Bilateral Temporomandibular Joint Reconstruction With Custom Alloplastic Implants for Pediatric Ankylosis.

For pediatric patients with refractory temporomandibular joint (TMJ) ankylosis, reconstruction with autologous techniques such as costochondral grafts or distraction osteogenesis has long been considered the gold standard. Many surgeons believed the use of alloplastic joint replacement to be contraindicated in pediatric patients due to concerns for growth restriction and the limited lifespan of the implants. However, recent data has supported TMJ prostheses in skeletally immature patients. This study aims to present a case series of pediatric patients undergoing bilateral TMJ reconstruction with custom-made implants and evaluate their postoperative results. A retrospective chart review was performed of all consecutive pediatric patients undergoing bilateral alloplastic TMJ reconstruction for refractory ankylosis. All patients underwent bilateral TMJ release and total joint replacement with custom-made implants. Preoperative and postoperative cephalometric and volumetric airway data was obtained using cone-beam computed tomography. Three patients, aged 8 to 17, underwent bilateral TMJ replacement with custom-made implants. There were no postoperative complications, and no implants required explantation or replacement. Postoperatively, all patients had increases in maximal interincisal opening, which was stable over months/years of follow-up. The patients also subjectively reported improved speech and mastication; 1 patient had significant improvements in sleep apnea symptoms. Volumetric airway analysis revealed an average airway size increase of 25.6%. Alloplastic TMJ reconstruction is a safe, effective solution for refractory ankylosis in pediatric patients and represents a promising new frontier in craniofacial surgery. Continued long-term evaluation will provide further evidence of the utility of this procedure.

Use of Alloplastic Total Joint Replacement of the Temporomandibular Joint of Growing Patients: A Scoping Review

Temporomandibular joint (TMJ) disorders pose unique challenges in pediatric patients undergoing craniofacial growth. Alloplastic total temporomandibular joint reconstruction (ATJR) techniques have emerged as promising solutions, offering immediate joint function and symptom relief. However, the efficacy and safety of ATJR in this population remain under scrutiny. This scoping review aims to comprehensively summarize the evolving literature on ATJR in growing patients with end-stage TMJ pathology. Our methodology adhered to PRISMA-ScR guidelines, identifying 9 relevant articles. The discussion emphasizes preoperative considerations, treatment selection criteria, and potential complications. While ATJR offers short-term benefits, concerns regarding long-term outcomes persist, necessitating further research. Collaboration among multidisciplinary teams is crucial for optimal patient care. Future research should focus on comparative efficacy and safety, refining treatment algorithms, and long-term follow-up to enhance outcomes and minimize complications in pediatric TMJ pathology.

Poor condyle position after mandibular reconstruction and a classification system for patients with "VSCU" based on computed tomography: a cross-sectional study with retrospective data collection.

Although the application of vascularized free bone muscle flap to reconstruct the mandible has become a standardized approach for mandible reconstruction, the results of its reconstruction are not always satisfactory. The purpose of this study was to identify the types of mandibular and condylar defects by analyzing the unsatisfactory cases of mandibular reconstruction in clinical practice, and to provide some clinical experience of reconstruction. Our study retrospectively analyzed 364 patients who underwent mandibular resection and vascularized free bone flap reconstruction of the mandible and temporomandibular joint (TMJ). We innovatively proposed a "VSCU" classification system (V: vertical position, S: sagittal position, C: coronal position, U: condylar resection is not required) by analyzing computed tomography (CT) scans of mandibular branches and TMJs. In all, 221 cases of free iliac muscle flap and 143 cases of fibula muscle flap were included in this study, of which 23 cases had unsatisfactory results after TMJ reconstruction. We classified 23 patients with unsatisfactory mandibular reconstruction according to the "VSCU" classification system. The most common type was U + V + SfC (n=8), followed by V - SfC + U + (n=4), V - s + C + U + (n=3), V - sbcou - (n=3), V - SBC + U + (n=2), V - s + C + U - (n=1). The most common classification was insufficient mandibular rami length, followed by condylar sagittal anteriorization. There was no significant change in the position of condyle on the healthy side during mandibular reconstruction involving condyle. P1 on the affected side was 52.28±4.17 mm before operation and 58.94±5.65 mm after operation, P<0.01; P2 was 12.83±3.49 mm before operation and 24.90±7.15 mm after operation. S2 was 4.54±2.84 mm before operation and 19.10±8.54 mm after operation. A2 was 11.46±3.35 mm before operation and 24.15±8.29 mm after operation. The P values were all less than 0.01, and the differences were statistically significant. We propose to use the "VSCU" classification system for accurate 3-dimensional (3D) analysis and positioning, and then obtain accurate models through computer-aided design and manufacturing (CAD/CAM), which can reduce the occurrence of poor reconstruction effect and unreasonable joint position, and is worthy of clinical promotion.

