MOG-IgG-Associated Demyelination Induced by Pembrolizumab Treatment in a Patient With Malignant Melanoma.

A 39-year-old man with melanoma who was treated with pembrolizumab for 6 months presented with acute-onset paraparesis. MRI showed multiple contrast-enhancing intraparenchymal lesions in the brain and spinal intramedullary lesions (Figure, A–F). Anti–myelin oligodendrocyte glycoprotein (MOG) antibody immunoglobulin G was positive in serum (1:320) and CSF (1:10). Visual evoked potentials were normal. After methylprednisolone treatment, the limb paraparesis improved, but the tumor lesion enlarged (Figure, H–I). Because of the tumor growth, 1 month later, the patient stopped taking hormones, which led to recurrence of limb paraparesis. Fewer cases of MOG immunoglobulin G–associated neurologic disorders than other CNS demyelination have been described in patients receiving PD-1 inhibitors.1,2

Disseminated Cysticercosis in Indian Scenario - Experience from a Teaching University Hospital.

Neurocysticercosis (NCC) due to Taenia Solium is a major public health problem. Our objective was to study patients with disseminated cysticercosis (DCC) who had NCC in the brain along with an additional site in the body and assess their clinical, radiological profile, and response to therapy. A chart review of DCC with a high lesion load of NCC ≥20 (DNCC) in the brain was performed. Sixteen (M:F = 13:3) patients were diagnosed with DNCC with a mean age of presentation of 35.1 ± 14.2 years. Headache was the predominant symptom, followed by seizures (93.75%), vomiting (43.75%), behavioral disturbances (31.25%), fever (12.5%), encephalopathy (12.5%), visual disturbances (6.25%), and muscle pain and limb weakness (6.25%). CT brain showed multiple active parenchymal cysts in all, and calcifications in 68.75%. MRI brain revealed involvement of cortex and subcortical structures in all, followed by cerebellum (81.25%) and brainstem (75%). Intramedullary spinal lesion was observed in 12.5% cases. Albendazole with steroids was used in 15 patients. In 93.3% patients, there was complete improvement in seizures; 12.5% subjects had persistent memory and behavioral abnormalities. One subject required decompressive craniectomy; mortality was observed in two subjects. We hereby report one of the largest case series on disseminated cysticercosis with a high lesion load of NCC in the brain. A comprehensive clinical, imaging, therapeutic response with repeat imaging and long-term follow-up has given us a better understanding of this difficult-to-treat neurological disorder. We suggest cautious use of anti-parasitic therapy under the cover of corticosteroids to prevent irreversible neurological sequelae.

Cranial nerve and intramedullary spinal malignant peripheral nerve sheath tumor associated with neurofibromatosis-1

Background: Malignant peripheral nerve sheath tumors (MPNSTs) are uncommon but aggressive neoplasms associated with radiation exposure and neurofibromatosis Type I (NF1). Their incidence is low compared to other nervous system cancers, and intramedullary spinal lesions are exceedingly rare. Only a few case reports have described intramedullary spinal cord MPNST.Case Description: We describe the clinical findings, management, and outcome of a young patient with NF1 who developed aggressive cranial nerve and spinal MPNST tumors. This 35-year-old patient had familial NF1 and a history of optic glioma treated with radiation therapy (RT). She developed a trigeminal MPNST that was resected and treated with RT. Four years later, she developed bilateral lower extremity deficits related to an intramedullary cervical spine tumor, treated surgically, and found to be a second MPNST.Conclusion: To the best of our knowledge, this is the first report of cranial nerve and intramedullary spinal MPNSTs manifesting in a single patient, and only the third report of a confined intramedullary spinal MPNST. This unusual case is discussed in the context of a contemporary literature review.

Application of sodium fluorescein for spinal cord lesions: intraoperative localization for tissue biopsy and surgical resection.

