Changes in Structural Neural Networks in the Recovery Process of Motor Paralysis after Stroke.

In recent years, neurorehabilitation has been actively used to treat motor paralysis after stroke. However, the impacts of rehabilitation on neural networks in the brain remain largely unknown. Therefore, we investigated changes in structural neural networks after rehabilitation therapy in patients who received a combination of low-frequency repetitive transcranial magnetic stimulation (LF-rTMS) and intensive occupational therapy (intensive-OT) as neurorehabilitation. Fugl-Meyer assessment (FMA) for upper extremity (FMA-UE) and Action Research Arm Test (ARAT), both of which reflected upper limb motor function, were conducted before and after rehabilitation therapy. At the same time, diffusion tensor imaging (DTI) and three-dimensional T1-weighted imaging (3D T1WI) were performed. After analyzing the structural connectome based on DTI data, measures related to connectivity in neural networks were calculated using graph theory. Rehabilitation therapy prompted a significant increase in connectivity with the isthmus of the cingulate gyrus in the ipsilesional hemisphere (p < 0.05) in patients with left-sided paralysis, as well as a significant decrease in connectivity with the ipsilesional postcentral gyrus (p < 0.05). These results indicate that LF-rTMS combined with intensive-OT may facilitate motor function recovery by enhancing the functional roles of networks in motor-related areas of the ipsilesional cerebral hemisphere.

Impact of changes in river network structure on hydrological connectivity of watersheds

River network connectivity is a key factor affecting watershed ecosystems and an important criterion for evaluating the health of river. The river network structure of the basin has changed dramatically in recent years due to human activity, but the response of connectivity to changes in physical structure is still poorly understood. In this paper, a dynamic index (connectivity index) based on daily-scale flow and probability density function is proposed to characterize the connectivity capacity of river networks. The Haihe River basin in China is selected as the study area, and a graphic model is constructed to assess the evolutionary structure of the river network. The 54 sections were classified into Low human impact (LHI) and High human impact (HHI), and the trends and causes of connectivity changes of the sections with the evolution of the network were analyzed. The results reveal that the river network degree and clustering coefficient keep increasing, the path length decreases and becomes more dense downstream, and the river network gradually evolves into a “small-world” network. The “small-world” characteristics improve the potential efficiency and specific functionality of the river network, but make the physical structure mismatch with the hydraulic gradient, and the network flow disperses severely, leading to a significant decrease in connectivity (HHI connectivity index is lower than LHI 52.1%). Moreover, the structural changes enhance the spatial and temporal differences in hydrological connectivity, which further restricts the function of the river system. “Small-world” and connectivity indicators (duration, start time, and intermittency) show strong correlations. Therefore, physical structure is an important factor influencing connectivity capacity of river networks. The planning and management of the watershed should consider the changes in hydrological connectivity caused by the structural modifications of the river network.

831 Pain Networks Connectivity Changes Following the Relief of Prolonged Cancer Pain

INTRODUCTION: The “salience network” is normally engaged in humans’ response to acute internal or environmental stimuli, including pain. It comprises connections between the anterior insula (AI) and dorsal anterior cingulate cortex (dACC), among others. Chronic pain conditions have been associated with abnormally increased functional connectivity (FC) within the salience network in comparison to healthy individuals. METHODS: We prospectively followed patients that underwent cervical cordotomy (n=15) and cingulotomy (n=11) for relief of intractable oncological pain during the years 2016-2019. All patients did resting-state functional Magnetic Resonance Imaging (rsfMRI) before and after the procedures. Clinical data was collected, including pre- and post-operative numeric rating scale (NRS) pain scores. RESULTS: All patients (aged 62±11) reported a significant improvement in their pain from NRS 9±1 to 1±1 in cordotomy (p=0.001) and 2±3 in cingulotomy (p=0.005). By comparing pre- and post-operative rsfMRI scans, we demonstrated a significant decrease in the FC between components of the salience network following both cordotomy (p=0.007) and cingulotomy (p=0.009). In the cordotomy group we found a significant decrease in the connectivity between the thalamus and dACC (p=0.019) and within the sensory-motor cortex (p=0.003). CONCLUSIONS: Surgical interventions along the pain pathways can lead to a sudden relief of chronic pain in parallel to a significant decrease in the functional connectivity within pain related networks.

Functional connectivity underlying cognitive and psychiatric symptoms in post-COVID-19 syndrome: is anosognosia a key determinant?

