The Plumbline: A Novel Radiographic Tool for Assessment of Metatarsus Adductus with Hallux Valgus

Category: Bunion; Midfoot/Forefoot Introduction/Purpose: Metatarsus adductus (MTA) is a complex multiplanar deformity of the foot which has a 30% reported approximate radiographic incidence in patients with symptomatic hallux valgus (HV). Various radiographic angles have been proposed for the assessment of MTA, but these measurements can be complex with a broad range of interrater reliability. We propose a simplistic radiographic assessment method termed the ‘plumbline’ to identify the presence of MTA and to ascertain if enough physical space is available to align the first metatarsal to the longitudinal foot axis via an isolated 1st ray procedure, or if necessary, through a combined approach which also addresses the lesser metatarsals. This method provides a visual reference to determine if a physical blockade to 1st ray correction is present. Methods: The plumbline is assessed on standard weight-bearing AP radiographs. Neutral foot positioning is key as a pronated or supinated clinical position of the foot will impact the alignment of this and other radiographic angular relationships. A transverse midfoot axis line is drawn by identifying and linking two individual points marked at the calcaneocuboid joint and the talonavicular joint. A third point is marked at the distal lateral apex of the medial cuneiform at the first tarsometatarsal joint. A 90° line is subtended from the transverse midfoot axis line, crosses the third point, and extends distally to the 2nd metatarsal head. A positive radiographic plumbline occurs when this line intersects the 2nd metatarsal indicating the need to correct the adducted lesser metatarsal position prior to addressing HV. A negative plumbline remains tangential to the 2nd metatarsal and indicates an isolated HV correction is possible. Results: The metatarsus adductus angle (MAA) measured via Sgarlato's method (SA), and the plumbline were recorded in patients (N=20) scheduled for HV surgery. Initially the cohort was subdivided into subjects deemed to have a normal MAA (SA < 15°; N=10) and an abnormal MAA (SA>15°; N=10). A mean pre-operative MAA measurement of 8.1° (SD 2.3; range 5, 12.1) and 26.5° (SD 4.5; range 19.2, 33.7) were found in these respective subsets. All subjects (100%) with an MAA < 15° (N=10) were found to have a negative plumbline radiographically and subjects (100%) with a MAA >15° (N=10) presented with a positive plumbline. The plumbline findings identified when an isolated HV correction would have been possible or impeded based upon 2nd metatarsal position, thus driving the surgical decision making algorithm. Conclusion: The plumbline is used to determine when concomitant MTA and HV correction is beneficial without the need to measure other more traditional radiographic angular relationships. It is a quick and simple method of evaluating the proximity of the metatarsal segments to determine if the first metatarsal is amenable to reduction in isolation or if correction will be limited due to a 2nd metatarsal bone blockade. The visual simplicity of the measurement can also aid in patient education and surgeon satisfaction when describing the surgical approach to this complex three-dimensional anatomic pathology.

Predicting valgus malalignment after mobile-bearing UKA using a new method: the arithmetic HKA of the arthritic knee.

Valgus malalignment is one of the most common reasons for the progression of osteoarthritis in the lateral compartment of the knee after mobile-bearing unicompartmental knee arthroplasty (UKA). The arithmetic hip-knee-ankle angle (aHKA) of the Coronal Plane Alignment of the Knee (CPAK) classification could reflect the constitutional alignment of the arthritic knee. The purpose of this study was to observe the relationship between the aHKA and valgus malalignment after mobile-bearing UKA. This retrospective study was conducted using 200 knees undergoing UKA from January 1, 2019, to August 1, 2022. These radiographic signs, including preoperative hip-knee-ankle angle (HKA), mechanical proximal tibial angle (MPTA), mechanical lateral distal femoral angle (LDFA), and postoperative HKA, were measured using standardized weight-bearing long-leg radiographs. Patients with postoperative HKA > 180° and postoperative HKA ≤ 180° were classified as the valgus group and non-valgus group, respectively. The aHKA was calculated as 180° + MPTA - LDFA in this study, which had the same meaning as that (aHKA = MPTA - LDFA) in the CPAK classification. The Spearman correlation analysis, the Mann-Whitney U test, the chi-square test, the Fisher's exact test, and multiple logistic regression were used in the study. Of the 200 knees included in our study, 28 knees were classified as the valgus group, while 172 knees were in the non-valgus group. The mean ± standard deviation (SD) of aHKA (all groups) was 177.04 ± 2.58°. In the valgus group, 11 knees (39.3%) had a value of aHKA > 180°, while 17 knees (60.7%) had a value of aHKA ≤ 180°. In the non-valgus group, 12 knees (7.0%) had a value of aHKA > 180°, while 160 knees (93.0%) had a value of aHKA ≤ 180°. In Spearman correlation analysis, aHKA was positively correlated with postoperative HKA (r = 0.693, p < 0.001). In univariate analysis, preoperative HKA (p < 0.001), LDFA (p = 0.02), MPTA (p < 0.001), and aHKA (p < 0.001) showed significant differences between the valgus and non-valgus groups. Variables with p < 0.1 in univariate analysis were further analyzed using multiple logistic regression analysis, and the variable-aHKA (> 180° vs ≤ 180°, odds ratio (OR) = 5.899, 95% confidence interval (CI) = 1.213 to 28.686, and p = 0.028) was expressed as the risk factor of postoperative valgus malalignment. The aHKA is correlated with the postoperative alignment of mobile-bearing UKA and a high aHKA (> 180°) will increase the risk of postoperative valgus malalignment. Therefore, mobile-bearing UKA should be performed with caution in patients with preoperative aHKA > 180°.

