Assessment of salivary tumor necrosis factor-alpha level in the initial stages of treatment with fixed appliances and clear aligners.

To assess and compare the tumor necrosis factor-alpha (TNF-α) levels in saliva samples during the initial stages of orthodontic treatment with fixed orthodontic appliances (FAs) and clear aligners (CAs). This longitudinal study comprised 40 patients (22 males, 18 females, mean age 22 ± 7 years) who were categorized into two equal-sized groups. Group A comprised 20 patients treated with FA, and Group B comprised 20 patients treated with CA. Unstimulated saliva was collected before the intiation of treatment and then collected again after the placement of the FA/CA at 24 hrs, 7th day, and on the 21st day in both groups. TNF-α levels were determined through ELISA. The data were subjected to statistical analysis. For intragroup comparison of TNF-α at different time points, the Wilcoxon matched-pairs signed-rank test was used, and for intergroup comparison of FAs and CAs at different time points, the Mann-Whitney U test was used. TNF-α levels in the saliva increased significantly at 24 hours, followed by a decline on the 7th day and 21st day in both groups. Changes in TNF-α levels were significantly higher in the FA group than those in the CA group at different time points. This study showed that the salivary TNF-α levels increased significantly during the initial stages of FA and CA treatment at different time points. The mean salivary TNF-α level in both FA and CA groups increased significantly at 24 hours, followed by a decline on the 7th day and then on the 21st day. There was a significant difference between the FA and CA treatment, where the CAs showed a significantly low level of TNF-α in saliva at different intervals of time when compared to the FAs.

Impact of irregular appointment interval on Orthodontic treatment amid COVID-19 Pandemic- A Cross sectional Observational Study.

Introduction: The COVID-19 pandemic situation has affected all professions including orthodontics. The present study proposed determination of impact of irregular appointments on Orthodontic treatment amid COVID-19 pandemic. Materials and Methods: The cross-sectional observational study design enrolled 134 patients over period of 2 weeks were recorded such as treatment start date, total number of appointments scheduled and frequency of missed appointments after 24 March 2020, Stage of Orthodontic treatment on or before 24 March 2020 i.e., Alignment & levelling b) Space closure c) Finishing and Detailing d) Retention. Results: The distribution of malocclusion based on Angle Molar criteria showed prevalence of 53.73 % Class I, 37.32 % Class II and 8.95% Class III. The malocclusions were treated predominantly by extraction in 57.46 % and non-extraction in 42.44 % patients. The total treatment duration revealed that more than 51 % patients’ treatment lasted beyond 24 months but finished before 36 months. Similarly, 35.82 % patients’ treatment duration was less than 12 months. It was found that more than 47 % were in Stage I, 28.36 % were in stage II, 18.66 % in stage III and lastly 5.97 % were in stage IV. Conclusion: Orthodontic emergencies and un-schedule appointments had prevalence of Class I, followed by Class II and Class III with extraction treatment therapy. Majority patients treated with fixed mechanotherapy followed by Clear aligners and Retainers. More than half of the patients had increased treatment duration due to irregular follow up. The effect of the presence of brackets on the reliability of measurements made on three-dimensional digital models

Objectives: Digital model scans are frequently used today at the beginning and intermediate stages of orthodontic treatment. The aim of this study is to evaluate the effect of orthodontic brackets on linear measurements made on three-dimensional digital models. Material and Methods: Dental models were obtained from 40 patients before orthodontic treatment and in the first treatment session. Dental plaster models were converted into digital models using the 3Shape R700 (3Shape, Copenhagen, Denmark) model scanner. Intraoral scans were created with the Shape Trios3 (3Shape, Copenhagen, Denmark) intraoral scanner. Digital models were transferred to the Orthoanalyzer program (3Shape, Copenhagen, Denmark), and linear measurements were made. Results: There was no significant difference in the measurements made in intraoral scans scanned with and without brackets. No significant difference was observed in the measurements made on model scans with and without brackets (P > 0.05). Conclusion: Digital scanner systems, which are increasingly used with developing technology, provide safe and effective results even in the presence of brackets.