Lack of awareness of cognitive impairment (i.e. anosognosia) could be a key factor for distinguishing between neuropsychological post-COVID-19 condition phenotypes. In this context, the 2-fold aim of the present study was to (i) establish the prevalence of anosognosia for memory impairment, according to the severity of the infection in the acute phase and (ii) determine whether anosognosic patients with post-COVID syndrome have a different cognitive and psychiatric profile from nosognosic patients, with associated differences in brain functional connectivity. A battery of neuropsychological, psychiatric, olfactory, dyspnoea, fatigue and quality-of-life tests was administered 227.07 ± 42.69 days post-SARS-CoV-2 infection to 102 patients (mean age: 56.35 years, 65 men, no history of neurological, psychiatric, neuro-oncological or neurodevelopmental disorder prior to infection) who had experienced either a mild (not hospitalized; n = 45), moderate (conventional hospitalization; n = 34) or severe (hospitalization with intensive care unit stay and mechanical ventilation; n = 23) presentation in the acute phase. Patients were first divided into two groups according to the presence or absence of anosognosia for memory deficits (26 anosognosic patients and 76 nosognosic patients). Of these, 49 patients underwent an MRI. Structural images were visually analysed, and statistical intergroup analyses were then performed on behavioural and functional connectivity measures. Only 15.6% of patients who presented mild disease displayed anosognosia for memory dysfunction, compared with 32.4% of patients with moderate presentation and 34.8% of patients with severe disease. Compared with nosognosic patients, those with anosognosia for memory dysfunction performed significantly more poorly on objective cognitive and olfactory measures. By contrast, they gave significantly more positive subjective assessments of their quality of life, psychiatric status and fatigue. Interestingly, the proportion of patients exhibiting a lack of consciousness of olfactory deficits was significantly higher in the anosognosic group. Functional connectivity analyses revealed a significant decrease in connectivity, in the anosognosic group as compared with the nosognosic group, within and between the following networks: the left default mode, the bilateral somatosensory motor, the right executive control, the right salient ventral attention and the bilateral dorsal attention networks, as well as the right Lobules IV and V of the cerebellum. Lack of awareness of cognitive disorders and, to a broader extent, impairment of the self-monitoring brain system, may be a key factor for distinguishing between the clinical phenotypes of post-COVID syndrome with neuropsychological deficits.

Longitudinal Changes of Resting-State Networks in Children With Attention-Deficit/Hyperactivity Disorder and Typically Developing Children

BackgroundAttention-deficit/hyperactivity disorder (ADHD) is a prevalent childhood neurodevelopmental disorder. Given the profound brain changes that occur across childhood and adolescence, it is important to identify functional networks that exhibit differential developmental patterns in children with ADHD. This study sought to examine whether children with ADHD exhibit differential developmental trajectories in functional connectivity compared with typically developing children using a network-based approach. MethodsThis longitudinal neuroimaging study included 175 participants (91 children with ADHD and 84 control children without ADHD) between ages 9 and 14 and up to 3 waves (173 total resting-state scans in children with ADHD and 197 scans in control children). We adopted network-based statistics to identify connected components with trajectories of development that differed between groups. ResultsChildren with ADHD exhibited differential developmental trajectories compared with typically developing control children in networks connecting cortical and limbic regions as well as between visual and higher-order cognitive regions. A pattern of reduction in functional connectivity between corticolimbic networks was seen across development in the control group that was not present in the ADHD group. Conversely, the ADHD group showed a significant decrease in connectivity between predominantly visual and higher-order cognitive networks that was not displayed in the control group. ConclusionsOur findings show that the developmental trajectories in children with ADHD are characterized by a subnetwork involving different trajectories predominantly between corticolimbic regions and between visual and higher-order cognitive network connections. These findings highlight the importance of examining the longitudinal maturational course to understand the development of functional connectivity networks in children with ADHD.

Brain Network Integrity Changes in Subjective Cognitive Decline: A Possible Physiological Biomarker of Dementia.