Sodium fluorescein (NaFL) has been used to aid in the resection of primary and secondary lesions within the brain. Comparatively, there is limited research on clinical applications for lesions within the spinal cord. Fluorescein-guided microsurgery may increase the ability to localize and safely surgically treat spinal lesions. Twelve patients with spinal cord lesions received fluorescein sodium 10% (Alcon Laboratories INC, Fort Worth, TX, USA) at 3mg/kg prior to surgical resection. Intraoperative visualization of fluorescence was performed using a Zeiss Pentero (Carl Zeiss AG, Oberkochen, Germany) microscope equipped with a Yellow560 filter or a Leica OH6 (Leica Microsystems, Wetzlar, Germany) equipped with a FL560 filter. Administration of NaFL resulted in lesional fluorescent contrast extravasation and facilitated surgical resection and localization in all twelve patients. In patients with a goal of complete resection, NaFL aided in complete resection of the spinal lesions in seven patients. In surgical resection patients, pathology was consistent with WHO grade I myxopapillary ependymoma in one patient, WHO grade II ependymoma in five patients, and nerve sheath tumor in one patient. In the other five patients, NaFL allowed for intraoperative tissue identification and successful tissue biopsy. In patients undergoing biopsy, tissue samples were positive for an intramedullary abscess, EBV-driven lymphoproliferative disease, and primary glial neoplasms. Fluorescein is a helpful microsurgical tool in guiding surgical resection and in the localization of intramedullary spinal lesions. Further research is necessary to explore fluorescein sodium applications in the resection of spinal cord lesions.

Intramedullary Spinal Cystic Lesions Mimicking Cavernoma with Spontaneous Myelum Hemorrhage in Children: A Case Report

Background: Intramedullary spinal masses is a rare yet devastating and challenging. One of the biggest difficulty is to reveal the mass type and feature, thus determine the definitive treatment. Despite its difficulties, many controversies persist regarding diagnosis and management. 
 Case Presentation: We report a case of 6 years old female came with gradual right limb weakness for 1 week before admission. It preceded by neck stiffness and for 2 weeks ago. Radiological examination revealed intradural intramedullary mass suggesting a cavernoma at VC1-C2 and VTh12-L1 level. The histopathological results shows unspecified hematoma.
 Conclusions: Intramedullary tumors in pediatric population is rare and can mimic any other mass lesion. MRI is the mainstay diagnostic tool of this patient. Complete surgical resection is the main goal of treatment, but the histopathologic features is the most important predictor of the functional outcome.

Use of intraoperative sodium fluorescein for diagnostic tissue biopsy of spinal cord lesions

BackgroundRecent evidence suggests that fluorescein-guided microsurgery facilitates resection of intrinsic and metastatic lesions in the brain. Use of sodium fluorescein for angiography has previously been reported without non-specific diffusion into the adjacent spinal parenchyma. Diagnostic biopsy of spinal cord lesion is often challenging and the localization of such lesions intraoperatively is paramount. We present the first known report of use of sodium fluorescein for localization and diagnostic biopsy of intrinsic spinal cord lesions. MethodsThree patients with spinal cord lesions received fluorescein sodium 10% (Alcon Laboratories INC, Fort Worth, TX, USA) at 3 mg/kg prior to surgical resection. Intramedullary spinal tumor-like lesions.

The development of magnetic resonance imaging (MRI) has led to an increasingly frequent detection of changes in the spinal cord. The most common intramedullary lesions are: demyelinating; vascular; inflammatory; infectious; and congenital, largely called tumor-like lesions. Spinal cord tumors are relatively rare, as compared with brain tumors. The hardest task is to conclude whether the spinal cord lesion is a tumor or a tumor-like lesion. This review is intended to help evaluate the spinal cord and gives an overview of the tumor-like lesions occurring in the spinal cord along with their characteristic.

Intramedullary schwannoma of conus medullaris: rare site for a common tumor with review of literature.

Schwannomas at an intramedullary location constitute only 0.3% of primary spinal tumors. Those in the conus are extremely rare, especially in the absence of neurofibromatosis, with only 12 cases reported in the literature so far, excluding the present case. We report the case of a 27-year-old female who presented with complaints of backache since 1 year, weakness in both lower limbs with tingling and numbness since 6 months, and urinary incontinence since 1 month. There was no history of trauma or any skin lesions. Magnetic resonance imaging (MRI) of the spine provided a provisional diagnosis of ependymoma. The patient was operated upon, histopathology revealed it to be a schwannoma. Intramedullary schwannomas without neurofibromatosis are extremely rare at the conus and can be completely resected unless there is infiltration. Gliomas and ependymomas are commoner entities at this site, which are usually subtotally resected. This case highlights that although extremely rare, the possibility of a schwannoma may be considered in the differentials of an intramedullary spinal lesion.

3T proton MR Spectroscopy evaluation of spinal cord lesions.

