Seroprevalence of viral transfusion transmissible infections (HBsAg, anti-HCV, anti-HIV, Syphilis) and coinfection among healthy volunteer blood donors during 5-years in Luanda, Angola

BackgroundThe transmission of diseases by blood products continues to be a worldwide health problem, especially in Africa. Seroprevalence rates of the Hepatitis B virus (HBV), Hepatitis C virus (HCV), Human Immunodeficiency Virus (HIV), Syphilis, and Coinfection in Angola are poorly documented. This study aims to identify the seroprevalence of markers with positive results for Hepatitis B, C, HIV, Syphilis, and Coinfection in blood donors. Material and methodsA retrospective study was conducted using a database of positive serological markers for these infections and coinfection in 2734 blood donors traced from 2011 to 2016 in Luanda, Angola. The Chi-Square test (χ2) or Fisher's exact test was used to evaluate serological positivity and donors’ characteristics. A p-value < 0.05 was considered statistically significant. Results2734 blood donors aged 18 to 64 (median age 32 ± 9) were screened from 2011 to 2016. 73.9 % of the donors were positive for one Transfusion-Transmitted Infection (TTI), and 5.9 % showed evidence of multiple infections. The overall seroprevalence rate was 50.2 % (1373) for HBV, 20 % (436) for Syphilis, 7 % (191) for HIV, 5.1 % (140) for HCV, and 5.8 % for coinfected donors. 2467 (90 %) were men, and 267 (10 %) were women. We identified 118 (5.8 %) coinfected donors. Of those, 40 (33.9 %) simultaneously presented Hepatitis B virus surface antigen (HBsAg)/Syphilis, 24 (20.3 %) HBsAg/HIV, 22 (18.6 %) HBsAg/HCV, 20 (16.9 %) HIV/Syphilis, 8 (6.8 %) HCV/Syphilis, and 4 (3.4 %) HIV/HCV. ConclusionA high transfusion-transmissible infection prevalence was found compared to some countries in Sub-Saharan Africa. Therefore, intensifying the screening for these transfusion-transmitted infections in blood donors is critical to ensure blood safety.

Seroprevalence of HAV and Mandatory Markers of Transfusion- Transmitted Infections in Blood Donors from Chiapas, Mexico: an Exploratory Study

Introduction: Blood transfusion saves millions of lives annually. However, blood transfusion is one of the leading risk factors for Transfusion-Transmitted Infections (TTIs). Therefore, the objective of this study was to explore the seroprevalence of both HAV and mandatory markers of TTIs in blood donors from Chiapas, Mexico. Materials and Methods: The study included 3,067 blood donors from the Dr. Domingo Chanona Rodríguez Blood Bank located in Tuxtla Gutiérrez, Chiapas, and from 9 collection centers located in different regions of Chiapas. Screening was performed using Enzyme immunoassay (anti-HAV IgM) or Chemiluminescent microparticle immunoassay (HBV HBsAg, anti-HCV IgG/IgM, HIV Ag/Ab, anti-T. pallidum IgG/IgM, and anti-T. cruzi IgG). Results: The seroprevalence of T. pallidum was the highest at 1.3%, followed by the seroprevalence of HAV and T. cruzi, both with 0.7%. Regions X and XV of the state of Chiapas presented the six markers of the analyzed TTIs. Blood donors at risk of being TTI reactive donated in regions IX and X. However, donors with qualified and unqualified employment and those who have donated more than 2 times are considered protective factors against TTIs. Conclusion: We observed a high seroprevalence of HAV in this exploratory study. It is indispensable to increase the sample size of blood donors from the state of Chiapas because HAV is endemic to Mexico and it is probably transmitted through blood transfusion. Also, it is necessary to augment voluntary non-remunerated blood donations in the state of Chiapas, mainly in the Southern border regions, to decrease the risk of TTIs.

Prevalence and Trends of Transfusion Transmissible Infections in Blood Donors in a Tertiary Care Centre- An Institutional 4-year Retrospective Study