Objective: The current study seeks to illustrate potential early and objective neurophysiological biomarkers of neurodegenerative cognitive decline by evaluating features of brain network physiological performance and structure utilizing different modalities.Methods: This study included 17 clinically healthy individuals with self-reported cognitive decline (Subjective Cognitive Decline group, SCD, no objective finding of cognitive decline), 12 individuals diagnosed with amnestic Mild Cognitive Impairment (aMCI), 11 individuals diagnosed with Dementia, and 15 healthy subjects. All subjects underwent computerized cognitive performance testing, MRI scans including T1 for gray matter (GM) volume quantification, DTI for quantification of white matter (WM) microstructure fractional anisotropy (FA) and mean diffusivity (MD), and brain network function evaluation using DELPHI (TMS-EEG) measures of connectivity, excitability, and plasticity.Results: Both DELPHI analysis of network function and DTI analysis detected a significant decrease in connectivity, excitability, and WM integrity in the SCD group compared to healthy control (HC) subjects; a significant decrease was also noted for aMCI and Dementia groups compared to HC. In contrast, no significant decrease was observed in GM volume in the SCD group compared to healthy norms, a significant GM volume decrease was observed only in objectively cognitively impaired aMCI subjects and in dementia subjects.Conclusions: This study results suggest that objective direct measures of brain network physiology and WM integrity may provide early-stage biomarkers of neurodegenerative-related changes in subjects that have not yet displayed any other objective measurable cognitive or GM volume deficits which may facilitate early preventive care for neurodegenerative decline and dementia.

Characterizing resting-state networks in Parkinson's disease: A multi-aspect functional connectivity study.

PurposeResting‐state functional magnetic resonance imaging (Rs‐fMRI) can be used to investigate the alteration of resting‐state brain networks (RSNs) in patients with Parkinson's disease (PD) when compared with healthy controls (HCs). The aim of this study was to identify the differences between individual RSNs and reveal the most important discriminatory characteristic of RSNs between the HCs and PDs.MethodsThis study used Rs‐fMRI data of 23 patients with PD and 18 HCs. Group independent component analysis (ICA) was performed, and 23 components were extracted by spatially overlapping the components with a template RSN. The extracted components were used in the following three methods to compare RSNs of PD patients and HCs: (1) a subject‐specific score based on group RSNs and a dual‐regression approach (namely RSN scores); (2) voxel‐wise comparison of the RSNs in the PD patient and HC groups using a nonparametric permutation test; and (3) a hierarchical clustering analysis of RSNs in the PD patient and HC groups.ResultsThe results of RSN scores showed a significant decrease in connectivity in seven ICs in patients with PD compared with HCs, and this decrease was particularly striking on the lateral and medial posterior occipital cortices. The results of hierarchical clustering of the RSNs revealed that the cluster of the default mode network breaks down into the three other clusters in PD patients.ConclusionWe found various characteristics of the alteration of the RSNs in PD patients compared with HCs. Our results suggest that different characteristics of RSNs provide insights into the biological mechanism of PD.

On the functional connectivity between heart, muscle, and frontal brain cortex during exercise fatigability

We analyze the functional connectivity of the cortico-cardiac-muscular network during muscle fatigability due to exercise. For our experiments, we recruited ten volunteers who performed two cycling routines. Our volunteers were classified according to physical activity level as active or sedentary. First, we measured the electromyography (EMG) signal of the vastus lateralis muscle to find the volunteer’s individualized physical workload capacity at the fatigue threshold. Two to three days later, we measured the volunteers’ EMG, electrocardiography, and electroencephalography (EEG) data during the development of muscle fatigue with the individualized pedaling load previously obtained. We analyzed the EEG to identify the brain rhythm with highest power, then functional connectivity was assessed by computing the pairwise partial directed coherence of our measurements at such rhythm. Finally, we assessed significant changes in functional connectivity from start to finish of the exercise. Our results indicate that the sedentary group showed a significant decrease in connectivity, while the active group showed an increase in left-frontal connectivity. This might be explained in terms of frontal asymmetry phenomena which, for the case of active volunteers, may be a reflection of positive emotional states. Additionally, the active group showed increased self-regulatory processes in heart and muscle, while the sedentary group showed a decreased connectivity from heart to muscle together with a reduction in the muscle’s self-regulation. These last results may be explained by a preference of the body in the sedentary group to attend vital requirements instead of exercise.

Creative Connections: Computational Semantic Distance Captures Individual Creativity and Resting-State Functional Connectivity.

