An Accessible Nephrectomy and Cystotomy Simulator for Use in Teaching Small Animal Surgery.

The use of simulators in veterinary education has been increasing over the last few years. This is fundamental for surgical classes, as simulators can provide a better learning environment for the students. Two procedures commonly taught in veterinary surgical practical classes are nephrectomy and cystotomy. However, the lack of simulators for use in these classes limits the training options to the use of cadavers, which have a number of associated disadvantages, including the potential for autolysis. The aim of this study was to develop and assess the value of a simple nephrectomy and cystotomy simulator that could replace the use of cadavers in surgical practical classes. The simulator, which represented the abdominal cavity, bladder, kidneys, ureter, vessels and adipose tissue, was constructed by using synthetic materials. To evaluate its usefulness and acceptance by the students, the learning outcomes and student satisfaction, for both the simulator and an ethically sourced dog cadaver, were compared. The students completed a Likert scale questionnaire, and the answers were evaluated by using the Diagnostic Content Validation (DCV) model. The simulator was well accepted by the students, with the best scores achieved for the ureter divulsion and ligation procedures; good results were also recorded for the kidney vessel ligature and urinary bladder suture practice. The scores showed that the simulator provided an acceptable experience during the training process and increased the confidence of the students in performing the procedure.

The Omentum-A Forgotten Structure in Veterinary Surgery in Small Animals' Surgery.

The greater and lesser omentum are derived from embryonic mesogastrium. The expansive greater omentum in dogs covers intestinal coils, while in cats, it is smaller. Comprising distinct portions, the greater omentum is rich in lymphatics and blood vessels. Conversely, the lesser omentum spans the liver, stomach, and duodenum. Studies on canine omentum reveal unique immune cell composition and regenerative potential attributed to adipose tissue-derived stromal cells (ADSCs). These cells hold promise in regenerative medicine, showing enhanced abilities compared with ADSCs from other sources. The omentum is critical in tissue repair and pathology, making it invaluable in veterinary surgery across various medical fields. The aim of this article was to research current knowledge about the applications of the omentum in veterinary surgery and the possibilities of using this structure in the future.

Laparoscopy and thoracoscopy in small animal surgery: A 2020 survey of small animal diplomate and resident veterinary surgeons.

To document the utilization and training of laparoscopic and thoracoscopic minimally invasive surgery (MIS) techniques within the American, European, Australian and New Zealand Colleges of Small Animal Veterinary Surgeons (ACVS, ECVS, and ANZCVS) in 2020. Observational study. Diplomates and residents of the ACVS, ECVS, and FANZCVS. An electronic survey was sent using veterinary list servers. Questions were organized into categories evaluating (1) the demographics of the study population and the caseload, (2) comfort level with specific procedures, (3) motivating factors and limitations, and (4) surgical training and the role of the governing bodies. Respondents included 111 practicing surgeons and 28 residents. Respondents' soft-tissue MIS caseloads had increased since they first started performing MIS; however, most respondents were only comfortable performing basic laparoscopy. Over half of the respondents agreed on the patient benefits and high standard of care provided by MIS. Perceived adequate soft-tissue training in MIS during residency was strongly associated with perceived proficiency at the time of survey response. Most respondents agreed that the specialty colleges should take a more active role in developing standards for soft-tissue MIS, with residents agreeing that a required standardized course would be beneficial. Soft-tissue MIS is widely performed by diplomates and residents. Perceived adequate soft-tissue MIS training was strongly associated with perceived proficiency. There is substantial underutilization of advanced MIS techniques in veterinary specialty surgical practice, which might be improved by a stronger focus on MIS training during residency.

Residency program factors affecting ACVS board examination pass rates from 2017 to 2021: A survey of ACVS small animal surgery residency programs.

