Colonic Anastomosis Using the Valtrac Biofragmentable Anastomosis Ring: A New and Useful Surgical Technique in Gynecologic Oncology

Patients with a primary malignancy of the female genital tract may present with the majority of their disease confined to the pelvis. Not infrequently, infiltrating contiguous extension of disease may involve the rectosigmoid colon, resulting in symptoms of partial obstruction. This presentation in the patient with an epithelial ovarian malignancy may represent a patient that can be adequately cytoreduced if a segmental resection of the rectosigmoid colon is performed. Following resection, the continuity of the lower GI tract can be restored by anastomosis of the proximal and distal ends of the colon. In our institution three patients explored for large abdominal pelvic masses required segmental resection of the rectosigmoid colon. In each patient the colonic anastomosis was performed using the valtrac biofragmentable anastomosis ring (V-BAR). None of the patients experienced any intraoperative complications, and postoperatively there was no evidence of any anastomotic leaks. The average time of return of GI function was 6 days and there was no prolongation of their hospital stay. Currently, with 3 months of follow-up no patients have reported any symptoms suggestive of anastomotic stricture. Although our experience is limited, we found that this technique can be performed much faster than traditional hand-sewn or staple techniques. We submit that in the presence of uncompromised colon, the V-BAR is a safe and effective new alternative for colonic anastomosis in gynecologic oncological surgery.

SUPERIOR VENA CAVA—RIGHT MAIN PULMONARY ARTERY ANASTOMOSIS

Ebstein's anomaly and congenital hypoplasia of the right ventricle give rise to signs and symptoms of partial obstruction to outflow from the right atrium, and this may lead to right heart failure. Successful surgical correction or relief of these defects has not been previously reported. Anastomoses between the superior vena cava and the right pulmonary artery have been established in a 4-year-old girl with congenital hypoplasia of the right ventricle and in two patients, aged 8 and 14 years, with Ebstein's anomaly. Relief of the distention and of the elevated pressure in the right atrium was immediately apparent in these three patients. The 14-year-old girl died four days after operation, and autopsy revealed a thrombosis of the left main pulmonary artery, but the two patients aged 4 and 8 years continued to show marked clinical improvement. The desirability of eventual closure of the septal defect remains to be decided.

Intraluminal lipoma of small intestine producing intussusception: Report of a case

<h2>Summary</h2> A case of intussusception due to an intraluminal lipoma of the terminal ileum is reported. Benign tumors of the small intestine are rare, occurring only slightly more than half as frequently as malignant tumors of the small intestine. The preoperative diagnosis may be made by x-ray and by the symptoms of partial obstruction or intussusception in an adult, and the findings of a palpable, mobile tumor mass within the abdominal cavity. The treatment is surgical and should be done promptly if signs of intussusception or acute intestinal obstruction are present.
