Changepoint Detection in Heart Rate Variability Indices in Older Patients Without Cancer at End of Life Using Ballistocardiography Signals: Preliminary Retrospective Study.

In an aging society such as Japan, where the number of older people continues to increase, providing in-hospital end-of-life care for all deaths, and end-of-life care outside of hospitals, such as at home or in nursing homes, will be difficult. In end-of-life care, monitoring patients is important to understand their condition and predict survival time; this information gives family members and caregivers time to prepare for the end of life. However, with no clear indicators, health care providers must subjectively decide if an older patient is in the end-of-life stage, considering factors such as condition changes and decreased food intake. This complicates decisions for family members, especially during home-based care. The purpose of this preliminary retrospective study was to determine whether and how changes in heart rate variability (HRV) indices estimated from ballistocardiography (BCG) occur before the date of death in terminally ill older patients, and ultimately to predict the date of death from the changepoint. This retrospective pilot study assessed the medical records of 15 older patients admitted to a special nursing home between August 2019 and December 2021. Patient characteristics and time-domain HRV indices such as the average normal-to-normal (ANN) interval, SD of the normal-to-normal (SDNN) interval, and root mean square of successive differences (RMSSD) from at least 2 months before the date of death were collected. Overall trends of indices were examined by drawing a restricted cubic spline curve. A repeated measures ANOVA was performed to evaluate changes in the indices over the observation period. To explore more detailed changes in HRV, a piecewise regression analysis was conducted to estimate the changepoint of HRV indices. The 15 patients included 8 men and 7 women with a median age of 93 (IQR 91-96) years. The cubic spline curve showed a gradual decline of indices from approximately 30 days before the patients' deaths. The repeated measures ANOVA showed that when compared with 8 weeks before death, the ratio of the geometric mean of ANN (0.90, 95% CI 0.84-0.98; P=.005) and RMSSD (0.83, 95% CI 0.70-0.99; P=.03) began to decrease 3 weeks before death. The piecewise regression analysis estimated the changepoints for ANN, SDNN, and RMSSD at -34.5 (95% CI -42.5 to -26.5; P<.001), -33.0 (95% CI -40.9 to -25.1; P<.001), and -35.0 (95% CI -42.3 to -27.7; P<.001) days, respectively, before death. This preliminary study identified the changepoint of HRV indices before death in older patients at end of life. Although few data were examined, our findings indicated that HRV indices from BCG can be useful for monitoring and predicting survival time in older patients at end of life. The study and results suggest the potential for more objective and accurate prognostic tools in predicting end-of-life outcomes.

Heart Rate Variability as a Predictor of +Gz Tolerance During the High-G Selective Test.

INTRODUCTION: Preselection of pilot candidates in the military is critical and determines the quality of subsequent selection. The Aero Medical Institute in Belgrade uses the following centrifuge endpoints: peripheral vision loss, heart rate above 180 bpm, cardiac arrhythmias, and G-induced loss of consciousness to assess relaxed +Gz tolerance. The aim of this study was to evaluate heart rate variability (HRV) associated with cardiovascular adaptability to different types of stress as a predictor of +Gz tolerance.METHODS: Thirty-six candidates were exposed to centrifuge runs, consisting of the following +Gz-acceleration phases: a 2-G plateau followed by an increase to 5.5 G, a decrease to 2 G, and ending with a plateau. Time-domain HRV indices were calculated for candidates, for a 60-s 2-G plateau, and for the entire test. The correlation was made between the groups that did (Group 1) and did not meet the criteria (Group 2).RESULTS: The results show significantly lower values in all time domain HRV indices, namely standard deviation of the normal-to-normal interval (SDNN) and root mean square of successive differences, in Group 2. Mean SDNN values were 45.98 ± 24.80 ms (2-G plateau) and 109.99 ± 39.64 ms (entire test) in Group 1, while the SDNN were 22.99 ± 12.04 ms and 69.70 ± 33.45 ms in Group 2. Root mean square of successive differences was higher in Group 1 for the 2-G plateau and for the entire test.DISCUSSION: The results suggest that HRV is positively correlated with +Gz-tolerance and can be used as an additional selection tool for military aircrew.Bacevic N, Ninkovic M, Drvendzija M, Vidakovic J, Bacevic M, Stepanic P. Comparison of time domain indices of heart rate variability between alcohol-dependent individuals and non-alcoholic subjects

