The effect of coffee on contralateral suppression of transient evoked otoacoustic emissions.

Background: Coffee is a popular non-alcoholic beverage consumed by humans across the world. It contains caffeine, which is a type of stimulant of the central nervous system. In the auditory system, it has a positive effect on auditory brainstem response and perception of speech in noise. Further, caffeine has an inhibitory effect in the cochlea, but studies have rarely investigated its effect on otoacoustic emissions (OAEs) in humans. OAEs are low-intensity sounds produced by the cochlea, which could be recorded in the ear canal. The present study was carried out to investigate the effect of coffee on transient evoked otoacoustic emission (TEOAE) and contralateral suppression of TEOAE. Method: A total of 52 young adults participated in the study. A cross-over study design was used for the present investigation. The TEOAE and contralateral suppression of TEOAE were recorded before and after consumption of coffee and milk. The contralateral suppression of TEOAE was measured by presenting white noise to the contralateral ear at 40, 50, and 60 dB sound pressure level (SPL). Results: The mean amplitude of TEOAE before and after consumption of coffee was similar in both ears. Further, the mean contralateral suppression of TEOAE was slightly larger after consumption of coffee in both ears. However, the mean difference was not significant in both the ears. Conclusions: Based on the findings of present study, coffee has no significant effect on the amplitude of TEOAE and contralateral suppression of TEOAE.

The Magnitude of Contralateral Suppression of Otoacoustic Emissions Is Ear- and Age-Dependent

The maturation of the uncrossed medial olivocochlear (UMOC) efferent remains poorly documented to date. The UMOC efferent system allows listeners to not only detect but also to process, recognize, and discriminate auditory stimuli. Its fibers can be explored non-invasively by recording the effect of contralateral acoustic stimulation (CAS), resulting in a decrease in the amplitude of transient evoked otoacoustic emissions (TEOAE). The objective of the present cross-sectional study was to investigate how the effectiveness of this system varies with age in healthy subjects aged 8 years to adulthood. For this purpose, 120 right-handed native French-speaking subjects (57 females and 63 males) were divided into five age groups of 24 subjects each: 8y–10y, 10y–11y6m, 11y6m–13y, 13y–17y, and ≥18y. TEOAE amplitudes with and without CAS were recorded. The equivalent attenuation (EA) was calculated, corresponding to the change in TEOAE amplitude equivalent to the effect generated by CAS. General linear models were performed to control for the effect of ear, sex, and age on EA. No sex effect was found. A stronger EA was consistently found regardless of age group in the right ear compared to the left. In contrast to the right ear, for which, on average, EA remained constant across age groups, an increasingly weaker TEOAE suppression effect with age was found in the left ear, reinforcing the asymmetrical functioning of the UMOC efferent system in favor of the right ear in adulthood. Further studies are needed to investigate the lateralization of the UMOC efferent system and its changes over time in cases of atypical or reversed cortical asymmetries, especially in subjects with specific learning disorders.

Contralateral suppression of transient evoked otoacoustic emissions in adults: A normative study

Whilst otoacoustic emission (OAE) testing has proved to be valuable in revealing information about cochlear outer hair cell integrity, it does not provide insight into the afferent and efferent pathways once the stimulus has reached neural receptors. This information can be obtained objectively through contralateral acoustic stimulation (CAS) suppression. However, obtaining normative data is essential in the implementation of such tests. The primary aim was to undertake a small pilot study to collect the CAS suppression across a predefined frequency range in order to provide a preliminary normative data set to be used with the newly developed transient evoked otoacoustic emission (TEOAE)-CAS module (PATH MEDICAL, Germering, Germany). Secondary aims included the analysis of the relationships between left and right CAS suppression, between male and female CAS suppression and between TEOAE signal-to-noise ratio (SNR) and CAS suppression. The purpose of this study was to determine preliminary normative data for contralateral TEOAE suppression from 40 normal ears of 20 healthy young adults (10 males and 10 females). Subjects were recruited using purposive sampling. The CAS suppression responses were obtained automatically by means of the data-collection protocol on the device used. From the data obtained, correlations between TEOAE SNR and CAS suppression were made using Pearson's correlation coefficient. The data were statistically processed to form a normative database which possesses the potential of serving as a basis for further research aimed at determining the utility of CAS suppression testing when evaluating ear pathology. A mean CAS suppression of 0.8 decibels (dB) (0.61 SD) was obtained. There was no statistically significant relationship between TEOAE SNR and CAS suppression. There was no significant suppression difference in terms of laterality of ears or gender. Normative values for CAS suppression of TEOAEs in a group of normal-hearing individuals were obtained using the newly developed TEOAE-CAS module (PATH MEDICAL, Germering, Germany). The availability of normative data for contralateral TEOAE suppression using the studied module allows for it to become commercially available, which will enable researchers and audiologists to perform this measurement in different populations in the evaluation of ear pathology.