Alignment Efficiency of Two Archwires in Adolescents with Moderate to Severe Crowding

There is no consensus in clinical orthodontics on which aligning arch wire should be used at the initial stage of treatment. This randomized clinical trial evaluated the efficiency of multistranded Nitinol (Supercable) aligning arch-wire versus a standard arch-wire (Nitinol) in the first stage of orthodontic treatment in adolescents with severe crowding. Forty patients ages 12 to 17 with an Irregularity Index as proposed by Little between 5 to 17 mm were selected. Vinyl polysiloxane impressions (VPS) were taken before treatment (T1) at 6 weeks (T2) and 12 weeks (T3). Roth prescription .022 x .028 Edgewise brackets were bonded using light cure adhesive to the six anterior teeth of each dental arch and bands were placed on all first molars. Subjects were paired by gender and Irregularity Index into two groups of 20 each and assigned either to the control group (Nitinol) or the experimental archwire (Supercable). The predictor variables in this study were the type of archwire, type of dental arch and the time intervals. The outcome variable was the Irregularity Index score. The Irregularity Index was measured using 3D digital study models (e-models) and GeoDigm software from the VPS impressions taken at T1, T2 and T3. The data was analyzed with both a Two and Three-way Analysis of Variance (p≤0.05) to identify differences in the alignment of the maxillary and mandibular teeth and to determine differences between the two archwires. The Two and Three way ANOVA showed that there was a statistically significant difference (p≤0.05) in the alignment capabilities between the maxilla and the mandible. However, there was no evidence of a statistically significant difference (p=0.301) between NiTi and Supercable archwires at any time interval.

Elemental composition and structural characteristics of Bio-active™ orthodontic archwire

The Bio-active™ archwires are the latest generation multi-force orthodontic archwires made of a Ni-Ti alloy. It is of particular importance to orthodontists to know what their composition and structural characteristics are so that they can determine which one is suitable for a given stage of orthodontic treatment. The aim of this work is to characterize as-received Bio-active™ archwires, consisting of three segments (anterior, bicuspid and posterior), by determining their physicochemical properties. Laser-induced breakdown spectroscopy (LIBS) was used to determine the elemental composition in the three different segments of the archwires, along with X-ray diffraction analysis (XRD), scanning electronic microscopy (SEM), energy dispersive X-ray spectroscopy (EDX) and differential scanning calorimetry (DSC). A LIBS and EDX analysis of the elemental composition showed that nickel (55wt.%) and titanium (45wt.%) are the main elements, and in some segments Fe and Cr registered as trace elements. A XRD analysis, at room temperature, showed two similar peaks, characteristic of a Ni-Ti alloy, proof that the archwire is an austenite phase. The DSC data was obtained by measuring the Af temperatures for each segment (heated up to +80°C and cooled down to -80°C), showing that they can be classified as martensite-active wires (heat-activated). Based on that a recommendation can be made to cool down the unused, as-received archwires before clinical use to ensure that they will fit in the brackets easier. On the surface of the as-received archwires small grains can be seen from the SEM micrographs. The obtained results provide orthodontists important information regarding the physicochemical properties of the as-received Bio-active™ archwires. The results can also serve as a foundation for future research on the elemental composition and morphology of clinically applied Bio-active™ archwires.

