Impacted Permanent Mandibular Canines: Epidemiological Evaluation.

The aim of this study was to evaluate the epidemiology of impacted and transmigrated mandibular canines in a large orthodontic population referred to the University of Turin. Panoramic radiographs, intraoral photographs, and dental casts of 2119 patients referred to the Department of Orthodontics at the University of Turin, Italy, between 1995 and 2022 were reviewed. These patients were divided into two groups. Group A included 1479 patients found in the Dental School archive before 2017, more specifically between 1995 and 2017. These patients were examined in order to calculate the prevalence of impacted and transmigrated mandibular canines. From 2017 to 2022, the records of 640 new patients were examined (GROUP B) in order to calculate the incidence of these occurrences. The prevalence of mandibular canine impaction in Group A was found to be 1.7%, with a total of 25 patients having mandibular canine impaction. A prevalence of 0.3% was found for mandibular canine transmigration (Group A). The incidence of mandibular canine impaction was found to be 2%, with a total of 13 patients with mandibular canine impaction (Group B). Mandibular canine transmigration was found in 1 of 640 participants (Group B). Twenty-five of 1479 patients had impacted mandibular canines, resulting in a prevalence value of 1.7%. The incidence was found to be 2%, with 13 of 640 patients having impacted mandibular canines. These results show higher prevalence and incidence rates of mandibular canine impaction when compared with previously published data.

Intra osseous transmigration of mandibular canines – A report of two cases

Background: A canine is classified as "transmigrated" when it crosses the midline. Transmigration of mandibular canines bilaterally is an extremely rare developmental dental ectopia with obscure etiology. Case Description:A 13 year old female and 10 year old male patient reported to the department with the chief complaint of missing teeth in the upper front teeth region and retained deciduous tooth in the lower front teeth region respectively. Panoramic radiographs revealed horizontally impacted permanent mandibular canines crossing the midline and located below the apices of the incisors. Based on the clinical, radiographic and CBCT findings, a diagnosis of transmigration of mandibular canines was established.Conclusion: Dental anomalies and pathologies such as cystic lesions, odontoma and root resorption of the adjacent teeth have been reported to be associated with transmigrated canines. Early diagnosis by radiographic evaluation and correction either by orthodontic intervention or by surgical transplantation is important to protect oral structures and preserve the canines to ensure facial harmony and improved function.

We present five instances of transmigrated mandibular canines together with an audit of the writing etiology, clinical highlights, treatment alternatives, case reports and conclusion at the end.

Effect of Diagnosis by Two-Dimensional Radiography Versus CBCT on Surgical Aspects of Transmigrated Impacted Mandibular Canines.

The transmigration of mandibular canines is a rare clinical finding. The present study aimed to compare the effect of diagnosis by two-dimensional radiography and cone beam computed tomography on surgical aspects of transmigrated impacted mandibular canines. A total of 20 patients (mean age 20.18 ± 3.36years; male/female = 1.5:1), reporting to the Department of Oral and Maxillofacial Surgery with transmigrated impacted mandibular canines and planned for surgical removal, were randomly divided into two groups of ten patients each. Group A patients were diagnosed using 2D radiographs (orthopantomograms and occlusal view radiographs), and Group B patients were diagnosed using CBCT scans. The effects of the diagnosis on surgical aspects were compared in both the groups using various intraoperative and post-operative parameters. The operating surgeons experienced lesser operating time and more accuracy in terms of prediction of surgical approach in Group B patients than in Group A. Post-operative complications, including post-operative vitality of the adjacent tooth and lower lip paraesthesia, were seen to be more in Group A patients than in Group B. Increased precision in localization of transmigrated impacted mandibular canines was seen by using CBCT, leading to better surgical experience and lesser post-operative complications.

Transmigration of cuspids: Report of three rare cases

The pre-eruptive and intraosseous migration of teeth across the midline is called as transmigration. This rare phenomenon is common in the mandible compared to the maxilla. The occurrence of bilateral transmigration of mandibular canine is even rarer than unilateral transmigration. In these three cases, two of them were unilateral transmigration and the third was bilateral transmigration. In unilateral transmigration cases, one among them showed compound odontome-induced transmigration of canine and displaced premolar toward the third molar of the same side and another with unilateral transmigration of canine just crossing the midline. The case had bilaterally transmigrated canines. The occurrence of bilateral transmigration of the mandibular canine is very rare than the unilateral transmigration. Many factors had been suggested as causative agents for transmigration of mandibular canines, but there is no clear, established etiology for this phenomenon. Mupparapu proposed classifications for unilateral and bilateral transmigration of mandibular canines after reviewing literature.