Controlled (re)evaluation of the relationship between speech perception in noise and contralateral suppression of otoacoustic emissions

In people with normal hearing (NH), speech perception in noise (SPIN) improves when the speech signal is presented not gated with noise but after a delay. The medial olivocochlear reflex (MOCR) was thought to be involved in the neural dynamic range adaptation (NDRA) responsible for this adaptive SPIN; however, some of the recent studies do not support this hypothesis and suggest that adaptive SPIN involves the NDRA to noise-level statistics, irrespective of MOCR activation. A plausible reason for this discrepancy could be the variations and limitations of the experimental designs used in different studies. Using a relatively controlled and comprehensive study design, this study attempts to verify whether a delay between the delivery of speech and the noise improves the SPIN and whether MOCR mediates such effects. The SPIN was estimated by measuring speech reception thresholds (SRT) in noise under simultaneous-onset and delayed-onset (noise precedes speech onset by 300 ms) conditions. The SPIN in both ears was independently examined for ipsilateral, contralateral, and bilateral noise in women with normal hearing (N = 18; age range, 18–25 years). Contralateral suppression of transient-evoked otoacoustic emissions (CSOAEs) was used to estimate the MOCR based cochlear gain reduction. Under all test conditions, SPIN was improved in delayed-onset than in simultaneous-onset conditions, and the mean improvement in the SRT ranged from 0.7±1.7 to 1.8±1.8 dB. No significant correlation was obtained between CSOAEs and the mean temporal improvement in SRT, suggesting that MOCR may not be a predominant mechanism for the temporal improvement in SPIN.

Short-Term Reliability of Different Methods of Contralateral Suppression of Transient Evoked Otoacoustic Emission in Children and Adults.

Purpose This study aimed to investigate the reliability of 3 methods to measure contralateral suppression of transient evoked otoacoustic emissions (TEOAEs) in children and adults. Method Contralateral suppression of TEOAEs was measured in 14 adults and 14 children using 3 methods with and without contralateral acoustic stimulus (CAS). Method-I having "2 s on-off" and Method-II having "10 s on-off" interleaved presentation of white noise. Method-III used "continuous presentation of white noise". Test-retest reliability was checked in adults without removing the probe (same-probe recording) and reinserting the probe (different-probe recording) and in children using a different-probe recording. Results The absolute suppression amplitude of TEOAEs was higher for "continuous noise," followed by "10 s on-off" and "2 s on-off" CAS. There was no significant effect of age across the 2 probe recordings, 3 methods of TEOAEs with and without CAS, and for the absolute suppression amplitude. Also, in adults, there was no significant difference between same-probe and different-probe recordings across the 3 methods. High internal consistency was observed on Cronbach's alpha (α > .9) for the 3 methods and 2 probe recordings. High agreement and correlation between the recordings for all 3 methods were seen using Bland-Altman plots and Pearson product-moment correlation coefficient. Conclusion The study demonstrated that highly reliable contralateral suppression of TEOAE can be measured using the 3 methods in adults and children. However, continuous presentation of CAS resulted in greater TEOAE suppression amplitude compared to interleaved presentation of CAS; hence, the former is recommended.

The Influence of Efferent Inhibition on Speech Perception in Noise: A Revisit Through Its Level-Dependent Function.

