrodes, and we confirmed this with thermogravimetric analysis (TGA). The transparent polyurethane based paste was made with stretchable electrodes through a screen printing method. The manufactured stretchable electrodes were demonstrated by mechanical and adhesion tests. Finally, a permittivity test was conducted to determine the suitability of the film for application to printed electrodes for antennas in the future. The genetic rate of transparent polyurethane films was better than that of conventional polyurethane films. Moreover, the adhesion of the transparent polyurethane film and stretchable electrodes was as good as that of conventional polyurethane film and stretchable electrodes, and observation by optical microscopy confirmed that the printing performance was also excellent. In addition, the conductive paste made based on the transparent polyurethane film material was cured for 1 hour at 120 °C, and TGA analysis confirmed that both the binders and curing agent responded well in the test for curing the developed stretchable electrodes and transparent polyurethane.

Prevalence and associated factors of medical adhesive-related skin injury in cardiac critical care units.

To identify and analyze the point prevalence of medical adhesive-related skin injury in patients hospitalized in cardiac Intensive Care Units and demographic and clinical factors associated to their occurrence. Cross-sectional study conducted in Intensive Care Units of two public hospitals, reference centers for cardiology, located in São Paulo city, with 123 patients. Demographic and clinical data from patients' medical records were collected and lesions were identified through skin inspection. The data were analyzed through descriptive statistics and bivariate and multivariate analysis (Classification and Regression Tree). Medical adhesive-related skin injury was presented by 28 patients, which amounts to a 22.7% prevalence. Its main causative agent was transparent polyurethane film (46.9%) and the cervical region was the most affected area (25.1%). From the multivariate analysis, the associated factors were found to be the presence of serum urea higher than or equal to 48.5 mg/dL and platelets lower than 193,500 mm3 or platelets higher than or equal to 193,500 mm3 and systolic blood pressure higher than or equal to 122 mmHg. This study has contributed to knowledge related to the epidemiology of this type of injury, favoring preventive care planning.

Prevention of pressure injury in the operating room: Heels operating room pressure injury trial.

The objective was to evaluate the efficacy of multi‐layered silicone foam (intervention) compared with transparent polyurethane film (control) in preventing heel pressure injuries caused by surgical positioning of individuals undergoing elective surgery. It was designed an intra‐patient, open, parallel, randomised controlled trial was conducted in a university hospital in southern Brazil, from March 2019 to February 2020, with patients undergoing elective surgeries of cardiac and gastrointestinal specialties. The patients who met the selection criteria constituted, simultaneously, a single group receiving the intervention and active control, through paired analysis of the cutaneous sites (right heel and left heel). The outcome was the occurrence of PI, within the follow‐up period was 72 hours. Brazilian Registry of Clinical Trials: RBR‐5GKNG5. There was analysis of 135 patients/270 heels, with an overall incidence of 36.7%. The pressure injury incidence was significantly lower in the intervention group (26.7%), compared with the control group (P = .001); relative risk of 0.57. In the intervention group, the estimated pressure injury‐free time (survival) was 57.5 hours and in the control group, 43.9 hours. It was concluded that Multi‐layered silicone foam (intervention) is more efficacious than transparent polyurethane film (control) in the prevention of pressure injuries caused by surgical positioning of individuals undergoing elective surgery.

529 Collagen Plus Alginate Wound Dressing for the Treatment of Split-thickness Skin Graft Donor Sites: A Cost-effectiveness Study

Abstract Introduction The collagen plus alginate dressings for donor sites have been used in our country since 2003. Before that, the use of rayon was the main and only preference of use. We were looking for something new at that time that could be less painful and maybe presented a good relation cost-benefit. This alternative dressing seemed to offer collagen to the wound so that the imbalance of metalloproteases would be reduced. We started a study to evaluate the use of collagen calcium-alginate dressing for donor sites. Methods A prospective clinical study was conducted for 2010 to 2017, when this material was donated (high cost in our service). Patients were selected consecutively and randomly allocated into three groups according to the treatment used on the donor site: rayon soaked in 0.9% saline; rayon covered with sterile cotton gauze and bandage; 90% bovine collagen with 10% calcium-alginate dressing covered with transparent polyurethane film. Following parameters was comparatively analyzed: pain, time to epithelialization, length of stay and costs. Results We studied 30 patients, ranging from 12 to 60 years of age. Fifteen of these patients had their donor sites covered with collagen calcium-alginate dressing, which showed pain reduction of 79.5% (p< 0.01) shorter hospital stay and epithelialization, average of 5.8 days (p< 0.01) and reduction in hospital costs about 47% (p< 0.01) in comparison with rayon dressing. None presented infection in the donor site. Conclusions Collagen calcium-alginate dressing showed better cost-benefit than rayon to cover donor sites, with significant reduction of pain, epithelialization time, length of stay and costs. Anchored quaternary ammonium salts adsorbed on polyurethane film surfaces