Evaluation of the efficiency of the method of biological feedback in complex therapy of patients with hypertonus of the chewing muscles

The study included 40 patients aged 20 to 32 years with complaints of fatigue and discomfort in the area of chewing muscles, teeth compression during the day, nocturnal bruxism, crowding of teeth. Two groups were formed: the study group, whose patients underwent splint therapy in combination with biofeedback therapy trainings; in the control group, patients received treatment with splint therapy without the use of biofeedback therapy. Electromyography of the masticatory muscles at rest and during compression were used to diagnose hypertonicity of the masticatory muscles. To assess the psycho-emotional state, a medical and social questionnaire was conducted using questionnaires: «Health Questionnaire», Beck Depression Inventory, Spielberger-Khanin Anxiety Scale, SF-16 Quality of Life Scale. A decrease in the level of depression, anxiety was revealed with the stabilization of emotional sensitivity in relation to other people, emotional involvement in everyday life, and an increase in the quality of life. When comparing electromyographic data at rest and during clenching of teeth in patients of the study group the indicators were lower than in the control group. The effectiveness of biofeedback therapy at the stages of orthodontic treatment using splint therapy was established in the form of a decrease in the activity of masticatory muscles in patients of the study group. In the control group, the index of masticatory muscle activity decreased to a lesser extent. Biofeedback methods have shown a high level of effectiveness in reducing the symptoms of anxiety and depression, improving the quality of life.

Properties of the stem cells of the primitives of third molars

Regenerative medicine becomes an interesting field of research that solves the problem of the treatment of severe illnesses by replacing the affected structures with cell-based therapy or tissue engineering using autogenous stem cells. Due to these researches over the past decade, scientists were able to find new sources of stem cells in the adult body, improve treatment methods of various diseases, regenerate and replace tissues of many organs, and even correct some congenital defects. Systematization of theoretical knowledge and clinical research in the field of regenerative medicine is the basis for further selection of the proper stem cells sources and development of the best methods of regeneration and their successful application in practical medicine.The dental germs of the third permanent molars were used as research objects. To obtain the materials, the germs of the third molars were taken from 20 patients aged from 12 to 25 years old during the surgical stage of orthodontic treatment. Dental manipulations were performed after the signing of the voluntary information consent on conducting clinical trials by patients. It was decided to create three observation groups, taking into account the stage of formation of the third molars germs and the age of patients. The first group consisted of patients with the third molars germs at the stage of unformed root; the second - patients with the third molars germs with formed root and unformed apex; the third onepatients with formed root and formed apex of the third molars germs. Methods of the research: radiological - to assess the stage of development of the third molar germs; histological - for the morphological characteristics of the third molar germs tissues; immunohistochemical - to characterize the degree of maturity of the third molar germs tissues.The unformed root stage contains more reserve of poorly differentiated cells of mesenchymal origin. The light color of the preparation indicates on a low concentration of protein Vimentin in the early stages of development, which indicates on low cell differentiation. At the same time, histological examination of third molars germs of the third observation group showed the presence of highly differentiated cells such as fibroblasts and fibrocytes, as well as a significant content of vimentin protein, which confirms the maturity of these cells.The obtained results allow us to identify the most suitable collection periods of the development for the third molars germs. It gives the opportunity to expand the scientific data on the properties of mesenchymal stem cells of odontogenic origin and to investigate further use of mesenchymal stem cells of odontogenic origin in practical medicine.

Metagenomic analysis of oral and intestinal microbiome of patients during the initial stage of orthodontic treatment

This prospective study analyzed changes in the oral and intestinal microbiomes in patients before and after fixed orthodontic treatment, elucidating the impacts of fixed orthodontic treatment on patient health and metabolism. Metagenomic analysis was conducted on stool, dental plaque, and saliva samples from 10 fixed orthodontic patients. All the samples were sequenced with Illumina NovaSeq 6000 with a paired-end sequencing length of 150 bp. Identification of taxa in metagenomes and functional annotation of genes of the microbiota were performed using the data after quality control. Clinical periodontal parameters, including the gingiva index, plaque index, and pocket probing depth, were examined at each time point in triplicates. Patients also received a table to record their oral hygiene habits of brushing, flossing, and dessert consumption frequency over 1 month. The brushing and flossing times per day of patients were significantly increased after treatment compared with baseline. The number of times a patient ate dessert daily was also fewer after treatment than at baseline. In addition, the plaque index decreased significantly, whereas the pH value of saliva, gingiva index, and pocket probing depth did not change. No significant differences were observed between the participants before and after orthodontic treatment regarding alpha-diversity analysis of the gut, dental plaque, or saliva microbiota. However, on closer analysis, periodontal disease-associated bacteria levels in the oral cavity remain elevated. Alterations in gut microbiota were also observed after orthodontic treatment. The richness and diversity of the microbiome did not change significantly during the initial stage of fixed orthodontic treatment. However, the levels of periodontal disease-associated bacteria increased.

