Quality of Communication between Dental Technicians and Dentists during the Fabrication of Removable Partial Dentures in Casablanca, Morocco

Introduction: Communication between the dentist and the dental laboratory technician plays an essential role in the success of prosthetic treatment. The purpose of this study was to describe the quality of communication between dentist and dental technicians, and to highlight the different constraints encountered by the laboratory technician when making removable partial prostheses. Materials and Methods: This is a cross-sectional study conducted among 94 dental technician working in the private sector in Casablanca. To collect data an auto administrated questionnaire was distributed to the dental technicians. This tool included five sections : Identification, communication methods, prosthetic design, communication difficulties and the added value of digitizing the prosthetic chain. Data were statistically processed using SPSS 20 software. Results: A total of 94 dental technicians working in Casablanca were interviewed. All these laboratory technicians (100%) used the laboratory form as their main means of communication. Additional means of communication were used to complete missing information (phone, photo sharing, e-mail). 72.3% of dental technicians designed the metal framework for cast partial dentures themselves and 67% of prosthetists said that digitizing the prosthetic chain could improve dentist - dental laboratory technician communication. Conclusion: Effective communication between dentist and dental technician is essential for successful prosthetic treatment and patient satisfaction

Prosthodontic rehabilitation on a patient with limited mouth opening related to rheumatoid arthritis: A clinical report

Specific considerations are needed for the dental treatment of patients with rheumatoid arthritis because they may experience disease-related problems such as trismus, impairment of the temporomandibular joint or hands, and side effects of medication that affect alveolar bone. Here we describe successful prosthetic treatment that addressed disease-related problems, including trismus, in a patient with rheumatoid arthritis. (Int J Maxillofac Prosthetics 2022;5:18-21)

Clinical Remount in Complete Denture

Proper occlusal relationships are a part of successful prosthetic treatment for edentulous patients. Fabrication of complete dentures involves clinical and laboratory procedures that should be performed precisely for achieving success with fabricated dentures. These occlusal errors can be corrected, (1) in a patient's mouth or (2) by recording new centric relation and remounting dentures on an articulator. The second method is more viable because the denture base moves over the oral mucosa which is unable to identify premature contacts in centric occlusion and tooth guided eccentric excursions. This article depicts the modest and effective clinical chairside remounting procedure.

Prosthodontic maintenance and peri-implant tissue conditions for telescopic attachment-retained mandibular implant overdenture: Systematic review and meta-analysis of randomized clinical trials.

The mandibular implant-retained overdentures (MIRO) are a highly successful prosthetic treatment option. However, an argument still present regarding its design and type of attachment system. This systematic review and meta-analysis aimed to perform a qualitative and quantitative analysis of the scientific literature regarding the telescopic attachments versus other attachment systems retaining mandibular implant overdentures. Manual and electronic database (PubMed and Cochrane) searches were performed to identify randomized controlled trials (RCTs) comparing telescopic versus other attachment systems. Independently, two investigators extracted the trials' data. The Cochrane tool was used for assessing the quality of included studies. Meta-analyses were performed for the included RCTs and reported the same outcome measures. Nine RCTs were identified. Three RCTs (corresponding to four publications) were included in the study. The other five trials were excluded from the study. The meta-analysis revealed no difference between telescopic crowns and ball attachment retaining mandibular implant overdenture as regards prosthodontic maintenance. Regarding peri-implant conditions, ball-retained mandibular overdenture showed statistically significant more probing depth around implants records in ball-retained overdenture when compared to the telescopic group. However, there are no statistically significant differences between two interventions in regard to marginal bone loss, bleeding index, gingival index, and plaque index. In conclusions, no significant differences in prosthodontic maintenance and peri-implant condition between telescopic attachments and ball attachments retaining MIRO. However, this should be considered with caution because of a limited number of included studies.

Orientation of handle for successful prosthetic treatment in patients with an anatomic compromise after a maxillectomy

This clinical technique provides a key procedure for successful prosthetic treatment in patients with an anatomical compromise after a maxillectomy. When making a preliminary impression, the clinician must keep the handle of the stock tray parallel to the patient's interpupillary line extraorally to maintain the correct position of the tray and to make a record of the inclination of the occlusal plane. This simple consideration during this first step of making a preliminary impression can be an important guide for both dentists and dental technicians. Information about the correct positioning of the tray and the inclination of the occlusal plane obtained by using a stock tray handle will greatly reduce the stress experienced by dentists, dental technicians, and patients during the fabrication of the prosthesis and will ultimately lead to successful prosthetic treatment in patients with anatomic compromise resulting from a maxillectomy.

Improved Construction of Auricular Prosthesis by Digital Technologies.

