The complex relation between obstructive sleep apnoea syndrome, hypogonadism and testosterone replacement therapy.

Obstructive sleep apnoea syndrome (OSAS) is an under-recognized medical disease. The main risk factors for OSAS are male sex, older age, obesity, and metabolic syndrome, that are also associated with male hypogonadism (MH). Therefore, obesity has been classically identified as the most evident link between OSAS and MH. However, OSAS is per se linked to the development of MH by a combined effect of hypoxia, increased night-time awakenings, reduced sleep efficiency and fragmented sleep. Similarly, MH might represent a risk factor for OSAS, mainly related to sleep disturbances that are frequently associated with low testosterone. Data on testosterone replacement therapy (TRT) in patients with OSAS are limited. Nevertheless, TRT is generally contraindicated by guidelines in the presence of untreated or severe OSAS. TRT might in fact worse OSAS symptoms in different ways. Furthermore, OSAS has been proposed to be a risk factor for secondary polycythaemia and TRT might exacerbate polycythaemia. Therefore, TRT in hypogonadal men affected by untreated OSAS or severe OSAS should be considered with caution and in a personalised way. Nevertheless, the type and dosage of TRT should be considered, as short-term high-dose TRT might worsen OSAS, whereas long-term lower doses could eventually determine a clinical improvement of symptoms of OSAS. Here we reviewed the data on the association between OSAS, MH and TRT, including the opportunity of assessment of patients who develop signs and symptoms of OSAS during TRT by polysomnography.

Wearable Sleep Monitoring System Based on Machine Learning Using Snoring Sound Signal

Abstract According to the obstructive sleep apnea Syndrome (OSAS), a wearable sleep monitoring system is designed based on machine learning using snoring sound signal. The system picks up snoring signal via bone conduction sensor, and calculates the apnea-hypopnea index (AHI). By analyzing the snoring signal in frequency domain, spectral entropy and other frequency-domain features are selected. Finally, the neural network classifier model is established. In the model, the input variables are eight frequency-domain features, and the output response is related to AHI value. Trained by machine learning, the result shows that the average accuracy in identifying the severity of the four kinds of OSAS categories is 59%. The system uses the measured data of snoring to analyze the symptoms of OSAS, so as to realize the preliminary forecast based on the snoring data. The system proposed in this paper has a good application development prospect in intelligent monitoring and medical instruments.

Assessment of Leptin Levels and Their Correlation With the Severity of Obstructive Sleep Apnea Syndrome: A Case-Control Study.

Background Obstructive sleep apnea (OSA) is characterized by a combination of structural issues in the upper airway and imbalances in the respiratory control system. While numerous studies have linked OSA with obesity, it remains uncertain whether leptin, a hormone associated with fat, plays a role in the functional and anatomical defects that lead to OSA. Therefore, the aim of this study was to investigate whether leptin levels could be used as a predictor of OSA syndrome (OSAS). Methodology A case-control observational study was conducted, enrolling study participants who reported obesity (BMI > 30) within the range of >30 to <35 kg/m2, along with a short neck and a history of snoring, excessive daytime drowsiness, fatigue, or insomnia. Leptin levels and fasting blood sugar (FBS) were measured in all individuals. Additionally, the study evaluated the severity of OSAS using indicators such as the STOP BANG scores, apnea-hypopnea index, uvula grade score, and Epworth Sleepiness Scale scores. Results A total of 80 participants (40 cases and 40 controls) were included in the study. The mean leptin and FBS levels were significantly higher in cases compared to controls. Moreover, leptin levels exhibited a significant correlation with the severity indices of OSAS. Conclusion The study findings indicate that individuals with higher leptin levels tend to exhibit more severe OSAS symptoms. Furthermore, these elevated leptin levels contribute to the worsening of various OSA symptoms. Larger controlled studies have suggested that pharmacologically restoring the altered leptin levels may serve as a beneficial adjunct to treatment for alleviating OSAS symptoms.

