Clinicopathological Evaluation of Thyroid Dysfunction in Women with Abnormal Uterine Bleeding

Introduction: Abnormal uterine bleeding describes any variation from normal bleeding patterns in nonpregnant, reproductive-aged women beyond menarche lasting for at least 6 months. Regular cyclic menstruation results the choreographed relationship between the endometrium and its regulating factors. Any type of disturbance between the regulatory mechanism of pituitary ovarian axis or pelvic diseases results in abnormal uterine bleeding. Thyroid hormones play a key role in the menstrual and reproductive function of women. It is recognized universally that menstrual disturbances may accompany clinical alterations in thyroid function. Objectives: To evaluate the thyroid dysfunction in patients with abnormal uterine bleeding. Materials and Methods: This cross sectional study was conducted in Department of obstetrics & gynaecology, Bangabandhu Sheikh Mujib Medical University (BSMMU), Shahbag, Dhaka from April 2023 to December 2023. Women with abnormal uterine bleeding were included in the study. Sample was selected by purposive sampling. Sample size was 100. Detail demographic data were collected from the informant and recorded in structured case report form. Clinical examination and relevant investigation were done meticulously. Data was collected by using a semi-structured questionnaire. Data was processed and analysed with the help of computer program SPSS and Microsoft excel. Quantitative data expressed as mean and standard deviation and qualitative data as frequency and percentage. Result was presented in the form of tables, pie chart, graphs, bar diagrams, histogram & charts etc. Results: In this study, the maximum numbers of cases (43.0%) were between 26-35 years age group. Mean age was 29.57 ± 8.27 years. Large numbers of respondents came from urban area (58%). On evaluation of types & causes of AUB, maximum patient belongs to AUB-L (leiomyoma) group (53%) followed by AUB-A (adenomyosis) (28%); AUB-P (polyp) (8%); AUB-O (ovulatory) (9%) and AUB-M (malignancy) (2%). Present study showed that prevalence of thyroid disorders is 26% in AUB patients. The prevalence of Hypothyroidism was 18% and Hyperthyroidism was 8%. Thyroid dysfunction is associated with menstrual abnormalities in females of all age groups. Conclusion: Abnormal uterine bleeding has a strong association with thyroid disorders. The most common type of disorder is subclinical hypothyroidism. Thus, all patient of AUB must be evaluated for thyroid dysfunction. Medicine Today 2023 Vol.36 (1): 1-6

Investigating the Bidirectional Association of Rheumatoid Arthritis and Thyroid Function: A Methodologic Assessment of Mendelian Randomization.

Rheumatoid arthritis (RA) and thyroid dysfunction are frequently observed in the same patient. However, whether they co-occur or exhibit a causal relationship remains uncertain. We aimed to systematically investigate the causal relationship between RA and thyroid function using a large sample and advanced methods. Bidirectional two-sample Mendelian randomization (MR) analysis was performed based on RA and six thyroid function trait data sets from the European population. The robustness of the results was demonstrated using multiple MR methods and a series of sensitivity analyses. Multivariable MR using Bayesian model averaging (MR-BMA) was performed to adjust for possible competing risk factors. A sensitivity data set, which included data from patients with seropositive RA and controls, was used to repeat the analyses. Furthermore, enrichment analysis was employed to discover the underlying mechanism between RA and thyroid functions. A significantly positive causal effect was identified for RA on autoimmune thyroid disease (AITD) as well as for AITD on RA (P < 0.001). Further sensitivity analyses showed consistent causal estimates from a variety of MR methods. After removing the outliers, MR-BMA results showed that RA and AITD were independent risk factors in their bidirectional causality, even in the presence of other competing risk factors (adjusted P < 0.05). Enrichment analysis showed immune cell activation and immune response play crucial roles in them. Our results illustrate the significant bidirectional causal effect of RA and AITD, which holds even in multiple competing risk factors. Clinical screening for thyroid dysfunction in patients with RA deserves further attention, and vice versa.

