Clinicopathologic and prognostic factors for colorectal cancer in children and adolescents: a population-based study.

Colorectal cancer (CRC) is rarely found in children and adolescents. The purpose of this study was to retrospectively conduct a population-based cohort of pediatric patients with CRC. All pediatric patients with CRC diagnosed between 1975 and 2018 were identified using the Surveillance, Epidemiology, and End Results (SEER) database. The demographics and clinical variables of the patients were summarized, and treatment outcomes and prognostic factors were examined. The study was presented in accordance with the STROBE reporting checklist. A total of 284 CRC patients were identified. At 3- and 5-year follow-up, the overall survival rates were 63.1% and 52.6%, respectively. Patients with local disease had a significantly improved overall survival (OS) than patients with distant disease. At 3- and 5-year follow-up, the overall survival rates of adenocarcinoma (nos) and adenocarcinoma (polyp) were similar and significantly better than those of patients with mucinous adenocarcinoma and signet ring cell carcinoma (P < 0.001). In terms of treatment, patients who underwent surgery outlived non-surgery patients (3-year OS, 70.4% versus 26.6%, P < 0.001). Multivariate analysis revealed that SEER stage and histologic type were important independent predictors of outcomes. Children and adolescents with CRC are likely to be in an advanced stage, have a worse histologic subtype, and have a poorly differentiated grade. Although surgical resection considerably increases survival for the majority of patients, pediatric patients with CRC have a poor prognosis. Considerable efforts are required to improve their survival outcomes.

Definitive chemoradiotherapy +/- induction chemotherapy in esophageal cancer: A real-world experience.

e16072 Background: The SEER 5-year overall survival rate for all stages of esophageal cancer (EC) was 25% in 2019. Definitive chemoradiation (CRT) remains the primary treatment approach for locally advanced EC in the US, however, there are data to support use of induction chemotherapy (CT) in addition to CRT, particularly in adenocarcinoma (AC) histology. The purpose of our study was to assess outcomes in EC patients treated with definitive CRT (+/- induction) to determine which prognostic factors predicted for better survival in a recent, real-world cohort of patients, including those with limited stage IV disease. Methods: This retrospective study included Stages II-IVB (AJCC 8th ed.) EC patients treated with definitive CRT (radiation dose of ≥40 Gy and at least two cycles of concurrent CT [+/- induction CT, +/- esophagectomy]) at our institution between 2008 and 2020. To analyze prognostic factors and estimate OS, univariate models (UVA) and a multivariate (MVA) Cox proportional hazards regression model including age, Stage (II, III, IVA, IVB), AC vs. SCC, esophagectomy, ECOG performance status (PS), and induction CT were performed. Results: Of the 183 patients treated with definitive CRT, 18 were stage II, 119 stage III, 21 stage IVA, and 25 stage IVB. There were 45 SCC and 138 AC patients (Table). Prognostic factors associated with prolonged OS on MVA included lower PS (p&lt;0.01) and esophagectomy (p = 0.05). Stage IVA was associated with shorter survival (p&lt;0.01). Induction CT (delivered in 53% of AC, 31% SCC) was associated with improved survival on UVA (p=0.04), but not MVA (p = 0.08). By histology, 5-year survival rate was 42.7% and 18.2% for AC and SCC, respectively. Conclusions: At our institution, those who received an esophagectomy and those with lower ECOG scores had better survival. The 5-year survival rate was higher for AC patients compared to SCC, with more AC patients receiving esophagectomy and induction CT (significant on UVA, but not MVA). The 5-year survival rate for AC in our study was nearly identical to that seen in the CROSS trial (Shapiro et al. 2015) AC cohort but included &gt;25% stage IV patients. This modern cohort also included poor PS (ECOG≥2) patients (9.2% AC and 13.6% SCC), suggesting induction CT, in addition to pre-operative CRT + surgery, may have added benefit in a real-world practice. Clinical features and prognosis of cervical adenocarcinoma and adenosquamous carcinoma: an analysis of 237 cases

To analyze the clinical features and prognosis of cervical adenocarcinoma (AC) and adenosquamous carcinoma of cervix (ASC). The clinical data of 237 patients, including 201 cases of AC and 36 cases of ASC (FIGO stage ⅠB1-ⅡA), who underwent surgery in Qilu Hospital between September 2007 and September 2016 were reviewed. Clinical features of two groups were compared, and Kaplan-Meier survival analysis was performed to evaluate the prognosis. A larger proportion of ASC patients had lymphovascular space invasion compared with AC patients (P<0.01), but no significant differences were observed in the age, FIGO stage, size of tumor, depth of stromal invasion, parametrial invasion, lymphatic metastasis and risk grade between two groups (all P>0.05). The 5-year overall survival rates of AC and ASC groups were 79.4% and 78.3%, and the 5-year recurrence-free survival rates were 77.4% and 73.0%. Among patients received concurrent chemoradiotherapy, the 5-year overall survival rates were 71.0% and 61.4%, and the 5-year recurrence-free survival rates were 68.8% abd 61.1%, respectively. No significant differences were observed in 5-year overall survival rates and recurrence-free survival rates between AC and ASC patients (all P>0.05). Lymphovascular space invasion was more likely to occur in patients with ASC, but there was no significant difference in the prognosis between AC and ASC patients.

