Assessment of glaucoma severity and deterioration by using Spaeth/Richman contrast sensitivity (SPARCS) and its correlation with retinal nerve fiber layer thickness and perimetry.

To assess the severity of glaucoma and its worsening using Spaeth/Richman contrast sensitivity (SPARCS). To evaluate its correlation with retinal nerve fiber layer thickness (RNFL) and perimetry. In this prospective observational study, 90 eyes of primary open-angle glaucoma were categorized into mild, moderate, and severe glaucoma (30 each). All eyes were subjected to contrast sensitivity measurements by using the Pelli-Robson (PR) chart and SPARCS, Humphrey automated perimetry, and optical coherence tomography (OCT) RNFL, along with routine ophthalmological evaluation at baseline. Contrast sensitivity was repeated at 1, 3, and 6 months. Other investigations were repeated at 6 months. The correlation between contrast sensitivity and other parameters was analyzed at baseline and 6 months. Total SPARCS showed a significant positive correlation with visual field index, pattern standard deviation, mean deviation, OCT RNFL, and a negative correlation with best corrected visual acuity. Central SPARCS and PR scores exhibited strong positive correlations. Both total and quadrantic SPARCS significantly reduced from mild to moderate to severe glaucoma. The quadrant-wise SPARCS also correlated well with opposite-side RNFL thickness. At 6 months, SPARCS showed a significant reduction along with RNFL measurements preceding any significant changes in visual field parameters. This study establishes SPARCS as a reliable and reproducible tool in assessing the deterioration of visual function in glaucoma patients even before significant perimetric changes. The specific relationship of quadrantic SPARCS with opposite-side RNFL is a novel yet expected finding. The findings advocate integrating SPARCS into routine glaucoma assessment for timely detection of any worsening and prompt intervention, improving the visual outcomes in these patients.

Central and peripheral contrast sensitivity in thyroid eye disease.

Assessment of central and peripheral contrast sensitivity (CS) in thyroid eye disease (TED) with and without dysthyroid optic neuropathy (DON). This cross-sectional study enrolled 33 eyes of 18 treatment-naïve TED patients and 18 age- and sex-matched healthy controls for comparative analysis. A detailed ophthalmic examination included visual acuity (VA), intraocular pressure measurement, slit-lamp biomicroscopy, and CS testing (central and four peripheral regions) using Spaeth-Richman Contrast Sensitivity test was done. The average age of TED patients was 47.17 ± 13.99 years and a female preponderance was noted (66.66%, n = 12). Twenty-five eyes (75.8%) were diagnosed as TED without DON, while eight eyes (24.2%) had DON. Nine eyes (27.2%) were in the active stage of disease and 29 eyes (87.8%) had proptosis. The difference in mean logMAR visual acuities between TED patients and controls was statistically insignificant (P = 0.189), but a significant difference was noted in central and total CS score (P < 0.001, Wilcoxon-Mann-Whitney test). On CS comparison between DON and non-DON eyes, a significant difference in average scores was noted in central and all peripheral areas (P < 0.05, Wilcoxon-Mann-Whitney test). With increasing clinical activity score, a statistically significant reduction was noted in CS in three out of four peripheral regions (Spearman correlation, P < 0.05). Visual function compromise can be detected in TED in the presence of intact VA, by testing CS. Peripheral CS deteriorates with increasing inflammation and in DON. Serial monitoring of both central and peripheral CS may help in diagnosing DON early.

Comparison of Central and Peripheral Contrast Sensitivity Between Healthy and Refractive Surgery Patients Using the Spaeth/Richman Contrast Sensitivity (SPARCS) Test.

To assess contrast sensitivity of central and peripheral vision with a newly developed, internet-based Spaeth/Richman Contrast Sensitivity (SPARCS) test in patients who underwent myopic photorefractive keratectomy (PRK) or femtosecond laser-assisted laser in situ keratomileusis (FSLASIK) refractive surgery in comparison with controls. In a retrospective study, a total of 186 eyes from 93 patients were analyzed: 62 eyes from 31 patients for each of the three groups under comparison. Patients who underwent a refractive surgery procedure and controls were evaluated using the SPARCS test. SPARCS scores were obtained for central and four peripheral areas (right upper, right lower, left upper, and left lower quadrants). Total, central, and peripheral SPARCS scores in patients with refractive surgery were compared with controls, adjusting for possible confounders. Multivariate and mixed linear regression models were used. Patients who had PRK had a lower score in all categories compared with the control group: by 5.9 points (95% CI: -9 to -2.8) in the total score, by 1.6 points (95% CI: -3 to -0.3) in the central score, and by 5.5 points (95% CI: -9.4 to -1.6) in the peripheral score. Patients who had FS-LASIK had non-statistically significant lower scores than the control group. PRK causes a decrease in central and peripheral contrast sensitivity. Effect of laser peripheral iridotomy on contrast sensitivity using Spaeth/Richman Contrast Sensitivity test.