Incidence and predictors of valgus tibiotalar tilt after progressive collapsing foot deformity reconstruction using subtalar fusion with concomitant procedures.

Development of valgus tibiotalar tilt is a significant complication after subtalar fusion for progressive collapsing foot deformity (PCFD) correction. However, its incidence and etiologic factors have not been extensively studied. The purpose of this study was to define the incidence of valgus tibiotalar tilt after subtalar fusion for PCFD reconstruction, and to determine predictors of this complication. This study included 59 patients who underwent PCFD reconstruction with subtalar fusion. Patients with tibiotalar tilt prior to surgery were excluded. On standard weightbearing radiographs, the talonavicular coverage angle, talo-1st metatarsal angle, calcaneal pitch, hindfoot moment arm (HMA), and medial distal tibial angle were measured. Weightbearing computed tomography (WBCT) was used to determine the presence of lateral bony impingement. A radiologist evaluated the superficial and deep deltoid ligaments using magnetic resonance imaging (MRI). Univariate regression analysis was used to identify the factors associated with development of postoperative valgus tibiotalar tilt, defined as tilt > 2 degrees. Seventeen patients (28.8%) developed postoperative valgus tibiotalar tilt at a mean of 7.7 (range 2-31) months. Eight (47.1%) of these patients developed valgus tibiotalar tilt within 3months. Univariate logistic regression demonstrated association between preoperative HMA and postoperative valgus tibiotalar tilt (odds ratio 1.06, P = 0.026), with a 6% increase in risk per millimeter of increased HMA. Deltoid ligament status and concomitant procedures on other joints did not correlate with postoperative valgus tilt. Our findings indicate that surgeons should be cognizant of patients with a greater degree of preoperative hindfoot valgus and their propensity to develop a valgus ankle deformity. Additionally, our relatively high incidence of valgus tibiotalar tilt suggests that weightbearing ankle radiographs should be included in the initial and subsequent follow-up of PCFD patients with hindfoot valgus treated with subtalar fusion.

Radiographic Cut-Off Values for Predicting Lateral Bony Impingement in Progressive Collapsing Foot Deformity: A Correlation Analysis with Weightbearing Computed Tomography

Category: Hindfoot; Midfoot/Forefoot Introduction/Purpose: Lateral bony impingement, one of the main causes of lateral foot pain in progressive collapsing foot deformity (PCFD), reflects disease severity and may affect surgical planning. Weightbearing computed tomography (WBCT) is known to provide better sensitivity in detecting impingement over simple radiographs, however, many Orthopaedic centers have not yet acquired WBCT imaging. This study aimed to (1) investigate the correlation of common radiographic parameters measured on standard weightbearing radiographs with talocalcaneal and calcaneofibular distance assessed with WBCT and (2) establish cut-off values for radiographic measurements on standard radiographs to detect lateral bony impingement as identified on WBCT. Methods: Ninety-one adult patients (mean age and standard deviation, 54.1 +- 17.2 years) treated for PCFD with standard preoperative radiographs and WBCT within 6 months of each other were retrospectively identified. Patients with previous ipsilateral foot and ankle surgery or asymmetric ankle arthritis (talar tilt &gt; 2 degrees) were excluded. The talocalcaneal distance at the sinus tarsi and subfibular calcaneofibular distance were measured in multiplanar reconstructed WBCT images. Bony impingement was defined as direct contact between the structures. The relationships between WBCT measurements and four common parameters (Talonavicular coverage angle [TNC], Talo-1st metatarsal angle, Calcaneal pitch, and hindfoot moment arm [HMA]) in standard radiographs were assessed with Pearson correlations. Receiver operating characteristic (ROC) curve analysis was performed to determine the ability of radiographic parameter thresholds to predict sinus tarsi or calcaneofibular bony impingement, and the area under curve (AUC), sensitivity, specificity, negative and positive predictive value were calculated. Results: Talocalcaneal distance narrowing at the sinus tarsi on WBCT was strongly correlated with TNC (r = 0.64, p &lt; 0.001), and the calcaneofibular distance narrowing on WBCT correlated with the HMA moderately yet best among the parameters (r=0.55, p&lt;0.001). Interrater and intrarater reliability of WBCT measurements was excellent. TNC (AUC=0.837, 95% confidence interval [CI], 0.745-0.906) and HMA (AUC=0.959, 95%CI, 0.895-0.989) provided the best predictive ability for sinus tarsi and calcaneofibular bony impingement, respectively (Figure 1). The cut-off value for TNC for predicting sinus tarsi bony impingement was 25.4 degrees, with a sensitivity of 80.4% and a specificity of 72.5%. The cut-off value for HMA for predicting calcaneofibular bony impingement was 25.4mm, with a sensitivity of 100% and a specificity of 81.2% (Table 1). Conclusion: This study provides evidence that common radiographic parameters in standard radiographs can be potentially used to detect lateral bony impingement in PCFD. Narrowing of talocalcaneal distance at the sinus tarsi was best correlated with abduction deformity of the foot, and the narrowing of calcaneofibular distance was best correlated with valgus hindfoot deformity. TNC and HMA on standard radiographs may be used for detecting sinus tarsi and calcaneofibular bony impingement, respectively.

