Do marked discharge summaries with recommendation text boxes enhance patient safety? A nationwide survey.

Danish hospital physicians are required to mark their discharge summaries addressing whether the patient's general practitioner (GP) is recommended to follow up as well as suggest follow-up actions. To investigate whether a new form of discharge summaries may contribute to improve the perceived patient safety following transition from hospitals to general practice. This paper reports data from a questionnaire sent to a representative sample of GPs in Denmark. A questionnaire was prepared for GPs based on background material, focus group interviews and discussions with relevant professionals. It was subsequently pilot tested by fellow researchers and GPs and revised prior to the presently reported survey. Of 310 participating GPs, 197 (63%) 'totally agree' or 'partly agree' that the marked discharge summaries with a recommendation text box contribute to a better handover to general practice, and 223 (72%) 'totally agree' or 'partly agree' that they improve patient safety. The majority of responding GPs believe that the marked discharge summaries with a recommendation text box enhance patient safety and facilitate the transition of care to general practice following hospital discharge.

Managing Complicated Nontuberculous Mycobacteria Infections in Plastic Surgery.

Nontuberculous mycobacteria (NTM) infections after cosmetic surgery have become an increasing concern. These infections are often initially misdiagnosed and treated with standard antibiotic regimens, which fail to resolve the underlying infection, leading to prolonged patient suffering. In this case study, we describe a chronic wound infection caused by Mycobacterium abscessus subsp. bolletii after a muscle-repair abdominoplasty. This case illustrates the diagnostic and therapeutic challenges plastic surgeons face in successfully treating such infections. Initial obstacles included the isolation of co-contaminating bacteria that masked the NTM infection, the use of antibiotics ineffective against the specific NTM species, and the failure to identify the infection source. In this instance, contaminated skin marker ink used to mark the rectus muscle, combined with a nonabsorbable (permanent) suture for muscle repair, led to the development of a biofilm that acted as a persistent reservoir for the infection, resistant to antibiotic treatments. Complete resolution was achieved only after evaluation by a plastic surgeon experienced in treating NTM infections and the subsequent removal of the permanent suture. The delayed suture removal contributed to a 15-month recovery period. This case underscores the importance of early recognition of NTM infections after cosmetic procedures. By sharing this case, we aim to raise awareness of NTM infections and help prevent future cases of misdiagnosis and prolonged antibiotic treatments. Diffusion model conditioning on gaussian mixture model and negative gaussian mixture gradient

Diffusion models (DMs) are a type of generative model that has had a significant impact on image synthesis and beyond.They can incorporate a wide variety of conditioning inputs—such as text or bounding boxes—to guide generation. In this work, we introduce a novel conditioning mechanism that applies Gaussian mixture models (GMMs) for feature conditioning, which helps steer the denoising process in DMs. Drawing on set theory, our comprehensive theoretical analysis reveals that the conditional latent distribution based on features differs markedly from that based on classes. Consequently, feature-based conditioning tends to generate fewer defects than class-based conditioning. Experiments are designed and carried out and the experimental results support our theoretical findings as well as effectiveness of proposed feature conditioning mechanism. Additionally, we propose a new gradient function named the Negative Gaussian Mixture Gradient (NGMG) and incorporate it into the training of diffusion models alongside an auxiliary classifier. We theoretically demonstrate that NGMG offers comparable advantages to the Wasserstein distance, serving as a more effective cost function when learning distributions supported by low-dimensional manifolds, especially in contrast to many likelihood-based cost functions, such as KL divergences.

Factors associated with Ontario dairy producers' management and care of down dairy cattle.

