A Standardised Protocol for Pre-operative Pelvic Radiographs for Templating in Total Hip Arthroplasty.

Digital templating using pre-operative radiographs enables pre-operative planning for total hip arthroplasty (THA). This allows surgeons to reproduce hip biomechanics effectively, reducing the risk of post-operative complications. Pelvic radiographs demonstrating the head, neck, trochanters, and proximal one-third of the femoral shaft allow calculation of key measurements including femoral offset and limb length discrepancy (LLD). Currently, no standardised guideline exists for obtaining pre-operative radiographs for templating in THA. Materials and methods: A single-blinded retrospective cohort study assessing the quality of pre- and post-operative radiographs of 195 patients who underwent elective THA for osteoarthritis over a two-year period was performed. Quality was rated as good, fair or poor, respectively, depending upon whether ≥2, 1 or none of the following were met: Pubic symphysis (PS) and coccyx in a straight line with 1-3 cm between the superior edge of the PS and tip of coccyx, trochanters distinguishable, obturator rings symmetric. Post-operative images were assessed to determine whether the distal end of the implanted prosthesis was visible. Results: The sample consisted of 195 patients. Pre-operatively 115 (59%) radiographs were classified as good, 71 (36.4%) fair and 9 (4.6%) poor. Post-operatively 46 (23.6%) were classified as good, 114 (58.4%) as fair and 30 (15.4%) as poor. In the post-operative radiographs, 25.6% did not include the distal tip of the prosthesis. Conclusion:This study highlights significant scope to improve the quality of pre-operative radiographs, allowing accurate templating to optimise outcomes for THA. A protocol is recommended whereby the pelvic radiograph is centred on the PS at the lesser trochanter level, ensuring adequate exposure of the proximal femur, acetabulum and iliac crests.

Preoperative Templating for Total Hip Arthroplasty: A Method for Calibrating Digital Radiographs Using Patient Demographics and Anthropometric Measurements.

Background Preoperative templating aids the surgeon in estimating implant size and placement. Calibration markers are used to set the correct magnification of digital images before templating. Improper marker placement or complete absence can lead to inaccuracy or an inability to calibrate images altogether. Aims This study describes a method for calibrating images using a patient's femoral head size (FHS) predicted using demographics and anthropometric data. Materials and methods A formula predicting the FHS was derived from a cohort of 507 patients who underwent hemiarthroplasty for an intracapsular fractured neck of the femur through multivariate regression analysis. A separate validation cohort (n=50) who had undergone total hip arthroplasty (THA) had postoperative radiographs calibrated using the predicted FHS and the native contralateral hip as a surrogate calibration marker. The THA femoral head implant size was subsequently measured and compared with the actual implant size selected intraoperatively. Measurements were performed by two independent assessors to determine intra- and interobserver reliability. Results Multivariate regression analyses showed four variables significantly correlated with the size of the femoral head: gender (p < 0.001), height (p < 0.001), weight (p < 0.001), and race (Asian) (p = 0.01). Using these, a regression model to predict the FHS was obtained with an R2 value of 0.65 and a standard error of 2.18 mm. The validation cohort showed that THA head implant size could be accurately measured with an average root-mean-squared error (RMSE) of 1.41 mm (SD = 0.97 mm; %RMSE = 4.7%). The implant head size was measured to be within 5%, 10%, and 15% RMSE in 57.5%, 93.0%, and 100.0% of cases, respectively. There was excellent intraobserver (R2= 0.94 and 0.95) and interobserver (R2= 0.94) reliability. Conclusions The novel method proposed and validated in this study, using a predicted FHS to calibrate digital images, provides an alternative means of templating THA for fractured neck of the femur patients, in whom external calibration markers are often absent.

Accuracy of Total Hip Arthroplasty Templating Using Set Calibration Magnifications.