Background: Alcohol dependence develops because of repeated alcohol intake. Alcohol damages the cardiac autonomic nerve fibers leading to autonomic imbalance, resulting in reduced heart rate variability (HRV). HRV refers to the complex beat-to-beat variation in the heart rate, produced by the interplay of sympathetic and parasympathetic neural activity at the Sino-atrial node. Our body works effectively in parasympathetic activity dominance. The time domain indices of HRV are specific markers of parasympathetic activity. A decrease in the time domain parameters indicates reduced parasympathetic activity in the heart, indicating cardiac autonomic dysfunction with vagal neuropathy. Aims and Objectives: The objectives of the study are as follows: (1) To assess the HRV in alcohol-dependent individuals using time domain indices. (2) To compare the HRV between alcoholic-dependent individuals and non-alcoholic subjects. Materials and Methods: A total of 110 individuals in the age group of 20–55 years were recruited for our study. Among them, 55 were alcohol-dependent individuals selected based on the diagnostic and statistical manual-IV, international classification of diseases-10 criteria, and 55 age and gender-matched volunteers were selected as healthy non-alcoholic controls. Short-term HRV was recorded in a supine posture, and the results were interpreted. Statistical analysis was done using the software SPSS version 21.0. Results: Our results showed a significant reduction in the time-domain parameters of HRV – SDNN, RMSSD, NN50, and pNN50 in the alcohol-dependent individuals compared to the controls, suggesting reduced parasympathetic activity in the heart indicating cardiac autonomic neuropathy. Conclusion: Alcohol affects the cardiac autonomic status leading to sudden death due to arrhythmias. HRV can be used as a non-invasive test to assess cardiac autonomic dysfunction and its biofeedback can help alcohol-dependent individuals to adopt healthy lifestyle measures.

0114 Interplay of School and Free Days on the Association of Circadian Misalignment with Heart Rate Variability in Adolescents

Abstract Introduction The circadian timing system, which governs the sleep-wake cycle, is involved in cardiometabolic and autonomic regulation. Heart rate variability (HRV), a marker of cardiac autonomic modulation (CAM), is known to predict cardiovascular morbidity. Therefore, circadian misalignment of the sleep-wake cycle, which is highly prevalent in adolescents and may express differently if youth are in school or free-days, is likely to impact CAM. We examined whether deviations in the circadian timing of sleep are associated with blunted HRV in adolescents as a function of being entrained to school or not. Methods We studied 337 population-based randomly-selected adolescents from the Penn State Child Cohort (median 16 years; 47% female; 21% racial/ethnic minority) who had at least 3-night at-home actigraphy (ACT), in-lab 9-h polysomnography (PSG) and 24-h Holter-monitoring heart rate variability (HRV) data. ACT-measured sleep midpoint (SM) was calculated as the midpoint of the sleep period for weekdays (5-nights) and weekends (2-nights), whereas ACT-measured sleep regularity (SR) was calculated as the intra-individual standard deviation of the SM across 5- and 7-nights. Frequency and time-domain HRV indices were primary outcomes. Linear regression models were stratified by “in school” and “on break” to test SM and SR as predictors of HRV indices while accounting for sex, race/ethnicity, age, BMI percentile, ACT-sleep duration, ACT-sleep variability, and PSG-apnea/hypopnea index. Results Lower SR on schooldays was associated with lower nighttime Log-HF, Log-LF and SDNN, yet higher HR (all p&amp;lt; 0.01); e.g., each standard deviation increase in SR on schooldays (i.e., 45m higher) was associated with -7.15ms (2.57) in nighttime SDNN (p=0.006). A later SM on free weekends was associated with lower nighttime Log-HF, Log-LF, SDNN and RMSSD, yet higher HR and LF-HF ratio (all p&amp;lt; 0.03); e.g., each standard deviation increase in SM on free weekends (i.e., 1.5h later) was associated with -7.33ms (2.85) in nighttime RMSSD (p=0.011). Conclusion A delayed and an irregular sleep midpoint during breaks and during schooldays, respectively, were associated with impaired CAM in adolescents. These data suggest that not only circadian misalignment contributes to adverse cardiovascular outcomes but its distinct metrics require measurement under different entrainment conditions in adolescents. Phase-change material cooling blanket: A feasible cooling choice during transport after exercise-induced hyperthermia