The objective of this study was to better understand current management practices for down cows in Ontario, Canada, and to identify factors associated with the adoption of acceptable practices. An online survey was distributed to all dairy producers in Ontario, Canada (n = 3,367) and was available from November 2020 to March 2021, inclusive. Dairy producers were identified through their provincial dairy organization and contacted via email, and the survey was also promoted via social media. The survey was comprised of 134 questions, 31 of which were related to down cow management. Descriptive statistics were evaluated, and 2 logistic regression models were generated using Stata 17, exploring factors associated with 1) relocating down cows with hip lifters and 2) assisting cows to stand within an hour after discovering a down cow. A total of 226 producers responded (7.4%). Participants were predominantly male (68%), farm owners (78%), and 30-39 years old (29%). Producers reported relocating down cows with a boat or sled (32.6%), front-end loader bucket (31.4%), hip lifters (28.0%), or 'other' (with a text box to further describe) (8.0%). The median time to relocating a down cow after identifying her was 1 h (range 0 - 17h). Farms that relocated a down cow sooner after identifying her as down, were more likely to use appropriate methods to move the cow. However, we also found that farms that provided feed and water sooner to down cows, were more likely to use an inappropriate method (hip lifters) to move her. Farms that used hip lifters to move cows had higher odds of assisting a cow to stand within an hour following the discovery of recumbency. Additionally, producers who waited longer to relocate a down cow were less likely to assist the cow to stand within one hour of finding them down. Research has identified effective management practices for down cows, yet there remains a gap in understanding the implementation and the decision-making process of producers. Data from this study will be helpful in designing future research that further explores the barriers and motivations of producers when implementing evidence-based management plans to care for down dairy cows and may help inform current industry extension efforts.

Improving discharge summaries from hospital with a brief recommendation text box - results from a nationwide survey.

Almost ¾ of the general practitioners (72%) 'totally or partly agree' that the recommendation text box is easy to find. In addition, our results show significant differences between software used by the GPs. Almost 2/3 (63%) 'totally agree' or 'partly agree' that the recommendation text box provides brief and precise information about the recommended follow-up. GPs generally find that the recommendation text box provides them with brief and precise information about the recommended follow-up. In addition, the software used by the GPs has a significant influence on how the recommendation text box is presented.

“He’s used drugs - he’s biased! He’s not a drug user - what would he know!”: A cross-sectional, online study of drug researchers’ experiential knowledge of drug use and disclosure

IntroductionDespite the recognized value of experiential knowledge, drug use and disclosure of drug use within the drug research community is rarely discussed or studied. MethodsWe distributed a cross-sectional online survey using targeted recruitment. Researchers provided information on drug use, disclosure of use (or abstinence) professionally, and their impact via write-in text boxes. We used the general inductive approach to analyze the data. ResultsOf the sample (n=669, 43 countries), 52 % were cisgender women, 89 % had post-graduate education, and 79 % worked in academia. Most (86 %) reported lifetime drug use and 47 % past 3-month use. Among 557 researchers who used drugs, 59 % disclosed their use to institutional colleagues, 59 % to colleagues outside their institution, 25 % to research participants, and 11 % in their research/scholarship. Themes included frequency; context; meaning of drug use disclosure personally, professionally, and socially; and how drug use experience and disclosure informs research. Respondents connected their concerns about disclosure in research with issues of social identity, professional risk, and the role of stigma related to lived experience. Some respondents felt that such concerns reinforce a vacuum, noting that the inability to disclose drug use limits research questions and the knowledge base overall. DiscussionOur findings support the dichotomy of thought surrounding the lived experience of drug use: “[They’ve] used drugs- [they’re] biased!” and “[They’re] not a drug user-what would [they] know!” Our findings provide an opportunity to reflect upon our positionality and the impact researchers’ own drug use may have on the field.

