Prismatic adaptation coupled with cognitive training as novel treatment for developmental dyslexia: a randomized controlled trial

Despite intense and costly treatments, developmental dyslexia (DD) often persists into adulthood. Several brain skills unrelated to speech sound processing (i.e., phonology), including the spatial distribution of visual attention, are abnormal in DD and may represent possible treatment targets. This study explores the efficacy in DD of rightward prismatic adaptation (rPA), a visuomotor adaptation technique that enables visuo-attentive recalibration through shifts in the visual field induced by prismatic goggles. A digital intervention of rPA plus cognitive training was delivered weekly over 10 weeks to adolescents with DD (aged 13–17) assigned either to treatment (N = 35) or waitlist (N = 35) group. Efficacy was evaluated by repeated measures MANOVA assessing changes in working memory index (WMI), processing speed index (PSI), text reading speed, and words/pseudowords reading accuracy. rPA treatment was significantly more effective than waitlist (p ≤ 0.001; ηp2 = 0.815). WMI, PSI, and reading speed increased in the intervention group only (p ≤ 0.001, ηp2 = 0.67; p ≤ 0.001, ηp2 = 0.58; p ≤ 0.001, ηp2 = 0.29, respectively). Although modest change was detected for words and pseudowords accuracy in the waitlist group only (words: p ≤ 0.001, d = 0.17, pseudowords: p = 0.028; d = 0.27), between-group differences were non-significant. rPA-coupled cognitive training enhances cognitive and reading abilities in adolescents with DD. This innovative approach could have implications for early remedial treatment.

Gaze Patterns in Children With Autism Spectrum Disorder to Emotional Faces: Scanpath and Similarity.

Autism spectrum disorder (ASD) one of the fastest-growing diseases in the world is a group of neurodevelopmental disorders. Eye movement as a biomarker and clinical manifestation represents unconscious brain processes that can objectively disclose abnormal eye fixation of ASD. With the aid of eye-tracking technology, plentiful methods that identify ASD based on eye movements have been developed, but there are rarely works specifically for scanpaths. Scanpaths as visual representations describe eye movement dynamics on stimuli. In this paper, we propose a scanpath-based ASD detection method, which aims to learn the atypical visual pattern of ASD through continuous dynamic changes in gaze distribution. We extract four sequence features from scanpaths that represent changes and the differences in feature space and gaze behavior patterns between ASD and typical development (TD) are explored based on two similarity measures, multimatch and dynamic time warping (DTW). It indicates that ASD children show more individual specificity, while normal children tend to develop similar visual patterns. The most noticeable contrasts lie in the duration of attention and the spatial distribution of visual attention along the vertical direction. Classification is performed using Long Short-Term Memory (LSTM) network with different structures and variants. The experimental results show that LSTM network outperforms traditional machine learning methods.

Trait Anxiety Influences Negative Affect-modulated Distribution of Visuospatial Attention

Visuospatial attention allows humans to selectively gate and prioritize visual (including salient, emotional) information for efficiently navigating natural visual environments. As emotions have been known to influence attentional performance, we asked if emotions also modulate the spatial distribution of visual attention and whether any such effect was further associated with individual differences in anxiety. Participants (n = 28) discriminated the orientation of target Gabor patches co-presented with distractors, speedily and accurately. The key manipulation was randomly presenting a task-irrelevant face emotion prime briefly (50 ms), conveying Neutral/Disgust/Scrambled (Null) emotion signal 150 ms preceding the target patches. We calculated attention gradient (change in negative inverse attentional efficiency with unit change in distance from the source of emotion signal) as a metric to answer our questions. Specifically, the Disgust signal modulated the direction of attention gradients differentially in individuals with varying degrees of trait - anxiety, such that the gradients correlated negatively with individual trait-anxiety scores. This implies spatial shifts in Disgust-signalled visual attention with varying trait - anxiety levels. Neutral yielded attention gradients comparable to Scrambled, implying no specific effect of this signal and there was no association with anxiety levels in both. No correlation was observed between state - anxiety and the emotion-cued attention gradients. In sum, the results suggest that individual trait - anxiety levels influence the effect of negative and physiologically arousing emotion signals (e.g., Disgust) on the spatial distribution of visual attention. The findings could be of relevance for understanding biases in visual behaviour underlying affective states and disorders.