Objective:The objective of this study was to evaluate intramedullary spinal cord lesions using magnetic resonance spectroscopy and correlate the results with histo-pathological examination (HPE).Materials and Methods:Approval for this study was obtained from our institute ethical committee. Overall, 50 patients were recruited (29 male and 21 female), with a maximum age of 53 years and minimum age of 7 years. The mean age group of the study was 33 years. Standard magnetic resonance imaging (MRI) spine was done on a Siemens Skyra 3Tesla MRI scanner. MR Spectroscopy (MRS) was performed for all patients with intramedullary spinal lesions after getting written consent. It was performed using single-voxel method. The change in the metabolite peak was observed in each case and the results were compared with HPE. These collected data were analyzed using SPSS 16.0 version. Descriptive statistics, frequency analysis, and percentage analysis were used for categorical variables; and for continuous variables, mean and standard deviation were analyzed. McNemar's test was used to find the significance between conventional MRI MRS. In the above statistical tool, the probability value 0.05 is considered as significant level.Results:From our study, we observed that by applying routine MRI sequences alone, we could only detect around 58% of the cases correctly. However, when MRS was done along with the conventional MR imaging, the number of cases detected significantly increased to 84%. By applying McNemar's test and comparing the conventional MRI and MRS with HPE, it was found that statistically significant difference exists with P value of 0.007.Conclusion:MRS of the spinal cord is a promising tool for research and diagnosis because it can provide additional information complementary to other non-invasive imaging methods. It is an emerging tool and adds new biomarker information for characterization of spinal cord tumors, to differentiate benign from malignant lesions and to prevent unnecessary biopsies and surgeries.

Qualitative analysis of spinal intramedullary lesions using PET/CT.

OBJECT Although the usefulness of PET for brain lesions has been established, few reports have examined the use of PET for spinal intramedullary lesions. This study investigated the diagnostic utility of PET/CT for spinal intramedullary lesions. METHODS l-[methyl-11C]-methionine (MET)- or [18F]-fluorodeoxyglucose (FDG)-PET/CT was performed in 26 patients with spinal intramedullary lesions. The region of interest (ROI) within the spinal cord parenchyma was placed manually in the axial plane. Maximum pixel counts in the ROIs were normalized to the maximum standardized uptake value (SUVmax) using subject body weight. For FDG-PET the SUVmax was corrected for lean body mass (SULmax) to exclude any influence of the patient's body shape. Each SUV was analyzed based on histopathological results after surgery. The diagnostic validity of the SUV was further compared with the tumor proliferation index using the MIB-1 monoclonal antibody (MIB-1 index). RESULTS A total of 16 patients underwent both FDG-PET and MET-PET, and the remaining 10 patients underwent either FDG-PET or MET-PET. Pathological diagnoses included high-grade malignancy such as glioblastoma multiforme, anaplastic astrocytoma, or anaplastic ependymoma in 5 patients; low-grade malignancy such as hemangioblastoma, diffuse astrocytoma, or ependymoma in 12 patients; and nonneoplastic lesion including cavernous malformation in 9 patients. Both FDG and MET accumulated significantly in high-grade malignancy, and the SULmax and SUVmax correlated with the tumor proliferation index. Therapeutic response after chemotherapy or radiation in high-grade malignancy was well monitored. However, a significant difference in SULmax and SUVmax for FDG-PET and MET-PET was not evident between low-grade malignancy and nonneoplastic lesions. CONCLUSIONS Spinal PET/CT using FDG or MET for spinal intramedullary lesions appears useful and practical, particularly for tumors with high-grade malignancy. Differentiation of tumors with low-grade malignancy from nonneoplastic lesions may still prove difficult. Further technological refinement, including the selection of radiotracer or analysis evaluation methods, is needed.

Pediatric intramedullary spinal cord lesions: Pathological spectrum and outcome of surgery.

