Deep Learning-Based High-Resolution Magnetic Resonance Angiography (MRA) Generation Model for 4D Time-Resolved Angiography with Interleaved Stochastic Trajectories (TWIST) MRA in Fast Stroke Imaging.

The purpose of this study is to improve the qualitative and quantitative image quality of the time-resolved angiography with interleaved stochastic trajectories technique (4D-TWIST-MRA) using deep neural network (DNN)-based MR image reconstruction software. A total of 520 consecutive patients underwent 4D-TWIST-MRA for ischemic stroke or intracranial vessel stenosis evaluation. Four-dimensional DNN-reconstructed MRA (4D-DNR) was generated using commercially available software (SwiftMR v.3.0.0.0, AIRS Medical, Seoul, Republic of Korea). Among those evaluated, 397 (76.3%) patients received concurrent time-of-flight MRA (TOF-MRA) to compare the signal-to-noise ratio (SNR), image quality, noise, sharpness, vascular conspicuity, and degree of venous contamination with a 5-point Likert scale. Two radiologists independently evaluated the detection rate of intracranial aneurysm in TOF-MRA, 4D-TWIST-MRA, and 4D-DNR in separate sessions. The other 123 (23.7%) patients received 4D-TWIST-MRA due to a suspicion of acute ischemic stroke. The confidence level and decision time for large vessel occlusion were evaluated in these patients. In qualitative analysis, 4D-DNR demonstrated better overall image quality, sharpness, vascular conspicuity, and noise reduction compared to 4D-TWIST-MRA. Moreover, 4D-DNR exhibited a higher SNR than 4D-TWIST-MRA. The venous contamination and aneurysm detection rates were not significantly different between the two MRA images. When compared to TOF-MRA, 4D-CE-MRA underestimated the aneurysm size (2.66 ± 0.51 vs. 1.75 ± 0.62, p = 0.029); however, 4D-DNR showed no significant difference in size compared to TOF-MRA (2.66 ± 0.51 vs. 2.10 ± 0.41, p = 0.327). In the diagnosis of large vessel occlusion, 4D-DNR showed a better confidence level and shorter decision time than 4D-TWIST-MRA. DNN reconstruction may improve the qualitative and quantitative image quality of 4D-TWIST-MRA, and also enhance diagnostic performance for intracranial aneurysm and large vessel occlusion.

Ex-Vivo MRI of the Normal Human Placenta: Structural-Functional Interplay and the Association With Birth Weight.

Advanced magnetic resonance imaging (MRI) methods are increasingly being used to assess the human placenta. Yet, the structure-function interplay in normal placentas and their associations with pregnancy risks are not fully understood. To characterize the normal human placental structure (volume and umbilical cord centricity index (CI)) and function (perfusion) ex-vivo using MRI, to assess their association with birth weight (BW), and identify imaging-markers for placentas at risk for dysfunction. Prospective. Twenty normal term ex-vivo placentas. 3 T/ T1 and T2 weighted (T1 W, T2 W) turbo spin-echo, three-dimensional susceptibility-weighted image, and time-resolved angiography with interleaved stochastic trajectories (TWIST), during passage of a contrast agent using MRI compatible perfusion system that mimics placental flow. Placental volume and CI were manually extracted from the T1 W images by a fetal-placental MRI scientist (D.L., 7 years of experience). Perfusion maps including bolus arrival-time and full-width at half maximum were calculated from the TWIST data. Mean values, entropy, and asymmetries were calculated from each perfusion map, relating to both the whole placenta and volumes of interest (VOIs) within the umbilical cord and its daughter blood vessels. Pearson correlations with correction for multiple comparisons using false discovery rate were performed between structural and functional parameters, and with BW, with P < 0.05 considered significant. All placentas were successfully perfused and scanned. Significant correlations were found between whole placenta and VOIs perfusion parameters (mean R=0.76 ± 0.06, range=0.67-0.89), which were also significantly correlated with CI (mean R=0.72 ± 0.05, range=0.65-0.79). BW was correlated with placental volume (R=0.62), but not with CI (P=0.40). BW was also correlated with local perfusion asymmetry (R=-0.71). Results demonstrate a gradient of placental function, associated with CI and suggest several ex-vivo imaging-markers that might indicate an increased risk for placental dysfunction. 1 TECHNICAL EFFICACY: Stage 1.