Background:Laser peripheral iridotomy (LPI) is the current standard of care for primary angle-closure glaucoma. The existing literature lacks evidence regarding the effects of LPI on contrast sensitivity (CS) after the procedure.Objective:This study evaluates central and peripheral CS in patients undergoing LPI using the computer-based, Spaeth/Richman Contrast Sensitivity (SPARCS) test.Methods:We performed a pilot, prospective, interventional cohort study including 30 patients of primary angle-closure suspect (PACS) or primary angle closure (PAC) in both eyes. LPI was performed after a detailed history and clinical examination using standard procedure in all eyes. Intraocular pressure (IOP) and CS testing using SPARCS was performed before, 2 weeks and 3 months after LPI.Results:Data analyses revealed female predominance (66.67%, 20/30); the mean age of enrolled patients was 49.93 ± 10.43 years, and presenting acuity was 0.02 ± 0.06 (Log of Minimum Angle of Resolution [LogMAR]). The mean vertical cup-to-disc ratio (VCDR), mean deviation (MD in dB) and pattern standard deviation (PSD in dB) were 0.34 ± 0.09, –2.36 ± 1.72 and 2.34 ± 0.81, respectively. There was a statistically significant decrease between the pre- (15.17 ± 3.83 mmHg) and 2 weeks post-LPI (11.70 ± 1.53 mmHg) IOP (p < 0.001). However, CS in the pre- (73.47 ± 9.88) and 3 months post-LPI (75.20 ± 11.98) SPARCS scores did not reveal any statistical difference. The group-wise analysis showed a similar trend between PAC and PACS patients.Conclusion:LPI does not affect central as well as peripheral CS assessment in patients with the primary angle-closure disease.

Change in contrast sensitivity and OCT parameters in idiopathic intracranial hypertension.

Background:Deterioration in peripheral contrast sensitivity (CS) can be an indicator to detect progressive deterioration of visual function in patients with idiopathic intracranial hypertension (IIH).Objectives:The aim of this study was to evaluate the changes in central and peripheral CS and optical coherence tomography (OCT) parameters and in patients with IIH.Design and Methods:In this pilot observational study, data of 20 eyes of 10 ‘treatment-naïve’ IIH patients were analyzed. Detailed ocular examination was performed including CS assessment using both Pelli–Robson (PR) test and Spaeth–Richman Contrast Sensitivity Test (SPARCS) along with the OCT for macular and optic nerve head (ONH) parameters. A comparative analysis was done for CS and OCT parameters from baseline to a follow-up visit > 12 months (range: 18–24 months).Results:The study population had a female preponderance (80%, n = 8), and mean age at diagnosis was 31.9 ± 10.3 years. Mean follow-up period was 21.2 months (range: 15–24 months). At presentation, 6/6 visual acuity was noted in 75% eyes (n = 15) while all eyes had 6/6 vision at the last follow-up. Average PR score increased from 1.96 ± 0.36 to 2.30 at the last visit (Wilcoxon test: V = 0.0, p ⩽ 0.001) while average SPARCS score (total) increased from 71.85 ± 9.10 to 77.55 ± 6.20 (Paired t-test: t = −2.3, p = 0.035). Change in SPARCS score was significant in average total score (p = 0.035), and quadrant-wise score in superonasal (p = 0.014), inferonasal (p = 0.001), and inferotemporal (p = 0.021) quadrants. Same trend in SPARCS scores was observed when eyes with and without recurrence were analyzed. Statistically significant difference in retinal nerve fiber layer (RNFL) thickness (p = 0.007) and macular thickness (MT) in nasal quadrant (p = 0.006) was seen between the eyes with recurrence and without recurrence.Conclusion:Peripheral CS showed significant difference in all eyes over time. It showed significant change in eyes with recurrence, even in the presence of intact visual acuity and preserved central CS. Changes in RNFL thickness and nasal MT could differentiate eyes which developed recurrence from normal eyes.

