DESIGNING AND IMPLEMENTING APPLIED BACHELOR'S PROGRAM: A COMPREHENSIVE DEVELOPMENT MODEL

Modern education faces the challenge of integrating theoretical knowledge with practical skills, which is particularly crucial for the training of mid-level professionals. Practice-oriented educational programs shorten the adaptation period for graduates and enhance their competitiveness in the labor market. Special attention is given to applied bachelor's programs that combine theory and practice, fostering rapid professional development of graduates. The European higher education system offers short cycles leading to a bachelor's degree. The practical focus of these programs is ensured through partnerships between educational institutions and industry organizations, enabling students to develop the professional skills required by employers. This makes applied bachelor's programs key elements of educational reforms in the EU. In Kazakhstan, applied bachelor's programs have not yet gained widespread popularity due to a lack of regulatory support, but successful pilot projects demonstrate the potential of this direction. These programs integrate theoretical and practical training, approaching education with consideration of real-world functional tasks. The need to adapt educational programs to meet the current demands of the labor market is extremely important for preparing qualified personnel. This underscores the importance of continually improving educational initiatives to develop programs that successfully combine theory with practice and meet the needs of a dynamic economic environment.

Comprehensive analysis of integrating smart grids with renewable energy sources: Technological advancements, economic impacts, and policy frameworks

This study presents a comprehensive analysis of integrating smart grids with renewable energy sources, focusing on technological advancements, economic impacts, and policy frameworks. The primary objective is to explore how smart grid technologies can efficiently incorporate renewable energy sources, thereby enhancing grid reliability, efficiency, and sustainability. Utilizing a multidisciplinary approach, the study examines successful case studies, pilot projects, and innovative practices that highlight the potential and challenges of this integration. Key findings reveal that advanced technologies such as artificial intelligence (AI), the Internet of Things (IoT), and blockchain are crucial for the real-time monitoring, predictive maintenance, and optimized management of energy systems. These technologies address the inherent variability and intermittency of renewable energy sources like solar and wind power. Case studies, including the Brooklyn Microgrid and Germany’s Energiewende, demonstrate significant improvements in energy resilience, efficiency, and consumer empowerment through decentralized energy systems. Economic analysis underscores the dual impact of cost savings from operational efficiencies and the financial challenges posed by substantial upfront investments in smart grid infrastructure. Policy frameworks play a pivotal role, with recommendations for supportive regulatory policies, increased funding for research and development, and enhanced public-private partnerships to drive innovation and consumer engagement. The study concludes that overcoming the technical, economic, and regulatory barriers requires coordinated efforts among stakeholders. Recommendations include developing consistent regulatory frameworks, fostering public-private partnerships, and implementing educational programs to encourage consumer participation in renewable energy initiatives. By addressing these challenges, the integration of smart grids with renewable energy sources can pave the way for a more sustainable, resilient, and efficient energy future. Activity theory perspective to the scale‐up and sustainability of mhealth in armed conflict zones of developing countries

AbstractMobile Health (mHealth) presents the potential to facilitate healthcare and improve the quality of life if successful pilot projects are scaled up and sustainable. However, the persistent termination of mHealth at the pilot phase challenges its potential in developing countries. Existing research suggests an absence of understanding of these phenomena for two reasons. First, literature is scarce in the broad context of armed conflict zones in developing countries, particularly in the IS domain. Second, extant literature emphasizes mHealth implementation, while neglecting the scale‐up and sustainability of these technologies. Therefore, this study employs activity theory as a holistic lens to explore how contradictions shape the scale‐up and sustainability of mHealth in the armed conflict zone of developing countries. In this study, rather than viewing contradiction as a challenge, it serves as a driver for effecting changes that can foster scale‐up and sustainability. The Algorithm for the Management of Acute Childhood Illnesses (ALMANACH), provides an empirical basis for this study. The research employs the interpretivism philosophy and the case study method. In‐depth interviews, participatory observations, and document reviews were used to collect data. Purposive and snowball sampling techniques were used to select the study's participants. The study reveals a lack of professional expertise among the CHWs, the non‐involvement of the users and beneficiaries in designing ALMANACH, and the lack of financial compensation for using ALMANACH in practice as contradictions. The study further exposes how community volunteer groups and the ALMANACH scientific board were established to steer demand.