Dynamic contrast-enhanced magnetic resonance lymphangiography in pediatric patients with central lymphatic system disorders.

Central conducting lymphatics (CCLs) disorders represent a broad spectrum of clinical entities ranging from self-limiting traumatic leaks treated by conservative strategies, to complex lymphatic circulation abnormalities that are progressive and unresponsive to currently available treatments. Dynamic contrast-enhanced magnetic resonance lymphangiography (DCMRL) performed by intranodal injection of gadolinium-based contrast material is a recently developed technique which allows a minimally invasive evaluation of the CCL abnormalities providing a dynamic assessment of lymph flow and its pathways. In our institution, DCMRL is performed after bilateral cannulation of inguinal lymph nodes, using a MR protocol which includes volumetric 3D T2-SPACE (sampling perfection with application-optimized contrasts using different flip-angle evolution) and free-breathing respiratory navigated sequence and TWIST (time-resolved angiography with Interleaved stochastic trajectories) MR angiography sequence, during intranodal injection of paramagnetic contrast medium. Although DCMRL applications in clinical practice are still improving, a minimally invasive assessment of lymphatic pathways is particularly important both in pediatric patients with primitive lymphatic system disorders and in children with complex congenital heart disease associated with CCL impairment.

Abstract No. 698 ■ FEATURED ABSTRACT Role of Time-resolved angiography With Interleaved Stochastic Trajectories (TWIST) magnetic resonance angiography for treatment planning in Nonspecific aorto-arteritis: a prospective study

Nonspecific aorto-arteritis (NSAA) is a rare, chronic granulomatous panarteritis affecting primarily aorta and its branches. DSA is gold standard for the assessment of vascular involvement in these patients. Recent advances in MR Angiography such as Time-resolved angiography With Interleaved Stochastic Trajectories (TWIST) sequence can noninvasively assess the vascular involvement. We conducted a prospective study to evaluate the role of TWIST MR angiography for treatment planning in NSAA patients. Over the period between 2017 and 2019, 17 patients with NSAA were included in the study. Contrast-enhanced MRA using TWIST and diagnostic digital subtraction angiography (DSA) was performed in all the 17 patients. The average age of our patient cohort was 27.13 years and majority were females (11 were females and 6 patients were males). 17 patients underwent both MRA using TWIST sequence and DSA. Data from both MRA and DSA findings were compared, considering DSA is gold standard for the evaluation of arterial system. Common vessel involved are right renal artery (82.35%) followed by abdominal aorta (70.6%) followed by left renal artery (64.7%). Left subclavian artery was involved in 10 (58.8%) patients and right subclavian artery in 6 (35.3%) patients. In all the patients, the involved vessels were correctly identified in both TWIST angiography images and DSA. The sensitivity and specificity of TWIST MRA is 100% as compared to DSA in the detection of lesions involving Aorta, arch, arch vessels, celiac artery, SMA. However, the sensitivity and specificity of TWIST MR angiography in the assessment of renal arteries are 100% and 71.4% respectively. However, the sensitivity and specificity of VA lesion evaluation in 85.7% and 33.3%, respectively. TWIST MRA can be effective for the evaluation of vascular involvement in treatment planning in NSAA patients. TWIST MRA will be useful in those patients where breath holding is an issue or in uncooperative patients. It provides dynamic assessment of vascular system noninvasively. To the best of our knowledge, there is no study in the literature, which used TWIST MRA in the vascular evaluation in the patients of NSAA.

