Patellar Tracking in Total Knee Arthroplasty-Influence on Clinical and Functional Outcome.

Anterior knee pain is a common problem after primary total knee arthroplasty (TKA). The aim of this study was to find parameters in patellar positioning which influence the clinical and functional outcome after TKA. Included were 59 patients who underwent TKA, of which three patients were treated bilaterally (n = 62 included knees). In a periodical follow-up of up to 5 years, each patient had to answer three questionnaires (HSS, WOMAC, SF-36) and underwent three radiographies of the knee (including merchant view) and a clinical examination, including Range Of Motion (ROM). All radiographs were evaluated by a single observer blinded to clinical data, who collected multiple parameters of sagittal and axial patellar alignment including newly developed methods for measuring patellar shift and tilt. Depending on the measurement results, three groups were built for each parameter and the influence on the outcome was determined. A lateral patellar tilt of more than 4° resulted in lower scores for both the HSS and WOMAC. The rarely investigated patellar facet angle showed a significantly inferior clinical and functional outcome in late follow-up of >24 months if lower than 142°, possibly due to progressive osteosclerotic changes of the patella caused by increased contact stress with corresponding patellar morphology. No significant difference was found for all other parameters. The newly developed method for measuring patellar shift has proven to be a valuable and easy instrument in the postoperative setting.

Improving radiographic patello-femoral tracking in total knee arthroplasty with the use of a flexion spacer: a case-control study.

The purpose of this study was to compare the effectiveness of a flexion spacer in the clinical and radiological outcomes of patients who underwent total knee arthroplasty (TKA) and to compare these patients to a group of patients subjected to the same type of surgery but without the use of a flexion spacer. It was hypothesized that patients who underwent TKA using a flexion spacer would have better clinical and radiological outcomes than those without a flexion spacer in both short- and medium-term follow-ups. A consecutive series of patients undergoing TKA were included, yielding 20 patients in the study group. The control group was identified from the consultant database of the senior author, yielding 21 patients who underwent the same operation. All 41 patients received a Vanguard Knee System (Zimmer-Biomet, Warsaw, Indiana, USA). Cases were defined as those patients who had undergone TKA using a flexion spacer device for gap balancing; controls were defined as patients who had undergone TKA without the support of a flexion spacer device. Patients were clinically and radiographically evaluated at two consecutive follow-ups: T1-13.1 ± 1.3months and T2-108 ± 6months. Clinical evaluation was performed using the Knee Society Scoring System and the Western Ontario, McMaster Universities Osteoarthritis Index score. Radiographic evaluation included the femoral angle (α), the tibial angle (β), the sagittal femoral (γ) angle and the tibial slope (δ). Furthermore, the lateral patellofemoral angle (LPFA) and the Caton-Deschamps index were evaluated. No statistically significant clinical differences were found between the two groups at T1 and T2; moreover, the clinical outcomes of the two groups were stable between the two follow-ups, with no significant improvement or worsening. Radiographic evaluation showed no difference in the two groups between T1 and T2; the only significant radiographic difference between the two groups concerned the LPFA (both at 30° and 60°) at each follow-up, which was significantly greater in cases than in controls (p = 0.001). The current study demonstrates that the use of a flexion spacer significantly improves radiographic patello-femoral tracking, although no significant clinical differences were found between the two groups. Effect of three-dimensional printing guide plate on improving femoral rotational alignment and patellar tracking in total knee arthroplasty

To investigate the effect of three-dimensional (3D) printing guide plate on improving femoral rotational alignment and patellar tracking in total knee arthroplasty (TKA). Between January 2018 and October 2018, 60 patients (60 knees) with advanced knee osteoarthritis who received TKA and met the selection criteria were selected as the study subjects. Patients were randomly divided into two groups according to the random number table method, with 30 patients in each group. The TKA was done with the help of 3D printing guide plate in the guide group and following traditional procedure in the control group. There was no significant difference in gender, age, disease duration, side, and preoperative hip-knee-ankle angle (HKA), posterior condylar angle (PCA), patella transverse axis-femoral transepicondylar axis angle (PFA), Hospital for Special Surgery (HSS) score, and American Knee Society (AKS) score ( P>0.05). All incisions healed by first intention and no complications related to the operation occurred. All patients were followed up 10-12 months, with an average of 11 months. HSS score and AKS score of the two groups at 6 months after operation were significantly higher than those before operation ( P<0.05), but there was no significant difference between the two groups ( P>0.05). Postoperative X-ray films showed that the prosthesis was in good position, and no prosthesis loosening or sinking occurred during follow-up. HKA, PCA, and PFA significantly improved in the two groups at 10 months after operation compared with those before operation ( P<0.05). There was no significant difference in HKA at 10 months between the two groups ( t=1.031, P=0.307). PCA and PFA in the guide group were smaller than those in the control group ( P<0.05). Application of 3D printing guide plate in TKA can not only correct the deformity of the knee joint and alleviate the pain symptoms, but also achieve the goal of the accurate femoral rotation alignment and good patellar tracking.