It has also opened up access to surgical education, improved patient and physician relations, and improved surgical outcomes. In this review, we will look at the many applications of 3D printing in the fields of Maxillofacial Trauma and Reconstruction, Orthognathic Surgery, Maxillofacial Protoplast, Temporomandibular Joint (TMJ) Reconstruction, Dentistry, Bone Tissue Engineering for Maxillofacial Regeneration, Clinical Education, and Patient Communication. 3D printing was initially used to create custom prosthetic products and implants. Scientists were able to construct organs from patients’ own cells supported by a 3D printed scaffold. In the last two decades, advances in technology have ushered in a new age of Oral and Maxillofacial Surgery.

Osteo-cutaneous microvascular free-flaps are a viable option for reconstructing the temporomandibular joint

BackgroundThere are multiple reconstructive options after mandibular resection and disarticulation of the condyle. The purpose of this study was to assess the clinical outcomes and functionality of osteocutaneous free flaps in the reconstruction of mandibular defects that involve the Temporo-Mandibular Joint (TMJ). MethodsOur study is a retrospective cohort study involving subjects who underwent mandibular resection and needed TMJ reconstruction with vascularized bony-free flaps from February 2016 to June 2018. Data gathered included patient demographics, post-operative function, maximum interincisal opening (MIO), occlusion status, deviation on opening, and TMJ symptoms. Data points collected from postoperative CT imaging included the following: position of the flap in relation to the glenoid fossa and articular eminence, and closest point of contact. ResultsEight out of the nine patients who underwent free flap reconstruction of mandibular defects involving the TMJ qualified for the study. The mean age was 39.7 years old. In all 8 cases, virtual surgical planning (VSP) was used. The mean follow-up time was 18.75 months. The flap success rate was 100%. The mean MIO was 37.37 mm. Six patients resumed their premorbid diet, and one patient developed dysphagia and was peg tube dependent. In seven cases the occlusion was intact and reproducible, one case was without sufficient teeth for occlusion. On imaging, the mean distance from the neo-condyle to the glenoid fossa was 14 mm and to the articular eminence 8.68 mm. The point of closest contact in all cases appeared to be the articular eminence. ConclusionVascularized Osteocutaneous-flaps such as FFFs and DCIA flaps provide a good option for the reconstruction of mandibular defects that involve the TMJ. Ipsilateral deviation on opening does not negatively affect clinical outcomes or function. Placing patients in Maxillomandibular fixation for 4–6 weeks may help to prevent condylar sag and provide stable post-operative occlusion.

Virtual Surgical Planning for Temporomandibular Joint Reconstruction with Stock TMJ Prostheses: Pilot Study.

