Synovial chondromatosis of knee joint: a case report

<p class="abstract">Synovial chondromatosis is a benign condition encountered infrequently within joints which is also known as synovial osteochondromatosis. They present as multiple nodules in the synovium which are either fibrin type or cartilaginous type. The fibrin type usually results from bleeding within the joint, rheumatoid arthritis and tuberculosis. The cartilaginous type is usually due to trauma or osteoarthritis. This typically involves large joints, knee being the most common, rarely small joints also may be involved. Here we report a case of 30-year-old female who presented to us with symptoms of swelling, restriction of movement and pain in the right knee. After clinical and radiological assessment, arthroscopic exploration with loose body removal and synovectomy done, following which patient achieved painless complete range of movements. Histopathological examination confirmed the diagnosis as synovial chondromatosis.</p>

A case of synovial chondromatosis of knee joint treated with total knee arthroplasty

Synovial Chondromatosis is a rare disease of the synovial membrane characterised by formation of multiple loose bodies in a joint. Knee is the most common joint to be affected by this disease. Though, this disease can be managed by synovectomy and removal of loose bodies with a favourable functional outcome but in some cases where there is damage of articular cartilage, simple synovectomy might not be sufficient. Here, we present a case of right knee pain in a 53 year old male since 4 years with a established diagnosis of synovial chondromatosis. No other joint was affected. Pain was associated with a fixed flexion deformity and restriction of knee range of motion and multiple slipping bodies. Radiography showed multiple loose bodies in the tibio-femoral as well as in patello femoral compartment. MRI of knee revealed several damaged areas in the knee articular surface. As a result, decision was taken to perform total knee arthroplasty. Intraoperatively all the loose bodies were removed along with the affected synovium and arthroplasty was done. Patient was discharged in fourth post operative day with proper instructions and was called for follow up at 1 month, 3 month, 6 month, 1 year. Functional outcome was assessed by Tegner Lysholm knee score, Knee Society score and Visual Assessment Scale.

Arthroscopic Treatment of Localized Synovial Chondromatosis of the Posterior Knee Joint

Synovial chondromatosis is an uncommon, benign neoplastic process typically affecting adult men and most commonly involving the knee, hip, and elbow joints. Intra-articular primary synovial chondromatosis in the knee joint may be localized or generalized. The symptoms associated with localized synovial chondromatosis may include diffuse pain, swelling, restricted motion, crepitus with symptoms of locking and giving way, and posteromedial tenderness mimicking medial meniscus tear. Diagnosis is usually established with magnetic resonance imaging (MRI) and confirmed by histological examination. To our knowledge, synovial chondromatosis of the knee joint in the posteromedial corner has not been reported in the literature.