BackgroundExercise-induced hyperthermia preceding the onset of exertional heatstroke requires a rapid reduction in the body core temperature (Tcore) to ensure safety. In recent years, phase-change material (PCM) cooling devices have been increasingly used for rapid cooling after hyperthermia due to their superior capacity for heat absorption. ObjectivesThis study aimed to evaluate the cooling performance and effectiveness of a PCM cooling blanket on heart rate (HR) and heart rate variability (HRV) recovery after exercise-induced hyperthermia. DesignRandomized cross-over. MethodsThe study participants were 12 male volunteers who were engaged in professional training and completed an endurance exercise for approximately 30 min in a hot and humid environment (temperature ≈ 30 °C; relative humidity ≈ 66%). The participants underwent a 30-min cooling trial after exercise, receiving either treatment with a PCM cooling blanket (PCM group) or natural cooling (CON group). The Tcore, HR, and HRV time-domain indices were used for analysis. ResultsThe Tcore values were significantly lower in the PCM group during cooling. Reductions in the Tcore from precooling to 20 min of cooling were significantly greater in the PCM group than in the CON group. The HR in the PCM group was lower than that recorded in the CON group at 10 and 20 min of cooling. The reduction in HR during cooling from precooling was also significantly greater in the PCM group. HRV time-domain indices during cooling in the PCM group were significantly lower compared with the CON group while elevations in some HRV time-domain indices from precooling to postcooling were significantly greater in the PCM group than in the CON group. ConclusionsThe PCM cooling blanket had good cooling performance and the ability to hasten recovery of both HR and HRV. It may serve as a feasible cooling choice during transport after exercise-induced hyperthermia.

Abstract MP47: Sleep Disordered Breathing is Adversely Associated With Cardiac Autonomic Modulation in Adolescents

Introduction: Sleep disordered breathing (SDB), as measured by the apnea/hypopnea index (AHI), is a known risk factor for cardiovascular disease (CVD) in adults. Although SDB is prevalent in pediatric populations, there has been less focus on SDB as a primary risk factor for CVD in youth. The recent AHA scientific statement on SDB and CVD in children and adolescents suggests that, in order to better understand the long-term CVD-related risk associated with SDB in youth, additional longitudinal studies incorporating noninvasive markers of CVD are needed. The current study aimed to fill this important gap. Hypothesis: We hypothesized that SDB is associated with decreased HRV in adolescents. Methods: We studied 421 subjects from the Penn State Child Cohort who were 8.7 (1.7) years old at baseline and were followed-up as adolescents 16.5 (2.3) years old. We estimated the AHI from 9-hour, in-lab polysomnography (PSG), and frequency and time-domain heart rate variability (HRV) indices from 24-h Holter EKG monitoring immediately following PSG. We stratified 24-h HRV data into daytime and nighttime periods. Linear regression analyses examined the association between AHI with 24-h HRV indices, while adjusting for age, sex, race/ethnicity, BMI percentile, and metabolic syndrome (MetS) score in adolescence as well as AHI and nighttime HRV indices in childhood. Results: While there were no significant associations between AHI and daytime (p=0.233) or nighttime (p=0.147) HR, there were significant associations with frequency and time-domain HRV indices. A higher AHI was associated with lower Log-HF, Log-LF, SDNN and RMSSD, yet higher LF/HF, for both daytime and nighttime periods (all p&lt;0.05). For example, for each additional apnea or hypopnea per hour of sleep there was a decrease of -1.74ms (0.73) and -1.65ms (0.77) in SDNN during the daytime (p=0.018) and nighttime (p=0.033), respectively, even after adjusting for all covariates including MetS, childhood AHI and SDNN. More specifically, the 24-h SDNN of adolescents with an AHI≥5, indicative of moderate-to-severe SDB, was 10.9ms (4.4) lower than that of adolescents with an AHI=0 (p=0.014). Conclusions: SDB is associated with impaired cardiac autonomic balance in adolescence independently of MetS, prior SDB and prior HRV status. These data further support the role of SDB as an independent risk factor for CVD early in life.