Purpose The study aimed to assess the relationship between the level-dependent function of efferent inhibition and speech perception in noise across different intensities of suppressor and across different signal-to-noise ratios (SNRs) of speech. Method Twenty-six young normal-hearing adults participated in the study. Contralateral suppression of transient evoked otoacoustic emissions (TEOAEs) was measured for 3 levels of suppressor (40, 50, and 60 dB SPL). Speech identification score (SIS) was measured at 5 ipsilateral SNR conditions (quiet, 0, -5, -10, and -15 dB), with and without contralateral broadband noise at 3 levels (40, 50, and 60 dB SPL). Furthermore, SNR-50 was measured with and without the same 3 levels of contralateral broadband noise. Results The results showed that the suppression magnitude of TEOAE increased with an increase in suppressor level. However, neither SIS nor SNR-50 was influenced by the contralateral noise. In addition, SIS and SNR-50 did not show significant correlation with contralateral suppression of TEOAEs. This was true at all the SNRs and contralateral noise levels used in the study. Conclusions The findings suggest that the intensity of noise directly influences medial olivocochlear bundle-mediated efferent inhibition. However, the role of the medial olivocochlear bundle in regulating speech perception in noise needs to be revisited. Supplemental Material https://doi.org/10.23641/asha.9336353.

Contralateral suppression of otoacoustic emissions in a clinical sample of children with auditory processing disorder

Objective: The suppression of evoked otoacoustic emissions (EOAE) may serve as a clinical tool to evaluate the medial olivocochlear (MOC) reflex, which is thought to aid speech discrimination (particularly in noise) by selectively inhibiting cochlear amplification. The present study aimed to determine if contralateral transient evoked otoacoustic emission (TEOAE) suppression was present in a clinical sample of children with listening difficulties with and without auditory processing disorder (APD).Design: A three-group, repeated measure design was used.Study sample: Forty three children aged 8–14 years underwent an auditory processing assessment and were divided into three groups: children with reported listening difficulties with APD, children with reported listening difficulties without APD, and children with normal hearing. APD was defined as per British Society of Audiology.Results: TEOAE suppression was present in all three participant groups. No significant group, age or ear effects were observed for TEOAE suppression in dB or as a normalised index.Conclusion: Contralateral TEOAE suppression method could not be used as a clinical tool to identify APD in this study’s participating children and did not support the hypothesised link between reduced MOC function and general listening difficulties in background noise in children with or without APD.

Adversarial relationship between combined medial olivocochlear (MOC) and middle-ear-muscle (MEM) reflexes and alarm-in-noise detection thresholds under negative signal-to-noise ratios (SNRs)

The role of auditory efferent feedback from the medial olivocochlear system (MOCS) and the middle-ear-muscle (MEM) reflex in tonal detection tasks for humans in the presence of noise is not clearly understood. Past studies have yielded inconsistent results on the relationship between efferent feedback and tonal detection thresholds. This study attempts to address this inconsistency. Fifteen human subjects with normal hearing participated in an experiment where they were asked to identify an alarm signal in the presence of 80 dBA background (pink) noise. Masked detection thresholds were estimated using the method of two-interval forced choice (2IFC). Contralateral suppression of transient-evoked otoacoustic emissions (TEOAEs) was measured to estimate the strength of auditory efferent feedback. Subsequent correlation analysis revealed that the contralateral suppression of TEOAEs was significantly negatively correlated (r = −0.526, n = 15, p = 0.0438) with alarm-in-noise (AIN) detection thresholds under negative signal-to-noise conditions. The result implies that the stronger the auditory efferent feedback, the worse the detection thresholds and thus the poorer the tonal detection performance in the presence of loud noise.

Olivocochlear Efferent Activity Is Associated With the Slope of the Psychometric Function of Speech Recognition in Noise.

