Indication: Pembrolizumab, in combination with trastuzumab, fluoropyrimidine- and platinum-containing chemotherapy, is indicated for the first-line treatment of adult patients with locally advanced unresectable or metastatic HER2-positive gastric or gastroesophageal junction (GEJ) adenocarcinoma, whose tumours express PD-L1 (combined positive score [CPS] ≥ 1), as determined by a validated test.

Industry specialisation of the Kemerovsk region – Kuzbass: trends and problems that affect economic growth and development of the region

The article is devoted to studying the influence of industry specialisation of the Kemerovo region – Kuzbass on the economic growth of the entity. The purpose of the study is to identify the most significant trends and problems of the industry specialisation that affect the development of the region. Using methods of scientific knowledge – description, synthesis and generalisation of the data obtained; literature review, graphical method – the following has been conducted: analysis of the dynamics of structural changes in the gross regional product (hereinafter referred to as GRP) of the Kemerovo region – Kuzbass; assessment of trends in the movement of main industry indicators and indicators of regional industry. The information base of the study consists of foreign and domestic publications devoted to the analysis of the GRP sectoral structure; statistical data from the Federal State Statistics Service and its territorial authority for the Kemerovo Region – Kuzbass. Based on the analysis of the structural changes dynamics, an increase in the contribution of the raw materials sector to the country’s gross domestic product and an increase in the share of mineral extraction in the industry structure of the GRP of the Kemerovo region – Kuzbass have been revealed. The trend of decrease in the industrial production index is analysed, it is shown that it has a trend similar to the dynamics of the decline in mining production, and the reasons for the decline are described. The strengthening trend of economic growth in the Kemerovo region – Kuzbass based on the raw model has been determined. East Asian models of economic growth are considered. The expediency of using international experience in the design of high-tech industries on the basis of private investment in the research and development sector, and its adaptation to the realities of the development of Russian regions is indicated. A conclusion is drawn about the need to support the technological sovereignty of the country and the transition of the development of the Kemerovo region – Kuzbass to a non-primary model.

The assessments inform non-binding recommendations that help guide the reimbursement decisions of Canada’s federal, provincial, and territorial governments, with the exception of Quebec. This review assesses elranatamab (Elrexfio), 40 mg/mL, solution for subcutaneous injection supplied as 76 mg/1.9 mL (40 mg/mL) in a single-dose vial and 44 mg/1.1 mL (40 mg/mL) in a single-dose vial. Indication: For the treatment of adult patients with relapsed or refractory multiple myeloma who have received at least 3 prior lines of therapy, including a proteasome inhibitor, an immunomodulatory agent, and an anti-CD38 monoclonal antibody, and who have demonstrated disease progression on the last therapy.

Analysis of appeals of the population of the Nizhny Novgorod region to the territorial health management authority during the COVID-19 pandemic

Introduction. Appeals of the population are important indicators of the realization of patient rights. Therefore, the Ministry of Health of the Nizhny Novgorod region pays great attention to the analysis of their causes. The purpose of this study was to study the impact of the COVID-19 pandemic on the number and reasons for written appeals from the population to the territorial health authority of the Nizhny Novgorod region regarding the medical care provided. The materials were 37,898 citizens’ appeals over 2019–2021. Results. The number of written appeals in 2109 was 4916, in 2020 — 14832, in 2021 — 18150. The share of complaints in the structure of appeals decreased from 10,5 ± 0,4% for 2019 to 6,0 ± 0,2% in 2020 and 4,6 ± 0,2% for 2021. Reasons for writing complaints included the quality of medical care (32,9 ± 2,1% during 2019, 34,2 ± 1,6% during 2020, 22,6 ± 1,4% during 2021); refusal to provide medical care (20,5 ± 1,8% in 2019, up to 15,7% ± 1,2% over 2020 and 11,5 ± 1,1% over 2021); violations of ethics and deontology (18,8 ± 1,7% for 2019, 15,8 ± 1,6% for 2020 and 15,3 ± 1,2% for 2021); problems with access to medical care (12,0 ± 1,4% during 2019, 15,2 ± 1,2% during 2020, 19,2 ± 1,4% duirng 2021 year). Research limitations. Repeated requests were excluded. Conclusion. The example of the Nizhny Novgorod region demonstrates the openness of the Ministry of Health, the formation of feedback channels with patients to help to effectively mobilize resources to promptly address issues of providing medical care to the population in an emergency situation.