Development and Validation of Extended Reality Attentional Visual Field Tasks

A visuospatial attention measurement tool such as the Attentional Visual Field (AVF) task has been widely used in prior research to assess the spatial distribution of visual attention using a computerized task displaying visual targets on a computer screen. Although this computer-based task has been useful for examining individual differences in visuospatial attention and associated safety implications such as crash risk while driving, the computer-based task is limited due to its constraints. Newer AVF tasks were developed using virtual reality (VR) and augmented reality (AR) technology, and the results of the two evaluation studies provided partial evidence for the validities of the new AVF tasks. Overall, our current findings suggest that VR and AR versions of the AVF task can capture some aspects of visuospatial attention and that further validation and improvement are needed to establish the robustness of these new methods.

Visual Attention of Anesthesia Providers in Simulated Anesthesia Emergencies Using Conventional Number-Based and Avatar-Based Patient Monitoring: Prospective Eye-Tracking Study.

BackgroundInadequate situational awareness accounts for two-thirds of preventable complications in anesthesia. An essential tool for situational awareness in the perioperative setting is the patient monitor. However, the conventional monitor has several weaknesses. Avatar-based patient monitoring may address these shortcomings and promote situation awareness, a prerequisite for good decision making.ObjectiveThe spatial distribution of visual attention is a fundamental process for achieving adequate situation awareness and thus a potential quantifiable surrogate for situation awareness. Moreover, measuring visual attention with a head-mounted eye-tracker may provide insights into usage and acceptance of the new avatar-based patient monitoring modality.MethodsThis prospective eye-tracking study compared anesthesia providers' visual attention on conventional and avatar-based patient monitors during simulated critical anesthesia events. We defined visual attention, measured as fixation count and dwell time, as our primary outcome. We correlated visual attention with the potential confounders: performance in managing simulated critical anesthesia events (task performance), work experience, and profession. We used mixed linear models to analyze the results.ResultsFifty-two teams performed 156 simulations. After a manual quality check of the eye-tracking footage, we excluded 57 simulations due to technical problems and quality issues. Participants had a median of 198 (IQR 92.5-317.5) fixations on the patient monitor with a median dwell time of 30.2 (IQR 14.9-51.3) seconds. We found no significant difference in participants' visual attention when using avatar-based patient monitoring or conventional patient monitoring. However, we found that with each percentage point of better task performance, the number of fixations decreased by about 1.39 (coefficient –1.39; 95% CI –2.44 to –0.34; P=.02), and the dwell time diminished by 0.23 seconds (coefficient –0.23; 95% CI: –0.4 to –0.06; P=.01).ConclusionsUsing eye tracking, we found no significant difference in visual attention when anesthesia providers used avatar-based monitoring or conventional patient monitoring in simulated critical anesthesia events. However, we identified visual attention in conjunction with task performance as a surrogate for situational awareness.

Visual attention is not deployed at the endpoint of averaging saccades.

The premotor theory of attention postulates that spatial attention arises from the activation of saccade areas and that the deployment of attention is the consequence of motor programming. Yet attentional and oculomotor processes have been shown to be dissociable at the neuronal level in covert attention tasks. To investigate a potential dissociation at the behavioral level, we instructed human participants to move their eyes (saccade) towards 1 of 2 nearby, competing saccade targets. The spatial distribution of visual attention was determined using oriented visual stimuli presented either at the target locations, between them, or at several other equidistant locations. Results demonstrate that accurate saccades towards one of the targets were associated with presaccadic enhancement of visual sensitivity at the respective saccade endpoint compared to the nonsaccaded target location. In contrast, averaging saccades, landing between the 2 targets, were not associated with attentional facilitation at the saccade endpoint. Rather, attention before averaging saccades was equally deployed at the 2 target locations. Taken together, our results reveal that visual attention is not obligatorily coupled to the endpoint of a subsequent saccade. Rather, our results suggest that the oculomotor program depends on the state of attentional selection before saccade onset and that saccade averaging arises from unresolved attentional selection.