Contrast circulation in adult fontan patients using MR Time Resolved Angiography: Application for CT pulmonary angiography

BackgroundWhen patients with Fontan circulation require a computed tomographic pulmonary angiogram (CTPA), there are significant challenges in achieving adequate contrast opacification due to the altered anatomical connections. This study used Time Resolved Angiography with Interleaved Stochastic Trajectories (TWIST) Magnetic Resonance Angiography (MRA) to examine contrast circulation in a cohort of patients with Fontan circulation who were having routine MRI follow up to inform the contrast timing of any subsequent CT. MethodsThis is a single centre, cross-sectional, observational, retrospective study. The time to peak (TTP) signal intensity from the MRA was recorded using regions of interest on the aorta, pulmonary arteries, cavae and Fontan conduit. Patients were grouped by ejection fraction, global longitudinal strain, indexed stroke volume and cardiac index to examine if these cardiac performance parameters affected the mean TTP. Statistical analysis was performed to find the mean TTP for each of the vessels, which was consequently compared between the different cardiac performance parameters. Results35 patients were included in the study. Mean TTP contrast enhancement was 31s in the thoracic aorta, 46s in the right pulmonary artery, 41s in the left pulmonary artery and 55s in the Fontan conduit. Cardiac performance shows no statistically significant relationship to the peak contrast enhancement whether measured by ejection fraction, global longitudinal strain, stroke volume index or cardiac index. ConclusionThe mean optimal timing for a single-phase examination of the Fontan circulation, following an upper limb injection, was 55 s following start of contrast injection irrespective of cardiac performance. In TWIST MRA, the IV bolus is 4–5 s duration. A longer bolus is required for CTA, around 20s, suggesting an additional delay will be required. We propose that an optimal single phase CTPA to be protocolled at 70 s following the start of contrast injection, assuming adequate iodinated contrast dose.

Evaluation of image quality of pituitary dynamic contrast-enhanced MRI using time-resolved angiography with interleaved stochastic trajectories (TWIST) and iterative reconstruction TWIST (IT-TWIST).

Time-resolved angiography with interleaved stochastic trajectories (TWIST) is a keyhole imaging with frequent sampling of central k-space data and view-sharing for the peripheral k-space of dynamic phases. IT-TWIST is a technique to reconstruct images with a smaller temporal footprint using the same raw data obtained with TWIST by iterative reconstruction. To compare image quality between TWIST and IT-TWIST in 3D pituitary DCE-MRI. Retrospective observation study. Fifty-one patients (23 men, 28 women) who underwent 3D pituitary DCE-MRI using TWIST between July 2016 and April 2017. 3T/TWIST and IT-TWIST. Visual evaluation was conducted for image quality of delineation of the pituitary stalk and posterior lobe during the early arterial phase, cerebral white matter near the sella turcica, and the mass lesion. Bolus sharpness of the pituitary stalk, posterior lobe, and bilateral cavernous sinus was evaluated on the enhancement slope map calculated from TWIST and IT-TWIST. Temporal stability of intensity of the nonenhanced area was evaluated on temporal standard deviation (SD) maps calculated from TWIST and IT-TWIST. Paired t-test or Wilcoxon rank-sum test was used to test the differences between TWIST and IT-TWIST in both visual evaluation and region of interest evaluation. Scores of visual evaluations for IT-TWIST were significantly better than those for TWIST (P < 0.001) in all items. Enhancement slope for IT-TWIST was significantly higher than that for TWIST in posterior lobe, and right and left cavernous sinus (P < 0.001). Temporal SD for IT-TWIST was significantly lower than that for TWIST in all items, with statistical significance (P < 0.001). IT-TWIST yielded better visualization, and better enhancement slope, and less temporal SD compared with TWIST in 3D pituitary DCE-MRI. Evidence Level: 4 Technical Efficacy: Stage 1 J. Magn. Reson. Time‐resolved three‐dimensional technique for dynamic magnetic resonance dacryocystography