Background Templating for total hip arthroplasty has been adopted over recent decades as a reliable and accurate method for pre-operative planning. The use of calibration markers for this process provides a recognised benefit at the expense of cost, availability and error.Many surgeons use a set magnification of 118% to account for calibration errors when templating total hip arthroplasty. This study aims to assess the accuracy of templating with standardised magnifications and assess the effect of BMI on templating accuracy. Materials and methods A retrospective analysis was performed using a single-surgeon series of 119 consecutive total hip arthroplasties. Anteroposterior radiographs were taken pre- or post-operatively without calibration hardware. Pre-operatively, the total hip arthroplasty was templated on TraumaCad (BrainLab Inc, Westchester, IL) using either 118% or 119% calibration magnification. Post-operative magnification was calibrated using the known femoral head diameter. Templated and implanted prostheses were compared for size. Results At 118%, 61.1% of cups matched those templated with 96.3% of cups within two sizes. At 119%, 52.5% of cups used matched their templates with 100%within two sizes. There was no significant difference between 118% and 119% cup size prediction (p=0.49). A trend was noticed in increasing magnification error with increasing BMI. However, BMI had no significant effect on the accuracy of templating cup size within two cup sizes (p=0.58). Conclusion. Templating acetabular cups using a set magnification of 118% or 119% yields accurate results and provides a reliable method to template without calibration equipment. Whilst BMI can affect magnification error, this has no significant effect on the accuracy of implanted cups and stems within two sizes.

A novel bi-planar calibration method for digital templating in total hip arthroplasty

In total hip arthroplasty and reconstructive orthopedic surgery, pre-operative digital templating is essential for surgical treatment optimization, risk management, and quality control. Calibration is performed before templating to address magnification effects. Conventional methods including fixed calibration factors, individual marker-based calibration and dual-scale marker methods are not reliable. A novel bi-planar calibration method is described aiming to reduce the error below clinical significance. The bi-planar calibration method requires two conventional orthogonal radiographs and a standard radiopaque marker ball. An algorithm computes the hip plane height parallel to the detector in the antero-posterior radiograph. Foreseeable errors (i.e., patient rotation and misplaced markers or lateral offset) are considered in a correction algorithm. Potential effects of errors are quantified in a standard model. Influence of rotation in lateral radiographs and lateral offset of marker on the calibration factor are quantified. Without correction, patient rotation in the lateral radiograph of 30° results in absolute calibration error of 2.2% with 0 mm offset and 6.5% with 60 mm lateral offset. The error is below the threshold of 1.5% for rotation less than 26° with 0 mm offset and 10° with 60 mm offset. The method is supposed to be reliable in precisely predicting the hip plane and thereby the calibration factor. It may be superior to other methods available. In theory, the method allows correction of clinically relevant rotation of at least 30° and marker displacement without impacting the computed calibration factor.

The effect of patient body mass index and sex on the magnification factor during pre-operative templating for total hip arthroplasty.

Pre-operative templating prior to hip arthroplasty has traditionally used implant-company-provided acetates, which assumed a magnification factor between 115% and 120%. In recent years, pre-operative planning has been performed with digital calibration devices, in order to calculate the magnification factor. However, these devices are not without their limitations and are not readily available at many institutions. As previous reports suggest a wide range of magnification factors, the determination of an optimal magnification factor is currently unclear. We investigated the relationship between obesity and gender on the magnification factor in order to improve the accuracy of pre-operative templating. Ninety-seven consecutive pre-operative calibrated pelvic radiographs using the KingMark calibration were analyzed using the TraumaCad templating software. The magnification factor calculated by the software was considered the true magnification factor and analysis was made in order to assess the effect of sex and body mass index (BMI) on the magnification factor. A linear regression analysis was utilized to create a predictive model for optimal magnification factor value. Magnification factor was significantly affected by sex (male, 120.0% vs. female 121.2%, p<0.01) and by categorized BMI (obese 121.8% vs. non-obese 119.9%, p<0.001). A positive linear association was found between BMI and the magnification factor (r=0.544). The magnification factor was significantly different between the following sub-groups: obese female, non-obese female, obese male, and non-obese male (p<0.001). When applying the model formulated by the linear regression analysis, the calculated magnification factor was within 2% of the true magnification factor for the majority of patients (n=83, 85.6%). BMI and gender have a significant effect on the magnification factor. Future determination of the magnification factor should consider the influence of these variables in order to improve the accuracy of pre-operative templating in THA.