The medial olivocochlear (MOC) efferent system can modify cochlear function to improve sound detection in noise, but its role in speech perception in noise is unclear. The purpose of this study was to determine the association between MOC efferent activity and performance on two speech-in-noise tasks at two signal-to-noise ratios (SNRs). It was hypothesized that efferent activity would be more strongly correlated with performance at the more challenging SNR, relative to performance at the less challenging SNR. Sixteen adults aged 35 to 73 years participated. Subjects had pure-tone averages ≤25 dB HL and normal middle ear function. High-frequency pure-tone averages were computed across 3000 to 8000 Hz and ranged from 6.3 to 48.8 dB HL. Efferent activity was assessed using contralateral suppression of transient-evoked otoacoustic emissions (TEOAEs) measured in right ears, and MOC activation was achieved by presenting broadband noise to left ears. Contralateral suppression was expressed as the decibel change in TEOAE magnitude obtained with versus without the presence of the broadband noise. TEOAE responses were also examined for middle ear muscle reflex activation and synchronous spontaneous otoacoustic emissions (SSOAEs). Speech-in-noise perception was assessed using the closed-set coordinate response measure word recognition task and the open-set Institute of Electrical and Electronics Engineers sentence task. Speech and noise were presented to right ears at two SNRs. Performance on each task was scored as percent correct. Associations between contralateral suppression and speech-in-noise performance were quantified using partial rank correlational analyses, controlling for the variables age and high-frequency pure-tone average. One subject was excluded due to probable middle ear muscle reflex activation. Subjects showed a wide range of contralateral suppression values, consistent with previous reports. Three subjects with SSOAEs had similar contralateral suppression results as subjects without SSOAEs. The magnitude of contralateral suppression was not significantly correlated with speech-in-noise performance on either task at a single SNR (p > 0.05), contrary to hypothesis. However, contralateral suppression was significantly correlated with the slope of the psychometric function, computed as the difference between performance levels at the two SNRs divided by 3 (decibel difference between the 2 SNRs) for the coordinate response measure task (partial rs = 0.59; p = 0.04) and for the Institute of Electrical and Electronics Engineers task (partial rs = 0.60; p = 0.03). In a group of primarily older adults with normal hearing or mild hearing loss, olivocochlear efferent activity assessed using contralateral suppression of TEOAEs was not associated with speech-in-noise performance at a single SNR. However, auditory efferent activity appears to be associated with the slope of the psychometric function for both a word and sentence recognition task in noise. Results suggest that individuals with stronger MOC efferent activity tend to be more responsive to changes in SNR, where small increases in SNR result in better speech-in-noise performance relative to individuals with weaker MOC efferent activity. Additionally, the results suggest that the slope of the psychometric function may be a more useful metric than performance at a single SNR when examining the relationship between speech recognition in noise and MOC efferent activity.

Utility of otoacoustic emissions and olivocochlear reflex in predicting vulnerability to noise-induced inner ear damage.

Aim:The aim of the present study was to explore the possible utility of otoacoustic emissions (OAEs) and efferent system strength to determine vulnerability to noise exposure in a clinical setting.Materials and Methods:The study group comprised 344 volunteers who had just begun mandatory basic training as Hellenic Corps Officers Military Academy cadets. Pure-tone audiograms were obtained on both ears. Participants were also subjected to diagnostic transient-evoked otoacoustic emissions (TEOAEs). Finally, they were all tested for efferent function through the suppression of TEOAEs with contralateral noise. Following baseline evaluation, all cadets fired 10 rounds using a 7.62 mm Heckler & Koch G3A3 assault rifle while lying down in prone position. Immediately after exposure to gunfire noise and no later than 10 h, all participants completed an identical protocol for a second time, which was then repeated a third time, 30 days later.Results:The data showed that after the firing drill, 280 participants suffered a temporary threshold shift (TTS) (468 ears), while in the third evaluation conducted 30 days after exposure, 142 of these ears still presented a threshold shift compared to the baseline evaluation [permanent threshold shift (PTS) ears]. A receiver operating characteristics curve analysis showed that OAEs amplitude is predictive of future TTS and PTS. The results were slightly different for the suppression of OAEs showing only a slight trend toward significance. The curves were used to determine cut points to evaluate the likelihood of TTS/PTS for OAEs amplitude in the baseline evaluation. Decision limits yielding 71.6% sensitivity were 12.45 dB SPL with 63.8% specificity for PTS, and 50% sensitivity were 12.35 dB SPL with 68.2% specificity for TTS.Conclusions:Interestingly, the above data yielded tentative evidence to suggest that OAEs amplitude is both sensitive and specific enough to efficiently identify participants who are particularly susceptible to hearing loss caused by impulse noise generated by firearms. Hearing conservation programs may therefore want to consider including such tests in their routine. As far as efferent strength is concerned, we feel that further research is due, before implementing the suppression of OAEs in hearing conservations programs in a similar manner.