To identify small animal surgery (SAS) residency program factors associated with board examination pass rates for the American College of Veterinary Surgeons (ACVS) board certification examinations. Online survey. Program directors of ACVS-registered SAS residency programs. An online survey was distributed to eligible 2021 ACVS SAS residency program directors. Respondents provided program information and Phase I and Phase II board examination pass rate data from 2017 to 2021. Programs were grouped based on overall combined board examination pass rates as "excellent" (≥90%) versus "adequate" (<90%). Data were analyzed using recursive partitioning. Responses from 36 ACVS SAS residency programs (18 private practice [PP], 18 academic programs [AP]) were included. Pass rates were considered "excellent" for 17/36 programs (11/17 AP, 6/17 PP). The strongest indicator of "excellent" programs was most mentors participating in rounds >75% of the time. Of those, the second indicator of "excellent" programs was a soft tissue caseload of <7 surgeries/resident/week. If the soft tissue caseload was >7 surgeries/resident/week, a high orthopedic caseload (>9 surgeries/resident/week) was indicative of "excellent" programs. If most mentors were not participating in rounds >75% of the time, APs were more likely to be "excellent." SAS APs may have higher ACVS board examination pass rates. Mentor involvement, caseload, and residency practice type may influence board examination pass rates. ACVS SAS residency programs' board examination pass rates may improve by optimizing mentor involvement and caseload regardless of practice type.

Quantification of skin wound tension using a newly designed wound tensiometer.

To (i) quantitatively measure wound tension in experimental skin wounds using a newly developed wound tensiometer and (ii) establish reference values for primary skin wound closure in medium- and large-breed dogs. Experimental cadaveric study. Nineteen dogs of medium to large breeds (BW 20 to 40 kg). Elliptical skin wounds of different sizes were created on the chest and abdomen. The wounds were gradually enlarged. Experienced surgeons (ECVS diplomates or professors of small animal surgery) and inexperienced surgeons (1st year after graduation) independently assessed wound tension through manual manipulation and determined whether the wound could be closed without tension-relieving measures. In addition, wound tension was objectively quantified using a newly developed wound tensiometer. The upper threshold for wound tension at which direct appositional wound closure was recommended by the experienced surgeons was 5.4 N, and the median minimal tension without recommendations for closure was 6.0 N. The data also demonstrate that wound tension and wound size do not necessarily correlate, and inexperienced surgeons need to develop a feel for wound tension. The intraoperative use of the wound tensiometer, in combination with established cut-off values, might facilitate decision-making regarding primary wound closure. The findings of this study provide evidence for the applicability of a wound tensiometer in guiding inexperienced surgeons in their choice of the skin wound closure method.

Minimally invasive longitudinal intravital imaging of cellular dynamics in intact long bone.

Intravital two-photon microscopy enables deep-tissue imaging at high temporospatial resolution in live animals. However, the endosteal bone compartment and underlying bone marrow pose unique challenges to optical imaging as light is absorbed, scattered and dispersed by thick mineralized bone matrix and the adipose-rich bone marrow. Early bone intravital imaging methods exploited gaps in the cranial sutures to bypass the need to penetrate through cortical bone. More recently, investigators have developed invasive methods to thin the cortical bone or implant imaging windows to image cellular dynamics in weight-bearing long bones. Here, we provide a step-by-step procedure for the preparation of animals for minimally invasive, nondestructive, longitudinal intravital imaging of the murine tibia. This method involves the use of mixed bone marrow radiation chimeras to unambiguously double-label osteoclasts and osteomorphs. The tibia is exposed by a simple skin incision and an imaging chamber constructed using thermoconductive T-putty. Imaging sessions up to 12 h long can be repeated over multiple timepoints to provide a longitudinal time window into the endosteal and marrow niches. The approach can be used to investigate cellular dynamics in bone remodeling, cancer cell life cycle and hematopoiesis, as well as long-lived humoral and cellular immunity. The procedure requires an hour to complete and is suitable for users with minimal prior expertise in small animal surgery.

A Spanish Survey on the Perioperative Use of Antimicrobials in Small Animals.