Novel Indices to Improve the Diagnostic Ability of Nocturnal Oximetry in Children with OSAS

Nocturnal pulse oximetry (NOx) is an alternative diagnostic test for obstructive sleep apnea syndrome (OSAS) in childhood yet with variable diagnostic performance. Our aim was to apply advanced signal analysis to develop novel and more accurate NOx indices. We studied 45 children aged 3-10 years who underwent adenotonsillectomy for adenotonsillar hypertrophy and OSAS symptoms. Participants performed NOx before and three months after surgery, and the changes in McGill oximetry score (MOS), oxygen desaturation ≥3% index (ODI3), and the novel parameters-cumulative saturation area (CSA) and oxygen saturation sample entropy (SSE)-were assessed. There was a significant improvement (p < 0.001) in all NOx indices. When pre- and post-adenotonsillectomy NOx recordings were compared, the MOS had an area under the curve (AUC) of 0.811 with 63.2% sensitivity and 100% specificity at a cutoff >1. The AUC of ODI3 was 0.994, with 97.8% sensitivity and 91.1% specificity at a cutoff of >3.6 events per hour. The CSA and SSE had an AUC of 1.00, with 100% sensitivity and specificity at a cutoff of >293 and >0.99, respectively. We conclude that the herein-introduced indices-CSA and SSE-hold promise in improving the diagnostic ability of NOx in children suspected of OSAS.

Adenotonsillectomy: the good, the bad and the unknown.

Adenotonsillar hypertrophy is the most common pathogenetic contributor to obstructive sleep apnea syndrome (OSAS) in childhood, and adenotonsillectomy is the standard initial treatment. Here, we summarize the most recent evidence on the efficacy and complications of adenotonsillectomy and explore knowledge gaps in clinical management. Favorable adenotonsillectomy effects have been reported in children with very severe OSAS [apnea-hypopnea index (AHI) &gt;20 episodes/h] and extremely severe OSAS (AHI &gt;100 episodes/h), without postoperative mortality, need for endotracheal intubation, prolonged hospital stay or re-admission after hospital discharge. However, the risk of residual OSAS after adenotonsillectomy, which may reach 30-60%, has not been thoroughly established. Behavior, OSAS-related symptoms and quality of life improve postoperatively even in children with AHI 1-5 episodes/h. Natural history of enuresis resolution is accelerated postadenotonsillectomy and office-based systemic blood pressure is decreased in OSAS and hypertension. However, which children younger than 2 years should undergo adenotonsillectomy instead of adenoidectomy only to prevent recurrence of OSAS symptoms and revision surgery remains unclear. Adenotonsillectomy in children with Prader-Willi syndrome is frequently accompanied by postoperative residual OSAS while complications are not uncommon. In the last 2 years, several studies have provided evidence supporting the efficacy and safety of adenotonsillectomy as treatment intervention for otherwise healthy children with OSAS.

Correlation between self-reported and questioned Epworth sleepiness scale results including polysomographic outcomes

IntroductionObstructive sleep apnea (OSA) is characterized by a recurrent complete or partial obstruction in the upper airways accompanied by desaturation attacks and recurrent awakenings (arousal) during sleep and is associated with excessive sleepiness and poor quality of life. Excessive daytime sleepiness (EDS) has long been recognized as the most important symptom of OSA and Epworth sleepiness scale (ESS) is a simple, easy-to-understand, self-reported, validated scale used in the evaluation of patients with obstructive sleep apnea syndrome (OSAS).ObjectiveSome researchers have reported that ESS has controversial results in evaluating OSAS due to the fact that it is based on self-reporting. It is known that ESS is a self-administered patient questionnaire, but it can also be administered by the doctor, and the information about the latter’s validity, reliability, and correlation with OSAS is limited. This study was conducted to compare the outcomes of ESS self-administered by the patient and administered by a doctor in patients with OSAS.MethodsNinety-three patients with OSAS symptoms, with polysomnography test results, and who were presented in a tertiary hospital in 2019 for snoring and apnea problems were included in this study. ESS, consisting of eight questions and used to measure daytime sleepiness in various situations on the basis of self-reporting, was used. Subsequently, ESS was repeated, but this time verbally administered by the doctor to the patient. ESS is a 4-point Likert-type scale scored with 0, 1, 2, and 3. Excessive daytime sleepiness is accepted in individuals with a total score of 11 or more. The data were analyzed using the Statistical Package for the Social Sciences (SPSS) program.ResultsOf the patients who participated in this study, 57% (n = 53) were male and 43% (n = 40) were female. The average age of the patients was 48.1±9.71. The body mass index (BMI) average was 29.7±5.66 kg/m2, and the average neck circumference was 38.6±4.62 cm. The mean value of the completed ESS self-administered by the patient (ESSp) was calculated as 11.0±5.25 and that of the ESS administered by the doctor (ESSd) was 7.9±4.76. For the results, while daytime sleepiness was 35.5% in ESSd, it was 52.7% in ESSp. A positive correlation was found between the ESSd and ESSp values and BMI. A positive correlation was also found between the apnea-hypopnea index (AHI) and BMI.ConclusionsThe ESS administered by a doctor will yield more accurate data about the severity of OSAS than the ESS self-administered by the patient.