Background:Pediatric intramedullary spinal cord lesions are not only rare but also different from adults in a number of aspects. We aimed to study the incidence and the frequencies of various pediatric intramedullary mass lesions, their outcome to treatment and the factors determining their outcome of treatment.Materials and Methods:Thirty-one consecutive children (aged 1–18 years, mean 11.1 years, male: female = 1.8:1) with pathologically proven intramedullary spinal cord lesions treated at our center were studied. Clinico-radiological, histopathological, operative, and outcome data were reviewed retrospectively. The functional status was assessed using the modified McCormick grading system.Results:Gross total tumor excision was performed in 19 patients (61.3%), subtotal in 9 patients (29%), partial excision was performed in 2 (6.5%) patient, and only biopsy was performed in 1 patient (6.5%). There was one peroperative death, 2 patients died at follow-up. Complications included wound related complications (n = 4), transient deterioration in the motor power, and respiratory complication requiring a tracheostomy. Six patients showed recurrence at a mean follow-up of 16.4 months. Developmental tumors, high-grade ependymomas, and incompletely excised grade 2 ependymomas showed a tendency to recur.Conclusions:Children constituted nearly 1/5th (17.4%) of intramedullary spinal cord tumors. Astrocytomas and ependymomas taken together constituted the most common intramedullary spinal lesions in children; however, developmental tumors predominated in the first decade. Children usually presented in good functional grades preoperatively and maintained good grades after surgery. Functional outcome was dependent on the preoperative neurological status and histopathology of the lesions.

Girdle Sensation Caused by Radiculitis due to Neurosarcoidosis.

A 46-year-old woman suffered from unbearable tightness in the chest and difficulty in closing her eyes for over 1.5 months. A neurological examination revealed V1, 2 and facial nerve palsies, absent patellar and Achilles tendon reflexes, a positive Lasegue sign and girdle sensation corresponding to the Th2-8 dermatomes. Blood tests, including measurements of the angiotensin-converting enzyme level, were all normal. Brain and spinal magnetic resonance imaging showed no intramedullary spinal lesions; however, remarkable gadolinium enhancement of the Th2-7 dorsal roots (Picture), both trigeminal nerve entries and right geniculate ganglion was noted. Intramedullary Spinal Lesions Involving the Conus Medullaris: MR Imaging Features for Differential Diagnosis

Purpose : Intramedullary spinal lesions in the conus medullaris (CM), including tumors and vascular lesion, are rarely reported. We reported various MR features of intramedullary spinal cord lesions involving the CM including ependymoma, hemangioblastomas, dermoid cyst, ventriculus terminalis and spinal AVF and tried to discuss them for differential diagnosis. Materials and Methods: Six patients (male: female = 4:2, mean age = 44.3 year old) were enrolled from the clinical database of our institute from 2004 to 2010 and their radiological images and clinical symptoms were reviewed retrospectively. All patients had taken initial and postoperative MRI with contrast enhancement using gadopentate dimeglumine (GdDTPA). These images were analyzed by tumor size, location, signal intensity relative to the spinal cord, vascular flow voids, syrinx or cyst, edema and enhancement pattern. Results: Contrast enhancement was seen in all intramedullary masses. An eccentric enhancing nodule was noted in two hemangioblastomas and unusual peripheral rim enhancement with septation was seen in ventriculus terminalis. Patchy enhancement of the CM was observed in spinal arteriovenous fistula (AVF). Extensive cord edema adjacent to the intramedullary lesions was seen in four cases and syrinx was noted in three cases. Vascular signal voids were found in two hemangioblastomas and one spinal AVF. Conclusion: In evaluation of intramedullary spinal lesions in the CM, it is necessary to consider these unusual MR findings and discriminate various pathologies with prudence and caution. Extramedullary Myxopapillary Ependymoma of the Filum Terminale Associated With Syringomyelia: A Case Report

Abstract Aim: Ependymomas compose the majority of all intradural intramedullary spinal lesions. Filum terminale lesions are rarely associated with syringomyelia. Here we describe a case of extramedullary myxopapillary ependymoma associated with extensive syringomyelia located in the filum terminale. Material and Methods: A 34-year-old man admitted to our clinic with complaints of bilateral leg weakness, back pain and urinary dysfunction. The magnetic resonance imaging (MRI) of the patient showed us a contrast enhanced intraspinal mass at the level of L2 with a syringomyelia extending from L2 level to Th7 level. Results: The patient was operated via lumbar laminoplasty and tumor was totally resected. Conclusion: The removal of tumor is also sufficient for the resolution of syringomyelia and no additional procedure is essential for the treatment of syringomyelia in such cases. This is the first case of extramedullary tumors of the cauda equine with extensive syringomyelia.