Contrast sensitivity assessment for early detection of hydroxychloroquine toxicity.

Hydroxychloroquine (HCQ), is known to cause visual dysfunction during its prolonged course of administration. Optical Coherence Tomography (OCT) and field tests are the commonly used screening tests. Not much is known about the role of contrast sensitivity (CS) as method for early detection of HCQ toxicity. This pilot study was designed to study the change in central and peripheral contrast sensitivity for detecting early signs of HCQ toxicity. Prospective study conducted at Government Medical College & Hospital, Chandigarh, India. Twenty eyes of 10 treatment naïve, autoimmune patients. All patients underwent functional tests, contrast sensitivity tests: Pelli Robson and Spaeth Richman Contrast Sensitivity (SPARCS). Average and quadrant wise SPARCS score; Pelli Robson score and structural parameters viz., macular OCT parameters were compared at baseline, 3- and 6-month post treatment with HCQ. Change in CS using SPARCS and Pelli Robson were recorded at each visit and compared with the baseline. Structural and functional parameters were correlated. The average SPARCS scores at baseline, 3-month post HCQ and at 6-month post HCQ were 82.37 ± 8.40, 73.17 ± 9.63 and 70.37 ± 10.08, respectively. Quadrant wise change from first visit to third visit was statistically significant only in superotemporal (p = 0.002), inferotemporal (p = 0.02) and central (p = 0.01) quadrants. There was no correlation between SPARCS and Pelli Robson (S = 0.1454, p = 0.466). Similarly, there was no significant correlation between the structural and functional parameters. HCQ affect the CS by virtue of its action on bipolar and ganglion cells. Decrease in central and peripheral CS as measured by SPARCS can be a sign of early visual dysfunction, hence must be considered as an early indicator of HCQ toxicity.

Spaeth/Richman Contrast Sensitivity in Staging Glaucoma.

PurposeTo investigate contrast sensitivity measures in glaucoma eyes with moderate to severe glaucoma.MethodsThe study included 50 eyes of 47 pseudophakic patients with moderate or severe glaucoma who were seen at a tertiary center from 2017 to 2020. Assessment of contrast sensitivity using the Spaeth/Richman contrast sensitivity (SPARCS) test and the Pelli–Robson (PR) chart was compared in eyes with mean deviation (MD) < –12 decibels (dB) and > –20 dB (group 1), MD < –20 dB and > –30 dB (group 2), or MD < –30 dB (group 3). Multivariate regression analysis was used to analyze the association of visual field MD with SPARCS/PR scores and various clinical variables, including age, diagnosis, and logMar visual acuity.ResultsSPARCS total scores and quadrant-wise scores were significantly different in all of the quadrants, whereas central scores and PR contrast sensitivity were similar across groups. The total SPARCS scores predicted the change in MD (β = 0.5, P < 0.001, R2 = 61.8%) with minimal association of other quadrantic or PR scores. Total SPARCS scores of <45 and <38 predicted severe glaucoma with MD crossing –20 dB (sensitivity, 70.5%; specificity, 80.9%) and –30 dB (sensitivity, 79.3%; specificity, 77.7%), respectively. The logMar visual acuity did not correlate with any contrast sensitivity measure or clinical variables in this study.ConclusionsThe total SPARCS score may be used in staging glaucoma severity and to assess visual function in eyes with severe glaucoma.Translational RelevanceThe SPARCS test is a useful tool for assessing visual function in advanced glaucoma beyond MD worse than –20 dB or –30 dB.

Contrast sensitivity in Idiopathic Intracranial Hypertension.