A flexible, thermoset and transparent polyurethane (PUR) film with high glass transition temperature, has been deployed as a substrate for anchoring anti-bacterial molecular layers. These layers are formed by adsorbing, from an aqueous solution, a silyl anchoring quaternary ammonium salt (AQAS) with a long alkyl chain (C18) onto the PUR film surface, followed by thermal curing at 160 °C. The presence of AQAS adsorbed on the PUR film surface is confirmed by the appearance of a distinctive N1 s level XPS peak at 402 eV, attributed to quaternary nitrogen (N+). The carbonyl groups on the PUR surface appear to be the main surface binding sites for the adsorbed AQAS molecules. This interpretation is validated by the relative decrease in the intensities observed for the ν (CO) at 1700 cm−1 in the infrared spectra, and decreases in the intensity of the C1 s binding energy peak for CO at 289 eV in the XPS spectra of the PUR following AQAS adsorption. Leaching studies suggest a chemisorbed first layer and several leachable higher layers of oligomeric forms of AQAS. A PUR surface incorporating a blended polydimethylsiloxane additive exhibits similar adsorptive capacity to pristine PUR. However exposing the PUR films to oxygen plasma treatment prior to AQAS adsorption leads to a higher coverage of the adsorbate on the PUR surface. The anchoring quaternary ammonium salt (AQAS) in this current work has established potencies against a wide range of critical organisms. Antibacterial assays adapted from the JIS 2801 wet fomite method confirm the potency (3-log reduction) of these PUR-AQAS films against Staphylococcus aureus and Escherichia coli.

Dressings for the central venous catheter to prevent infection in patients undergoing hematopoietic stem cell transplantation: a systematic review and meta-analysis

To identify the most effective dressing for covering long-term central venous catheter exit site to prevent catheter-related infections and skin irritation in patients undergoing hematopoietic stem cell transplantation. Systematic Review. The search was performed in the following electronic databases: CINAHL, Cochrane Library CENTRAL, EMBASE, LILACS, PubMed, Scopus, and Web of Science. Google Scholar was used for the gray literature search. Seven studies were included which tested different arrangements of dressings: sterilized gauze and adhesive tape with a transparent polyurethane film (n = 2), transparent polyurethane film with a different replacement interval frequency (n = 2), transparent polyurethane film with and without chlorhexidine released continuously by the dressing at the site of intravascular catheter insertion (n = 2), and dressings vs. no dressings (n = 1). The meta-analysis for catheter-related infection prevention showed no difference between type of dressing (RR 1.76, [95% CI 0.82; 3.75], I2 0%) and for the replacement frequency at different intervals (RR 1.04, [95% CI 0.67; 1.61], I2 0%). The meta-analysis for skin irritation evaluated the transparent polyurethane film replacement frequency and indicated that a longer dressing replacement interval (10 to 15days) reduces the risk of developing this outcome (RR 0.71, 0.52; 0.96, 95% CI, I2 24%). Regarding the type of the dressing, there is no evidence indicating the best dressing. Although there is no evidence available for the ideal replacement frequency, the risk to develop skin irritation is reduced in longer dressing replacements intervals.

A Prospective, Within-Patient Controlled Study to Compare the Ability of the Non Adherent Drawtex® Hydroconductive Dressing to a Transparent Polyurethane Film Dressing (Standard of Care) on the Healing of Split-Thickness Skin Graft Donor Sites*

Background: Dressing of split-thickness skin graft donor sites can be traumatic for the patient. The most advanced and expensive dressings have been compared to the most basic of dressings, with little or no consensus and an unpersuasive level of evidence. We aimed to determine the efficacy of the locally manufactured non-adherent, hydroconductive Drawtex® dressing and compare it to our current standard-of-care dressing, a thin transparent polyurethane film, in the healing of split-thickness donor sites. Methods: This prospective, within-patient controlled study included 27 adult participants, each with two split-thickness skin donor sites. The 54 donor site wounds were compared with regard to time to re-epithelialisation, perceived pain and healed wound quality. Results: By day 5, complete healing of donor site wounds, defined as >90% of epithelialized surface, was significantly higher in the hydroconductive dressing group compared to the polyurethane film group (22.2% and 3.7%, respectively; p < 0.0001). The hydroconductive dressing-treated donor site wounds were significantly less painful at 24-hours, 48-hours and 7-days post-operatively, and had fewer complications and superior wound healing quality. Conclusion: We have demonstrated that the relatively cheap and readily available dressing made locally in South Africa, Drawtex® is at least as safe, and potentially superior in wound healing, when compared to our current standard-of-care dressing.