Urban Waste Management Solution through SRF Cofiring in Coal-Fired Power Plants

Abstract Solid Recovery Fuel (SRF) is a highly effective waste product processed through sorting and separation from non-combustible materials in Indonesia. The SRF is then dried to increase its calorific value and ensure homogeneity. Most Indonesian SRF is derived from urban waste, which is predominantly organic material, making it comparable to biomass and having a calorific value akin to lignite coal. The state-owned electricity company, PLN, collaborates with local governments responsible for waste management to utilize SRF as a fuel source in coal-fired power plants through cofiring. This renewable energy mix is an optimal solution, eliminating the need to construct new power plants and optimize waste management programs and local government budgets without burdening PLN’s operational costs when managed through an appropriate collaborative scheme. PT PLN Indonesia Power has successfully conducted research and development efforts to transform urban waste into SRF through the Waste Treatment Facility program. They have also utilized SRF cofiring in coal-fired power plants up to the heat rate testing stage. This alternative solution benefits all stakeholders in addressing the waste management challenge. This program presents an opportunity to optimize waste management costs implemented by local governments, ultimately saving electricity generation costs. Moreover, it may help reduce the financial burden of constructing and operating conventional solar power plants. SRF cofiring in coal-fired power plants is a reliable and cost-effective renewable energy solution. The successful pilot project showcased the positive outcomes achievable through a collaborative management scheme that utilizes SRF products and environmentally friendly electricity generation for sustainable waste management. This approach effectively reduces greenhouse gas emissions and enhances renewable energy generation capacity, highlighting the significant impact possible through this method.

Brief history of lean in the public sector

The development of Lean principles within the public sector began in 2003 following Lean Construction Institute´s developing efforts on relational contracting. Later, challenges and opportunities for implementing Lean were identified in Californian construction law. Collaborations with entities like the California Department of Transportation and the University of California San Francisco showcased Lean's benefits, leading to legislative changes allowing broader implementation across the University of California system. Healthcare also saw streamlined processes through collaborations, in this case, between the P2SL, the Office of Statewide Health Planning and Development, and major healthcare companies, reducing project durations significantly. International expansion in 2009 led to successful Lean pilot projects in Germany and Finland. Inspired by Washington State's Lean governance practices, Lean in Public Sector Construction (LIPS) expanded its focus beyond construction. LIPS has conducted impactful global conferences, highlighting Lean's transformative role in reshaping diverse public sector operations and governance worldwide.

352 Integrated care programme for older persons—community specialist teams: evaluating the patient experiences

Abstract Background Following a successful Slaintecare pilot project (2019–2021) two teams were established in Community Healthcare Networks (CHN) 4&5 and CHN 6&7. The aim of our service is to provide a Comprehensive Geriatric Assessment (CGA) to older people. Service user experience/feedback are critically important to ensure that we are meeting their needs. We also looked to compare the results against previous patient experience survey undertaken in 2021. Methods The survey was designed by the multidisciplinary team, consisting of 15 closed/3 open questions. A Likert scale was utilised for closed questions and thematic analysis for open questions. 208 service users who attended our teams and have received a CGA from January 2023 to April 2023 were invited to participate in a postal survey. All participants received a covering letter, questionnaire and a stamped address envelope to return their completed surveys. Results There was a 35% response rate (73/208). 89% were aged ≥75. 49% of participants completed the survey themselves, 51% required assistance. 66% have had 2 or more interactions with the teams. Vast majority or those who responded highlighted the importance of having their care at home or at centres close to home. 97% agreed/strongly agreed that they were satisfied with the service. 99% felt they were treated with dignity/respect and had confidence in the service. 96% felt that when they had an important question they got an answer that they could understand. 94% reported their carers had the opportunity to discuss concerns. Conclusion Service users had an overwhelmingly positive experience when they engaged with both our teams. CGA assessment and team interventions in the home and local healthcare settings was seen as been beneficial The importance of having time to engage with members of the team, not feeling rushed was also a recurring theme.