Recent studies of creative cognition have revealed interactions between functional brain networks involved in the generation of novel ideas; however, the neural basis of creativity is highly complex and presents a great challenge in the field of cognitive neuroscience, partly because of ambiguity around how to assess creativity. We applied a novel computational method of verbal creativity assessment-semantic distance-and performed weighted degree functional connectivity analyses to explore how individual differences in assembly of resting-state networks are associated with this objective creativity assessment. To measure creative performance, a sample of healthy adults (n = 175) completed a battery of divergent thinking (DT) tasks, in which they were asked to think of unusual uses for everyday objects. Computational semantic models were applied to calculate the semantic distance between objects and responses to obtain an objective measure of DT performance. All participants underwent resting-state imaging, from which we computed voxel-wise connectivity matrices between all gray matter voxels. A linear regression analysis was applied between DT and weighted degree of the connectivity matrices. Our analysis revealed a significant connectivity decrease in the visual-temporal and parietal regions, in relation to increased levels of DT. Link-level analyses showed higher local connectivity within visual regions was associated with lower DT, whereas projections from the precuneus to the right inferior occipital and temporal cortex were positively associated with DT. Our results demonstrate differential patterns of resting-state connectivity associated with individual creative thinking ability, extending past work using a new application to automatically assess creativity via semantic distance.

All the patients had suprasellar tumor extension with compression of the optic chiasma. We investigated second-level effects by contrasting dummy-encoded covariates representing the effects of the sessions (first vs. second) on RSNs. We corrected for multiple comparisons using a false discovery rate of 0.05 (2-sided). Connectivity between the right and left precentral gyri (motor network) decreased significantly from the first to the second session (P= 0.0002), as did the connectivity between the postcentral gyri (P= 0.009). The same was valid for connectivity between the visual cortices (P= 0.0002). The salience network showed a significant decrease in the connectivity of the anterior part of the cingulate gyrus and insular cortex (P= 0.0001). The default mode network showed a decrease in the connectivity between the posterior part of the cingulate gyrus, parietal, and frontal cortices (P= 0.0002). There was no significant correlation between the reduction in connectivity and dose or duration of anesthesia. Different RSNs could be identified under anesthesia and used for intraoperative brain mapping and remapping during tumor resection. However, RSNs showed a significant decrease in connectivity with the continuation of anesthesia.

Disconnection of the right superior parietal lobule from the precuneus is associated with memory impairment in oldest-old Alzheimer's disease patients

There is a wide range of onset age in Alzheimer's disease (AD). Emerging evidence indicates variation of AD manifestations in oldest-old AD (OOAD); however, the pattern of cognitive dysfunctions remains unclear. We aimed to reveal cognitive performance characteristics and changes in brain functional connectivity in OOAD patients by a resting-state fMRI (rs-fMRI) study. We enrolled AD patients who had been referred to Kyushu University Hospital (KUH) or Sanno Hospital, and classified them into middle-old AD (MOAD) (65–79 years old) and OOAD (≥80 years old) according to the age of onset. Our subjects consisted of 19 OOAD, 17 MOAD, and 8 normal subjects. Cognitive performance was evaluated using Mini Mental State Examination-Japanese (MMSE-J) and Clinical Dementia Rating (CDR). rs-fMRI scanning and independent component analysis (ICA) were performed on Sanno Hospital patients and MOAD vs. OOAD patients were compared. The resulting significant regions were used as seeds for ROI-to-ROI analysis of the KUH dataset. Collectively, MMSE-J delayed recall sub-scores were significantly lower in OOAD patients compared with MOAD patients. ICA of the Sanno Hospital data indicated significant connectivity decrease in the default mode network (DMN) in the OOAD group compared with the MOAD group in the right superior parietal lobule (SPL). ROI-to-ROI analysis of the KUH dataset indicated significant disconnection in the OOAD group of the right SPL from the precuneus (p < 0.01). The functional connectivity from the right SPL to the precuneus was positively correlated with the MMSE-J delayed recall sub-score (p = 0.03) and negatively correlated with the CDR memory sub-scale (p = 0.04). These findings indicate that disconnection between the right SPL and the precuneus may contribute to worse memory capability in OOAD compared with MOAD.

Functional Connectivity of Default Mode Network Subsystems in the Presymptomatic Stage of Autosomal Dominant Alzheimer's Disease.