Evaluation of the effectiveness of immunomodulatory therapy in the treatment of traumatic lesions of the oral mucosa in orthodontic patients

Relevance. The problem of traumatization of the oral mucosa with fixed orthodontic structures, especially at the initial stage of treatment, is one of the most urgent in orthodontics. Prolonged use of such structures often leads to adverse reactions from the oral mucosa in the form of chronic mechanical injury, burning, soreness, etc. The effectiveness of the use of alginate plates of the drug «Superlimf» as a correction of side effects from non-removable orthodontic equipment has been studied.Objective. To study the effectiveness of the use of the domestic cytokine preparation «Superlimf» in the form of alginate plates in the treatment of traumatic lesions of the oral mucosa that occur in patients at the stages of orthodontic treatment with fixed equipment.Materials and methods. The study included 35 people aged 18 to 25 years, of which 17 people used alginate plates «Superlimf» and 18 people – the control group – used orthodontic wax to protect the mucosa from injury.Results. The use of alginate plates «Superlimf» showed a reduction in the duration of the inflammatory process, a significant reduction in painful manifestations and swelling.Conclusion. The data obtained indicate the effectiveness of the drug «Superlimf» and allow us to recommend it for use in dental clinical practice.

An Observational Study on Changes in the Oral and Gut Microbiota through Professional Mechanical Tooth Cleaning, including Tooth-Brushing Instructions in Patients with Multi-Bracket Appliances

Multi-bracket appliances have long been established as tools for treating malocclusions. However, the complex construction and difficulty in cleaning due to their long-term intraoral retention have hindered the prevention of caries and periodontitis. In this study, professional mechanical tooth cleaning (PMTC), including tooth-brushing instructions, was continuously performed for 3 months in 24 patients who had worn multi-bracket appliances for more than 6 months, and changes in the oral and gut microbiota were examined using one-way repeated-measures analysis of variance. Additionally, changes in bacterial flora associated with different treatment durations were verified using the Pearson correlation coefficient. The results showed that continuous PMTC significantly reduced the amount of plaque in the oral cavity. No significant changes were observed in the oral or gut microbiota and no significant increase in pathogenic bacteria was observed. Therefore, our results suggest that continuous PMTC during orthodontic treatment with multi-brackets may inhibit the growth of pathogenic bacteria by maintaining a clean oral environment and avoiding dysbiosis in both the oral and gut microbiota. Significant changes in the gut microbiota with different treatment durations suggested that differences in food intake and food choices at each treatment stage of orthodontic treatment may affect the gut microbiota.

Changes in the pH and the Flow Rate of Saliva During Orthodontic Treatment with Fixed Orthodontic Appliances: A Systematic Review.

This systematic review aimed to assess salivary flow and pH changes at various stages of orthodontic treatment with fixed appliances. A comprehensive searches in electronic databases, including Medline, ResearchGate, Web of Science, SAGE Journals, Cochrane Oral Health Group's Trials Register, and ScienceDirect, without any publication date restrictions until January 2022 was conducted. The Pre-ferred Reporting Items for Reporting Systematic Reviews and Meta Analyses (PRISMA 2020 version) protocol was adopted and the risk of bias assessments were performed using the Cochrane ROBINS-I tool for non-randomized studies. Out of 4902 articles, 25 were fully evaluated, and seven studies that met the inclusion criteria were included in the review. The results showed that orthodontic treatment with fixed orthodontic appliances increased the stimulated salivary flow rate during various stages of treatment. However, the unstimulated salivary flow rate showed different changes. Furthermore, stimulated salivary pH increased, whereas unstimulated salivary pH significantly decreased, depending on the specific period of orthodontic treatment. Overall, fixed orthodontic appliances have an impact on salivary flow rate and pH during different stages of treatment. Nevertheless, the current literature is insufficient to draw definitive conclusions. More well-designedrandomized studies with larger sample sizes are necessary to confirm these findings.

