Spontaneous Bladder Wall Rupture Due to Emphysematous Cystitis in a Diabetic Patient: A Case Report

Spontaneous rupture of the urinary bladder (SRUB) secondary to emphysematous. Cystitis (EC) in diabetic patients is extremely rare. Emphysematous cystitis is a relatively rare disease entity characterized by intramural and/or intraluminal bladder gas best depicted by cross-sectional imaging. Its disease mechanism is not well understood. Case report: A 61-year-old diabetic woman presented to the emergency department with diffuse abdominal pain and hematuria of tree hours duration. Physical examination revealed generalized abdominal tenderness. Multi-slice abdominal and pelvic CT scans showed parietal pneumatosis of bladder and communicating continuity solutions with a pre-vesical collection (extravasation of PDC into the collection). After proper resuscitation, the patient was transferred to the operating room for exploratory laparotomy. A thickness bladder rupture was noted, which was repaired. Conclusions: SRUB in patients with poorly controlled diabetes and EC is highlighted in this case study. Urinary bladder rupture secondary to EC should be considered when a diabetic patient with a history of urinary symptoms presents with an acute onset of abdominal pain. Uneventful recovery from SRUB is dependent on early diagnosis and treatment.

Spontaneous Rupture of the Urinary Bladder: A Case Report

Spontaneous rupture of the bladder (SRUB) is a rare occurrence globally (<1%). The most common cause of bladder rupture is trauma (96%). We present a case of a 34-year-old man with a history of heavy alcohol consumption and significant smoking history. He presented with diffuse abdominal pain and urinary retention. On blood tests, he was found to have increased creatinine. A urinary catheter was inserted and there was improvement in his kidney function. However, on removal of the catheter, his creatinine function would elevate. This occurred two times before further investigations were performed. CT cystography was performed and there was a significant amount of free intraperitoneal fluid found in the pelvis. After retrograde filling of the bladder with contrast material, a tear in the dome of the bladder was discovered. The patient underwent a surgical repair of the tear and there were no complications during surgery. In summation, spontaneous urinary bladder rupture is a rare emergency which can cause acute abdomen, oliguria, peritoneal effusion and abnormal renal function. In patients with heavy alcohol consumption and especially without an associated trauma history, physicians should consider SRUB. CT scan can help in diagnosis. Diagnostic laparoscopy or explorative laparotomy should not be delayed.

Manifestation of Spontaneous Rupture of the Urinary Bladder in Pregnancy: A Systematic Review of the Literature.

Spontaneous rupture of the urinary bladder (SRUB) during pregnancy is a potentially fatal event that necessitates immediate surgery. The aim of this systematic review is to determine the symptoms, causes, associated factors, and prognosis of SRUB in pregnancy. We searched the literature from inception until December 2022 using the Cochrane Central Register, PubMed, EMBASE, ProQuest, Scopus, and Google Scholar. Articles not in English and those unrelated to the topic were excluded. The JBI Critical Appraisal Checklist for case reports was employed for the risk of bias assessment. The search strategy yielded 312 citations; 29 full-text articles were evaluated for eligibility after screening. Five case reports were included in this review. The age range of the cases was 27-39 years. Four cases were in their second trimester of pregnancy, and one was in her third. Two cases had undergone previous cesarean sections, and one had an appendectomy and salpingectomy due to an ectopic pregnancy. One case reported a history of alcohol and drug abuse. The most common symptoms were abdominal pain, abdominal distension, oliguria, voiding difficulty, hematuria, fever, and vomiting. The diagnosis of SRUB was primarily made via emergency laparotomy due to the patients' critical conditions. Beyond its diagnostic role, laparotomy was also the treatment method in all cases. Tear repair, antibiotic therapy, and urinary catheterization were all integral parts of the treatment. Four cases resulted in an uneventful pregnancy and a healthy, full-term baby. In one case, a hysterectomy was performed due to a combined uterus and bladder rupture. SRUB often presents with non-specific symptoms, leading to a delayed diagnosis. A high index of suspicion is essential when a pregnant woman exhibits urinary symptoms and severe abdominal pain, especially in those with a history of previous surgery. Early detection and treatment of SRUB are critical for an uneventful recovery.