The temporomandibular joint (TMJ) is one of the most complex joints in the human anatomy. In advanced degenerative stages, conservative or minimally invasive surgical therapies have failed to restore joint function, and joint replacement with prostheses has been required. Stock prostheses, compared to custom-made prostheses, are much less expensive and require less pre-operative preparation time. Four patients followed for years for temporomandibular dysfunction and previously operated on by arthroscopy or open joint surgery that have been reconstructed with stock TMJ prostheses (STMJP) through virtual surgical planning (VSP) and an STL model with surgical and positioning guides were included. The median follow-up was 15 months; the median number of previous TMJ surgeries was 2. The mean preoperative MIO was 24.6 mm and at longest follow-up was 36.4 mm. The median preoperative TMJ pain score was 8, and the median postoperative TMJ pain was 3. All patients have improved their mandibular function with a clear improvement of their initial situation. In conclusion, we believe that stock TMJ prostheses with virtual surgical planning and surgical guides are a good alternative for TMJ reconstruction at the present time. Nonetheless, prospective and randomized trials are required with long-term follow up to assess their performance and safety.

Is an extended alloplastic replacement (eTMJR) device an option for the management of mandibular defects involving the temporomandibular joint? - A prospective cohort study

The reconstruction of large mandibular deficits involving the temporomandibular joint (TMJ) can be challenging. Extended alloplastic total temporomandibular joint reconstruction (eTMJR) is one treatment option.This single center, prospective, long-term observational study evaluates the objective and subjective outcomes of 21 eTMJR subjects followed >5 years postoperatively. The long-term stability of the eTMJR was determined by collecting objective data (mandibular kinematics, contralateral surface electromyography (sEMG), maximum voluntary clenching (MVC)) and subjective oral health related quality of life (oHrQoL).Except for mandibular deviation on opening, the objective variables markedly improved. A statistically significant decrease was found for ipsilateral condylar hypermobility (P = 0.001), and an increase in bilateral MVC (ipsilateral P = 0.029, contralateral P = 0.015). The subjective outcomes 'diet consistency’ (P = 0.001) and 'physical distress' (P = 0.043) improved significantly.Based on the stability of the eTMJR devices over the study period, improved objective and subjective variables, eTMJR appears to be a safe and effective predictable management option for patients with large mandibular deficits including the TMJ.

Anteriorly pedicled deep layer of temporalis muscle flap for disc replacement after temporomandibular discectomy

ObjectiveAn anteriorly pedicled temporalis muscle flap has a functional advantage for temporomandibular (TMJ) joint reconstruction. However, the greater thickness of the flap for disc replacement after discectomy might cause several postoperative complications including malocclusion. The temporalis muscle can be split into the thin superficial and thick deep layers; they are separated by a thin fatty layer. Therefore, we developed the modified anteriorly pedicled temporalis muscle flap using the deep layer of the temporalis muscle, which has a suitable size for the TMJ space after discectomy without losing the functional advantage. MethodsThe procedure was applied to three patients who underwent discectomy for the treatment of synovial osteochondromatosis. They were followed up from preoperatively to more than 1 year postoperatively. ResultsNo significant postoperative complication were observed in all patients. Additionally, all patients obtained normal jaw function without malocclusion from immediately after the surgery. ConclusionThis technique offers some advantages in disc replacement after discectomy.

Reconstruction of the temporomandibular joint using a vascularized medial femoral condyle osteocartilaginous flap: an experimental investigation in miniature pigs

BackgroundReconstruction of the temporomandibular joint (TMJ) is a significant challenge in maxillofacial surgery. A vascularized medial femoral condyle (MFC) osteocartilaginous flap is a good choice for TMJ reconstruction. In this study, we evaluated the radiographic and histological changes of MFC after TMJ reconstruction.MethodsA ramus-condyle unit (RCU) defect was created unilaterally in five adult male Bama miniature pigs. The ipsilateral vascularized MFC osteocartilaginous flap was used to reconstruct the TMJ, and the non-operative sides served as controls. Multislice spiral computed tomography (CT) was performed preoperatively, immediately postoperatively, and at two weeks, three months, and six months postoperatively. Three animals were euthanized at 6 months postoperatively. Their reconstructed condyles, natural condyles and the MFCs on the opposite side were collected and subjected to µCT and histological evaluation.ResultsIn the miniature pigs, the vascularized MFC osteocartilaginous flap was fused to the mandible, thus restoring the structure and function of the RCU. The postoperative radiographic changes and histological results showed that the reconstructed condyle was remodeled toward the natural condyle, forming a similar structure, which was significantly different from the MFC.ConclusionsIn miniature pigs, the RCU can be successfully reconstructed by vascularized osteocartilaginous MFC flap. The reconstructed condyle had almost the same appearance and histological characteristics as the natural condyle.