This study introduces a new technique in the assessment of nasolacrimal drainage with magnetic resonance imaging. To describe a new dynamic magnetic resonance dacryocystography (MRDCG) protocol and report normative findings on asymptomatic lacrimal drainage systems. A prospective observational study at the Royal Adelaide Hospital. Seventeen patients with unilateral epiphora were prospectively recruited. Patients underwent dynamic MRDCG. The time-resolved angiography with interleaved stochastic trajectories (TWIST) sequence was used to visualise contrast flow through the lacrimal drainage system. Tear transit times and anatomical dimensions were measured. Dynamic MRDCG with TWIST sequence reliably demonstrated contrast flow. Tear transit times and anatomical dimensions were comparable to previously reported values in other imaging modalities. Dynamic MRDCG provides good structural and temporal resolution to the assessment of lacrimal outflow obstructions. The physiological nature of this modality allows potential assessment for functional obstruction and future research on tear flow assessment.

Subtraction of time-resolved magnetic resonance angiography images improves visualization of the pulmonary veins and left atrium in adults with congenital heart disease: a novel post-processing technique.

To describe a novel time-resolved magnetic resonance angiography (TR-MRA) postprocessing technique using the time-resolved angiography with interleaved stochastic trajectories (TWIST) method to evaluate the pulmonary veins and left atrium in adults with congenital heart disease undergoing cardiac MRI. Institutional ethics committee approved the study. 21 consecutive adult patients (14 female, 7 male patients, mean age 28years) with known congenital heart disease who underwent a cardiac MRI were included. Post-processing of the TR-MRA sequences created novel "subtracted" datasets. Two independent observers reviewed the conventional TWIST and novel subtracted TWIST data sets in source and maximum intensity projection (MIP) coronal reformats to assess visualization of the pulmonary veins and left atrium based on a 5-point scale. Quantitative signal to noise (SNR) comparison was performed. TR-MRA yielded diagnostic image data in 20/21 patients (95.2%). The novel "subtracted" TR-MRA technique improved visualization of the pulmonary veins and left atrium compared to the source TR-MRA sequence in 16/20 patients (mean scores 3.34 ± 0.69 vs. 2.92 ± 0.69, p < 0.008). Further improved visualization of the pulmonary veins and left atrium was observed in the subtracted MIP TWIST sequences compared to the MIP TWIST images (mean scores 4.43 ± 0.80 vs. 3.02 ± 0.87 vs., p < 0.001). No significant SNR difference between the source and novel subtracted group was observed (85.4 vs. 70.4, p = 0.57). Compared to source TR-MRA images, subtraction of TR-MRA images is a novel postprocessing technique that improves visualization of the pulmonary veins and left atrium in a substantial number of patients.

Capturing Bone Signal in MRI of Pelvis, as a Large FOV Region, Using TWIST Sequence and Generating a 5-Class Attenuation Map for Prostate PET/MRI Imaging.

