A development and validation of the polish version of sexual communication self-efficacy scale in women

ABSTRACT Self-efficacy in sexual communication is essential for open conversations about sexual topics with partners, children, or medical providers. The Sexual Communication Self-Efficacy Scale was developed (SCSES) and validated in the Polish women population by analysing its reliability, validity, and factor structure. 204 Polish-speaking adult women were recruited to validate the tool. The reliability coefficient Cronbach’s, exploratory, and confirmatory factor analysis were performed. Moreover, the factor structure was investigated by combining it with the short version of the Female Sexual Function Index (FSFI-6). The adaption of SCSES in the Polish women population revealed the five-factor structure of the tool. The reliability was α = 0.976.The construct validity was confirmed. The results showed a significant, positive, moderate correlation between FSFI scores in all SCSES subscales and its total score r = {0.439; 0.534}. Moreover, the education level and place of residence differentiated the SCSES scores. The SCSES is a valid and reliable tool for the Polish women population. The findings highlight the need of recognising the importance of sexual communication in sexual functioning.

A Pilot Study of Psychosexual Functioning and Communication in Women Treated for Advanced Stages of Cervical Cancer After the Diagnosis

IntroductionThe purpose of this pilot study was to examine relationships between body image, perceived stress, partner and patient-provider sexual communication, and sexual functioning in women with advanced stages of cervical cancer (CC) after the cancer diagnosis.MethodsIn this pilot study, cancer patients (n = 30) and healthy women (n = 30) were compared. A study was conducted from January to March 2022. Sexual functioning and its predictors were assessed using the 6-item Female Sexual Function Index (FSFI-6), the Perceived Stress Scale (PSS-10), the Sexual Communication Self-Efficacy Scale (SCSES), the self-administered questionnaire contributing the patient-provider sexual communication, and the Body Esteem Scale (BES). The data was collected from January to June 2022.ResultsWomen with cervical cancer after the diagnosis reported impaired sexual functioning, which was associated with self-efficacy in sexual communication, feeling comfortable discussing sexual issues with a healthcare provider, perceived stress, and body image. Compared to the control group, CC patients had significantly lower sexual functioning (mean 8.83 vs 19.23; p < 0.001).ConclusionsSexual functioning in women with CC is significantly impaired even after the diagnosis and is associated with psychosocial variables. The expanded study will include other predictors of sexual functioning and quality of life in women with CC on the larger group of patients.Policy ImplicationsAs cancer becomes a more chronic disease that affects even younger individuals, social policy should promote the sexuality issues in cancer patients, as it is an integral part of every person’s life, regardless of health status or age.

Parental sexual communication self-efficacy with toddlers and young children: an active learning intervention

ABSTRACT Parents are in a powerful position to teach sexual topics to their children, promoting positive sex attitudes and lifelong sexual health. However, parents may lack sexual knowledge and confidence to address such topics. This study, grounded in social learning theory, tested the effects of an active learning intervention to increase parental efficacy regarding sexual communication with children aged one- to five-years-old. Parents were randomly assigned to a control group (n = 55), a factsheet group (n = 58), and an active learning group (n = 56). Multi-level Modelling (MLM) analyses showed the intervention was not effective at increasing parental sexual communication efficacy. However, a dosage effect was observed for the active learning and factsheet groups. In addition, factsheet and active learning intervention groups showed significant gains in knowledge of child sexual development. The results of this study suggest active learning may be a step towards promoting lifelong sexual health.

Sexual Communication Self-Efficacy and PrEP Literacy Helps to Meet HIV Prevention Outcomes Among Black and Latinx Individuals.