Background: Idiopathic Intracranial Hypertension (IIH) is a disease of elevated intracranial pressure without any known cause. Visual dysfunction is the major morbidity of this disease but not much is known about the way the contrast sensitivity (CS) function is affected. Objective: This prospective, interventional study attempted to evaluate the change in central and peripheral contrast sensitivity, after treatment in patients diagnosed with IIH. Materials and methods: Twenty eyes of 10 IIH patients underwent an internet based, Spaeth Richman Contrast Sensitivity (SPARCS) test. Average and quadrant wise SPARCS scores were compared at presentation (treatment naïve), 1-month post treatment and 3 months post treatment. Results: The average SPARCS scores pre-treatment, 1-month post-treatment and at 3 months post treatment were 68.8 + 10.16, 74.45 + 11.17 and 75.7 + 10.81 respectively. At 3 months visit, the average SPARCS score was nearly comparable to the average score in normal Indian subjects, observed in a previous study of ours. Quadrant wise change in contrast sensitivity from first visit to third visit was significant in superonasal (p=0.003), inferonasal (p=0.029) and inferotemporal (p= 0.007) quadrants.Discussion: Effect of IIH on visual system is still a relatively unexplored area, especially in the Indians. Not many studies have concentrated on its impact on central as well as peripheral CS. Previous studies have hinted at a possible role of CS as a better indicator of visual dysfunction than other parameters. Conclusions: IIH affects both central and peripheral contrast sensitivity and therapy results in the improvement of contrast deficit. Poor contrast can possibly be explained by relatively more involvement of Magnocellular pathway over the Parvocellular pathway. Effects of Citicoline, Homotaurine, and Vitamin E on Contrast Sensitivity and Visual-Related Quality of Life in Patients with Primary Open-Angle Glaucoma: A Preliminary Study.

The aim of the present study was to evaluate the effects of supplementation with a fixed combination of citicoline 500 mg, homotaurine 50 mg, and vitamin E 12 mg (CIT/HOMO/VITE) on contrast sensitivity and visual-related quality of life in patients with primary open-angle glaucoma (POAG) in mild stage. This was a multicenter, observational, cross-over, short-term, pilot study on POAG patients with stable controlled intraocular pressure (IOP). Patients were randomly assigned to Group 1 (current topical therapy for 4 months and then current topical therapy plus CIT/HOMO/VITE for 4 months) or Group 2 (CIT/HOMO/VITE in addition to current topical therapy for 4 months and then topical therapy alone for 4 months). Best-corrected visual acuity, IOP, visual field, and the Spaeth/Richman contrast sensitivity (SPARCS) test score were recorded at baseline and after 4 and 8 months. The Glaucoma Quality of Life-15 (GQL-15) questionnaire was administered at each check time. Forty-four patients were assigned to Group 1 and 65 to Group 2. Over the follow-up period, there were no significant changes in IOP or visual field findings, whereas SPARCS and GQL-15 findings significantly varied from baseline, both being improved in subjects treated with CIT/HOMO/VITE fixed combination. These results demonstrate that a daily intake of a fixed combination of citicoline, homotaurine, and vitamin E in addition to the topical medical treatment significantly increased the total score of the contrast sensitivity test and the quality of life in patients with POAG.

Glaucoma-Associated Visual Task Performance and Vision-Related Quality of Life in South India

Performance-based measures may provide an objective assessment of how glaucoma affects daily functioning. We sought to validate a clinically-applicable performance-based measure of visual ability for patients with glaucoma in south India and to describe its relationship to clinical and patient-reported outcomes. Cross-sectional validation study. 145 participants with glaucoma were recruited at Aravind Eye Hospital. We modified the compressed assessment of activities related to vision (CAARV), a performance-based measure validated in the U.S., to be culturally relevant in south India. Participants underwent a series of tests, including the Indian CAARV (I-CAARV), Indian Visual Functioning Questionnaire (IND-VFQ), Spaeth/Richman Contrast Sensitivity (SPARCS) test, standard automated perimetry, and visual acuity (VA). Factor analysis and Rasch modeling were used to validate the I-CAARV. Correlations between the I-CAARV and other outcomes were evaluated. Psychometric properties of the I-CAARV for individuals with glaucoma in south India. The study included 142 participants (51.7% female, mean age 56.4 years). Average presenting visual acuity and visual field mean deviation (MD) in the better-seeing eye were 0.26 logMAR and -6.57 dB, respectively. The four tasks of the I-CAARV were found to measure a single underlying construct. Rasch analysis of the I-CAARV revealed that the outcome measure had moderate reliability, good construct and content validity, and fair measurement precision. Tasks were well-targeted to the study sample. Rasch-calibrated scores on the I-CAARV were significantly correlated with Rasch-calibrated IND-VFQ scores (r=-0.54) and with visual field MD, presenting VA, best-corrected VA, and SPARCS contrast sensitivity in both the better-seeing eye (r=0.60, -0.51, -0.53, 0.76, respectively) and worse-seeing eye (r=0.48, -0.61, -0.46, 0.69, respectively). The I-CAARV is a valid performance-based measure of vision-dependent functioning in glaucoma in south India. This study also found that I-CAARV task performance was strongly correlated with contrast sensitivity and suggests that performance-based and patient-reported outcomes are related but distinct measures of the impact of glaucoma on functioning and vision-related quality of life. Future studies are needed to determine the sensitivity of the I-CAARV to detect changes due to disease progression that are relevant to functioning and vision-related quality of life.