Implant-retained auricular prostheses are a successful prosthetic treatment option for patients who are missing their ear(s) due to trauma, oncology, or birth defects. The prosthetic ear is aesthetically pleasing, composed of natural looking anatomical contours, shape, and texture along with good color that blends with surrounding existing skin. These outcomes can be optimized by the integration of digital technologies in the construction process. This report describes a sequential process of reconstructing a missing left ear by digital technologies. Two implants were planned for placement in the left mastoid region utilizing specialist biomedical software (Materialise, Belgium). The implant positions were determined underneath the thickest portion (of anti-helix area) left ear that is virtually simulated by means of mirror imaging of the right ear. A surgical stent recording the implant positions was constructed and used in implant fixtures placement. Implants were left for eight weeks, after which they were loaded with abutments and an irreversible silicone impression was taken to record their positions. The right existing ear was virtually segmented using the patient CT scan and then mirror imaged to produce a left ear, which was then printed using 3D printer (Z Corp, USA). The left ear was then duplicated in wax which was fitted over the defect side. Then, it was conventionally flasked. Skin color was digitalized using spectromatch skin color system (London, UK). The resultant silicone color was mixed as prescribed and then packed into the mold. The silicone was cured conventionally. Ear was trimmed and fitted and there was no need for any extrinsic coloring. The prosthetic ear was an exact match to the existing right ear in shape, skin color, and orientation due to the great advantages of technologies employed. Additionally, these technologies saved time and provided a base for reproducible results regardless of operator.

Psychological therapy in prosthetic treatment

Prosthetic treatment aims at the restoration of the stomatognathic system normal function. An extremely important element of prosthetic rehabilitation is an appropriate psychological-pedagogical conduct of doctors towards patients, which is related to psychological problems on the part of patients. A successful psychological therapy is frequently decisive for a successful prosthetic treatment, especially in completely edentulous patients, a large percentage of whom are elderly people.

Custom ocular prosthesis: Comparison of two different techniques

Patients and techniquesThis article describes two different techniques of iris customization on two different patients. First technique describes the iris customization with the use of conventional paint on technique while second describes the photographic method of fabricating the eye prosthesis. DiscussionA sequence of steps of construction of ocular prostheses is outlined, and the critical areas of fabrication and artistic techniques employed in the successful prosthetic treatment are described in this article. Conventional iris paint on technique and digital photographic technique to customize the iris are explained along with their advantages, disadvantages and indications. ConclusionIn addition to decrease treatment time and increased simplicity, digital photography technique is particularly advantageous in cases requiring thin acrylic shell type of prosthesis.

Fabrication of complete dentures comprises of clinical and laboratory procedures that should be executed accurately for achieving success with fabricated dentures. Errors occurring during the clinical and laboratory procedures of a denture may subsequently lead to the occlusal errors in the final prosthesis. These occlusal errors can be corrected in two ways: i) in patient's mouth ii) by recording new centric relation and remounting dentures on an articulator. The latter method is more feasible because the mobility of denture base on the mucosa in oral cavity does not permit the identification of premature contacts in centric occlusion and tooth guided eccentric excursions. This article describes a modest and effective clinical chair-side remount procedure using customized mounting platforms.

Subjective assessment of mastication as parameter for successful prosthetic therapy

Introduction. Success in functional rehabilitation of the craniomandibular system in patients without teeth, which have total prosthesis, can be assessed using different clinical and functional methods. Subjective assessment, motivation, comfort level and functional efficacy are important elements for adaptation to dental prosthesis as well as base for success in prosthetic therapy. The aim of this study was to evaluate the importance of subjective assessment of the mastication in people with new dental prosthesis as well as to assess the value of that parameter in determining the successful prosthetic therapy. Material and Methods. Study was conducted at the Dental Clinic in Novi Sad. Thirty patients (16 males and 14 females) with average age of 62.5 years who received total prosthesis were included. Analysis was done according to the data from the medical records and conducted survey in the form of questionnaire which was adjusted to our clinical examinations. Results. Results showed that 96.6% of the patients were satisfied with new prosthesis. Subjective assessment of the quality of their mastication before therapy showed that 63.4% consider their mastication as unsatisfied, 26.6% satisfied while 10% of toothless patients thought their mastication was good. Statistic analysis showed there was significant difference in subjective assessment before and after the therapy (p<0.01). In fact, there was significant connection between subjective assessment of the mastication and satisfaction with new prosthesis and therapy at all (p<0.01). Conclusion. Patients satisfaction with total prosthesis and subjective assessment of mastication are in direct correlation with successful prosthetic treatment. Results of subjective assessment of the mastication are important and can be used as a parameter for success along with precise survey.

Mandibular implant-retained overdentures: A literature review

The implant-retained overdenture for the mandible has been shown to be a highly successful prosthetic treatment similar to the fixed implant denture. However, controversy persists as to its design and indications. Few literature reviews have been published on the topic. This article critically analyzes the existing mandibular implant overdenture literature relative to bone preservation, effect on antagonist jaw, number of implants required, anchorage systems, maintenance, and patient satisfaction. A MEDLINE search was completed (from 1987 to 2001), along with a manual search, to locate relevant English-language articles on mandibular implant overdentures. Twelve treatment concepts are elucidated from a distillation of the literature review. (J Prosthet Dent 2001;86:468-73.)