Abstract 13: Circadian Misalignment is Associated With Impaired Heart Rate Variability in Adolescents

Introduction: Adequate sleep has been recognized by AHA as part of Life’s Essential 8 in the prevention of cardiovascular disease (CVD). The circadian system, which governs the sleep-cycle, is also involved in cardiac, metabolic and autonomic regulation. Heart rate variability (HRV), a marker of cardiac autonomic modulation (CAM), is known to predict CVD. As a result, circadian misalignment of the sleep-wake cycle, which is highly prevalent in adolescents, may impact CAM and be a potential risk factor for adverse CVD outcomes in early life. Hypothesis: We hypothesize that deviations in the circadian timing of sleep are associated with blunted HRV in adolescents. Methods: We analyzed data from 337 population-based randomly-selected adolescents from the Penn State Child Cohort (median 16 years; 47% female; 21% racial/ethnic minority) who had at least 3-night at-home actigraphy (ACT), in-lab 9-h polysomnography (PSG) and 24-h Holter-monitoring heart rate variability (HRV) data. ACT-sleep duration and sleep midpoint (SM) were calculated as the intra-individual mean total sleep time and midpoint of the sleep period, respectively. Frequency and time-domain HRV indices, including HR oscillations at high frequencies (HF), standard deviation between normal heart beats or R-R intervals (SDNN), and root mean square of successive differences between normal heartbeats (RMSSD), were primary outcomes. Linear regression models tested SM and SR as predictors of HRV indices, while accounting for sex, race/ethnicity, age, body mass index, ACT-sleep duration, ACT-sleep variability, and PSG-apnea/hypopnea index. Results: While there were no meaningful associations between SM or SR with daytime HRV, there were significant associations between SM with nighttime HRV. A later SM was associated with lower nighttime Log-HF, SDNN and RMSSD, yet higher nighttime LF/HF (all p&lt;0.03). For each standard deviation increase in SM (i.e., 1.5 hours later), there was a decrease of -5.50ms (1.92) in nighttime RMSSD (p=0.004). More specifically, the nighttime RMSSD of adolescents with a SM at 5:30 (i.e., sleep onset at 2:00, sleep offset at 9:00), was 18.2ms (6.7) lower (p=0.007) than that of adolescents with a SM at 2:30 (i.e., sleep onset at 23:00, sleep offset at 6:00). Conclusions: A delayed sleep phase is associated with impaired CAM in adolescents. These data support that circadian misalignment of the sleep-wake cycle may independently contribute to CVD risk.

Heart Rate Variability Analysis on Electrocardiograms, Seismocardiograms and Gyrocardiograms of Healthy Volunteers and Patients with Valvular Heart Diseases.

Heart rate variability (HRV) is the physiological variation in the intervals between consecutive heartbeats that reflects the activity of the autonomic nervous system. This parameter is traditionally evaluated based on electrocardiograms (ECG signals). Seismocardiography (SCG) and/or gyrocardiography (GCG) are used to monitor cardiac mechanical activity; therefore, they may be used in HRV analysis and the evaluation of valvular heart diseases (VHDs) simultaneously. The purpose of this study was to compare the time domain, frequency domain and nonlinear HRV indices obtained from electrocardiograms, seismocardiograms (SCG signals) and gyrocardiograms (GCG signals) in healthy volunteers and patients with valvular heart diseases. An analysis of the time domain, frequency domain and nonlinear heart rate variability was conducted on electrocardiograms and gyrocardiograms registered from 29 healthy male volunteers and 30 patients with valvular heart diseases admitted to the Columbia University Medical Center (New York City, NY, USA). The results of the HRV analysis show a strong linear correlation with the HRV indices calculated from the ECG, SCG and GCG signals and prove the feasibility and reliability of HRV analysis despite the influence of VHDs on the SCG and GCG waveforms.

Day-night patterns in heart rate variability and complexity: differences with age and cardiopulmonary disease.

Heart rate variability (HRV) measures provide valuable insights into physiology; however, gaps remain in understanding circadian patterns in heart rate dynamics. We aimed to explore day-night differences in heart rate dynamics in patients with chronic cardiopulmonary disease compared with healthy controls. Using 24-hour heart rate data from patients with chronic obstructive pulmonary disease (COPD) and/or heart failure (n = 16) and healthy adult controls (older group: ≥50 years, n = 42; younger group: 20-49 years, n = 136), we compared day-night differences in conventional time and frequency domain HRV indices and a multiscale-entropy-based complexity index (CI1-20) of HRV among the 3 groups. Twenty-four-hour HRV showed significant day-night differences (marked with "△") among younger healthy (mean age: 34.5 years), older healthy (mean age: 61.6 years), and cardiopulmonary patients (mean age: 68.4 years), including change in percentage of adjacent intervals that differ > 50 ms (△pNN50), high frequency (△HF), normalized low frequency (△nLF), ratio (△LF/HF), and △CI1-20. Among these, △LF/HF (2.13 ± 2.35 vs 1.1 ± 2.47 vs -0.35 ± 1.25; P < .001) and △CI1-20 (0.15 ± 0.24 vs 0.02 ± 0.28 vs -0.21 ± 0.27; P < .001) were significant in each pairwise comparison following analysis of variance tests. Average CI1-20 was highest in younger healthy individuals and lowest in cardiopulmonary patients (1.37 ± 0.12 vs 1.01 ± 0.27; P < .001). Younger healthy patients showed a heart rate complexity dipping pattern (night < day), older healthy patients showed nondipping, and cardiopulmonary patients showed reverse dipping (night > day). As measures of 24-hour variability, traditional and complexity-based metrics of HRV exhibit large day-night differences in healthy individuals; these differences are blunted, or even reversed, in individuals with cardiopulmonary pathology. Heart rate variability during high-speed treadmill exercise and recovery in Thoroughbred racehorses presented for poor performance.