Purpose:Prostate imaging is a major application of hybrid positron emission tomography/magnetic resonance imaging (PET/MRI). Currently, MRI-based attenuation correction (MRAC) for whole-body PET/MRI in which the bony structures are ignored is the main obstacle to successful implementation of the hybrid modality in the clinical work flow. Ultrashort echo time sequence captures bone signal but needs specific hardware–software and is challenging in large field of view (FOV) regions, such as pelvis. The main aims of the work are (1) to capture a part of the bone signal in pelvis using short echo time (STE) imaging based on time-resolved angiography with interleaved stochastic trajectories (TWIST) sequence and (2) to consider the bone in pelvis attenuation map (µ-map) to MRAC for PET/MRI systems.Procedures:Time-resolved angiography with interleaved stochastic trajectories, which is routinely used for MR angiography with high temporal and spatial resolution, was employed for fast/STE MR imaging. Data acquisition was performed in a TE of 0.88 milliseconds (STE) and 4.86 milliseconds (long echo time [LTE]) in pelvis region. Region of interest (ROI)-based analysis was used for comparing the signal-to-noise ratio (SNR) of cortical bone in STE and LTE images. A hybrid segmentation protocol, which is comprised of image subtraction, a Fuzzy-based segmentation, and a dedicated morphologic operation, was used for generating a 5-class µ-map consisting of cortical bone, air cavity, fat, soft tissue, and background (µ-mapMR-5c). A MR-based 4-class µ-map (µ-mapMR-4c) that considered soft tissue rather than bone was generated. As such, a bilinear (µ-mapCT-ref), 5 (µ-mapCT-5c), and 4 class µ-map (µ-mapCT-4c) based on computed tomography (CT) images were generated. Finally, simulated PET data were corrected using µ-mapMR-5c (PET-MRAC5c), µ-mapMR-4c (PET-MRAC4c), µ-mapCT-5c (PET-CTAC5c), and µ-mapCT-ref (PET-CTAC).Results:The ratio of SNRbone to SNRair cavity in LTE images was 0.8, this factor was increased to 4.4 in STE images. The Dice, Sensitivity, and Accuracy metrics for bone segmentation in proposed method were 72.4% ± 5.5%, 69.6% ± 7.5%, and 96.5% ± 3.5%, respectively, where the segmented CT served as reference. The mean relative error in bone regions in the simulated PET images were −13.98% ± 15%, −35.59% ± 15.41%, and 1.81% ± 12.2%, respectively, in PET-MRAC5c, PET-MRAC4c, and PET-CTAC5c where PET-CTAC served as the reference. Despite poor correlation in the joint histogram of µ-mapMR-4c versus µ-mapCT-5c (R2 > 0.78) and PET-MRAC4c versus PET-CTAC5c (R2 = 0.83), high correlations were observed in µ-mapMR-5c versus µ-mapCT-5c (R2 > 0.94) and PET-MRAC5c versus PET-CTAC5c (R2 > 0.96).Conclusions:According to the SNRSTE, pelvic bone, the cortical bone can be separate from air cavity in STE imaging based on TWIST sequence. The proposed method generated an MRI-based µ-map containing bone and air cavity that led to more accurate tracer uptake estimation than MRAC4c. Uptake estimation in hybrid PET/MRI can be improved by employing the proposed method.

Application of TWIST MR angiography to aid successful central venous access in challenging patients: initial single-centre experience

To investigate whether time-resolved angiography with interleaved stochastic trajectories (TWIST) with GeneRalised Autocalibrating Partially Parallel Acquisitions (GRAPPA) parallel acquisition could be used successfully to non-invasively and efficiently image patients with more complex vascular access issues. TWIST magnetic resonance angiography (MRA) in the GRAPPA algorithm was performed on 15 patients at our centre using the 1.5 T Siemens Magnetom Avanto MRI system. Images were interpreted by cardiac radiologists. TWIST provided excellent dynamic imaging of the venous system, demonstrating venous occlusion, stenoses, and collaterals, as well as providing good anatomical detail. TWIST MRA enables successful identification of candidate sites for central/tunnelled line access, whilst diagnosing complications of long-term access such as venous thrombosis or congenital venous anomalies.

Contrast-enhanced time-resolved 4D MRA of congenital heart and vessel anomalies: image quality and diagnostic value compared with 3D MRA