Computed tomography confirmation of component rotation in nanosensor-balanced total knee arthroplasty.

Balanced gaps and proper rotation are felt to be essential for optimum range of motion, stability, and patellar tracking in total knee arthroplasty. The purpose of this study is to assess, using computed tomography, the rotation of femoral and tibial components in fresh-frozen human cadaver knees that have been balanced using nanosensor trials while also observing how this rotation affects measured compartment loads and requirement for ligament balancing adjustment. We found that minor degrees of rotational malalignment of the femur and tibia were common using standard instrumentation and measured resection technique. Quantitative balance and rotational congruence are aided by nanosensor guidance, and femoral malrotation of up to 8° does not appear to affect compartment loads significantly as long as rotational congruity is present.

Influence of the Rotational Alignment of the Femoral and Patellar Components on Patellar Tilt in Total Knee Arthroplasty.

PurposeThe purpose of this study was to analyze the influence of rotational alignment of the femoral and patellar components on patellar tilt after total knee arthroplasty (TKA).Materials and MethodsA total of 56 patients (76 knees) who underwent TKA using Advance Medial Pivot Knee system between May 2009 and April 2011 and were available for minimum 1-year follow-up were enrolled in this study. Whiteside's line and the transepicondylar line were used to determine the rotational alignment of the femoral component. Patella cut was aimed to be parallel to the anterior patellar cortex during surgery. Radiographic evaluation was performed using plain axial radiographs. The rotational alignment of the femoral component was measured as the angle between the anterior condylar axis and the surgical transepicondylar axis. The patellar resection angle was measured between the patellar resection axis and the anterior cortical line of the patella. Patellar tilt was evaluated to investigate the correlation with the rotation of the femoral component and patellar resection angle.ResultsThe mean rotation of the femoral component was 0.42°±3.18° of internal rotation. The mean patellar resection angle was 1.82°±3.44°, indicating medial overresection. The mean patellar tilt was 6.12°±4.31° of lateral tilt. The rotational angle of the femoral component showed a negative correlation with patellar tilt in the linear regression analysis (p=0.749), but the patellar resection angle showed a positive correlation with patellar tilt (p<0.001).ConclusionsAccurate patellar resection is recommended for proper patellar tracking in TKA.

The effect of femoral component design on patellar tracking in total knee arthroplasty: Genesis II prosthesis versus Vanguard prosthesis

The Genesis II knee system incorporates 3° of external rotation into the femoral component and the femoral component is implanted in neutral rotation to the femur. The purpose of this study was to compare patellar tracking of the Genesis II knee system with that of the Vanguard knee system, in which the femoral component is routinely implanted in a 3° externally rotated position to the posterior condylar axis (PCA) of the femur. One hundred consecutive knees scheduled to undergo total knee arthroplasty (TKA) were enrolled. Fifty knees underwent TKA with the posterior-stabilized (PS) Genesis II prosthesis and 50 knees underwent TKA with the PS Vanguard prosthesis. Rotation of the femoral component was calculated by measuring the acute angle between the transepicondylar axis (TEA) and the PCA on axial computed tomography (CT) images. The postoperative patellar tilt and displacement were compared between groups. The range of motion and Knee Society scores were also compared. Forty-eight knees in each group were followed up for 2 years. There was no difference in the angle between the PCA and the TEA on postoperative CT scans between the two designs. There was also no difference in patellar tracking between groups. Both the Genesis II and Vanguard knee systems showed good clinical results at 2 years postoperatively. The patellar tracking of the Genesis II prosthesis was comparable to that of the Vanguard prosthesis. Prospective cohort study, Level II.

Influence of tourniquet application on patellar tracking in total knee arthroplasty.