Externally Driven Border Control in West Africa: Local Impact and Broader Ramifications

Over the last two decades and with notable increase since 2015, millions of euros have been invested in territorial border governance in West Africa. Targeting migration policy frameworks, capacity building, and the provision of material, the EU and individual European states have sought to improve control mechanisms along these vast and porous borders. This article explores the local impact and broader ramifications of primarily externally funded policy efforts as they are implemented along Ghana's three international borders with Burkina Faso, Côte D’Ivoire, and Togo. Drawing on observations at official checkpoints and interviews with border control officers and border crossers, the article finds that recent initiatives have facilitated the modernization, expansion, and professionalization of border control. Yet, these enhancements have concurrently led to increased reliance on external support, altered local social relations in border checkpoint areas, and triggered the criminalization of legal emigration. The article situates these developments within its geopolitical landscape, illustrating how externally driven migration governance, when detached from local realities, yields both immediate and far-reaching ramifications. Drawing on and extending critical migration governance analysis and border theory, this study underscores the importance of scrutinizing not only explicit, but also the more subtle and rippling effects of European externalization policies in Africa, as they extend beyond local contexts to influence wider societal structures.

Terraced landscapes: risk and liability

Terraced landscapes are preserved elements of the human-influenced environment, maintained by ongoing agricultural activities. Farmers are subject to the periodic collapse of terraces, which are repaired by means of traditional techniques. Unless it can be proven that the collapse was accidental, the owner is liable for the damage caused by the collapse of the terraces or landslides. This topic, applied to Minori (Amalfi Coast, Italy), is interesting for two reasons. First, with the advancement of scientific knowledge, the unpredictability of events diminishes and the liability increases. Second, the intensive construction at the base of the terraces and the economic value of the buildings, enhanced by the landscape context, expose the owners to claims for damages from third parties that are disproportionate to the agricultural income, including claims for amnestied illegal constructions. The result has been an increase in the renunciation of property rights by farmers in areas of high hydro-geological risk. However, the State, which becomes the owner, denies the validity of property relinquishments made by owners solely to transfer risk, cost, and liability to the state treasury. This paper discusses these issues, going beyond the Italian case study, and proposes a different risk allocation related to territorial governance. This study reveals a discrepancy in government decision making and suggests that the division of risk into multiple components could be a solution to this inconsistency.

The current policy focus shift from a sectoral to a territorial governance of wildfire reduction is aligned with forest owners’ preferences

Land management for wildfire reduction has been said to require new governance arrangements that ensure the coordination at the local level among landowners and between them and other relevant private and public actors. To promote a shift in the policy approach to wildfire mitigation from the current focus on fuel management, within the forestry sector, to a more effective cross-sectoral landscape transformation, this governance problem takes on even greater importance. Given the diversity of owners within a territory, our main goal is to identify a governance arrangement that match the heterogeneity of owners’ preferences for alternative governance options.Based on a face-to-face survey of 497 owners in a fire-prone region, four types of owners have been identified using a cluster analysis based on management practices and socioeconomic context. Our results show that the type of owner helps explaining their preferences for alternative governance options. Owners that are more active (e.g. regular forest interventions) and have stronger links to the forestry sector, through regular timber sales and participation in forest owner organizations, favor the keeping of individual management over the delegation of management on others or the renting or selling of the land. Although many of these owners see forest organizations alone as the best entities to promote wildfire mitigation, the majority would prefer solutions that also integrate local authorities. Owners that are less active (e.g. no forest interventions) and have weaker links to the sector (no timber sales and no enrollment in forest organizations) consider renting or selling the land, and view a combination of both local authorities and forest organizations as the best solution to promote wildfire reduction at the local level.We conclude that a multi-actor (public-private) territorially-based governance model fits both the required transition in the policy approach and the diverse preferences of different owner types.

Economic geography’s contribution to understanding the circular economy

Abstract This article explores the role of evolutionary economic geography in enhancing understanding and implementation of the circular economy (CE). By incorporating spatial and territorial dimensions into CE research, this study emphasizes the significant influence of geographical factors on achieving economic and environmental objectives. The research highlights the importance of localizing CE practices and emphasizes the contributions of proximity theory and territorial governance in promoting collaborative networks that are crucial for CE success. Furthermore, the article introduces the concept of Territorial Circular Ecosystems, which provides a framework for analyzing regional variations and the interplay between local actors, resources, and institutional support in the deployment of the CE. The findings offer fresh insights for policymakers and researchers, advocating for place-based policies to address CE challenges and optimize sustainability strategies.