P006 A new digital front door for existing rheumatology patient queries

Abstract Background/Aims NICE guidelines recommend people with rheumatological conditions need rapid access to specialist care to manage flares and to advise on medication. With growing demand for NHS services, rheumatology advice lines are coming under pressure. We carried out an audit on our advice line queries over four clinics in summer 2021 with a view to developing a triaging system to improve response time. Over four advice line clinics, there were 44 calls averaging 13 minutes per call. The response time was two to three weeks. 34% of calls were deemed to be inappropriate. Inappropriate calls were considered ones that could be better directed elsewhere. Methods In 2022 we started working with DrDoctor, a digital health company to design a digital nurse advice line form to redirect inappropriate calls away from the nurse advice line call backs. This involved designing a digital pathway addressing common inappropriate themes from our audit including redirection of calls to secretaries, patient booking team, prescription management and a digital booking form for phlebotomy appointments. The new digital tool enabled us to expand the service and we used the opportunity to adopt patient assessment tools and educational videos as part of the service. We promoted the digital pathway through patient information leaflets, department website and sent a text with the link to all rheumatology patients. We continued to offer the traditional contact telephone number into the service as well. Results The digital nurse advice line went live at the end of July 2023 and after two months the response time has reduced to 1.5- 2 weeks. Since then there have been 529 form submissions from 417 unique patients. 47% of submissions were inappropriate and redirected. 97 patients called to book a blood test and 76 patients opted for the online self-service clinic. 151 patients had a query about their condition. 50% of patient had RA or inflammatory arthritis. 5% AS. 5% SLE. 17% PsA. 23% selected ‘other’ and a free text box captured conditions such as fibromyalgia, osteoporosis, PMR and OA. 26% of patients with rheumatoid arthritis opted to see the educational video on self-examination and 17% watched the educational video on rheumatoid arthritis. Based on 251 submissions that were inappropriate for the advice line, assuming each inappropriate call takes an average of nine minutes based from our previous audit, 39 hours of phone time was saved from avoiding booking inappropriate nurse advice line appointments in two months. Conclusion The digital rheumatology advice service is an efficient and effective method of responding to patient queries and can be a source of patient education and self assessment. Explaining the Mechanism Behind mRNA Vaccines Influences Perceived Vaccine Effectiveness but not Vaccination Intentions

Vaccine effectiveness and safety concerns can prevent people from receiving their first Covid-19 vaccines or boosters. Understanding the vaccine mechanism may lead people to perceive vaccine effectiveness appropriately. This study tested whether helping people understand the vaccine’s mechanism could improve their perceived vaccine safety and effectiveness. In a preregistered study, N = 1,548 unvaccinated or non-boosted participants were randomly presented with one of three communication formats: a fact box (a benefit-risk profile in tabular format; control condition), an expository text (i.e., a purely factual explanation) plus fact box, or an analogy plus fact box. Participants rated the vaccine’s effectiveness in preventing a Covid-19 disease, their perceived risk of getting vaccinated, and their intention to get vaccinated or boosted (depending on their vaccination status). Reading either additional text about the vaccines’ mechanism increased participants’ effectiveness ratings for the vaccine to prevent Covid-19 but did not affect risk ratings or vaccination intentions. The participants’ vaccine-related perceptions and intentions did not differ between the two text types. Elaborating on the vaccine’s mechanism of protection, in addition to presenting the benefit-risk profile of a vaccine, can lead people to perceive the vaccine effectiveness as slightly higher, yet it is insufficient to increase vaccination intentions.

The seed data of the crops are collected here, with the appropriate parameters like temperature, humidity and moisture content, which helps the crops to achieve a successful growth. In addition as the software, a mobile application for Android is being developed. The users are encouraged to enter parameters like temperature and their location will be taken automatically in this application in order to start the prediction process

Organology Analysis of Dol Musical Instruments as a Mathematics Learning Media Based on the Local Wisdom of Bengkulu in Elementary School

The research was held back by the unique background of the Dol instrument as ritual music at the ceremony of the Tabut and the identity of the Bengkulu traditional music. The purpose of the study is to describe the organology of the Dol instrument and its method of production and describe the design of the implementation of musical instruments on mathematics learning materials in elementary schools in both physical and sociocultural detail. The theory used is ethnomusicology and mathematics learning media. The method used is case analysis, which requires a qualitative and quantitative descriptive approach to the organology of the Dol musical instrument and learning media. The data sources used are their sources and a validator with observation techniques interview techniques, and documentation techniques. The validity of data uses their types of triangulation (technique, source, and time). The result of this research was either a text box or a materials module. The result of the Dol instrument’s organological analysis as a media learning mathematics based on local cultural wisdom at the elementary school is expected to assist every teacher in educating students in understanding the preservation of local culture and implanting the local area’s cultural arts character. Besides, the results of this study will contribute to the literacy and media sources of common education and general education.