The default mode network (DMN) could be divided into subsystems, the functional connectivity of which are different across the Alzheimer's disease (AD) spectrum. However, the functional connectivity patterns within the subsystems are unknown in presymptomatic autosomal dominant AD (ADAD). To investigate functional connectivity patterns within the subsystems of the DMN in presymptomatic subjects carrying PSEN1, PSEN2, or APP gene mutations. Twenty-six presymptomatic mutation carriers (PMC) and twenty-nine cognitively normal non-carriers as normal controls (NC) from the same families underwent resting state functional MRI and structural MRI. Seed-based analyses were done to obtain functional connectivity of posterior and anterior DMN. For the regions that showed significant connectivity difference between PMC and NC, volumes were extracted and compared between the two groups. Connectivity measures were then correlated with cognitive tests scores. The posterior DMN showed connectivity decrease in the PMC group as compared with the NC group, which was primarily the connectivity of left precuneus with right precuneus and superior frontal gyrus; the anterior DMN showed significant connectivity decrease in the PMC group, which was the connectivity of medial frontal gyrus with middle frontal gyrus. In the brain regions showing connectivity changes in the PMC group, there was no group difference in volume. A positive correlation was observed between the precuneus connectivity value and Mini-Mental State Examination total score. Functional connectivity within both posterior and anterior DMN were disrupted in the presymptomatic stage of ADAD. Connectivity disruption within the posterior DMN may be useful for early identification of general cognitive decline and a potential imaging biomarker for early diagnosis.

Involvement of the Precuneus/Posterior Cingulate Cortex Is Significant for the Development of Alzheimer's Disease: A PET (THK5351, PiB) and Resting fMRI Study.

Background: Imaging studies in Alzheimer’s disease (AD) have yet to answer the underlying questions concerning the relationship among tau retention, neuroinflammation, network disruption and cognitive decline. We compared the spatial retention patterns of 18F-THK5351 and resting state network (RSN) disruption in patients with early AD and healthy controls.Methods: We enrolled 23 11C-Pittsburgh compound B (PiB)-positive patients with early AD and 24 11C-PiB-negative participants as healthy controls. All participants underwent resting state functional MRI and 18F-THK5351 PET scans. We used scaled subprofile modeling/principal component analysis (SSM/PCA) to reduce the complexity of multivariate data and to identify patterns that exhibited the largest statistical effects (variances) in THK5351 concentration in AD and healthy controls.Findings: SSM/PCA identified a significant spatial THK5351 pattern composed by mainly three clusters including precuneus/posterior cingulate cortex (PCC), right and left dorsolateral prefrontal cortex (DLPFC) which accounted for 23.6% of the total subject voxel variance of the data and had 82.6% sensitivity and 79.1% specificity in discriminating AD from healthy controls. There was a significant relationship between the intensity of the 18F-THK5351 covariation pattern and cognitive scores in AD. The spatial patterns of 18F-THK5351 uptake showed significant similarity with intrinsic functional connectivity, especially in the PCC network. Seed-based connectivity analysis from the PCC showed significant decrease in connectivity over widespread brain regions in AD patients. An evaluation of an autopsied AD patient with Braak V showed that 18F-THK5351 retention corresponded to tau deposition, monoamine oxidase-B (MAO-B) and astrogliosis in the precuneus/PCC.Interpretation: We identified an AD-specific spatial pattern of 18F-THK5351 retention in the precuneus/PCC, an important connectivity hub region in the brain. Disruption of the functional connections of this important network hub may play an important role in developing dementia in AD.

Abnormal connectivity in dorsolateral prefrontal cortex in schizophrenia patients and unaffected relatives

ObjectivesThe aim of this study is to explore connectivity of the left dorsolateral prefrontal cortex (LDLPC) by functional magnetic resonance imaging during resting state, in subjects affected by schizophrenia and unaffected relatives.MethodsWe recruited a group of 29 patients diagnosed with schizophrenia, who were treated with atypical antipsychotics, who are and were clinically stable in the last 6 months and had an illness duration range from 5 up to 15 years. We also recruited a group of 23 unaffected relatives, without history of other mental, neurological or somatic disease and a group of 37 healthy volunteers. No subject in any of the three groups met criteria for substance use disorders.All three groups were clinically evaluated, and a functional magnetic resonance during Resting State was performed.Functional images were reoriented to the first scan, normalized to the MNI EPI template and smoothed with an 8 mm Gaussian kernel, with SPM. The CONN-FMRI Toolbox v1.2 was used to create individual subject seed-to-voxel connectivity maps, to the corresponding seeds of the default mode network.ResultsFig. 1.ConclusionsOur results show a significant increase in connectivity between LDLPC and anterior prefrontal cortex, dorsolateral prefrontal cortex and somatosensory association areas, especially between patients and controls. It is noteworthy to mention that we found a significant decrease in connectivity between LDLPC and supramarginal gyrus, superior temporal gyrus and somatosensory association areas between unaffected relatives and controls.Disclosure of interestThe authors have not supplied their declaration of competing interest.