Pathologic-Histology and Prognosis of Cancer of the Uterine Cervix

In this series of Uterine Cervical Cancers 1955-1958, the five-year survival rate for adenocarcinotna was lower than that for epidermoid carcinoma. The survival rate was about equal in both when the lymph nodes were free of tumour cells whereas in the presence of lymph node metastasis the survival rate for adenocarcinoma was much lower. The incidence of lymph node metastases was also higher with adenocarcinoma. From a review of the literature and our personal experience the authors feel that the present forms of treatment are ineffective for adenocarcinoma of the cervix and propose that future treatment should be: Surgery followed by chemotherapy for adenocarcinoma and chemotherapy and/or irradiation followed by surgery for small cell carcinoma.

The significance of radiotherapy for adenocarcinoma of uterine cervix

5590 Background: We conducted the present study to determine the significance of radiotherapy for cervical adenocarcinoma. Methods: A total of 1,133 patients with cervical cancer, who underwent radiotherapy in 7 institutes between 1996 and 2005, were enrolled in this study. There were 966 squamous cell carcinoma (SCC) and 167 adenocarcinoma (AC). The mean age of patients with AC was 53.6 (range: 28–81) and 55.9 (range: 22–92) for SCC. Patients were distributed as follows: 370 in stage 1b, 116 in stage IIa, 344 in stage IIb, 16 in stage IIIa, 240 in stage IIIb, and 47 in stage IVa. All patients were identified with the medical records. Of 1,133 patients, 544 received radiotherapy as primary treatment, and 589 underwent adjuvant radiotherapy. Results: Regarding primary treatment, the 5-year survival rate for AC was 41.8% and that for SCC was 62.5% (p=0.0172). The rate of refractory or recurrence for patients with AC was significantly higher than that for SCC (53.3% vs. 33.3%, p=0.0068). Patients with AC more commonly showed recurrence inside irradiated field compared with SCC (83.3% vs. 58.2%, p=0.0240). The outcome for patients with AC was significantly worse compared with SCC in adjuvant treatment (5-year survival rate: 27.5% vs. 45.4%, p=0.0434). Recurrence for AC were significantly higher observed than that for SCC (40.0% vs. 24.7%, p=0.0008). The incidence of recurrence inside irradiated field and lymph node involvement did not differ between AC and SCC (30.1% vs. 43.8%, 46.7% vs. 44.0%). Conclusions: Cervical adenocarcinoma had poorer prognosis because of its low radiosensitivity compared with SCC. New strategy for cervical adenocarcinoma should be necessary. Small bowel cancer: A 30-year review

Small bowel cancer is a relatively rare tumor with an incidence of 2,700 new cases and 900 deaths per year. The influence of stage on survival has been reported only once previously. Patterns of recurrence are unreported. All cases of small bowel cancer treated at our hospital over a 30-year period (1960-1989) were reviewed. The site of most cancers was the duodenum (46%), followed in frequency by the jejunum (33%) and the ileum (21%). Adenocarcinoma was the most common histology (63%), followed in frequency by lymphoma (15%), leiomyosarcoma (13%), carcinoid tumors (6%), and miscellaneous (3%). Analysis of stage distribution by site showed a decrease in stages I and II with more distal locations. Associated cancers occurred in 11%, but none were seen in the group with carcinoid tumors. Actuarial 10-year survival rates were 24% for those with adenocarcinoma (all stages) 75% for stage I, 25% for those with stage II, and 0% for stage III. A subgroup of 10 patients who underwent a pancreaticoduodenectomy (one stage I, seven stage II, two stage III) had a 30% 10-year survival rate. Those patients with lymphoma had a 12% 10-year survival rate, and those with leiomyosarcoma had a 20% 10-year survival rate. A 100% 10-year survival rate was observed in those with carcinoid tumors. Peritoneal carcinomatosis was the most common failure pattern (33%), followed in frequency by local recurrence in 23% and abdominal wall recurrence in 15%. A correlation exists between the pathologic stage and the survival rate for adenocarcinoma. The most common recurrence pattern for adenocarcinoma was carcinomatosis, followed in frequency by abdominal wall recurrence. Leiomyosarcoma preferentially metastasizes to the liver.