Impacted and transmigrated mandibular canines: an analysis of 3D radiographic imaging data.

Impacted and transmigrated mandibular canines differ greatly in incidence, etiopathology, associated anomalies, and treatment prospects, when compared to their maxillary counterparts. The aim of this study was to provide a detailed analysis of 3D radiographic imaging data of impacted mandibular canines. In a retrospective cross-sectional study, CT/CBCT data of 88 patients with a total of 94 impacted mandibular canines were analysed. Evaluated parameters included location, morphology, neighbouring structures, associated anomalies, the influence of those factors on mandibular canine transmigration, as well as applied treatment. Transmigration was found to occur in 40.4% of impacted mandibular canines. Transmigrated canines were located significantly more basally and horizontally angulated. Further, transmigration was significantly associated with a lack of contact to adjacent teeth and the canine's apex not contacting the mandibular cortical bone. The overall incidence of root resorptions of adjacent teeth related to impacted mandibular canines was 7.3% and was more likely, if the canine was lingually impacted. While about half of the non-transmigrated impacted canines were orthodontically aligned, half of the transmigrated canines were surgically removed. Monitoring was the second most applied treatment strategy for both groups, and no canines were autotransplantated. Root resorption of adjacent teeth and transmigration are commonly occurring phenomena related to impacted mandibular canines. Treatment often entails the surgical removal of the canine-especially in cases of transmigration. The findings emphasise the importance of early diagnosis and CT/CBCT imaging for further diagnostics and future research of impacted mandibular canines.

Transmigration of the left mandibular canine observed during 2 years in cone-beam computed tomography examination

Transmigration is considerably a rare preeruptive dislocation of an affected tooth across the midline. Cone-beam computed tomography (CBCT) is a relatively new technology in dentomaxillofacial radiology. The aim of the study is to present a case report of progressing transmigration in an 11-year-old girl examined by means of CBCT. A 9-year-old girl was referred for CBCT twice before and during orthodontic treatment. In the baseline examination, the impacted lower left canine was impacted and incorrectly positioned, but 9 months later, the canine had already crossed the midline. In conclusion, to the authors' best knowledge, this is the first case of observation of mandibular canine transmigration by means of CBCT that allows precise evaluation of the transmigrating tooth.

Aims and Objectives:Transmigration is a rare phenomenon seen almost exclusively in the mandibular canines. The aim of the present study is to review transmigration phenomenon.Materials and Methods:Appropriate guidelines for a systematic review were followed. The time period selected for the present systematic review was 2001–2016. The studies were selected from various electronic databases on the basis of their title, study, design, keywords, and abstracts. A total of 150 citations were searched initially, and after proper screening, 59 relevant articles were included. Additional data was obtained by searching journals and reference lists.Results:The literature search shows that transmigration is more frequent in the mandible than maxilla. The etiology of the condition is obscure; however, multiple factors have been attributed to the condition. They are more readily recognized now with the advent of panoramic radiographs. Transmigration is a rare anomaly causing varied manifestations and requires an interdisciplinary approach for management.Conclusion:Early diagnosis of impacted canines is mandatory for timely treatment to ensure facial harmony and improved function.

Unusual transmigration of canines – report of two cases in a family

Transmigration is pre-eruptive migration of tooth across the midline. The etiology of this rare anomaly is unknown. Transmigration is largely related to mandibular canines. Although maxillary canine transmigrations are found in the literature, they are still a rare entity. Objective: The aim of the present paper is to report two cases of unusual transmigrations of canines in two immediate members of a family and to report the first case of simultaneous transmigration of maxillary and mandibular canines in an Indian adolescent. Case report: A rare case of simultaneous transmigration of maxillary and mandibular canine in Indian adolescent along with bilateral transmigration of mandibular canines in her father is described here. Conclusion: Transmigration of canines in two immediate members of family needs to be further studied for familial occurrence of transmigration.