Transnational networks of energy communities as counterpower in the European Union renewable energy transition framework

Abstract This article explores the communicative process by which transnational networks of energy communities (ECs) in Europe build counterpower vis-a-vis the European Union (EU) institutional framework, with the purpose of effectively contributing to the shaping and management of the EU policy and regulatory framework for renewable energy transition. Such a process consists in the identification of obstacles to effective participation, the facilitation of access to information on EU regulation and policy frameworks, and the production of technical knowledge that reframes how renewable energy transition is understood as a field of political intervention. The impact of EC networks, and other networks of related community-led initiatives, on the EU energy policy framework is still relatively small when compared to centralized, large-scale providers of energy. It is constrained by how differently EU regulatory and policy packages address ‘techno-energetic systems’ in energy transition, as well as disparities in the transposition of EU legislation by Member States. Still, EC networks are building counterpower by facilitating bottom-up information and skill sharing, focusing on facilitating access to information on EU regulation and policy frameworks, as well as producing knowledge that frames the obstacles to effective participation of ECs in policymaking. It expands the understanding of renewable energy transition as a field of policy intervention beyond merely instrumental logic, centred on regulation and policy support to technological innovation, by mainstreaming a substantive logic based on a comprehensive socioecological framing that embeds energy transition in a multidimensional understanding of territorial development and governance.

Transforming territorial governance to make it transformative: reflections from and for action research for territorial development

The challenge of the climate emergency has highlighted the relevance of local transformative action for the development of new paths to sustainability, but also the relevance of the connection between the local and the global so that these paths have a real impact. Delving into this local-global connection is key for action research, and specifically, for Action research for territorial development (ARTD) and the Pedagogical approach (PA), two action research approaches that work from and for the territories, and which are the subject of analysis in this special issue. With the aim of contributing to deepening of this relationship, this article adapts the concept of transformative governance proposed by IPBES to build an actionable framework for reflection on the contribution of territorial development policies and research to transformative governance. It then analyses the IADT and the EP with this framework and suggests strengthening the critical dimension of facilitation so that these approaches are better adapted to the challenge of climate change.

Spatiotemporal Evolution of Territorial Spaces and Its Effect on Carbon Emissions in Qingdao City, China

Land use change has always been a significant factor affecting global carbon emissions. Dissecting the characteristics of territorial space evolution and its impact on carbon emissions is crucial for developing low-carbon-oriented territorial space optimization and governance strategies. This paper calculates the carbon emissions associated with territorial spaces in Qingdao from 2000 to 2020, utilizing land use data alongside various statistical data. Based on the accounting results, the evolution characteristics of territorial spaces and their corresponding carbon emissions, as well as the carbon transition dynamics resulting from space transfer, are analyzed. A carbon transition decomposition formula is then proposed to quantify the differential and spatially heterogeneous impacts of changes in space types and socio-economic development on emissions. The results indicate that: (1) the evolution of territorial spaces in Qingdao during 2000–2020 is characterized by an expansion of living space and a contraction of production and ecological spaces; (2) net carbon emissions rose from 313.98 × 104 tons to 1068.58 × 104 tons, with urban production space contributing the most (69.96% in 2020) due to its significantly high emission density. The spatial distribution of carbon emissions exhibited a stable “northwest–southeast” pattern, with increased dispersion and weakened directionality; (3) the transformation of territorial spaces promoted carbon emissions in Qingdao, with the conversion of urban production space to other uses yielding the most favorable carbon transitions, while the expropriation of agricultural production spaces for urban production and residents’ living has resulted in the most detrimental carbon transitions; (4) socio-economic development shapes the overarching pattern of regional emission density changes, whereas space transfers account for local variations. This paper also identifies priorities for spatial optimization and key sectors for emission reduction. The findings contribute to a deeper understanding of the carbon emission consequences of territorial space transformation in Qingdao, thereby providing valuable insights for regional spatial planning and optimization aimed at promoting low-carbon development.

Insights into water insecurity in Indigenous communities in Canada: assessing microbial risks and innovative solutions, a multifaceted review.