A Study on Thyroid Dysfunction in Patients with Psoriasis

Background: Psoriasis is a common and chronic inflammatory skin disease which is characterize by clearly defined red and scaly plaques. The plaques of psoriasis can be disfiguring and severely pruritic and/or painful. Itching is often the most bothersome symptom of psoriasis. Methodology: In this hospital based cross-sectional study, a total of 180 people in the all age group were included. Individuals attending the Dermatology Out-patient Department of Bangabandhu Sheikh Mujib Medical University (BSMMU) were selected during the study period from July’2023 to Januray’2024. Total 90 patients with diagnosed cases of psoriasis were selected as study group and 90 age and sex matched healthy individuals were selected as controls. Results: No significant difference was noted in the values of FT3, FT4 and TSH hormones in the values among the control group and patient group. In males 10 (10.2%) were hypothyroid, 51 (86.4%) euthyroid and 2(3.4%) was hyperthyroid whereas, in females 5(16.13%) were hypothyroid, 25 (80.65%) euthyroid and 1 (3.2%) were hyperthyroid. Conclusion: No significant difference was noted in the values of FT3, FT4 and TSH hormones in the values among the control group.

IMPACT OF THYROID DYSFUNCTION IN PATIENTS WITH ATRIAL FIBRILLATION

Atrial fibrillation (AF) is the most common sustained cardiac arrhythmia in clinical practice, affecting approximately 2% of the general population and increasing the risk of stroke, heart failure and mortality. AF is often associated with several clinical conditions, including thyroid dysfunction, which can alter metabolism, function and cardiac structure. Thyroid dysfunction can be classified as hypothyroidism (low production of thyroid hormones) or hyperthyroidism (excess production of thyroid hormones), both of which can cause or worsen AF. The mechanism by which thyroid dysfunction affects AF is complex and involves electrophysiological, hemodynamic, inflammatory and structural changes in the atria. Appropriate diagnosis and treatment of thyroid dysfunction can improve AF control and reduce thromboembolic and hemorrhagic complications. However, the prevalence, incidence, risk factors, prognosis and management of AF in patients with thyroid dysfunction are still controversial topics in the literature. Objective: to evaluate the impact of thyroid dysfunction in patients with AF, addressing the following aspects: epidemiology, pathophysiology, diagnosis, treatment and clinical outcomes. Methodology: This review was carried out in accordance with the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) protocol. The PubMed, Scielo and Web of Science databases were searched, using the following descriptors: "atrial fibrillation", "thyroid dysfunction", "hypothyroidism", "hyperthyroidism" and "thyrotoxicosis". Articles published in the last 10 years, in Portuguese, English or Spanish, that addressed the proposed topic were included. Articles that were not original, did not have sufficient data or were not relevant to the research question were excluded. Results: 18 studies were selected. The diagnosis of AF in patients with thyroid dysfunction requires confirmation of the heart rhythm by electrocardiogram (ECG) and assessment of serum thyroid hormone levels (TSH, free T4 and free T3). Treatment of AF in patients with thyroid dysfunction aims to restore and maintain sinus rhythm, control ventricular rate, prevent thromboembolic events, and correct thyroid dysfunction. Therapeutic options include antiarrhythmic drugs, antithyroid drugs, anticoagulant drugs, electrical cardioversion, catheter ablation, and surgical thyroid treatment. The clinical outcomes of AF in patients with thyroid dysfunction are influenced by the type, severity and duration of thyroid dysfunction, as well as rhythm, frequency and anticoagulation control. AF in patients with thyroid dysfunction is associated with a higher risk of arrhythmia recurrence, heart failure, stroke and mortality. Conclusion: Thyroid dysfunction is a frequent and important clinical condition in patients with AF, as it can cause or worsen arrhythmia, as well as increase the risk of complications. Appropriate diagnosis and treatment of thyroid dysfunction can improve AF control and reduce adverse outcomes. However, there are still gaps in knowledge about the epidemiology, pathophysiology, prognosis and management of AF in patients with thyroid dysfunction, which require further studies of high quality and clinical relevance.

Prevalence and Impact of Thyroid Dysfunction in Patients With Chronic Pulmonary Obstructive Pulmonary Disorder: A Systematic Review and Meta-Analysis.

