The use of verapamil for the prevention and treatment of hypertensive complications of pregnancy and its effect on cardiohemodynamic indexes

A study is performed of the efficacy of verapamil for the prevention and treatment of late toxemia of pregnancy. The preparation is shown both to produce a therapeutic effect in developed pathology and to prevent the development of severe forms of hypertensive complications in women with high-risk pregnancies

Functional state of the pituitary-thyroid and pituitary-adrenal systems in normal pregnancy and pregnancy with late toxemia.

1. The blood ACTH and T3 levels are low in women with late toxemia of pregnancy; the blood cortisol, aldosterone, and T4 levels and the thyroxine-binding capacity are raised. 2. The concentrations of TSH, STH, and catecholamines in blood from the umbilical cord of newborn infants whose mothers had late toxemia of pregnancy are raised. 3. Changes in concentrations of pituitary, thyroid, and adrenal hormones during the normal course of pregnancy and late toxemia reflect protective and adaptive responses of the mother aimed at maintaining homeostasis.

In 200 healthy, normotensive nulliparous women, a single determination of serum uric acid concentration was done between weeks 28-32 of gestation in order to identify a possible increased risk of developing hypertensive complications. If serum concentrations higher than 3.6 mg/dl were considered as increased ("positive test"), women who developed toxaemia of late pregnancy (proteinuric hypertension), had a significantly elevated mean serum uric acid concentration already at the beginning of the third trimester (p less than 0.01). The incidence of toxaemia and hypertensive disease without proteinuria was significantly higher in the group of women with an elevated uric acid value (p less than 0.001). Only 9% of pregnant women with a "negative test" ultimately developed a mild form of a hypertensive complication. Conversely, 74% of the patients with a "positive test" remained normotensive. Thus, the predictive value of a "positive test" is low (26%) and that of a "negative test" relatively high (91%).

Urography and renography in the follow-up of patients with toxemia of late pregnancy.

We have studied 129 patients with toxemia of pregnancy two months post partum by urography and renography. The same investigations were performed on 17 healthy parturients post partum. 49 patients (38.8%) had an abnormality either in urography or renography. 20 patients (15.5%) had an abnormal urogram and 42 (32.6%) an abnormal renogram. The findings correlated so that 7 patients had an abnormal urogram with a normal renogram and 13 patients had an abnormality in both investigations. 29 patients had only an abnormal renogram. In the control series 3 patients had an abnormal urogram and the same patients also had an abnormal renogram. We divided the studied patients into groups according to the clinical severity of toxemia. Urographic and renographic findings were divided evenly in the series so that abnormalities were no more common in the severe cases. Renography cannot be considered merely as a screening method when compared with urography in the follow-up of toxemic patients, but these investigations are complementary.

Renal angiography in the follow-up examination of toxaemia of late pregnancy.

A series of 35 patients with toxaemia of pregnancy is presented. These patients were subjected to renal angiography in order to diagnose any renal artery changes. The patients formed part of a series of 139 toxaemic patients, all of whom had been followed up by intravenous pyelography with normal renal findings. One patient showed a unilateral obstruction of the main branch of the renal artery, obviously of importance. Another patient was found to have a saccular aneurysm in the abdominal aorta. These changes were recorded in the group of patients with a history of hypertension preceding pregnancy, and with superimposed toxaemia of late pregnancy. The groups with pure preeclampsia showed no pathological findings, and renal angiography in these groups shed no additional light on the aetiology of toxaemia.

Fetal head growth measured by ultrasound in the last few weeks of pregnancy in normal, toxaemic and diabetic women.

The authors measured the fetal biparietal diameter by an ultrasound technique in 41 normally pregnant women in the 32nd, 36th, 38th and 40th weeks of gestation and found that the diameter increased by an average of 1.8 mm per week. The growth rate decreased as the calculated term approached. In addition, the weekly increase in biparietal diameter was observed in 22 patients in hospital because of toxaemia of late pregnancy and in six pregnant diabetics. Since no significant deviations from normal growth could be shown in these groups of patients, the determination of the fetal biparietal diameter was considered of value for the estimation of the “true” fetal maturity in toxaemia of late pregnancy and in diabetic pregnancy.

Volume of whole blood and absolute amount of serum proteins in the early stage of late toxemia of pregnancy

In late toxemia of pregnancy the volume of the whole blood and the absolute amount of the serum proteins decreases not only in the developed clinical picture of illness, but also even before any clinical symptoms occur. In these potential patients the volume of the whole blood was lower by about 495 ml. and the absolute amount of the serum proteins about 23.60 Gm. than in physiologic pregnancy. These findings at the beginning of late toxemia of pregnancy may serve as an “early indicator” and may also help in the prevention of this disease.

Urinary dehydroepiandrosterone in late toxemia of pregnancy

Reduced average urinary excretion of dehydroepiandrosterone (DHEA) in 24 hours was ascertained in 27 cases of late toxemia of pregnancy and 19 normally pregnant women in the ninth and tenth lunar month, as compared with a control group of II normal nonpregnant women of the same age group (20 to 34 years). The lowest excretion rate was found in late toxemia of pregnancy with a total absence of DHEA in 92 per cent of the cases of this group. The excretion of DHEA did not depend on the average excretion of total 17-ketosteroids in urine. A statistically highly significant indirect dependence was, however, found between the average excretion of DHEA and the average level of serum β-lipoproteins. Values for the latter are highest in late toxemia of pregnancy. In explanation of this dependence, it is therefore proposed that DHEA affects the level of serum β-lipoproteins. According to this hypothesis, the DHEA, being an inhibitor of one dehydrogenase of glucose-6-phosphate, regulates the acivity of the pentose cycle and thereby the synthesis of lipids, including β-lipoproteins.

Metabolic Toxemia of Late Pregnancy: A Disease of Malnutrition

In some ways it is reassuring to learn from Brewer that the day of the pamphleteer is not yet over. For example, we look back with pleasure to the raging arguments over obstetrical anesthesia since we know that the heat of the invective hastened the spread of medical information. But the book is otherwise disturbing. Brewer has concluded that the acute toxemia of pregnancy is a disease of malnutrition which leads to limitations of hepatic conjugation of injurious substances, some of which derive from bacterial decompositions in the bowel and some of which are steroids produced by the placenta. The clinical manifestations are presumably results of the impact of these insults. It follows that all other etiologic hypotheses are wrong and must be destroyed. Therapy, other than lifesaving procedures in the terminal phases of the disease, preeminently delivery, consists of administration of a high protein diet and bowel antiseptics. All

Good Prenatal Nutrition

Toxemia of late pregnancy has virtually disappeared in areas where pregnant women receive good prenatal nutrition. The disease is more severe when protein intake is very low; dietary salt restriction and administration of saluretic diuretics have no significant effect in preventing toxemia. A weekly lecture emphasizing good nutrition and presented to patients on their initial visit to a prenatal clinic has been effective in preventing toxemia.