Orthognathic surgery and obstructive sleep apnea syndrome: a systematic review

Introduction: In the scenario of Obstructive Sleep Apnea Syndrome (OSAS), Orthognathic Surgery (OS) corrected the deformities of the maxillary and mandibular bones OS has evolved a lot in the last two decades. OS treats patients with moderate and severe facial deformities, allowing the achievement of functional balance and harmony in facial aesthetics. Objective: To present the current findings of the importance of orthognathic surgery in the treatment of obstructive sleep apnea syndrome. Methods: A total of 78 articles were found involving mesh terms about Orthognathic Surgery and OSAS. Initially, was held the exclusion of existing title and duplications following the interest described in this work. After this process, the summaries were evaluated, and a new exclusion was held. Thus, 20 articles were included and discussed in this study. Major considerations and conclusion: In recent years, with the involvement and deepening of oral medicine in the diagnosis and treatment of OSAS, the role of OS in OSAS has become increasingly recognized. Early appropriate diagnosis and treatment can significantly improve patients' quality of life, reduce sudden death, and prevent various complications. OS corrects maxillofacial deformities through an incision of the upper and lower jaws, which has a significant relief effect on the symptoms of OSAS in patients with upper airway stenosis, especially in small mandibular patients.

Evaluation of anthropometric measurements and clinical tests in the diagnosis of difficult airway in patients undergoing head and neck surgery.

The aim of this study is to research the incidence of difficult airways and the effectiveness of anthropometric measurements and clinical tests used to predict difficult airways in patients undergoing head and neck surgery. This study was performed on a total of 200 patients over the age of 18 who underwent head and neck surgery between December 2019 and March 2020. The demographic data of the patients in the preoperative period, previous operations/ radiotherapy history applied to the head and neck region, and obstructive sleep apnea syndrome symptoms were recorded. In the physical examination, the jaw structure, mouth opening, jaw movement, and tooth structure of the patients, modified Mallampati classification, head and neck movements, neck circumference, thyromental and sternomental distance, atlantooccipital joint mobility, upper lip bite test, Wilson risk scoring, and Cormack-Lehane classification were evaluated. The difficult laryngoscopy rate was identified as 19%, and the difficult intubation rate as 8%. Operation history related to head and neck (p = 0.002), presence of at least two of the obstructive sleep apnea syndrome symptoms (p = 0.008), Modified Mallampati score (p = 0.009), Wilson risk score (p = 0.004), upper lip bite test (p < 0.0001) and mouth opening (p = 0.001) were found to be associated with difficult laryngoscopy. Modified Mallampati score (p = 0.002), Wilson risk score (p < 0.0001), upper lip bite test (p < 0.0001), mouth opening (p < 0.0001), sternomental distance (p = 0.003), Atlantooccipital joint mobility (p = 0.001), and Cormack-Lehane classification (p < 0.0001) were found to be associated with difficult intubation. According to multiple logistic regression analysis, the results obtained for sternomental distance and mouth opening were OR 0.8, 95% CI 0.6-1.1 and OR 0.2, 95% CI 0.1-0.4, respectively. In patients who underwent head and neck surgery, it was observed that the frequency of difficult airway was higher, and particularly the Modified Mallampati score, Wilson risk score, upper lip bite test, and mouth opening were associated with both difficult laryngoscopy and difficult intubation.