Background:Ending the HIV Epidemic is a campaign dedicated to reducing new HIV infections in the United States by 90% by 2030. Preexposure prophylaxis (PrEP) holds significant promise as a prevention tool. However, uptake has not improved much since then. As a result, this research looked at how PrEP literacy relates to sexual communication self-efficacyMethods:Between August 4 and 21, 2020, we performed a community-informed cross-sectional study of Black and Latino residents in Indiana. We collected demographic information as well as PrEP literacy, sexual communication self-efficacy, and awareness of HIV status.Results:Being unaware of one’s own HIV status and being unsure of one’s sexual identity were both linked to reduced self-efficacy in the domain of sex communication. Those who had a greater level of PrEP literacy felt more confident in their ability to communicate with others about sexually related topics. People living in rural areas showed a significantly lower degree of sexual communication self-efficacy than participants in urban settings.Conclusion:Providing information about PrEP to those at risk of acquiring the virus may help them connect with treatment or prevention services. Healthcare professionals should take the time to understand their patients’ levels of PrEP literacy and communication self-efficacy regarding sexual health.

Examining the link between sexual self-concept and sexual communication among adolescents

ABSTRACT The purpose of this study was to move beyond a sexual risk framework to investigate the possible associations among three sex-positive constructs for adolescents: their sexual self-concept (i.e., their positive/negative feelings about themselves as sexual beings), their sexual communication with romantic/sexual partners, and their sexual communication self-efficacy. We also examined differences in these constructs by sexual intercourse experience and gender. Participants were 171 adolescents who had been in a dating or sexual relationship in the past year (M age = 16.32 years; 64.3% girls). Compared to girls, boys had more positive sexual self-concepts but less self-efficacy to communicate with their partners about sex. Adolescents who reported having had sexual intercourse had more positive sexual self-concepts as well as more frequent partner sexual communication compared to adolescents without sexual intercourse experience. Adolescents with a more positive sexual self-concept had higher sexual communication self-efficacy and reported more frequent sexual communication. In addition, sexual communication self-efficacy partially mediated the relationship between sexual self-concept and sexual communication. Results highlight the connection between sexual self-concept and sexual communication and contribute to a growing body of work on the positive aspects of adolescent sexuality.

Social self-efficacy and sexual communication among adolescents in the United States: a cross-sectional study.

Data were collected in a cross-sectional survey from 222 high school girls in a rural school district in the south-eastern United States (Mage = 15.2; 38% White, 29% Latina, 24% Black; 50% were in a dating relationship in the past 3 months). Variables were measured with Likert-type scales. Bivariate correlation and regression analyses were conducted. Social self-efficacy was significantly positively associated with sexual assertiveness and sexual communication self-efficacy for all girls, and there was a positive trend in the relationship between social self-efficacy and communication frequency among the subsample of girls who had a dating partner. The significant relationship with sexual assertiveness (β = 0.22, s.e. = 0.07, P = 0.001) and sexual communication self-efficacy (β = 0.17, s.e. = 0.04, P = 0.013) remained when controlling for sexual activity status. Strengthening social self-efficacy may enhance girls' sexual communication and assertiveness skills. Future studies are needed to confirm the causal and temporal nature of these associations.

Efficacy of a multimedia intervention in facilitating breast cancer patients' clinical communication about sexual health: Results of a randomized controlled trial.

Many women with breast cancer (BC) hesitate to raise sexual concerns clinically. We evaluated a multimedia intervention to facilitate BC patients' communication about sexual/menopausal health, called Starting the Conversation (STC). Female BC patients (N=144) were randomly assigned to either STC (20-min video, workbook, and resource guide) or control (resource guide only). Audio-recorded dialogue from patients' next oncology clinic encounter was coded for patients' sexual health communication. Self-report surveys assessed patients' beliefs about sexual health communication, self-efficacy for clinical interactions, sexual function/activity, anxiety/depression symptoms, and quality of life at baseline, post-intervention, and 2-month follow-up. T-tests or mixed-effects logistic regression compared study arms. Women in the STC arm were more likely to raise the topic of sexual health (51%; OR=2.62 [1.02, 6.69], p=0.04) and ask a sexual health question (40%; OR=2.85 [1.27, 6.38], p=0.01) during their clinic encounter than those in the control arm (30% and 19% for raise and ask, respectively). At follow-up, women in the STC arm showed greater improvements in sexual health communication self-efficacy (p=0.009) and in anxiety symptoms (p=0.03), and more women were sexually active at follow-up, compared to the control arm (OR=1.5, 70% vs. 46%, p=0.04). The STC intervention facilitated women's clinical communication about sexual health and reduced women's anxiety, possibly due to increased confidence in expressing their medical needs. Helpful information gained from clinical discussions could have improved women's willingness or ability to engage in sexual activity. Future studies should identify aspects of the clinical encounter most critical to improving women's sexual outcomes.