Objective To explore, the influence of tourniquet application on patellar tracking with using the surgical transepieondylar axis as distal femur alignment in total knee arthroplasty (TKA). Methods From December 2002 to August 2008, 349 cases (526 knees) of primary TKA were performed. There were 124 males and 225 females with an average age of 68 years from 33 to 84 years. A total of 387 knees had varus deformity and 94 had valgus deformity. All of prosthesis were rotating platform with posterior cruciate ligament retaining provided by Gemini MK Ⅱ. All patients underwent anterior medial parapatellar approach. The surgical transepieondylar axis had been principally used as the distal femur alignment. Patellar tracking was checked with no thumb test. For maltracking ones, we prefered to use a single retinacular closing stitch at the superior medial arthrotomy and deflate tourniquets. Patellar tracking was checked again. Finally, later-al retinaeular releases (LRR) had been performed to obtain optimizing patellar tracking. Results Intraoper-atively, no thumb test were positive in 138 knees, including patellar subluxation (vaigus deformity) in 12 knees and patellar inclining (74 of valgns deformity and 52 of varus deformity) in 126 knees. After using a single retinacular closing stitch at the superior medial arthrotomy and deflating tourniquets, no thumb test were positive in 40 knees, including patellar subluxation (valgus deformity) in 8 knees and patellar inclining (29 of valgns deformity) in 32 knees. The total rate of LRR was 7.6% (40/526), the rate of LRR in valgus deformity was 39.4% (37/94) and that in yarns deformity and no deformity was 0.7% (3/432). Conclusion There was significant influence of tourniquet application on patellar tracking during the TKA. We were not falsely correcting patellar tracking problems with using a single retinaeular closing stitch at the superior me-dial arthrotomy anti deflating tourniquets intraoperatively and reducing the rate of LRR. Background Optimizing patellar tracking in total knee arthroplasty is a surgical priority. Despite this, a comparison of the effects of different component placements on patellar tracking is not available; the biomechanical impact of the patellar resection angle has not been studied; and the similarity between intraoperative and postoperative effects, fundamental to improving patellar tracking, is unknown. Our objective was to compare the impact of the major controllable femoral, tibial and patellar component positions on patellar kinematics during both passive and loaded flexion. Methods We tested eight cadaveric knee specimens in two rigs, simulating intraoperative and weightbearing flexion. Optoelectronic marker arrays were attached to the femur, tibia and patella to record kinematics throughout the range of motion. We modified posterior-stabilized fixed-bearing knee components to allow for five types of variations in component placement in addition to the neutral position: femoral component rotation, tibial component rotation, patellar resection angle, patellar component medialization and additional patellar thickness, for a total of 11 individual variations. Findings The major determinants of patellar tilt and shift were patellar component medialization, patellar resection angle and femoral component rotation. The relative order of these variables depended on the structure (bone or component), kinematic parameter (tilt or shift) and flexion angle (early or late flexion). Effects of component changes were consistent between the intraoperative and weightbearing rigs. Interpretation To improve patellar tracking, and thereby the clinical outcome, surgeons should focus on patellar component medialization, patellar resection angle and femoral component rotation. These have been linked with anterior knee pain as well. Neither tibial component rotation nor patellar thickness should be adjusted to improve patellar tracking.

A Technique of Staged Lateral Release to Correct Patellar Tracking in Total Knee Arthroplasty

Optimal patellar tracking and component alignment are important in achieving a well-functioning total knee arthroplasty (TKA). The patella is constrained partly by design of the prosthetic trochlear groove, and patellar tracking is governed by a combination of static and dynamic factors. Maltracking may result from excessive or unbalanced tension in the surrounding soft tissues. This article describes a staged progressive lateral release of the patellar retinaculum in TKA, which is classified into 6 stages. Stage 1 transects the deep lateral patellofemoral ligament; stages 2 to 6 extend the lateral patellar incision distally from vastus lateralis to the tibial tubercle. This technique was used in a series of 96 primary TKAs. We report the rates of the various stages of lateral release and the variables that might affect the decision to perform such a release.

The Effect of Femoral Component Rotation on Patellar Tracking in Total Knee Arthroplasty

Rotational adjustment of the femoral component in total knee arthroplasty influences patellar tracking. Sixty patients underwent TKA; the femoral component was placed parallel to the epicondylar axis in 30 patients and the femoral component was placed in 3 degrees external rotation to the posterior condylar axis in 30 patients. The epicondylar axis was identified using an image-guided navigation system. Mean patellar shift was 2.65 degrees for the computer assisted group and 3.50 degrees for the control group. Mean patellar tilt was 4.88 degrees for the computer assisted group and 6.68 degrees for the control group. Aligning the femoral component parallel to the epicondylar axis leads to significantly better patellar tracking.