Assessment of sleep-disordered breathing in pediatric otitis media with effusion

Given the role of adenoid in the pathogenesis of otitis media with effusion (OME) and sleep-disordered breathing (SDB), the present study aimed to investigate the sleep quality and the impact of adenoidectomy on improvement of sleep symptoms in children with OME. Children with SDB or OME, aged 3-12 years, were prospectively enrolled before undergoing adenoidectomy in a tertiary medical center. The symptom severity of obstructive sleep apnea syndrome (OSAS) was evaluated by OSA-18 questionnaire on the day before and 6 months after surgery. In total, 79 pediatric patients including 50 children with SDB and 29 children with OME were enrolled. The mean total OSA-18 score was 56.5±15.1 and ten (34.5%) children experienced moderate to severe symptoms of OSAS as indicated by OSA-18 score≥60 in OME group. After surgery, patients in both SDB and OME groups experienced improvement (both p<0.001) and presented no difference in the scores of OSA-18. The proportional change in OSA-18 scores after surgery was related to pre-operative OSA-18 scores and BMI in OME patients (rs=0.400 and 0.542, p=0.047 and 0.008), and to pre-operative OSA-18 scores in patients with SDB (rs=0.393, p=0.008). Children with OME experienced significant symptoms of OSAS and associated impairment of quality of life. Adenoidectomy is effective in the improvement of sleep symptoms in these patients. Comprehensive assessment of sleep breathing as planning surgery for OME children is suggested.

Association of presence and severity of obstructive Sleep Apnoea Syndrome with accident risk in city bus drivers.

To determine the relationship between frequency of traffic accidents and presence and severity of the disease in bus drivers who are at risk of having obstructive sleep apnoea syndrome (OSAS). In the present study, polysomnography (PSG) was applied on 162 city bus drivers directed to the sleep laboratory from a total of 1450 drivers after being determined as risky with regard to OSAS symptoms according to the questionnaire results. Their demographic characteristics, health status and accidents were compiled. Statistical analyses were made for those diagnosed with OSAS according to the PSG result and those with a traffic accident after which comparisons were made. Obstructive sleep syndrome was detected in 127 out of the 162 drivers determined to be risky with regard to OSAS based on the Berlin questionnaire result. While 50 (35%) of the drivers were normal according to the polysomnography (PSG) results in the study, 39 (24.1%) were determined as light OSAS, 35 (21.6%) as moderate OSAS and 53 (32.7%) as severe OSAS. While 105 (64.8%) of the cases had no accident, 37 (22.8%) were almost involved in an accident due to sleepiness and 20 (12.3%) were actually involved in an accident. A statistically significant relationship was determined between accident rates and OSAS severity (p:0.009; p<0.05). It was concluded that presence and increased severity of OSAS is an important risk factor for being almost or directly involved in an accident among city bus drivers, even though they were not driving for long distances.

Efficacy of Rapid Maxillary Expansion with or without Previous Adenotonsillectomy for Pediatric Obstructive Sleep Apnea Syndrome Based on Polysomnographic Data: A Systematic Review and Meta-Analysis

Aim: To systematically review international literature related to rapid maxillary expansion (RME) as the treatment for obstructive sleep apnea syndrome (OSAS) in children less than 18 years-old, followed by a meta-analysis of the apnea-hypopnea index (AHI) before and after RME, with or without a previous adenotonsillectomy (AT). Methods: Literature on databases from PubMed, Wiley online library, Cochrane Clinical Trials Register, Springer link, and Science Direct were analyzed up to March 2020. Two independent reviewers (S.G. and R.J.M.) screened, assessed, and extracted the quality of the publications. A meta-analysis was performed to compare AHI values before and after the treatment with RME. Results: Six studies reported outcomes for 102 children with a narrow maxillary arch suffering from OSAS with a mean age of 6.7 ± 1.3. AHI improved from a M ± SD of 7.5 ± 3.2/h to 2.5 ± 2.6/h. A higher AHI change in patients with no tonsils (83.4%) and small tonsils (97.7%) was detected when compared to children with large tonsils (56.4%). Data was analyzed based on a follow-up duration of ≤3 year in 79 children and &gt;3 years in 23 children. Conclusion: Reduction in the AHI was detected in all 102 children with OSAS that underwent RME treatment, with or without an adenotonsillectomy. Additionally, a larger reduction in the AHI was observed in children with small tonsils or no tonsils. A general improvement on the daytime and nighttime symptoms of OSAS after RME therapy was noted in all the studies, demonstrating the efficacy of this therapy.