Incidence and Predictors of Valgus Tibiotalar Tilt after Progressive Collapsing Foot Deformity Reconstruction using Subtalar Fusion

Despite successful correction through subtalar fusion, the development of valgus tibiotalar tilt at the ankle joint has been documented as a significant complication. This can be detrimental since abnormal forces on the ankle joint can induce cartilage wear and subsequent ankle arthritis over time. However, the incidence of valgus tilt of the ankle following subtalar fusion for PCFD reconstruction has not been extensively studied, and little is known about its etiological factors. The purpose of this study was to define the incidence of valgus tibiotalar tilt after subtalar fusion for PCFD reconstruction, and to determine if any demographic and radiographic parameters predict which patients develop this complication. Methods: This study reviewed 59 patients (median age: 59 years) who underwent PCFD reconstruction with subtalar fusion and had pre-and postoperative weightbearing anteroposterior radiographs of the ankle in the registry. Patients with a tibiotalar tilt prior to surgery were excluded. On standard weightbearing radiographs, the talonavicular coverage angle, talo-1st metatarsal angle, calcaneal pitch, and hindfoot moment arm (HMA), and medial distal tibial angle were measured. Weightbearing computed tomography (WBCT) was used to determine the presence of preoperative sinus tarsi or calcaneofibular bony impingement. A radiologist evaluated the superficial and deep deltoid ligaments using magnetic resonance imaging (MRI). Postoperative valgus tibiotalar tilt was defined as tilt &gt;2 degrees. Univariate regression analysis was used to identify the factors associated with development of postoperative valgus tibiotalar tilt. These factors included age, gender, BMI, as well as concomitant procedures, radiographic parameters, lateral bony impingement on WBCT, and deltoid ligament status on MRI. Results: A total of 17 patients (28.8%) developed postoperative valgus tibiotalar tilt at a mean of 7.7 (range, 2-31) months. Eight (47.1%) of the 17 patients developed valgus tibiotalar tilt (mean talar tilt of 5.3 degrees, range: 4-8) on postoperative weightbearing within 3 months. In bivariate analysis, male gender and preoperative HMA were significantly associated with development of valgus tibiotalar tilt (Table 1). Univariate logistic regression demonstrated that preoperative HMA was associated with postoperative valgus tibiotalar tilt (odds ratio 1.06, P = .026), with a 6% increase in risk per millimeter of increased HMA. Deltoid ligament status on MRI and concomitant procedures on other joints including fusion did not correlate with postoperative valgus tibiotalar tilt. Conclusion: The incidence of valgus tibiotalar tilt after subtalar fusion in PCFD reconstruction was 28.8% in this study. Preoperative valgus hindfoot alignment rather than the condition of the deltoid ligament was a significant predictor of postoperative valgus tibiotalar tilt. Our findings indicate that surgeons should be cognizant of patients with a greater degree of hindfoot valgus and of their propensity to develop a valgus ankle deformity. Additionally, our relatively high incidence of valgus tibiotalar tilt suggests that weightbearing ankle radiographs should be included in the initial and subsequent follow-up of PCFD patients with hindfoot valgus treated with subtalar fusions.

Mid-Term Functional Outcomes Following Reoperation after Total Ankle Arthroplasty: A Case- Control Study

Category: Ankle Arthritis; Ankle Introduction/Purpose: A relatively frequent need for secondary surgery is an important consideration when treating end-stage ankle arthritis with total ankle arthroplasty (TAA). Despite a significant reported rate of non-revision reoperation after TAA, it is uncertain if these subsequent procedures have an impact on the longer term outcomes. It is imperative to know whether patients undergoing reoperation after TAA would have comparable functional ability compared to those with uncomplicated TAA in order to optimize patient outcomes when faced with complications after TAA. This study aimed to compare minimum five-year functional outcomes of patients who had undergone reoperation after TAA to those who did not require reoperation. Methods: Seventy-six ankles (74 patients) who underwent TAA with the Salto Talaris fixed bearing system between 2007 to 2014 were allocated into two groups based on presence (n=16) or absence of reoperation (n=60). Reoperations included all secondary procedures after index TAA, excluding revision surgeries requiring removal or exchange of the original prosthesis. The foot and ankle outcome score (FAOS) at preoperative and minimum five-year follow-up was compared between the groups. Additionally, improvements in FAOS subscales were compared. Preoperative deformity, postoperative implant alignment, number of periprosthetic cysts, and subsidence between groups were compared using standard weightbearing radiographs. A subgroup analysis was performed to compare clinical and radiographic outcomes of gutter impingement patients to those of uncomplicated group. Results: The most common cause of reoperation was gutter impingement (n=11), followed by stress fracture of the medial malleolus (n=2), tarsal tunnel syndrome (n=1), periprosthetic cyst (n=1), and infection (n=1). In comparison to the non- reoperation group, all FAOS subscales except for the Sports and Recreational Activities subscale were significantly lower in the reoperation group at final follow-up. The reoperation group exhibited significantly less improvement in the Pain, Symptoms, and Quality of Life subscales at final follow-up (p&lt;0.05, Figure 1). There were no statistical differences in the radiographic parameters between both groups. In a subgroup analysis, all FAOS subscales at final follow-up were significantly lower in the gutter debridement patient group. There was significantly less improvement in the Pain, Symptoms, and QoL subscales in the gutter debridement patient group compared to the non-reoperation group (p&lt;0.05 in all three subscales). Postoperative radiograph showed no differences between groups. Conclusion: Patients who underwent reoperation after TAA demonstrated inferior functional outcomes at mid-term follow-up. At the time of the primary TAA, an emphasis should be placed on carefully examining and preventing possible causes of reoperation to achieve favorable patient outcomes. Additionally, when faced with performing a reoperation following TAA, a thorough evaluation to determine the underlying cause of the reoperation should be performed to guide treatment.

