Contraceptive plans before preoperative assessment and at procedure in surgical abortion patients

To describe changes in contraceptive method plans pre-appointment, after counseling, and post-procedure in patients having an abortion. We reviewed electronic medical records of University of California, Davis Health patients who had an operating room abortion from January 2015 to December 2016. We excluded persons with procedures for fetal anomaly or demise. We extracted patient demographics and contraceptive plans reported at each encounter (telephone intake, pre-operative appointment, and day of abortion). We evaluated individual contraceptive plans across the encounters, identified patient characteristics that contributed to plan change, and created a multivariable logistic regression model for predictors of contraception method plan change from telephone intake to post-procedure. The 747 patients had a mean gestational age of 16 4/7 ± 5 0/7 weeks with 244 (32.7%) <15 weeks and 235 (31.5%) ≥20 weeks. At telephone intake, 273 (36.4%) wanted a long-acting method (139 [50.9%] intrauterine device [IUD]; 99 [36.3%] implant; 35 [12.3%] unspecified), 11 (3.9%) permanent contraception, and 248 (33.2%) a less effective or no method; 215 (28.8%) stated they were undecided. Most (357/433 [82.4%]) patients who planned a reversible method based on the telephone intake obtained that or a similar method. Of the 273 patients planning a long-acting method, 258 (94.5%) received an IUD (158 [40.9%]) or implant (100 [36.6%]). Of the 215 undecided patients, 88 (40.9%) received an IUD and 55 (25.6%) an implant. No demographic factors predicted a change in method plan. Most patients will receive the method they initially identified at the telephone intake after an abortion, especially those planning an IUD or implant. Undecided patients are commonly open to discussing options.

Use of telephone intake for family history taking at a cancer genetics service in Asia.

Family history taking is a fundamental part of genetic counseling, and however, it is also a time-consuming process. To cope with the increasing demands at the Cancer Genetics Service in Singapore, alternative methods to collect patients' family history were implemented to reduce the duration of the initial consultation and increase the clinic's capacity. Two interventions were performed in this study, where a family history questionnaire and telephone intakes (telephone calls to collect patient family history) were implemented prior to a cancer genetics consultation. The primary outcome of this study is the duration of the initial consultation in relation to both interventions while the secondary outcome is the clinic attendance rate before and after implementing the telephone intake. The impact of interventions was evaluated with a Plan-Do-Study-Act (PDSA) methodology. The use of a family history questionnaire could not be evaluated due to poor patient response while the telephone intake was found to be feasible among the local population. Two improvements were observed after the implementation of telephone intake: (a) a significant reduction in the duration of the initial consultation from 60 to 45min (p=.001) and (b) a significant increase of 29.7% in clinic attendance (p=.01). This study demonstrates that collecting family history information ahead of genetic counseling via telephone intake is a useful measure in improving clinic capacity, which potentially resulting in optimization of clinical resources.

Defining Episodes of Care in Children's Mental Health Using Administrative Data.

Criteria to define an episode of care in children's mental health services are needed. Various criteria were applied to 5 years of visit data from children 4-11 years (N = 5,206) at their first visit to 1 of 3 children's mental health agencies. A minimum of 3 visits with 180 days between episodes optimized agreement with other dates (e.g., telephone intake assessment) marking the start and end of an episode, and clinician-rated number of episodes. Grouping visits into episodes provides a clearer representation of how services are distributed over extended periods of time, facilitating research and enhancing accuracy in service planning.

Diagnostic Profiles and Clinical Characteristics of Youth Referred to a Pediatric Mood Disorders Clinic