To evaluate time-resolved interleaved stochastic trajectories (TWIST) contrast-enhanced 4D magnetic resonance angiography (MRA) and compare it with 3D FLASH MRA in patients with congenital heart and vessel anomalies. Twenty-six patients with congenital heart and vessel anomalies underwent contrast-enhanced MRA with both 3D FLASH and 4D TWIST MRA. Images were subjectively evaluated regarding total image quality, artefacts, diagnostic value and added diagnostic value of 4D dynamic imaging. Quantitative comparison included signal-to-noise ratio (SNR), contrast-to-noise ratio (CNR) and vessel sharpness measurements. Three-dimensional FLASH MRA was judged to be significantly better in terms of image quality (4.0 ± 0.6 vs 3.4 ± 0.6, P < 0.05) and artefacts (3.8 ± 0.4 vs 3.3 ± 0.5, P < 0.05); no difference in diagnostic value was found (4.2 ± 0.4 vs 4.0 ± 0.4); important additional functional information was found in 21/26 patients. SNR and CNR were higher in the pulmonary trunk in 4D TWIST, but slightly higher in the systemic arteries in 3D FLASH. No difference in vessel sharpness delineation was found. Although image quality was inferior compared with 3D FLASH MRA, 4D TWIST MRA yields robust images and added diagnostic value through dynamic acquisition was found. Thus, 4D TWIST MRA is an attractive alternative to 3D FLASH MRA. • New magnetic resonance angiography (MRA) techniques are increasingly introduced for congenital cardiovascular problems. • Time-resolved angiography with interleaved stochastic trajectories (TWIST) is an example. • Four-dimensional TWIST MRA provided inferior image quality compared to 3D FLASH MRA but without significant difference in vessel sharpness. • Four-dimensional TWIST MRA gave added diagnostic value.

Contrast Dose, Temporal Footprint, and Spatial Resolution Tradeoffs in Dynamic Contrast-Enhanced MRA Performed in a Porcine Model of a Carotid Aneurysm

To evaluate the tradeoffs between temporal and spatial resolution and contrast dosing in dynamic contrast-enhanced magnetic resonance angiography (CE-MRA). Bilateral carotid artery aneurysms were created in a swine model. Dynamic CE-MRA using 1 mol/L gadobutrol was performed at 3 T, with high temporal (high-temp), middle temporal (mid-temp), and low temporal (low-temp) resolutions. High temporal CE-MRA was performed twice using 1 mL and 2 mL gadobutrol (2 mL/s). Middle temporal and low-temp sequences were performed once with 2 mL gadobutrol (2 mL/s). The signal-to-noise ratio (SNR) was quantitatively assessed. Blinded reads were used to qualitatively evaluate contrast dose and image quality. The mean SNRs of high-temp, mid-temp, and low-temp resolutions were 56.7, 47.5, and 48.1. There was no significant difference between the 3 sequences with 2 mL gadobutrol. The mean SNR of the high-temp resolution with 2 mL was significantly higher than that with 1 mL (56.7 vs 39.9). In qualitative analysis, the 3 temporal sequences with 2 mL gadobutrol showed no significant differences regarding overall image quality and diagnostic value. High temporal resolution with 2 mL consistently showed the superiority of image quality than that with 1 mL. High temporal dynamic CE-MRA with 2 mL (0.04 mmol/kg body weight) gadobutrol can produce consistently superior image quality over that with 1 mL (0.02 mmol/kg body weight). For a given contrast dose, the tradeoffs between temporal and spatial resolution will not result in significant differences in image quality in TWIST (time-resolved angiography with interleaved stochastic trajectories).

Time‐resolved MR venography of the pulmonary veins precatheter‐based ablation for atrial fibrillation