Angle of BRINK - a new way to measure Haglund's deformity.

Haglund's deformity, an abnormality of the postero-superior corner of the calcaneum, is a common, critically debated cause of posterior heel pain. Several radiological indices such as Fowler-Philip angle, Ruch pitch, Chauveaux-Liet angle, calcaneal pitch angle, parallel pitch lines, and X-Y ratio have been described to measure this deformity. However, most of these lack specificity and have variable intra- and inter-observer reliability. The study aims to describe a new radiological "angle of BRINK"(Botchu-Reilly-Iyengar-Nischal-Kakarala)to measure Haglund's deformity. We performed a retrospective cohort analysis, assessing 20 weight-bearing lateral ankle radiographs of patients with Haglund's deformity (Haglund's cohort) and 100 radiographs without the deformity (normal cohort). Demographic details and angle of BRINK to measure Haglund's deformity were measured for each patient. Statistical analysis was performed using t-test and inter-observer reliability was calculated using kappa coefficient. The mean angle of BRINK to measure Haglund's deformity in the normal cohort was 20.04° (SD 4.88), and in the Haglund's cohort was 25.1° (SD 3.3). This was statistically significant with a p-value of less than 0.0001. There was excellent intra- and inter-observer reliability with kappa value of 0.8. Our proposed radiological angle of BRINK to measure Haglund's deformity is simple and easy to calculate on standard weight-bearing radiographs. Contrary to the traditional measurements used to estimate the deformity, it has shown a good intra- and inter-observer reliability and can support surgical decision-making process for management of patients with symptomatic Haglund's deformity.

Radiographic Cutoff Values for Predicting Lateral Bony Impingement in Progressive Collapsing Foot Deformity.

The relationships between WBCT measurements and 4 common parameters (talonavicular coverage angle [TNC], talo-first metatarsal angle, calcaneal pitch, and hindfoot moment arm [HMA]) in standard radiographs were assessed with Pearson correlations. Receiver operating characteristic curve analysis evaluated the ability of radiographic cutoffs to detect sinus tarsi or calcaneofibular bony impingement, and the area under curve (AUC), sensitivity, specificity, negative and positive predictive value (PPV) were calculated. Talocalcaneal distance narrowing at the sinus tarsi strongly correlated with TNC (r = 0.64, P < .001), and the calcaneofibular distance narrowing correlated with the HMA moderately yet best among the parameters (r = 0.55, P < .001). TNC (AUC = 0.837, 95% CI 0.745-0.906) and HMA (AUC=0.959, 95% CI 0.895-0.989) provided the best predictive ability for sinus tarsi and calcaneofibular bony impingement, respectively. A TNC threshold of 41.2 degrees had a 100% PPV for predicting sinus tarsi impingement, whereas an HMA threshold of 38.1 mm had a 100% PPV for calcaneofibular impingement. This study provides evidence that TNC and HMA measurements made on standing radiographs could be used to indicate potential lateral bony impingement in PCFD. Narrowing of talocalcaneal distance best correlated with abduction deformity of the foot, and the narrowing of calcaneofibular distance was best correlated with valgus hindfoot deformity. The Influence of Metatarsus Adductus Angle on Fifth Metatarsal Jones Fractures

Category:Trauma; Midfoot/ForefootIntroduction/Purpose:Previous studies have reported increasing metatarsus adductus angle (MAA) to be associated with delayed union and refracture following intramedullary screw fixation of fifth metatarsal Jones fractures. The purpose of this study was to determine the influence of MAA on postoperative course following intramedullary screw fixation of Jones fractures. We also sought to identify associations between elevated MAA and both fracture and patient characteristics.Methods:We performed a retrospective review of all Jones fractures treated with primary intramedullary screw fixation by 4 foot and ankle fellowship-trained orthopaedic surgeons at a single institution from 1995 through 2015. Exclusion criteria included concomitant foot/ankle procedures and revision surgery. Charts were reviewed for patient and injury characteristics, implant, and postoperative course. Radiographs were examined for fracture classification, radiographic union, and MAA. MAA calculations were performed on standard weight-bearing digital radiographs using the traditional method, with the 5th metarso-cuboid joint as a reference. Based on severity of MAA, comparative and correlation analyses were performed. Primary outcomes were the number of surgical failures (defined as delayed union, nonunion, or refracture) and time to radiographic union, weight bearing, and pain resolution. Data was analysed using independent T test, one-way ANOVA, chi-square, and correlation analyses with significance defined as p<0.05.Results:59 feet in 58 patients were identified with a mean age of 30 years and average follow-up of 9.6 months. The pooled union rate was 96.6%. The mean MAA was 20.9 (SD 6.7). Eleven feet had MAA<15, 18 mild (MAA 15-20), 12 moderate (MAA 20- 25), 18 severe (MAA>25), and 1 unknown. 11 patients had failures (18.6%), which consisted of 7 delayed unions (11.9%), 2 delayed unions (3.4%), and 3 refractures (5.1%). Compared to the uncomplicated unions, there was no significant difference in mean MAA (24.3 vs. 20.1, p=0.16). Three of the 4 non-union or refracture patients had MAA>25. MAA was correlated with time to weight bearing (r=0.365, p=0.005), weight (r=0.503, p<0.001), BMI (r=0.280, p=0.03), and approached significance with age (r=0.230, p=0.082). No significant correlation was found with time to radiographic union.Conclusion:To our knowledge, this is the largest series investigating MAA in fifth metatarsal Jones fracture patients treated with intramedullary screw fixation. Our mean MAA is consistent with previous reports. We found an association between increased MAA and postoperative recovery time, given increased time to initiate weight bearing. We did not find significant associations with prolonged radiographic healing, age, nor failure. The prolonged time to weight bearing may reflect surgeon preference in patients with higher MAA. These results suggest that reported associations with MAA may not be as strong as previously thought.