Cost comparison of pressure ulcer preventive dressings: hydrocolloid dressing versus transparent polyurethane film

To evaluate and compare the costs of using a transparent polyurethane film (PF) and hydrocolloid dressing (HD) in the prevention of pressure ulcers (PUs). This descriptive, observational, longitudinal, comparative study was conducted in the intensive care units, coronary care unit and medical clinic of a charity hospital in Brazil. Data were collected during a 30-day study period, consisting of physical examination, assessment of risk factors for PU development and application of the Braden scale, which were performed at inclusion in the study and once daily during hospitalisation. Either PF or HD was applied bilaterally in the sacral and trochanteric regions for prevention of PUs in patients at a moderate to high risk of PUs according to the Braden scale, and costs of using PU preventive dressings were estimated. The mean total costs per dressing change per patient when using the HD and PF to prevent PUs were 413.60 BRL and 74.04 BRL, respectively. There were significant between-group differences in mean costs for all variables, except for saline solution and nurse-technician services. Results showed that the mean cost per dressing change per patient was lower when using the transparent PF than when using the HD.

Prevenção de úlceras por pressão no calcanhar com filme transparente de poliuretano

OBJETIVO: Avaliar a efetividade do filme transparente de poliuretano na prevenção de úlceras por pressão no calcâneo. MÉTODOS: Uma amostra de 100 pacientes inscritos em um ensaio controlado não-randomizado recebeu uma análise pareada de ambas as áreas do calcâneo; cada um deles recebeu a intervenção experimental (filme transparente poliuretano) no calcanhar esquerdo e a intervenção controle no calcanhar direito (somente diretrizes clínicas), constituindo um total de 200 áreas de calcanhar para análise. RESULTADOS: A incidência de úlceras por pressão foi de 32%, com 6% ocorrendo na intervenção experimental, 18% na intervenção de controle e 8% bilateralmente, com incidência significante nos primeiros 15 dias de hospitalização. A duração de tempo sem ocorrências de úlceras por pressão na intervenção experimental foi de 19,2 dias, com um intervalo de confiança de 95%. CONCLUSÃO: Foi concluído que o filme transparente de poliuretano associado às diretrizes clínicas das úlceras por pressão foi efetivo na prevenção da úlcera por pressão no calcanhar.

Clinical Trial Comparing 3 Different Wound Dressings for the Management of Partial-Thickness Skin Graft Donor Sites

A review of the literature reveals a lack of consensus regarding local management of skin graft donor sites. This study was undertaken to determine the effects of 3 different dressings on healing of donor sites and patient discomfort related to donor sites. This study is a comparison cohort study. We recruited 34 burn patients scheduled for partial-thickness skin grafts; their mean age was 36 ± 18 years (mean ± SD, range 20 to 54 years), and 63% were male. All subjects were managed at Burn Unity Care, located in Sao Paulo, Brazil. Subjects were randomly allocated to 1 of 3 groups: (1) a study group (group A) whose donor sites were dressed with a bovine collagen calcium-alginate dressing covered with transparent polyurethane film; (2) a second intervention group (group B), whose donor sites were dressed only with transparent polyurethane film dressing; and (3) a control group whose donor sites were dressed with rayon soaked in 0.9% saline (group C). Two independent observers assessed donor site wounds for epithelialization, scabbing, quantity and characteristics of exudate, and complications. Pain was measured using the visual analog scale, the brief pain inventory, and Index of Pain Management. Subjects managed with the bovine collagen calcium-alginate dressing covered with transparent polyurethane film (group A) achieved the greatest epithelialization (6.3 vs 8.2 for thin film dressing only P < .02 and 6.3 vs 11.7 days for control group P < .01). Patients managed with the bovine collagen calcium-alginate dressing covered with transparent polyurethane film also reported less pain that subjects allocated to the control (group C) or thin film only group (group B), (P < .05). Ninety percent of subjects allocated to the calcium alginate covered with thin film dressing reported mild pain intensity on the Visual Analog Scale, 85% of did not report pain localized to the donor site on the brief pain inventory, and scores on the Index of Pain Management ranged from 23 to 11. Study findings suggest that use of a collagen calcium-alginate dressing with a transparent film covering reduces the time for complete epithelialization and may reduce pain related to skin graft donor sites.