УСОВЕРШЕНСТВОВАНИЕ КОМПЛЕКСНОЙ ТЕРАПИИ ГИПЕРТРОФИЧЕСКОГО ГИНГИВИТА У ПАЦИЕНТОВ С ПАТОЛОГИЕЙ ПРИКУСА

The prevalence of inflammatory periodontal diseases, and in particular hypertrophic gingivitis, among adolescents ranges from 50 - 99%, which is often combined with malocclusion and this combination is about 70% of cases, which is an urgent and unresolved problem of modern periodontology and orthodontics. Therefore, the study of the issues of correlation between the severity of morphological, functional and aesthetic disorders of the masticatory-speech apparatus and the severity of the inflammatory process in the tissues of the periodontal complex is important and necessary for the development of modern complexes for the treatment of adolescents with hypertrophic gingivitis and malocclusion class 1 according to Angle during the hormonal period. perestroika. The purpose of the study: to optimize the complex treatment of malocclusion in adolescents with hypertrophic gingivitis. 32 patients aged 14 to 17 years with E.H. class I malocclusion. ANGLE and mild edematous hypertrophic gingivitis were included in this study. All patients underwent orthodontic treatment using non-removable orthodontic equipment (brackets), but for periodontal treatment, patients were divided into 2 groups of 16 people, one group underwent traditional conservative periodontal treatment, and the other group of patients underwent removal of dental biofilm using GBT therapy on apparatus "PROPHYLAXIS MASTER". Before treatment, after 6 months and after the removal of orthodontic equipment, an index assessment of the state of periodontal tissues was carried out and a comparison of the average statistical data of teleradiography to study changes in the size of the facial part of the skull before and after orthodontic treatment, as well as a study of the correlation between the studied parameters was carried out for all patients. According to the results of the study, it was found that during orthodontic treatment of adolescents with malocclusion class I according to E.H. ANGLE and GBT-periodontal therapy increases the effectiveness of complex treatment for this category of patients by 40% compared to traditional therapy for inflammatory periodontal diseases at the stages of orthodontic treatment.

Performance of the Malmgren Index for Assessing Root Resorption on 2D vs. 3D Radiographs: A Pilot Study.

To compare the performance of the Malmgren index on 2D and 3D radiographs. Patients with a panoramic radiograph and a cone beam computed tomography (CBCT) taken at an interval of <3 months and presenting root resorption (RR) on at least one incisor and/or canine were retrospectively included. RR was scored twice by two observers using the Malmgren index in both the 2D and 3D sets, and intra-class correlation coefficient (ICC) was calculated. 155 teeth were analyzed. The ICC was the lowest in 2D, followed by overall, transversal and sagittal 3D. Malmgren scores were systematically higher in 2D, which overestimated RR, especially in the transversal plane on all incisors and canines and in the sagittal plane on the maxillary incisors. 2D respectively leads to 28.0-34.8% of false positives and negatives when discriminating between RR or not. The early stages of RR are often misdiagnosed in 2D, while later stages are more accurate. The original Malmgren index is not suited for 3D images, especially axial, where using dichotomized values (resorption yes/no) leads to overestimation of RR. A low-dose CBCT of the upper incisors could detect RR with high diagnostic accuracy in the early stages of orthodontic treatment, especially in patients with dental trauma or familial RR history.