A Study of Transmigrated Canine in an Indian Population.

Aim. The purpose of this study was to investigate the prevalence of transmigrated canines in a north Indian population and association with gender, side, associated pathologies, and dental anomalies. Subjects and methods. The prospective study consisted of panoramic radiographs of 3000 patients from two dental colleges in north India. The panoramic radiographs were screened for radiographically identified position of the transmigrated tooth, retained canine, and other coexisting dental anomalies. Results. The overall prevalence of transmigrated canines (15 mandibular and 5 maxillary) was 0.66%. The prevalence of mandibular transmigrated canine was 0.5% and maxillary transmigrated canine was 0.16%. All the transmigrated canines were unilateral. The age range was 15–53 years (average age 24.1 years) and there were 12 males (60%) and 8 females (40%). Type 1 mandibular canine transmigration was the commonest type found in our study (10 cases), followed by types 2 and 4 (2 cases each) and 1 case of type 5 transmigration. Conclusion. The prevalence of transmigrated canines in the north Indian population was 0.66% and no gender predilection was evident. The transmigrated canines have a low complication rate (10.0%) and no correlation with other dental anomalies was found. Type 3 canine is the rarest form of mandibular canine transmigration.

Intraoral examination determined hypodontia of right second premolar and delayed eruption of left second premolar in maxilla, as well as persistent deciduous teeth: right second molar, left canine and second molar. The patient was referred for a Cone-Beam CT examination, which allowed precise visualization of the transmigrating canine as well as ruled out resorption of roots of mandibular incisors.ResultsThe treatment with a maxillary fixed orthodontic appliance was finished after obtaining a satisfactory result. Proper alignment of the incisors in the anterior-posterior plane and correct midline position were accepted by the patient. Transmigrating canine after consultation with the surgeon was designed to further radiological observation.

Transmigration of teeth is a relatively less well known phenomenon. Its etiology is not so well understood. We present a case of bilateral transmigration of mandibular canines with a type 5 classification pattern (Muparappu) in a 21 year old male patient with emphasis on its etiology and complications accompanying its management.

Transmigration of mandibular canines.

The purpose of this paper is to present the first case of unusual reverse oblique (110 degrees to midsagittal plane) migration of mandibular right canine crossing the jaw midline and piercing the lower border of the mandible at the level of the opposite canine and also to report two more cases of transmigrated mandibular canine and one case of transmigrating mandibular canine. Mandibular canines are “cornerstone” of dental arch; their importance is manifested by their efficiency in masticatory function, stability of dental arch, and aid in maintaining natural facial expression. Early detection of this anomaly can help preserving these canines by orthodontic intervention or by surgical transplantation. This developmental anomaly is properly diagnosed by radiographic evaluation, which is primarily based on the panoramic radiograph. In patients with overretained deciduous canines or missing permanent canines, an intraoral radiograph should be supplemented with panoramic radiograph. This paper discusses the importance of early diagnosis of canine transmigration in treatment planning and reviews the various possible treatment options.

An Unusual Occurrence of Multiple Dental Anomalies in a Single Nonsyndromic Patient: A Case Report

Dental anomalies are the formative defects caused by genetic disturbances or environmental factors during tooth morphogenesis. Simultaneous occurrence of various multiple anomalies has been reported previously, particularly in cases of chromosomal abnormalities that often manifest with multisystem involvement. Very few cases of multiple anomalies have been documented in patients with no known generalized abnormalities. The present paper shows an unusual occurrence of a combination of dental anomalies like mandibular canine transmigration, taurodontism in permanent mandibular molars, congenital agenesis of 14 numbers of permanent teeth excluding third molars, canine impaction, primary molars with pyramidal roots, midline diastema and generalized microdontia in an apparently normal 13-year-old Indian girl.

A review of the literature relating to transmigrating canines. An illustrated case report

Canine impaction is not an uncommon finding in the dental literature, but transmigration of mandibular canines is a rare phenomenon, and some of them are far more extreme than others. We report an extremely rare case of bilateral transmigration of impacted permanent canines crossing the midline of the mandible. Clinical Relevance: To understand the role of early interception so that the patient can be treated before canines transmigrate in order to avoid surgical extraction, as well as complex orthodontic and restorative therapies.