Bladder malignancy as a cause of spontaneous bladder rupture: A systematic review.

To characterise cases of spontaneous rupture of the urinary bladder in the context of bladder cancer. A systematic review was performed to characterise cases of spontaneous bladder rupture in patients with bladder cancer. The Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) system was utilised, with databases being searched for relevant cases. Patient characteristics were extracted, including age, sex, presenting signs and symptoms, management modalities, tumour histology and mortality. Thirty cases were included. Seventeen (57%) were male, and the median age of presentation was 59. Abdominal pain and peritonism were the most common presenting symptoms, in 80% and 60% of patients, respectively. Most patients (n = 16, 53%) had urothelial cell carcinoma. Nine patients (30%) died during their initial hospitalisation. Spontaneous bladder perforation in the context of bladder cancer is a rare cause of acute abdomen. The diagnosis is associated withhigh mortality, highlighting the aggressive nature of the malignancies that cause spontaneous bladder rupture. This raises important questions about the role of emergency cystectomy, the timing of systemic therapy and the appropriate involvement of palliative care.

After proper resuscitation, the patient was transferred to the operating room for exploratory laparotomy. A 2 cm full-thickness bladder rupture was noted at the dome of the bladder, which was repaired.Clinical discussionWe noticed free air in the urinary bladder wall postoperatively in the CT scan, which is the radiological sign of EC. The Pathology result was in concordance with the diagnosis.ConclusionsSRUB in patients with poorly controlled diabetes and EC is highlighted in this case study. Urinary bladder rupture secondary to EC should be considered When a diabetic patient with a history of urinary symptoms presents with an acute onset of abdominal pain suggestive of peritonitis. Uneventful recovery from SRUB is dependent on early diagnosis and treatment.

Preceding severe acute respiratory syndrome coronavirus-2 (SARS‐CoV‐2) infection predisposing to such eventuality is unprecedented. Experience with such a singular case is presented and discussed.

Abstract Description of the clinical case: A 59-year-old male with a history of lower urinary obstructive symptoms presented to the emergency department with a clinical picture of acute abdomen and hematuria of 10 days of evolution. Spontaneous bladder rupture associated with a complication of bladder diverticulum rupture was diagnosed. Relevance: Spontaneous bladder rupture is a rare condition that can be fatal. In general, this pathology presents with non-specific symptoms, so it is necessary to think about the diagnosis. The case of spontaneous bladder rupture is presented and how the diagnosis was reached, for timely treatment. Clinical implications: In a patient with a clinical picture of acute abdomen, nitrogen elevation and hematuria in the context of a history of bladder emptying problems, it is important to consider the diagnosis of spontaneous bladder rupture. Conclusions: Spontaneous rupture of a bladder diverticulum is a very rare entity that presents with nonspecific clinical symptoms and signs. Suspicion should be high when assessing a patient with an acute abdomen, elevated creatinine, and hematuria.

Sudden Death Due to Spontaneous Rupture of the Urinary Bladder following Acute Alcohol Inebriation- A Case report and Review of Literature

Spontaneous rupture of the urinary bladder (SRUB) is a rare clinical condition. Prompt diagnosis is often difficult both clinically and radiologically and necessitates a high index of suspicion as the patients present with non-specific abdominal pain and may not offer a clear history. The depressant effect of alcohol further complicates the diagnosis. We report a case of a young male who was unable to seek medical support and was found dead within 12 hours of the onset of abdominal discomfort following acute alcohol intoxication. At autopsy, rupture of the urinary bladder with blood and blood clots was found in the pelvic cavity with no any corresponding external injury. Spontaneous rupture of the urinary bladder is a rare cause of death in acute alcohol intoxication. In order to further understand this rare condition, the review of related literature has been done.