Study on quantitative analysis of bracket-induced nonlinear response of labio-cheek soft tissue during the orthodontic process

In the orthodontics process, intervention and sliding of an orthodontic bracket during the orthodontic process can arise large response of the labio-cheek soft tissue. Soft tissue damage and ulcers frequently happen at the early stage of orthodontic treatment. In the field of orthodontic medicine, qualitative analysis is always carried out through statistics of clinical cases, while quantitative explanation of bio-mechanical mechanism is lacking. For this purpose, finite element analysis of a three-dimensional labio-cheek-bracket-tooth model is conducted to quantify the bracket-induced mechanical response of the labio-cheek soft tissue, which involves complex coupling of contact nonlinearity, material nonlinearity and geometric nonlinearity. Firstly, based on the biological composition characteristics of labio-cheek, a second-order Ogden model is optimally selected to describe the adipose-like material of the labio-cheek soft tissue. Secondly, according to the characteristics of oral activity, a two-stage simulation model of bracket intervention and orthogonal sliding is established, and the key contact parameters are optimally set. Finally, the two-level analysis method of overall model and submodel is used to achieve efficient solution of high-precision strains in submodels based on the displacement boundary obtained from the overall model calculation. Calculation results with four typical tooth morphologies during orthodontic treatment show that: ① the maximum strain of soft tissue is distributed along the sharp edges of the bracket, consistent with the clinically observed profile of soft tissue deformation; ② the maximum strain of soft tissue is reduced as the teeth align, consistent with the clinical manifestation of common damage and ulcers at the beginning of orthodontic treatment and reduced patient discomfort at the end of treatment. The method in this paper can provide reference for relevant quantitative analysis studies in the field of orthodontic medical treatment at home and abroad, and further benefit to the product development analysis of new orthodontic devices.

Effects of Low-Level Laser Therapy and Bracket Systems on Root Resorption during Orthodontic Treatment: A Randomized Clinical Trial.

This study aimed to assess the outcomes of low-level laser therapy (LLLT) with the conventional bracket (CB) and self-ligating (SL) bracket systems on root resorption (RR) during orthodontic treatment. A total of 32 patients were included in this randomized clinical trial. All the patients were randomly divided into four individual groups (SLL: self-ligating laser, CBL: conventional bracket laser, SLNL: self-ligating non-laser, CBNL: conventional bracket non-laser). RR was measured from the cone-beam computed tomography (CBCT) radiographs which were taken at two stages of the orthodontic treatment: pre-treatment (T1) and after leveling and alignment stage (T2). Wilcoxon rank test for the comparison was conducted to compare the RR at T1 and T2 stages within each group and showed a significant difference (p < 0.05) for various variables. Mann Whitney test compared the RR in laser and non-laser groups irrespective of the bracket systems and exhibited no significant differences except the left lateral incisor. Moreover, CB and SL groups showed no significant difference in RR among any tooth. Kruskal Wallis test was performed to compare the RR among all groups which presented no significant differences. LLLT and bracket systems have no consequences on RR until the leveling and alignment stage of orthodontic treatment.

Studying the variation of roots’ lengths of mandibular incisors during surgically-assisted orthodontic alignment using Cone Beam Computed Tomography

IntroductionOrthodontic root shortening is one of the complications which may cause movement of the teeth or even losing them. Many previous studies have found that accelerated orthodontic treatment using alveolar corticotomy could minimize the shortening and resorption of the roots. However, the X-ray imaging techniques used did not demonstrate the resorption on all root surfaces, and in some cases, they did not demonstrate root shortening unless it becomes aggressive. This study is to Evaluate variations in roots’ lengths of mandibular incisors during surgical-assisted orthodontic (SAO) alignment by alveolar corticotomy using cone beam computed tomography (CBCT). MethodsIn this study, we evaluated 96 mandibular incisors of 24 patients who suffered from moderate dental crowding in the anterior region of the mandible. The patients were divided into two groups: the control group which was submitted to conventional orthodontic treatment and the study group which was submitted to surgically-assisted orthodontic treatment using alveolar corticotomy. CBCT images were used to evaluate roots’ lengths at two times (T0, T1). ResultsThe result of this study showed statistically significant differences in root shortening of the two groups, and the mean of this shortening was lower in the study group in comparison with the control group. ConclusionsThe (SAO) technique using alveolar corticotomy minimizes root shortening of the mandibular incisors during the initial alignment stage of orthodontic treatment.

