Umbilicus Reconstruction with Bilateral Square "Pumpkin-Teeth" Advancement Flaps.

The creation of a neoumbilicus is a common challenge for plastic surgeons. The authors present their technique of umbilical reconstruction with bilateral square "pumpkin-teeth" advancement flaps. The technique described is applicable in both the setting of a vertical midline incision and when no midline incision is present (e.g., an abdominoplasty). The reconstruction has been performed in over 50 patients with a vertical midline incision and five patients without a midline incision. The only complications noted were persistent serous drainage in several patients with vertical incision and inadequate depth in one patient without a midline incision. This technique is quickly performed, and avoids small flaps, external incisions, and healing by secondary intention.

Umbilical reconstruction with the bow tie flap

The umbilicus can be absent in congenital malformations that are associated to umbilical agenesia such as bladder exstrophy, gastroschisis or omphalocele or it can be excised during surgical procedures such as umbilical herniorrhaphy, abdominoplasty and laparotomy. We report a new technique for umbilical reconstruction, using a “bow tie”-shaped flap, partially made of scar tissue. We treated three female patients with absent umbilicus as a consequence of congenital malformations or previous surgical treatments. This method provided a good conical shape to the neoumbilicus with adequate depth and a wide external ring. Follow-up at 2 years showed that a satisfactory shape was maintained. Previously described techniques for neoumbilicoplasty were unsatisfying or seemed too complex in our hands. The reported technique is easy and simple, with good, stable and natural aesthetic results, and it can be effectively used for umbilical reconstruction in all primary or secondary cases of umbilical absence.

A Novel Technique for Umbilical Reconstruction Using Four Transposition Flaps

The umbilicus is of paramount aesthetic importance for the abdomen, and its absence can be psychologically distressing to patients for cosmetic reasons. An aestheticallypleasing umbilicus tends to be small and vertically oriented in nature, with superior hooding and shadow, inferior retraction and slope, and a position at the topmost level of the iliac crest. A 42-year-old woman had undergone delayed breast reconstruction using a transverse rectus abdominis myocutaneous (TRAM) flap. Unfortunately, the patient developed umbilical necrosis following surgery. She underwent complete surgical debridement, which resulted in a large vertical scar. Delayed umbilical reconstruction was performed with four transposition flaps. The patient underwent follow-up at 11 months postoperatively, and the umbilicus had a satisfactory appearance. This case shows that using four transposition flaps can yield sufficient depth and an aesthetically pleasing shape for the umbilicus.

Umbilical Reconstruction as an Adjuvant Procedure After Umbilical Hernia Repair or Midline Abdominal Incision

Introduction: Umbilicus construction is usually needed post umbilical or Para umbilical hernia repair and in some techniques the umbilicus needs reconstruction after exploratory laparotomy. There are many techniques for umbilical reconstruction and the common objective of all these techniques is to mention umbilicus that looks natural in terms of location, size and depth. And avoid ischemic necrosis of the edges of the umbilicus flap. The author in this study described a new and simple technique for umbilical reconstruction. Patients and methods: The present study included 50 cases who were subjected to anterior abdominal wall incision (umbilical, Para umbilical or midline incision) Crimean Medical Academy named after S.I. Georgievsky Crimean Federal University named after V.I. Vernadsk Russia in department of general and gastrointestinal surgery. The study started from January 2012 to may 2015. End points: The primary end point of the study was the aesthetic appearance of the new umbilicus and the second end point was patient satisfaction of the operative outcome. Results: Over 50 cases of midline abdominal incision we performed reconstruction of the umbilicus with a new and simple technique. There were 40 females and 10 males with age ranging from 28 to 52 years with the mean value as 40 ± 7.35 years. Conclusion: The technique for reconstruction of the umbilicus presented in the present study is a simple technique without complications, with long term success and good aesthetic appearance. The new constructed umbilicus exhibits appropriate features of both depth and size and avoids the appearance of scarring and secondary stenosis or necrosis.

A New Umbilical Reconstruction Technique Used for 306 Consecutive Abdominoplasties

The search for a natural-appearing umbilicus that does not compromise the aesthetic outcome of abdominoplasty began in the 1950s and coincides with the period that saw increasing exposure of the female body. Since then, umbilical transposition has been favored by most surgeons. This report describes a new technique for umbilical reconstruction using a small X-shaped incision that creates four V-shaped flaps, which are sutured to the aponeurosis with absorbable sutures. This technique was used for 306 consecutive abdominoplasty patients, and a survey was performed to determine their level of satisfaction with the surgical outcome. A natural-appearing new umbilicus was created with no visible scars, a low complication rate (9 %), and a high level of patient satisfaction. The described technique had a low complication rate and allowed the creation of a new umbilicus with a natural appearance in a rapid, safe, and simple manner. This journal requires that authors assign a level of evidence to each article.

Umbilical reconstruction after repair of large umbilical hernia: the “lazy-M” and omega flaps

A simple and easy-to-perform technique of umbilical reconstruction after repair of a large umbilical hernia is described. Two opposing skin flaps, an upper inverted Omega shaped flap, and a lower, lazy M-shaped flap were designed to create a deep, 3-dimensional, normal-appearing umbilicus in identical twins.

Reconstruction of an Extreme Frontal and Frontobasal Defect by Microvascular Tissue Transfer and a Prefabricated Titanium Implant

This article presents our technique of umbilical reconstruction after the repair of omphalocele and gastroschisis. We have treated 8 patients with an average follow-up period of 13 months (range, 6 approximately 24 months). No major complications have occurred; minor complications have included delayed wound healing, decreased umbilical depth, and hematoma. Our procedure is especially useful for patients who have a midline abdominal scar and relatively intact bilateral rectus abdominis muscles. Most of the patients and their parents have been satisfied with the results of umbilical reconstruction.

New technique for scarless umbilical reinsertion in abdominoplasty procedures.

The most visible scar in the conventional abdominoplasty procedure or TRAM flap donor site closure is the scar around the reinserted umbilicus. In an attempt to increase the aesthetic outcome, the authors have introduced a new technique of umbilical reconstruction and reinsertion during abdominoplasty avoiding any paraumbilical scar, which results in a more natural appearance in that area. This new technique, potential pitfalls, and long-term results are reported.

A method of umbilical reconstruction

A technique of umbilical reconstruction is described. This procedure uses a tubed skin flap consisting of scar tissue; a deep and cosmetically excellent umbilicus is achieved even in patients with a thin abdominal wall consisting entirely of scar tissue. This is especially suitable in the cases of deficient umbilicus with a significant vertical scar and a thin abdominal wall due to the prior trauma or surgery.