Patients' perceived barriers to discussing sexual health with breast cancer healthcare providers.

We characterized barriers reported by women with breast cancer (BC) regarding clinical discussions of sexual health, clustered women by their endorsement of these barriers, and compared these groups. BC outpatients (N = 144; M age = 56, 67% White) in a sexual health communication intervention study provided baseline data. Exploratory factor analysis characterized sexual health communication barriers on a standardized measure. Hierarchical cluster analysis detected patient groups with similar response patterns on these factors. ANOVAs, Chi-Square and Fisher's Exact tests compared groups on socio-demographic/medical factors, sexual health communication self-efficacy and outcome expectancies, general clinical self-efficacy, sexual concerns, emotional distress, and discussion about sexual health with an oncology provider. Two factors emerged: patients' own beliefs about or perceived inability to discuss sexual health (self-centered barriers), and patients' perceptions of providers' reactions to discussing sexual health (provider-centered barriers); self-centered barriers were endorsed more than provider-centered (P < .001). Three clusters emerged: women who endorsed (a) high self-centered and high provider-centered (n = 19; 13%), (b) high self-centered and low provider-centered (n = 95; 66%), and (c) low self-centered and low provider-centered (n = 30; 21%). Women endorsing more barriers reported lower sexual communication self-efficacy, outcome expectancies, and general clinical self-efficacy (P's ≤ .001); no differences in sexual concerns, emotional distress, or discussing sexual health were found. Women more strongly endorsed self-centered barriers than provider-centered. When grouped according to these barriers, women differed in their confidence and expectations for sexual health communication, regardless of degree of sexual or emotional distress. Interventions are needed to improve both patients' and providers' skills for discussing sexual health.

Sexual communication self-efficacy, hegemonic masculine norms and condom use among heterosexual couples in South Africa

ABSTRACTHegemonic masculine norms (HMN), which promote sexual risk-taking among males and the subordination of women, are believed to play a key role in the HIV epidemic among heterosexual couples in South Africa (SA). Sexual communication self-efficacy (SCSE) (i.e., a couple's confidence in their ability to communicate about HIV prevention) may be a key leverage point for increasing HIV prevention behaviors among this population. We interviewed 163 sexually active heterosexual couples in Soweto, SA to investigate the association between SCSE, HMN, and consistent condom use. We collected information on demographics, relationship dynamics, and sexual activity. We utilized the SCSE scale to measure couples' SCSE, and a subscale of the Gender Equitable Men scale to measure HMN among males. We performed bivariate and multivariable analyses to determine the association of consistent condom use with couples' SCSE as well as the male partner's endorsement of HMN. We found that couples with higher SCSE have greater odds of consistent condom use (adjusted odds ratio [AOR] = 1.30, 95% CI: 1.15–1.47). Furthermore, male endorsement of HMN was found to be negatively associated with consistent condom use among couples (AOR = 0.47, 95% CI: 0.24–0.89). Joint HIV serostatus was not significantly associated with the outcome. Future interventions that equip heterosexual couples with sexual communication skills, while simultaneously promoting more gender equitable norms, may increase consistent condom use and thereby reduce the transmission of HIV among this at-risk population.

Validation of the Sexual Communication Self-Efficacy Scale.