The Reduction of Household Waste using Black Soldier Fly using Stack Bucket system

ousehold waste poses economic, social, and environmental challenges, with a significant portion in the form of organic waste. Improper disposal of organic waste can lead to soil and water contamination, ecosystem degradation, and health risks. Black Soldier Fly (BSF) bioconversion is a promising method to address this issue, converting organic waste into valuable resources. This study focuses on using BSF at a community scale to reduce household waste. The research methodology involves waste sorting, organic waste treatment using BSF larvae, and harvesting the larvae for various purposes. The results show that 76% of households successfully sorted their waste, highlighting the role of housewives in waste reduction. Challenges in waste management and public perception of BSF usage are discussed. This research aims to promote BSF use in communities, enhancing the economic value of household waste while minimizing environmental pollution. Collaboration among government bodies, universities, waste management agencies, and the public is crucial to address the challenges and conduct successful pilot projects.

Using telehealth to support antimicrobial stewardship at four rural VA medical centers: Interim analysis

Background: Healthcare settings without access to infectious diseases experts may struggle to implement effective antibiotic stewardship programs. We previously described a successful pilot project using the Veterans Affairs (VA) telehealth system to form a Videoconference Antimicrobial Stewardship Team (VAST) that connected multidisciplinary teams from rural VA medical centers (VAMCs) with infectious diseases experts at geographically distant locations. VASTs discussed patients from the rural VAMC, with the overarching goal of supporting antibiotic stewardship. This project is currently ongoing. Here, we describe preliminary outcomes describing the cases discussed, recommendations made, and acceptance of those recommendations among 4 VASTs. Methods: Cases discussed at any of the 4 participating intervention sites were independently reviewed by study staff, noting the infectious disease diagnoses, recommendations made by infectious diseases experts and, when applicable, acceptance of those recommendations at the rural VAMC within 1 week. Discrepancies between independent reviewers were discussed and, when consensus could not be reached, discrepancies were discussed with an infectious diseases clinician. Results: The VASTs serving 4 different rural VAMCs discussed 96 cases involving 92 patients. Overall, infection of the respiratory tract was the most common syndrome discussed by VASTs (Fig. 1). The most common specific diagnoses among discussed cases were cellulitis (n = 11), acute cystitis (n = 11), wounds (n = 11), and osteomyelitis (n = 10). Of 172 recommendations, 41 (24%) related to diagnostic imaging or laboratory results and 38 (22%) were to change the antibiotic agent, dose, or duration (Fig. 2). Of the 151 recommendations that could be assessed via chart review, 122 (81%) were accepted within 1 week. Conclusions: These findings indicate successful implementation of telehealth to connect clinicians at rural VAMCs with an offsite infectious diseases expert. The cases represented an array of common infectious syndromes. The most frequent recommendations pertained to getting additional diagnostic information and to adjusting, but not stopping, antibiotic therapy. These results suggest that many of the cases discussed warrant antibiotics and that VASTs may use the results of diagnostic studies to tailor that therapy. The high rate of acceptance suggests that the VASTs are affecting patient care. Future work will describe VAST implementation at 4 additional VAMCs, and we will assess whether using telehealth to disseminate infectious diseases expertise to rural VAMCs supports changes in antibiotic use that align with principles of antimicrobial stewardship.Disclosures: None

Enhancing Water Resource Management Efficiency through Smart Technologies: A Case Study in Kashkadarya Region, Uzbekistan

This article presents a comprehensive study on the implementation of "Smart Water" devices in water management facilities in the Kashkadarya region of Uzbekistan. The goals of this initiative were to achieve efficient water control, real-time monitoring, and rapid water management. Through the installation of smart devices, including "Smart Water" devices and "DIVER" devices, water resources were effectively managed, resulting in online water control and significant water resource savings. The automation of large hydrotechnical facilities, such as the Karshi hydroelectric plant, enabled remote management and reduced technical water losses. Additionally, the pilot project on automation, based on solutions provided by the Australian company "Rubicon Water Pty. Ltd," showcased the potential for digitalization and automation in water resources management. The project demonstrated successful outcomes, including increased accuracy in land reclamation assessment, reduced water shortage, enhanced water resource utilization, and significant cost savings. The findings highlight the effectiveness and advantages of smart technologies in water management, underscoring the need for their widespread adoption in addressing water scarcity and sustainable water resource management globally.