The influence of naturalistic, directionally non-specific motion on the spatial deployment of visual attention in right-hemispheric stroke

An impairment of the spatial deployment of visual attention during exploration of static (i.e., motionless) stimuli is a common finding after an acute, right-hemispheric stroke. However, less is known about how these deficits: (a) are modulated through naturalistic motion (i.e., without directional, specific spatial features); and, (b) evolve in the subacute/chronic post-stroke phase. In the present study, we investigated free visual exploration in three patient groups with subacute/chronic right-hemispheric stroke and in healthy subjects. The first group included patients with left visual neglect and a left visual field defect (VFD), the second patients with a left VFD but no neglect, and the third patients without neglect or VFD. Eye movements were measured in all participants while they freely explored a traffic scene without (static condition) and with (dynamic condition) naturalistic motion, i.e., cars moving from the right or left. In the static condition, all patient groups showed similar deployment of visual exploration (i.e., as measured by the cumulative fixation duration) as compared to healthy subjects, suggesting that recovery processes took place, with normal spatial allocation of attention. However, the more demanding dynamic condition with moving cars elicited different re-distribution patterns of visual attention, quite similar to those typically observed in acute stroke. Neglect patients with VFD showed a significant decrease of visual exploration in the contralesional space, whereas patients with VFD but no neglect showed a significant increase of visual exploration in the contralesional space. No differences, as compared to healthy subjects, were found in patients without neglect or VFD. These results suggest that naturalistic motion, without directional, specific spatial features, may critically influence the spatial distribution of visual attention in subacute/chronic stroke patients.

One's own name distorts visual space

Here, we demonstrated that one's own name attracts the subjective location of a visual target. We simultaneously presented observers their own name and others’ name in the left and right visual fields. A target circle was presented for 53ms around the center of the display 200ms after the names disappeared. Ten observers were required to manually reproduce the target location by pointing with the mouse. The results indicated that the observers significantly mislocalized the target 1.61′ on average toward the location of their own name. These observations indicated that the visual space is distorted by one's own name, which biases the spatial distribution of visual attention.

Emotional prosody rarely affects the spatial distribution of visual attention

Emotional manipulations have been demonstrated to produce leftward shifts in perceptual asymmetries. However, much of this research has used linguistic tasks to assess perceptual asymmetry and there are therefore two interpretations of the leftward shift. It may reflect a leftward shift in the spatial distribution of attention as a consequence of emotional activation of the right hemisphere; alternatively it may reflect emotional facilitation of right hemisphere linguistic processing. The current study used two non-linguistic attention tasks to determine whether emotional prosody influences the spatial distribution of visual attention. In a dual-task paradigm participants listened to semantically neutral sentences in neutral, happy or sad prosodies while completing a target discrimination task (Experiment 1) and a target detection task (Experiments 2 and 3). There was only one condition in one experiment that induced perceptual asymmetries that interacted with emotional prosody, suggesting that task-irrelevant emotional prosody only rarely directs attention. Instead a more likely cause of the leftward perceptual shift for comprehension of emotional speech is facilitation of right hemisphere linguistic processing.