This study examined the diagnostic profiles and clinical characteristics of youth (ages 6-18 years) referred for diagnostic evaluation to a pediatric mood disorders clinic that specializes in early-onset bipolar disorder. A total of 250 youth were prescreened in an initial telephone intake, and 73 participated in a full diagnostic evaluation. Trained psychologists administered the Schedule for Affective Disorders and Schizophrenia for School-Age Children-Present and Lifetime Version (K-SADSPL) to the child and to at least one parent, and a child psychiatrist conducted a separate pharmacological evaluation. Evaluators then met with a larger clinical team for a consensus diagnosticconference. Based on consensus diagnoses, 13 of the 73 referred youth (18%) met lifetime DSM-IV-TR criteria for a bipolar spectrum disorder (BSD; bipolar I, II or not otherwise specified disorder, or cyclothymic disorder). Of these 73, 27 (37%) were referred with a community diagnosis of a bipolar spectrum disorder, but only 7 of these 27 (26%) met DSM-IV-TR criteria for a bipolar spectrum diagnosis based on a structured interview and consensus diagnoses. The most common Axis I diagnoses (based on structured interview/consensus) were attentiondeficit/hyperactivity disorder (31/73, 42.5%) and major depressive disorder (23/73, 32%). When youth referred for evaluation of BSD are diagnosed using standardized interviews with multiple reporters and consensus conferences, the "true positive" rate for bipolar spectrum diagnoses is relatively low. Reasons for the discrepancy between community and research-based diagnoses of pediatric BSD- including the tendency to stretch the BSD criteria to include children with depressive episodes and only 1-2 manic symptoms-are discussed.

Sifting the wheat from the chaff—the organization of telephone intake and the selection of social assistance inquirers in Sweden

In Sweden, the initial contact between help-seeking people and a social welfare office normally takes place over the telephone, with designated intake officers handling those aiming to apply for social assistance. Citizens seeking help are here sorted according to various eligibility criteria as well as according to different intake routines. The outcome of the sorting process is that some receive an appointment for a further assessment while others are sent away. This article describes and classifies the degree of selection at seven telephone intakes, discusses relations between the degrees of selection and reasons for selection and distinguishes several selection strategies.

The Place of Single Session Family Consultations: Five Years’ Experience in Canberra

Open Day, in the form of two half day single session family clinics, has operated weekly in the ACT Child and Adolescent Mental Health Service since April 1993 and over a thousand families have been seen. Clinicians are often resistant to the concept of single sessions and frequently overestimate the amount of assistance that clients feel they require. However in an era of sharply increasing demand for services, selective use of single family sessions for milder problems, screened by a telephone intake process, has value to both families and workers. Telephone follow‐ups of 100 families in 1994 and 70 families in 1996 found that single session family interviews were well accepted by the large majority of families seen. Clinicians see the program as reducing pressure from clients for early attention, enhancing client motivation when seen at crisis times, providing readily available consultation support from peers, increasing learning opportunities and building inter‐disciplinary team work.

Brief report a comparison of three “interventions” on pretreatment dropout rates in an outpatient substance abuse clinic

Studies examining pretreatment dropout in outpatient substance abuse clinics have consistently demonstrated that fewer days between phone contact and scheduled intake appointment result in higher client show rates. To compare time to intake with two other interventions hypothesized to increase show rates, individuals ( N = 128) seeking treatment were randomly assigned to one of four groups. Individuals in Group 1 were scheduled within 48 hours of the telephone intake. Individuals in Groups 2, 3, and 4 were scheduled 48 hours or more after the telephone intake and received either a reminder call 24 hours prior to their appointment (Group 2), an appointment card and clinic brochure in the mail (Group 3), or no intervention (Group 4). The results show that Group 1 had a higher show rate as compared to the other three groups, and receiving a reminder call or an appointment card did not improve show rates beyond that of the no-contact control group. Implications for intake procedures are discussed.

Testing clinical hypotheses by means of "falsificationist epistemology".

A family therapist using the "falsificationist epistemology" can, during the first session, test the hypotheses formulated on the telephone intake chart and also gather further information to reexamine and redefine these hypotheses. We shall examine a case treated by Dr. G. Prata, analyzing the most important phases of the first session, and stressing what methodological and tactical choices were designed to test the leading hypothesis of the "game" played by this family.

Short-term group psychotherapy was used for the screening and treatment of walk-in and telephone intake cases. The therapeutic results of the treatment of 78 cases with this method was compared with those of 90 cases treated at the same clinic with various other methods. The advantages and some of the clinical experiences from this type of group psychotherapy have been discussed and detailed.