This study assessed a newly developed Sexual Communication Self-Efficacy Scale designed to measure the sexual communication self-efficacy of adolescent men and women. Three-hundred and seventy-four U.K. adolescents completed this new scale, along with several other validity measures. Factor analysis revealed that the Sexual Communication Self-Efficacy Scale consisted of five underlying factors: contraception communication, positive sexual messages, negative sexual messages, sexual history, and condom negotiation. These factors demonstrated high internal consistency and presents evidence to support construct validity. This scale may have utility in assessing the effectiveness of interventions designed to enhance sexual communication and sexual health behaviors among young people.

Influence of sexual arousability on partner communication mediators of condom use among African American female adolescents.

Background Ample evidence shows that partner sexual communication is related to condom use. Although communication about safer sex may often occur when sexual arousal is high, no studies have examined arousability, one's propensity for sexual arousal and partner sexual communication. The purpose of this study was to examine associations between sexual arousability and partner-related mediators of condom use among African American female adolescents, who have disproportionate risk for HIV and sexually transmissible infections (STIs). The study analysed self-reported baseline data from 701 African American females aged 14-20 years participating in a HIV/STI trial. Linear regression models examined associations between arousability and partner-related mediators of condom use (partner sexual communication self-efficacy, partner sexual communication frequency, sex refusal self-efficacy and condom use self-efficacy), controlling for age, impulsivity and relationship power. Greater arousability was significantly associated with reduced levels of each partner communication outcome assessed (partner sexual communication self-efficacy, partner sexual communication frequency and sex refusal self-efficacy) but was not associated with condom use self-efficacy. Arousal and other positive aspects of sex have largely been ignored by HIV/STI prevention efforts, which primarily focus on individual behaviour. A population-level sexual health approach focusing on sexual wellbeing may reduce stigma, facilitate partner sexual communication and be more effective at reducing HIV/STI rates than traditional approaches.

WiLLOW: Reaching HIV-Positive African-American Women Through a Computer-Delivered Intervention

WiLLOW is an evidence-based, group level HIV prevention program for African-American women living with HIV. This study evaluated the efficacy of a multimedia adaptation of WiLLOW in enhancing protective sexual behaviors and psychosocial mediators associated with HIV risk reduction. Using a randomized controlled design, 168 participants completed baseline, satisfaction, and three-month follow-up assessments. At follow-up intervention participants reported higher proportions of condom protected sex acts (p = .002) with both HIV-negative (p = .040) and HIV-positive (p = .003) partners. They were also more likely to report 100 % condom use (OR = 9.67; p = .03); fewer unprotected vaginal and anal sex acts (p = .002); significantly greater sexual communication self-efficacy (p = .004); and less stress (p = .012). Participants rated Multimedia WiLLOW favorably in four satisfaction categories-enjoyment (p < .001); information utility (p = .018); information clarity (p = .015) and held attention (p = .01).

Relational correlates of unprotected oral and vaginal sex and among African-American adolescent females

To identify relational correlates of unprotected oral sex (UOS) and vaginal intercourse (UVI) among African-American females. Participants (n=715) provided data on demographics, sexual communication self efficacy (SCSE), sexual communication frequency, condom self-efficacy, power in sexual relationships, fear of negotiating condom use, UOS and UVI. Participants reporting low SCSE were 2.5 and 1.6 times more likely to report UOS and UVI respectively. Additionally, participants who reported fear of condom negotiation were 3.1 times more likely to report UVI. Interventions promoting stronger SCSE may be a protective factor against having UOS and UVI among African-American females.