Thyroid gland dysfunction (TGD) has been increasingly recognized as a potential comorbidity in patients with chronic obstructive pulmonary disease (COPD). This study was designed to determine the prevalence of TGD in COPD patients.This systematic review and meta-analysis was conducted according to the guidelines of Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA).To comprehensively identify relevant studies, a systematic search was conducted in major electronic databases, including PubMed, Embase, and the Cumulative Index to Nursing and Allied Health Literature (CINAHIL).The search was limited to English-language studies published after 31December 2000. To determine the prevalence of TGDand assess the impacts, we compared forced vital capacity (FVC) (%), forced expiratory volume in one second (FEV1) (%), partial pressure of oxygen (PaO2) (mmHg), and partial pressure of carbon dioxide (PaCO2) (mmHg) between patients with and without TGD. A total of nine articles were included in this meta-analysis.The sample size of included studies ranged from 50 to 309.The pooled prevalence of TGD in patients with COPD was 45% (95% CI: 25% to 65%). The most common form of TGD was hypothyroidism. The study identified a lack of significant associations between TGD and COPD severity or various characteristics, highlighting the need for future prospective multi-center research, particularly with larger sample sizes to determine the clinical factors and biomarkers affecting the development of thyroid dysfunction in this population.

THYROID FUNCTION ABNORMALITIES IN PATIENTS WITH CHOLELITHIASIS: A HOSPITAL-BASED CROSS-SECTIONAL STUDY

Objective: The objective of the study is to analyze the association between thyroid function abnormalities and cholelithiasis, focusing on the prevalence and types of thyroid dysfunctions in patients with gallstone disease compared to a healthy control group. Methods: A comparative, hospital-based cross-sectional study was conducted in the department of general surgery at a tertiary care medical college. The study included 60 patients diagnosed with cholelithiasis (Group A) and 60 age-matched healthy individuals (Group B). Detailed history, physical examinations, ultrasound imaging, and thyroid function tests (TFTs) were performed. TFTs included measurements of free triiodothyronine (FT3), free thyroxine (FT4), and thyroid-stimulating hormone. Patients were categorized based on thyroid status into euthyroid, subclinical and clinical hypothyroidism, and hyperthyroidism. Statistical analysis was performed using SPSS version 21.0, with significance set at p&lt;0.05. Results: The study revealed a female preponderance in cholelithiasis cases (76.67% in Group A vs. 65.00% in Group B). The mean age and BMI were comparable between the groups. Thyroid function abnormalities were more prevalent in the cholelithiasis group (p=0.0251), with a higher incidence of hypothyroidism compared to the control group. Subclinical hypothyroidism was the most common thyroid dysfunction in cholelithiasis patients. Common complaints in the cholelithiasis group included anemia, menstrual irregularities, skin changes, and weakness. Conclusion: The study demonstrates a significant association between cholelithiasis and thyroid function abnormalities, particularly hypothyroidism. The findings suggest the need for routine thyroid function evaluation in patients with cholelithiasis, which could influence management strategies and improve patient outcomes.

Immune-related thyroid dysfunction in patients with non-small cell lung cancer.

Immune-related thyroid dysfunction (irTD) is a common immune-related adverse event (irAE). The potential biomarkers of irTDs and their impact on the clinical outcomes of patients with non-small cell lung cancer (NSCLC) treated with immune checkpoint inhibitors (ICIs) remain unclear. We aimed to identify potential biomarkers of irTDs and reveal the association between irTDs and the clinical outcomes in patients with NSCLC treated with ICIs. We conducted a retrospective study on 126 patients with NSCLC, who were treated with pembrolizumab, sintilimab, atezolizumab, or camrelizumab, as first-line therapy, at the First Affiliated Hospital, College of Medicine, Zhejiang University, between July 2019 and February 2023. Anti-thyroid antibodies (ATAs), thyroid peroxidase antibody (TPOAb), thyroglobulin antibody (TGAb), serum interleukin-6 (IL-6), thyroid ultrasonography, overall survival (OS), and progression-free survival (PFS) were the main indicators. Most (92.9%) irTD cases occurred no later than one year after ICIs initiation. Patients with irTDs had higher positive rates for ATAs and TPOAb [33.3% vs. 1.3%, and 30.3% vs. 1.3%, both P<0.01, odds ratio (OR) =39.81, and OR =35.46, respectively]. Irregular echo pattern and diffuse changes were more common in patients with irTDs (70.7% vs. 47.2%, and 19.5% vs. 1.4%, P<0.05 and P<0.01, OR =2.70, and OR =17.21, respectively). OS and PFS were similar in patients with and without irTDs (P>0.05). The ATAs, TPOAb, and abnormal thyroid ultrasonographic findings (irregular echo patterns and diffuse changes) are potential biomarkers of irTDs. Patients with NSCLC treated with ICIs (pembrolizumab, sintilimab, atezolizumab, and camrelizumab) who developed irTDs had no advantage in terms of clinical outcomes compared to euthyroid patients.