Purchase intentions in a chatbot environment: An examination of the effects of customer experience

Research background: Chatbots represent valuable technological tools that allow companies to improve customer experiences, meet their expectations in real time, and provide them with personalized assistance. They have contributed to the transformation of conventional customer service models into online solutions, offering accessibility and efficiency through their integration across various digital platforms. Nevertheless, the existing literature is limited in terms of exploring the potential of chatbots in business communication and studying their impact on the customer's response. Purpose of the article: The main objective of this study is to examine how consumers perceive chatbots as customer service devices. In particular, the paper aims to analyze the influence of the dimensions of “Information”, “Entertainment”, “Media Appeal”, “Social Presence” and “Risk for Privacy” on the “Customer Experience” and the latter on the “Purchase Intention”, under the consideration of the Uses and Gratifications Theory. Moderations due to Chatbot Usage Frequency for some of the relationships proposed are also analyzed. Methods: An empirical study was performed through a questionnaire to Spanish consumers. The statistical data analysis was conducted with R software through the lavaan package. To test the hypotheses from the conceptual model a structural equation modelling approach was adopted. Findings & value added: The results obtained identify the main characteristics of chatbots that can support brands to effectively develop their virtual assistants in order to manage their relational communication strategies and enhance their value proposal through the online customer journey. Findings demonstrate the contribution that chatbot dimensions make to the online consumer experience and its impact on the purchase intention, with the consideration of the moderating effect exercised by the user's level of experience (novice vs. experienced) with the use of chatbots. Regarding managerial implications, this research offers recommendations for e-commerce professionals to manage chatbots more effectively. The “Entertainment” and “Social Presence” dimensions can be operationalized at a visual (e.g., appearance of the avatar and text box, use of designs aligned with the website) and textual level (e.g., style and tone of voice, use of expressions typical of the target audience) to generate a feeling of proximity with the chatbot and facilitate its adoption. “Media Appeal” requires that the chatbot be easy to use, effective, and accessible, to facilitate its usability. Finally, mitigation of “Privacy Risk” concerns should be achieved by presenting an appropriate privacy policy and requesting permission for the use of customers’ private information.

The study analyzed user preferences and expressive characteristics of design components for vertically oriented educational video content on vertical media platforms. Through a literature review, theoretical background was established, and design components were derived. A case study on content from the Photoshop YouTube account and focus group interviews (FGI) involving five experts helped design evaluation criteria. An experiment with 48 participants measured their preferences and the importance of characteristics for each component. Results showed clear preferences for certain design components like screen, caption font, and sound. Specific expressive characteristics deemed important included 9:16 aspect ratio, top and bottom placement, outline clarity, text box readability, and voice clarity. Subsequent research aims to provide foundational data for vertically oriented educational video content production on vertical media platforms.

ViTEraser: Harnessing the Power of Vision Transformers for Scene Text Removal with SegMIM Pretraining