Appropriate use of perioperative antimicrobials can significantly reduce the risk of post-operative infections. However, inappropriate antimicrobial use can result in the creation of multidrug-resistant bacteria, increased costs, host flora disruption, side effects and increased risk of hospital-acquired infections. This survey evaluated the current perioperative use of antimicrobials in small animals by Spanish veterinarians using a web-based questionnaire. Responses were represented using descriptive statistics and a statistical analysis of the association between demographic data and perioperative antimicrobial use was performed. Pre-operative antimicrobials were administered in clean surgery by up to 68.3% of participants, 81.0% in clean-contaminated surgery and 71.3% in dirty surgery, while in the post-operative period, antimicrobials were administered by up to 86.3% of participants in clean surgery, 93.2% in clean-contaminated surgery and 87.5% in dirty surgery. Factors considered "very important" for antimicrobial selection were the degree of wound contamination, patient immunosuppression and use of prosthesis. The most frequently used antimicrobial was beta-lactamase-resistant (or potentiated) penicillin. Post-operative antimicrobial use was associated with participants without specific surgical postgraduate training. This study highlights an overuse of antimicrobials in perioperative procedures in small animal surgery in Spain. Therefore, evidence-based guidelines and further education regarding the correct use of antimicrobial prophylaxis are recommended.

Evaluating gender discrepancies in the time to successful match into a small animal surgery residency program.

To determine any association between gender and likelihood of first attempt match and overall time to match into an American College of Veterinary Surgery (ACVS)-registered small animal surgical residency program (SASRP). Online survey. A total of 100 (77 female, 23 male) ACVS small animal surgery residents or diplomates participating in a SASRP during the past 5 years. An online survey was sent to eligible individuals. Respondents anonymously provided information related to demographics, postgraduate internships, and qualifications at the time of each surgical residency application. First attempt and overall match success were evaluated by gender through univariable analysis and then via a multivariable model. Men were 2.89 times more likely to match directly into a SASRP following a rotating internship than women (p = .041), and women completed more total internships than men prior to successful match into a SASRP (p = .030); however, men were found to have more publications at the time of first residency application (p < .001) or successful match (p = .018). Multivariable analysis revealed no association between gender on overall match success or first attempt match rate when considering all other qualifications. No evidence for gender bias was found during the Veterinary Internship and Residency Matching Program (VIRMP) SASRP applicant selection process; however, gender specific patterns in research qualifications were identified. Gender-blinded assessment is not considered necessary for the VIRMP small animal surgical resident selection process. Efforts should be made to educate applicants about the impact of research on the residency selection process and encourage research engagement of female students and graduates.

Explainable automated pain recognition in cats

Manual tools for pain assessment from facial expressions have been suggested and validated for several animal species. However, facial expression analysis performed by humans is prone to subjectivity and bias, and in many cases also requires special expertise and training. This has led to an increasing body of work on automated pain recognition, which has been addressed for several species, including cats. Even for experts, cats are a notoriously challenging species for pain assessment. A previous study compared two approaches to automated ‘pain’/‘no pain’ classification from cat facial images: a deep learning approach, and an approach based on manually annotated geometric landmarks, reaching comparable accuracy results. However, the study included a very homogeneous dataset of cats and thus further research to study generalizability of pain recognition to more realistic settings is required. This study addresses the question of whether AI models can classify ‘pain’/‘no pain’ in cats in a more realistic (multi-breed, multi-sex) setting using a more heterogeneous and thus potentially ‘noisy’ dataset of 84 client-owned cats. Cats were a convenience sample presented to the Department of Small Animal Medicine and Surgery of the University of Veterinary Medicine Hannover and included individuals of different breeds, ages, sex, and with varying medical conditions/medical histories. Cats were scored by veterinary experts using the Glasgow composite measure pain scale in combination with the well-documented and comprehensive clinical history of those patients; the scoring was then used for training AI models using two different approaches. We show that in this context the landmark-based approach performs better, reaching accuracy above 77% in pain detection as opposed to only above 65% reached by the deep learning approach. Furthermore, we investigated the explainability of such machine recognition in terms of identifying facial features that are important for the machine, revealing that the region of nose and mouth seems more important for machine pain classification, while the region of ears is less important, with these findings being consistent across the models and techniques studied here.

Waste not want not: piloting a clinical waste audit at a United States university veterinary teaching hospital.