Radiographic Analysis and Clinical Efficacy of Hindfoot Arthrodesis With Versus Without Cotton Osteotomy in Stage III Adult Acquired Flatfoot Deformity

Forefoot varus develops as a result of longstanding adult-acquired flatfoot deformity (AAFD). This occurs with varying degrees of deformity and flexibility. Residual forefoot varus following hindfoot realignment in AAFD can lead to lateral column loading and a persistent pronatory moment in efforts to reestablish contact between the forefoot and the ground. The Cotton osteotomy may serve as a reasonable adjunct procedure to help avoid complications and poor outcomes associated with residual forefoot varus in patients undergoing hindfoot arthrodesis for stage III AAFD. The aim of this study was to compare the radiographic outcomes in patients undergoing isolated hindfoot arthrodesis to patients undergoing hindfoot arthrodesis with adjunctive cotton osteotomy. We retrospectively reviewed 47 patients matched based upon age, sex, and comorbidities who underwent hindfoot reconstruction for the treatment of stage III AAFD between 2015 and 2019. A retrospective radiographic review was performed on standard weightbearing radiographs including anterior-posterior and lateral views preoperatively, postoperatively at the initiation of full weightbearing, and at final follow-up. Statistical analysis utilizing paired t test to calculate p values where <.05 was statistically significant. At final follow-up, radiographic measurements showed statistically significant differences in CAA, calcaneal inclination, talo-calcaneal, and talar tilt (p value <.05). The Cotton osteotomy group showed a quicker return to presurgical activity level and a decreased incident of tibiotalar valgus. Our study suggests that the Cotton osteotomy can address residual forefoot varus and potentially prevent further progression of ankle valgus in AAFD when used in combination with hindfoot arthrodesis.

Reconstruction of a missing or insufficient distal fibula in the setting of a total ankle replacement: The Milanese technique

BackgroundThe presence of an intact distal fibula is reported as a prerequisite when considering total ankle replacement and its lack is usually considered a contraindication. The purpose of the study was to describe the results of a surgical technique to reconstruct the distal fibula (Milanese fibular reconstruction technique) in the setting of a total ankle replacement and insufficient fibula. MethodsRetrospective analysis of 15 patients (15 ankles) who underwent rotational fibular reconstruction technique with minimum follow-up of 24 months. Patients were evaluated preoperatively and at the last follow-up with AOFAS Ankle and Hindfoot score, VAS pain scale, SF-12 and range of motion. Standard weightbearing radiographs were used for radiographic assessment. ResultsThe average of all clinical scores significantly improved (P<.001). The average radiographic parameters showed a neutral alignment. No cases of reabsorption of the new lateral malleolus were observed even in cases with complications that accounted for 2 superficial wound infection and 2 deep infections. ConclusionsThe Milanese fibular reconstruction technique can successfully reestablish a lateral strut and in the setting of an ankle prosthesis in patients with an insufficient fibula.

Dome-shaped high tibial osteotomy with semi-circular Ilizarov pin fixator: Mid- to long-term results of a novel technique

BackgroundThe aim of this study was to assess the clinical and radiological results of the dome-shaped high tibial osteotomy (HTO) which was fixed with a novel construct comprised of semi-circular Ilizarov frames and pins. MethodsThe patients with at least five years of follow-up were evaluated. One-hundred and thirty-two knees of 114 patients were included in the final analysis. The clinical evaluation included range of motion and Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) score. Anatomical femorotibial angle (aFTA), anatomical medial proximal tibial angle (aMPTA), tibial slope and Insall–Salvati ratio were calculated on standard weight-bearing radiographs. ResultsThe average aFTA was improved from 1.6° varus to 8.7° valgus (P < 0.001). The average WOMAC score (P < 0.001) and flexion value (P = 0.014) were improved at the latest follow-up (WOMAC: 17.2, flexion: 142.5) compared with the preoperative period (WOMAC: 59.6, flexion: 129.2). The sagittal radiological parameters were not significantly affected. The five-year survival was 96.2%, and 10-year survival was 83.3%. ConclusionsThe semi-circular Ilizarov pin construct provided satisfactory outcomes both clinically and radiologically at mid- to long-term follow-up.