Clinical profile of patients with moderate to severe obstructive sleep apnea syndrome in a centre from South Kerala

Obstructive sleep apnea syndrome (OSAS), a common but under-recognized sleep-related breathing disorder (SRBD) which is associated with significant morbidity and increased mortality. The aim of this study done in South Kerala was to assess the clinical profile and comorbidities of patients with moderate to severe form of OSASPatients included in the study were from a speciality sleep clinic and had symptoms of OSAS or risk factors for significant sleep apnea. They underwent a supervised polysomnography (PSG) with a portable equipment at home after history taking, relevant clinical evaluation, and pre-requisite workup.One hundred and forty patients were included in the study and 87.9% (123) of the patients were detected to have moderate or severe OSAS (Apnea Hypopnea Index AHI&amp;#62;15 episodes/hour), 81 % (100) of these patients with significant ( moderate or severe) sleep apnea were males having a mean age of 47.9 years, mean body mass index of 33.6 kg/mand an average neck circumference of 41.2 cm. 80% of patients with moderate to severe OSAS were between the ages of 31-60 years.61.7% and 41% of patients with significant OSAS had hypertension and diabetes respectively. Body mass index and neck circumference had a direct correlation with the severity of OSAS.The data was analysed using SPSS 20.0. The factors and co-morbidities associated with OSAS were analysed using Chi-square tests and Independent Sample t-tests. Pearson's correlation coefficient and a linear regression model was made to find out the possible predictors for AHI Scores and a p value of less than 0.05 was taken as significant.Our study on the clinical profile of patients with obstructive sleep apnea syndrome from a centre in South Kerala shows male predilection, frequent association with obesity and cardiovascular co-morbidities such as hypertension and diabetes.

Sleep disordered breathing in children with Down syndrome in the Republic of Ireland.

Down syndrome (DS) is associated with a complex respiratory phenotype, including obstructive sleep apnea syndrome (OSAS). The study explored parent-reported prevalence and presentation of OSAS symptoms in children and adolescents with DS in Ireland. It also investigated treatment and compliance in those who have OSAS. A cross-sectional survey was distributed to parents registered with Down Syndrome Ireland (DSI) (n = 1,343). Data was collated and analyzed using SPSS v23. The response rate was 393 (29%). Twenty-one percent of parents (n = 84) reported a diagnosis of OSAS in their child. The parents of children reported as compliant with continuous positive airway pressure (CPAP) were more likely to report a perceived benefit of treatment (p = .018). Ninety-two percent (n = 212) of children without a formal diagnosis of OSAS had at least one symptom (median 4 symptoms) of the condition. This, the largest survey of parental reporting of OSAS or its symptoms, demonstrates a high reported prevalence of symptoms in children and adolescents with DS in Ireland without a formal diagnosis of OSAS, indicating under-recognition. In treated patients, perception of the benefit of CPAP correlated with reported compliance, suggesting a need for tailored education.

Self-reported snoring is associated with nonalcoholic fatty liver disease

Although nonalcoholic fatty liver disease (NAFLD) is associated with obstructive sleep apnea syndrome (OSAS), studies on the direct relationship between NAFLD and snoring, an early symptom of OSAS, are limited. We evaluated whether snorers had higher risk of developing NAFLD. The study was performed using data of the Tongmei study (cross-sectional survey, 2,153 adults) and Kailuan study (ongoing prospective cohort, 19,587 adults). In both studies, NAFLD was diagnosed using ultrasound; snoring frequency was determined at baseline and classified as none, occasional (1 or 2 times/week), or habitual (≥3 times/week). Odds ratios (ORs) and hazard ratios (HRs) with 95% confidence intervals were estimated using logistic and Cox models, respectively. During 10 years’ follow-up in Kailuan, 4,576 individuals with new-onset NAFLD were identified at least twice. After adjusting confounders including physical activity, perceived salt intake, body mass index (BMI), and metabolic syndrome (MetS), multivariate-adjusted ORs and HRs for NAFLD comparing habitual snorers to non-snorers were 1.72 (1.25–2.37) and 1.29 (1.16–1.43), respectively. These associations were greater among lean participants (BMI < 24) and similar across other subgroups (sex, age, MetS, hypertension). Snoring was independently and positively associated with higher prevalence and incidence of NAFLD, indicating that habitual snoring is a useful predictor of NAFLD, particularly in lean individuals.