Emergency laparoscopic exploration of postpartum spontaneous rupture of the urinary bladder:A case report and review of literature

Introduction: Spontaneous rupture of the urinary bladder(SRUB) is a relatively rare cause of acute abdomen disease, whcih was easily complicated with diffuse peritonitis, septic shock, acute renal failure. Athough a few cases about postpartum patient sufferred from SRUB have been reported recently,the management of postpartum SRUB was not well established. Case Report: The subject of this case was a 37-year-old postpartum woman who presented with abdominal pain for one day.Based on his clinical presentation, investigation results, it was highly suspected for urinary disease complicated with diffuse peritonitis, such as SRUB, ureteral rupture. Given that vital signs were unstable and peritonitis signs were aggravating gradually, emergency laparoscopic exploration were performed. There was a visible rupture in the left posterior of the bladder wall, with a diameter of 2cm.Laparoscopic repair was employed and patient recovered well. In this article, we also reviewed relevant literatures to initially establish main diagnostic basis for postpartum SRUB. Conlcusion: Postpartum women with a complaint of acute abdomen pain maybe caused by SRUB, especially companied with oliguria and dysuria. And laparoscopy have an obvious advantage to the diagnosis and treatment for highly suspected postpartum SRUB.

Introduction:Spontaneous rupture of the urinary bladder is a rare, difficult to diagnose surgical emergency with a high mortality, there are many causes for spontaneous rupture of the urinary bladder, but we only found 2 reports on this condition in our literature search. A 36-year-old male patient was admitted with “whole abdominal pain associated with hematuria for 5 hours.” Our patient did not have a history of definite allergy, but a long-term history of alcohol abuse. This patient was followed up for 1 year, and the cystoscopy recheck showed that the bladder lesion had healed.Conclusions:Since eosinophilic cystitis is associated with long-term alcohol consumption, we recommended that the patient should stop drinking and taking antihistamines.

Introduction: Spontaneous rupture of the urinary bladder [SRUB] is a rare event. Patients usually present with features of peritonitis and diagnosis is usually made at operation. There are several factors that can predispose patients to an event like this, but most commonly it includes a chronic increase in bladder wall pressure and chronic inflammatory states besides malignancy. Case Report: A 42 years old male came in emergency surgery department with pain in abdomen. Ultrasonography of abdomen was suggestive of peritonitis for which he underwent exploratory laparotomy and was found to have perforation of urinary bladder which was repaired. Patient recovered uneventfully. Conclusion: Rupture of urinary bladder must be included in the differential diagnosis of acute abdomen presenting with peritonitis. A high index of suspicion is essential in the presence of urinary symptoms. In the majority of cases, underlying bladder pathology has been identified, although idiopathic SRUB may occur.

Spontaneous rupture of the urinary bladder (SRUB) is a rare clinical condition. Prompt diagnosis is often difficult both clinically and radiologically and necessitates a high index of suspicion as the patients present with non-specific abdominal pain and may not offer a clear history. We report this young male who presented with SRUB which occurred after alcohol intoxication and presented to us with vague abdominal pain, anuria, and intraperitoneal fluid collection. He had presented with a painful urinary retention few days prior to this visit, was catheterized temporarily and was discharged on antibiotics with a diagnosis of prostatitis. Currently, he stated that he had not urinated since the recent discharge, and noticed increasing abdominal distension. He also had diffuse severe abdominal pain. Patient stated that prior to this, he had consumed 12 beers. Examination revealed a male in acute distress, with normal ranges vital signs. Abdomen was soft, diffusely tender, with sluggish bowel-sounds, and no rebound-tenderness. Rest of examination was normal. Blood workup was significant for elevated BUN and creatinine from his baseline, and leukocytosis. A Foley catheter was placed. His abdomen computed tomography (CT) scan showed a newly developed pneumoperitoneum. CT cysto-urethrogram confirmed intraperitoneal bladder rupture. The patient had an exploratory laparotomy. The bladder defect was closed, and an indwelling catheter was left to ease healing. He had an uneventful postoperative course, and was discharged with a follow-up appointment in the urology clinic. Spontaneous urinary-bladder rupture is a rare event. Excessive alcohol consumption causes diuresis, over-distension and rupture of the urinary bladder. Initial presentation remains imprecise; continuous urinary leakage leads to abscesses, sepsis, and metabolic derangements. CT scan is imaging modality of choice. Idiopathic spontaneous rupture of the urinary bladder - A rare cause of death