Radiographic Assessment of Relative First Metatarsal Length Following Modified Lapidus Procedure

The modified Lapidus is a surgical procedure for managing moderate to severe hallux valgus, especially in the presence of first tarsometatarsal joint arthritis or hypermobility. It has good long-term results but reportedly can lead to transfer metatarsalgia due to inherent shortening of the first metatarsal. A retrospective analysis of all adult patients who underwent a modified Lapidus procedure during a 3-year period was performed. Clinical notes were evaluated to look for nonunion or any other complications related to the surgery. Pre- and postoperative standard weightbearing radiographs were used to establish the relative metatarsal length (RML), intermetatarsal angle (IMA), hallux valgus angle (HVA), and distal metatarsal articular angle (DMMA). A total of 69 modified Lapidus procedures were identified, with 32 included in the study. The mean pre- and postoperative RMLs were -0.8 and -4.9 mm, respectively. The average RML shortening due to the procedure was -4.1 (P < .0001). The mean pre- and postoperative IMAs were 15 and 5 degrees, respectively (P < .0001). The mean pre- and postoperative HVAs were 33 and 9 degrees, respectively (P < .0001). One patient reported transfer metatarsalgia, which was attributed to elevation of the first metatarsal. We found a statistically significant degree of shortening of the relative length of the first metatarsal without any clinically significant metatarsalgia. The low rate of transfer metatarsalgia following the modified Lapidus procedure could be attributed to the sagittal plane correction and stability obtained by performing a first tarsometatarsal fusion. Effectiveness of subtalar arthroereisis with endorthesis for pediatric flexible flat foot: a retrospective cross-sectional study with final follow up at skeletal maturity

BackgroundPediatric flexible flatfoot is sometimes asymptomatic but it can rarely cause physical impairment, pain, and difficulty walking. We evaluated the radiographic effectiveness of intervention of subtalar arthroereisis with endorthesis for pediatric flexible flatfoot with final follow-up at skeletal maturity. MethodsThis is a retrospective cross-sectional study. 56 consecutive patients (112 feet) who underwent surgical treatment with subtalar arthroereisis for pediatric flexible flatfoot (mean age at final follow-up 15.5±1.2 years, 39.3% female) were enrolled. All the radiographic studies were performed in the hospital. Radiographs (standard weight-bearing radiographs of the foot with anteroposterior and lateral view) were used to measure lateral talocalcaneal angle (LTC), calcaneal pitch angle (CP), Meary’s angle (MA), anteroposterior talonavicular angle (APTN), talonavicular uncoverage percent (TNU). Minimum follow-up was 18 months. Measures were assessed pre-operatively and at the final follow-up. Clinical and functional parameters were assessed at the final follow-up. ResultsChildren who underwent surgical treatment with subtalar arthroereisis for pediatric flexible flatfoot exhibited a statistically significant improvement in all radiographic measurement parameters at the last follow-up at skeletal maturity (all, p<0.004). Mean follow-up was 40.1±23.6months. Clinical parameters were not correlated with the foot radiographic parameters at follow up period. ConclusionsOur results suggest that endorthesis in pediatric flexible flatfoot was effective for improving radiographic parameters at skeletal maturity. The amount of the morphologic correction at the end of the skeletal growth should be expected mainly for lateral tarso-metatarsal alignment and talo-navicular congruency (MA, APTN, TNU). Metal artifact reduction MRI for total ankle replacement sagittal balance evaluation

BackgroundRestoration of anatomical relationship between talus and tibia is crucial for longevity of total ankle replacement (TAR). Weight-bearing (WB) radiographs are the standard for evaluating the sagittal balance alignment, but are prone to rotational misalignment and altered measurements. Metal artifact reduction sequence (MARS) MRI allows visualization of periprosthetic landmarks and alignment of the image plane to the true sagittal axis of the implant. The purpose of this study was to compare TAR sagittal balance measurements on MARS MRI and WB radiographs. MethodsTwenty-three subjects with TAR [10 men/13 women, age 60 (41–73) years; 13 (3–24) months post-op] underwent MARS MRI and standard lateral WB radiographs. Standardized MARS MR images were aligned to the sagittal talar component axis. Three observers performed sagittal balance alignment measurements twice in an independent, random and blinded fashion. Lateral Talar Station (LTS), tibial axis-to-talus (T-T) ratio and normalized tibial axis-to-lateral-process (T-L) distance were measured. Concordance correlation coefficients (CCC) and intraclass correlation coefficients (ICC) were used for statistical analysis. In addition, mixed effects linear models were employed to assess overall concordance of the two image types. ResultsThe intraobserver agreement was excellent for radiographic (CCC=0.96) and MRI (CCC=0.90–0.97) measurements. Interobserver agreements were good-to-excellent with overall slightly higher agreements for MRI (ICC=0.78–0.94) than radiography (ICC=0.78–0.90) measurements. The T-T ratios of radiographs and MRI showed a high degree of concordance, whereas LTS was significantly lower on MRI when compared with radiographs, and T-L distance showed notable disagreement between the two imaging types. ConclusionSagittal balance measurements performed on standardized weight-bearing radiographs and standardized MARS MRI demonstrate substantial correlation and similarity. Given its high intra and interobserver agreement, MARS MRI may be helpful for the evaluation of TAR sagittal balance.Level of evidence: Level II - Prospective Comparative Study.