Biomedical waste is a well-recognized environmental problem, yet less is known about the waste generated in the delivery of veterinary care compared to human medicine. The objective of this project was to develop and pilot a waste audit protocol for veterinary medicine that could inform waste management at a US university veterinary teaching hospital and the broader veterinary community. We conducted a multi-day review of the Colorado State University Veterinary Teaching Hospital's small animal surgery and anesthesia units to measure the types and amounts of waste generated during routine surgeries. Metrics included total weight, number of bags, and individual counts for specific items of concern and items with sustainable alternatives. We calculated frequencies and percentages of waste by waste audit material category and noted any erroneous materials sorted. Despite waste being a prioritized sustainability issue in veterinary medicine, this work highlighted opportunities for better education on managing and optimizing existing resources through behavior modification. This article explores ways the 5Rs (Rethink, Reduce, Reuse, Recycle, Research) could be better operationalized in veterinary hospitals.

A randomized crossover study of the effect of butorphanol-lidocaine and tramadol-lidocaine on sevoflurane's minimum alveolar concentration in dogs.

Inhalational anesthesia is routinely used in small animal surgery. Selecting a suitable drug combination is vital since it may negatively affect the patient's physiological condition. We conducted this study to examine the sparing effect of butorphanol-lidocaine (BUT-LID) and tramadol-lidocaine (TRM-LID) on sevoflurane's minimum alveolar concentration (MAC) in 10 healthy mongrel dogs aged 1-2 years and weighing 11.5 ± 0.8 kg (mean ± SD). Sevoflurane's MAC was measured on three separate occasions. The three dog treatment groups were control (CONT) anesthetized only with sevoflurane, TRM-LID (TRM, i.v. 1.5 mg kg-1, then 1.3 mg kg-1 h-1 and LID, i.v. 2 mg kg-1, then 3 mg kg-1 h-1) or BUT-LID treatment (BUT, i.v. 0.1 mg kg-1 then 0.2 mg kg-1 h-1 and LID, i.v. 2 mg kg-1, then 3 mg kg-1 h-1). We hypothesized that both TRM-LID and BUT-LID would result in a significant MAC sparing effect in healthy dogs. The TRM-LID treatment resulted in a non-significant MAC reduction. MAC was lowered significantly in the BUT-LID group (p = 0.009). The sevoflurane MAC-sparing effects of TRM-LID and BUT-LID treatments were 7.05 ± 22.20 and 19.90 ± 5.91%, respectively, a difference that was not statistically significant (p = 0.13). Bradycardia was observed in the TRM-LID (p < 0.001) treatment. The esophageal temperature was significantly higher for the TRM-LID treatment than the CONT (p < 0.001) treatment. No statistically significant changes were detected between the three groups in f R, Pe'CO2, and MABP. In conclusion, there was a significant sparing effect after adding BUT-LID co-infusion than the control group. No sparing effect was noticed when adding TRM-LID co-infusion. However, no difference in the MAC sparing percentages between the TRM-LID and BUT-LID treatments. The BUT-LID co-infusion resulted in a sevoflurane MAC reduction superior to TRM-LID in addition to minimal cardiorespiratory changes. Both BUT-LID and TRM-LID may be clinically beneficial to dogs during anesthesia. However, BUT-LID produced higher sparing effect and reduction of sevoflurane MAC value.

The Use of Waterless Alcohol-based Antiseptic for Surgical Skin Preparation in Rhesus Macaques (Macaca mulatta).

Ensuring asepsis of the surgical site before surgery is an essential component of safe surgical practices to reduce the incidence of surgical site infections in veterinary medicine. The current accepted method of skin preparation is a multistep process that alternates either a povidone-iodine or chlorhexidine soap scrub with a 70% alcohol rinse. After cleansing, the site is left to dry before draping. The goal of this study was to assess the effectiveness of a waterless alcohol-based (WAB) antiseptic as part of a 2-step procedure after the soap scrub. WAB antiseptics are commonly used as a presurgical hand scrub for the surgeon as they evaporate quickly and provide effective antisepsis. Previous studies have examined the WAB antiseptics in small animal surgeries. We tested this approach in large animal surgery. Twenty-four rhesus macaques were divided into 4 groups that received one of the following treatments: saline and alcohol, iodine-alcohol-iodine, soap scrub/WAB, and chlorhexidine-alcohol-chlorhexidine. The surgical site was swabbed before and after treatment and plated to assess sterility. Overall, no colonies were recovered from skin treated with WAB antiseptic, establishing it as an effective alternative to the current standard protocol. This method will simplify the current 3-step procedure and reduce animal handling, the use of materials, and the time necessary for surgical preparation.