Influence of resin modified glass ionomer cement incorporating protein-repellent and antimicrobial agents on supragingival microbiome around brackets: an in-vivo split-mouth 3-month study.

To explore the influence of resin modified glass ionomer cement (RMGIC) adhesives containing protein-repellent and quaternary ammonium salt agents on supragingival microbiome, enamel and gingival health around brackets. Ten patients (21.4 ±3.5 years) about to receive fixed orthodontics were enrolled in this study. Unilateral upper teeth bonded with RMGIC incorporating 2-Methacryloyloxyethyl phosphorylcholine (MPC) and Dimethylaminohexadecyl methacrylate (DMAHDM) were regarded as experimental group (RMD), while contralateral upper teeth bonded with RMGIC were control group (RMGIC), using a split-mouth design. Supragingival plaque was collected from both groups before treatment (T0), and at 1 month (T1) and 3 months (T2) of treatment. High-throughput sequencing was performed targeting v3-v4 of 16S rRNA gene. Streptococcus mutans and Fusobacterium nucleatum quantification was done by qPCR analysis. Bracket failures, enamel decalcification index (EDI), DIAGNODent scores (Dd), plaque index (PI) and gingival index (GI) were monitored at indicated time points. Within 3 months, alpha and beta diversity of supragingival plaque had no difference between RMGIC and RMD groups. From T0 to T2, the relative abundance of Streptococcus depleted in RMD but remained steady in RMGIC group. Streptococcus, Prevotella, and Fusobacterium became depleted in RMD, Haemophilus and Capnocytophaga became depleted in RMGIC group but Prevotella enriched. Quantification of Fusbacterium nucleatum and Streptococcus mutans showed significant difference between RMGIC and RMD groups at T2. Teeth bonded with RMD had significant lower plaque index (PI) and DIAGNODent (Dd) score at T2, compared with teeth bonded with RMGIC (p < 0.05). No difference in bracket failure rate was examined between both groups (p > 0.05). By incorporating MPC and DMAHDM into RMGIC, the material could affect the supragingival microbial composition, inhibit the progress of plaque accumulation as well as the key pathogens S. mutans and F. nucleatum in the early stage of orthodontic treatment.

Human salivary concentrations of brain derived neurotrophic factor correlates with subjective pain intensity associated with initial orthodontic therapy

Current study aimed to evaluate presence & concentration of salivary molecular pain biomarkers Calcitonin Gene Related Peptide (CGRP) and Brain-Derived Neurotrophic Factor (BDNF) during initial stages of orthodontic treatment and correlation with subjective pain scales, Numerical Rating Scale (NRS), Visual Analogue Scale (VAS), Verbal Rating Scale (VRS) and McGill Pain Questionnaire (MPQ). Consented, healthy-pain free patients (n = 40) undergoing orthodontic therapy, having moderate crowding with pre-molar extraction were recruited. Unstimulated whole saliva was collected and stored at -80 °C in cryotubes. Levels of CGRP & BDNF in salivary samples was assessed by enzyme–linked immunosorbent assay. Samples were collected under stipulated 5 time periods using saliva collection tube by passive drooling method: immediately after bonding but before wire placement (T0-baseline), after 24 h (T1), 48 h (T2), 72 h (T3) & 168 h (T4) after wire placement. Consolidated subjective pain scales were administered concurrently. Regression value (R2 > 0.9) confirmed BDNF & CGRP in saliva. Significant change was observed from baseline to 168 h in all subjective parameters (p < 0.05). CGRP did not correlate with subjective pain scales statistically (p > 0.05). BDNF levels correlated with all the subjective pain scales, NRS (T3-p = 0.0092&T4-p = 0.0064), VRS (T3-p = 0.0112&T4-p = 0.0500), VAS (T3-p = 0.0092 &T4-p = 0.0064) &MPQ (T1-p = 0.0255). Mean BDNF & median subjective pain scale graphs were similar. Knowledge and Perception of Orthodontic Treatment among General and Non-Orthodontic Dental Specialists: A Comparative Study.