Heart rate variability (HRV) analysis measures the inter-beat interval variation of successive cardiac cycles. Measurement of these indices has been used to assess cardiac autonomic modulation and for arrhythmia identification in exercising horses. To report HRV indices during submaximal exercise, strenuous exercise and recovery, and explore relationships with clinical conditions (arrhythmias, lameness, equine gastric ulcer syndrome [EGUS], lower airway inflammation and upper respiratory tract obstructions [URTOs]) in Thoroughbred racehorses. Retrospective, observational cross-sectional study. One hundred and eighty Thoroughbred horses underwent a treadmill exercise test with simultaneous electrocardiographic recording. Time-domain HRV indices (standard deviation of the R-R interval [SDRR]; root mean square of successive differences [RMSSD]) were derived for submaximal and strenuous exercise and recovery segments. Clinical conditions (arrhythmia [during each phase of exercise], lameness, EGUS, lower airway inflammation and URTO) were assigned to binary categories for statistical analysis. Relationships between selected HRV indices and the clinical conditions were explored using linear regression models. During submaximal exercise, lameness was associated with decreased logRMSSD (B=-0.19 95% confidence interval [CI] -0.31 to -0.06, p=0.006) and arrhythmia was associated with increased logRMSSD (B=0.31 95% CI 0.01-0.608, p=0.04). During strenuous exercise, arrhythmia was associated with increased HRV indices (logSDRR B=0.51 95% CI 0.40-0.62, p < 0.001; RMSSD B=0.60 95% CI 0.49-0.72, p < 0.001). During recovery, arrhythmia was associated with increased HRV indices (logSDRR B=0.51 95% CI 0.40-0.62, p < 0.001, logRMSSD B=0.60 95% CI 0.49-0.72, p < 0.001). The main limitations of this retrospective study were that not every horse had the full range of clinical testing, therefore some horses may have had undetected abnormalities. The presence of arrhythmia increased HRV in both phases of exercise and recovery. Lameness decreased HRV during submaximal exercise.

Wavelet Transform Based Heart Rate Variability Analysis of ECG

Electrocardiography (ECG) is recording of heart electrical activity. For analyzing and diagnosis of heart diseases ECG is very important. In graphical ECG which used for clinical diagnosis all features are not visible. Different types of signal processing methods are present which can be used for extracting ECG signal features. Wavelet transforms is one kind of signal processing tool which is used for analyzing ECG signal. For features extraction multi-resolution wavelet transform can be used. During recording of ECG different kind of noise are added with ECG. So noise should be removed from ECG, than R peaks were detected which amplitude is higher than the other peaks. Referring to R peaks the others peak as P, Q, S and T were detected. Then different feature of the ECG signal were detected. Time differences between R peaks were calculated and then heart rate calculated from mean RR interval. In ECG RR interval indicate the change between consecutive heart rate (HR). Heart rate variability (HRV) explored how RR interval varies over time. HRV is calculated from RR interval series obtained from ECG signal analysis. From the RR intervals time domain indices of HRV were determined by using MATLAB programming and MIT-BIH database signal were used as input. In the time domain method SDNN, RMSSD, and pNN50 etc were determined here.

