Hand hygiene competence in the intensive care unit

The hand hygiene knowledge and competence of staff in an adult intensive care unit (ICU) was measured at a 450 bed Australian metropolitan teaching hospital. Staff working in or entering the ICU to perform patient care over a two-month period were asked to complete a questionnaire that examined their knowledge of current hand hygiene policy and procedures. Hand hygiene practice was also assessed by observation. Sixty-six staff participated, including 50 registered nurses (RNs), eight medical staff (MOs), five surgical dressers (SDs) and three radiographers (X-Ray technicians). Radiographers were excluded from the statistical analysis due to low numbers participating. Scores on the knowledge component of the questionnaire ranged from 5-10/10 (mean 7.39 ± 1.04 std dev). Scores on the practical component ranged from 3-6/6 (mean 5.83 ± 0.56). Mean knowledge scores by profession were: 7.75 (MOs), 7.38 (RNs), and 6.60 (SDs). Mean competence scores by profession were: 5.75 (MOs), 5.88 (RNs), and 5.80 (SDs). There were no significant differences in hand hygiene knowledge (p=0.178) and hand hygiene competence between professions (p=0.780). The greatest knowledge deficit lay in what constituted a routine handwash and in which situations routine handwashing was required.

Clinical skills in final year medical students

Like many older readers (I qualified in 1976), I have been somewhat bemused by Goodfellow and Claydon's report (October 2001 JRSM1), and the subsequent correspondence. First of all, we should acknowledge that the students of today are at least as able as their fore- (and in many cases, real) fathers. The ‘failings’ in the eight core clinical skills (venepuncture, venous canulation, rectal examination, nasogastric intubation, suturing, urinary catheterization, ECT recording and arterial blood sampling) are significant because these, along with lumbar puncture, inserting chest drains and using an ophthalmoscope, are what our ‘duties’ were as medical clerks and surgical dressers admitting patients. Our generation valued competency as an aim in itself, a view shared by our bosses in sharp contrast to today's views on teaching. Is what is needed prolonged clinical attachments to anaesthetic rooms as Mr Sado suggests2 or a ‘skills laboratory’? Surely better to allow a return to clinical apprenticeships where students do things, not just observe. Stress is part of medical life, and if trainees find keeping a logbook a bit much, then they should remember that the end-product of their training is to treat fellow humans safely and competently. As final year students at St George's in the 1970s, we had a month each as ‘shadow’ house officers in both medicine and surgery. It meant what it said, and we did everything except writing up drugs. On qualifying we were able to start our house-jobs knowing we could cope—and so did our consultants. In the district general hospitals the students we see are just as keen as we were, and many are all too aware of the practical deficiencies of their training: that's why they come to us; that's why they head for the ‘third world’ for their electives, so they can learn to be ‘useful’.

Surgical dressers: the theatre experience of junior clinical students.

The teaching of junior surgical dressers in theatre and on the wards was audited in a survey organized by students over a 3-year period. The average dresser in Birmingham, England spent 5 1/2 hours per week in theatre and was scrubbed for more than half of that time. Most students found their time in theatre both instructive and enjoyable. No major differences in teaching practices were found between teaching and peripheral hospitals.

Clinical experience of clerks and dressers: a three-year study of Birmingham medical students.

A survey of the clinical experience of junior medical clerks and surgical dressers on their first clinical attachment in the Birmingham University Teaching Hospitals was organized by students over a three-year period. A typical clinical student had performed basic clinical procedures such as putting up a drip, doing an ECG, bladder catheterization and seen a wide range of other procedures. Most students felt involved in the work of the firm, had clerked routine admissions, and regularly attended the firm's emergency admission night. Consultants on most firms were felt to be concerned about student progress. Students attached to district general hospitals gained a wider range of clinical experience than those at established teaching hospitals. Student-run audit is a useful way of monitoring the clinical experience gained by medical students.