Spontaneous rupture of the urinary bladder is a rare clinical condition, with a prevalence of about 1 in 126.000 hospitalizations. Idiopathic rupture is even more rarely encountered, constituting less than 1% of all cases. Given the low incidence, atypical symptoms and poor history data, the diagnosis is often established late and treatment is delayed, resulting in a high rate of mortality. Also, spontaneous rupture of the urinary bladder may be a surprise at the autopsy. In this paper, the authors present the case of a 67 years old woman who died due to idiopathic spontaneous rupture of the urinary bladder after ingestion of alcoholic beverages. The autopsy showed no signs of pathological or traumatic injury and the toxicological examination revealed a blood alcohol concentration of 2.05 g ‰. The case is analyzed in the context of existing literature data, taking into account the elements of anatomy, physiology and pathophysiology that can determine or favour the idiopathic spontaneous rupture of the urinary bladder.

Spontaneous rupture of the urinary bladder is defined as rupture of the urinary bladder into the peritoneal cavity without a history of trauma 1. Aetiological factors implicated include but are not limited to radiotherapy, chronic infective diseases like tuberculosis, necrotizing cystitis and in pregnancy, bladder injuries caused in the event of uterine rupture 2. Its incidence is 1 in 126,000 hospital admissions 3. We report a rare case of spontaneous intraperitoneal rupture of the urinary bladder in the early third trimester of pregnancy. This report highlights the diagnostic challenges obstetricians and surgeons may encounter with presentations of acute abdomen in pregnancy. The patient presented with acute abdominal pain, voiding difficulty with pyrexia and features of peritonitis. At laparotomy, 3.5 litres of urine were drained from the abdominal cavity and a 6cm rent in the vault of the bladder was found. Laparotomy, bladder repair with antibiotic cover and indwelling catheterization were the basis for a favourable outcome.

Investigation of Patients with Spontaneous Rupture of the Urinary Bladder

Non-traumatic rupture of the urinary bladder is termed “spontaneous rupture of the urinary bladder”. Although this disorder is relatively rare, when the bladder ruptures into the abdominal cavity, urine leaking into the abdominal cavity leads to the development of urinary peritonitis. We encountered seven patients with spontaneous rupture of the urinary bladder at our institution between 1987 and 2012. Six of these patients were women, all of whom had undergone surgery and radiotherapy to treat malignant uterine tumor. All seven patients suddenly developed abdominal pain and ascites. Urea nitrogen (UN) and creatinine (Cre) levels were greater in ascites than in the blood in all patients. Although all patients were treated conservatively, recurrence was identified in three patients. Recurrence was observed in only one of the four patients who received hyperbaric oxygen therapy. Conclusion: Hyperbaric oxygen therapy may be effective for recurrence of spontaneous rupture of the urinary bladder.

A Case of Spontaneous Bladder Rupture Mimicking Diabetic Nephropathy in a Patient with Type 2 Diabetes Mellitus

<p>Spontaneous rupture of the urinary bladder is a rare clinical entity, with the incidence reported as 1 in 126,000 hospital admissions. It is often associated with malignancy, inflammatory lesions, irradiation, calculus, diverticulum, binge alcohol drinking, continuous bladder irrigation, and neurogenic bladder. In rare instances, bladder rupture occurs without obvious causes. This rare clinical condition is difficult to diagnose because of vague symptoms. High index of suspicion is needed as the mortality rate is high if untreated. A 37-year-old woman with uncontrolled type 2 diabetes, was admitted to the emergency room complaining of progressive abdominal distension and discomfort. She had a past history of tubo-ovarian and bladder abscess, and had undergone multiple surgical operations. From ascites fluid study, she was diagnosed as spontaneous bladder rupture. A transurethral catheter was inserted and the symptoms and signs resolved. Bladder rupture, mimicking acute kidney injury of diabetic nephropathy was disclosed without surgery.</p>
