Influence of bicortical techniques in internal connection placed in premaxillary area by 3D finite element analysis

The aim of study was to evaluate the stress distribution in implant-supported prostheses and peri-implant bone using internal hexagon (IH) implants in the premaxillary area, varying surgical techniques (conventional, bicortical and bicortical in association with nasal floor elevation), and loading directions (0°, 30° and 60°) by three-dimensional (3D) finite element analysis. Three models were designed with Invesalius, Rhinoceros 3D and Solidworks software. Each model contained a bone block of the premaxillary area including an implant (IH, Ø4 × 10 mm) supporting a metal-ceramic crown. 178 N was applied in different inclinations (0°, 30°, 60°). The results were analyzed by von Mises, maximum principal stress, microstrain and displacement maps including ANOVA statistical test for some situations. Von Mises maps of implant, screws and abutment showed increase of stress concentration as increased loading inclination. Bicortical techniques showed reduction in implant apical area and in the head of fixation screws. Bicortical techniques showed slight increase stress in cortical bone in the maximum principal stress and microstrain maps under 60° loading. No differences in bone tissue regarding surgical techniques were observed. As conclusion, non-axial loads increased stress concentration in all maps. Bicortical techniques showed lower stress for implant and screw; however, there was slightly higher stress on cortical bone only under loads of higher inclinations (60°).

Stress distribution in implant-supported prostheses using different connection systems and cantilever lengths: digital photoelasticity

Photoelastic analysis was used to evaluate the biomechanical behaviour of implant-supported, double-screwed crowns with different connection systems and cantilever lengths. Three models were made in PL-2 photoelastic resin and divided into six groups, on the basis of the implant connection system (external hexagon [EH] or Morse taper [MT]), type of abutment (Mini Pilar [Neodent, Curitiba, Paraná, Brazil] or “UCLA”) and number of crowns in the cantilever (one or two). The implant-prosthesis unit was placed in a circular polariscope. Occlusal surfaces of the crowns were subjected to 100-N loads in the axial and oblique (45°) directions in a universal testing machine (EMIC). Generated stresses were recorded and analysed qualitatively in a graphics program (Adobe Photoshop). Under axial loading, all of the groups had similar numbers of fringes, which were increased when the crowns were subjected to oblique loading. The highest number of fringes was found during oblique loading in the EH + Mini Pilar group. In conclusion, although the type of implant connection system did not have a direct influence on the stress distribution for axial loading, the cantilever length did have a direct influence on stress distribution. Models with two crowns in the cantilever showed more stress, with a greater concentration of force on the cervical part of the implant.

Photoelastic stress analysis of splinted and unitary implant-supported prostheses

Stress distribution in implant-supported prosthesis with external and internal implant-abutment connections

Objective. This study aimed to investigate the stress distribution in screwed implant-supported prostheses with different implant-abutment connections by using a photoelastic analysis. Materials and methods. Four photoelastic models were fabricated in PL-2 resin and divided according to the implant-abutment connection (external hexagon (EH) and Morse taper (MT) implants (3.75 × 11.5 mm)) and the number crowns (single and 3-unit piece). Models were positioned in a circular polariscope and 100-N axial and oblique (45) loading were applied in the occlusal surface of the crowns by using a universal testing machine. The stresses were photographically recorded and qualitatively analyzed using software (Adobe Photoshop). Results. Under axial loading, the MT implants exhibited a lower number of fringes for single-unit crowns than EH implants, whereas for a 3-unit piece the MT implants showed a higher number of fringes vs EH implants. The oblique loading increased the number of fringes for all groups. Conclusion. In conclusion, the MT implant-abutment connection reduced the amount of stress in single-unit crowns, for 3-unit piece crowns the amount of stress was lower using an external hexagon connection. The stress pattern was similar for all groups. Oblique loading promoted a higher stress concentration than axial loading.

Influence of Cusp Inclination on Stress Distribution in Implant‐Supported Prostheses. A Three‐Dimensional Finite Element Analysis

The aim of this study was to assess the influence of cusp inclination on stress distribution in implant-supported prostheses by 3D finite element method. Three-dimensional models were created to simulate a mandibular bone section with an implant (3.75 mm diameter x 10 mm length) and crown by means of a 3D scanner and 3D CAD software. A screw-retained single crown was simulated using three cusp inclinations (10 degrees, 20 degrees, 30 degrees). The 3D models (model 10d, model 20d, and model 30d) were transferred to the finite element program NeiNastran 9.0 to generate a mesh and perform the stress analysis. An oblique load of 200 N was applied on the internal vestibular face of the metal ceramic crown. The results were visualized by means of von Mises stress maps. Maximum stress concentration was located at the point of application. The implant showed higher stress values in model 30d (160.68 MPa). Cortical bone showed higher stress values in model 10d (28.23 MPa). Stresses on the implant and implant/abutment interface increased with increasing cusp inclination, and stresses on the cortical bone decreased with increasing cusp inclination.