Introduction: In today’s time, safe transfusion begins with the screening and confirmation of availability of healthy donors. An important criterion to make safe blood available for one and for all is to try and prevent Transfusion Transmitted Infections (TTIs); and once if a donor is identified to be reactive for any one of the TTIs, then to notify and counsel them. Aim: To study the seroprevalence and trends of TTIs in blood donors at MGM Medical College and Hospital, Navi Mumbai, Maharashtra, India. Materials and Methods: This retrospective study was done in the Department of Immunohaematology and Blood Transfusion (IHBT) at MGM Medical College and Hospital, Navi Mumbai, Maharashtra, India for period of four years from January 2017 to December 2020 (48 months). Screening for anti-Human Immunodeficiency Virus (HIV)-1/2, anti-Hepatitis C Virus (HCV), and Hepatitis B surface Antigen (HBsAg), Venereal Disease Research Laboratory (VDRL) for syphilis, and slide test for malaria were done. Data was collected from various registers within the blood centre. It was entered in MS Excel spreadsheets and calculation was done for calculating the prevalence. R-square value was used to determine the trend of TTIs. Results: A total of 19,864 donors were analysed for prevalence of TTIs from 2017 to 2020. Of these 99.16% were voluntary donors and 0.84% were replacement donors. Prevalence of TTI in total donors was 1.11%. Prevalence of hepatitis B was highest (0.84%) followed by syphilis (0.11%), HIV (0.07%) and HCV (0.09%) and malaria (0%). Prevalence was more in male replacement donors. Conclusion: The prevalence of TTI in total donors was 1.11%. The number of total blood donations has been on the rise. Cases of HCV were steadily declining, but HIV, HBsAg, and syphilis showed a non linear trend. Strict adherence to the donor selection rules is needed to supply quality and safe blood to the patient.

Trends and epidemiological analysis of hepatitis B virus, hepatitis C virus, human immunodeficiency virus, and human T-cell lymphotropic virus among Iranian blood donors: strategies for improving blood safety

BackgroundBlood transfusion is associated with potential risks of transfusion-transmitted infections (TTIs). Different strategies are needed to monitor blood safety and screen the donors’ efficacy, such as evaluation of the prevalence and trends of TTIs. This study was conducted to evaluate the prevalence and trends of TTIs, including hepatitis B virus (HBV), hepatitis C virus (HCV), human immunodeficiency virus (HIV), and human T-cell lymphotropic virus (HTLV 1/2), and the impact of the donors’ characteristics such as age, sex, and donor status on the prevalence of TTIs in blood donors in seven large provinces of Iran from 2010 to 2018.MethodsThis study was conducted on the data collected from all blood donations in seven Iranian Blood Transfusion Centers including Ardabil, Alborz, Guilan, West Azarbaijan, North, Razavi, and South Khorasan from April 2010 to March 2018. Demographic characteristics, number of donations, donor status, and screening and confirmatory serological results of all blood donations were collected from Iranian Blood Transfusion Organizations (IBTO) national database. The prevalence and trend of HBV, HCV, HIV, and HTLV 1/2 infections were reported according to the donation year and donor’s characteristics.ResultsThe analysis of the prevalence and trend of TTIs in 3,622,860 blood donors showed a significant decreasing trend in first-time and regular donors. Additionally, compared to first- time donors, regular donors made safer blood donations with lower risks of HBV, HIV, HCV and HTLV 1/2 (P < 0.0001). Although the prevalence of HTLV 1/2 and HBV was higher in females, TTIs had a significant decreasing trend in males and females. Finally, it was found that the prevalence of HBV and HTLV 1/2 increased with age up to 40–49 years and then decreased thereafter.ConclusionsThe decreasing trends of TTIs in Iranian donors during 9 years may indicate that the various strategies implemented by IBTO have been effective in recent years. Other factors such as a decrease in the prevalence of specific TTIs in the general population might have also contributed to these declines.

Seroprevalence and trends in transfusion transmitted infections in blood donors: a 6-year experience in a tertiary care hospital

Introduction: Blood transfusion continues to be a possible source of pathogens in transfused patients. The pattern of transfusion-transmitted infections (TTI) varies between regions. Knowledge of prevalence of TTIs is essential to monitor safety of blood transfusion by ensuring better practices of collection and processing of blood components. The aim of the study was to determine the prevalence of TTI in blood donors in a tertiary care hospital in central India. Materials and Methods: A retrospective cross sectional study was performed from 2012 to 2017 in a tertiary care government hospital in central region of India. The study analyzes all voluntary and replacement blood donations received through the hospital in its blood bank and blood donation camps organized by the hospital. Results:A total of 54,831 units of blood was collected over a period of 6 years. Prevalence of TTI in the blood donors was 1.45%. Prevalence of seroprevalence of human immunodeficiency virus (HIV), hepatitis B virus (HBV), hepatitis C virus (HCV), syphilis and malaria were 0.31 %, 0.75%, 0.065%, 0.22% and 0.1% respectively. Among all the potential donors, the most common TTI prevalence was that of Hepatitis B followed by HIV and syphilis. Conclusion:Public awareness, careful donor selection,vigilant screening and adoption of newer techniques for inactivation of pathogens are needed to ensure safety of blood products and their transfusion.The public outreach programs such as blood donation camps could be used as a way of spreading awareness to improve the safety and efficacy of a blood donation program.