Evaluation of Heart Rate Variability in Children with Congenital Heart Diseases before and after Surgical Repair

Background: Surgical repair of congenital heart diseases (CHD) affects heart rate variability (HRV) in different ways. It leads to varying degrees of cardiac autonomic derangement, clinically detectable as depression of HRV parameters. The aim of this work was to evaluate HRV in children with CHDs before and after surgical repair to detect the effect of CHDs (acyanotic or cyanotic) and cardiac surgery on cardiac autonomic function.&#x0D; Methods: This observational case-control study enrolled thirty cases with CHD. Cases were divided in two equal groups: group1 included children with CHDs (not repaired) before surgical intervention and group 2 included children after surgical repair of CHDs. Another fifteen healthy children, matched for age and sex, were enrolled as a control group. All children in this study were subjected to full history taking, clinical evaluation, Holter monitor, twenty-four-hour ambulatory electrocardiographic recordings and the seven time-domain indices of HRV.&#x0D; Results: There was significant increase of HR in patients before surgical repair as compared to patients after surgical repair as compared to control group. There was significant decrease of PNN50, r MMSD, SDNN index and SDANN of children before surgical repair as compared to children after surgical repair as compared to controls. There was a negative significant correlation between HR and age before surgery, after surgery there was a negative significant correlation between HR and age, pNN50, rMMSD, SDNN index and SDANN, in control there was negative significant correlation with age, SDNN index, SDANN and SDNN.&#x0D; Conclusion: HRV parameters increased significantly in children with CHDs after surgical repair as compared to these before surgical repair denoting effect of cardiac surgery on cardiac autonomic function.

Effect of Prenatal Yoga on Heart Rate Variability and Cardio-Respiratory Synchronization: A Prospective Cohort Study.

The objective was to assess the effects of prenatal yoga on heart rate variability (HRV) and cardio-respiratory synchronization, used as proxies of autonomic nervous system activity. Sixty-nine healthy pregnant women were included; 33 in a yoga group attending at least one 90-min yoga class weekly throughout pregnancy, and 36 controls not involved in formal pregnancy exercise programs. Measurements of the time domain (SDNN, standard deviation of regular R-R intervals, and RMSSD, square root of mean squared differences of successive R-R intervals) and frequency domain (ln(LF/HF), natural logarithm of low-frequency to high-frequency power) HRV indices, as well as cardio-respiratory synchronization indexes were performed once per trimester before and after yoga or 30-min moderate-intensity walk. A statistical comparison was performed using a three-way analysis of the variance (p < 0.05 significant). Both the time domain and frequency domain HRV indices showed significant shifts towards parasympathetic dominance following yoga when compared to the controls throughout pregnancy (p = 0.002 for SDNN, p < 0.001 for RMSSD, and p = 0.006 for ln(LF/HF), respectively). There was a statistically non-significant trend towards higher synchronization between respiratory frequency and heart rate following yoga vs. controls (p = 0.057). Regular prenatal yoga was associated with enhanced parasympathetic activation persisting throughout pregnancy.

PurposeThis study assessed the autonomic nervous system in patients with central serous chorioretinopathy (CSC) by simultaneously measuring pupillary responses and heart rate variability (HRV).MethodsWe recruited 33 patients with CSC and 26 age- and sex-matched healthy controls. Using a pupillometry and acceleration plethysmography system, we measured the participants’ pupillary light reflex and HRV simultaneously, and compared the following parameters between the two groups: the pupil diameters, diameter changes, and time and frequency domain HRV indices (high frequency power: HF; low frequency power: LF; log LF/HF ratio). Additional data from the Profile of Mood States (POMS) and pupillary responses during mental tasks were also analyzed.ResultsThe CSC group had a significantly lower constriction amplitude and a higher re-dilation ratio compared with the control group, indicating parasympathetic inhibition and sympathetic activation. For the HRV measures, the CSC group demonstrated significantly lower HF and higher LF and log LF/HF ratio, indicative of higher sympathetic activity. The CSC group also showed significantly larger pupil dilation during tasks of moderate difficulty, and higher negative/lower positive POMS mood scores. Further analyses also revealed that the baseline pupil diameter was significantly larger in patients with active as opposed to chronic CSC.ConclusionsPupillary responses and HRV measures both revealed sympathetic activation and parasympathetic attenuation in patients with CSC. Larger pupil dilation during mental tasks in CSC could be a potential marker of psychophysiological stress.

