Infant growth and body composition from birth to 24 months: are infants developing the same?

Given the importance of infancy for establishing growth trajectories, with later-life health consequences, we investigated longitudinal body composition among infants from six economically and ethnically diverse countries. We recruited mother-infant dyads using the WHO Multicenter Growth Reference Study criteria. We measured fat-free mass (FFM) in 1393 (49% female) infants from birth to 6 months of age (Australia, India, and South Africa; n = 468), 3-24 months of age (Brazil, Pakistan, South Africa, and Sri Lanka; n = 925), and derived fat mass (FM), fat mass index (FMI), and fat-free mass index (FFMI). Height-for-age (HAZ), weight-for-age (WAZ), and weight-for-length (WHZ) Z-scores were computed. Sex differences were assessed using a t-test, and country differences using a one-way analysis of covariance. We further compared subsamples of children with average (-0.25 > HAZ < +0.25), below-average (≤-0.25) and above-average (≥+0.25) HAZ. HAZ performed well between 0 and 6 months, but less so between 3 and 24 months. The stunting prevalence peaked at 10.3% for boys and 7.8% for girls, at 24 months. By 24 months, girls had greater FMI (10%) than boys. There were significant differences in FFM (both sexes in all countries) and FM (Brazilian boys, Pakistani and South African girls) by 24 months of age between infants with average, above-average, and below-average HAZ. In a multi-country sample representing more ideal maternal conditions, body composition was heterogeneous even among infants who exhibited ideal length. Having a mean HAZ close to the median of the WHO standard for length reduced FFM between-country heterogeneity but not FM, suggesting that other factors may influence adiposity.

PSAT108 Impact of recombinant human growth hormone (rhGH) on skeletal muscle strength, power, endurance and agility in prepubertal boys with significant short stature: preliminary results

Abstract Objectives To investigate the impact of rhGH on measures of skeletal muscle strength, power, endurance, agility, and lean body mass accrual in prepubertal boys with significant short stature compared to age-matched healthy controls growing normally. Methods 40 prepubertal boys were recruited, 25 with significant short stature -either isolated GH deficiency or idiopathic (mean (SE) age: 8.9±0.3yrs; HT SDS: -2.3±0.1) and 15 normally growing healthy controls (age: 8.8±0.4yrs; HT SDS: -0.3±0.2). All had testing at baseline including anthropometry, blood, DEXA scan (body composition), and indirect calorimetry (resting energy expenditure (REE)). Protocolized muscle function tests were performed by expert personnel including measures of skeletal muscle strength (isokinetic dynamometry (flexion/extension) upper and lower extremities), power (vertical jump), endurance (modified push-ups), and agility (timed shuttle-runs). Boys with short stature were started on daily rhGH (Norditropin®, 0.3mg/kg/w) and baseline studies repeated identically in all at 6 and 12 months. Mixed effects model ANOVA (and 2 sample t-test) was used to compare main outcomes over time between groups (significance p ≤0.05). Results Baseline N=40, 6 months N=34, 12 months N=27 participants thus far. All grew well and remained prepubertal throughout the study. At baseline, there were measurable differences in fat free mass (FFM) between groups: short stature: 15.9±0.5kg, controls: 20.4±1.1 (p=0.001); both groups gained FFM at 12 months, short stature group gained 27.3% (p&amp;lt;0.001) vs. controls 10.7% (p&amp;lt;0.001) (p&amp;lt;0.001 between groups). Measures of agility and power were comparable at baseline between groups, agility improved by 12 months 5.5% in the rhGH-treated group (p=0.007) vs. 2.7% in controls (p=0.178) (p=0.157 between groups); measures of power improved 9.1% in the rhGH-treated group (p=0.001) vs. 3.8% in controls (p=0.386) at 12 months (p&amp;lt;0.258 between groups). There were significant differences between groups in muscle strength at baseline, lower particularly in elbow flexion (biceps) in the short stature group: 79.7±3.7 Newtons vs. controls: 97.6±5.5 (p=0.008), and knee flexion (hamstrings): 82.6±3.4 Newtons vs. 95.7±3.2 (p=0.013). At 12 months the rhGH-treated group improved biceps strength (25.7%, p&amp;lt;0.001), vs. controls (12.5%, p=0.008) (p=0.019 between groups); whereas hamstrings strength improved comparably at 12 months in the rhGH-treated group (24.1%, p=0.008) vs. controls (28.0%, p=0.01) (p=0.79 between groups). Measures of endurance (modified push-ups) at baseline were 17.5±1.5 reps in the short stature group vs. 24.9±3.3 in controls (p=0.057), improving in the rhGH-treated (63%, p&amp;lt;0.001) vs. controls (43%, p=0.07) at 12 months (p=0.959 between groups). There were no significant differences in REE (kcal/kg/d) between groups during 12 months. Conclusion There are significant differences in FFM and skeletal muscle strength and endurance in prepubertal boys with severe short stature compared to healthy age-matched controls. FFM accrual and isokinetic measures of muscle strength, endurance, agility and power improve after 12 months of rhGH, changes approaching measures of those of healthy controls. BODY COMPOSITION AMONG UNIVERSITY FEMALE ATHLETES OF TEAM SPORTS