Recent Advances in Temporomandibular Joint Surgery.

Temporomandibular disorders (TMDs) affect a high percentage of children and adults worldwide. Surgery may be indicated in severe or recalcitrant cases. Several recent advancements in TMD and temporomandibular joint (TMJ) surgery have elevated understanding and the ability to treat affected patients. We discuss recent advances in TMD epidemiology, juvenile idiopathic arthritis (JIA) of the TMJ, and surgical techniques and technologies. Technical advancements have been identified in TMJ arthroscopy, the treatment of TMJ subluxation and dislocation, and extended prosthetic total TMJ reconstruction (eTMJR). Overall, this review provides valuable insights into significant recent advancements in TMJ disorders and their surgical management.

Biomechanical Evaluation of a Standard Temporomandibular Joint Prosthesis and Screw Arrangement Optimization: A Finite Element Analysis.

Artificial total joint replacement is an important method of temporomandibular joint (TMJ) reconstruction, which has been advocated for TMJ osteoarthrosis, ankylosis, tumors, and other diseases. We designed one type of standard TMJ prosthesis fit for Chinese patients. This study aimed to explore the biomechanical behavior of the standard TMJ prosthesis using finite element analysis and selects an optimal screw arrangement scheme for clinical application. A female volunteer was recruited for a maxillofacial computed tomography scan, then the Hypermesh software was used to establish a finite element model of a mandibular condyle defect repaired with an artificial TMJ prosthesis. An advanced universal finite element program software was used to calculate the stress and deformation under a simulated maximum bite force loading. Also, the forces of screws under different numbers and arrangements were analyzed. Meanwhile, we designed an experiment to verify the calculation model. The average maximum stress of the fossa component of the standard prosthesis model was 19.25MPa. The average maximum stress of the condyle component was 82.58MPa, mainly concentrated near the top row hole. The fossa component should be fixed with at least 3 screws, and the optimal number of screws was 4. The condyle component should be fixed with at least 4 screws, and its optimal number was 6. The best scheme of screw arrangement was determined. The results of the verification experiment showed that the analysis was reliable. The stress distribution of the standard TMJ prosthesis is uniform, meanwhile, the number and arrangement of the screws significantly affect the contact force of the screws.

1009 Surgery: Problem and Solution. Temporomandibular Joint Reconstruction For Treating Obstructive Sleep Apnea After Cosmetic Surgery

Abstract Introduction Orthognathic surgery is often considered for the treatment of obstructive sleep apnea (OSA); however, in some circumstances, specific interventions may precipitate OSA. Report of case(s) A 34 year-old female presented with complaints of snoring and fragmented sleep. She had undergone bimaxillary osteotomy and mandibuloplasty for cosmetic reasons outside of the country in 2019. Prior to the surgery, she was asymptomatic and did not snore. Post operatively, she developed snoring, witnessed apneas, fragmented sleep, and daytime sleepiness. A home sleep apnea test revealed severe obstructive sleep apnea with Respiratory Event Index (REI) 66.4/hr. She was initially treated with Positive Airway Pressure (PAP) while seeking oral surgery consultation. PAP via nasal mask was initially effective with residual Apnea Hypopnea Index (AHI) of 3/hr., but the patient developed difficulty tolerating PAP due to aerophagia, nausea, and eructation, despite limiting pressures to 5-7cm H2O. In 2022, the patient successfully underwent temporomandibular joint (TMJ) reconstruction with total joint prosthesis, TMJ fat grafting, bilateral coronoidectomy, maxillary osteotomy with bone plate and grafting, and partial inferior turbinatectomy. She reported resolution of snoring and significant symptomatic improvement after surgical revision. Repeat home sleep apnea testing 1 month after the surgery revealed REI 0.6/hr. Conclusion This case illustrates the potential for development of OSA following orthognathic surgery. Patients and providers should be aware of this potential complication when pursuing surgical intervention. Support (if any)