Scene text removal (STR) aims at replacing text strokes in natural scenes with visually coherent backgrounds. Recent STR approaches rely on iterative refinements or explicit text masks, resulting in high complexity and sensitivity to the accuracy of text localization. Moreover, most existing STR methods adopt convolutional architectures while the potential of vision Transformers (ViTs) remains largely unexplored. In this paper, we propose a simple-yet-effective ViT-based text eraser, dubbed ViTEraser. Following a concise encoder-decoder framework, ViTEraser can easily incorporate various ViTs to enhance long-range modeling. Specifically, the encoder hierarchically maps the input image into the hidden space through ViT blocks and patch embedding layers, while the decoder gradually upsamples the hidden features to the text-erased image with ViT blocks and patch splitting layers. As ViTEraser implicitly integrates text localization and inpainting, we propose a novel end-to-end pretraining method, termed SegMIM, which focuses the encoder and decoder on the text box segmentation and masked image modeling tasks, respectively. Experimental results demonstrate that ViTEraser with SegMIM achieves state-of-the-art performance on STR by a substantial margin and exhibits strong generalization ability when extended to other tasks, e.g., tampered scene text detection. Furthermore, we comprehensively explore the architecture, pretraining, and scalability of the ViT-based encoder-decoder for STR, which provides deep insights into the application of ViT to the STR field. One of them is making teaching materials at TPQ Nurul Jihad which requires supporting applications so that the displayed learning can be understood because currently some of the teaching materials displayed are still using conventional methods, using applications Microsoft Powerpoint in making TPQ teaching materials it is still not maximally used because some TPQ educators still don't understand the use of the applications used, so the service team make training Microsoft Powerpoint. This is intended so that educators are more familiar with the use of the application Microsoft Powerpoint for presentation purposes. The method used in providing education about the use Microsoft Powerpoint is a lecture to provide an explanation of theory and simulation, questions and answers as well as evaluation tests for practical methods. The results of this community service stated that the beginning of the teacher meeting had several obstacles in moving and changing sizes text box as well as shape because they are not used to using the laptop keypad. However, after being guided by the service team, these obstacles were minimized until finally the educators were able to work with text and shape. So, it can be said that this service goes according to the target of the service team.

Personal space invasion to prevent cyberbullying: design, development, and evaluation of an immersive prevention measure for children and adolescents

The previous work on cyberbullying has shown that the number of victims is increasing, and the need for prevention is exceptionally high among younger school students (5th–9th grade). Due to the omnipresence of cyberattacks, victims can hardly distance themselves psychologically, thus experience an intrusion in almost all areas of life. The perpetrators, on the other hand, feel the consequences of their actions even less in cyberspace. However, there is a gap between the need and the existence of innovative prevention programs tied to the digital reality of the target group and the treatment of essential aspects of psychological distance. This article explores the design space, feasibility, and effectiveness of a unique VR-based cyberbullying prevention component in a human-centered iterative approach. The central idea is reflected in creating a virtual personal space invasion with virtual objects associated with cyberbullying making the everyday intrusion of victims tangible. A pre-study revealed that harmful speech texts in bright non-removable message boxes best transferred the psychological determinants associated with a personal space invasion to virtual objects contextualized in cyberbullying scenarios. Therefore, these objects were incorporated into a virtual prevention program that was then tested in a laboratory study with 41 participants. The results showed that the intervention could trigger cognitive dissonance and empathy. In the second step, the intervention was evaluated and improved in a focus group with the actual target group of children and adolescents. The improved application was then evaluated in a school workshop for 5 days with 100 children and adolescents. The children understood the metaphor of virtual space invasion by the harmful text boxes and reported the expected psychological effects. They also showed great interest in VR. In summary, this paper contributes to the innovative and effective prevention of cyberbullying by using the potential of VR. It provides empirical evidence from a laboratory experiment and a field study with a large sample from the target group of children and adolescents and discusses implications for future developments.

Validating a measure of graph selection and graph reasoning for dynamic situations

Using a mixed methods approach, we report results from the evaluation and validation stages of a fully online Measure of Graph Selection and Reasoning for Dynamic Situations, implemented with undergraduate college algebra students across three U.S universities. The measure contains six items; each includes a video animation of a dynamic situation (e.g., a fishbowl filling with water), a declaration of understanding, four Cartesian graphs from which to select, and a text box for explanation. In the evaluation stage, we demonstrate usability and content validity, drawing on individual cognitive interviews (n = 31 students). In the validation stage (n = 673 students), we use Rasch modeling to evidence reliability and internal structure, establishing a continuum of item difficulty and confirming the viability of a partial credit scoring approach for graph selection. Rasch results provide statistical support that the theorized graph reasoning framework (Iconic, Motion, Variation, Covariation) from Johnson et al. (2020) forms a hierarchical scale.