1943 Thyroid dysfunction and its relationship with cardiometabolic risk in a South American cohort, cross-sectional study

Abstract Introduction A growing body of evidence suggests that metabolic syndrome is associated with endocrine disorders, including thyroid dysfunction. Thyroid dysfunction in patients with metabolic syndrome may further increase the risk of cardiovascular disease, thus increasing mortality. This study was conducted to assess thyroid function in patients with metabolic syndrome and to assess its relationship to components of metabolic syndrome. Methods A cross-sectional study was carried out among 170 geriatric patients. Anthropometric measurements (height, weight, waist circumference) and blood pressure were taken. Fasting blood samples were analysed for glucose, triglycerides, high-density lipoprotein (HDL) cholesterol, and thyroid hormones (triiodothyronine, thyroxine, and thyroid-stimulating hormone). Results Thyroid dysfunction was observed in 31.9% (n = 54) of the patients with metabolic syndrome. Subclinical hypothyroidism (26.6%) was the main thyroid dysfunction, followed by overt hypothyroidism (3.5%) and subclinical hyperthyroidism (1.7%). Thyroid dysfunction was much more common in women (39.7%, n = 29) than in men (26%, n = 25), but not statistically significant (p = 0.068). The relative risk of having thyroid dysfunction in women was 1.525 (CI: 0.983-2.368) compared to men. Significant differences (p = 0.001) were observed in waist circumference between patients with and without thyroid dysfunction and HDL cholesterol that had a significant negative correlation with thyroid-stimulating hormone. Conclusion Thyroid dysfunction, particularly subclinical hypothyroidism, is common among patients with metabolic syndrome and is associated with some components of metabolic syndrome (waist circumference and HDL cholesterol). Analysis of Thyroid Function in ANCA-Associated Vasculitis Patients with Renal Injury.

Previous studies indicated common thyroid dysfunction in various kidney diseases. This study aimed to investigate the thyroid function in patients with antineutrophil cytoplasmic antibody-associated vasculitis (AAV) with renal injury. Briefly, 174 patients diagnosed as having AAV with renal injury and without previous thyroid disease history were included in the retrospective and prospective study. The clinical parameters were collected and compared between different groups. Of the patients included, 24 exhibited normal thyroid function, while 150 had thyroid dysfunction, including 55 (36.67%) with hypothyroidism. Those AAV patients with thyroid dysfunction showed different clinical parameters from those with normal thyroid function. The patients were followed up for a median of 68.6 (64.3; 72.8) months. Those with thyroid dysfunction were more prone to progressing to dialysis dependence compared to the group with normal thyroid function. Logistic regression analysis showed advanced age and decreased albumin as independent risk factors for thyroid dysfunction in patients with AAV. Survival analysis and multivariate Cox regression analysis showed that thyroid dysfunction was a risk factor for AAV patients with renal injury to progress to the endpoint of dialysis dependence. Thyroid dysfunction, predominantly hypothyroidism, was commonly complicated in AAV patients with renal injury. AAV patients with thyroid dysfunction were presented with different clinical parameters and more prone to progressing to dialysis dependence compared to those with normal thyroid function.

P-76 Down Syndrome and Thyroid hormone levels - how low can you go?