The Impact Of Varying Blood Flow Restriction Cuff Size On Cardiac Autonomic Modulation In Healthy Males

Blood flow restriction (BFR) training is commonly utilized to stimulate skeletal muscle hypertrophy and assist in strength gains in various populations, ranging from healthy, athletic, clinical, and elderly populations. BFR has the potential to influence the sympathetic and parasympathetic interaction by stimulating acute changes in the vascular endothelium, thereby altering cardiac autonomic modulation (CAM). Heart rate variability (HRV) is a non-invasive way to assess changes in CAM. What is unknown is the influence BFR may have on CAM due to the changes in venous and arterial pressure. PURPOSE: To determine if different BFR cuff sizes and materials alter CAM when assessed in healthy young men. METHODS: Twenty healthy males (n = 20 males, Age 25 ± 5 years, HR 66 + 11.3 bpm, SBP 125.1 + 13.4 mmHg, DBP 75.1 + 12.5 mmHg) completed a randomized, counter-balanced, cross-over design that utilized a widely-employed wide rigid (WR) cuff, along with the largest (NE) and manufacturer recommended sizes (NER) of a novel narrow-elastic cuff. HRV was measured in the supine position at baseline and amidst all three BFR cuffs at 80% arterial occlusion pressure using an elastic belt and Bluetooth monitor (Polar H7). CardioMood software was used to process HRV variables high frequency (HF), low frequency (LF), total power (TP) was assessed for frequency domain, and standard deviation of all NN intervals (SDNN) and the square root of the mean of the squares of successive R-R interval differences (RMSSD) for the time domain. The data were analyzed with SPSS using a repeated-measures ANOVA to examine differences between dependent variables, including selected time and frequency domain HRV indices. RESULTS: HRV frequency domain variables LF, HF, LF/HF, and TP, and time domain, SDNN, and RMSSD variables were not significantly altered with WR, NE, or NER cuff protocols when compared to baseline. There were no significant differences between cuff protocols. CONCLUSION: There were no changes in CAM with BFR cuff protocols in healthy young men. The lack of changes indicates that changes in the venous and arterial pressure ratio due to BFR were not great enough to offset the sympathetic and parasympathetic systems.

Effect of Lesion of Central Amygdala on Heart Rate Variability in Albino Wistar Rats

Background and Aim: Heart rate variability (HRV) reflects the pre-frontal cortex (PFC) and amygdalar connections. HRV is also influenced by emotions, which is controlled by limbic system. Thus, amygdala-limbic connection is suggested to be the major integrator of HRV and the alterations in the regulation lead to cardiometabolic risks. The present study was conducted to assess the central amygdala in the heart rate variability in albino Wistar rats. Methods: A total of 8 albino Wistar rats were taken for the study. Stereotaxic procedure was performed and lesion was made into the central amygdalar nuclei bilaterally. HRV was assessed before and after lesion of central amygdala. Results: Following lesion of central amygdala, there was decrease in total power (TP), high frequency (HF), and other time domain indices of HRV, while there was increase in low frequency (LF) and LF-HF ratio. Conclusion: Lesion of central amygdala decreased TP indicating the crucial role of central amygdala in the control of HRV. The control of central amygdala on sympathovagal balance is profound.

Effect of Transcranial Pulsed Current Stimulation on Fatigue Delay after Medium-Intensity Training.

The purpose of this study was to investigate the effect of transcranial pulsed current stimulation (tPCS) on fatigue delay after medium-intensity training. Materials and Methods: Ninety healthy college athletes were randomly divided into an experimental group (n = 45) and control group (n = 45). The experimental group received medium-intensity training for a week. After each training, the experimental group received true stimulation of tPCS (continuous 15 min 1.5 mA current intensity stimulation). The control group received sham stimulation. The physiological and biochemical indicators of participants were tested before and after the experiment, and finally 30 participants in each group were included for data analysis. Results: In the experimental group, creatine kinase (CK), cortisol (C), time-domain heart rate variability indices root mean square of the successive differences (RMSSD), standard deviation of normal R-R intervals (SDNN), and frequency domain indicator low frequency (LF) all increased slowly after the intervention. Among these, CK, C, and SDNN values were significantly lower than those in the control group (p < 0.05). Testosterone (T), T/C, and heart rate variability frequency domain indicator high frequency (HF) in the experimental group decreased slowly after the intervention, and the HF value was significantly lower than that in the control group (p < 0.05). The changes in all of the indicators in the experimental group were smaller than those in the control group. Conclusion: The application of tPCS after medium-intensity training enhanced the adaptability to training and had a significant effect on the maintenance of physiological state. The application of tPCS can significantly promote the recovery of autonomic nervous system function, enhance the regulation of parasympathetic nerves, and delay the occurrence of fatigue.

