Predicting High-Flow Nasal Cannula Oxygen Therapy Failure in Patients With Acute Hypoxaemic Respiratory Failure Using Machine Learning: Model Development and External Validation.

To develop and validate a prediction model for high-flow nasal cannula (HFNC) failure in patients with acute hypoxaemic respiratory failure (AHRF). AHRF accounts for a major proportion of intensive care unit (ICU) admissions and is associated with high mortality. HFNC is a non-invasive respiratory support technique that can improve patient oxygenation. However, HFNC failure, defined as the need for escalation to invasive mechanical ventilation, can lead to delayed intubation, prolonged mechanical ventilation and increased risk of mortality. Timely and accurate prediction of HFNC failure has important clinical implications. Machine learning (ML) can improve clinical prediction. Multicentre observational study. This study analysed 581 patients from an academic medical centre in Boston and 180 patients from Guangzhou, China treated with HFNC for AHRF. The Boston dataset was randomly divided into a training set (90%, n = 522) and an internal validation set (10%, n = 59), and the model was externally validated using the Guangzhou dataset (n = 180). A random forest (RF)-based feature selection method was used to identify predictive factors. Nine machine learning algorithms were selected to build the predictive model. The area under the receiver operating characteristic curve (AUC) and performance evaluation parameters were used to evaluate the models. The final model included 38 features selected using the RF method, with additional input from clinical specialists. Models based on ensemble learning outperformed other models (internal validation AUC: 0.83; external validation AUC: 0.75). Important predictors of HFNC failure include Glasgow Coma Scale scores and Sequential Organ Failure Assessment scores, albumin levels measured during HFNC treatment, ROX index at ICU admission and sepsis. This study developed an interpretable ML model that accurately predicts the risk of HFNC failure in patients with AHRF. Clinicians and nurses can use ML models for early risk assessment and decision support in AHRF patients receiving HFNC. TRIPOD checklist for prediction model studies was followed in this study. Characteristics of oral mucosal pressure injuries in children with orotracheal intubation in intensive care units: An observational study.

Tracheal intubation can be used for ventilation to ensure an unobstructed respiratory tract, and it is the most common respiratory support technique used in paediatric intensive care unit (PICU) patients. Orotracheal intubation is usually the preferred method of tracheal intubation. However, it can cause stress-related damage to the oral mucosa. Identifying the factors that cause oral mucosal pressure injury (OMPI) can prevent its occurrence in children with oral endotracheal intubation. To examine the characteristics of OMPI in children who underwent orotracheal intubation in the PICU and to assess their influencing factors. An observational, prospective study. Data were gathered from the PICU of a tertiary hospital in China between January 2023 and October 2023. The patient data were obtained from the 'General Information Questionnaire', 'Paediatric Critical Illness Score', 'STRONGkids Scale' and 'OMPI Staging and Assessment Tools'. Data analysis was subsequently performed using univariate and logistic regression analyses. A total of 187 children who underwent orotracheal intubation were analysed. During the observation period, 44.92% (n = 84) of the children developed OMPI. It comprised 63.10% (n = 53) of stage I injuries, 33.33% (n = 28) of stage II injuries and 3.57% (n = 3) of stage III injuries. The common injury sites were the lower jaw (48.81%), upper jaw (29.76%), tongue (20.24%) and joints (10.71%). The logistic regression analysis results revealed that high critical illness (OR = 0.835, 95% CI: 0.726-0.961), long intubation time (OR = 1.043, 95% CI: 1.021-1.067), prone ventilation (OR = 6.708, 95% CI: 1.421-31.670), hypothermia (OR = 5.831, 95% CI: 1.208-28.149), use of dental pads (OR = 5.520, 95% CI: 1.150-26.487) and low albumin levels (OR = 6.238, 95% CI: 1.285-30.281) were the main contributing factors for OMPI in children with orotracheal intubation (p < .05). The occurrence of OMPI in children who underwent orotracheal intubation in the PICU was notable and was predominantly observed in stages I and II. Consequently, clinical nursing personnel should proactively recognize risk factors and administer timely interventions to mitigate the occurrence of OMPI in such children. The incidence of OMPI in children who underwent orotracheal intubation was relatively high. Nurses and doctors should closely monitor the risk factors for orotracheal intubation in children to prevent the occurrence of OMPI.

Focus on the Role of Non-Invasive Respiratory Support (NRS) during Palliative Care in Patients with Life-Limiting Respiratory Disease.