The objective of this study is to discover any possible association of gender or practitioner classification with orthodontic knowledge and attitudes towards orthodontic treatment. Descriptive cross-sectional research was conducted on two groups of dentists in the Ha'il region of Saudi Arabia. Group I included general practitioners, while Group II had non-orthodontic specialists. The findings suggest a statistically significant (p < 0.05) difference between the knowledge and attitudes of general practitioners and non-orthodontic specialists. Independent two tailed t-scores for difference in Knowledge (t-score = 3.19919, p = 0.003) and Attitude (t-score = 2.16314, p = 0.048319), highlight significant disparities in the knowledge and attitudes of general practitioners and non-orthodontic specialists. However, no statistically significant difference was observed in terms of knowledge and attitudes based on gender differences. This study captures and highlights subtle information that is very significant in dealing with critically important orthodontics issues. The study suggests that it is possible that a non-orthodontic specialist may suggest an altogether different line of treatment with different consequences when compared to a general practitioner and vice versa. This may result in unwanted, permanent orthodontic effects, highlighting the significance of the early stage of orthodontic treatment awareness. This research reveals disparities between the perceptions of general practitioners and non-orthodontic specialists about the stage and relevance of orthodontic treatment. It is strongly advised to see an orthodontic expert rather than a general practitioner or non-orthodontic specialist for orthodontic issues.

Analysis of Smile Aesthetic Changes With Fixed Orthodontic Treatment.

The mouth and teeth are vital in facial aesthetics and the face as a whole is the most critical individual component determining one's physical look. As dentists, we need to be aware that this might significantly alter the care given to a patient since the patient's and the dentist's ideas of beauty may be quite different. This study sought to ascertain how the general public, general dental specialists, and orthodontics all rated the attractiveness of patients who had received orthodontic treatment using a visual scale, as well as how the facial reference could be used to gauge the improvement in smiles. The attractiveness of 80 continuously treated patients was assessed by comparing their pre- and post-treatment posed grin images. The attractiveness of participants' smiles was evaluated using the Visual Simple Scale, which measures affect, and the Graph of Facial Stylish Reference, which measures objectively. "Making a Jazzy Face Chart" -The analysts were able to fairly evaluate changes in elements such as the symmetry of the smile twist, the position of the gingival peak of the front teeth relative to each other, the height of the incisal edges of the front teeth, the width of the connector band of the front six teeth, and the general tip of the front teeth by referring to photographs taken during treatment. We put the Visual Basic Scale to the test by having five male orthodontists, five male general dental specialists, and five male laypeople rate how much of an improvement they noticed between the before and after photos of a patient's smile. According to the findings of the first section of the research, orthodontic treatment generally leads to an increase in all indicators. Some instances, however, demonstrated worsening in characteristics such as grin arc consonance, gingival zenith location relative to each other, and incisal edge height. The second half of this research revealed that the opinions of orthodontic specialists, general dentists, and the general public varied with regard to the beauty of a smile. Based on this research, we can say the following: Orthodontic treatment led to improvements in many of the variables that contribute to smile attractiveness, factors include the interproximal width of contact area, the height of the incisal margins of the front teeth, and the location of the gingival zenith in relation to one another, to name a few. The results of this research support the idea that the aesthetics of the smile should be assessed at the last stages of orthodontic treatment when fine adjustments are being made.