Abstract Introduction Down syndrome (DS) is the most common chromosomal condition among live-born infants. It is associated with intellectual disability as well as medical issues ranging from congenital heart disease, obstructive sleep apnea, celiac disease, to endocrinopathies, namely thyroid disorders. The spectrum of thyroid dysfunction in patients with DS include congenital hypothyroidism, subclinical hypothyroidism, acquired hypothyroidism, and hyperthyroidism. Identifying medical comorbidities in these patients can be crucial to optimize their quality of life. Clinical Case This is the case of a male patient, 30 years old, caucasian, institutionalized, with DS and known medical history of esophageal dilation, macroglossia, valvular heart disease with mild aortic insufficiency and left ventricular moderated hypertrophy, macrocytic anemia, and pulmonary embolism 10 months earlier. Due to pallor, somnolence, fatigue, thin and fragile hair, and bradycardia, blood tests were requested and revealed an elevated TSH (385.20 mUI/L, N 0.27-4.20) with undetectable free T4 levels (&amp;lt;0.5 pmol/L, N 12.0-22.0), macrocytic anemia (Hb 11.4 g/dL; MCV 102 fL), renal dysfunction (creatinine 1.73 mg/dL) and hypercholesterolemia (total cholesterol 310 mg/dL; LDL 236 mg/dL). The chest X-ray revealed an heart silhouette enlargement and the echocardiogram showed left ventricular hypertrophy with preserved ejection fraction, diastolic dysfunction, and mild pericardial effusion. The thyroid ultrasound revealed a globally reduced gland, with diffuse hyperechogenicity and heterogeneity, suggesting thyroiditis. The patient was referred to the emergency department and the additional study revealed the presence of anti-TPO and anti-TG antibodies and a severe CK elevation (9280 UI/L, N 46-171). The patient, although lethargic, presented with normal blood pressure, heart rate, and body temperature during the hospital admission. Searching our national health system database, it was found the patient already had TSH elevation with undetectable free T4 levels at least 12 years before, in another hospital, but had lost follow-up there after the blood tests, and therefore remaining undiagnosed and untreated. Oral levothyroxine reposition was started, first with 50 micrograms and then 100 micrograms and the patient was discharged after 5 days with follow-up at the Endocrinology department. Two months later, there were already clinical and laboratory changes, with improved level of consciousness and normalized free T4 levels. Conclusion Although hypothyroidism is a common and widely recognized condition, this case emphasizes how challenging the diagnosis can be in a person with DS. Knowing that this syndrome is associated with several organic dysfunctions, namely endocrine disorders with non-specific clinical manifestations, there must be a high level of suspicion in order to screen, diagnose, and treat the comorbidities that may appear.

Pathophysiological features of thyroid dysfunction in patients with type 2 diabetes and chronic kidney disease (a literature review and own observations)

Background. Diabetes mellitus (DM) is a metabolic di­sease that causes disorders of all types of metabolism. Currently, there is an increasing prevalence of not only DM, but also its serious complications. One of the common chronic complications of DM is diabetic kidney disease (DKD). As of today, it is also known about the effect of DM on other endocrine organs, in particular thyroid gland. Thyroid dysfunction in combination with type 2 DM and DKD are interrelated conditions. In the literature review, the causes, theories of development, stages, course, and criteria for making a diagnosis of DKD are highlighted, probable mechanisms for the development of phenotypes are described. The impact of pathophysio­logical mechanisms of metabolic and secretory disorders on thyroid function has been demonstrated. PubMed and Google Scholar databases were used to search for literature data. The purpose of the study is to investigate the frequency and prevalence of phenoty­pic forms of DKD and the thyroid functional state. Materials and methods. We have analyzed the data of 1,874 patients with type 2 DM who were receiving inpatient treatment at the Lviv Regional Clinical Diagnostic Center, branch of the Center for Endocrinolo­gical Population Health, in 2022 and the first three quarters of 2023. Among them, 56 % were women, 44 % were men. The average age of the patients was 56.1 ± 8.2 years. The frequency and prevalence of phenotypic forms of DKD, the thyroid functional state in such patients were studied. Results. The share of patients with confirmed DKD was 26 % (n = 487). According to the results of the studies, DKD develops by a phenotype of non-albuminuric renal dysfunction in 288 patients (59 %), by an albuminuric phenotype — in 192 cases (39.6 %), and as a progressive decrease in kidney function — in 7 patients (1.4 %). Non-albuminuric renal dysfunction phenotype was 1.5 times more frequent than albuminuric one. Thyroid dysfunction was diagnosed in 166 patients with DKD (34 %). The obtained data confirm the greater prevalence of hypothyroidism as compared to hyperthyroidism in patients with DKD. This allows us to suspect that the structural and functional changes in the kidneys in type 2 DM, which lead to a decrease in the filtration capacity of the kidneys, may be independent of albuminuria. Conclusions. The presence of DKD, regardless of the stage of DM and phenotype, has a direct and indirect effect on the regulation and functioning of the thyroid gland. In turn, adequate production of thyroid hormones is necessary for a balanced metabolism, energy homeostasis and renoprotection. The presence of thyroid dysfunction can be a cause of unsatisfactory control of diabetes and lead to the development of complications.