Functional brain networks in REM sleep behavior disorder

REM sleep behavior disorder (RBD) has a strong association with synucleinopathies. Prior studies of psychometrics have demonstrated subtle abnormalities of task-switching and visuospatial tasks in iRBD, similar to deficits in synucleinopathies. In this study, we used resting state functional connectivity MRI to assess the strength of brain networks and regions involved in attention, executive control, visual, auditory, and motor function. Ten individuals (6 male, 4 female) with polysomnogram-confirmed iRBD were compared to ten age- and sex-matched controls. All underwent brain MRI including two blood-oxygen level dependent (BOLD) sequences acquired resting awake with eyes closed. Following standard pre-processing and regression of noise signals, BOLD time series were extracted from pre-defined regions of interest (ROI). The correlation coefficient between BOLD time series of each pair of ROIs was Fisher-z transformed to obtain normally-distributed data, prior to constructing cross-correlation matrices. Correlation coefficients for ROI-pairs were averaged to assess the connectivity strength of each network. Paired t-tests were used to compare iRBD cases and controls. There were no significant differences between iRBD cases and controls in the strength of the executive control, attention, or default mode networks. However, there was a significant decrease in connectivity between visual cortex and attention network (iRBD 0.06 vs. Control 0.14, p = 0.02). Additionally, the iRBD group had almost no correlation between the visual and primary motor regions, significantly lower than the robust correlation in controls (iRBD 0.08 vs. Control 0.33, p = 0.008). Resting state functional connectivity MRI analysis demonstrates significantly reduced strength of connections between the visual regions and the attention network, and between the visual and motor regions. The strength of canonical cognitive networks—including the default mode, attention, and executive networks—and connectivity of other sensory and motor regions, was not different between iRBD cases and controls. These findings suggest that the decline in visuo-spatial performance in iRBD reflects a specific disconnection of the visual regions from their usual functional networks, rather than a generalized brain dysfunction. Association Between the Basal Ganglia and Large-Scale Brain Networks in Epilepsy

Epilepsy may affect connectivity between the putamen and cortex even during the resting state. Putamen is part of the basal ganglia resting state network (BG-RSN) which is anti-correlated with the default mode network (DMN) in healthy subjects. Therefore, we aimed at studying the functional brain connectivity (FC) of the putamen with the cortical areas engaged in the DMN as well as with the primary somatomotor cortex which is a cortical region engaged in the BG-RSN. We compared the data obtained in patients with epilepsy with that in healthy controls (HC). Functional magnetic resonance imaging (fMRI) was performed in 10 HC and 24 patients with epilepsy: 14 patients with extratemporal epilepsy (PE) and 10 patients with temporal epilepsy (PT). Resting state fMRI data was obtained using the 1.5 T Siemens Symphony scanner. The Group ICA of fMRI Toolbox (GIFT) program was used for independent component analysis. The component representing the DMN was chosen according to a spatial correlation with a mask typical for DMN. The FC between the putamen and the primary somatomotor cortex was studied to assess the connectivity of the putamen within the BG-RSN. A second-level analysis was calculated to evaluate differences among the groups using SPM software. In patients with epilepsy as compared to HC, the magnitude of anti-correlation between the putamen and brain regions engaged in the DMN was significantly lower. In fact, the correlation changed the connectivity direction from negative in HC to positive in PE and PT. The disturbed FC of the BG in patients with epilepsy as compared with HC was further illustrated by a significant decrease in connectivity between the left/right putamen and the left/right somatomotor cortex, i.e. between regions that are engaged in the BG-RSN. The FC between the putamen and the cortex is disturbed in patients with epilepsy. This may reflect an altered function of the BG in epilepsy.

Trabecular bone remodeling: An experimental model

An experimental model, capable of inducing controlled stress fields to the distal femoral metaphyses of large dogs, is presented. This model utilized an implantable hydraulic device incorporating five loading cylinders and platens in direct contact with an exposed plane of trabecular bone. A microprocessor controls the loading characteristics, and finite element models were created to calculate the induced stress and strain fields. The trabecular remodeling response is measured using serial in vivo computed tomography, in vitro microcomputed tomography, and histologic analysis. The results of the experiment indicate that significant remodeling can be induced by the activated implant. An increase in trabecular orientation toward the loaded platens was observed, and a statistically significant decrease in connectivity was documented. The greatest effect was associated with a change in the loading rate. A fast rise time (70 ms) loading waveform induced significant bone ingrowth at the implant interface when compared to a slow rise time waveform (700 ms), and demonstrated high correlations with the calculated stress fields as remodeling approached an equilibrium state.