ABSTRACT Introduction: The assessment of body composition in female athletes of different sports is important for health monitoring. Objective: To compare body composition in university athletes of different team sports (indoor soccer, flag football and volleyball). Methods: A cross-sectional study carried out with 45 female athletes, aged 18 to 35 years (22.8 ± 3.55). The dependent variables were body fat mass (FM) and fat-free mass (FFM) measured by air displacement plethysmography. Bone mineral content, adjusted for height (BMC/height) and bone mineral density (BMD), were measured by dual energy X-ray absorptiometry. The independent variable was sport [flag football (n = 12); indoor soccer (n = 20); volleyball (n = 13)] and the covariates were age (complete years), training volume (minutes per week) and length of time playing the sport (complete years). Analysis of covariance was used. Results: Adjusting the model for covariates, volleyball athletes (19.27 kg ± 2.20) presented higher FM values compared to the flag football (16.00 kg ± 1.70) and indoor soccer players (12.20 kg ± 1.30). There was no significant difference in FFM, BMC/height and total BMD between sports, even after adjusting for covariates. Conclusion: Volleyball athletes presented higher FM compared to flag football and indoor soccer athletes. There were no differences in FFM, BMC/height and BMD among the players of the different team sports. This study can help coaches and other sports professionals to prevent injuries to athletes in sports such as higher FM (volleyball), or to prevent diseases such as menstrual irregularities, which are common in athletes who may have low levels of body fat (indoor soccer players), this being one of the risk factors for the female athlete triad (eating disorders, menstrual irregularities and low BMD). Suppression of Insulin Secretion in the Treatment of Obesity: A Systematic Review and Meta-Analysis.

This proof-of-concept study aimed to evaluate the efficacy and safety of suppression of insulin secretion in the treatment of obesity. A search of PubMed, Embase, and Cochrane databases was performed to identify randomized controlled trials (up to January 1, 2020) that used drugs that directly suppress insulin secretion (diazoxide or octreotide) in the treatment of obesity. The extracted data were analyzed using random-effects meta-analysis. A total of seven randomized controlled trials were included, with four using diazoxide and three using octreotide to suppress insulin secretion. Suppression of insulin secretion significantly reduced fasting insulin level (mean difference: -3.94 mIU/L; 95% CI: -7.40 to -0.47) but slightly increased fasting blood glucose level (mean difference: 0.48 mmol/L; 95% CI: 0.24 to 0.72). Following the suppression of insulin secretion, significant reductions in body weight (mean difference: -3.19 kg; 95% CI: -5.71 to -0.66), BMI (mean difference: -1.65 kg/m2 ; 95% CI: -2.41 to -0.90), and fat mass (mean difference: -5.92 kg; 95% CI: -8.28 to -3.56) were observed compared with placebo in the pooled data. No significant difference in fat-free mass was observed (mean difference: 0.56 kg; 95% CI: -0.40 to 1.52). Results suggest that suppression of insulin secretion may lead to reduced body weight and fat mass with slightly increased blood glucose in individuals with obesity.

