Modern Methods of Surgical Treatment of Gastric Ulcer and Duodenal Ulcer

More than a century has passed since the production of the first perforating ulcer suturing operation in world practice. During this period, many researchers have put a lot of work into finding ways to reduce mortality and postoperative complications in perforated ulcers of the stomach and duodenum.

<p>A morphometric study of operated stomach walls after different methods of surgical treatment of gastric ulcer was performed. It was shown that in the remote period operated stomach wall structure had a tendency to regenerate. The most optimal changes were detected after organsparing method of surgical treatment – selective proximal vagotomy with sighting resection of ischemic segment of the stomach by L. Ya. Kovalchuk.</p>

Surgical treatment of perforated gastric ulcer

Peptic ulcer perforation is a complication of ulcer disease which requires urgent surgical treatment. The aim of this paper was to point out our experience in surgical treatment of perforated peptic ulcer. This retrospective study analyzes results of surgical treatment in 365 patients with perforated peptic ulcer during the period January 1996 to December 2000. During the last 5-year period 365 patients were treated following peptic ulcer perforation. The average age was 43.53 +/- 8.26, with the span from 18 to 86. The most frequent surgical procedures in treatment of peptic ulcer perforation were: simple closure with biopsy (55.88%), excision of the ulcer with a pyloroplasty and vagotomy (35.29%) as nonresection surgical procedures and stomach resection after Billroth II (8.83%). The postoperative mortality was 4.41%. The methods of choice in surgical treatment of gastric ulcer perforation are nonresection surgical procedures with drug therapy and eradication of Helicobacter pylori, if present.

Effect of cimetidine on healing and surgical treatment of gastric ulcers

In this retrospective study, a cohort of gastric ulcer patients treated with antacids was compared with a well-matched group of patients treated with a combination of antacids and cimetidine. Cimetidine combined with antacids enhanced gastric ulcer healing, thereby significantly reducing the number of gastric ulcer operations. Cimetidine appears to act by providing a less acidic environment in which gastric ulcers can heal. The exact mechanism of cimetidine's action on gastric ulcers is not clear. Nonprophylactic administration of cimetidine apparently has no effect on the recurrence rate of gastric ulcers.

Selective mucosectomy is proposed, in which the mucous membrane of the lesser curvature of the stomach is removed, followed by sutures covering the demucosated surface. A functional study of the stomach revealed a stable reduction of the secretory potential, as well as the normalization of its motor-evacuation function, which contributes to the healing of the ulcer. Due to its low trauma, the operation can be used in surgical practice, especially in cases where organ resection or intervention on the nerve plexus is undesirable.

Report of a contemporary series of 142 patients with gastric ulcer, treated largely by resection, reflecting the renewed interest in nutritional results of such procedures and conservative approaches to management. Since the incidence of recurrent ulcer after vagotomy and drainage has not been established, the authors prefer to reserve this operation for patients in whom resection is likely to be followed by serious nutritional problems.

Ulcerating lesions of the stomach are of frequent occurrence, and the possibility of their being malignant can never be ruled out except by microscopic examination. They are not infrequently associated with serious complications. Medical treatment of such ulcers has proved ineffective in a high percentage of cases. Gastric resection, on the other hand, can be done with a low mortality, and end-results with few exceptions are most gratifying. The purpose of this presentation is to submit evidence to show that these statements are true and to urge that all ulcerating lesions of the stomach be considered a surgical problem from the moment of diagnosis. Although gastric ulcer and duodenal ulcer produce similar symptoms, the clinical implications differ radically. Primary treatment of duodenal ulcer is nonsurgical, operation being reserved for intractability (of ulcer or patient), obstruction, or hemorrhage. Gastric ulcer should be treated by gastric resection unless it is benign by all available criteria, and patient and physician will carry out periodic check-ups.

RESULTS OF SURGICAL TREATMENT FOR GASTRIC ULCER

Gastric ulcer presents a serious problem in its treatment because of possible malignant degenerative changes arising in a benign ulcer and especially because of the considerable percentage of diagnostic errors made in distinguishing between benign and malignant ulceration of the stomach. We believe it will be of value to discuss these serious aspects of the problem and to present the results of experiences at the Lahey Clinic with the surgical treatment of gastric ulcer over a ten year period (1936 to 1945 inclusive). From this study an attempt will be made to draw some conclusions regarding the management of peptic ulcer arising in the stomach. This study is in order because in the past we have advised a more or less conservative approach; that is, employment of a medical regimen rather than a surgical procedure in the treatment of the majority of patients jwith gastric ulcer. Results of surgical treatment of gastric and duodenal ulcers

The question of indications for surgical treatment of gastric ulcers and duodenal ulcers cannot be considered fully resolved until now, as well as the question of the nature of the surgical intervention itself. The readings are divided into absolute and relative. The first include: a) perforation of the ulcer into the free abdominal cavity, b) its malignant degeneration, c) cicatricial narrowing of the pylorus. Chronic gastric and duodenal ulcers are considered relative indications. There is no disagreement about absolute indications among surgeons and therapists, which is not the case for relative indications.