Evaluation of the structural organization of sleep in obstructive apnea syndrome from the perspective of modern pathophysiology

To evaluate the features of structural organization of sleep in men 35-55 years of age with varying degrees of severity and duration of clinical manifestations of obstructive sleep apnea syndrome (OSAS) from the perspective of modern pathophysiology. The study included 49 male patients, aged 35-55 years (average age was 43.4±9.2 years), with a body mass index (BMI) of 33.64±1.2 kg/m2 with complaints of snoring of varying degrees of intensity, daytime sleepiness, arrest of breathing during sleep (according to others). The control group consisted of 15 healthy men who did not have complaints of sleep disorders and clinical manifestations of OSAS. The Pittsburgh sleep quality index (PSQI) questionnaire and polysomnographic monitoring study (PSG) were used. According to the results of the polysomnographic study, all patients were divided into 2 subgroups: 12 men of group 1with moderate OSAS (duration of clinical manifestations 4.5±1.0 years) and 37 men of group 2 with severe OSAS (duration 1.5±1.5 years). A decrease in the sleep quality was detected in 61.9% of patients in the group 1 and 89.2% of patients in the group 2. Significantly higher scores for the components of «sleep disturbance» and «daytime dysfunction» in patients with a longer duration of clinical manifestations were detected. Changes in the structure of sleep were identified in patients compared with controls. A statistically significant increase in WASO led to the reduction of sleep efficiency. In patients of group 2, a deficiency of the deep stages and REM-sleep was determined. There was a significant increase in the number of arousals in patients of both groups relative to the controls. Moreover, significant between-group differences were found. The short duration of OSAS symptoms causes a restructuring of the physiological system of sleep homeostasis to a different level of functioning with compensatory hyperfunction of the structures responsible for REM sleep and a decrease (economization) in the activity of anabolic processes during sleep due to a decrease in SWS, which is a characteristic feature of long-term adaptation. Severe course of OSAS leads to the appearance of specific phenomena of gross structural disorganization of sleep - «alpha-delta sleep» and disruption of its segmental organization, which shows the depletion and disruption of compensatory-adaptive mechanisms in the system of sleep homeostasis, disintegration and transformation of functional physiological system in the pathological condition.

Determination of anthropometric measurements in obstructive sleep apnea syndrome in Turkish population

In this study, we aimed to determine the values of anthropometric measurements and rates used in the evaluation of obstructive sleep apnea syndrome (OSAS) in our country. Twenty accredited sleep centers in thirteen provinces participated in this multicenter prospective study. OSAS symptoms and polysomnographic examination and apnea-hypopnea index (AHI) ≥ 5 cases OSAS study group; patients with AHI < 5 and STOP-Bang < 2 were included as control group. Demographic characteristics (age, sex, body mass index-BMI) and anthropometric measurements (neck, waist and hip circumference, waist/hip ratio) of the subjects were recorded. The study included 2684 patients (81.3% OSAS) with a mean age of 50.50 ± 0.21 years from 20 centers. The cases were taken from six geographical regions of the country (Mediterranean, Eastern Anatolia, Aegean, Central Anatolia, Black Sea and Marmara Region). Demographic characteristics and anthropometric measurements; age, neck, waist, hip circumference and waist/ hip ratios and BMI characteristics when compared with the control group; when compared according to regions, age, neck, waist, hip circumference and waist/hip ratios were found to be statistically different (p< 0.001, p< 0.001, p< 0.05, respectively). When compared by sex, age, neck and hip circumference, waist/hip ratio, height, weight and BMI characteristics were statistically different (p< 0.001, respectively). Neck circumference and waist/hip ratio were respectively 42.58 ± 0.10 cm, 0.99 ± 0.002, 39.24 ± 0.16 cm, 0.93 ± 0.004 were found in women. The neck circumference was lower than the standard value in men, but higher in women. The waist/hip ratio was above the ideal measurements in both men and women. In this context, the determination of the country values will allow the identification of patients with the possibility of OSAS and referral to sleep centers for polysomnography.

Sleep-related breathing disorders in children and teenagers: diagnosis, consequences and comorbidities

Obstructive sleep apnoea syndrome (OSAS) is a frequent sleep-related breathing disorder in children with a prevalence of approximately 3% between the ages of 3 and 8. The symptoms and clinical signs to look for during the day and at night are directly related to upper airway (UA) obstruction or poor sleep quality. After a complete anamnestic and clinical evaluation, including nasofibroscopy, the ENT specialist will refer to an adeno-amygdalectomy or night recording by polysomnography or polygraph to confirm the diagnosis in the child. Among adolescents, the prevalence of OSAS is reported to be between 0.5 and 6%. The main risk factors are obesity, male sex and a history of tonsillectomy. In addition to the classic symptoms of childhood OSAS, this syndrome may, in adolescents, manifest itself as a misleading semiology of dyssomnia, excessive daytime sleepiness and/or mood disorders. Differential diagnoses with risk behaviours, phase delay or narcolepsy should be systematically discussed. It is essential to record breathing during sleep. Even if the obstructive apnea/hypopnea index is low, it must be considered. In both children and adolescents, multidisciplinary management (ENT, orthodontist, maxillofacial physiotherapist, pulmonologist) should be early in order to avoid neurocognitive, behavioural, cardiovascular and metabolic complications. Maxillofacial surgery can be discussed from the age of 15. It is important not to forget to pay attention to the rules of healthy living and sleep as well as the management of obesity.