Body composition of children with cancer

Nutritional status, as represented by body composition, is an important consideration in the treatment of pediatric cancer patients because it is linked to poor outcomes. Little is known about how a child's body composition responds to cancer and treatment. We aimed to compare the body composition of children undergoing treatment of cancer with that of healthy controls and to compare body composition between children with hematologic malignancies and children with solid tumors. This cross-sectional study measured height, weight, body cell mass, fat-free mass, and fat mass in 48 children undergoing treatment of cancer and blood-related disorders and in age-matched healthy controls. Patients with cancer had a significantly lower body cell mass index z score (body cell mass/height raised to the power of 2.5 for females and 3 for males) than did controls (P = 0.0001), and 45% of the patients with cancer were considered malnourished according to body cell mass. Subjects with cancer had a significantly higher percentage of body fat (P = 0.0001) and fat mass (P = 0.0001) than did controls; however, there was no significant difference in fat-free mass (P = 0.09). On the basis of percentage fat, 77% of subjects with cancer were considered obese. No difference in body composition was observed between cancer types. This study showed that children undergoing treatment of all types of cancer have a significantly lower body cell mass and a significantly higher fat mass than do healthy controls. Nutritional support is suggested for all children undergoing treatment of cancer.

Protein supplementation during 6 mo weight loss enhances body composition changes in older women

Adverse increases in adiposity and reductions in fat free mass (FFM) are age related changes in body composition and are linked to declines in physical function in older women. Higher protein intake in weight loss may augment fat mass (FM) loss and attenuate FFM loss, which may benefit function. This study assessed effects of 6 mo weight loss supplemented with powdered protein or placebo on body composition and function in older obese women. Women (63.8±3.4 yrs, BMI 32.9±4.1 kg/m2) were prescribed an energy‐restricted diet (1500 kcal/d; 15, 65, 30% of energy from protein, CHO and fat, respectively) and randomly assigned to 2 × 25g/d of whey protein (PRO, n=8) or maltodextrin (CARB, n=7) supplementation. Measures included whole body FM and FFM with DXA; thigh muscle, subcutaneous (SUBQ), intermuscular and intramyocellular fat with MRI; knee extension and flexion strength with isokinetic dynamometry. PRO trended towards greater weight loss (−10.6±6.7%, −5.0±4.0%; p=0.078), had greater reductions in FM (−18.9±10.6%, −8.0±4.2%; p=0.024) than CARB, but no significant difference in FFM (−4.0±4.9%, −2.0±4.1%; p=0.414). %SUBQ thigh fat change (−6.1±3.0%, −1.3±1.7%; p=0.003) was the only significant MRI outcome. Groups did not differ in leg strength changes. Higher protein intake in weight loss in older women enhances FM reductions while preserving FFM; however, these changes do not impact changes in physical function.Grant Funding Source: National Dairy Council, Carraway Foundation

Fat free mass index as parameter for nutritional status in non-dialysis chronic kidney disease patients stage 3, 4 and 5

The aim of this study is to obtain body composition parameters for early detection of PEM in non dialysis CKD (ND-CKD) patients. The study was carried out using the cross sectional design. The subjects of the study consist of 45 ND-CKD patients and 45 healthy subjects matched for age, gender, height and body mass index (BMI). The nutritional status of patients and healthy subjects were classified based on BMI (WHO, 1995) into low, normal and high nutritional status groups. Fat free mass (FFM), FFM-index (FFM-I) and percentage of fat mass (FM percentage) in patients measured by anthropometric technique showed no significant difference with healthy subjects. Using the BIA method, FFM and FFM-I were significantly lower in the ND-CKD patients compared to the healthy subjects (p &lt; 0,05). Significant difference in FFM, FFM-I, FM and FM percentage was observed between the patients with different nutritional status. (p &lt; 0,001). Trend analysis statistical test showed that there is linear correlation of FFM, FFM-I and FM with nutritional status classification. FFM, FFM-I, FM and FM percentage in ND-CKD patients were not significantly different between the three stages of CKD. There was an acceptable degree of agreement between BMI with FFM-I for nutritional assessment in ND-CKD patients. The Receiver Operating Curve test showed the cut off points of FFM-I 14.23 kg/m2 to differentiate undernutrition and normal nutritional status in ND-CKD patients. This study showed FFM-I has good correlation with BMI and can be used to differentiate degrees of nutritional status in stage 3, 4 and 5 ND-CKD patients. Analyse longitudinale des variations de la composition corporelle et du métabolisme de base d'adolescents obèses pris en charge en institution ou à domicile