To evaluate time-resolved magnetic resonance angiography (TR-MRA) of the pulmonary venous circulation using the time-resolved angiography with interleaved stochastic trajectories (TWIST) method and compare it with the more commonly used conventional contrast-enhanced magnetic resonance angiography (CE-MRA) approach in atrial fibrillation patients referred for preablation pulmonary vein mapping. This study was approved by the Institutional Review Board. Twenty-six patients (15 males; age 59.6 ± 12.7 years) referred for preablation pulmonary vein mapping underwent both conventional CE-MRA and TR-MRA with TWIST. Imaging was performed on a 1.5 T (Magnetom Avanto, Siemens Healthcare, Erlangen, Germany) MRI scanner. Source partition and maximum intensity projection (MIP) images were evaluated retrospectively. For quantitative analysis, pulmonary vein ostium orthogonal dimensions were measured using double oblique multiplanar reformatting. The results were analyzed using paired t-tests, Lin's concordance correlation coefficient, and Bland-Altman plots. For qualitative analysis, both source partition images and MIP images were assessed by two observers (A.P. and M.G.). The presence of common ostiums or accessory veins was recorded and analyzed using unweighted Cohen's kappa. Pulmonary vein conspicuity was scored on a scale of 1-4 (1 = poor, 2 = fair, 3 = good, 4 = excellent) and analyzed using paired t-tests, intraclass correlation coefficients, and quadratic weighted kappa statistics. Orthogonal venous diameters were comparable for both TR-MRA and conventional CE-MRA (1.34 ± 0.37 vs. 1.38 cm ± 0.36, respectively). Results of paired t-tests, Lin's concordance correlation coefficient, and Bland-Altman analysis revealed relatively close comparison between methods. The magnitude of the mean difference for any of the statistical comparisons did not exceed 0.10 cm. The visualization of variant pulmonary vein anatomy was very similar for both techniques. Agreement between techniques was determined to be "good" to "very good" (κ = 0.78-0.85). Conspicuity scores for each pulmonary vein were also very close. Paired t-tests, intraclass correlation coefficients, and quadratic weighted kappa statistics all revealed strong agreement between methods. TR-MRA using TWIST produces comparable anatomic images and pulmonary venous dimensions to the more widely used CE-MRA technique. Additionally, the TWIST technique improves arterio-venous separation, does not need exact bolus timing, requires less gadolinium, and gives additional information on vein perfusion.

Imaging evaluation of calf arteries in patients with peripheral arterial occlusive disease by using time-resolved angiography with interleaved stochastic trajectories on MR scanner

Objective To explore the value of time-resolved angiography with interleaved stochastic trajectories (TWIST)in providing hemodynamic indices and morphological imaging of calf arteries in patients with peripheral arterial occlusive disease(PAOD) with 3.0 T MR scanner. Methods Forty patients with confirmed or suspected PAOD underwent TWIST MRA for the calf arteries and conventional contrast-enhanced MRA(CE-MRA) for the whole lower peripheral arteries. TWIST MRA data were used to determine the bolus arriving time of the popliteal artery, the time difference of the bolus arrival in the popliteal artery between the two legs, calf artery transit time, and the mean peak enhancement time of calf arteries. The mean value of peak enhancement time of calf arteries was correlated with ankle-brachial index(ABI). The calf arteries were divided into 13 segments. The visible score of arterial segment was recorded, and the degree of arterial stenosis was graded too. In 16 patients, DSA was used as the gold standard to evaluate the sensitivity and specificity of TWIST MRA and CE-MRA. Results The bolus arriving time of the popliteal artery was (0.7±7.6) s the time difference of the bolus arrival in the popliteal artery between the two legs was (2.1±2.5) s, the calf artery transit time was (35.6±16.9) s, the mean value of peak enhancement time of calf arteries was (52.6±17.6) s. ABI was acquired from 24 patients and 48 legs, which correlated well with the mean value of peak enhancement time in calf arteries (r＝0.627,P＜0.01). The mean visible score was (2.83±0.48) of TWIST MRA images and (2.78±0.43) of CE-MRA images. There was no significant difference between them (P＞0.05). On CE-MRA, there were 38 legs with varying degrees of early venous enhancement; in contrast, there was no venous contamination on TWIST MRA. The sensitivity and specificity of TWIST MRA were 96.7% (118/122) and 100.0% (72/72) in showing normal artery and mild stenosis, 94.1% (32/34) and 96.2% (154/160) in showing severe stenosis, and 94.7% (36/38) and 98.7% (154/156) in showing occlusion. The sensitivity and specificity of CE-MRA were 95.9% (117/122) and 100.0% (72/72) in showing normal artery and mild stenosis, 91.2% (31/34) and 95.6% (153/160) in showing severe stenosis, and 94.7% (36/38) and 98.1% (153/156) in showing occlusion. Conclusion TWIST MRA is a preferable choice for calf arterial occlusive disease, which provided nice morphological images and useful hemodynamic indices with simple operation. Time-resolved mr venography pre catheter-based ablation for atrial fibrillation