NATSpert ID TripleH: A novel individual donor multiplex nucleic acid amplification test to reduce risk of transfusion-transmitted infections: Two-year experience of a blood bank in central India

Background: In India serological screening is mandatory for all donated blood for hepatitis B virus (HBV), hepatitis C virus (HCV) and human immunodeficiency virus (HIV) while Nucleic Acid Amplification Testing (NAT) is not a mandatory screening test. There is a risk of transfusion-transmitted infections (TTI) during window period using serology. Aims and Objectives: The aim of this study was to analyse NAT screening using an indigenously developed, Indian manufactured Individual Donor NAT (ID-NAT) and compare it with serology. Materials and Methods: All blood donations between June 2017-March 2019 were screened serologically for HBV, HCV and HIV at an Indian blood bank. Blood donations also underwent ID-NAT screening using the NATSpert ID TripleH detection assay based on real time PCR. The results were analysed to identify yield cases. Results: In the study, 30,772 blood donor samples were screened serologically out of which 214 were reactive. 30,558 serologically non-reactive blood donations and 77 randomly selected, serologically reactive blood donations were screened using NATSpert. Out of 30,635, 85 donor samples were reactive on the NATSpert which included 77 serology positive and 8 NAT yield cases. The NAT yield found was 2 each for HBV/HIV and 4 of HCV. Conclusion: The NATSpert ID TripleH offers a statistically significant advantage over EIA in ability to detect TTI in blood donors (P < 0.05, Fishers Exact Test). The NAT yield of 1:3829 was in line with other Indian studies. NATSpert assay will provide a significant improvement in blood safety and offer a cost benefit compared to the imported products.

Prevalence of transfusion transmissible infections in blood donors of Pakistan.

BackgroundTransfusion-transmitted infections threaten the safety of patients requiring blood transfusion, which in turn imposes serious challenges for the availability of safe blood products that are still affordable in health care systems with limited resources. The aim of the study was to determine the prevalence of transfusion-transmitted infections in blood donors and to evaluate the demographic characteristics of reactive and non-reactive blood donors.MethodsA prospective cohort study was conducted at our institute in Karachi, Pakistan. Donors were required to fill a detailed questionnaire and were screened for Hepatitis B, Hepatitis C, Human immunodeficiency viruses, Syphilis and Malaria by ELISA and thick film (malaria).ResultsOf the 16,602 blood donors, 16,557 were males and 45 females (mean age 28.6 ± 2). Nine hundred and seventy three (5.8%) donations were reactive in any screening assay, with 58 (0.35%) donations reacting in more than one assay. The prevalence of Hepatitis B, Hepatitis C, Human immunodeficiency viruses, Syphilis and Malaria was found to be 1.84, 1.7, 0.04, 2.1 and 0.07% respectively. Characteristics among the infections were evaluated and it was found that unmarried donors had a higher chance to be infected by Hepatitis B virus and Syphilis as compared to the other infections. On the other hand, construction workers and married donors were at more risk to be infected by Syphilis rather than the other infections. In case of co-infections, personnel with different occupations and marital status were infected by more than one pathogen.ConclusionA substantial percentage of the blood donor’s harbored transfusion-transmitted infections. Prevention of TTIs should be the main goal right now. There is a need for stringent selection of blood donors with the emphasis on getting voluntary donations and comprehensive screening of donor’s blood for TTIs using standard methods to ensure the safety of blood recipient.

Metagenomic profiling of the viromes of plasma collected from blood donors with elevated serum alanine aminotransferase levels.

In Japanese Red Cross (JRC) blood centers, blood collected from donors with serum alanine aminotransferase (ALT) levels of more than 60 U/L are disqualified even if serologically negative for transfusion-transmitted infections (TTIs). To assess potential risks of TTIs in plasma with elevated serum ALT levels in the current donor screening program of the JRC, we conducted a metagenomic analysis (MGA) of virome profiles in the plasma of blood donors with or without elevated serum ALT levels. Based on serum ALT levels, donors were classified into three groups: "high," more than 79 U/L; "middle," 61 to 79 U/L; and "low," less than 61 U/L. We individually analyzed 100 plasma samples from each group by MGA, employing shotgun sequencing. Viral sequences detected using MGA were partly confirmed using real-time polymerase chain reaction (PCR). Donors with high and middle ALT levels were significantly younger than those with low ALT levels, and more than 90% were males. Herpesviridae, Anelloviridae, Picornaviridae, and Flaviviridae sequences were identified in plasma samples, and their distribution and frequency were not significantly different among the three groups. The serum ALT test may be unsuitable for monitoring for additional risks of TTIs in blood donors who were negative for typical TTIs using serologic and nucleic acid tests. Although MGA is less sensitive than PCR, it remains the best technology to detect known viruses in these donors.