Canada is considered a freshwater-rich country, despite this, several Indigenous reserves struggle with household water insecurity. In fact, some of these communities have lacked access to safe water for almost 30 years. Water quality in Canadian Indigenous reserves is influenced by several factors including source water quality, drinking water treatments applied, water distribution systems, and water storage tanks when piped water is unavailable. The objective of this multifaceted review is to spot the challenges and consequences of inadequate drinking water systems (DWS) and the available technical and microbiological alternatives to address water sanitation coverage in Indigenous reserves of Canada, North America (also known as Turtle Island). A comprehensive literature review was conducted using national web portals from both federal and provincial governments, as well as academic databases to identify the following topics: The status of water insecurity in Indigenous communities across Canada; Microbiological, chemical, and natural causes contributing to water insecurity; Limitations of applying urban-style drinking water systems in Indigenous reserves in Canada and the management of DWS for Indigenous communities in other high-income countries; and the importance of determining the microbiome inhabiting drinking water systems along with the cutting-edge technology available for its analysis. A total of 169 scientific articles matched the inclusion criteria. The major themes discussed include: The status of water insecurity and water advisories in Canada; the risks of pathogenic microorganisms (i.e., Escherichia coli and total coliforms) and other chemicals (i.e., disinfection by-products) found in water storage tanks; the most common technologies available for water treatment including coagulation, high- and low-pressure membrane filtration procedures, ozone, ion exchange, and biological ion exchange and their limitations when applying them in remote Indigenous communities. Furthermore, we reviewed the benefits and drawbacks that high throughput tools such as metagenomics (the study of genomes of microbial communities), culturomics (a high-efficiency culture approach), and microfluidics devices (microminiaturized instruments) and what they could represent for water monitoring in Indigenous reserves. This multifaceted review demonstrates that water insecurity in Canada is a reflection of the institutional structures of marginalization that persist in the country and other parts of Turtle Island. DWS on Indigenous reserves are in urgent need of upgrades. Source water protection, and drinking water monitoring plus a comprehensive design of culturally adapted, and sustainable water services are required. Collaborative efforts between First Nations authorities and federal, provincial, and territorial governments are imperative to ensure equitable access to safe drinking water in Indigenous reserves.

Indication: As an adjunct to diet as a replacement therapy to treat the complications of leptin deficiency in lipodystrophy patients: with confirmed congenital generalized lipodystrophy (Berardinelli-Seip syndrome) or acquired generalized lipodystrophy (Lawrence syndrome) in adults and children 2 years of age and above with confirmed familial partial lipodystrophy (PL) or acquired PL (Barraquer-Simons syndrome), in adults and children 12 years of age and above with persistent significant metabolic disease for whom standard treatments have failed to achieve adequate metabolic control.

Influence of Technology and Metaverse on Traditional Absolute Sovereignty Control of Modern National States Territories

The metaverse can extend the physical world using augmented and virtual reality technologies, allowing users to seamlessly interact within virtual and simulated environments using avatars and holograms. This investigation strives to analyze the relationship between technology and the metaverse on traditional absolute sovereignty control of modern national states’ territories to understand better the potential impact of emerging virtual environments on the established structures and systems of physical governance. As technology continues to evolve and the metaverse grows in size and complexity, examining how it may challenge traditional notions of territorial sovereignty and governance is essential. It can occur in several ways, such as borderless access, jurisdictional disputes, decentralization of power, and cyber security concerns. Based on secondary data, the study showed different ways technology and the metaverse intersect with traditional state control. It can identify potential conflict areas and develop strategies to adapt to this rapidly evolving landscape. Therefore, it is to gain a deeper understanding of the complex relationship between technology, the metaverse, and the traditional structures of governance that have shaped our world. The ultimate goal is to structure the metaverse in morally acceptable ways and collectively the most democratically beneficial for society.

Towards Net-Zero Emissions from Urban Transport: Ex Post Policy Evaluation in Canberra, the Australian Capital Territory

The achievement of net-zero emissions is a major governmental challenge to ameliorate the adverse impacts of climate change, and to reduce the health risks associated with poor air quality. In Australia, the transport sector is a major contributor to particulate matter and greenhouse gas emissions, especially in urban areas. The Australian Capital Territory (ACT) Government recognises that one of the greatest sustainability challenges to achieving a carbon-neutral society with net-zero emissions is the transport sector because it contributes 60 percent of all emissions under its jurisdiction. The ACT is a suitable case study on climate change, energy policy, and transport emissions because its electricity is powered by renewable energy; its governance has included a continuous planning philosophy of integrating transport with land use. The methodology is based on identifying and summarising the international literature on net-zero emissions policy (n = 50), the relevant policy documents and reports by the Australian Government (n = 8) and by the ACT Government (n = 32). An appraisal of policy outcomes in the ACT is based on an analysis of quantitative and qualitative data. In a car-dependent city (77% of trips by private transport), the most realistic policies for achieving net-zero emissions are to encourage, through fiscal incentives and regulations, the adoption of electric vehicles for buses and private cars, electric or hydrogen vehicles for the commercial fleet, and regulations to phase out petrol- and diesel-powered vehicles.