The management of patients with life-threatening respiratory disease in the ICU and at home has become increasingly of interest over the past decades. Growing knowledge supports the use of NRS, aimed at improving patient comfort and improving quality of life. However, its role during palliative care is not well defined, and evidence of support remains limited. The aim of this narrative review is to examine the recent evidence relating to the use of non-invasive respiratory support at the end of life, in order to clarify who benefits and when. The literature research was conducted on PubMed, using MeSH words. A review of the relevant literature showed that non-invasive respiratory support techniques for patients with life-limiting respiratory disease vary (from high-flow oxygen therapy to conventional oxygen therapy, from CPAP to NPPV) and each has precise indications. To date, from the hospital to the home setting, the monitoring and application of these respiratory support techniques have varied widely. In conclusion, the choice of respiratory support in this category of patients should be based on the technique that will optimize the comfort of the patient and improve the quality of their life. On the other hand, regarding monitoring, both telemedicine and ultrasound diagnostics help to satisfy the patient's wish to spend the last period of his life in the home environment, to avoid inappropriately aggressive diagnostic interventions, and to reduce the high costs of hospitalized procedures in this category of patients.

High-Flow Nasal Cannula Therapy as an Adjuvant Therapy for Respiratory Support during Endoscopic Techniques: A Narrative Review.

High-flow nasal cannula (HFNC) is a respiratory support technique that delivers a controlled concentration of oxygen with high flow, heat, and humidity via the nasal pathway. As it has many physiological effects, its use has increased for a variety of clinical indications; however, there is limited guidance on using HFNC as a respiratory support tool during endoscopic procedures. We conducted a narrative review to evaluate the effect of HFNC as an adjuvant tool during fiberoptic bronchoscopy (FOB), upper gastrointestinal tract endoscopy, and surgical procedures in adults. A search of the PubMed and Cochrane databases were performed. Approximately 384 publications were retrieved, and 99 were selected (93 original works and 6 case reports with a literature review). In patients who underwent FOB, HFNC appears to be superior to conventional oxygen therapy (COT) in preventing hypoxaemia. In contrast, for gastrointestinal endoscopy, the current evidence is insufficient to recommend HFNC over COT in a cost-effective manner. Finally, in surgical procedures such as laryngeal microsurgery or thoracic surgery, HFNC has been shown to be a safe and effective alternative to orotracheal intubation. As the results are heterogeneous, we advocate for the need for more quality studies to understand the effectiveness of HFNC during endoscopic procedures.

Use of veno-venous extracorporeal membrane oxygenation in a female patient with severe acute respiratory distress syndrome and peri-intubation cardiac arrest. Clinical observation

Acute respiratory distress syndrome (ARDS) is a life-threatening form of respiratory failure which can occur in people of any age. It comprises about 10 % of all admissions in intensive care units. The cornerstone of treatment of severe forms of this disorder is protective mechanical ventilation but in patients with initial hypoxia and/or hypotension tracheal intubation is associated with the risk of significant hemodynamic abnormalities. Mortality from severe forms of ARDS remains high. COVID-19 pandemic led to worldwide wider use of such techniques of respiratory support as high-flow nasal oxygen therapy and non-invasive ventilation, the necessity of which is proposed to be used as one of classifying signs of ARDS in addition to the Berlin criteria. If the respiratory therapy is ineffective, veno-venous extracorporeal membrane oxygenation is used for gaseous exchange and decreasing undesirable effects of high pressure in the respiratory pathways. The success of its application strongly depends on the optimal timing of its initialization. The performance of this technique is associated with a number of complications (hemorrhage, thrombosis, sepsis, etc.). Effective treatment of ARDS requires utilization of a large amount of resources and educated staff.

Physical exercise-intervention can be valuable therapy for COVID-19 confinement and post-COVID-19 periods.

