• The goal of locoreginal treatment has converted from R0 resection to achieve no evidence of disease (NED). • Patients are initially NED-eligible or non-NED-eligible depending on the technical feasibility of locoreginal therapy to achieve the goal of NED. • Only patients who are technically and biologically NED-eligible will benefit most in locoreginal treatment. • Neoadjuvant/transformational therapy and postoperative adjuvant therapy are selected based on the patient's risk factors. More than half of the colorectal cancer patients develop liver metastases in the disease course, affecting at least 900,000 people worldwide each year. Regarding this clinical scenario, the treatment modality combining locoregional therapy with systemic therapy has been widely accepted. However, the heterogeneity of patients with colorectal cancer liver metastases (CRCLM) has made it tough choices between different treatment plans. We searched MEDLINE, EMBASE, Cochrane Library and Google Scholar by 2021 June for publications on CRCLM. Studies include original articles and reviews. Physicians has to assess the factors including the site of primary colorectal cancer (CRC), RAS gene status, number and location of intrahepatic metastases, whether they are combined with extrahepatic metastases, and the general condition of the patient. Patients initially able to achieve the state of technically no evidence of disease (NED), locoregional treatment is recommended. Neoadjuvant therapy is applied to patients with risk factors indicating more aggressive biological behavior. All patients are indicated to receive adjuvant chemotherapy after surgery. For patients with initially non-NED-eligible state, conversion therapy is recommended, and multidisciplinary team (MDT) assessment should be arranged to comprehensive decide the subsequent treatment. If technically and biologically eligible, locoregional therapy is indicated to improve the oncological outcome. Based on technical factors and biological behavior, NED-eligible or non-NED-eligible patients are referred to different treatment goals and modalities.