Perceived gender inequality, sexual communication self-efficacy, and sexual behaviour among female undergraduate students in the Mekong Delta of Vietnam

Worldwide, the literature on sexual behaviour has documented associations between gender-based relationship inequality and sexual communication ability and the actual use of condoms or other contraceptives among young women. This study aimed to examine these associations among undergraduate female students in the Mekong Delta of Vietnam. A cross-sectional survey of 1181 female third-year students from two universities in the Mekong Delta was conducted. Latent variable modelling and logistic regression were employed to examine the hypothesised associations. Among the 72.4% of students who had ever had boyfriends, 44.8% indicated that their boyfriends had asked for sex, 13% had had penile-vaginal sex and 10.3% had had oral sex. For those who had had penile-vaginal sex, 33% did not use any contraceptive method, including condoms, during their first sexual intercourse. The greater a student's perception that women were subordinate to men, the lower her self-efficacy for sexual communication and the lower her actual frequency of discussing safer sex matters and asking her partner to use a condom. Sexual communication self-efficacy was associated with actual contraceptive use (P=0.039) but only marginally with condom use (P=0.092) at first sexual intercourse. Sexual health promotion strategies should address the influence of gender relations on young women's sexual communication self-efficacy and the subsequent impact on actual contraceptive and condom use.

Validation of the worry about sexual outcomes scale for use in STI/HIV prevention interventions for adolescent females.

This study examined the psychometric properties of a new scale to measure adolescents' worry regarding outcomes of risky sexual behavior (i.e. sexually transmitted infections, including HIV [STI/HIV], and unintended pregnancy). The 10-item worry about sexual outcomes (WASO) scale, resulting in two subscales STI/HIV worry and pregnancy worry, was administered to a sample of 522 African-American female adolescents ranging in age from 14 to 18, residing in the southeast United States and participating in a sexual risk reduction intervention. The WASO demonstrated internal consistency across multiple administrations and yielded satisfactory construct validity. Worry was found to negatively correlate with sexual communication self-efficacy (with a new male partner and a steady male partner), frequency of sexual communication with male partner, attitudes about condom use and social support; worry was positively correlated with perceived barriers to condom use, condom negotiation, locus of control and depression. Overall, the results indicate that the WASO is a reliable and valid measure of assessing adolescents' worry about STIs, HIV and pregnancy. The WASO represents a brief self-administered instrument that can be easily integrated into sexual risk reduction assessments and interventions. Future studies employing the WASO might consider testing it with more diverse samples in terms of gender, race/ethnicity, age and sexual orientation.

Validation of a Partner Sexual Communication Scale for Use in HIV/AIDS Prevention Interventions

Abstract This paper reports on three studies in which the Partner Communication Scale (PCS) is validated. The PCS assesses African American adolescent females' frequency of sexual communication with male sex partners. The PCS was administered to three samples of African American female adolescents (N = 522; N = 280; N = 715) ranging in age from 14 to 21. The PCS demonstrated strong internal consistency and acceptable stability over 6-month and 12-month follow-up periods. Scores on the PCS were correlated with frequency of parent communication, sexual communication self-efficacy, sexual refusal self-efficacy, fear of communication about condoms, partner-related barriers to condom use, relationship satisfaction, self-esteem, exposure to communication-related sex education in schools and condom use, particularly with nonsteady male sex partners. Validation of a Parent-Adolescent Communication Scale for Use in STD/HIV Prevention Interventions

This study reports on the validation of a scale to assess adolescent girls' frequency of sexual communication with their parents. The Parent-Adolescent Communication Scale (PACS) was administered to 522 African American female adolescents ranging in age from 14 to 18. The PACS demonstrated satisfactory internal consistency (across multiple administrations) and acceptable test-retest reliability over a 12-month follow-up period. Concurrently, scores on the PACS were correlated with frequency of sexual communication with partner, sexual communication self-efficacy (boyfriend), perceived parental knowledge, family support, depression, and condom use with steady male sex partners. Prospectively, baseline PACS scores were correlated with frequency of sexual communication with partner and condom use. The present investigation indicates that the PACS is a reliable and valid measure of frequency of sexual communication between female adolescents and their parents. Utility of the PACS for researchers and practitioners is discussed.
