An analysis of the etiologic technical factors leading to the failure of the primary reconstruction is critical toa predictable outcome of the revision anterior cruciate ligament surgery. Major issues involving the tibial tunnel during revision ACL reconstruction including removal of primary fixation, tunnel exchange, same-tunnel revision, tibial enlargement, and graft fixation are discussed, with a focus on techniques and available equipment. An algorithmic approach to the management of these issues is presented. With careful planning, predicable, successful revision can be achieved.