Scaling up the “24/7 BHU” strategy to provide round-the-clock maternity care in Punjab, Pakistan: a theory-driven, coproduced implementation study

BackgroundPakistan’s maternal mortality rate remains persistently high at 186/100,000 live births. The country’s government-run first-level healthcare facilities, the basic health units (BHUs), are an important source of maternity care for rural women. However,BHUsonly operate on working days from 8:00 am to 2:00 pm. Recognizing that this severely constrains access to maternity services, the government is implementing the “24/7 BHU” initiative to upgrade BHUs to provide round-the-clock care. Although based on a successful pilot project, initial reports reveal challenges in scaling up the initiative. This implementation research project aims to address a key concern of the Government of Punjab: How can the 24/7 BHU initiative be successfully implemented at scale to provide high-quality, round-the-clock skilled maternity care in rural Punjab?MethodsThe project consists of two overlapping work packages (WP). WP1 includes three modules generating data at the directorate, district and BHU levels. Module 1 uses document analysis and policy-maker interviews to explicateprogrammetheory and begin to build a system model. Module 2 compares government-collected data with data generated from a survey of 1500 births to assess BHU performance. Module 3 uses institutional ethnographies in 4–5 BHUs in three districts to provide a detailed system for understanding and identifying processes that influence scale-up. WP2 includes two modules. First, two workshops and regular meetings with stakeholders integrate WP1 findings, identify feasible changes and establish priorities. Next, “change ideas” are selected for testing in one district and 2–3 BHUs through carefully documented pilots using the PDSA (plan–do–study–act) improvement approach. An integrated knowledge translation approach will engage key policy and practice stakeholders throughout the project.DiscussionThis theory-driven implementation research project willcoproducesignificant new understandings of the wider system in which the 24/7 BHU initiative is being implemented, and actionable knowledge that will highlight ways the implementation processes might be modified to enable BHUs to meet service provision goals. This study will also produce insights that will be relevant for other South Asian and low- and middle-income countries (LMICs) that experience similar challenges of programme scale-up and delivery of maternal health services to remote and marginalized communities.

Implications of CO[formula omitted] mass transport dynamics for large-scale CCS in basalt formations

Flood basalts have the potential for relatively rapid mineral trapping when used as an injection target for CO2 storage. Although CO2 mineral trapping in basalt has been studied in various ways, including two successful small-scale pilot projects, questions remain about how the system will behave during a full-scale CO2 storage project. These questions include whether a full-scale CO2 injection can expect complete mineralization on time scales similar to those observed during small-scale injections, as well as how the properties of the target formation will be altered by decades of geochemical reactions. Recently, we developed VIRTra, a vertically integrated reactive transport model specifically designed for efficient field-scale simulation of CO2 storage in reactive rocks. The present work uses this new method to explore the behavior of the water-CO2-basalt system during large-scale injection of separate-phase CO2 in a deep saline aquifer. Trends in the assembled data indicate that a high rate of CO2 dissolution into the aqueous phase results in faster mineralization. However, the time scales on which full mineralization of the injected CO2 is achieved are on the order of centuries, orders of magnitude larger than those observed in small-scale field tests. This appears to be a direct result of the increase in scale of the injection. During the injection period, changes in porosity are observed to be highly dependent on mineral reaction kinetics. Important areas of further research include the impact of mineralogy and formation water composition on the mineralization process, and the relationship between porosity and permeability in vesicular rock types.

Working Paper 40: Developing anti-corruption interventions addressing social norms

This Working Paper provides guidance on developing anti-corruption interventions based on a Social Norms and Behaviour Change (SNBC) approach. Still a relatively nascent field, SNBC interventions typically address social norms that make corruption acceptable or expected, and attempt to influence behaviours away from corrupt practices. 
 The guidance is based on lessons learned from a largely successful pilot project in Tanzania that targeted social norms fuelling bribery ("gift giving") in health facilities and attempted to change the behaviours of both health care providers and users away from exchanging gifts. Survey results showed a 14–44% decrease in gift-giving intentions, attitudes and positive beliefs among hospital users following the pilot intervention.