Combining the advantages of 3-D and 2-D templating of total hip arthroplasty using a new tin-filtered ultra-low-dose CT of the hip with comparable radiation dose to conventional radiographs

BackgroundInaccurately scaled radiographs for total hip arthroplasty (THA) templating are a source of error not recognizable to the surgeon and may lead to inaccurate reconstruction and thus revision surgery or litigation. Planning based on computed tomography (CT) scans is more accurate but associated with higher radiation exposure. The aim of this study was (1) to retrospectively assess the scaling deviation of pelvic radiographs; (2) to prospectively assess the feasibility and the radiation dose of THA templating on radiograph-like images reconstructed from a tin-filtered ultra-low-dose CT dataset.Methods120 consecutive patients were retrospectively analyzed to assess the magnification error of our current THA templates. 27 consecutive patients were prospectively enrolled and a radiographic work-up in the supine position including a new tin-filtered ultra-low-dose CT scan protocol was obtained. THA was templated on both images. Radiation dose was calculated.ResultsScaling deviations between preoperative radiographs and CT of ≥ 5% were seen in 25% of the 120 retrospectively analyzed patients. Between the two templates trochanter tip distance differed significantly (Δ2.4 mm, 0–7 mm, p = 0.035)), predicted femoral shaft size/cup size was the same in 45%/41%. The radiation dose of the CT (0.58 mSv, range 0.53–0.64) was remarkably low.ConclusionScaling deviations of pelvic radiographs for templating THA may lead to planning errors of ≥ 3 mm in 25% and ≥ 6 mm in 2% of the patients. 2-D templating on radiograph-like images based on tin-filtered ultra-low-dose CT eliminates this source of error without increased radiation dose.Level of evidenceRetrospective and prospective comparative study, Level III.

Dual-scale single marker calibration for digital templating of total hip arthroplasty in standing radiographs: a prospective clinical study

PurposeAccuracy of calibration of radiographs significantly influences the quality of digital templating for total hip arthroplasty (THA). The standard of care is calibration with external calibration markers (ECM). This method is associated with significant errors. Dual-scale single marker (DSSM) calibration methods may improve accuracy. The present prospective observational study is the first to analyze the application of a DSSM method in standing pelvis radiographs.Methods100 patients with unilateral THA underwent antero-posterior pelvis radiographs with ECM and DSSM. The hip components were used as reference calibration factor (internal calibration factor; ICM). Absolute differences of calibration factors for ECM and DSSM from ICM were calculated. Absolute relative deviations (ARD) were calculated. Subgroup analysis for sex and WHO BMI category was performed. Furthermore, patients reported subjective comfort for each marker using a 10-point scale and choosing the preferred marker.ResultsMaximum magnification factor differences from the ICM were 23.3% and 9.5% and mean absolute differences were 12.5% and 2.1% for the ECM and DSSM, respectively. ARD from ICM was significantly lower for DSSM compared to ECM (p < 0.001). Absolute differences increased with BMI category using ECM; calibration by DSSM was consistent in all subgroups. Patients preferred DSSM over ECM (n = 53) or were indifferent (n = 20). Comfort was rated significantly higher for DSSM versus ECM (p < 0.001).ConclusionDSSM method showed superior results in comparison to the ECM method for calibration of digital radiographs. DSSM could be used to improve digital templating in standing radiographs.