Introduction of temporomandibular joint and skull base combined reconstruction by autogenous bone graft.

This study introduces the application of autogenous bone graft for the reconstruction of temporomandibular joint (TMJ) and skull base combined defects. Patients treated with autogenous bone grafts for reconstruction of the TMJ and skull base were reviewed. All patients underwent virtual surgical design to confirm the osteotomies of the combined lesion and the selections of autogenous bone graft, fabrication of surgical templates to transfer the plan to actual operation, and reconstruction of autogenous bone graft for the TMJ and/or skull base. Surgical outcomes were assessed by clinical examinations and radiological data. Twenty-two patients were involved in this study. Ten patients underwent reconstruction of the skull base by a free iliac or temporal bone graft and preservation of the TMJ. Twelve patients underwent skull base reconstruction by the same methods and total reconstruction of the TMJ by half sternoclavicular joint flap or costochondral bone graft. No severe complications occurred after surgery. The occlusion relationship was stable and similar to that of the preoperative state. The pain and maximal interincisal opening were significantly improved by the 101.2-month follow-up. Autogenous bone graft is a good alternative for repairing the TMJ and the skull base structure and function. The study introduced the application of autogenous bone graft for the reconstruction of temporomandibular joint and skull base combined defect, which is a good way to repair the defect and restore the function.

Do Case Reports on Total Mandibular, Extended, and Bilateral Total Temporomandibular Joint Reconstruction Prostheses Adhere to the Surgical CAse REport Guidelines?

The Surgical CAse REport (SCARE) guidelines are a standardized format for reporting surgical cases. The aim of this study was to evaluate the completeness of case reports documenting alloplastic reconstruction of large craniomaxillofacial defects involving total mandibular, bilateral, and extended temporomandibular joint in major high-quality craniomaxillofacial journals, based on the SCARE guidelines. An extensive online search was performed according to the Priority Reporting Items for Systematic Reviews and Meta-Analyses statement in PubMed, Embase, Scopus, Google Scholar, and Dimensions databases to identify relevant case reports. Each selected case report was assessed on 16 topics (38 items) of the SCARE guidelines, using a scoring scale of "0" (No/noncompliance), "1" (Yes/compliance), and "2" (unclear). The completeness of reporting (COR) score was calculated as the ratio of "yes" responses to "total" (ie, yes+no+unclear) responses. Adequacy of case reporting was denoted by a COR score of 70% or more. A total of 35 case reports were selected, where the male to female patients ratio was 3:4 cases, mean±standard deviation (SD) age: 34.9±16.7years, mean±SD follow-up duration: 17.0±12.9months, and number of patients with left, right, and bilateral temporomandibular joint reconstruction prostheses were 16, 10, and 09, respectively. The mean±SD COR score for all 35 case reports and the individual item of the SCARE guidelines was 70.2±10.5% and 66.5±31.2%, respectively. The minimum and maximum COR score was found for "Keywords" (0.0%) and "Introduction" (100%) and "Clinical Findings" (100%), respectively. Adequate reporting was found for 20/35 (57%) case reports. This study revealed that case reports in major high-quality craniomaxillofacial journals suffer from insufficient reporting. Widespread adoption of available standards, such as SCARE guidelines, is proposed to improve the quality and robustness of case reporting.