Scaling Up Citizen Workshops in Public Libraries to Disseminate and Discuss Primary Care Research Results: Quasi-Experimental Study.

BackgroundLittle is known about engaging patients and stakeholders in the process of scaling up effective knowledge translation interventions targeting the public.ObjectiveUsing an integrated knowledge translation approach, we aimed to scale up and evaluate an effective pilot program to disseminate research results in public libraries.MethodsWe conducted a scaling-up study targeting the public. On the basis of our successful pilot project, we codeveloped and implemented a large-scale program of free citizen workshops in public libraries, in a close research partnership with stakeholders and patient representatives. Citizen workshops, each facilitated by 1 participating physician and 1 science communicator, consisted of a 45-minute computer-assisted presentation and a 45-minute open exchange. The intervention outcome was knowledge gained. The scale-up outcomes were satisfaction, appropriateness, coverage, and costs. An evaluation questionnaire was used to collect data of interest. Both quantitative and qualitative analyses were performed.ResultsThe workshop theme chosen by the patient and stakeholder representatives was the high prevalence of medication overuse among people aged ≥65 years. From April to May 2019, 26 workshops were conducted in 25 public libraries reaching 362 people. The mean age of participants was 64.8 (SD 12.5) years. In total, 18 participating physicians and 6 science communicators facilitated the workshops. Participants reported significant knowledge gain (mean difference 2.1, 95% CI 2.0-2.2; P<.001). The median score for overall public satisfaction was 9 out of 10 (IQR 8-10). The public participants globally rated the workshops as having a high level of appropriateness. Coverage was 92% (25/27) of the total number of public libraries targeted. Costs were CAD $6051.84 (US $4519.69) for workshop design and CAD $22,935.41 (US $17,128.85) for scaling them up.ConclusionsThis project successfully established a large-scale and successful implementation science or knowledge translation bridge among researchers, clinicians, and citizens via public libraries. This study provides a model for a dissemination practice that benefits the public by both engaging them in the dissemination process and targeting them directly.

Precision Medicine Approach to Alzheimer's Disease: Successful Pilot Project.

Background:Effective therapeutics for Alzheimer’s disease are needed. However, previous clinical trials have pre-determined a single treatment modality, such as a drug candidate or therapeutic procedure, which may be unrelated to the primary drivers of the neurodegenerative process. Therefore, increasing data set size to include the potential contributors to cognitive decline for each patient, and addressing the identified potential contributors, may represent a more effective strategy.Objective:To determine whether a precision medicine approach to Alzheimer’s disease and mild cognitive impairment is effective enough in a proof-of-concept trial to warrant a larger, randomized, controlled clinical trial.Methods:Twenty-five patients with dementia or mild cognitive impairment, with Montreal Cognitive Assessment (MoCA) scores of 19 or higher, were evaluated for markers of inflammation, chronic infection, dysbiosis, insulin resistance, protein glycation, vascular disease, nocturnal hypoxemia, hormone insufficiency or dysregulation, nutrient deficiency, toxin or toxicant exposure, and other biochemical parameters associated with cognitive decline. Brain magnetic resonance imaging with volumetrics was performed at baseline and study conclusion. Patients were treated for nine months with a personalized, precision medicine protocol, and cognition was assessed at t = 0, 3, 6, and 9 months.Results:All outcome measures revealed improvement: statistically significant improvement in MoCA scores, CNS Vital Signs Neurocognitive Index, and Alzheimer’s Questionnaire Change score were documented. No serious adverse events were recorded. MRI volumetrics also improved.Conclusion:Based on the cognitive improvements observed in this study, a larger, randomized, controlled trial of the precision medicine therapeutic approach described herein is warranted.