Demographic Data Reliably Predicts Total Hip Arthroplasty Component Size

BackgroundPreoperative radiographic templating for total hip arthroplasty (THA) has been shown to be inaccurate, although essential for streamlining operating room efficiency. Although demographic data have shown to predict total knee arthroplasty component sizes, the unique contour and design among femoral stem implants have limited a similar application for hip arthroplasty. The purpose of this study was to determine whether demographic data may predict cementless THA size independent of the stem design. MethodsA consecutive series of 1,653 index cementless metaphyseal-fitting THAs were reviewed between 2007 and 2019. This included 12 unique femoral component designs, 6 acetabular component designs, 60 femur size-design combinations, and 23 acetabular size-design combinations. Implanted component sizes and patient demographic data were collected, including gender, height, weight, laterality, age, race, and ethnicity. Multivariate linear regressions were formulated to predict implanted femur and acetabular component sizes from the demographic data. ResultsThere was a significant linear correlation between gender, implant model, age, height, and weight for femur (R2 = 0.778; P < .001) and acetabular (R2 = 0.491; P < .001) sizes. Calculated femur and acetabular component sizes averaged within 0.97 and 0.95 sizes of those implants, respectively. Femur and acetabular sizes were predicted within 1 size 79.1% and 78.2% and within 2 sizes 94.3% and 94.6% of the time, respectively. ConclusionsMultivariate regression models were created based on specific demographics data to predict femur and acetabular component sizes. The model allows for simplified preoperative planning and potential cost savings implementation. A free phone application named EasyTJA was constructed for ease of implementation.

The Effect of Femoral Malrotation in Offset Templating for Total Hip Arthroplasty: A Cadaveric Study and Curvilinear Analysis

AbstractTechniques allow assessment of preoperative offset in hip arthroplasty. This study assessed femoral rotation in preoperative offset templating using a cadaveric model. Ten femurs were imaged at degrees of rotation. Offset was measured. A generalizable equation predicting change in offset was derived with a curvilinear model. Statistically significant differences at rotational positions were found. For 30 degrees of internal rotation, 2.1 mm change in adjusted mean offset existed; for 30 degrees of external rotation, 8.4 mm change existed. Improved awareness of malrotation of the proximal femur and templating adjustments may improve total hip arthroplasty outcomes. This study reflects level of evidence III.

Digital templating in total hip arthroplasty using contralateral healthy hip results in decreased accuracy

The accuracy of pre-operative digital templating for total hip arthroplasty (THA) using the diseased versus unaffected contralateral joint remains unclear. As such, we devised a study to compare templating precision between the operated hip joint versus the healthy side for patients with osteoarthritis (OA). The study hypothesis was that preoperative templating accuracy of THA on the ipsilateral diseased hip joint would be higher compared to the contralateral healthy hip in patients with OA. We retrospectively reviewed 100 patients who underwent THA for unilateral OA at our center from January 2018 to January 2020. Retrospective preoperative digital templating was performed separately on both the operated hip joint and the healthy contralateral hip joint by a single surgeon who was blinded by the in-situ components sizes. Accuracy of each group was compared to the implanted components. Assessment of the 100 included cases demonstrated superior acetabular component size prediction when templating was performed using the diseased hip compared to the healthy contralateral side (68.0% versus 51.0%, p<0.001). No differences between the cohorts were found regarding templating accuracy of femoral stem sizes (72.0% and 69.0%, p=0.375) or neck offset (73.0% and 69.0%, p=0.289). Templating acetabular cup size using the ipsilateral diseased hip is more accurate than using the contralateral healthy hip in patients with unilateral OA.

A Novel Measurement Using Digital Radiography to Minimize Fluoroscopy in Total Hip Arthroplasty

AbstractPreoperative templating for total hip arthroplasty (THA) is fraught with uncertainty. Specifically, the conventional measurement of the lesser trochanter to the center (LTC) of the femoral head used in preoperative planning is easily measured on a template but not measurable intraoperatively. The purpose of this study was to examine the utility of a novel measurement that is reproducible both on templating and in surgery as a more accurate and practical guide. We retrospectively reviewed 201 patients with a history of osteoarthritis who underwent primary THA. For preoperative templating, the distance from the top of the lesser trochanter to the equator (LeTE) of the femoral head was measured on a calibrated digital radiograph with a neutral pelvis. This measurement was used intraoperatively to guide the choice of the trial neck and head. As with any templating technique, the goal was to construct a stable, impingement-free THA with equivalent leg lengths and hip offset. In evaluating this novel templating technique, the primary outcomes measured were the number of trial reductions and the amount of fluoroscopic time, exposures, and radiation required to obtain a balanced THA reconstruction. Using the LeTE measurement, the mean number of trial reductions was 1.21, the mean number of intraoperative fluoroscopy images taken was 2.63, the mean dose of radiation exposure from fluoroscopy was 0.02 mGy, and the mean fluoroscopy time per procedure was 0.6 seconds. In hips templated with the conventional LTC prior to the LeTE, the mean fluoroscopy time was 0.9 seconds. There was a statistically significant difference in fluoroscopy time (p &lt; 0.001). The LeTE is a reproducible measurement that transfers reliably from digital templating to surgery. This novel preoperative templating metric reduces the fluoroscopy time and consequent radiation exposure to the surgical team and may minimize the number of trial reductions.