Contralateral suppression of transient evoked otoacoustic emissions for various noise signals

Purpose: Efferent auditory system has an inhibitory function in the auditory system, and has been hypothesized to improve speech perception in noise. The function of efferent auditory system can be measured non-invasively using suppression of otoacoustic emissions (OAEs). OAEs are very small amplitude sounds generated in the cochlea by motile function of outer hair cells, and activation of the efferent system reduces the amplitude of OAEs, referred as suppression of OAEs. Several studies have been carried out by various investigators to understand the role of efferent auditory pathways during auditory processing. These investigations have used white noise, narrowband noise, pure-tones, modulated tones, etc., to activate the efferent system. However, in real-life situation, these noises are seldom found, hence the findings of above investigations may not be readily generalized to real-life situations. Hence, the present study was carried out to investigate the effect of real-life noise signals on the suppression of OAEs.Method: A total of 19 young adults aged between 18 and 23 years participated in the study. Transient evoked otoacoustic emissions (TEOAEs) were recorded with and without suppressor signal in contralateral ear conditions. Suppressor signals included white noise, amplitude-modulated noise, and real-life noise signals. The amount of suppression elicited by these noise signals were measured and compared across the noise conditions.Results: Findings of present study shows the largest suppression of TEOAE for white noise. In contrast, speech babble elicited least suppression of TEOAE compared to other noise signals used in the present investigation. For amplitude-modulated noise, the amount of suppression increased with modulation frequency of noise. Among real-life noise signals, the suppression was largest for cafeteria noise compared with traffic noise.Conclusion: Real-life noise signals used in the present study also elicited suppression, but the amount of suppression was found to be variable depending on the type of noise.

Effect of practicing yoga on functioning of efferent auditory system

Introduction: The present study attempted to determine the effect of practicing Yoga on the functioning of the efferent auditory system using contralateral suppression of otoacoustic emissions (OAE). Method: The efferent suppression of transient evoked OAE (TEOAE), and distortion product OAE (DPOAE) was recorded from 40 participants (20 who practice Yoga regularly and 20 who do not practice yoga regularly). Results: The results of this study showed that there was an increase in the amount of suppression of both TEOAE and DPOAE in individuals who practice Yoga. The result suggests that Yoga enhances the sensory perception and improves auditory attention. The constant practice of Yoga could have led to the plasticity of the efferent auditory system. Conclusions: Thus, the practice of Yoga may be used to strengthen the efferent auditory system. However, further studies on a larger group of the population are essential for better clinical applicability of the results.

Avaliação eletroacústica da via eferente olivococlear em indivíduos com queixa de zumbido

RESUMO Objetivo: investigar o efeito supressor das emissões otoacústicas por estímulos transientes em indivíduos com queixa zumbido e audiometria normal e analisar sua relação com as variáveis idade, sexo, lateralidade do zumbido e grau de incômodo. Métodos: foram avaliados 60 sujeitos, 14 do gênero masculino e 46 do gênero feminino, entre 20 e 59 anos de idade, sendo 30 com queixa de zumbido (grupo experimental) e 30 sem zumbido (grupo controle). Foi realizada a pesquisa da supressão das emissões otoacústicas por estímulos transientes, para ruído branco de 50 dBNA, na condição contralateral nas bandas de frequência de 700, 1000, 1400, 2000, 2800 and 4000Hz. Resultado: no grupo experimental, a supressão das emissões otoacústicas transientes média variou de 2,14 a 4,38. No grupo controle o valor médio da supressão das emissões otoacústicas transientes variou de 2,27 a 4,88. Conclusão: os valores de supressão das emissões otoacústicas foram semelhantes nos indivíduos com e sem zumbido, embora o grupo com o sintoma tenha tido resultados menores, sugerindo pior desempenho do Complexo Olivar Superior.

Concurrent measures of contralateral suppression of transient-evoked otoacoustic emissions and of auditory steady-state responses.