Surgical management of a traumatic lateral hernia involving the spleen in a cat: A case report

Traumatic body wall hernias are characterized by protrusion of organs from a body cavity via a traumatically created defect. A one year old, male, Persian cat was presented to the Small Animal Surgery - Out-patient unit of Madras Veterinary College, Chennai, with a history of being attacked by a dog the previous day. On physical examination, a soft, painless, reducible mass with a hernial ring was palpable on the left lateral abdomen. On clinical examination, the pet was normal in habits, therefore, surgical repair of the hernia was opted. A skin incision was made over the mass and the subcutaneous tissue was undermined. The herniated organs were diagnosed as the spleen with omentum. Adhesions between the spleen and the hernial ring were relieved and the spleen was exteriorized to assess vitality. The spleen and omentum were replaced back into the abdominal cavity as no abnormalities were evident. Herniorrhaphy was chosen over hernioplasty. The animal recovered uneventfully and sutures were removed 10 days post-operatively.

Serological Evidence of SARS-CoV-2 Infection in Companion Animals in Pakistan: A Cross-sectional Survey

Background Coronaviruses infect both humans and animals. One of these viruses, SARS-CoV-2, associated with the COVID-19 pandemic, is believed to be zoonotic in nature with its origins from live animal markets in Wuhan, China. The virus has been reported to transmit from humans to other animals and within animal species both naturally and experimentally in many countries. Being a novel virus, studies are needed to understand how it affects animals, possible spread to humans, and other potential roles in the epidemiology of COVID-19 following a one health approach. Objective Therefore, we conducted a cross-sectional sero-survey among companion animals (pet dogs and cats) in twin cities of Islamabad and Rawalpindi, Pakistan with the aim to determine the seroprevalence of COVID-19 among these animals. Methods The study was conducted from November 2020 to April 2021, coinciding with the end of the second wave and the peak of the third wave of COVID-19 in Pakistan. We collected serum samples from 83 animals (68 dogs and 15 cats) from 10 small animal clinics and surgeries in twin cities. The data were collected on the species, age, sex, clinical history, travel history, and confirmation of COVID-19 among the owners or their families, presented with various clinical histories. The samples were tested by ID Screen SARS-CoV-2 double antigen multispecies enzyme-linked immunosorbent assay developed by ID.Vet, France. Results A combined seroprevalence of COVID-19 among these companion animals of 7.23% (6/83; 95% CI 2.7%-15.07%) was found. Further, the seroprevalence of COVID-19 among dogs and cats were 7.35% (5/68; 95% CI 2.43%-16.33%) and 6.67% (1/15; 95% CI 0.17%-31.95%), respectively. Both species were found to be similarly susceptible to COVID-19 (odds ratio 1.11, 95% CI 0.12-10.27; P=.92). Animals in families with a history of COVID-19 among owners were more likely to be seropositive for SARS-CoV-2 (odds ratio 11.8, 95% CI 1.93-71.89; P=.002). Conclusions The results suggest a possible transmission of SARS-CoV-2 from pet owners to their companion animals. However, further studies may be needed to evaluate this hypothesis and the role of pets as potential reservoirs for SARS-CoV-2 infection for humans and other animals.