The Accuracy and Ability of Preoperative Templating to Restore Femoral Offset in Calcar-Guided Short-Stem Total Hip Prosthesis through Direct Anterior Approach

AbstractThe aim of this study was to assess the applicability and ability of preoperative templating to restore femoral offset and hip length with a calcar-guided short-stem implant design in total hip arthroplasty through the direct anterior approach. Preoperative measurements were performed of femoral offset, hip length, and stem size and compared with the perioperative placed prosthesis in 100 patients undergoing primary total hip replacement through direct anterior approach. Additionally, the pre- and postoperative femoral offset and hip length were compared to evaluate the ability to restore the offset and hip length with this kind of femoral short stem. With an acceptance of 1 size difference pre- and postoperatively, a 94% accuracy of predicting the size of the calcar-guided short stem was achieved with templating. Femoral offset was within means of 5 mm in 82.2% of the patients. Postoperative hip length was within 6 mm in 90% of the patients compared with the preoperative length.The use of preoperative templating for total hip arthroplasty with calcar-guided short-stem implants was proven to be a useful tool to predict the definite implanted size of the femoral prosthesis. Our results show that this new stem design does not significantly differ from previous reported outcomes with other stem designs and is competent to restore the femoral offset and hip length within clinical acceptable range. Implications of component size and calibration error on digital templating for total hip arthroplasty. A visual matrix from a simple mathematical model

ObjectivePreoperative digital templating is a standard procedure in total hip arthroplasty. Deviations between template size and final implant size may result from inaccurate calibration, templating as well as intraoperative decisions. So far, the explicit effect of calibration errors on templating has not been addressed adequately.Materials and MethodsA mathematical simulation of calibration errors up to ± 24% was applied to the templating of acetabular cups (38 to 72 mm diameter). The effect of calibration errors on template component size as deviation from optimal size was calculated.ResultsThe relationship between calibration error and component size deviation is inverse and linear. Calibration errors have a more pronounced effect on larger component sizes. Calibration errors of 2–6% result in templating errors of up to two component sizes. Common errors of up to 12% may result in templating errors of 3–4 sizes for common implant sizes. A tabular matrix visualizes the effect.ConclusionCalibration errors play a significant role in component size selection during digital templating. Orthopedic surgeons should be aware of this effect and try to identify and address this source of error.

The trochanteric double contour is a valuable landmark for assessing femoral offset underestimation on standard radiographs: a retrospective study

BackgroundInaccurate projection on standard pelvic radiographs leads to the underestimation of femoral offset—a critical determinant of postoperative hip function—during total hip arthroplasty (THA) templating. We noted that the posteromedial facet of the greater trochanter and piriformis fossa form a double contour on radiographs, which may be valuable in determining the risk of underestimating femoral offset. We evaluate whether projection errors can be predicted based on the double contour width.MethodsPlain anteroposterior (AP) pelvic radiographs and magnetic resonance images (MRIs) of 64 adult hips were evaluated retrospectively. Apparent femoral offset, apparent femoral head diameter and double contour widths were evaluated from the radiographs. X-ray projection errors were estimated by comparison to the true neck length measured on MRIs after calibration to the femoral heads. Multivariate analysis with backward elimination was used to detect associations between the double contour width and radiographic projection errors. Femoral offset underestimation below 10% was considered acceptable for templating.ResultsThe narrowest width of the double line between the femoral neck and piriformis fossa is significantly associated with projection error. When double line widths exceed 5 mm, the risk of projection error greater than 10% is significantly increased compared to narrower double lines, and the acceptability rate for templating drops below 80% (p = 0.02).ConclusionThe double contour width is a potential landmark for excluding pelvic AP radiographs unsuitable for THA templating due to inaccurate femoral rotation.