Vision-related Performance and Quality of Life of Patients With Rapid Glaucoma Progression.

The purpose of this study was to determine how clinical measures, performance-based measures and subjective assessments of vision-related quality of life (VRQoL) are affected in patients with rapid glaucoma progression. Prospective longitudinal study that included 153 patients diagnosed with moderate glaucoma. A subset of patients that presented with rapid glaucoma progression (n=22), defined as visual field (VF) mean deviation (MD) loss >2.0 dB/y, were compared with patients with nonrapid progression (n=131). Groups were compared using t tests, χ, or Fisher exact test. Main outcome measures were visual acuity (VA), VF MD, retinal nerve fiber layer thickness (RNFL), contrast sensitivity (CS), Compressed Assessment of Ability Related to Vision (CAARV), and Rasch calibrated National Eye Institute Visual Function Questionnaire (NEI-VFQ-25) scores. At baseline, patients who progressed rapidly had lower measurements of VA (P=0.041), VF MD (P<0.001), Pelli-Robson score (P=0.004), Spaeth/Richman Contrast Sensitivity (SPARCS) score (P=0.001), RNFL thickness (P=0.009), CAARV total score (P<0.001), and NEI-VFQ-25 composite score (P=0.03). A multivariable logistic regression was performed and showed VF MD to be the only baseline independent predictor of rapid progression. After 1 year, patients who progressed rapidly also had a significant decrease in SPARCS score (P=0.04). Factors associated with rapid glaucoma progression included worse VF MD decreased scores of performance-based measures and subjectively worse VRQoL. After 1 year, rapid progressors had a significant reduction in contrast sensitivity as measured by SPARCS.

Establishment of a normative database and evaluation of the test-retest repeatability of the Spaeth/Richman contrast sensitivity test.

To evaluate the test-retest repeatability of a computer-based contrast sensitivity (CS) test, the Spaeth/Richman contrast sensitivity (SPARCS) test, and to determine the effects of age and lens status on CS in normal eyes. Prospective cross-sectional study. The participants were assessed by use of the SPARCS test in each eye 3 times. The first 2 sessions were supervised, while the third was unsupervised. CS was determined for 5 areas of vision (central, superotemporal, superonasal, inferotemporal, and inferonasal) and combined to provide a total score. The test-retest repeatability was determined using Bland-Altman analysis and the intraclass correlation coefficient (ICC). The total SPARCS scores (maximum possible score = 100) ranged from 86.37 (±1.09) (for those aged 20 to 29 years) to 70.71 (±2.64) (for those aged 80 to 87 years). Individuals aged between 10 and 87 years with a normal eye examination (n = 205) were enrolled. When the SPARCS scores for the first 2 sessions were compared, the ICC was 0.79, and the repeated tests were fairly equivalent (mean difference = -0.29, P = .491). The test-retest 95% limits of agreement (95% LoA) ranged from -11.07 to +11.35. When the supervised sessions were compared with the unsupervised session, the ICC was 0.80, and there was slight improvement in the CS scores during the unsupervised session (mean difference = -1.15, P = .0001). The test-retest 95% LoA ranged from -9.18 to +10.60. The CS declined with advanced age and increased cataract severity (P <0.0001). Strong agreement was found between repeated SPARCS scores. Older age and increased lens opacity were associated with decline in CS in 5 areas of the visual field. The SPARCS test provides reliable and reproducible assessment of CS in normal eyes.

The relationship between contrast sensitivity and retinal nerve fiber layer thickness in patients with glaucoma.