Prevalence of Thyroid Dysfunction in Patients with Type 2 Diabetes Mellitus in Mumbai, Maharashtra, India: A Cross-sectional Study

Introduction: India bears a high burden of Type 2 Diabetes Mellitus (T2DM) and Thyroid Dysfunction (TD). Though the coexistence of T2DM and TD has been evaluated, such studies from the perspective of metropolitan cities, including Mumbai, are lacking. Aim: To assess the prevalence of TD in patients with T2DM and to evaluate the association of TD with glycaemic control, duration, and complications of T2DM. Materials and Methods: The present observational cross-sectional study was performed in the Department of General Medicine, The Grant Government Medical College and Sir JJ Group of Hospitals, Mumbai, Maharashtra, India, from January 2016 to December 2017. The study involved 220 diagnosed patients with T2DM, of either gender, aged 18 years or more. All patients were evaluated for TD by performing thyroid profile {T3, T4 and Thyroid Stimulating Hormone (TSH)}. The association of TD prevalence with age, gender, Body Mass Index (BMI), duration of T2DM, Glycated Haemoglobin (HbA1c), and complications was assessed. The qualitative and quantitative parameters were compared with Chi- square and independent sample t-test, respectively, with a p-value &lt;0.05 considered statistically significant. Results: The patients were predominantly males, 127 (57.7%) with a mean±Standard Deviation (SD) age and BMI of 63.4±14.02 years and 25.9±4.5 Kg/m2, respectively. The prevalence of TD was 36.8%. The predominant TD in patients with T2DM was subclinical hypothyroidism (75.9%). TD was significantly associated with advancing age (p-value=0.0167). However, gender and BMI were not significantly associated with TD. Poor glycaemic control was significantly associated with subclinical hypothyroidism (p-value=0.0255). The duration of T2DM was significantly associated with the prevalence of subclinical hypothyroidism (p-value=0.0144), primary hypothyroidism (p-value=0.0257), and subclinical hyperthyroidism (p-value=0.0310). TD was found to be significantly associated with both micro and macrovascular complications of T2DM (all p-values &lt;0.05). Conclusion: In Mumbai, the prevalence of T2DM-associated TD was 36.8%. TD was significantly associated with advancing age, duration of T2DM, high HbA1c levels, and both micro and macrovascular complications.

Prognostic impact of thyroid dysfunction in patients with incident heart failure: a nationwide study

Abstract Background Thyroid hormones directly affect the cardiovascular system. Thyroid dysfunction can cause a variety of haemodynamic alterations and has been associated with an increased risk of atrial fibrillation (AF), heart failure (HF), and all-cause mortality. However, a paucity remains regarding knowledge on the prognostic impact of thyroid dysfunction in patients with HF. Purpose To examine the impact of the whole spectrum of thyroid dysfunction in patients with HF on the associated risk of AF, HF admissions, and all-cause mortality. Methods This Danish cohort study included patients with a first-time HF diagnosis from 2000–2021. According to blood samples within 6 months prior to the HF diagnosis, the study population was categorized based on thyroid function in 1) overt hypothyroidism, 2) subclinical hypothyroidism, 3) euthyroidism, 4) subclinical hyperthyroidism, and 5) overt hyperthyroidism. With up to one year of follow-up from the date of HF diagnosis, we used Aalen-Johansen estimator to examine the cumulative incidence frequency (CIF) of AF and HF admissions and Kaplan-Meier estimates to assess all-cause mortality. Multivariable Cox proportional hazard analysis was performed to investigate differences in outcomes among groups. Results Of the 7,380 individuals included in this study (56.6% female, median age 76.5 years [p25-p75: 67.9-84.1]), 102 (1.4%) had overt hypothyroidism, 1,706 (23.1%) had subclinical hypothyroidism, 4,374 (59.3%) were euthyroid, 691 (9.4%) had subclinical hyperthyroidism, and 507 (6.9%) had overt hyperthyroidism. Compared with euthyroid patients, the adjusted one-year hazard ratio (HR) for AF was 0.77 (95% CI, 0.49-1.20) for overt hypothyroidism, 1.01 (95% CI, 0.90-1.13) for subclinical hypothyroidism, 1.18 (95% CI, 1.01-1.38) for subclinical hyperthyroidism, and 1.19 (95% CI, 1.01-1.39) for overt hyperthyroidism (Figure). The adjusted one-year HR for HF hospitalizations was 1.51 (95% CI, 1.06-2.14) in patients with overt hypothyroidism, 1.07 (95% CI, 0.96-1.20) in patients with subclinical hypothyroidism, 0.99 (95% CI, 0.84-1.16) in patients with subclinical hyperthyroidism, and 1.26 (95% CI, 1.06-1.50) in patients with overt hyperthyroidism. The adjusted one-year HR for mortality was 1.81 (95% CI, 1.25-2.63) in patients with overt hypothyroidism, 1.25 (95% CI, 1.10-1.42) in patients with subclinical hypothyroidism, 1.15 (95% CI, 0.96-1.37) in patients with subclinical hyperthyroidism, and 1.46 (95% CI, 1.20-1.78) in patients with overt hyperthyroidism. Conclusion In patients with HF, overt and subclinical hyperthyroidism was associated with an increased rate of AF. Both hypo and hyperthyroid states were associated with increased risk of HF admission and mortality. Increased awareness regarding thyroid status in patients with HF is warranted.