The COVID-19 pandemic and its transition into an endemic phase have profoundly impacted physical health, well-being, mental health, education, and various aspects of society, including the economy and social networks. Home confinement, social distancing, and physical inactivity have exacerbated numerous health issues, including obesity, diabetes mellitus, hypertension, hyperlipidemia, cardiovascular diseases, depression, and poor sleep quality. A systematic review has revealed significant findings: Regular aerobic programs (such as cycling or walking at an intensity of 60-80% of HR max for 20-60 minutes per session, repeated 2-3 times a week) have proven effective in improving both physical and mental health, as well as immune function. This type of physical activity has been shown to increase immunological markers, including lymphocytes, leukocytes, neutrophils, monocytes, and interleukin-6 (IL-6), while reducing low-grade inflammation. Therefore, in this study we aimed to assess the impact of tailored exercise interventions on the physical and mental health of COVID-19 patients. Based on the results, we can establish exercise intervention strategies to mitigate the negative health consequences during and after the COVID-19 pandemic. We conducted a search of the PubMed database from January 2020 to August 2023 using predefined search terms such as "COVID-19 and post-COVID-19," "exercise intervention and immunity," and "mental health." By examining references, we explored the links between exercise interventions and the mental and physical health of COVID-19 patients. A tailored, multifaceted exercise intervention should be developed and implemented to address the existing mental challenges and enhance mental health during both the pandemic and the post-COVID-19 periods. Breathing exercises and respiratory support techniques, including yoga, thoracic expansion exercises, airway clearance methods, and breathing control, are likely to be beneficial.

Design and application of decompression fixator to prevent HFNC facial pressure injury

As a new respiratory support technique, high-flow nasal cannula oxygen therapy (HFNC) has been widely used in clinical practice in recent years. During HFNC treatment, due to the long time and continuous wearing of nasal stopper and fasteners on the patient face, it is easy to cause medical device-related pressure injury on multiple facial skin. Moreover, when the patient's position changes greatly, because there is no good fixed design at the HFNC nasal stopper, it is easy to shift or turn the nasal stopper outward, causing abnormal ventilation and failure to achieve the purpose of clinical oxygen therapy. To overcome above problems, medical staff in the intensive care unit of department of infectious diseases, Tongji Hospital Tongji Medical College of HUST designed a new type of decompression fixator to prevent HFNC face pressure injury, and obtained national utility model patent (ZL 2022 2 0754626.1). The integrated design structure of the device has the functions of decompression of facial skin and fixation of nasal stopper, which can ensure the effect of oxygen therapy and improve the oxygen therapy experience and patient comfort, which is suitable for clinical promotion.

The effect of using BCPAP in the management of Respiratory Distress

In the developing world, one of the biggest cause why infant die is respiratory distress. A noninvasive respiratory support technique called bubble continuous positive airway pressure (BCPAP) is used to treat neonates who are experiencing respiratory distress. It was originally mentioned in 1971 as a way to help premature newborns' respiration. BCPAP is a less invasive method of providing respiratory assistance to newborns than mechanical ventilation. Method: 63 babies with respiratory distress were put on BCPAP as part of a clinical trial study that was undertaken at Al-Forat Al-Awsat Teaching Hospital in An'najaf city from February 1 to June 30, 2018. Results: It was shown that although 38 patients (60.31%) with respiratory distress were helped and effectively weaned off of BCPAP, CPAP failed to help 25 patients (39.68%) with respiratory distress, and they died as a result. Conclusion: BCPAP was beneficial in treating respiratory distress.

Noise Produced by Neonatal Ventilators Inside and Outside of the Incubators.

Insufficient data are available about the noise produced by modern neonatal ventilators. We aimed to measure their noise under different ventilatory modes and parameters. This was a bench study measuring the noise produced by 9 neonatal ventilators set in conventional or high-frequency oscillatory ventilation (HFOV), nasal mask-delivered CPAP with variable- or continuous-flow configuration, or bi-level positive airway pressure (considered as noninvasive ventilation [NIV]). Conventional ventilation and HFOV were tested in 2 distinct settings with moderate or higher parameters. Sound measurements were performed inside and outside an incubator mimicking the clinical setting and using a high-end meter meeting the international ISO 226:2003 standard. Four ventilators remained below the internationally recommended safety threshold but only for measurements outside the incubator. Conventional ventilation (49.1 [3.4] dBA) and HFOV (56.3 [5.2] dBA) were the least and most noisy respiratory support technique, respectively. Noise was greater inside than outside the incubators (P < .0001) and different between the ventilators (P < .0001); better results were achieved by Servo-u and Fabian family devices for conventional ventilation; by fabian HFO for HFOV; and by Servo-u, VN500, and fabian family devices for CPAP and NIV. Noise levels were similar when using moderate or higher parameters in conventional ventilation (P = .81) and in HFOV (P = .45). Modern ventilators often produce relevant noise, independent of the respiratory support modality, with acceptable noise levels being measured only outside the incubator. Better results were achieved with Servo-u, VN500, and Fabian family devices.