To evaluate the correlation between contrast sensitivity (CS) and retinal nerve fiber layer (RNFL) in different areas of vision. This report is a sub-analysis of a prospective, observational cohort study investigating changes in performance-based assessment, vision-related quality of life, and clinical measures in patients with moderate to advanced glaucoma. The study included 161 participants with at least a 2-year history of glaucoma who underwent annual testing for 4years. Contrast sensitivity was measured using the Spaeth/Richman contrast sensitivity (SPARCS) test, while RNFL thickness (RNFLT) was measured using Cirrus optical coherence tomography (OCT). Statistical analyses were performed to determine correlations between CS and RNFLT; the correlations were calculated for each annual visit, totaling four correlation coefficients for each patient over the course of 4years. The SPARCS score in the left upper area of vision correlated the most strongly with the RNFLT of the inferior quadrant for both eyes at each annual visit, specifically in the seven o'clock sector for the left eye and the six o'clock sector for the right eye (p<0.05). There were no discernible trends for the correlations between the other areas of CS and RNFL quadrants or clock hours over the 4years of the study. Linear regression between the SPARCS total score and average RNFLT showed a significant direct correlation at each visit (p<0.01). Contrast sensitivity in the left upper area of vision for both eyes correlated most strongly with the thickness of the inferior quadrant of the RNFL. These fibers project to the temporal portion of the right occipital lobe, implying a potential center for contrast perception in this area. The longitudinal nature of the study suggests that CS may be a predictive tool for changes in RNFL in patients with glaucoma. Despite this finding, retinal damage and its relationship to CS was diffuse. In addition, SPARCS was shown to predict RNFLT. Further research is warranted to understand how CS can be used as a tool in the clinical setting.

SPARCS and Pelli–Robson contrast sensitivity testing in normal controls and patients with cataract

PurposeTo determine the ability of the newly developed internet-based Spaeth/Richman Contrast Sensitivity (SPARCS) test to assess contrast sensitivity centrally and peripherally in cataract subjects and controls, in comparison with the Pelli-Robson (PR) test.MethodsIn this prospective cross-sectional study, cataract subjects and age-matched normal controls were evaluated using the SPARCS and PR tests. Contrast sensitivity testing was performed in each eye twice in a standardized testing environment in randomized order. SPARCS scores were obtained for central, right upper (RUQ), right lower (RLQ), left upper (LUQ), and left lower quadrants (LLQ). PR scores were obtained for central contrast sensitivity. PR and SPARCS scores in cataract subjects were compared with controls. Intraclass correlation coefficients (ICC) and Bland Altman analysis were used to determine test-retest reliability and correlation.ResultsA total of 162 eyes from 84 subjects were analyzed: 43 eyes from 23 cataract subjects, and 119 eyes from 61 controls. The mean scores for SPARCS centrally were 13.4 and 14.5 in the cataract and control groups, respectively (P=0.001). PR mean scores were 1.31 and 1.45 in cataract and control groups, respectively (P<0.001). ICC values for test-retest reliability for cataract subjects were 0.75 for PR and 0.61 for the SPARCS total. There was acceptable agreement between the ability of PR and SPARCS to detect the effect of cataract on central contrast sensitivity.ConclusionsBoth SPARCS and PR demonstrate a significant influence of cataract on contrast sensitivity. SPARCS offers the advantage of determining contrast sensitivity peripherally and centrally, without being influenced by literacy.