Intramedullary spinal capillary hemangiomas: clinical features and surgical outcomes

Intramedullary capillary hemangiomas are exceedingly rare lesions in the spinal cord, and have been previously reported in the literature as case reports. The authors review their experience in 5 surgically treated patients with intramedullary capillary hemangiomas and discuss the clinical features, radiological findings, and surgical outcomes. Clinical and imaging data of 5 patients with intramedullary spinal capillary hemangiomas treated at a single institution were retrospectively analyzed. There were 4 men and 1 woman, and their ages ranged from 18 to 63 years (mean 47.2 years). The thoracic cord was affected in 4 patients, and the cervicothoracic cord in 1 patient. Four patients underwent a gross-total resection. One patient initially underwent a subtotal removal and then underwent a second surgery due to enlargement of the residual lesion. Long-term neurological function was improved in all patients and MRI showed no tumor recurrence. Capillary hemangiomas should be considered in the differential diagnosis of intramedullary spinal vascular lesions. Complete resection is the treatment of choice for these lesions. When total resection cannot be achieved, subtotal or even partial removal of the tumor is advised and repeat removal of the lesion is indicated when residual tumor regrowth is associated with neurological deterioration.

Review of stereotactic radiosurgery for intramedullary spinal lesions.

Stereotactic radiosurgery (SR) represents an increasingly utilized modality in the treatment of intracranial and extracranial pathologies. Stereotactic spine radiosurgery (SSR) uses an alternative strategy to increase the probability of local control by delivering large cumulative doses of radiation therapy (RT) in only a few fractions. SSR in the treatment of intramedullary lesions remains in its infancy-this review summarizes the current literature regarding the use of SSR for treating intramedullary spinal lesions. Several studies have suggested that SSR should be guided by the principles of intracranial radiosurgery with radiation doses placed no further than 1-2mm apart, thereby minimizing exposure to the surrounding spinal cord and allowing for delivery of higher radiation doses to target areas. Maximum dose-volume relationships and single-point doses with SSR for the spinal cord are currently under debate. Prior reports of SR for intramedullary metastases, arteriovenous malformations, ependymomas, and hemangioblastomas demonstrated favorable outcomes. In the management of intrame-dullary spinal lesions, SSR appears to provide an effective and safe treatment compared to conventional RT. SSR should likely be utilized for select patient-scenarios given the potential for radiation-induced myelopathy, though high-quality literature on SSR for intramedullary lesions remains limited.

ICG Videography Facilitates Interpretation of Vascular Supply and Anatomical Landmarks in Intramedullary Spinal Lesions

This is an illustration of two case reports. We present two cases where intradural indocyanine-green (ICG) videography facilitated interpretation of vascular supply patterns and anatomic landmarks in intramedullary spinal lesions. ICG videography is a new, complimentary imaging method, that is routinely employed in the context of vascular neurosurgical procedures and has recently been described to facilitate localization of intradural spinal lesions before dural opening. Its role in the setting of intramedullary lesions is less well defined. Two cases are presented. The first patient (case 1) had a small, diffuse, intramedullary lesion of unknown etiology who was referred for additional work-up and surgery. The second patient had presented with progressive paraparesis and ataxia due to an extensive intradural lesion, and he was referred for resection of the tumor.Routine exposure of the spinal cord via (hemi-)laminectomy and durotomy was followed by ICG videography (injection of intravenous indocyanine green, visualization with operating microscope in combination with an additional fluorescent light source) to guide the surgical strategy. In case 1, ICG videography helped to delineate the anatomic midline for optimization of myelotomy. In case 2, imaging was able to identify arterial feeders of the highly vascularized lesion and simplified safe removal of the lesion. ICG videography may represent a useful adjunct for optimization of surgical approach and strategy in intramedullary spinal tumors in selected cases. The anatomic midline can be safely identified in cases where usual landmarks are distorted, and better interpretation of supply patterns in highly vascularized lesions may increase safety of resection.

Intramedullary lumbar lesion mimicking spinal cord tumor: a case of non-neoplastic intramedullary spinal cord lesion

Non-neoplastic intramedullary spinal lesion cases are rarely seen in the literature. We would like to present this case for differential diagnosis of intramedullary spinal tumors. The aim of this case report is to attract attention on the MRI findings with lack of contrast enhancement and long syrinx formation which differs these types of lesions from the intramedullary spinal tumors. Intraoperative, pathological and immunohistochemical findings of non-neoplastic intramedullary spinal lesion were discussed.