Accuracy of digital templating of uncemented total hip arthroplasty at a certified arthroplasty center: a retrospective comparative study

IntroductionTo investigate the accuracy of preoperative digital templating for total hip arthroplasty (THA) at a certified arthroplasty center (EndoCert EPZmax).Materials and methodsIn a retrospective study design, we analysed 620 uncemented primary THAs for templating accuracy by comparing the preoperatively planned THA component size and the implanted size as documented by the surgeon. Templating was determined to be a) exact if the planned and the implanted component were the same size and b) accurate if they were exact ± one size. Moreover, we investigated factors that potentially influence templating accuracy: overweight and obesity (WHO criteria), sex, implant design, surgeon experience, preoperative diagnosis. Digital templating was done with MediCAD software. The Mann–Whitney U test and the Kruskal–Wallis test were used for statistical analysis.ResultsTemplating was exact in 52% of stems and 51% of cups and was accurate in 90% of the stems and 85% of the cups. Regarding the factors potentially influencing templating accuracy, the type of cup implant had a significant influence (p = 0.016). Moreover, greater accuracy of stem templating was achieved in female patients (p = 0.004). No such effect was determined for the other factors investigated.ConclusionsWe conclude that preoperative 2D templating is accurate in 90% of the stems and 85% of the cups. Greater accuracy may be achieved in female patients. In addition to gender, the type of implant used may influence planning accuracy as well. Surgeon experience, BMI and preoperative diagnosis did not influence templating accuracy.Level of evidenceLevel III (retrospective comparative study with prospective cohort).

Is the contralateral lesser trochanter a reliable reference for planning of total hip arthroplasty \u2013 a 3-dimensional analysis

BackgroundPreoperative templating in total hip arthroplasty (THA) is mandatory to achieve appropriate offset and leg length equality. However, templating methods using the contralateral hip might be susceptible to errors resulting from side-differences in the femoral morphology. The distance of the lesser trochanter to the femoral head center (LTFHD) is a frequently used reference parameter for preoperative planning and intraoperative validation during THA. However, currently no three-dimensional (3D) analysis of side differences of the LTFHD exists.MethodsUsing Computer tomography (CT)-based surface models from 100 paired femora (50 cadavers), side-to-side asymmetry of the LTFHD, femoral length, femoral head diameter (FHD) and femoral antetorsion were analyzed. Univariate linear regression models were established to evaluate potential associations between sides regarding LTFHD and FHD as well as a correlation of these parameters with each other.ResultsStatistically significant side-differences were found for the LTFHD (p = 0.02) and FHD (p = 0.03) with a mean absolute side-difference of 1.6 ± 1.4mm (range 0.1–5.5mm) and 0.4mm ± 0.6mm (range 0–3mm), respectively. The ratio between the LTFHD and FHD was consistent with an average value of 1.16 ± 0.08 and reliable between sides with a correlation coefficient (r) of 0.72 (p < 0.01).ConclusionsThe LTFHD is a reliable reference parameter for preoperative templating and intraoperative validation during THA with a high correlation between sides (r = 0.93, p < 0.01). However, 8 % of the investigated specimens revealed a LTFHD of more than 4mm, which should be anticipated during THA to avoid unsatisfiable results.

Pre-operative templating for total hip arthroplasty: How does radiographic technique and calibration marker placement affect image magnification?