A Longitudinal Test of Political Self-Effects on Social Media

ABSTRACT This registered report details a longitudinal experiment testing political self-effects – changes in issue-related attitudes and indicators of political self-concept – resulting from political expression on social media. We also tested whether such effects were due to the composition or release of political messages on social media over time. American adults (N = 576), logged on to a fictional social media platform and engaged with discussions involving one of two political issues at three time points over a week. We examined whether participants who wrote a comment about the issue publicly (on the social media platform) or privately (in a text box) experienced changes in their issue-related attitudes or political self-concepts (compared to a control condition). Overall, we did not find consistent main effects of message composition or release at individual timepoints. Instead, message composition sustained issue-related attitudes over time (vs. control). Interaction analyses found that among those with high baseline issue interest, expression (vs. control) led to decreased issue interest and importance over time. Among those high in baseline political interest, message release (vs. control) increased positive issue-related attitudes. We outline an agenda for studying political self-effects that addresses, a) their cumulative nature, b) conditional effects, and c) the challenge of endogeneity.

Agroforestry in the UK: exploring consumer knowledge and interest

PurposeThis paper's purpose is to explore the current interest and knowledge that UK consumers hold around agroforestry. Despite the many reported benefits of agroforestry systems, uptake in the UK, as well as other temperate nations, has been low. As the consumer has a role to play in the transition of agriculture to methods that are more environmentally friendly it is vital to have an understanding of their perceptions. Yet to date no work has looked at agroforestry from the perspective of the UK consumer.Design/methodology/approachAn online survey was conducted using a convenience sample accessed by floating a link through social media and messaging apps. The survey was also shared to the members of a private Facebook group associated with an organic vegetable box service. A mix of multiple choice and open text boxes were used. The survey received 139 responses.FindingsNon-parametric tests indicate that this sample of UK consumers would be mostly likely to buy, and willing to pay more for, agroforestry produce; and the sample showed a split group regarding familiarity. Inductive thematic analysis of the qualitative data highlighted some important barriers to the purchase as well as capturing a snapshot of this sample's perceptions.Originality/valueThis paper presents, to the authors knowledge, the first set of data regarding a sample of UK consumers' perspective of agroforestry produce. The findings could bolster producers' confidence in adopting agroforestry practices, but also highlight the need for policymakers to bolster consumer support through parallel means.

Surveying the landscape: First Nations, Métis, and Inuit cancer resources in radiation therapy

Introduction/BackgroundAlberta Health Services (AHS) is Canada's largest provincial integrated health care authority. AHS services over four million people with over 100,000 employees. In 2018, AHS introduced Indigenous training modules which were mandatory for all staff. These eight modules included topics detailing residential schools, Indian hospitals, The Indian Act, The 60′s Scoop, Alberta's Indigenous populations, present day realities, including customs, practices and traditional medicines. Limited research exists in general in the field of radiation therapy with Canadian Indigenous populations. No formal research exists on the effectiveness of these AHS training modules. MethodsA mixed methods survey was developed and distributed by the radiation therapy managers to practicing radiation therapists in the four cancer centers in Alberta. The survey contained both drop down and open text boxes. ResultsSurvey respondents indicated that 90 % of radiation therapists completed the AHS Indigeneous training modules. However, 50 % of the respondents did not know what resources are available for their Indigenous patients. Furthermore, only 67 % felt that the information provided in the modules created enough comfort for them to have a conversation with Indigenous patients with cancer and their families. DiscussionRespondents indicated that although they felt knowledgeable and educated from the training modules, this did not necessarily translate into daily clinical practice. This included a lack of confidence to lead a conversation, knowing what resources are available and how to document if patient education material was given. ConclusionCreation of an Indigenous specific oncology module highlighting both resource availability and cultural customs that may be part of a person's cancer journey, may be of benefit to health care providers providing radiation treatment. Having Elders and Knowledge Keepers be part of the development of an oncology module may help create mutual understanding for both the cancer patient and their families and health care providers. This research suggests that the current cultural competency training for radiation therapists at AHS may not be enough for radiation therapists to feel comfortable with translating learnings from Indigenous online modules into their practice.