Ensaio clínico controlado sobre o curativo de cateter venoso central

OBJETIVO: Avaliar a eficácia dos curativos de gaze e fita e filme transparente de poliuretano para cobertura de cateteres venosos centrais. MÉTODOS: Ensaio clínico controlado randomizado. RESUlTADOS: Não foi identificada uma diferença significativa com relação à infecção relacionada ao cateter (p=1) e à fixação do curativo (p=0,670). Foi identificada diferença estatisticamente significativa com relação à absorção de exsudato (p&lt;0,001). A probabilidade de ocorrer reação local no grupo controle é diferente do grupo estudo (p=0,024). CONCLUSÃO: O tipo de curativo não diminui a incidência de infecção relacionada ao cateter, a capacidade de fixação é semelhante, o curativo de gaze e fita possui capacidade de absorção de exsudato, porém apresenta probabilidade maior de desenvolver reação local. Transparent Film Dressings for Intravascular Catheter Exit-Site

Transparent polyurethane film is recommended for catheter site insertion dressing by the American guidelines for the prevention of infections associated with intravascular catheters. It has been proven to offer the advantages of excellent adhesion, firm support of the catheter, good tolerability, ease of application and fewer replacements per catheter lifetime. The last two features save nursing time and result in healthcare cost savings.

Central venous catheter dressings: a systematic review.

Gauze and tape or transparent polyurethane film dressings such as Tegaderm, Opsite or Opsite IV3000 are the most common types of dressing used to secure central venous catheters (CVCs). Currently, there are no clear guidelines as to which type of dressing is the most appropriate. To identify whether there are any differences between gauze and tape and/or transparent polyurethane film dressings in the incidence of CVC-related infection, catheter-related sepsis, catheter security, tolerance to dressing material, dressing condition and ease of application in hospitalized patients. The Cochrane Controlled Trials Register and Medline, Embase and CancerLit databases were searched to identify any controlled trials comparing the effects of gauze and tape and/or transparent polyurethane dressings on CVCs. Additional references were sought from published and non-published literature. Twenty-three studies were reviewed. Data were extracted independently from each paper by two members of the review team and results compared. Differences were resolved either by consensus or referral to a third person. Authors were contacted for missing information. Of the 23 studies reviewed, 15 were excluded. Of the remaining eight, data were available for meta-analysis from six studies. Of the six included studies, two compared gauze and tape with Opsite IV3000, two compared Opsite with Opsite IV3000, one compared Tegaderm with Opsite IV3000, and one compared Tegaderm with Opsite. There was no evidence of any difference in the incidence of infectious complications between any of the dressing types compared in this review. Each of these comparisons was based on no more than two studies and all of these studies reported data from a small patient sample. Therefore it is unlikely that any of these comparisons would have had sufficient power to detect any differences between groups.

Central venous catheters facilitate venous access, allowing the intravenous administration of complex drug treatments, blood products and nutritional support, without the trauma associated with repeated venepuncture. However, central venous catheters are associated with a risk of infection. Some studies have indicated that the type of dressing used for central venous catheters may affect the risk of infection. Gauze and tape or transparent polyurethane film dressings such as Tegaderm, Opsite or Opsite IV3000 are the most common types of dressing used to secure central venous catheters. Currently, it is not clear which type of dressing is the most appropriate. To compare gauze and tape and transparent polyurethane central venous catheter dressings in terms of catheter related infection, catheter security, tolerance to dressing material and dressing condition in hospitalised adults and children. The Cochrane Wounds Group Specialised Trials Register (October 2002), the Cochrane Controlled Trials Register (4th Quarter 2002) and the databases; MEDLINE (1966-December 2002, CINAHL (1982-October 2002) and EMBASE (1980-December 2002) were searched to identify any randomised controlled trials comparing the effects of gauze and tape and/or transparent polyurethane dressings for central venous catheter sites. Additional references were identified from bibliographies of published literature and were also sought from other sources. Therefore, at this stage it appears that the choice of dressing for central venous catheters can be based on patient preference. To identify the most appropriate central venous catheter dressings, further research is necessary. It is paramount that any future studies investigating this issue must be rigorously performed randomised controlled trials.