Effects of Acute Exercise on Cardiac Autonomic Response and Recovery in Non-Dialysis Chronic Kidney Disease Patients

ABSTRACT Purpose: Heart rate variability (HRV) has gained acceptance as a key marker of cardiovascular health. We compared HRV responses after continuous moderate-intensity exercise (CMIE) and high-intensity interval exercise (HIIE) matched for intensity and duration in individuals with midspectrum chronic kidney disease (CKD). Methods: Twenty men and women (age 62.0 ± 10 yrs.) diagnosed with CKD stages G3a and G3b participated in a 2 (condition) x 4 (time point) repeated cross-over measures design study. HRV time-domain indices were based on the standard deviation of all NN intervals (SDNN) and the square root of the mean of the sum of the squares of differences between adjacent NN intervals (RMSSD) and frequency domain. High-frequency (HF), low-frequency (LF), total power (TP) were examined. CMIE consisted of treadmill walking for 30 minutes at a 2% incline and speed corresponding to 60%–65% of reserve volume of oxygen (VO2R). HIIE included five intervals of 3 minutes at 90% of VO2R and 2 minutes at 20% VO2R intervals. Conditions were designed to be of the same average intensity (60% to 65% of VO2R) and caloric expenditure (~144 kcal). Results: Immediately following exercise SDNN, RMSSD, HF, LF, and TP were significantly lower compared to before exercise (p <.05). HRV responses were not different between conditions and conditions X time (p >.05). Conclusions: Thirty minutes of either CMIE or HIIE decreased HRV indices, pointing to an autonomic imbalance favoring vagal mediation. HRV’s responses regarding HIIE were no different from CMIE, therefore, from an autonomic function point of view this similarity may be useful for CKD exercise prescription and programming.

The Effect of Breathing Route on Heart Rate Variability—A within Subject Comparative Study

Background: The effect of traffic related emissions on Heart Rate Variability (HRV) has been previously demonstrated. However, the results of different studies on the relationship between ambient pollutants and specifically carbon monoxide (CO) on HRV are inconclusive and appear to reflect personal differences. The differences in methodology including breathing style and participants under study can possibly attribute to this variability. The aim of the present study is to assess the effect of the breathing route (Nasal/Oral) on the relationship between CO concentration and HRV. Methodology: Forty healthy participants (25 females, 15 males) between the ages of 15 - 50 years were included in the study. The participants strolled in a central bus station, staying for 10 minutes in three designated locations alternating nasal and oral breathing. CO concentration and HRV were continuously monitored. Frequency (LnLF, LnHF, LnLF/HF) and time domain (LnSDNN, LnRMSSD) HRV indices were computed. Analysis: MANCOVA, with HRV indices being the dependent variables and CO, gender and age being the independent variables was employed. Results: Significant interaction effects between breathing route and CO, and breathing route and gender on HRV (LnLF/HF) were found (p = 0.04 and 0.01 respectively). Both CO and age were found to affect LnSDNN and LnRMSSD. Conclusion: Breathing route emerges as a possible modifier of the relationship between air pollution and HRV and thus contributes to interpersonal differences obtained in studies investigating the effect of environmental pollution and HRV.

Electrocardiography clues in assessment of patients with premature ventricular contractions

Some electrocardiography (ECG) parameters such as Tp-e interval, Tp-e / QT ratio, fragmented QRS (fQRS), and heart rate variability (HRV) are related to cardiovascular mortality and morbidity. We aim to investigate the relation between premature ventricular contraction burden and these parameters on 24-h ECG recording. The study is a retrospective investigation of the 24-h Holter ECG and echocardiography of 199 patients who underwent the procedures due to complaints of palpitation. A frequency of < 10% PVCs / 24 h was classified as seldom group (98 patients), while > 10% PVCs / 24 h was designated as frequent group (101 patients). Tp-e interval was significantly longer (62 [54–78] vs 75 [60–84], p < 0.001), Tp-e / QT ratio was significantly increased (0.18 [0.16–0.20] vs 0.21 [0.18–0.22], p = 0.001) in frequent PVC group. The percentage of fQRS was significantly increased in frequent PVC group (30.6% vs 47.5%, p = 0.015). When the groups were compared, no significant difference was found in HRV time domain indices. Positive correlations were observed between PVC burden and Tp-e (r = 0.304, p < 0.001), Tp-e / QT (r = 0.275, p < 0.001). Our study showed that Tp-e interval, Tp-e / QT and fQRS are associated with frequency of PVCs. These measurements in patients with PVCs may form part of assessment of cardiovascular risk.