“Ah Lai’s Crossing” – Malaysia’s first artificial road canopy bridge to facilitate safer arboreal wildlife crossings

Abstract Roads negatively affect wildlife populations globally, causing habitat fragmentation that can facilitate mortality by access to forest for poaching and wildlife collisions with vehicles. Primate habitats are vulnerable to fragmentation exposing them to anthropogenic threats. Endangered dusky langurs (Trachypithecus obscurus) in Malaysia are often found in urban areas and have been frequent victims of vehicle collisions. To mitigate the hazardous impact of a busy asphalt road on wild dusky langurs in Teluk Bahang, Penang, we built the first artificial road canopy bridge in Malaysia in February 2019 to assist arboreal wildlife to move between habitat fragments more safely. Four days after the bridge construction, the first long-tailed macaque (Macaca fascicularis) was recorded via a camera trap crossing the bridge, and plantain squirrels (Callosciurus notatus) and dusky langurs first crossed one and eight months later, respectively. The bridge was upgraded from a single-firehose rope to a double-rope in August 2020 to assess the effectiveness of the two designs. In total, 2128 animal crossings comprising three mammal species were recorded between March 2019 and May 2021. Plantain squirrels crossed the bridge most frequently with 2075 crossings, long-tailed macaques crossed 32 times, and dusky langurs 21 times. There was a significant difference in bridge use according to species between the two designs (Chi-Square, p &lt; 0.001). Since the construction of the bridge, the rate of previously frequently reported primate roadkills near the bridge location has dropped to zero. This successful pilot project has spurred the efforts to implement more canopy bridges country-wide to support primate conservation efforts and better protect arboreal wildlife from the negative impacts of linear infrastructure.

Implementing Home Health Monitoring for Chronic Disease Management in British Columbia

Home health monitoring (HHM) enables health care providers to monitor a patient’s health status remotely using digital technology. In 2013, the government of British Columbia (BC) partnered with a telecommunications provider and invested $52M in HHM programs for patients with complex care needs. Telehealth has evolved over the past two decades to improve access and delivery of care, with HHM emerging as a strategy for proactive chronic disease management. The goals of the provincial HHM initiative are to reduce acute care utilization, reduce health system costs and improve patient self-management of chronic conditions. Following a series of successful pilot projects, funding for expanded HHM initiatives was made available through a provincial Strategic Investment Fund and continued support from Canada Health Infoway. Evaluations of several HHM programs demonstrated high levels of patient satisfaction, reduced emergency department visits, health system cost savings, and improved patient self-care and quality of life. Optimizing referral rates and expanding HHM programs to include a wider range of chronic conditions are opportunities for future growth, with sustainability dependent on securing long-term funding sources.

The Fourth Industrial Revolution and the introduction of the Industry 4.0 innova-tions create a unique opportunity for enterprises to increase productivity, opera-tional efficiency and rational use of resources, scale up successful pilot projects and acquire qualitatively new expertise for employees. The current stage of mod-ernisation of production can provide a significant impetus for economic growth and is a priority for governments and business leaders. Therefore, it is important to study project management strategies in the context of digital transformation in order to implement them in the enterprises of Kazakhstan. The purpose of the pa-per is to investigate the features and specifics of project management in modern conditions and search for methods of sustainable development of the state. The leading research method was the analysis, which made it possible to identify global trends in the field of project management for the implementation of innovations, as well as to assess the current state of enterprises in Kazakhstan after the Gov-ernment of the Republic of Kazakhstan approved the State Program "Digital Ka-zakhstan". As a result of the study, the features of project management for creat-ing a platform for the implementation of the Industry 4.0 for sustainable devel-opment of the state were identified. The authors concluded that the knowledge in-tensive business services need a deep understanding of user preferences and needs. Data-driven business models are the entry point for the digital transformation of enterprises.

Business Opportunities in Power Supply with Gas Engines in the Health Sector

In the research, we have developed an operational concept based on new energy generation devices, which, in addition to providing heat and electricity, guarantees uninterrupted supply locally and the sale of system services off-site. Based on international experience in the sector, the overall business concept needs to be developed for successful pilot projects. The trends in the technical-economic and regulatory environment examined show that the direction of change, with uncertain outcomes today, is unanimous: they all support the market penetration and the continued profitability of smallscale power plant devices suitable for secondary regulation. An important task is to identify unique elements in the value propositions. The aim of the research is to identify which of the factors such as realized price differential, heat-side revenues, generation utilization, optimized plant operation, scaling, etc. have an impact on the investment, operation, as well as the overall business model and profitability, and to what extent.