Objective The purpose of this study was to evaluate Time-resolved MR venography (TR-MRV) of the pulmonary venous circulation using the time-resolved angiography with interleaved stochastic trajectories (TWIST) method of time-resolved MRA (TR-MRA) and compare it with the more commonly used conventional Contrast enhanced magnetic resonance angiography (CE-MRA) approach in atrial fibrillation patients referred for pre-ablation pulmonary vein mapping. Background Catheter-based ablation of the pulmonary veins prevents recurrence of atrial fibrillation in 70-80% of patients during the first year of follow-up 1,2 . CE-MRA depicts the left atrium and pulmonary veins with high spatial resolution, enabling accurate measurement of pulmonary vein ostia to be made with depiction of their relationship to other structures. 1,3 Conventional CE-MRA however requires timing of contrast enhancement and produces images with overlap of venous and arterial structures, potentially obscuring pulmonary vein ostia. TR-MRA is an alternative to conventional CE-MRA and has been used successfully in other vascular territories. 4 Such an approach may be particularly advantageous in the pulmonary circulation with its rapid arteriovenous transit time, allowing acquisition of pure pulmonary venous phase images with a simpler imaging protocol. Material and methods 26 patients (15 males; age 60.0 ± 12.7y) referred for preablation pulmonary vein mapping underwent both conventional CE-MRA and TR-MRV with TWIST. Imaging was performed on a 1.5 Tesla (MAGNETOM Avanto, Siemens Healthcare) MRI scanner. Source partition and MIP images were evaluated. Quantitative Analysis: Pulmonary vein ostium orthogonal dimensions were measured using double oblique multiplanar reformatting. Qualitative Analysis: For qualitative analysis, both source partition images and MIP images were assessed by two observers. Pulmonary vein conspicuity was scored on a scale of 1-4 (1=poor, 2=fair, 3=good, 4=excellent). The number of pulmonary veins (3 veins= common ostium, 4 veins = normal, 5 veins=accessory vein) was recorded.

Time-Resolved MR Angiography in the Evaluation of Central Thoracic Venous Occlusive Disease

The objective of our study was to assess the feasibility and diagnostic performance of time-resolved MR angiography (MRA) in the evaluation of central thoracic venous occlusive disease and to compare time-resolved MRA with conventional MRA and catheter angiography. Twenty patients (eight women and 12 men; age range, 19-74 years) with suspected central thoracic venous occlusive disease underwent time-resolved MRA using time-resolved angiography with interleaved stochastic trajectories (TWIST) and parallel acquisition, followed by conventional MRA. Catheter angiography was performed within 1-14 days after MRA and was available for a total of 60 segments for correlation. Time-resolved and conventional MRA images were evaluated in separate reading sessions by two independent radiologists for image quality and level of confidence and degree of venoocclusive disease. The interobserver and intermodality agreement, sensitivity, and specificity were calculated using catheter angiography as the standard of reference. Time-resolved MRA resulted in diagnostic-quality images that did not differ significantly in quality compared with conventional MRA. Thirty-one segmental venous stenoses were identified. The kappa coefficient revealed moderate intermodality agreement (kappa = 0.54; 95% CI, 0.32-0.76) between time-resolved MRA and conventional MRA. When compared with catheter angiography, the sensitivity and specificity for the diagnosis of significant stenosis (> or = 70%) were 87.5% and 68% for time-resolved MRA and 90% and 90% for conventional MRA, respectively. Time-resolved MRA, as described in this study, has the potential to be used as an initial and screening diagnostic tool obviating conventional MRA and its associated higher contrast dose in normal and near-normal examinations. However, because of its relatively lower specificity, adjunct use of conventional MRA is still required for accurate grading of venous occlusive disease.