Pattern of Thyroid Dysfunction in Type II Diabetes Mellitus Patients in a Tertiary Care Center: A Cross-Sectional Study

Background: Early identification and intervention of thyroid dysfunction which occurs in association with type-II diabetes mellitus may significantly reduce the risk of adverse cerebrovascular and cardiovascular events in such patients. Hence, this study analyzed the pattern of thyroid dysfunction in T2DM (Type 2 Diabetes mellitus) patients. Methods: The present cross-sectional study was done on 250 T2DM patients, who visited the department of General Medicine in a tertiary care teaching center. Medical history was recorded and venous blood samples were collected for investigations (HbA1C (Hemoglobin A1C), FBS (Fasting blood sugar), PPBS (Postprandial blood sugar), TSH (Thyroid stimulating hormone), T4 (Tetraiodothyronine), anti-TPO (Thyroid peroxidase), and fasting lipid profile). Thyroid dysfunction in patients with T2DM was considered as the primary outcome variable. P value &lt;0.05 was considered statistically significant. Data were analyzed using coGuide software, V.1.03. Results: Females (55.6%) outnumbered males (44.4%) in this study. The prevalence of thyroid dysfunction in our study was 23.6% (95%CI 0.184 to 0.293). Subclinical hypothyroidism was found in 67.79% (95%CI 0.543 to 0.793) participants, overt hypothyroidism in 27.11% (95%CI 0.163 to 0.402), and hyperthyroidism in 5.10% (95%CI 0.010 to 0.141). Females (84.6%) had significantly higher prevalence of anti-TPO positivity compared to males (15.4%)(p=0.013). Conclusions: Findings of this study showed that T2DM patients have higher prevalence of thyroid dysfunction with predominance of subclinical hypothyroidism. Hence, this study emphasizes the importance of annual investigation of TSH levels in all the patients with T2DM. EVALUATION OF THYROID PROFILE AND ITS PROGNOSTIC VALUE IN PATIENTS OF ACUTE CORONARY SYNDROME

Objective: The objective of the study is to study the association of thyroid dysfunction with acute coronary syndrome (ACS), its complications and duration of hospital stay. Methods: This prospective comparative study was done on 100 patients divided equally into ACS patients with control (normal thyroid function) and ACS patients with thyroid dysfunction admitted in a tertiary care center, Udaipur, Rajasthan with ACS during the period of February 2021–July 2022. We studied the prevalence, prognostic factor of thyroid hormone in these patients. Results: The mean ejection fraction of the control group was 51.16±11.72%, of hypothyroid group was 49.00±13.55% and of hyperthyroid group was 51.12±13.78%. 29 cases of cardiac failures, 38 cases of arrhythmias, and 32 cases of major adverse cardiac events were observed. 56% of patients required thrombolysis, out of which 67.86% were from the subclinical hypothyroid group. The mean hospital stay in control group was 4.53±1.55, in hypothyroid group was 5.27±1.84 and in hyperthyroid group was 7.00±1.92.18% mortality. Conclusion: Patients with acute myocardial infarction, initially develop alteration in thyroid hormone levels, which is possibly a compensatory mechanism to reduce the metabolic demand of the heart, by reducing myocardium contraction and cardiac output but may cause cardiac failure and higher rates of cardiovascular complications like arrhythmias and death in some of the patients. Thyroid dysfunctions in patients with ACS may also need a longer duration of intensive care and close monitoring during follow-up.