The Association of Obstructive Sleep Apnea Syndrome and Accident Risk in Heavy Equipment Operators.

Background and Objectives: Obstructive sleep apnea syndrome (OSAS) is the most frequent sleep disorder, characterized by the repeated collapse of the upper respiratory tract during sleep. In this study, we aimed to determine the prevalence of OSAS in heavy equipment operators and to determine the relationship between the work accidents that these operators were involved in and the OSAS symptoms and severity. In doing this, we aimed to emphasize the association of OSAS, which is a treatable disease, and these accidents, which cause loss of manpower, financial hampering, and even death. Materials and Methods: STOP BANG questionnaire was provided to 965 heavy equipment operators and polysomnography (PSG) was performed, in Izmir Esrefpasa Municipality Hospital, to the operators at high risk for OSAS. Demographic data, health status, and accidents of these operators were recorded. Results: All operators who participated in the study were male. The ages of the cases ranged from 35 to 58 and the mean age was 45.07 ± 5.54 years. The mean STOP BANG questionnaire results were 4.36 ± 3.82. In total, 142 operators were identified with high risk for OSAS and PSG could be performed on 110 of these 142 operators. According to the PSG results of the operators, 41 (37.3%) patients had normal findings, while 35 (31.8%) had mild, 20 (18.2%) had moderate, and 14 (12.7%) had severe OSAS. Among those 110 patients, 71 (64.5%) of the cases had no history of any accidents, 25 (22.8%) were almost involved in an accident due to sleepiness, and 14 (12.7%) were actually involved in an accident. There was a statistically significant relationship between the accident rate and OSAS severity (p: 0.009). Conclusion: Based on the data acquired in the present study, a positive correlation was determined between the accident statuses of drivers with OSAS severity. We want to attract attention to the necessity of evaluating the OSAS symptoms in professional heavy equipment operators during the certification period and at various intervals afterwards, and to carry out OSAS evaluations by PSG for those having a certain risk.

Prevalence of Obstructive Sleep Apnea Syndrome in Psoriasis Patients

Aim: Psoriasis is a frequently seen chronic systemic inflammatory disease accompanied by several comorbid conditions that affects 2-3% of the general population. One of the comorbidities rarely accompanying psoriasis is obstructive sleep apnea syndrome (OSAS). OSAS is a disease characterized by recurrent complete (apnea) or partial (hypopnea) upper airway obstruction episodes and frequently by decreased blood oxygen saturation. The purpose of this study was to evaluate the risk factors for OSAS by investigating OSAS symptoms in psoriasis patients.Material and Methods: Eighty-two patients aged over 16, diagnosed with psoriasis, and under follow-up by the Duzce University Medical of Faculty, Dermatology and Venereal Diseases Polyclinic, Turkey, were included in the study. Patients’ OSAS symptoms were investigated. The Epworth Sleepiness Scale was used to assess excessive daytime somnolence.Results: Of the patients 51.2% (n=42) were male and 48.8% (n=40) female. Patients’ mean age was 47.09±14.10 years. Patients’ mean time of psoriasis diagnosis was 16.97±10.55 years, and mean Psoriatic Area Severity Index (PASI) score was 11.25±6.32. Severe disease was present in 54.9% of patients (n=45) based on PASI classification. Prevalence of 56.1% for snoring, 25.6% for excessive daytime sleepiness, and 15.9% for witnessed apnea were determined in these patients. Forty-seven (57.3%) cases had at least one major symptom. The most common minor symptoms were inability to sleep with 25.6% (n=21) and insufficient disrupted sleep with 22.0% (n=18).Conclusion: Prevalence of OSAS symptoms in psoriasis patients were found high. Psoriasis patients with OSAS symptoms must be referred to relevant specialists for polysomnographic evaluation.