Flecting Osteotomy of the Distal Tibia for Salvage of an Asymmetric Osteoarthritic Ankle Joint

Category: Ankle Arthritis Introduction/Purpose: Deformity of the distal tibia in the sagittal plane with increased posterior tilt of the articular surface (recurvatum deformity) results in altered biomechanics and high contact pressure in the anterior tibiotalar joint with consecutive osteoarthritis (OA). As the talus becomes anteriorly extruded out of ankle mortise, the distance between its center of rotation and longitudinal axis of tibia is typically seen to be increased. In an attempt to restore physiologic load of such misaligned ankles in the sagittal plane, we have started to use a correcting osteotomy of the distal tibia to realign the center of rotation of talus and tibial axis. The aim of this study was to analyze the radiological and clinical outcome in a consecutive series of patients. Methods: 39 patients (female, 12; male 27; age 47 [28 to 72, SD 10.6] years) were treated with a flecting osteotomy of the distal tibia for a symptomatic misalignment in the sagittal plane with the use of an anterior opening wedge osteotomy (n = 28), posterior closing wedge osteotomy (n = 9), or dome-shaped osteotomy from medially (n = 2). If necessary, simultaneous corrections in the frontal plane were performed to address additional valgus/varus deformities. Standard weight-bearing radiographs were used pre- and postoperatively to evaluate the tibial anterior surface angle (TAS), tibiotalar surface angle (TTS), tibial lateral surface angle (TLS), calcaneal pitch and talar offset ratio (TOR). A four-staged flecting score was used to classify the grade of OA of the tibiotalar joint in the sagittal plane, also taking the coronal joint congruency into account. Results: The cumulative survival rate of the joint was 77% (95% CI: 48-86%) after 3 years, with 9 patients needing a joint sacrificing procedure (total ankle arthroplasty, 7; ankle fusion, 2). In the remaining 30 patients, pain decreased 2.0 points on the VAS (p &lt;0.001), and the AOFAS hindfoot score improved by 17 points (p&lt;0.001). The ROM did not change significantly. Patient satisfaction with the outcome was good in 68% and moderate in 25%, 7% were not satisfied. The mean TLS increased by 6.6 (SD 5.84) degrees, the mean TOR decreased 0.239 (SD 0.1814). TAS, TTS and calcaneal pitch did not change significantly. Ten ankles (26%) showed an improvement, 22 (56%) no change and 7 (18%) a worsening in the flecting score. Conclusion: The flecting osteotomy of the distal tibia was found to be an effective method to restore the tibiotalar joint congruency through moving the tibia axis anteriorly to the center of rotation of the talus, and lengthening the lever arm of the Achilles tendon. Besides normalizing the joint reaction forces of the tibiotalar joint, the procedure was also found to be effective to stabilize the talus against anterior extrusion. However, with a failure rate of 23%, there is need for further studies to determine the indication and limitation of this procedure.

Hallux valgus correction using Scarf-Osteotomy leads to shortening of the first metatarsal

Category: Bunion Introduction/Purpose: In hallux valgus deformity less weight can be borne by the first ray which may lead to transfer metatarsalgia and lesser toe deformities. Depending on the exact configuration of the bone cuts during the scarf procedure, an iatrogenic shortening of the first metatarsal may occur which may diminish weightbearing ability of the first ray as well, causing transfer metatarsalgia. The aim of the present study was therefore to determine preoperative and postoperative changes in length of the first metatarsal by using different methods of measuring metatarsal length. Methods: A consecutive series of 118 feet in 106 patients (89% female, 11% male) was enrolled, who underwent correctional osteotomy (Scarf-with/without Akin-Osteotomy) from May 2015 to July 2017 at a single institution. Patients, who underwent additional shortening osteotomy of the metatarsals, were excluded. Average age at the time of surgery was 51 years (range, 14 to 83 years). Pre- and postoperative angle measurement of hallux valgus- and intermetatarsal angle was assessed at between six weeks and three months postoperatively on standardized weight-bearing radiographs in dorsoplantar plane. We also identified early complications in hallux valgus surgery. An assessment and comparison of different methods of measuring metatarsal length (length of first metatarsal, ratio first to second metatarsal, Coughlin method) postoperatively was performed to identify the amount of shortening with this technique. Results: Hallux valgus angle was statistically significant reduced by an average of 18.6° (28.3° preoperatively to 9.7° postoperatively, p &lt; 0.001), intermetatarsal angle by 7.7° (12.8° to 5.1°, p &lt; 0.001). Measuring the length of the first metatarsal, in all three methods a statistically significant reduction of the first metatarsal length could be detected. Mean absolute shortening of 1.8 mm was measured (p &lt; 0.001). The ratio of the first metatarsal to the second metatarsal averaged -0.03 (p = 0.02). The mean relative lengthening of the second metatarsal, using the method described by Coughlin, was 0.42 mm (from 4.51 to 4.89 mm, p &lt; 0.001) on average. Of those three methods, the Coughlin method showed the highest correlation. 6 minor complications were observed (5%). Conclusion: In the current study we could demonstrate a significant reduction of hallux valgus angle and intermetatarsal angle with hallux valgus correction using Scarf-/Akin-Osteotomy, with a low complication rate. However, statistically significant shortening of the first metatarsal could be detected as well. Further research is required to improve and establish a hallux valgus correction technique without shortening of the first metatarsal.

