Swallowing Disorders in Patients with Chiari Malformation Type I After Decompression Neurosurgery

The present study aimed to investigate postoperative swallowing disorders in patients diagnosed with CM I after decompression neurosurgery. We selected 54 patients after decompression neurosurgery procedure for CM I from January 2017 to December 2019. We evaluated the speech-language assessment (CSLSA) 24 h after post-surgical extubation. CSLSA observed aspects of posture, sensitivity, tone, and extraoral and intraoral mobility of the FAO and visualizing the functions of breathing, phonation, articulation, chewing, and swallowing. We correlated the severity of the dysphagia and the presence of associated malformations in the CM I patients. In our study, 39 patients were female (mean age 41.3 ± 47.1 years). Thirty-seven patients had associated malformations (p = 0.0243): 21 patients syringomyelia, 11 basilar invagination, and 5 syringomyelia and basilar invagination. Dysphagia was present in 37 of the cases (68.5%; p = 0.0016). Dysphonia in 13 (24.1%; p = 0.4785) and dysarthria in 3 (5.6%; p = 0.0042). We observed in the CSLSA: orofacial hypomobility (29 patients — 53.7%; p = 0.0204), orofacial hyposensitivity (26 patients — 48.1%; p = 0.3023), reduced GAG reflex (24 patients — 44.5%; p = 0.1041), ineffective cough (19 patients — 35.2%; p = 0.0022), ineffective saliva swallowing (18 patients — 33.3%; p = 0.0396), dyspnea (13 patients — 24.1%; p = 0.4785), orofacial hypotonia (11 patients — 20.4%; p = 0.0004), altered cervical auscultation (10 patients — 18.5%; p = 0.0003), and dysarthria (3 patients — 9.3%; p = 0.0042). The presence of associated malformations significantly influenced the appearance of swallowing disorders, the severity of oropharyngeal dysphagia, the post-surgical complications (17 patients — 31.5%; p < 0.001), and the clinical outcomes (p = 0.0029) of patients with CM type I. Decompressive neurosurgery for CM I had low rates of post-surgical complications overall. However, there was a high incidence of dysphagia. The presence of associated malformations is related with poorer prognosis and greater severity of oropharyngeal dysphagia in patients with CM I.

Rehabilitation of tongue cancer patients with dysphagia path action research

Objective To evaluate the action research method in the effect of swallowing disorder in patients with tongue cancer recovery path. Methods Based on the recovery path construction, according to the questions, plan, action, observation and reflection, improvement of summarizing the research process, through two stages of the research, assessment, diagnosis, planning, implementation, evaluation and comparison of stage 1 and stage 2 swallowing disorder in patients with rehabilitation evaluation, quality of life score, spirit to adapt to the score. Results Nearly 79.69% (51/64) of first phase swallowing rehabilitation effectively, and 93.75% (60/64) effectively in the second stage. Compared to the first stage,the second stage had an obvious increase. Two stages at the university of Washington Quality of Life Score, the first phase of (770.400 ±87.299) points, (1117.100 ± 43.153) points in the second stage, two stages of life quality score comparison, the difference was statistically significant (t=-19.500, P= 0.012). The comparison of two stage patients mental adjustment scale scores, the first phase of (15.933±1.285) points, (31.733±2.083) points in the second stage, two stages score spirit to adapt to the comparison, the difference was statistically significant (t=-35.357, P=0.003). Conclusions Tongue cancer patients with swallowing disorder treatment on the basis of action study method to build and implement path specification, can improve the quality of care and quality of life of patients. A videofluoroscopic study comparing severe swallowing disorders in patients treated surgically or with radiation for oropharyngeal cancer

The aim of this study was to analyze the causal mechanisms of severe swallowing disorders after the treatment of oropharyngeal cancer. Twenty-six patients with severe swallowing disorders at ≥12 months after treatment for oropharyngeal cancer were analyzed retrospectively using videofluoroscopy. Fourteen patients (54%) had been treated with surgery (±postoperative radiotherapy), while 12 patients (46%) had been treated with (chemo)radiotherapy. Videofluoroscopy analysis showed a localized alteration in the surgical excision area resulting in impaired tongue root retraction in the surgical group (P=0.012), while general impairment of the pharyngeal, laryngeal, and upper oesophagus sphincter was found in the non-surgical group. Aspirations in the surgical group most often occurred after swallowing, while in the non-surgical group, they occurred during and after swallowing (P=0.039). This analysis by videofluoroscopy provides important insights into the mechanisms giving rise to swallowing disorders after the treatment of oropharyngeal cancer.

Is drooling secondary to a swallowing disorder in patients with Parkinson's disease?

Drooling is a common manifestation in Parkinson's disease (PD). It causes psychosocial difficulties and can result in aspiration and chest infection. Previous studies point to an association between swallowing problems and sialorrhea. The aim of this study was to determine if drooling is associated with dysphagia in PD patients. Sixteen PD patients with diurnal drooling were assessed using a modified barium swallowing with videofluoroscopy, and a drooling score. Changes in the oral stage of swallowing were seen in 100% of the patients; and in the pharyngeal stage, in 94% of the patients. The results showed a correlation between the drooling scale score and the level of dysphagia (−0.426; p<0.05). Patients with the worst dysphagia had the worst drooling.