Transitional Meningioma in a Golden Retriever Dog - A Case Report

Meningiomas are the common central nervous system tumors seen in dogs. There are nine histological subtypes of meningioma are reported so far. A 7-year-old male golden retriever dog presented to Small Animal Medicine and Surgery Unit with a history of progressive paralysis of all four limbs and enophthalmos of the left eye along with depressed left frontal region. Computerized Tomography scan of skull showed enlarged rostral alar foramen. Animal collapsed suddenly and brought for post mortem to Department of Veterinary Pathology, Madras Veterinary College, Chennai. Muscle atrophy noticed in left frontal area and examination of cranial cavity revealed a well encapsulated gray white mass noticed in the brain stem region. Cytological examination revealed the presence of epithelial and mesenchymal cells arranged in loose clusters as well as in pseudo palisading pattern in some areas. Cytological features were suggestive of meningioma. Histopathology revealed the presence of spindle shaped cells arranged in loose clusters and whirling pattern along with meningothelial cells arranged in fascicles which is suggestive of transitional type of meningioma. Positive histochemical staining with Masson trichome observed. For the confirmatory diagnosis immunohistochemical staining with cytokeratin, vimentin and epithelial membrane antigen was performed which is commonly used in human meningioma cases. Moderate cytoplasmic expression of cytokeratin and vimentin noticed in neoplastic cells. Immunohistochemical examination with epithelial membrane antigen showed moderate positive reaction by meningothelial as well as fibroblast cells. Based on the above findings the present case was diagnosed as transitional meningioma.

Applications of 3-dimensional printing in small-animal surgery: A review of current practices.

Three-dimensional (3D) printing, also called rapid prototyping or additive manufacturing, transforms digital images into 3D printed objects, typically by layering consecutive thin films of material. This technology has become increasingly accessible to the public, prompting applications in veterinary surgery. Three-dimensional prints provide direct visualization of complex 3D structures and also haptic feedback relevant to surgery. The main objective of this review is to report current applications of 3D printing in small-animal surgery, including surgical education, preoperative planning, and treatment of tissue defects. The reported uses of 3D prints, their proposed advantages, and current limitations are discussed considering published evidence. Aspects of the manufacturing process specific to each application are described, along with current practices in veterinary surgery.

Real-time analysis of intraoperative delays and variations in intraoperative workflow with level of experience of the primary surgeon in small animal surgery.

To describe the causes of intraoperative delays and the changes in delays and surgical workflow with the level of training of the primary surgeon. Prospective observational study. Seventy-three small animal surgical procedures performed at an academic teaching institution between January 17, 2018 and April 10, 2018. Procedures (trainee=37, faculty=36) totaling 103.2h were observed and video recorded. Operative time was allocated to the surgical approach, exploration, exposure, intervention, and closure phases. Suballocations were made to specific tasks within these categories (such as cutting or hemostasis). Intraoperative delays and reasons were recorded. Differences in use of time between trainee and faculty surgeons were analyzed. Delays constituted 9.2% (±4.4) of the operative time, of which 6.5%(±3.6) were surgeon controlled and 2.6% (±2.7) were non-surgeon controlled. Surgeons preparing instrumentation outside of the operative field and retrieval of equipment from supply areas were the greatest contributors to delays. Intraoperative delays did not increase when the trainee was placed in the primary surgeon role (P= .78). During the approach faculty surgeons spent proportionally less time on hemostasis (P= .02), and during closure they spent less time suturing (P= .03) than trainees. Trainee surgeons did not have greater intraoperative delays. Delays were created when surgeons prepared their own instrumentation. Workflow differed between experts and trainees. Advancing a trainee surgeon into the primary role is unlikely to increase intraoperative delays, which can be reduced by the inclusion of trained scrub technicians. A focus on efficient hemostasis and fluidity when suturing may improve operative efficiency for surgical trainees.

Occupational segregation by gender in veterinary specialties: Who we are choosing, or who is choosing us.

Names for matched and unmatched residents with addresses in the United States or Canada were coded for gender for seven programs: large and small animal surgery, large and small animal medicine, cardiology, neurology, and oncology. Match rate by gender was compared using chi-square tests. Gender demographics of applicants were compared to demographics of graduates using tests of two proportions. No differences were observed between genders for the likelihood of successfully matching into each residency program evaluated except in large animal internal medicine. Women (44.2%) were slightly more likely to match, overall, than men (39.0%, p=.003). The proportions of women applying for residencies overall (70.7%), in large and small animal surgery (66.1%, 62.2%), cardiology (70.2%), and neurology (70.7%) were lower than the proportion of female graduates (79%; p's < .001). No evidence for gender bias was detected in the VIRMP resident selection process. Female veterinary graduates seemed less likely to apply for residencies than their male counterparts. Occupational segregation seems to stem from the decision to apply for residency. Interventions aimed at altering gender demographics in specialized medicine should target potential applicants.