Realignment osteotomy in fibular malunion

Category: Ankle Arthritis Introduction/Purpose: The incidence of fibula fractures continues to increase. The exact incidence of distal fibula malunions after fibular reconstructions is not known, but incidence up to 33% is described in the literature. The most frequent malunions of the fibula are shortening and malrotation, resulting in the widening of the ankle mortise and talar instability. It has been demonstrated that substantial fibular displacement may substantially increase the contact pressures in the ankle joint. Therefore distal fibular malunion is a risk factor for development of posttraumatic ankle osteoarthritis. The objectives of this study were to (1) describe our treatment algorithm and surgical technique in patients with posttraumatic fibula malunions; (2) determine intra- and postoperative complications rates, (3) to describe mid-term clinical and radiological outcomes and quality of life. Methods: 21 consecutive patients (12 male, 9 female, mean age 42 years) with symptomatic fibular malunions were included into this prospective study. The initial injury was Weber B and C fracture in 7 and 14 ankles. The mean time between the injury and reconstructive surgery was 17.5 months. In all patients a z-shaped osteotomy of the fibula was performed to achieve the appropriate length/rotation of the fibula. Fixation was performed using a plate. If necessary, supramalleolar and inframalleolar deformities were corrected by supramalleolar tibial and calcaneal osteotomies, respectively. Prior to the osteotomies an anterior ankle arthroscopy was performed in all patients. All patients were evaluated pre- and postoperatively (mean follow-up 5.6 years). Radiological outcomes were assessed using standardized weight-bearing radiographs. Clinical outcomes were assessed using visual analogue scale (VAS), American Orthopaedic Foot and Ankle Society (AOFAS) hindfoot scale, SF-36 questionnaire. Results: There were no intraoperative complications. In two patients early wound healing problems were observed, and resolved with i.v. antibiotics. Osseous healing was observed in all ankles within 10 weeks after surgery. The length and rotation of the fibula was improved in all patients, according to Weber criteria. All patients experienced significant pain relief (VAS: 6.5 ± 1.1 to 1.1 ± 0.9, P &lt; 0.001) and functional improvement (AOFAS hindfoot scale: 47.1 ± 14.5 to 85.0 ± 7.5, P &lt; 0.001; ROM:37° ± 6° to 45° ± 5°, P &lt; 0.001). The SF-36 score also significantly increased in all 8 subgroups. In 11 patients hardware was removed due a discomfort after a mean of 11.7 months (range 7.2 – 22.8). Conclusion: A z-shaped osteotomy is an efficient and successful method to restore fibula length and rotation in patients with posttraumatic malunion. Our findings in this series of 19 ankles confirm that this realignment surgery results in significant pain relief and functional improvement.

Outcomes of Total Ankle Arthroplasty in Moderate and Severe Valgus Deformity

Category: Ankle Arthritis Introduction/Purpose: Among patients with end-stage ankle arthritis, total ankle arthroplasty (TAA) utilization has significantly increased in recent years, while ankle arthrodesis utilization has declined. Significant coronal plane deformity is frequently encountered in this patient population, and was previously considered a contraindication to TAA. However, the advent of newer fixed-bearing prostheses, coupled with improved surgical techniques and a better understanding of ligamentous balancing, have allowed surgeons to extend their indications for TAA with respect to deformity correction. Several authors have demonstrated good outcomes from TAA in patients with significant varus deformities, but not specifically in patients with valgus deformities. We aimed to determine the clinical, radiographic, and patient-reported outcomes of patients with moderate to severe valgus deformity who underwent TAA for end-stage ankle arthritis. Methods: Eighty patients with valgus deformities =10 degrees who underwent TAA were retrospectively reviewed. All surgeries were performed by one of three fellowship-trained orthopaedic foot and ankle surgeons with extensive experience in TAA. One of three prostheses were used: INBONE (Wright Medical Technology, Arlington, TN), Salto-Talaris (Integra, Plainsboro, NJ), or the Scandinavian Total Ankle Replacement (STAR; Stryker, Kalamazoo, MI). We assessed the coronal tibiotalar angle on standardized weightbearing radiographs preoperatively, at one year, and at final follow-up. The visual analog scale (VAS) for pain, Short Form (SF)-36 scale, American Orthopaedic Foot and Ankle Society (AOFAS) hindfoot-ankle scale, and Short Musculoskeletal Function Assessment (SMFA) scores were assessed preoperatively and at final follow-up. Subgroup analyses were performed to determine differences in outcome scores, deformity correction, and maintenance of alignment between patients with moderate (=10 degrees, &lt;20 degrees) and severe (&gt;20 degrees) preoperative deformity. Complication, reoperation, and revision rates were collected from chart review. Results: Mean preoperative valgus deformity was 15.5 ± 5.0 degrees, and was corrected to a mean 1.2 ± 2.6 degrees of valgus postoperatively (Figure; P&lt;.001). An associated flatfoot deformity was present in 33% of patients, 65% of whom required concomitant procedures to address associated deformity. The VAS, SF-36, AOFAS, and SMFA scores improved significantly postoperatively (P&lt;.001 for all), with no difference in amount of improvement between the moderate and severe deformity groups. Deformity correction was maintained at a mean 3.5 (range 2.0-5.9) years of follow-up, with no significant change in the mean tibiotalar angle between one year and final follow-up in either the moderate or severe deformity groups (P=.134 and P=.155, respectively). Reoperation and revision rates did not differ between the moderate and severe deformity groups. Conclusion: Correction of coronal alignment was achieved and maintained following TAA in patients with both moderate and severe preoperative valgus malalignment. Patients demonstrated significant improvement in patient-reported outcome scores regardless of amount of preoperative deformity. Additional procedures may be necessary at the time of TAA to balance the ankle and correct associated deformity in the foot.