The prevalence of obesity in children has increased dramatically during the past decades, and requires efficient care. Objectives. – To determine changes in anthropometric parameters and basal metabolic rate (BMR) in obese adolescents during and after 2 types of weight-reduction programs. Subjects and methods. – Twenty-six adolescents (group I, Z-score of BMI = 4.72) followed a 9-month-weight reduction program including a moderate energy restriction and regular physical activities in a specialized institution. In addition, 39 adolescents (group E, Z-score of BMI = 2.83) followed at home a 9-month-weight reduction program including medical and dietetic advices. Body composition (by impedancemetry) and BMR (by indirect calorimetry) were assessed before the beginning (M0), 4 months after (M4) and at the end (M9) of the programs, then 4 months (M13) and 16 month (M25) after the end of the weight-reduction programs. Results. – Twenty-two adolescents in group I and 20 adolescents in group E completed the study. At M0, age, body weight (BW), fat-free mass (FFM) and BMR of subjects of group I were higher (1.0 year, 36 %, 30 % and 23 %, respectively, P < 0.001), than those of group E. Pubertal stage and percentage of fat mass (FM) were not significantly different between the 2 groups. At M9, adolescents of group I showed significant reductions in BW and FM (–19 and –37 %, respectively, P < 0.001), but not significant differences in FFM. In addition, BMR decreased significantly between M0 and M4, both in absolute value (–6.7%, P < 0.001) and after adjustment for FFM (–5.8%, P < 0.001), and the difference was maintained until M9. Between M9 and M13, BW and Z-score of BMI were maintained in 12 adolescents, but increased (+9.7% and 14.8%, respectively, P < 0.001) in 10 adolescents. However, BMR did not change significantly in all adolescents. Between M13 and M25, BW, FM, FFM and BMR increased significantly (+ 13%, + 34%, + 6% et + 5%, respectively, P < 0.001). During the 25 month period, adolescents of group E showed significant increases in BW, FFM and BMR (+ 8%, + 14% and + 10%, respectively, P < 0.001), and maintained their Z-score and FM. Conclusion. – The reduction in BMR during the weight-reduction program at the institution could contribute to body weight regain in the post-obese adolescents if they do not maintain the lifestyle habits taught during the weight-reduction period. In other respects, Z-score was stabilized in 51% of domiciled obese adolescents.

Body Cell Mass Measured by Total Body Potassium in Normal-Weight and Obese Men and Women

Objective: Total body potassium (TBK) concentration is linearly correlated with the size of the body cell mass (BCM). The aim of this study was to compare BCM in normal-weight and obese individuals.Methods: 271 individuals (207 males, 64 females), 20 to 67 years of age, participated in this study. Subjects were separated by body mass index (BMI): BMI < 25 kg/m2 (BMI-L) and BMI > 25 kg/m2 (BMI-H). 40K was assessed using a whole-body counter and BCM was calculated.Results: BCM and TBK were significantly greater in men, with a trend to be greater in women in BMI-H compared to men in BMI-L. TBK/body weight was significantly lower, while TBK/height was significantly greater for men and women in BMI-H compared to men and women in BMI-L. Fat-free mass (FFM) was significantly greater for men in BMI-H, with no significant differences in FFM between the two groups of women.Conclusions: The healthy obese subjects in the present study had a greater BCM than the non-obese subjects. These results indicate that it is important to assess BCM in obese individuals because it could influence the type of weight loss regimen that will be used in order to preserve BCM.