Respiratory Support Techniques for COVID-19-Related ARDS in a Sub-Saharan African Country: A Multicenter Observational Study

Limited data from low-income countries report on respiratory support techniques in COVID-19-associated ARDS. Which respiratory support techniques are used in patients with COVID-19-associated ARDS in Uganda? A multicenter, prospective, observational study was conducted at 13 Ugandan hospitals during the pandemic and included adults with COVID-19-associated ARDS. Patient characteristics, clinical and laboratory data, initial and most advanced respiratory support techniques, and 28-day mortality were recorded. Standard tests, log-rank tests, and logistic regression analyses were used for statistical analyses. Four hundred ninety-nine patients with COVID-19-associated ARDS (mild, n= 137; moderate, n= 247; and severe, n= 115) were included (ICU admission, 38.9%). Standard oxygen therapy (SOX), high-flow nasal oxygen (HFNO), CPAP, noninvasive ventilation (NIV), and invasive mechanical ventilation (IMV) was used as the first-line (most advanced) respiratory support technique in 37.3%(35.3%), 10%(9.4%), 11.6%(4.8%), 23.4%(14.4%), and 17.6%(36.6%) of patients, respectively. The first-line respiratory support technique was escalated in 19.8%of patients. Twenty-eight-day mortality was 51.9%(mild ARDS, 13.1%; moderate ARDS, 62.3%; severe ARDS, 75.7%; P< .001) and was associated with respiratory support techniques as follows: SOX, 19.9%; HFNO, 31.9%; CPAP, 58.3%; NIV 61.1%; and IMV, 83.9%(P< .001). Proning was used in 79 patients (15.8%; 59 of 79 awake) and was associated with lower mortality (40.5%vs54%; P= .03). The oxygen saturation to Fio2 ratio (OR, 0.99; 95%CI, 0.98-0.99; P< .001) and respiratory rate (OR, 1.07; 95%CI, 1.03-1.12; P= .002) at admission and NIV (OR, 6.31; 95%CI, 2.29-17.37; P< .001) or IMV (OR, 8.08; 95%CI, 3.52-18.57; P< .001) use were independent risk factors for death. SOX, HFNO, CPAP, NIV, and IMV were used as respiratory support techniques in patients with COVID-19-associated ARDS in Uganda. Although these data are observational, they suggest that the use of SOX and HFNO therapy as well as awake proning are associated with a lower mortality resulting from COVID-19-associated ARDS in a resource-limited setting.

Algorithms for management of patients with severe A/H1N1 influenza: from diagnosis to respiratory support techniques. Guidelines

In 2022, the incidence of swine (A/H1N1) influenza markedly increased. It causes acute lung injury similar to that caused by SARS-CoV-2. These practice guidelines focus on the management of patients with severe influenza: from the diagnostic algorithm to the specifics of therapy and the use of respiratory support techniques. The guidelines provide recommendations on the rational pharmacotherapy principles: antiviral, antibacterial, mucoactive, anticoagulant, and symptomatic. The need to avoid prescribing drugs that may worsen the condition of patients with A/H1N1 influenza that are effective in other viral lung diseases, including SARS-CoV-2, has been pointed out. A detailed review of the criteria for initiation and principles of respiratory support is given.

Efficacy of positive airway pressure therapy and high flow nasal cannula oxygen in acute cardiogenic pulmonary oedema: A protocol for systematic review and network meta-analysis.

Positive airway pressure (PAP) therapy is currently the first-line respiratory support technique for acute respiratory failure (ARF) due to acute cardiogenic pulmonary edema (ACPE), but the accompanied adverse events and patient's intolerance with treatment in some cases limited its use in clinical practice. Some recent trials indicated that high-flow nasal cannula oxygen (HFNO) is a promising alternative to PAP therapy. In order to choose the optimum treatment for patients with ACPE, this network meta-analysis will firstly compares the efficacy of HFNO, PAP, and conventional oxygen therapy (COT). The Preferred Reporting Items for Systematic Review and Meta-Analysis Protocols 2015 statement and its extension for network meta-analysis will be followed in the conduct of this investigation. We will examine these databases: PubMed, EMBASE, Cochrane Central Register of Controlled Trials and Web of Science. The ClinicalTrials.gov and World Health Organization International Clinical Trials Registry Platform Search Portal will be used to search ongoing trials. Only randomized controlled trials meeting the eligibility criteria will be included. Through the Cochrane Collaboration's tool, the included studies' risk of bias will be assessed. The pairwise meta-analysis will be performed with RevMan 5.4.1 software. A Bayesian network meta-analysis will use random-effects models to derive odds ratios for the treatment effects of all interventions compared to each other using R software (version 3.6.1), and the rjags and gemtc packages. The Q statistic and I2 index will be used for investigating the heterogeneity, and subgroup analysis or sensitivity analysis will be used to explore the source of heterogeneity. In addition, the Grading of Recommendations Assessment, Development and Evaluation system will be used to inspect the quality of evidence.