Visual attention of anaesthetists during simulated critical incidents

Situation awareness (SA) is considered to be an important non-technical skill for delivering safe anaesthesia. The spatial distribution of visual attention (VA) is an underlying process for attaining adequate SA. In the present study, a novel technology was used to assess the distribution of VA in anaesthetists delivering anaesthesia. The impact of a critical incident on VA in relation to individual experience is analysed in a descriptive and exploratory manner. Fifteen anaesthetists induced general anaesthesia in a full-scale simulator while wearing a head-mounted eye-tracking camera system. After an uneventful session, workload was increased in a randomized order by simulation of a critical incident in the second or third session. Eye tracking was used for the assessment of individual's distribution of VA to monitors, patient, and environment. A post hoc video analysis revealed information about the spatial distribution of VA. Descriptive statistics and exploratory analysis were used. Twenty per cent of VA was directed to the patient monitor (30% during critical incident scenarios, P=0.003). The more experienced anaesthetists (more than 2 yr of work experience) increased the amount of time dedicated to manual tasks from 21% to 25% during critical incidents, whereas the less experienced decreased from 20% to 14% (P=0.061). Distribution of attention is different during anaesthesia induction with critical incidents compared with uneventful anaesthesia induction. Less experienced anaesthesia providers spend more time on monitoring tasks. Further investigation in confirmatory designs is needed.

Early deafness results in a redistribution of more attentional resources to the visual periphery in near space, specifically under conditions of selective attention, probably to compensate for the loss of auditory alertness to potentially dangerous stimuli from outside the current attentional focus. It remains poorly understood, however, whether spatial distribution of attention in far space is altered by early deafness as well. In the present study, we investigated whether and how early deafness alters the distribution of visuospatial attention in far space, compared to hearing controls. We asked deaf individuals and hearing controls to perform a flanker task with either peripheral or central distractors, either in near or far space. Sizes of compatibility effect were used to assess the amount of attentional resources received by the peripheral and central distractors. In near space, peripheral distractors induced significantly larger compatibility effect in deaf individuals than in hearing controls while central distractors induced significantly larger compatibility effect in hearing controls than in deaf individuals. On the other hand in far space, although peripheral distractors induced equivalent sizes of compatibility effect in the deaf and hearing groups, central distractors caused significant compatibility effect only in deaf individuals, but not in hearing controls. Our results suggest that early deafness results in a redistribution of visuospatial attention not only in near space but also in far space, with enhanced peripheral attention in near space and enhanced central attention in far space.

Effects of acute alcohol intoxication on visuospatial attention

The aim of the present study was to examine the effects of acute alcohol intoxication on the spatial distribution of visual attention measured with simple reaction times (RTs) to targets presented over an extended region of the visual field. Control (n=10) and alcohol groups (n=14) were tested with the same protocol. Participants were tested in two different conditions; in Experiment I, participants were instructed to direct their visual attention to the centre, while in Experiment II they were asked to orient their attention covertly to both right and left, but not to the centre. Throughout participants were required to fixate a small cross in the centre of the computer screen. In the alcohol group, participants received an alcohol dose of 0.4 g/kg so as to produce a blood alcohol concentration (BAC) in the range of 0.08% during the experiments. The spatial distribution of RTs was analysed graphically with geostatistical methods and statistically through analysis of variance of particular regions of the visual field. Results showed that controls were able to direct their attention tightly towards the centre (Expt I) and also to divide attention (Expt II) to the right and left. Participants in the alcohol group fixed their attention more diffusely in the centre (Expt I) and were unable to disengage attention from the centre in Experiment II. We conclude that acute alcohol intoxication impairs the ability to dissociate attention from gaze.

Aging and Visual Attention: The Effect of Perceptual Load on Dual-Task Performance

The ability to use information that is not directly in front of us (information located in the periphery) is important in many aspects of everyday life (e.g., using a computer, driving). It may be critical that a person be able to effectively use the information in the periphery. The current study examined how age-related changes in perceptual processing capacity and display-related variations in perceptual load affected the spatial distribution of visual attention. The results suggest that displays that make high demands on perceptual processing capacity reduce the functional field of view in older adults. Younger adults, however, were unaffected by the perceptual load manipulation. The primary practical implication is that displays that are intended to be used by older adults should not make high demands on their limited perceptual processing capacity.