Biomechanical Evaluation of Temporomandibular Joint Reconstruction Using Individual TMJ Prosthesis Combined with a Fibular Free Flap in a Pediatric Patient.

The main aim of this study was to perform a complex biomechanical analysis for a custom-designed temporomandibular joint (TMJ) prosthesis in combination with a fibular free flap in a pediatric case. Numerical simulations in seven variants of loads were carried out on 3D models obtained based on CT images of a 15-year-old patient in whom it was necessary to reconstruct the temporal-mandibular joints with the use of a fibula autograft. The implant model was designed based on the patient's geometry. Experimental tests on a manufactured personalized implant were carried out on the MTS Insight testing machine. Two methods of fixing the implant to the bone were analyzed-using three or five bone screws. The greatest stress was located on the top of the head of the prosthesis. The stress on the prosthesis with the five-screw configuration was lower than in the prosthesis with the three-screw configuration. The peak load analysis shows that the samples with the five-screw configuration have a lower deviation (10.88, 0.97, and 32.80%) than the groups with the three-screw configuration (57.89 and 41.10%). However, in the group with the five-screw configuration, the fixation stiffness was relatively lower (a higher value of peak load by displacement of 171.78 and 86.46 N/mm) than in the group with the three-screw configuration (where the peak load by displacement was 52.93, 60.06, and 78.92 N/mm). Based on the experimental and numerical studies performed, it could be stated that the screw configuration is crucial for biomechanical analysis. The results obtained may be an indication for surgeons, especially during planning personalized reconstruction procedures.

The Impact of 3D Printing on Oral and Maxillofacial Surgery

3D printing technology has driven major medical, dental, engineering, and education innovations. In oral and maxillofacial surgery (OMFS), 3D printing technology has been implemented to improve precision in treatment planning, increase surgical predictability, reduce operation times, and lower overall costs. Furthermore, 3D printing has opened access to surgical training, facilitated patient-physician relationships, and generated greater surgical outcomes. The aim of this review is to summarize the impact of 3D printing technology in the field of OMFS. We discuss its many applications in the management of maxillofacial trauma and reconstruction, orthognathic surgery, maxillofacial prosthodontics, temporomandibular joint (TMJ) reconstruction, dental implants (3D-printed surgical guides and 3D-printed dental implants), bone tissue engineering for maxillofacial regeneration, clinical education, and patient communication.

Is the risk of chronic pain after total temporomandibular joint replacement independent of its indications? A prospective cohort study

Chronic pain is a possible long-term complication after alloplastic temporomandibular joint reconstruction (TMJR). This study was developed to evaluate various subjective and objective measurements to determine the presence and degree of TMJ pain in patients treated with TMJR regardless of the indication for the operation. A prospective, single-centre study was performed. Data on 36 patients (56 TMJR) were collected preoperatively and at follow up two to three years postoperatively. The primary outcome variable was subjective TMJ pain (none/mild, moderate/severe) at follow up. The predictor variables were objective pressure pain thresholds (PPTs) at the ipsilateral joint(s) and muscle(s), functional parameters (incisal range of motion, maximum voluntary clenching), subjective oral health-related quality of life (OHRQoL), and demographic and surgical variables. The number of patients with moderate/severe pain decreased from 17 preoperatively to 10 at follow up. Self-reported TMJ pain was significantly reduced in the entire group (p = 0.001). Patients with moderate/severe pain at follow up were more restricted in their OHRQoL but did not differ in PPT and functional parameters from the no/mild pain group. Moderate/severe TMJ pain at follow up was associated with unilateral TMJR and more preoperative pain. This study provides preliminary evidence that despite good pain reduction in most patients, persistent pain after TMJR is common and, in rare cases, may even worsen regardless of the original diagnosis. At follow up there was a close relation between OHRQoL and TMJ pain. TMJ pain after TMJR cannot be confirmed by objective measurement methods (PPTs and functional parameters).