The physiological underpinnings of life-saving respiratory support.

Treatment of respiratory failure has improved dramatically since the polio epidemic in the 1950s with the use of invasive techniques for respiratory support: mechanical ventilation and extracorporeal respiratory support. However, respiratory support is only a supportive therapy, designed to “buy time” while the disease causing respiratory failure abates. It ensures viable gas exchange and prevents cardiorespiratory collapse in the context of excessive loads. Because the use of invasive modalities of respiratory support is also associated with substantial harm, it remains the responsibility of the clinician to minimize such hazards. Direct iatrogenic consequences of mechanical ventilation include the risk to the lung (ventilator-induced lung injury) and the diaphragm (ventilator-induced diaphragm dysfunction and other forms of myotrauma). Adverse consequences on hemodynamics can also be significant. Indirect consequences (e.g., immobilization, sleep disruption) can have devastating long-term effects. Increasing awareness and understanding of these mechanisms of injury has led to a change in the philosophy of care with a shift from aiming to normalize gases toward minimizing harm. Lung (and more recently also diaphragm) protective ventilation strategies include the use of extracorporeal respiratory support when the risk of ventilation becomes excessive. This review provides an overview of the historical background of respiratory support, pathophysiology of respiratory failure and rationale for respiratory support, iatrogenic consequences from mechanical ventilation, specifics of the implementation of mechanical ventilation, and role of extracorporeal respiratory support. It highlights the need for appropriate monitoring to estimate risks and to individualize ventilation and sedation to provide safe respiratory support to each patient.

Analysis of risk factors that determine the severity of transient tachypnea of the newborn and allow predicting treatment tactics

Transient tachypnea of the newborn is a parenchymal lung disease characterized by respiratory distress in the first hours after birth. The consequences of underestimating the severity of the disease, incorrectly chosen respiratory support technique or its untimely use at the stage of the delivery room are the increase in respiratory failure, transfer to the intensive care unit, and the need for more invasive and expensive methods of treatment. In the study, we set the task to determine the risk factors that allow us to predict the severity of the course of transient tachypnea of the newborn at the stage of the delivery room.Purpose. Analysis of risk factors that determine the severity of transient tachypnea in the newborn and predicting the tactics of their treatment.Methods. Retrospective analysis of full-term newborns (n = 201) diagnosed with transient tachypnea of the newborn in 2020, who received any type of respiratory support in the first hours of life.Results. Most patients with transient tachypnea of the newborn required respiratory support at the stage of the delivery room, which we associate with a combination of risk factors in the history of pregnancy and childbirth. A high incidence of cerebral pathology in the studied newborns was revealed. According to our data, the following factors can be considered as predictors of the severity of the condition and hospitalization in the intensive care unit from the delivery room of patients with transient tachypnea of the newborn: delivery by Casarean section, low Apgar score at 1 and 5 minutes, Downs scale score of 4–5 points, and the need for respiratory support during the delivery room stage.Conclusions. The results of the study proved a high frequency of transient tachypnea of the newborn registered in full-term infants, allowed to identify risk factors for a severe course of transient tachypnea of the newborn and hospitalization in the intensive care unit.

Negative-Pressure Ventilation in Neuromuscular Diseases in the Acute Setting.

Mechanical ventilation started with negative-pressure ventilation (NPV) during the 1950s to assist patients with respiratory failure, secondary to poliomyelitis. Over the years, technological evolution has allowed for the development of more comfortable devices, leading to an increased interest in NPV. The patients affected by neuromuscular diseases (NMD) with chronic and acute respiratory failure (ARF) may benefit from NPV. The knowledge of the available respiratory-support techniques, indications, contraindications, and adverse effects is necessary to offer the patient a personalized treatment that considers the pathology’s complexity.

Notes on the epidemiology of artificial mechanical ventilation and stroke

Artificial mechanical ventilation in the course of stroke presents a high mortality. Even today, with advanced knowledge in the physiopathology of the neurological event and modern techniques of respiratory support, it continues to be a problem of interest, which invites to reflect on the benefits of ventilation in this field. In Cuba, there is not enough research on the subject, however, mortality due to this cause is high. This phenomenon, which implies high costs and limited functional recovery in survivors, should continue to be studied.