IntroductionPre‐operative templating using digital radiography is an effective method of planning for total hip arthroplasty (THA) and requires a generalised fixed magnification factor (MF) or external calibration markers (ECM). The effect on image magnification when changing source‐to‐image distance (SID), object‐to‐image distance (OID) and different imaging conditions is not well described. This study aims to quantify the range of effects manipulation of radiographic parameters can have on image magnification across different body habitus and imaging conditions.MethodsA simple phantom study was performed. A 25 mm ECM was placed at eight different OID values along the anterior–posterior phantom plane at three different SID values and imaging conditions, and X‐rays were obtained. On each radiograph, the ECM was measured using a line calliper tool by three radiographers and recorded. The MF was calculated and recorded.ResultsThe smallest observed image MF was 1.16, for an 8 cm OID, 120 cm SID with the ECM placed within the central ray and the X‐ray detector in bucky underneath the X‐ray table. The largest image MF was 1.40 for a 15 cm OID, 100 cm SID with the X‐ray detector placed underneath an emergency department imaging trolley.ConclusionsDigital pre‐operative templating for THA relies on accurate radiographic positioning and is dependent of the patient body habitus, radiographic parameters and imaging conditions selected by the radiographer. The use of appropriately positioned ECMs – placed medially between the patient’s internally rotated legs at the level of the greater trochanter, lowers the potential for magnification inaccuracies.

Accuracy of a New Digital Templating Method for Total Hip Arthroplasty Using Picture Archiving and Communication System (PACS) and iPhone Technology: Comparison With Acetate Templating on Digital Radiography

BackgroundPreoperative templating for total hip arthroplasty (THA) on digital radiography can be achieved using templating software or hybrid methods (acetate templates overlaid on digital images). No studies have examined templating with a mobile phone. We evaluated the accuracy and reproducibility of a new digital templating method using the picture archiving and communication system (PACS) and iPhone, compared with the hybrid method for cementless THA. MethodsA total of 113 hip radiographs were retrospectively templated by three observers. For the digital method, a circle was drawn on the acetabulum using PACS to represent the cup. The photograph of the computer screen was taken with an iPhone and imported into the Keynote presentation software. The femoral stem was then templated with transparent digital templates, which had been digitized from acetate templates. For the hybrid method, an acetate template was placed over the onscreen digital radiographs. Templated results were compared with the actual components used. ResultsThe digital method was more accurate than the hybrid method to predict ±1 size of femoral stem [93.8% (106 hips) vs 84.1% (95 hips), P = .032] and offset [90.3% (102 hips) vs 75.2% (85 hips), P = .004)]. The accuracies of digital and hybrid techniques were comparable with predict ±1 size for acetabular cup [92.9% (105 hips) vs 89.4% (101 hips), P = .483] and neck length [98.2% (111 hips) vs 96.5% (109 hips), P = .683]. Both techniques had substantial to almost perfect agreement for intraobserver and interobserver reliability. ConclusionDigital templating using PACS and iPhone is accurate and reproducible for predicting implant size of cementless THA.

A more accurate method to determine the magnification of radiographs when templating for hip arthroplasty?

The use of digital templating for Total Hip Arthroplasty (THA) is now the standard approach for pre-operative planning. Digital templating holds potential to reduce operative time and post-op complications however, this often relies on imprecise assumptions. The relationship between the X-ray source, subject and detector alters the perceived magnification. We therefore determine if Body Mass Index (BMI) is positively correlated with true magnification and if a predictive model based these parameters exists. A single surgeon series (n=107) was included in this study. Two independent observers assessed both pre- and post-operative AP pelvis radiographs using TraumaCad™. Post-operative radiographs were assessed to calculate the true magnification by calibrating from a known femoral head prosthesis size. Finally, a scatter plot with regression was used to determine if a predictive model of magnification existed using the Body Mass Index. The mean pre-operative magnification using a scaling marker was 124.2 ± 8.90%. The mean post-operative magnification using a known femoral head prosthesis size (true magnification) was 123.7 ± 3.98%. Significant variability exists in pre-operative marker data. Regression modelling showed no significant correlation between BMI and true magnification (post-op magnification). This study’s suggests that the precision and reliability of the radiographic marker in daily practice is poor. Regression modelling showed no significant correlation between BMI and the true magnification factor. Therefore, a pre-op predictive model cannot be reliably used. The data from this study suggest that a fixed magnification factor of 124% remains the most reliable and accurate method.