How an abrupt onset cue can release motion-induced blindness

In motion-induced blindness (MIB), a target within rotating random dots is occasionally hidden from observers’ consciousness during observation. In the present study, a red ring-like cue was centered on a target and presented immediately after observers reported subjective disappearance of the target in MIB (Experiment 1). The radius of the cue was systematically modulated. Observers quickly regained awareness of the disappeared object only after they were provided with a pinpoint cue of its location. We also found that a flickering cue at 1 Hz hindered MIB when the radius of the cue was critically small (Experiment 2). Furthermore, abrupt onset of a small square was enough to regain awareness of the target (Experiment 3). Successful revival of the target with a small cue indicates that critical spatial distribution of visual attention determines what in the visual scene is included in visual awareness.

Effect of action video games on the spatial distribution of visuospatial attention.

The authors investigated the effect of action gaming on the spatial distribution of attention. The authors used the flanker compatibility effect to separately assess center and peripheral attentional resources in gamers versus nongamers. Gamers exhibited an enhancement in attentional resources compared with nongamers, not only in the periphery but also in central vision. The authors then used a target localization task to unambiguously establish that gaming enhances the spatial distribution of visual attention over a wide field of view. Gamers were more accurate than nongamers at all eccentricities tested, and the advantage held even when a concurrent center task was added, ruling out a trade-off between central and peripheral attention. By establishing the causal role of gaming through training studies, the authors demonstrate that action gaming enhances visuospatial attention throughout the visual field.

MEASURING SPATIAL DISTRIBUTION OF VISUAL ATTENTION IN ACTION VIDEO GAME

In this study, we applied a gaze-contingent window method to measure the spatial distribution of visual attention while playing an action video game. The peripheral visual field of a player was restricted to an area around the gaze by masks with various spatial extents. The quantitative relationship between window size and game performance confirmed that superior players had a wider distribution of visual attention than inferior players. An additional analysis of eye movements showed that the mean length of saccadic eye movements (indicating an overt allocation of attention) of inferior players was comparable to that of superior players. These results suggest that the difference in game performance between inferior and superior players may be due to the qualitative difference in the efficiency of covert allocation of visual attention. The present study also indicates the applicability of the gazecontingent window method for the online evaluations of performance and learning during dynamic visual-motor tasks.

Tachistoscopic treatment of dyslexia changes the distribution of visual–spatial attention

Twelve children with developmental dyslexia underwent a four-month treatment with tachistoscopic presentation of words, according to Bakker’s methodology. One group received standard lateral presentation of words on a PC screen, while the other group received the same stimuli in random lateral position. The spatial distribution of visual attention was measured by means of the Form-Resolving Field (FRF; Geiger, Lettvin, & Zegarra-Moran, 1992), which was administered along with reading tests, before and after treatment. The FRF of children who received random presentation widened at −12.5° on the left side, while the FRF in the group that received standard lateral presentation narrowed at that position. Both groups significantly improved in reading accuracy for both words and nonwords. Some hypotheses are proposed concerning the mechanisms responsible for the changes in the FRF and their correlation with improvements in word and nonword reading. The results of the present study are also compared with data suggesting a left “minineglect” in dyslexia.

The spatial distribution of attention in perceptual latency priming

The spatial distribution of visual attention is a yet unresolved question. One of the main topics is whether attention is distributed in a graded fashion around an attended location (e.g., Downing, 1988; Zimba & Hughes, 1987). The present experiments explore whether, and on which conditions, gradients of attention arise and contribute to perceptual facilitation. A masked or unmasked prime precedes one of two targets whose temporal order has to be judged. The prime captures attention, which shortens the perceptual latency of the primed target (perceptual latency priming; Scharlau & Neumann, 2003a; Shore, Spence, & Klein, 2001). No strong evidence for an attentional gradient was found. (1) Accuracy of temporal order judgements was independent of the distance between the two targets that were judged. That is, facilitation of the second target by the first target was spatially invariant. (2) With targets of short duration, facilitation was independent of prime-target distance. (3) With targets of long duration, gradients were found: Facilitation declined continuously with distance. Thus, long duration of stimuli may be a sufficient precondition for an attentional gradient. A control experiment showed that object-based attention contributes only marginally to perceptual latency priming.