Noninvasive High-Frequency Oscillatory Ventilation vs Nasal Continuous Positive Airway Pressure vs Nasal Intermittent Positive Pressure Ventilation as Postextubation Support for Preterm Neonates in China

Several respiratory support techniques are available to minimize the use of invasive mechanical ventilation (IMV) in preterm neonates. It is unknown whether noninvasive high-frequency oscillatory ventilation (NHFOV) is more efficacious than nasal continuous positive airway pressure (NCPAP) or nasal intermittent positive pressure ventilation (NIPPV) in preterm neonates after their first extubation. To test the hypothesis that NHFOV is more efficacious than NCPAP or NIPPV in reducing IMV after extubation and until neonatal intensive care unit discharge among preterm neonates. This multicenter, pathophysiology-based, assessor-blinded, 3-group, randomized clinical trial was conducted in 69 tertiary referral neonatal intensive care units in China, recruiting participants from December 1, 2017, to May 31, 2021. Preterm neonates who were between the gestational age of 25 weeks plus 0 days and 32 weeks plus 6 days and were ready to be extubated were randomized to receive NCPAP, NIPPV or NHFOV. Data were analyzed on an intention-to-treat basis. The NCPAP, NIPPV, or NHFOV treatment was initiated after the first extubation and lasted until discharge. Primary outcomes were total duration of IMV, need for reintubation, and ventilator-free days. These outcomes were chosen to describe the effect of noninvasive ventilation strategy on the general need for IMV. A total of 1440 neonates (mean [SD] age at birth, 29.4 [1.8] weeks; 860 boys [59.7%]) were included in the trial. Duration of IMV was longer in NIPPV (mean difference, 1.2; 95% CI, 0.01-2.3 days; P = .04) and NCPAP (mean difference, 1.5 days; 95% CI, 0.3-2.7 days; P = .01) compared with NHFOV. Neonates who were treated with NCPAP needed reintubations more often than those who were treated with NIPPV (risk difference: 8.1%; 95% CI, 2.9%-13.3%; P = .003) and NHFOV (risk difference, 12.5%; 95% CI, 7.5%-17.4%; P < .001). There were fewer ventilator-free days in neonates treated with NCPAP than in those treated with NIPPV (median [25th-75th percentile] difference, -3 [-6 to -1] days; P = .01). There were no differences between secondary efficacy or safety outcomes, except for the use of postnatal corticosteroids (lower in NHFOV than in NCPAP group; risk difference, 7.3%; 95% CI, 2.6%-12%; P = .002), weekly weight gain (higher in NHFOV than in NCPAP group; mean difference, -0.9 g/d; 95% CI, -1.8 to 0 g/d; P = .04), and duration of study intervention (shorter in NHFOV than in NIPPV group; median [25th-75th percentile] difference, -1 [-3 to 0] days; P = .01). Results of this trial indicated that NHFOV, if used after extubation and until discharge, slightly reduced the duration of IMV in preterm neonates, and both NHFOV and NIPPV resulted in a lower risk of reintubation than NCPAP. SIMEU position paper on non-invasive respiratory support in COVID-19 pneumonia.

The rapid worldwide spread of the Coronavirus disease (COVID-19) crisis has put health systems under pressure to a level never experienced before, putting intensive care units in a position to fail to meet an exponentially growing demand. The main clinical feature of the disease is a progressive arterial hypoxemia which rapidly leads to ARDS which makes the use of intensive care and mechanical ventilation almost inevitable. The difficulty of health systems to guarantee a corresponding supply of resources in intensive care, together with the uncertain results reported in the literature with respect to patients who undergo early conventional ventilation, make the search for alternative methods of oxygenation and ventilation and potentially preventive of the need for tracheal intubation, such as non-invasive respiratory support techniques particularly valuable. In this context, the Emergency Department, located between the area outside the hospital and hospital ward and ICU, assumes the role of a crucial junction, due to the possibility of applying these techniques at a sufficiently early stage and being able to rapidly evaluate their effectiveness. This position paper describes the indications for the use of non-invasive respiratory support techniques in respiratory failure secondary to COVID-19-related pneumonia, formulated by the Non-invasive Ventilation Faculty of the Italian Society of Emergency Medicine (SIMEU) on the base of what is available in the literature and on the authors' direct experience. Rationale, literature, tips & tricks, resources, risks and expected results, and patient interaction will be discussed for each one of the escalating non-invasive respiratory techniques: standard oxygen, HFNCO, CPAP, NIPPV, and awake self-repositioning. The final chapter describes our suggested approach to the failing patient.