Contralateral suppression of otoacoustic emissions (OAEs) is frequently used to assess the medial olivocochlear (MOC) efferent system, and may have clinical utility. However, OAEs are weak or absent in hearing-impaired ears, so little is known about MOC function in the presence of hearing loss. A potential alternative measure is contralateral suppression of the auditory steady-state response (ASSR) because ASSRs are measurable in many hearing-impaired ears. This study compared contralateral suppression of both transient-evoked otoacoustic emissions (TEOAEs) and ASSRs in a group of ten primarily older adults with either normal hearing or mild sensorineural hearing loss. Responses were elicited using 75-dB peak sound pressure level clicks. The MOC was activated using contralateral broadband noise at 60 dB sound pressure level. Measurements were made concurrently to ensure a consistent attentional state between the two measures. The magnitude of contralateral suppression of ASSRs was significantly larger than contralateral suppression of TEOAEs. Both measures usually exhibited high test-retest reliability within a session. However, there was no significant correlation between the magnitude of contralateral suppression of TEOAEs and of ASSRs. Further work is needed to understand the role of the MOC in contralateral suppression of ASSRs.

IPSILATERAL SUPPRESSION OF TRANSIENT EVOKED OTOACOUSTIC EMISSIONS IN ADULTS

Background Measurement of the medial olivocochlear (MOC) reflex provides useful clinical information for understanding the function of the auditory system. Although transient evoked otoacoustic emission (TEOAE) suppression has been demonstrated to be an indicator of MOC activity, its full role and fine details of TEOAE suppression spectra are still not clear. The aim of this study was to investigate details of ipsilateral suppression of TEOAEs in normally hearing adults. Material and Methods Exactly 29 adults (13 males, 16 females, mean age 26.5 years, range 18–42 years), who passed a battery of tests including otoscopy, pure tone audiometry, immittance, and TEOAE tests, participated in the study. Suppression was evaluated by comparing TEOAEs obtained with and without an ipsilateral suppressor in a forward-masking paradigm. Results In general, suppression was small – less than 1.4 dB at all frequencies studied. The spectrum of mean TEOAE suppression showed suppression was greatest between 586 and 3711 Hz (0.6–1.4 dB) but less than 0.6 dB between 3906 and 4883 Hz. Mean suppression increased with post-stimulus time from 0.1 dB in the 2–4 ms time window to a maximum of 2.26 dB in the 16–18 ms window; the mean suppression between 8 and 18 ms after noise stimulation was 1.32 dB (range=0.22–3.23 dB). There were no significant gender or ear-laterality effects. Noise levels as measured in the ear canal were found to have a significant effect on calculated suppression at some frequencies. Conclusions The present study provides evidence of small ipsilateral TEOAE suppression by forward-masking noise in normally hearing adults. However, care is needed in interpreting the findings as noise in the ear canal can be a confounding factor during measurement of TEOAE suppression.

Assessing the Effects of Pesticide Exposure on the Auditory System based on the Suppression of Transient-Evoked Otoacoustic Emissions

Introduction: Although noise-induced hearing loss is the most significant occupational hazard affecting the auditory system, studies on hearing conservation have shown that frequent exposure to chemicals in the workplace may also cause hearing disorders. Objective: The aim of this study was to evaluate the medial olivocochlear system in individuals exposed to pesticides. Method: This is a cross-sectional descriptive epidemiological study. After applying the inclusion criteria, we evaluated 205 students from an agricultural school located in the city of Nova Friburgo, state of Rio de Janeiro, Brazil, who were exposed to pesticides. Participants were evaluated based on a questionnaire to assess the degree of pesticide exposure, transient-evoked otoacoustic emissions (TEOAEs), and investigation of the suppressive effect of TEOAEs. Results: The assessment considering the suppression of TEOAEs demonstrated that 38% of participants had absence of suppression in the right ear, 45.9% had absence of suppression in the left ear, 38% had unilateral absence, and 39% had bilateral absence. Our findings suggest that there is an association between absence of suppression and quartiles of exposure. Conclusion: Chronic exposure to pesticides can affect the medial olivocochlear system, regardless of concomitant exposure to noise.

On the Relationship Between Musicianship and Contralateral Suppression of Transient-Evoked Otoacoustic Emissions.