To our knowledge, few studies have combined an objective measure of vision-related performance (VRP) and subjective measures of vision-related quality of life (VRQoL) with clinically related visual parameters, particularly in a large, prospective, cohort study setting. To examine the relationships between clinical visual assessments and both a VRP and 2 self-reported VRQoL measurements. Patients (N = 161) with moderate-stage glaucoma recruited from the Glaucoma Service at Wills Eye Hospital, Philadelphia, Pennsylvania, were enrolled from May 2012 to May 2014 in an ongoing prospective, 4-year longitudinal observational study. This report includes cross-sectional results from the baseline visit. Patients received a complete ocular examination, automated visual field (VF) test and Cirrus optical coherence tomographic scan. Contrast sensitivity was measured with the Pelli-Robson and the Spaeth-Richman Contrast Sensitivity (SPARCS) tests. Vision-related performance was assessed by the Compressed Assessment of Ability Related to Vision (CAARV) test. Vision-related QoL was assessed by the National Eye Institute Visual Function Questionnaire 25 (NEI-VFQ-25) and a modified Glaucoma Symptom Scale (MGSS). Correlations between clinical measures and CAARV, NEI-VFQ-25, and MGSS scores. A total of 161 patients were enrolled in the study. The strongest correlation was found between SPARCS score in the better eye and total CAARV score (r = 0.398; 95% CI, 0.235-0.537; P < .001). The CAARV score also correlated with the Pelli-Robson score (r = 0.353; 95% CI, 0.186-0.499; P = .001), VF mean deviation (r = 0.366; 95% CI, 0.200-0.510; P < .001), and VA (r = -0.326, 95% CI = -0.476 to -0.157; P = .003) in the better eye. There were more statistically significant correlations between contrast sensitivity tests and VF mean deviation with VRQoL measurements than with other clinical measures (visual acuity, intraocular pressure, Disc Damage Likelihood Scale, and mean retinal nerve fiber layer thickness). The MGSS scores were lower (worse) in women compared with men (P = .03 for binocular, P = .01 for better eye, and P = .05 for the worse eye). Structural measures (eg, Disc Damage Likelihood Scale, and retinal nerve fiber layer thickness) were generally not informative with respect to VRP or VRQoL. Contrast sensitivity tests and VF mean deviation were associated with both objective measures of the ability to act and subjective measurements of VRQoL. The strongest correlation was between SPARCS score (contrast sensitivity) in the better eye and total CAARV score. Therefore, measurement of contrast sensitivity should be considered when evaluating patients' VRQoL. The results of this study were limited by the patient population and apply only within the bounds of the tested cohort.

A prospective, longitudinal, observational cohort study examining how glaucoma affects quality of life and visually-related function over 4 years: design and methodology.

BackgroundThe aim of this study is to summarize the design and methodology of a prospective, longitudinal, observational cohort study to investigate how glaucoma affects patients’ quality of life and visually-related function over a 4-year period.Methods/DesignOne hundred sixty-one (161) subjects were enrolled in this ongoing study. Patients between the ages of 21–85 years with a minimum 2-year diagnosis of primary open-angle glaucoma, chronic primary angle-closure glaucoma or pseudoexfoliation glaucoma were included. Each patient visited Wills Eye Hospital for a baseline visit. Follow-up is planned for a minimum of 4 years, with annual visits. Each visit includes (1) Clinical evaluation: a slit lamp examination, fundoscopy, intraocular pressure measurement, visual field examination, spectral domain optical coherence tomography, Pelli-Robson Contrast Sensitivity test and the Spaeth-Richman Contrast Sensitivity test; (2) a performance based measure: the Compressed Assessment of Ability Related to Vision; and (3) Subjective measures of vision-related quality of life (the National Eye Institute Visual Functioning Questionnaire 25 and the Modified Glaucoma Symptom Scale).DiscussionThe results of this ongoing, prospective, longitudinal study are expected to shed light on the relationships between clinical measures, performance-based measures and subjective measures of well-being, in order to assess changes in the quality of life and the ability to function of patients with glaucoma over time.

SPARCS: a new method of evaluating contrast sensitivity in patients with glaucoma

AbstractPurpose To evaluate a new contrast sensitivity instrument as a diagnostic test for glaucoma.Methods Glaucoma patients, glaucoma suspects, and controls were prospectively evaluated using the Spaeth‐Richman Contrast Sensitivity (SPARCS) method. Testing was performed monocularly in a standardized testing environment. Contrast thresholds for the central, superonasal, superotemporal, inferonasal, and inferotemporal areas of vision were determined. The contrast sensitivity scores for each area of vision and the cumulative scores were then compared to subjects’ visual field and the amount of optic nerve damage, measured by the Disc Damage Likelihood Scale (DDLS) and the vertical cup to disc ratio. The results were analyzed with Spearman coefficients for continuous variables and the Kruskal‐Wallis test for categorical variables.Results One hundred and eighteen patients with glaucoma, 18 glaucoma suspects, and 125 controls completed the study. The mean SPARCS scores were 63.0 for patients with glaucoma, 71.9 for glaucoma suspects, and 77.1 for controls (p &lt; 0.01). SPARCS had strong correlations with patients’ visual field (r = 0.76, p &lt; 0.01), DDLS (r = ‐0.73, p &lt; 0.01), and vertical cup to disc (r = ‐0.64, p &lt; 0.01).Conclusion SPARCS was able to discriminate well between patients with glaucoma, glaucoma suspects, and controls. SPARCS had strong relationships with the degree of visual field and optic nerve damage. SPARCS may become a standard technique to assess visual ability in glaucoma patients. Commercial interest