The frequency of thyroid dysfunction in patients with a diagnosis of depressive disorder.

Thyroid dysfunctions are among the most common endocrine disorders in society. An increase or decrease in thyroid hormone levels may present with neurological and/or psychiatric symptoms. In this study, we aimed both to determine the prevalence of this disorder in our region by determining the frequency of thyroid dysfunction in patients diagnosed with major depressive disorder in our outpatient clinic and to raise awareness during the evaluation process of patients. Thyroid-stimulating hormone (TSH) levels of 1035 patients diagnosed with major depressive disorder in our hospital between January 2020 and January 2022 were retrospectively scanned from the hospital information management system and those outside the reference ranges (0.38-5.33 mIU/L) were determined. It was observed that TSH was not within the reference ranges in approximately 7% of the patients diagnosed with depressive disorder. 1035 patients were included in the study. When the blood results of 1035 patients included in the study were examined retrospectively, 32 of them had TSH values below 0.38 mIU/L. TSH value was found to be above 5.33 mIU/L in 44 of them. Obtained data have shown that thyroid dysfunctions can be encountered frequently in patients presenting with depressive complaints. It is thought that the evaluation of patients with depressive complaints in terms of thyroid dysfunction, and the treatment of the underlying thyroid dysfunction will contribute to the regression of psychiatric symptoms.

Association of severe autoimmune hypothyroidism with reversible proteinuria: A case report

Hypothyroidism is a prevalent medical condition characterized by an underactive thyroid gland, resulting in insufficient thyroid hormone production. This hormone plays an important role in regulating the body's metabolism, so when its levels are low, it can lead to various metabolic disturbances. In this article, we present a case report of a 50-year-old female with severe hypothyroidism who exhibited several symptoms of renal impairment and hypothyroidism. She had proteinuria along with elevated lipid levels. Following a diagnosis of Hashimoto's thyroiditis, the patient was treated with levothyroxine, resulting in a remarkable reversal of her proteinuria and other symptoms. The lipid profile also showed significant improvement without the need for cholesterol-lowering medications. This case report highlights the importance of recognizing and managing thyroid dysfunction in patients with renal abnormalities. It provides valuable insights into the complex interplay between thyroid function, renal abnormalities, and lipid levels.

Prevalence and clinical correlates of thyroid dysfunction in first-episode and drug-naïve major depressive disorder patients with metabolic syndrome

BackgroundFew studies have investigated the relative factors of thyroid dysfunction in major depressive disorder (MDD) patients with Metabolic syndrome (MetS). This study aimed to explore the prevalence and related factors associated with thyroid dysfunction in drug-naïve (FEDN) MDD patients with MetS. Methods1718 FEDN MDD patients were recruited and their demographic data, clinical data were collected. Various biochemical indicators including fasting blood glucose (FBG), blood lipids and thyroid hormones were measured. The 17-item Hamilton Rating Scale for Depression (HAMD-17), 14-item Hamilton Anxiety Rating Scale (HAMA-14) and positive subscale of the Positive and Negative Syndrome Scale (PANSS) were used to assess clinical symptoms. ResultsAmong FEDN MDD patients, MetS was an independent risk factor for TSH abnormality (P < 0.001, Adjusted OR = 3.77, 95%CI: 2.82–5.05). In patients with MetS, those with TSH abnormality had significantly longer duration of illness, higher HAMD, HAMA, and PANSS positive subscale scores, higher levels of TC, LDL-C, blood glucose, pressure, lower levels of HDL-C, and a higher probability of suicide attempt (all P < 0.01). ConclusionsMetS is significantly associated with thyroid dysfunction in patients with FEDN MDD. Related factors for thyroid dysfunction include a number of clinical indicators and psychiatric symptoms.