The medial olivocochlear (MOC) efferent reflex that modulates outer hair cell function has been shown to be more robust in musicians versus nonmusicians as evidenced in greater contralateral suppression of transient-evoked otoacoustic emissions (TEOAEs). All previous research comparing musical ability and MOC efferent strength has defined musicianship dichotomously (i.e., high-level music students or professional classical musicians versus nonmusicians). The objective of the study was to further explore contralateral suppression of TEOAEs among adults with a full spectrum of musicianship ranging from no history of musicianship to professional musicians. Musicianship was defined by both self-report and with an objective test to quantify individual differences in perceptual music skills. A single-factor between-subjects and correlational research designs were employed. Forty-five normal-hearing young adults participated. Participants completed a questionnaire concerning their music experience and completed the Brief Profile of Music Perception Skills (PROMS) to quantify perceptual musical skills across multiple musical domains (i.e., accent, melody, tempo, and tuning). TEOAEs were evaluated with 60 dB peak equivalent sound pressure level click stimuli with and without a contralateral 65 dB sound pressure level white noise suppressor. TEOAE suppression was expressed in two ways, absolute TEOAE suppression in dB and a normalized index of TEOAE suppression (i.e., percentage of suppression). Participants who considered themselves musicians scored significantly higher on all subscales and total Brief PROMS score (p < 0.05). There was no statistically significant difference between musicians and nonmusicians in absolute TEOAE suppression or percentage of TEOAE suppression (p > 0.05). There were no statistically significant correlations or linear predictive relationships between subscale or total Brief PROMS scores with absolute and percentage of TEOAE suppression (p > 0.05). The findings do not support the notion of a graded enhancement of MOC efferent suppression among adults with varied degrees of musicianship from nonmusicians to professional musicians.

Reliability of measures of transient evoked otoacoustic emissions with contralateral suppression

The reliability of measures of transient evoked otoacoustic emissions (TEOAEs) with contralateral suppression was examined. The effect of test session (i.e., initial test; retest without probe removal; retest with probe removal; and retest 1–2 days post initial test), gender, and ear was examined in 14 young adult females and 14 young adult males. TEOAEs were obtained bilaterally with 60dB peSPL linear click stimuli with and without a contralateral 65dB SPL broadband noise suppressor. Absolute TEOAE suppression and a normalized index of TEOAE suppression (i.e., percentage of suppression) were examined. Reliability of these measures was assessed with repeated measures linear mixed model analysis of variance, a coefficient of reliability, and Bland–Altman analyses. There were no statistically significant (p>0.05) main effects of test, gender, and ear or interactions for both absolute dB and % TEOAE suppression values. Cronbach's α were greater than 0.90 across the four tests for both TEOAE measures. Mean test differences or bias (i.e., between the initial and subsequent tests) for absolute and % TEOAE suppression ranged from −0.05 to 0.11dB and −1.5% to 1.1%, respectively. There was no proportional/systematic bias with the mean differences of the first and subsequent measurements. Data herein were consistent with the view that bilateral TEOAE suppression measures are reliable across test sessions of 1–2 days among females and males and may provide a method to monitor medial olivocochlear efferent reflex status over time.

Right-ear advantage drives the link between olivocochlear efferent 'antimasking' and speech-in-noise listening benefits.

The mammalian cochlea receives feedback from the brainstem medial olivocochlear (MOC) efferents, whose putative 'antimasking' function is to adjust cochlear amplification and enhance peripheral signal detection in adverse listening environments. Human studies have been inconsistent in demonstrating a clear connection between this corticofugal system and behavioral speech-in-noise (SIN) listening skills. To elucidate the role of brainstem efferent activity in SIN perception, we measured ear-specific contralateral suppression of transient-evoked otoacoustic emissions (OAEs), a proxy measure of MOC activation linked to auditory learning in noisy environments. We show that suppression of cochlear emissions is stronger with a more basal cochlear bias in the right ear compared with the left ear. Moreover, a strong negative correlation was observed between behavioral SIN performance and right-ear OAE suppression magnitudes, such that lower speech reception thresholds in noise were predicted by larger amounts of MOC-related activity. This brain-behavioral relation was not observed for left ear SIN perception. The rightward bias in contralateral MOC suppression of OAEs, coupled with the stronger association between physiological and perceptual measures, is consistent with left-hemisphere cerebral dominance for speech-language processing. We posit that corticofugal feedback from the left cerebral cortex through descending MOC projections sensitizes the right cochlea to signal-in-noise detection, facilitating figure-ground contrast and improving degraded speech analysis. Our findings demonstrate that SIN listening is at least partly driven by subcortical brain mechanisms; primitive stages of cochlear processing and brainstem MOC modulation of (right) inner ear mechanics play a critical role in dictating SIN understanding.