The Effect of Patellar Thickness on Intraoperative Knee Flexion and Patellar Tracking in Total Knee Arthroplasty

We designed a simple IRB-approved study to evaluate the intraoperative effect of patellar thickness on knee flexion and patellar tracking during total knee arthroplasty. Routine PCL-retaining total knee arthroplasty was performed in 31 consecutive knees. With clinical balance and congruent patella tracking established, custom trial patellar components that were thicker than the standard trial by 2-mm increments (2-8 mm) were sequentially placed and trialed. Passive flexion was recorded to the nearest 5° with an intraoperative goniometer and gross mechanics of patellofemoral tracking were visually assessed. On average, passive knee flexion decreased 3° for every 2-mm increment of patellar thickness. Furthermore, for the knee system used in this study, increased patellar thickness had no gross effect on patellar subluxation or tilt.

The effect of surgical approaches and tourniquet application on patellofemoral tracking in total knee arthroplasty

The purpose of this study was to assess the influences of surgical approaches and tourniquet application on the lateral retinacular tension (LRT). Single-setting bilateral total knee arthroplasty was performed in 10 patients, and the medial parapatellar and midvastus approaches were randomly performed on each knee using tourniquet application. The LRT was measured using the buckle transducer before and after each approach. Next, the tourniquet was deflated and the measurement was repeated. After the parapatellar approach, LRT was significantly decreased. Conversely, no significant change was seen after the midvastus approach with the tourniquet inflated. However, in the midvastus approach, LRT was significantly decreased after tourniquet deflation. We concluded that both the parapatellar and midvastus approaches influence patellar tracking and LRT. © 2003 Elsevier Inc. All rights reserved.

The influence of the position of the patellar component on tracking in total knee arthroplasty.

The position of the patellar component and patellofemoral symptoms were assessed in a series of 72 cemented polyethylene patellar components in total condylar arthroplasties. A small (25 mm) component was used. The minimum follow up was 2 years and maximum 3 years. A lateral retinacular release was carried out in 22 knees. Clinical results were assessed by questionnaire and 86% had no patellofemoral symptoms. The mean postoperative patellar height was not changed compared to the preoperative height. Lateral placement of the patellar component was associated with both increased medial tilt and lateral subluxation. The component should be inserted so that its centre is slightly medial to the centre of the bone.

The Effects of Axial Rotational Alignment of the Femoral Component on Knee Stability and Patellar Tracking in Total Knee Arthroplasty Demonstrated on Autopsy Specimens

Four fresh-frozen anatomic knee specimens were tested for knee stability, patellar tracking, and patellofemoral contact points with the femoral component positioned in 5 degrees internal, 5 degrees external, or neutral axial rotational alignment of the femoral component referenced on the posterior femoral condyles. The externally rotated specimens had varus-valgus stability of the knee that was closest to the normal control. The internally rotated specimens shifted into valgus alignment with flexion. Patellar tracking also was closest to normal in the externally rotated specimens. Patellofemoral contact was more evenly distributed between the medial and lateral contact areas in the externally rotated specimens than in the internally rotated or in the neutral specimens. Internal rotation of the femoral component in the knee with perpendicular resection of the tibia causes undesirable changes in knee stability, patellar tracking, and patellofemoral contact points. Neutral positioning produces similar but less negative effects on knee stability and patellar kinematics. External rotation improves both patellar tracking and knee stability characteristics.

The Effect of Patellar Button Placement and Femoral Component Design on Patellar Tracking in Total Knee Arthroplasty

The effects of patellar button position and femoral component design on patellar tracking were investigated roentgenographically and arthroscopically using fresh-frozen adult anatomic specimen knees. Patellar tracking was evaluated for medial/lateral position and tilting angle deviation by comparing preoperative normal values to total knee replacement performance with the following variables: (1) femoral component design; symmetrical total condylar type with a 3-mm central patellar groove and a component with a 3-mm raised lateral patellar flange and 1-mm deepened patellar groove; and (2) patellar button placement; central insertion; and 10-mm medialized insertion. In the roentgenographic study, medialized position of the patellar button allowed the bony portion of the patella to assume its normal lateral position and tilt throughout the knee range of motion. A combination of medialized position of the patellar button and deepening of the patellar groove provided the most anatomically correct position and the most normal tilting of the bony structure of the patella. In the arthroscopic study, the centralized patellar position caused significant lateral tracking and subluxation relative to the femoral component, whereas the medialized position eliminated this tracking disorder. Deepening the patellar groove minimized the intraarticular tracking abnormality and constrained the patellar button in the patellar groove. These results show that the position of the patellar button and femoral component design exert a major influence on patellar tracking.
